Abstract:Novel antidepressants or treatment strategies that may offer a more rapid onset of action, improved efficacy, and greater tolerability are in desperate need. Because current clinically utilized antidepressants, which target high-affinity transporters for serotonin and norepinephrine, fail to provide satisfactory treatment outcomes for quite a portion of patients. In recent investigations, a low-affinity but high-capacity transporter organic cation transporter 2 (OCT2, SLC22A2) has been proposed as an important postsynaptic determinant of aminergic tonus and mood-related behaviors, a complementary system to the high-affinity transporters. In order to evaluate whether OCT2 inhibition may at least in part contribute to the pharmacological effects of antidepressants, several typical antidepressant compounds of various mechanism categories were employed to inhibit OCT2 activity in cells stably overexpressing OCT2. The tested antidepressant agents included selective serotonin reuptake inhibitors (SSRIs, fluoxetine, sertraline and paroxetine), tricyclic antidepressants (TCAs, amitriptyline, imipramine, desipramine), monoamine oxidase inhibitor (MAOI, moclobemide), serotonin–norepinephrine reuptake inhibitor (SNRI, venlafaxine) and reported antidepressant alkaloid piperine. Piperine was screened through synaptosomes before cell experiments, without the interference of monoamine oxidase. All of the nine antidepressant compounds showed moderate inhibitory effects on OCT2-mediated metformin, serotonin and/or norepinephrine uptake. Sertraline and desipramine tended to inhibit OCT2 activity via a competitive mechanism. The fact could be easily belied, since passive diffusion dominated the influx process. It remains to be seen whether OCT2 inhibition plays a role to the overall therapeutic effects in clinical practice.
Exploration of 1-(4-(7-Azaindole)-3,6-dihydropyridin-1-yl)alkyl-3-(1H-indol-3-yl)pyrrolidine-2,5-dione Derivatives with Affinity for the Serotonin Transporter

Abstract:We describe the design, synthesis and structure–activity relationship of a novel series of 1-(4-(7-azaindole)-3,6-dihydropyridin-1-yl)alkyl-3-(1H-indol-3-yl)pyrrolidine-2,5-dione derivatives with combined effects on the serotonin (5-HT1A) and dopamine (D2) receptors and the serotonin (5-HT), noradrenaline (NA), and dopamine (DA) transporters as multi-target directed ligands for the treatment of depression. All of the tested compounds demonstrated good affinity for the serotonin transporter (SERT). Among them, compounds 11 and 4 emerged as the lead candidates because of their promising pharmacological profile based on in vitro studies. Compound 11 displayed a high affinity for the 5-HT1A (Ki = 128.0 nM) and D2 (Ki = 51.0 nM) receptors, and the SERT (Ki = 9.2 nM) and DAT (Ki = 288.0 nM) transporters, whereas compound 4 exhibited the most desirable binding profile to SERT/NET/DAT among the series: Ki = 47.0 nM/167.0 nM/43% inhibition at 1 μM. These results suggest that compounds 4 and 11 represent templates for the future development of multi-target antidepressant drugs.

Abstract:We have shown that NH4+ and K+ compete for extracellular binding on the Na(+)-K(+)-adenosinetriphosphatase (Na(+)-K(+)-ATPase) in the rat terminal inner medullary collecting duct (tIMCD). The present study explored whether the Na(+)-K(+)-ATPase modulates transepithelial net acid flux [JH+ = total CO2 absorption (JtCO2) + total ammonia secretion (JtAM)]. Tubules from the tIMCD were dissected from deoxycorticosterone (DOC)-treated rats and perfused in vitro. Perfusate and bath were identical physiological saline solutions containing 25 mM NaHCO3 + 6 mM NH4Cl or were NH4Cl or were NH4Cl free. With NH4+ present, the fall in total CO2 from perfusate to collected fluid (delta tCO2, 2.5 +/- 0.4 mM; n = 6) was accompanied by an increase in collected total ammonia concentration (0.2 +/- 0.1 mM). However, in the absence of NH4Cl, delta tCO2 was only 0.9 +/- 0.2 mM (P < 0.05, n = 5). To determine the mechanism of this NH4Cl-induced increase in net acid secretion, the effect of Na+ pump inhibition on net acid secretion was explored. With NH4Cl present, JCO2 was 3.8 +/- 0.5 pmol.mm-1.min-1 (ouabain absent) but declined to 1.6 +/- 0.3 pmol.mm-1.min-1 with ouabain addition to the bath (n = 7, P < 0.05). Furthermore, in the presence of NH4Cl, intracellular pH (pHi) increased from 7.05 +/- 0.02 to 7.15 +/- 0.02 (P < 0.05, n = 5) with ouabain addition and returned to 7.06 +/- 0.03 (P < 0.05) with ouabain removal. However, in the absence of NH4Cl, ouabain failed to reduce JtCO2 (P = NS, n = 5), and an increase in pHi was not observed (n = 4, P = NS). In conclusion, NH4+ augments net acid secretion likely by serving as a proton source for bicarbonate absorption and titration of other luminal buffers. This ammonium pathway is dependent on the basolateral membrane Na(+)-K(+)-ATPase.
Development of potent dopamine-norepinephrine uptake inhibitors (DNRIs) based on a (2S,4R,5R)-2-benzhydryl-5-((4-methoxybenzyl)amino)tetrahydro-2H-pyran-4-ol molecular template

Abstract:Current therapy of depression is less than ideal with remission rates of only 25-35% and response rates of 45-60%. It has been hypothesized that a dysfunctional dopaminergic system in the mesocorticolimbic pathway in depressive disorder may lead to development of anhedonia associated with loss of pleasure and interest along with loss of motivation. The current antidepressants do not address dopamine dysfunction which might explain their low efficacy. In this report, we have described an SAR study on our pyran-based triple reuptake inhibitors (TRIs) which are being investigated as the next-generation antidepressants. In the present work we demonstrate that our lead TRIs can be modified with appropriate aromatic substitutions to display a highly potent SSRI profile for compounds 2a and 4a (Ki (SERT); 0.71 and 2.68nM, respectively) or a potent DNRI profile for compounds 6b and 6h (Ki (DAT/NET); 8.94/4.76 and 13/7.37nM, respectively). Compounds 4g-4i exhibited potencies at all three monoamine transporters. The results provide insights into the structural requirements for developing selective dual- and triple-uptake inhibitors from a unique pyran molecular template for an effective management of depression and related disorders.
Characterization of NCGC1360 and NCGC1366 as Highly-Selective D2 Dopamine Receptor Antagonists and Lead Candidates for the Treatment of Neuropsychiatric Disorders

Abstract:ID 90311 Poster Board 475 The D 2 dopamine receptor (D2R) is a G protein-coupled receptor (GPCR) and a validated drug target for the treatment of many neuropsychiatric disorders, including psychosis. All currently available antipsychotics are antagonists of the D2R; however, all of these drugs exhibit polypharmacology resulting in a plethora of off-target side effects including weight gain, dysphoria, and sedation, among others. A highly-selective D2R antagonist might overcome these pitfalls and increase patient compliance with taking antipsychotic medications. Previously, our lab conducted a high-throughput screen to identify D2R-selective antagonists with reduced activity at other GPCRs. This screen identified a promising hit compound that was chemically optimized into the lead drug candidates, NCGC1360 and NCGC1366. These drug leads have relatively high binding affinities at the D2R (~80 and ~50 nM, respectively), while displaying >100-fold selectivity over the D3R and >24-fold selectivity over the D4R. A β-arrestin recruitment assay revealed even higher D2R selectivity (>100-fold) of these compounds over the D3R and D4R. In a screen against an array of 46 GPCRs, channels, and transporters using radioligand binding assays, NCGC1360 and NCGC1366 only inhibited radioligand binding to the D2R. A functional screen of 168 GPCRs further determined that NCGC1360 and NCGC1366 were exceptionally selective with each compound completely antagonizing β-arrestin recruitment to the D2R with only minor effects at a few other receptors. Schild-type analyses using dopamine-stimulated β-arrestin recruitment revealed that both compounds are competitive antagonists. A comparative molecular docking study using the active structures of both the D2R and D3R showed that NCGC1366 occupies the orthosteric binding pocket of the D2R with extended contacts with residues in extracellular loop 2 (ECL2). The contacts in ECL2 are not conserved between the D2R and D3R, which may confer D2R selectivity. We thus swapped the ECL2 of the D2R and D3R and found that a D2R mutant with the D3R ECL2 exhibited diminished affinity for NCGC1360 and NCGC1366. Conversely, a D3R mutant with the D2R ECL2 exhibited increased affinity for NCGC1360 and NCGC1366, consistent with the molecular docking study. Pharmacokinetic studies of NCGC1360 in mice revealed a half-life of 1.8 hr in plasma and 1 hr in brain with excellent brain penetration. Finally, NCGC1360 was characterized in animal models that are predicative of antipsychotic efficacy and on-target side effects. NCGC1360 dose-dependently decreased amphetamine-induced hyperlocomotion in mice and also rescued deficits in pre-pulse inhibition induced by amphetamine. Importantly, NCGC1360 did not induce catalepsy up to the highest dose tested (10 mg/kg), suggesting that it would not induce extrapyramidal side effects in patients. Taken together, these studies provide support for continuing the development of this scaffold with preclinical and IND-enabling studies. Abstract:Depression has severely impaired the health of the people all over the world. Cognitive dysfunction due to depression has exerted serious economic burden to family and society induced by the reduction of social functioning for patients. NDRIs targeted with norepinephrine transport (hNET) and distribution than dopamine transport (hDAT) simultaneously treat depression and improve cognitive function, and they effectively avoid sexual dysfunction and other side effects. However, the drugs of NDIRs are still performing poor response to many patients. So it is urgent to discover novel NDRIs antidepressants with the ability of improving the cognitive fuction. The aim of this work was to identify novel NDRIs candicates selectively against hNET and hDAT from large compound libraries by comprehensive strategy integrating SVM, ADME/T, molecular docking, vitro binding assays, molecular dynamics simulation and binding energy calculation. Firstly, 6,522 compounds without inhibiting serotonin transporter (hSERT) were hitted by SVM models of hNET, hDAT and non-target hSERT with similarities analysis from compound libraries. ADME/T and molecular docking were then used to filter compounds which could potently bind in hNET and hDAT with good ADME/T, and 4 compounds were successfully identified. According to their docking scors and ADME/T information, 3719810 was advanced for profiling in vitro assays as novel NDRIs lead compound attributing to its best druggability and balance activities. Encouragingly, 3719810 performed comparative activities on two targets with Ki value 7.32 μM for hNET and 5.23μM for hDAT. In order to abtain candicates with more activities and balance activities of 2 targets, 5 analogs were optimized and 2 novel scaffold compounds were successively designed. By assessment of molecular docking, molecular dynamics simulation and binding energy calculation, 5 compounds were validated as NDRIs candicates with high activties, and 4 out of them performed acceptable balance actives acting on hNET and hDAT, which could supply promising novel NDRIs to exploit drugs for depression with cognitive dysfunction or other related neruodegenrative disorders, and also shed sight on screening inhibitors for dual-target with homological non-target on highly efficient and cost-effective.

Abstract:ID 92916 Poster Board 479 Dopamine receptors (DARs) are responsible for a plethora of physiologic functions, including cognition, mood, movement, and motivation. Not surprisingly, DARs play a role in the etiology and/or therapy of many neuropsychiatric disorders, including schizophrenia and substance use disorder (SUD). There are five DAR subtypes divided into two categories: D1-like (D1R and D5R) and D2-like (D2R, D3R, and D4R). Current antipsychotic medications are antagonists of the D2-like subfamily. However, the lack of selectivity among these D2R-preferring antagonists frequently produce motor-related extrapyramidal side effects, often leading to a lack of compliance among patients. Compounds that selectively antagonize the D3R may lead to fewer side effects while providing effective attenuation of drug craving and/or psychotic symptoms, making the discovery of novel D3R-selective antagonists a priority. A challenge to this goal is posed by the high sequence homology between the D2R and D3R within their orthosteric binding sites. Our lab aims to overcome these selectivity challenges by identifying D3R negative allosteric modulators (NAMs), compounds that inhibit the D3R via binding to an allosteric site. To this end, we screened the NIH Molecular Libraries Program 400,000+ small molecule library and identified MLS6357 as a promising hit compound that exhibited selectivity for the D3R over the D2R in multiple signaling outputs, while displaying an allosteric mechanism of action. Interestingly, radioligand binding assays demonstrated that MLS6357 increases D3R affinity for agonists, indicating that this scaffold is a D3R positive allosteric modulator-antagonist (PAM-antagonist), a special class of NAMs that potentiate agonist-receptor binding while concomitantly decreasing agonist-stimulated receptor signaling. Iterative medicinal chemistry and functional assays were used to synthesize and characterize >100 analogs of MLS6357, with several compounds displaying several-fold increases in potency as antagonists in both β-arrestin recruitment and G-protein activation assays, compared to the parent compound MLS6357. Among them, analogs UNC8747 and UNC6869 also maintained global D3R-selectivity. Additionally, both UNC8747 and UNC6869 appeared to recapitulate the activity of MLS6357 as PAM-antagonists and exhibited brain penetrance at sufficient concentrations to occupy the D3R in vivo , making them promising candidates for in vivo behavioral testing. In addition, several other analogs displayed functional selectivity for inhibiting G-protein activation versus β-arrestin recruitment and vice versa. Site-directed mutagenesis approaches are being utilized to determine the allosteric binding site for this PAM-antagonist scaffold at the D3R. The development of a novel, potent PAM-antagonist with high selectivity for the D3R may provide a therapeutic advance for many neuropsychiatric conditions, including SUD.

Abstract:Herein, we describe the discovery of inhibitors of norepinephrine (NET) and dopamine (DAT) transporters with reduced activity relative to serotonin transporters (SERT). Two compounds, 8b and 21a, along with nomifensine were tested in a rodent receptor occupancy study and demonstrated dose-dependent displacement of radiolabeled NET and DAT ligands. These compounds were efficacious in a rat forced swim assay (model of depression) and also had activity in rat spontaneous locomotion assay.

Abstract:Background and ObjectivesDoxepin, dosulepin, and clomipramine are tricyclic antidepressants (TCAs) that act as serotonin and noradrenaline reuptake inhibitors. The metabolites formed by N-dealkylation of these tricyclic antidepressants contribute to overall poor pharmacokinetics and efficacy. Deuteration of the methyl groups at metabolically active sites has been reported to be a useful strategy for developing more selective and potent antidepressants. This isotopic deuteration can lead to better bioavailability and overall effectiveness. The objective is to study the effect of site-selective deuteration of TCAs on their pharmacokinetic and pharmacodynamic profile by comparison with their nondeuterated counterparts.MethodsIn the current study, the pharmacokinetic profile and antidepressant behavior of deuterated TCAs were evaluated using the forced swim test (FST) and tail suspension test (TST), using male Wistar rats and male Swiss albino mice, respectively; additionally, a synaptosomal reuptake study was carried out.ResultsCompared with the nondeuterated parent drugs, deuterated forms showed improved efficacy in the behavior paradigm, indicating improved pharmacological activity. The pharmacokinetic parameters indicated increased maximum concentration in the plasma (Cmax), elimination half-life (t1/2), and area under the concentration-time curve (AUC) in deuterated compounds. This can have a positive clinical impact on antidepressant treatment. Synaptosomal reuptake studies indicated marked inhibition of the reuptake mechanism of serotonin (5-HT) and norepinephrine.ConclusionsDeuterated TCAs can prove to be potentially better molecules in the treatment of neuropsychiatric disorders as compared with nondeuterated compounds. In addition, we have demonstrated a concept that metabolically active, site-selective deuteration can be beneficial for improving the pharmacokinetic and pharmacodynamic profiles of TCAs. A further toxicological study of these compounds is needed to validate their future clinical use.

Abstract:Atypical dopamine transporter (DAT) inhibitors have shown therapeutic potential in the preclinical models of psychostimulant use disorders (PSUD). In rats, 1-(4-(2-((bis(4-fluorophenyl)methyl)sulfinyl)ethyl)-piperazin-1-yl)-propan-2-ol (JJC8-091, 3b) was effective in reducing the reinforcing effects of both cocaine and methamphetamine but did not exhibit psychostimulant behaviors itself. Improvements in DAT affinity and metabolic stability were desirable for discovering pipeline drug candidates. Thus, a series of 1-(4-(2-bis(4-fluorophenyl)methyl)sulfinyl)alkyl alicyclic amines were synthesized and evaluated for binding affinities at DAT and the serotonin transporter (SERT). Replacement of the piperazine with either a homopiperazine or a piperidine ring system was well tolerated at DAT (Ki range = 3-382 nM). However, only the piperidine analogues (20a-d) showed improved metabolic stability in rat liver microsomes as compared to the previously reported analogues. Compounds 12b and 20a appeared to retain an atypical DAT inhibitor profile, based on negligible locomotor activity in mice and molecular modeling that predicts binding to an inward-facing conformation of DAT.
A Novel Dual Serotonin Transporter and M-channel Inhibitor D01 for Antidepression and Cognitive Improvement

Abstract:Cognitive dysfunction is a core symptom common in psychiatric disorders including depression that is primarily managed by antidepressants lacking efficacy in improving cognition. In this study, we report a novel dual serotonin transporter and voltage-gated potassium Kv7/KCNQ/M-channel inhibitor D01 (a 2-methyl-3-aryloxy-3-heteroarylpropylamines derivative) that exhibits both anti-depression effects and improvements in cognition. D01 inhibits serotonin transporters (Ki = 30.1 ± 6.9 nmol/L) and M channels (IC50 =10.1 ± 2.4 μmol/L). D01 also reduces the immobility duration in the mouse FST and TST assays in a dose-dependent manner without a stimulatory effect on locomotion. Intragastric administrations of D01 (20 and 40 mg/kg) can significantly shorten the immobility time in a mouse model of chronic restraint stress (CRS)-induced depression-like behavior. Additionally, D01 dose-dependently improves the cognitive deficit induced by CRS in Morris water maze test and increases the exploration time with novel objects in normal or scopolamine-induced cognitive deficits in mice, but not fluoxetine. Furthermore, D01 reverses the long-term potentiation (LTP) inhibition induced by scopolamine. Taken together, our findings demonstrate that D01, a dual-target serotonin reuptake and M channel inhibitor, is highly effective in the treatment-resistant depression and cognitive deficits, thus holding potential for development as therapy of depression with cognitive deficits.

Abstract:Mood disorders cause much suffering and are the single greatest cause of lost productivity worldwide. Although multiple medications, along with behavioral therapies, have proven effective for some individuals, millions of people lack an effective therapeutic option. A common serotonin (5-HT) transporter (5-HTT/SERT, SLC6A4 ) polymorphism is believed to confer lower 5-HTT expression in vivo and elevates risk for multiple mood disorders including anxiety, alcoholism, and major depression. Importantly, this variant is also associated with reduced responsiveness to selective 5-HT reuptake inhibitor antidepressants. We hypothesized that a reduced antidepressant response in individuals with a constitutive reduction in 5-HTT expression could arise because of the compensatory expression of other genes that inactivate 5-HT in the brain. A functionally upregulated alternate transporter for 5-HT may prevent extracellular 5-HT from rising to levels sufficiently high enough to trigger the adaptive neurochemical events necessary for therapeutic benefit. Here we demonstrate that expression of the organic cation transporter type 3 (OCT3, SLC22A3 ), which also transports 5-HT, is upregulated in the brains of mice with constitutively reduced 5-HTT expression. Moreover, the OCT blocker decynium-22 diminishes 5-HT clearance and exerts antidepressant-like effects in these mice but not in WT animals. OCT3 may be an important transporter mediating serotonergic signaling when 5-HTT expression or function is compromised.

Abstract:In this research, a variety of novel amphetamine derivatives were synthesized and assessed for their potential as multifaceted antidepressant agents. Among these compounds, compound 11b demonstrated potent inhibitory effects on both serotonin and noradrenaline transporters (SERT/NET) and high affinity for histamine H3 receptor (H3R), and displayed low affinity for off-target receptors (H1, α1) and hERG channels, which can reduce the prolongation of the QT interval. Molecular docking studies offered a rational binding model of compound 11b when it forms a complex with SERT, NET, and the histamine H3 receptor. In vivo behavioral studies, compound 11b dose-dependently reduced the immobility duration in the mouse FST and TST assays without a stimulatory effect on the locomotor activity. Furthermore, compound 11b had a favorable pharmacokinetic profile in rats. Thus, compound 11b has the potential to develop a novel class of drugs for the treatment of depression.

Abstract:In this study, a series of novel arylpropylamine derivatives were designed, synthesized and evaluated as potential multi-target antidepressants. Among them, compound (R)-13j displayed unique pharmacological features, exhibiting excellent inhibitory potency against serotonin and noradrenaline transporters (SERT/NET) and high affinity for 5-HT2A/2C receptor, and showing low affinity for histamine H1, adrenergic α1 receptors and hERG channels (to reduce QT interval prolongation). Molecular docking studies provided a rational binding model of (R)-13j in complex with SERT and 5-HT2A/2C receptor. In animal models, compound (R)-13j dose-dependently reduced the immobility time in the tail suspension test (TST) and the forced swimming test (FST) in mice, with higher efficacy when compared to duloxetine, and showed no stimulatory effect on the locomotor activity. Moreover, compound (R)-13j significantly shortened the immobility time in the ACTH-induced rat model of treatment-resistant depression (TRD). Furthermore, compound (R)-13j also exhibited a higher threshold for acute toxicity than duloxetine. In addition, compound (R)-13j possessed a favorable pharmacokinetic profile in mice. Taken together, compound (R)-13j may constitute a novel class of drugs for the treatment of depression.

Abstract:The present work expands the chemical space known to offer potent inhibition of the serotonin transporter (SERT), norepinephrine transporter (NET), and dopamine transporter (DAT) and discloses novel bicyclic octahydrocyclopenta[c]pyrrole and octahydro-1H-isoindole scaffolds as potent triple reuptake inhibitors (TRIs) for the potential treatment of depression. Optimized compounds 22a (SEAT, NET, DAT, IC(50) = 20, 109, 430 nM), 23a (SERT, NET, DAT, IC(50) = 29, 85, 168 nM), and 26a (SERT, NET, DAT, IC(50) = 53, 150, 140 nM) were highly brain penetrant, active in vivo in the mouse tail suspension test at 10 and 30 mpk PO, and were not generally motor stimulants at doses ranging from 1 to 30 mpk PO. Moderate in vitro cytochrome P450 (CYP) and potassium ion channel Kv11.1 (hERG) inhibition were uncovered as potential liabilities for the chemical series.

Abstract:Dopamine (DA), the most abundant human brain catecholaminergic neurotransmitter, modulates key behavioral and neurological processes in young and senescent brains, including motricity, sleep, attention, emotion, learning and memory, and social and reward-seeking behaviors. The DA transporter (DAT) regulates transsynaptic DA levels, influencing all these processes. Compounds targeting DAT (e.g., cocaine and amphetamines) were historically used to shape mood and cognition, but these substances typically lead to severe negative side effects (tolerance, abuse, addiction, and dependence). DA/DAT signaling dysfunctions are associated with neuropsychiatric and progressive brain disorders, including Parkinson's and Alzheimer diseases, drug addiction and dementia, resulting in devastating personal and familial concerns and high socioeconomic costs worldwide. The development of low-side-effect, new/selective medicaments with reduced abuse-liability and which ameliorate DA/DAT-related dysfunctions is therefore crucial in the fields of medicine and healthcare. Using the rat as experimental animal model, the present work describes the synthesis and pharmacological profile of (S)-MK-26, a new modafinil analogue with markedly improved potency and selectivity for DAT over parent drug. Ex vivo electrophysiology revealed significantly augmented hippocampal long-term synaptic potentiation upon acute, intraperitoneally delivered (S)-MK-26 treatment, whereas in vivo experiments in the hole-board test showed only lesser effects on reference memory performance in aged rats. However, in effort-related FR5/chow and PROG/chow feeding choice experiments, (S)-MK-26 treatment reversed the depression-like behavior induced by the dopamine-depleting drug tetrabenazine (TBZ) and increased the selection of high-effort alternatives. Moreover, in in vivo microdialysis experiments, (S)-MK-26 significantly increased extracellular DA levels in the prefrontal cortex and in nucleus accumbens core and shell. These studies highlight (S)-MK-26 as a potent enhancer of transsynaptic DA and promoter of synaptic plasticity, with predominant beneficial effects on effort-related behaviors, thus proposing therapeutic potentials for (S)-MK-26 in the treatment of low-effort exertion and motivational dysfunctions characteristic of depression and aging-related disorders.

Abstract:Commonly designated as an indolent non-Hodgkin lymphoma, follicular lymphoma (FL) presents with striking pathobiological and clinical heterogeneity. Initial management strategies for FL have evolved to involve combination chemoimmunotherapy and/or radio-immunoconjugates. Unfortunately even with the best available nontransplant treatment, which nowadays results in higher frequency of response, FL remains incurable. Although considered a feasible therapeutic option, the use of hematopoietic cell transplantation (HCT) remains controversial. The appropriate timing, graft source, and intensity of HCT conditioning regimens in FL are often matters of debate. Herein we review the available published data pertaining to the use of autologous or allogeneic HCT in patients with FL across different stages of the disease, discuss major recent advances in the field, and highlight avenues for future research. The current literature does not support a role of HCT for FL in first remission, but in the relapsed setting autologous HCT remains appropriate for patients with early chemosensitive relapses, while allogeneic transplantation remains the sole curative modality for this disease, in relatively younger patients without significant comorbidities.
