Abstract:Background Persistent pulmonary hypertension of the newborn (PPHN) has been known for more than three decades, and lots of advancements have been made regarding its diagnosis and management. However, the exact causes of PPHN and the best treatment strategies remain debatable. This study aimed to compare the effectiveness of sildenafil and bosentan versus sildenafil and beraprost in the management of persistent pulmonary hypertension of the newborn (PPHN). Methodology This open-label, non-randomized, quasi-experimental study was conducted at the Department of Pediatric Cardiology and Neonatology, The Children's Hospital & The Institute of Child Health, Multan, Pakistan, from January 2021 to June 2021. We enrolled a total of 50 newborns (25 in each group) aged <10 days, gestational age above 34 weeks, who presented with respiratory distress and marked hypoxemia (PaO2 < 50 mmHg) as per arterial blood gas (ABG) analysis and confirmed echocardiographic diagnosis of PPHN within 24 hours of admission. A total of 25 cases were given sildenafil and bosentan, while the other 25 cases were given sildenafil and beraprost. Echocardiographic examination was done again after 72 and 120 hours, and the findings were noted. Outcomes were measured in terms of the reduction in tricuspid regurgitation (TR), mortality, and duration of hospital stay. Results Of the 50 neonates, 27 (54.0%) were male. Overall, the mean age was calculated to be 3.54 ± 0.7 days. The mean gestational age was 35.0 ± 0.7 weeks. The mode of delivery was cesarean section among 35 (70.0%) neonates. A significantly higher reduction in tricuspid regurgitation after 72 and 120 hours following the initiation of the treatment was observed in the sildenafil plus bosentan group in comparison with the sildenafil plus beraprost group (p < 0.05). No statistically significant difference was observed in terms of the duration of hospitalization between both study groups (p = 0.1776). Conclusion The combination of sildenafil and bosentan was found to be more effective than sildenafil and beraprost in reducing tricuspid regurgitation after 72 hours, while they have comparable efficacy at 120 hours of treatment in the management of persistent pulmonary hypertension of the newborn.

Abstract:Background: Persistent pulmonary hypertension of the newborn (PPHN) is a serious disease among newborns. Pulmonary vasodilators such as Sildenafil, Bosentan and Milrinone are important treatment modalities to treat persistent pulmonary hypertension of the newborn, especially in resource limited centers where inhaled nitric oxide is not available. Objective: To assess and compare the effectiveness of Bosentan and Sildenafil in persistent pulmonary hypertension of the newborn. Method: This randomized controlled trial was done in the NICU, ICMH, Matuail, Dhaka during July 2017 to July 2019. Following echocardiographic diagnosis, a total of 76 patients were randomly assigned into group A and group B (by lottery). Bosentan was given to group A (1mg/kg/body wt) 12 hourly for 7 days and Sildenafil was given to group B (2mg/kg/body wt.) 8 hourly for 7 days via oro-gastric tube/orally. Then heart rate, respiratory rate, cyanotic changes, SPO2 was recorded every day for 7 days and finally on the 8th day echocardiographic findings were obtained. Effects of two groups were compared. Results: The age of presentation of persistent pulmonary hypertension of newborn was < 24 hours and male newborns were predominant in both groups. Abstract:Objective: To compare milrinone with sildenafil for the treatment of persistent pulmonary hypertension of the newborn. Methodology: A single blinded randomized control trial was conducted in Department of Pediatrics, Aziz Fatima Hospital Faisalabad from October 2022 to September 2023. 160 infants of persistent pulmonary hypertension were included through non probablity consecutive sampling. Group 1 infants (n = 80) received a loading dose of milrinone intravenously administered over a 60-minute period at a dose of 50 μg/kg. Group 2 infants (n = 80) received sildenafil (2mg per kg per dose three times a day). The medication was continued until extubation. The primary outcome variables were the mortality and duration of hospital stay. Results: Ninety two(57.5%) of the one hubdred & sixty newborns in total were male. 111(69.4%) of the total, were full-term babies. In 114(71.2%) infants, the mode of delivery was C-section. Mortality was observed in 7 (8.8%) patients in milrinone group and 17 (21.2%) patients in sildenafil group with p-value = 0.027. Duration of hospital stay was less in milrinone group as compared to sildenafil group (14.22±3.75 days vs 17.72±5.7 days) with p-value = 0.0001. Conclusion: Milrinone has been proven to be a promising therapeutic choice for persistent pulmonary hypertension in neonates, and it is effective in doing so in low resource settings.

Abstract:Background: In newborns, persistent pulmonary hypertension (PPHN) poses a serious hazard to life. The principal treatment, inhaled nitric oxide, is believed to be unavailable in developing nations, where mortality is predicted to be between 10% and 20% higher. Objectives: This study aimed to assess the effectiveness, safety, and potential adverse reactions of bosentan and sildenafil when used in newborns with PPHN. Patients and Methods: Between July 2021 and August 2022, a double-blind clinical experiment was carried out at Damietta General Hospital's Neonatal Intensive Care Unit. In newborns with PPHN, the effectiveness, safety, and potential adverse reactions of bosentan and sildenafil were assessed. Comparisons were done between the two groups' echocardiographic results, length of oxygen reliance, need for invasive ventilator assistance, duration of medication, and short-term results such as: blood pressure, white blood cell, and haemoglobin counts. Results: Bosentan has comparable PAP-lowering and cardiac output-improving effects to sildenafil. Bosentan had a considerably shorter treatment period than sildenafil (P = 0.003). During 15 and 17 days, the oxygen need time was comparable between the two groups (P = 0.175). Both groups required invasive breathing support, which was equivalent (P = 0.867). The third echocardiographic research results, including pulmonic inadequacies (P = 0.183), tricuspid regurgitations (P = 0.357), and ejection fractions (P = 0.159), were comparable in the bosentan and sildenafil groups, despite pulmonary artery pressure (PAP) and the intensity of tricuspid valve inadequacy being higher before therapies. Conclusion: Neonatal PPHN is successfully treated with bosentan, which decreased it more quickly. Comparing to sildenafil, it is more effective at lowering PAP and lessens the degree of tricuspid valve dysfunction in a shorter period of time.

Abstract:Introduction: In persistent pulmonary hypertension of the newborn (PPHN), some neonates do not response to single pulmonary vasodilator drug, so recently combination therapy is being tried to decrease pulmonary artery systolic pressure (PASP). The aim of the study was to compare the efficacy of oral sildenafil alone with combination of oral sildenafil and milrinone in neonates suffering from PPHN. Methods: This RCT study was conducted in Dhaka Shishu (Children) Hospital, from July 2018 to June 2020. Sixty eight (68) term and near term neonates aged <10 days (34 cases in each group) with moderate to severe PPHN were taken. Monotherapy with oral sildenafil was given in group-A and combined therapy with oral sildenafil and i/v milrinone in Group-B. Serial echocardiography was done at before starting the therapy, at 3 days and at 5 days of treatment. Data regarding baseline characteristics, clinical features, PASP, PO2, Spo2, hospital stay, mortality and others investigations were recorded and analyzed with SPSS version-26. Results: Post treatment PASP was significantly reduced in group A (sildenafil) and group-B (sildenafil plus milrinone) (p<0.05). But in combination group PASP was more reduced than monotherapy group (p<0.05). Within the group, the oxygen saturation and PO2 significantly improved in both groups (p 0.05). Adrenaline infusion, hypotension and less hospital stay were significantly seen (P<0.05) in combination group. Conclusion: Combined therapy with oral sildenafil and milrinone are more effective than monotherapy with oral sildenafil in reduction of PASP. Both therapies are effective in improvement of oxygenation.

Abstract:The prevalence of persistent pulmonary hypertension of newborn (PPHN) has been estimated 1.9/1000 live births. Although the efficacy of inhaled nitric oxide and extracorporeal membrane oxygenation in PPHN is well established but it is difficult to administer and monitor in resource limited countries. Owing to this, other treatment options need to be evaluated.This is a prospective observational study conducted in the Pediatric Cardiology Department, NICVD, Karachi, from February 2020 to October 2020 after the approval from the Institutional Ethical Review Committee. All the neonates referred to our Unit were screened by echocardiography (echo) and those who fulfilled the inclusion criteria were included. In neonates who did not respond to increased dose of Sildenafil were added on oral Bosentan 1 mg/kg/dose twice a day.Total 82 newborns were enrolled. Fifty-two patients improved after 48 h so were continued on same treatment. Sildenafil dose was increased in 30 (37.9%) patients whose PO2 did not increase to at least 10% from baseline after 48 h of starting treatment. Three patients expired within 48–72 h. Out of 27 remaining patients, only four responded whereas 23 patients did not show any improvement. In these patients, Bosentan was supplemented along with sildenafil.The results of our study show effectiveness of oral Sildenafil in treating PPHN. The overall improvement observed in the patients was overwhelming. Combination of Sildenafil with Bosentan is beneficial in patients who did not respond on Sildenafil alone.

Abstract:Objective: To find out efficacy of sildenafil for the treatment of persistent pulmonary hypertension of the newborn (PPHN). Study Design: Observational study. Setting: Department of Neonatology, Sughra Shafi Medical Complex, Sahara Medical College, Narowal. Period: May 2020 to April 2021. Material & Methods: A total of 22 neonates of both genders with diagnosis of PPHN were enrolled during the study period. Demographic characteristic along with information about perinatal and postnatal characteristics were recorded among all neonates at the time of admission. X-ray chest were asked. Oral sildenafil was administered as 2mg/kg per dose thrice a day to all study cases and continued till extubation. Outcome was recorded on the basis of difference in FiO2 need after initiating sildenafil, time required to get ventilator off or mortality. Results: Out of a total of 22 neonates, 13 (59.1%) were male. Majority of the neonates, 15 (68.2%) were term newborns. Mode of delivery was lower segment cesarean section in 17 (77.3%) cases. APGAR score at 5 minutes was between 7 to 10 in 13 (59.1%) newborns. There were 13 (59.1%) newborns who required immediate resuscitation following birth. Meconium aspiration syndrome was the commonest associated condition found among 8 (36.4%) neonates. As per echocardiographic findings, 13 (59.1%) neonates had severe PPHN, 5 (22.7%) moderate and 4 (18.2%) mild. Mean oxygenation index at the time of admission was noted to be 46+14. Inotropic support was administered to 19 (86.4%) cases. Mean length of neonatal intensive care unit was 15+5 days. Mortality was reported among 10 (45.4%) neonates. Conclusion: Oral sildenafil was found to be a promising option for the treatment of neonates having PPHN. Oral sildenafil was found to successfully improve oxygenation among neonates having PPHN.

Abstract:Background Persistent pulmonary hypertension of the newborn (PPHN) is a multifactorial syndrome, with 0.43–6.8/1000 live birth incidence and 10–20% mortality rate. Aim To compare the efficacy of tadalafil and sildenafil in controlling PPHN. Patients and methods This prospective study was carried out on 40 neonates with PPHN. Patients were equally divided into two groups: group 1 received tadalafil (Cialis 10 mg), 1 mg/kg as a single dose for 48 h, and group 2 received sildenafil (Silden 25 mg), 1 mg/kg/8 h for 48 h. Echocardiography was done at the time of enrollment and 48 h after starting treatment measuring the change in estimated systolic pulmonary artery pressure (ESPAP), patent ductus arteriosus assessment, assessment of left ventricle functions, changes in oxygen saturation, and oxygen requirements. Results Both drugs were successful in improving oxygen saturation, decreasing oxygen requirements, and decreasing ESPAP. There was a significant difference before and after treatment in both groups, whereas the difference between the two groups was statistically insignificant ( P >0.05). Conclusion Tadalafil and sildenafil can similarly reduce ESPAP in neonates with PPHN. No serious short-term adverse effects of tadalafil and sildenafil were observed.

Abstract:Abstract Background Pulmonary Arterial Hypertension (PAH) carries a poor prognosis in both adult and pediatric patients. It is a life-threatening condition in newborns. Current recommendations advocate the use of targeted monotherapy as a first-line approach for the treatment of Persistent Pulmonary Hypertension of the Newborn (PPHN). In case of an inadequate clinical response to treatment, an addition of a second or third agent is considered. PAH is usually managed with a phosphodiesterase 5 inhibitor or an endothelin receptor blocker. There are limited pediatric studies that address questions like which class of therapy should be initiated first or if a combination should be initiated together. With this background, the present study was initiated to compare the efficacy, safety, and tolerability of bosentan as an adjuvant to sildenafil and sildenafil alone in PPHN. Results A total of 40 patients were enrolled in the study. Out of them, 26 were males (65%) and 14 were females (35%). PPHN was most commonly seen in the 29 (72.5%) of participants with a history of first order birth. Mean duration of symptoms was 14.05 ± 2.06 days. The participants were randomized to two groups. Group A consisted of total 25 participants that received both bosentan and sildenafil and group B had 15 participants that received sildenafil alone. Both groups were comparable in terms of birth weight and present weight, consanguinity, and mode of delivery. Efficacy was determined by the reduction in mean baseline Pulmonary Artery Systolic Pressure (PASP). PASP in group A was 75.56 ± 10.62 mm Hg and in group B was 64.86 ± 12.25 mm Hg which was not statistically significant ( P > 0.05). PASP on the third and seventh day in group A were 43.72 ± 8.63 and 24.47 ± 3.52 mm Hg compared to 42.28 ± 9.43 and 27.276 ± 8.38 respectively in group B which was statistically significant ( P < 0.05).There were two deaths each in both groups. Two participants in Group A developed liver function abnormalities. None of the participants in Group B had adverse effects. Conclusion Most common clinical manifestations were nonspecific. Cardiovocal syndrome was common in PPHN. We conclude that oral sildenafil treatment is a safe, simple and effective treatment for persistent pulmonary hypertension in newborn. Combination of bosentan with sildenafil is more effective and safe in reducing pulmonary artery (PA) pressures in high-risk patients with PPHN.

Abstract:Introduction: Persistent Pulmonary Hypertension of the Newborn (PPHN) reflects a disruption of the normal perinatal circulatory transition and is characterised by high pulmonary artery pressure. Inhaled Nitric Oxide (iNO) is considered the mainstay of treatment for PPHN. However, many developing and resource limited countries do not have access to such expensive therapy. Therefore, sildenafil, due to its easy availability and low cost, becomes the most commonly used drug in the management of PPHN in such settings. Aim: To study the treatment response and outcomes of neonates who received sildenafil for the management of PPHN. Materials and Methods: The present hospital-based prospective interventional study was conducted in the Neonatal Intensive Care Unit (NICU), BJ Medical College, Civil Hospital, Ahmedabad, Gujarat, between May 1, 2020, and April 30, 2021. Study was conducted on neonates with meconium aspiration syndrome and/or severe birth asphyxia admitted to the NICU. A total of 48 neonates with Two Dimensional (2D) echocardiography-proven PPHN were enrolled. Baseline clinical parameters including heart rate, respiratory rate, preductal and postductal Saturation of Peripheral Oxygen (SpO2), Oxygenation Index (OI), Saturation Oxygen Distending Pressure Index (SOPI), and Non Invasive Blood Pressure (NIBP) were monitored every six hours. Neonates who were on invasive/ non invasive ventilation had their Fraction of Inspired Oxygen (FiO2), Positive End Expiratory Pressure (PEEP), Peak Inspiratory Pressure (PIP), respiratory rate and flow rate monitored every six hours. Data were statistically analysed using Analysis of Variance (ANOVA) test. Results: During the study period, a total of 1080 neonates were admitted with meconium aspiration syndrome and/or severe birth asphyxia, out of which 48 neonates showed findings suggestive of PPHN on 2D echocardiography. A statistically significant improvement in oxygenation after sildenafil treatment was indicated by a significant reduction in OI from 35.3±8.6 to 13.2±2.1 (p-value<0.001), a reduction in SOPI from 3.6±0.3 to 1.5±0.2 (p-value<0.001), a reduction in FiO2 (%) from 94.6±8.19 to 24.2±4.5 (p-value<0.001), an increase in Partial pressure of oxygen (PaO2) (mmHg) from 52±5.6 to 72±3.4 (p-value<0.001), and an increase in SpO2 (%) from 83.5±8.6 to 93.5±5.1 (p-value<0.001). Conclusion: The findings of the present study suggest that oral sildenafil can be successfully used to improve oxygenation in patients with PPHN, especially in a resource limited setting where facilities like Extracorporeal Membrane Oxygenation (ECMO) and iNO are not available, as demonstrated in the present study.
Milrinone Versus Sildenafil in Treatment of Neonatal Persistent Pulmonary Hypertension: A Randomized Control Trial

Abstract:Persistent pulmonary hypertension of the newborn (PPHN) is a condition caused by failure of pulmonary vascular adaptation at birth, resulting in severe hypoxia. Several therapeutic modalities are being tried in developing countries where established therapies (inhaled nitric oxide and extracorporeal membrane oxygenation) are widely unavailable. This study aimed to assess the efficacy of milrinone versus sildenafil as available alternative therapeutics in treating PPHN. Forty neonates (>34 weeks) admitted to neonatal intensive care units with evidence of PPHN were randomly allocated to receive either oral sildenafil (0.5–2 mg/kg/6 hours) or intravenous milrinone (0.25–0.75 mic/kg/min). Primary outcomes included improvements in systolic pulmonary artery pressure and oxygen saturation index (OSI) at 24 and 48 hours after treatment. Secondary outcomes included the duration of hospitalization and mechanical ventilation. The ClinicalTrials identifier is NCT04391478. Both groups showed significant improvement in the post-treatment hemodynamic variables compared with pretreatment levels ( P < 0.05 for all parameters). Systolic pulmonary artery pressure and OSI values significantly improved in both study groups compared with baseline ( P < 0.001). The 24-hour and 48-hour post-treatment OSI values were much lower in the milrinone group than those in the sildenafil group ( P < 0.05). The length of hospital stay was significantly shorter in the milrinone group than that in the sildenafil group ( P < 0.05). There were no significant differences in the duration of mechanical ventilation, incidence of intracranial hemorrhage and pulmonary hemorrhage, or mortality between the 2 groups ( P > 0.05). In conclusion, milrinone and sildenafil are effective and well-tolerated in neonates with PPHN, particularly when inhaled nitric oxide and extracorporeal membrane oxygenation are not available. Milrinone is superior to sildenafil in improving oxygenation without lowering blood pressure parameters.

Methods: We conducted a retrospective cohort study of 42 neonates with PPHN treated with continuous intravenous treprostinil or oral sildenafil from January 2020 to October 2022 in China. Outcomes assessed included echocardiographic pulmonary artery systolic pressure (PASP), shunt direction, and arterial blood gas measures. Results: Treprostinil lowered PASP and improved oxygenation significantly better than sildenafil on days 1, 2, and 3 of treatment ( P < 0.05). Treprostinil also corrected shunt direction faster than sildenafil ( P < 0.05). The duration of mechanical ventilation, length of NICU stay, and overall hospital stay did not significantly differ between the two groups ( P > 0.05). Conclusions: Treprostinil effectively lowers pulmonary artery pressure and improves oxygenation in neonates with PPHN, without being associated with severe complications. It may serve as a beneficial adjunct therapy for neonates with PPHN.

Oral Sildenafil in Infants With Persistent Pulmonary Hypertension of the Newborn: A Pilot Randomized Blinded Study

Abstract:BACKGROUND. Persistent pulmonary hypertension (PPHN) occurs in as many as 6.8 of 1000 live births. Mortality is ∼10% to 20% with high-frequency ventilation, surfactant, inhaled nitric oxide, and extracorporeal membrane oxygenation but is much higher when these therapies are not available. Sildenafil is a phosphodiesterase inhibitor type 5 that selectively reduces pulmonary vascular resistance. OBJECTIVE. Our goal was to evaluate the feasibility of using oral sildenafil and its effect on oxygenation in PPHN. DESIGN. This study was a proof-of-concept, randomized, masked study in infants >35.5 weeks' gestation and <3 days old with severe PPHN and oxygenation index (OI) >25 admitted to the NICU (Hospital Niño Jesús, Barranquilla, Colombia). The sildenafil solution was prepared from a 50-mg tablet. The first dose (1 mg/kg) or placebo was given by orogastric tube <30 minutes after randomization and every 6 hours. Preductal saturation and blood pressure were monitored continuously. OI was calculated every 6 hours. The main outcome variable was the effect of oral sildenafil on oxygenation. Sildenafil or placebo was discontinued when OI was <20 or if there was no significant change in OI after 36 hours. RESULTS. Six infants with an OI of >25 received placebo, and 7 received oral sildenafil at a median age of 25 hours. All infants were severely ill, on fraction of inspired oxygen 1.0, and with similar ventilatory parameters. Intragastric sildenafil and placebo were well tolerated. In the treatment group, OI improved in all infants within 6 to 30 hours, all showed a steady improvement in pulse oxygen saturation over time, and none had noticeable effect on blood pressure; 6 of 7 survived. In the placebo group, 1 of 6 infants survived. CONCLUSIONS. Oral sildenafil was administered easily and tolerated as well as placebo and improved OI in infants with severe PPHN, which suggests that oral sildenafil may be effective in the treatment of PPHN and underscores the need for a large, controlled trial.

Abstract:Background Persistent pulmonary hypertension (PPHN) is defined as failure of normal pulmonary vascular adaptation at or soon after birth, resulting in a persistent high pulmonary vascular resistance, which leads to diminished pulmonary blood flow and shunting of unoxygenated blood into systemic circulation through an opened foramen ovale and/or the ductus arteriosus. The prevalence of this syndrome is about 1.9/1000 in the population of neonates born at term. Objective The aim was to evaluate the efficacy and safety of oral sildenafil in the treatment of PPHN. Patients and methods This prospective interventional study was conducted on 50 neonates who were of more than or equal to 37 weeks gestational age and less than 3 days old and were diagnosed as PPHN by echocardiogram and had an oxygenation index (OI) more than or equal to 20. All included cases were given oral sildenafil as per the study protocol with a starting dose of 0.5–2 mg/kg/dose. OI, oxygen saturation, alveolar arterial oxygen gradient, and mean airway pressure were monitored serially. Results This study showed a significant decrease in OI after 30 min of starting treatment and after 24 h of treatment in the studied cases 16.1±1.7 ( P =0.010); also, there was a significant decrease in ESPAP ? after treatment estimated by ECHO from mean 49.4±5,9 to 42.5±5.7 ( P <0.001). Furthermore, this study showed a significant decrease in brain-type natriuretic peptide in studied cases after treatment with sildenafil. Conclusion Oral sildenafil may be of benefit in improving oxygenation in infants with PPHN as it can effectively improve OI and reduce right ventricular systolic pressure.

Abstract:Background: Persistent pulmonary hypertension of the newborn is a life-threatening condition that affects about 1–2 per 1,000 live births worldwide. Bosentan is an oral dual endothelin receptor antagonist that may have a beneficial effect on persistent pulmonary hypertension of the newborn by reducing pulmonary vascular resistance and improving oxygenation. However, its role in persistent pulmonary hypertension of the newborn remains unclear. Objectives: To systematically evaluate the efficacy and safety of bosentan as an adjuvant therapy for persistent pulmonary hypertension of the newborn in newborns. Methods: We searched six English and two Chinese databases from their inception to 1 January 2023 following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. We included randomised controlled trials and retrospective studies that compared bosentan with placebo or other drugs for persistent pulmonary hypertension of the newborn in newborns. We performed a meta-analysis using random-effects models and assessed the risk of bias and heterogeneity in the included studies. Results: We included 10 studies with a total of 550 participants. Bosentan significantly reduced the treatment failure rate (relative risk = 0.25, P < 0.001), pulmonary artery pressure (mean difference = −11.79, P < 0.001), and length of hospital stay (mean difference = −1.04, P = 0.003), and increased the partial pressure of oxygen (mean difference = 10.02, P < 0.001) and blood oxygen saturation (SpO2) (mean difference = 8.24, P < 0.001) compared with a placebo or other drugs. The occurrence of adverse reactions was not significantly different between bosentan and a placebo or other drugs. Conclusions: Bosentan is effective in the treatment of persistent pulmonary hypertension of the newborn but adverse reactions such as abnormal liver function should be observed when using it.

Abstract:Objective: To explore the effect of sildenafil combined with inhalational nitric oxide (NO) therapy on the curative effects and serum levels of hypoxia-inducible factor (HIF)-1α, endothelin-1 (ET-1), and calcium in persistent pulmonary hypertension of the newborn (PPHN). Patients and methods: Eighty-six patients with neonatal pulmonary hypertension treated in Xuzhou Children's Hospital from March 2015 to February 2016 were randomly divided into the observation group and control group, treated with sildenafil and sildenafil combined with inhalational NO, respectively. The clinical efficacy of newborns in the two groups was compared. Fraction of inspiration O2 (FiO2), Oxygen Index (OI), blood oxygen partial pressure (PaO2), blood oxygen saturation (SpO2), and pulmonary arterial pressure of newborns in the two groups were compared before treatment and 2 h, 12 h, and 24 h after treatment. The serum levels of HIF-1α, ET-1, and calcium of patients in the two groups were compared before treatment and 3, 5, 7 days after treatment. Results: The total effective rate of the observation group (95.34%) was significantly higher than that of the control group (74.41%) (p<0.05). After treatment, FiO2, OI, and pulmonary arterial pressure of patients in the two groups decreased, and the decrease in the observation group was significantly lower than in the control group (p<0.05). After treatment, PaO2 and SpO2 of patients in the observation group were higher than those of the control group. The levels of HIF-1α and ET-1 of patients in the two groups decreased and were significantly lower in the observation group compared with the control group. The levels of calcium of patients in the two groups increased and were significantly higher in the observation group than the control group (p<0.05). Conclusions: Sildenafil combined with inhalational NO therapy for neonatal pulmonary hypertension can quickly improve oxygenation, effectively reduce pulmonary arterial hypertension, and is worthy of clinical application.
Efficacy and Safety of IV Sildenafil in the Treatment of Newborn Infants with, or at Risk of, Persistent Pulmonary Hypertension of the Newborn (PPHN): A Multicenter, Randomized, Placebo-Controlled Trial

Abstract:Objective: To investigate the efficacy and safety of sildenafil added to inhaled nitric oxide (iNO) for newborn infants with persistent pulmonary hypertension of newborn (PPHN) or hypoxic respiratory failure (HRF) at risk of PPHN. Study design: Part A of a multinational, randomized, double-blind, placebo-controlled trial. Infants ≤96 hours' old, >34 weeks of gestation, receiving iNO (10-20 ppm on ≥50% FiO2) for PPHN or HRF at risk of PPHN, and oxygen index >15 to <60, were randomized (1:1) to intravenous (IV) sildenafil (loading: 0.1 mg/kg, over 30 minutes; maintenance: 0.03 mg/kg/h) or placebo, for up to 14 days. Coprimary end points were treatment failure rate (day 14/discharge) and time on iNO without treatment failure. Secondary end points included time on ventilation and oxygenation measures. Results: Of 87 infants screened, 29 were randomized to IV sildenafil and 30 to placebo; 13 discontinued treatment (sildenafil, n = 6; placebo: n = 7), including 3 deaths (sildenafil: n = 2; placebo: n = 1). Treatment failure rates did not differ with sildenafil (27.6%) vs placebo (20.0%; P = .4935). Mean time on iNO was not different with sildenafil (4.1 days) vs placebo (4.1 days; P = .9850). No differences were noted in secondary end points. Most common adverse events (AEs) with sildenafil (≥10% infants) were hypotension (n = 8/29), hypokalemia (n = 7/29), anemia, drug withdrawal syndrome (n = 4/29, each), and bradycardia (n = 3/29). One serious AE (hypotension) was considered treatment-related. Conclusions: IV sildenafil added to iNO was not superior to placebo in infants with PPHN or HRF at risk of PPHN. A review of AEs did not identify any pattern of events indicative of a safety concern with IV sildenafil. Infants will have developmental follow-up (Part B).
