Abstract:Introduction: Approximately 9 million people in China use insulin but little is known of their injection techniques (IT). Our aim was to understand IT in China via direct examination and a detailed survey. Methods: Injection Technique Questionnaire (ITQ) results of 3853 patients from 20 representative centers in China were compared with the Rest of World (ROW). Results: Insulin pens are used by 92% of Chinese insulin injectors, with > 3/4 using 4- or 5-mm pen needles. This is consistent with Chinese BMI values (and thus subcutaneous fat thickness) being lower than in ROW, raising the risk of IM (intramuscular) injections with longer needles. Total daily insulin dose is ~ 33 IU in China vs. 45 IU elsewhere. Lipohypertrophy (LH) rates are lower than in ROW, but still found in at least 25% of Chinese patients. Patients apparently do not realize they have LH or are unaware of the hazards of injecting into LH since many of them do so, and those who do use a mean of 11 IU more of insulin than non-LH patients. This incremental insulin use possibly costs the Chinese health care service around 2 billion RMB annually. Needle reuse is also frequent in China (78% vs. 48% ROW), and the high cost of pen needles is the main reason patients cite for reusing (25% use a single needle > 10×). Conclusion: This study reveals many areas in which Chinese professionals and patients can optimize injection practices and thereby improve outcomes and reduce costs. The official guidelines of the Chinese Diabetes Society, based on the FITTER recommendations, offer a roadmap for reaching this goal. Funding: BD Diabetes Care. Abstract:Objective To explore the problems in the use of insulin pen needles.Method 188 patients using the insulin pen in clinic and in hospital were selected for the investigation with questionnaire of using insulin pen needles.Results The major problems in the use of insulin pen needles were injection without exhaust,disinfecting the needle with alcohol,inappropriate puncturing angel,withdrawing the needle without retention for enough long time,not taking off from the insulin pen after injection and using the used needle.Conclusions The health education should be done to the patients in clinic and hospital to enhance their correct understanding of the use of insulin pen needle,reduce the harm from incorrect injections and control their blood glucose.

Abstract:Objectives Appropriate insulin injection technique can help to enhance positive clinical outcomes in insulin‐treated patients. This cross‐sectional survey was undertaken to assess the present state of injection technique practices of patients administering insulin and the incidence of local complications associated with insulin injection. Methods This cross‐sectional study was conducted at 11 tertiary hospitals in Xi'an City. A total of 1370 patients with diabetes were enroled in the survey. Each patient was asked questions on the Injection Technique questionnaire, demonstrated insulin injection on an injection training pad and had their injection sites inspected by a nurse. Results About 98% of participants were type 2 diabetic patients. The 5 mm length needles were used by 640 (46.71%) patients. Around 1286 participants reused pen needles. Only one‐third of patients rotate the injection sites. Around 901 patients had ecchymosis at injection sites, 43 participants suffered rash and or pruritus and 661 patients had lipohypertrophy. About 2.99% of patients reported having infection at the injection sites. Most patients (72.55%) occasionally felt pain when injecting insulin. The results of the multivariable logistict analysis showed that gender (male), needle length, priming the pen before injections and correct site rotation were negatively associated with the incidence of injection site ecchymosis. Lower incidence of lipohypertrophy was associated with needle length 4 mm, needle length 6 mm, single use of pen needles, priming the pen before injections and correct site rotation. Higher incidence of injection site infection was related to not disinfecting injection sites before each injection. Conclusions Despite some improvements detected in some aspects of patients' insulin injection practice in recent years, a large gap still exists between insulin injection guidelines and patients' actual injection behaviour. Injection‐related complications at injection sites are still considerably common among our study population. Patients' education regarding insulin injection technique should be conducted repeatedly and regularly to make sure patients' compliance to injection guidelines.

Establishment of Hyperinsulinemic Euglycemic Clamp Technique and the Reference Value of Insulin Sensitivity Index in Chinese

Abstract:Objective To establish the technique of hyperinsulinemic euglycemic clamp and to study the reference value of insulin sensitivity index in healthy Chinese. Methods According to the feedback mathematical model developed by DeFronzo, the technique of hyperinsulinemic euglycemic clamp was used in 90 healthy Chi- nese [ male:female =71 = 19; age; (28. 3±6. 1) years; body mass index (20. 9±1.5) kg/m2 ] to study die glu-cose metabolized rate. Blood samples were obtained at timed intervals in the fasting state and during the clamp for the measurement of glucose, insulin and C peptide. Results During the clamp tests, the blood glucose levels were con-trolled within 10% of target value. The coefficient of variation of glucose levels was 3. 8% 0.1%. In the steady state, the insulin sensitivity index (glucose metabolized rate, M value ) was (7.78±2.30) mg· kg-1 min-1, which was distributed normally. The lowest quartile of M value was 6. 286 mg·kg -1 min-1'. The coefficient of variation of M value was 9.4%±2.8%. Conclusion The technique of hyperinsulinemic euglycemic clamp and the reference value of insulin sensitivity index in healthy Chinese are successfully established in our center.

Abstract:Objective:To investigate the status quo of the knowledge, attitude and practice (KAP) of standardized insulin injection at home in Shanghai community diabetic patients and analyze its influencing factors.Methods:In this cross-sectional study, totally 1 947 diabetic patients who signed to be managed by 21 community health service centers in Fengxian District, Shanghai from May to October 2020 who used insulin pen injections at home were selected by continuous enrolment. They were investigated with a general information questionnaire and a questionnaire on KAP of standardized insulin injection. Univariate analysis and multiple linear regression were used to analyze the influencing factors of patients' KAP of standardized insulin injection at home. A total of 1 947 questionnaires were distributed, and 1 697 valid questionnaires were recovered, with an effective recovery rate of 87.2%.Results:The total KAP score of insulin standard injection in the 1 697 diabetic patients was (51.01±6.81) , which was at a moderate level. Among all the items in the questionnaire, the 3 items with the lowest compliance rate were: checking the injection site every time before insulin injection (14.4%) , agreeing to put the discarded insulin needles into the capped hard-shell container (16.1%) , agreeing to and implementing insulin pen needle replacement after each use (20.6%) . Multiple linear regression analysis showed that age, education, type of medical insurance, and whether they had received insulin injection guidance were the influencing factors to the KAP of standardized insulin injection at home ( P<0.01) , which accounted for 8.6% of the total variation. Conclusions:Community medical staff should pay attention to the health education of insulin injection at home for diabetic patients, improve the content and form of education based on the characteristics of the community population, thereby improving the compliance rate of injection site inspection, replacement of insulin pen needles and standardized disposal of discarded needles, further standardizing insulin injection behavior, and improving clinical outcomes such as glycemic control in patients.

Abstract:Pediatric diabetes is growing in China. The annual incidence of childhood type 1 diabetes is about 2.02-5.3 per 100,000 person-years. Type 2 diabetes in children and adolescents is increasing dramatically with the high-speed urbanization of China. The prevalence of type 2 diabetes varies from 1.64/100, 000 to 15.16/100,000 based on the geography and economy. Monogenic diabetes used to be underestimated in China and now more cases are emerging. In this review, we give an overview of pediatric diabetes in China, present the progresses and challenges in management of pediatric diabetes, and discuss the government policy and potential actions in China, for better life quality of diabetic families.

Abstract:Objective To explore the hypoglycemic efficacy and safety of needle-free injection technique in patients with diabetes mellitus. Methods 80 cases of patients with diabetes mellitus admitted to our hospital were randomly divided into the experimental group and the control group, with 40 cases in each group. The experimental group received needle-free subcutaneous insulin injection, while the control group received insulin injection with insulin pen. The blood glucose levels, local skin adverse reactions and injection satisfaction degree were compared between the two groups. Results The levels of fasting blood glucose（FPG）, and the 2 h blood glucose（2hPG） after breakfast, lunch and dinner in the experimental group were lower than those in the control group（P ＜0.05）. The incidences of local skin pain and induration in the experimental group were lower than those in the control group（P ＜0.05）. The injection satisfaction degree of the experimental group was 95.00%, significantly higher than 77.50% of the control group（P ＜0.05）. Conclusions Needle-free injection technique in insulin injection treatment of patients with diabetes mellitus has significant application effect, which can improve the blood glucose control effect and injection safety. The patients are more willing to accept it.

Abstract:IntroductionThere are multiple antidiabetic drugs available in China, which vary in their efficacy and safety. However, no study exists that compares all the classes of antidiabetic drugs simultaneously. This study aimed to estimate and compare the efficacy of alternative classes of antidiabetic drugs for Chinese patients with type 2 diabetes, either in a monotherapy regimen or combined with metformin.MethodsA systematic literature review was conducted by searching various literature databases to identify relevant randomized controlled trials published from 1990 to 2016. A meta-analysis was conducted to compare the efficacy of antidiabetic drug monotherapy and placebo or lifestyle interventions (i.e., diet and exercise), and antidiabetic drug plus metformin versus metformin alone, in Chinese patients with type 2 diabetes. An indirect comparison was used to estimate the efficacy of antidiabetic drug plus metformin versus placebo or lifestyle-intervention using metformin as the common comparator.ResultsThe database search identified 354 relevant studies. Compared with placebo or lifestyle interventions, combination therapies achieved greater reductions in hemoglobin A1c (HbA1c) level (1.9% versus 0.9%), body mass index (BMI) (2.66 versus 0.98 kg/m2), and total cholesterol level (1.07 versus 0.35 mmol/L) than monotherapies. For monotherapies, the top three treatments for reducing HbA1c level were insulin, sulfonylurea, and glucagon-like peptide-1 (GLP-1) receptor agonist. The top three monotherapies for reducing BMI level were metformin, GLP-1 receptor agonist, and α-glycosidase inhibitor. The top three monotherapies for reducing total cholesterol level were metformin, GLP-1 receptor agonist, and dipeptidyl peptidase-4 (DPP-4) inhibitor. For combination therapies, the top three treatments for reducing HbA1c level were GLP-1 receptor agonist plus metformin, insulin plus metformin, and glinide plus metformin. The top three combination therapies for reducing BMI level were glinide plus metformin, GLP-1 receptor agonist plus metformin, and DPP-4 inhibitor plus metformin. The top three combination therapies for reducing total cholesterol level were insulin plus metformin, GLP-1 receptor agonist plus metformin, and α-glycosidase inhibitor plus metformin.ConclusionsPharmacological treatments had better efficacy than placebo or lifestyle interventions, while combination drug therapies were superior to monotherapies.

Abstract:Attempts at replicating physiological insulin secretion,as a means of restoring the normal metabolic milieu and thereby minimizing the risk of diabetic complications,and instead of subcutaneous injections of soluble insulin,which take inconveniences and pains to patients,has become an essential feature of insulin treatment.Many researchers in and out of China have made efforts in expanding the delivery routes of insulin,including mucosae of intranasal,oral-buccal,rectal,ocular and intravaginal,pulmonary,skin,and gastrointestinal tract.We reviewed significant articles and main reports in this field,analyzed these advancement,advantages and disadvantages,and reached the point that there is a broad view in research of various routes of insulin delivery,with some obstacles still on the way.

Survey of Glycemic Control in Type 2 Diabetic Patients Treated with Human Insulin in China

Abstract:Objective To assess the characteristics and glycemic control of type 2 diabetic patients currently treated with human insulin regimen in China, and find out the issues in diabetes management. Methods From April to July in 2013, type 2 diabetic outpatients on the therapy of human insulin or human insulin combined with oral antidiabetic drugs (OADs), meeting the inclusion and exclusion criteria, were selected from the endocrinology departments of 348 representative hospitals across China, and filled out the information form. General information of the patients, duration of diabetes, medical history, diabetes complications and concomitant diseases, blood glucose and treatment regiment were recorded.The percentage of patients achieving glycated hemoglobin A1c(HbA1c) target <7.0% intergroup difference was compared with t test. Results A total of 75 168 eligible patients with type 2 diabetes were included in this study, 41 199 males and 33 969 females. The mean age was (59 ± 10) years. The mean duration of diabetes was (8 ± 5) years. The mean duration of diabetes before initiating human insulin was (4 ± 4)years. Of the patients, 79.83%started insulin therapy due to the poor glycemic control with oral hypoglycemic agents. The mean dose of human insulin was (30±11) U/day. The proportion of patients receiving basal-bolus, prandial, basal, and premixed insulin regimen was 7.37%, 8.25%, 6.71% and 77.66%, respectively. The total percentage of patients achieving HbA1c target was 31.84%. The rates of achieving HbA1c target between the patients with and without regular self-monitoring of blood glucose, insulin dosage was stable or not, with or without hypoglycemia, with or without svere hyperglycemia, waited 30 min for the meal after injection or not, developed healthy diet habits or not, with physical training or not, with or without concomitant diseases were all significantly different(χ2=8.30-2056.59, all P<0.05). Conclusions Poor glycemic control with oral hypoglycemic agents is the main reason for insulin initiation. Timely initiating insulin may bring clinical benefit.
An Analysis on the Factors Associated with Reuse of Insulin Pen Needles in Type 2 Diabetic Patients in China

Abstract:Incorrect injection technique of insulin is a common problem worldwide,which can reduce the efficacy of insulin therapy and lead to poor glucose control.A cross-sectional,multiple-center survey from 44 hospitals was conducted from Nov.2015 to Dec.2015.Non-parametric Kruskal-Wallis analysis of variance and Mann-Whitney U test were used for multi-parametric analysis.Multivariable logistic regressions were performed to identify the factors associated with independent variables.Overall,a single needle was used at an average of 3.79 times,with the highest of 60 times.Analysis across all study participants showed that the frequency of a single needle was positively correlated with age (P =0.029),duration of diabetes (P≤0.001) and number of complications (P≤0.001).Multivariable logistic regressions analysis of insulin pen needle reuse and needle compliance showed that age,income,education,marital status,duration of diabetes,quality of life and cost of drug were significantly related to needle reuse.From this survey,we found that reuse of insulin pen needle was common in China.Frequency of needle reuse was related to the patients' demographic characteristics (income level,age,region,education,employment status and self-care),health-related variables (duration of diabetes,number of complications and EQ-5D index scores) and utilization of health services (clinical visits,hospitalization and cost of medications).

Abstract:Objective To investigate the insulin injection-related knowledge of the registered nurses in first-class general hospitals in Shanghai and look into its influence factors. Methods Totally 331 nurses in the hospitals were enrolled in this study. Univariate and multivariate stepwise regression were used to analyze the factors which influenced the insulin injection-related . Results The aggregate score of insulin injection-related knowledge was 54.20 ± 7.64, and the score index was 67.75%. Multiple linear regression analysis showed that such factors as professional title and nursing experience and training in the endocrinology department were the main factors of insulin injection-related knowledge (all P<0.05). Conclusion We should enhance the training to promote the injection skills so that they can assist the patients to effectively control blood sugar and improve the quality of life.
Self-reported Diabetes Treatment among Chinese Middle-Aged and Older Adults with Diabetes: Comparison of Urban Residents, Migrants in Urban Settings, and Rural Residents

Abstract:Purpose: To compare self-reported diabetes treatments among Chinese urban residents, rural migrants in urban settings, and rural residents.Methods: Data from 993 diabetics at least 45 years of age were collected from the China Health and Retirement Longitudinal Study conducted in 2011. Multiple logistic regressions were performed to examine the associations between individual characteristics and diabetes treatments.Results: In total, 719/ 993 (72.4%) of the respondents received treatment for diabetes; of which < 8% used insulin therapy. Urban residents were more likely than rural residents to use insulin therapy [odds ratio (OR) = 0.44, confidence interval (CI): 0.20-0.99; p < 0.05], and more likely to use traditional Chinese medicine than migrants (OR = 0.30, CI: 0.10-0.96; p < 0.05). Overall, rural residents showed lower treatment rates than urban and migrant populations.Conclusion: Efforts to improve and enhance diabetes treatments, particularly among rural residents, are urgently needed in China. Abstract:Objective To evaluate the effectiveness and adverse effects of insulin pen versus traditional syringe in patients with diabetes mellitus(DM). Methods Literatures were searched for in PubMed,EMBASE,Cochrane Central Register of Controlled Trials,Full-text Database of Chineses Academic Journal,VIP Chinese Scientific Journals and Database,Wanfang Database. Clinical randomized controlled trials of insulin pens versus traditional syringe in treatment of DM were enrolled and analyzed comparatively. A Meta analysis was performed on glycated hemoglobin(HbA 1c),daily insulin dosage,patients' preference,adverse reactions in groups therapy and control. Results A total of 18 literatures were enrolled. There was no significant difference in HbA 1c between two groups 〔MD =-0. 00,95% CI(-0. 34,0. 33),P = 0. 99〕,therapy group did not waste insulin as compared with control group〔MD = 0. 06,95% CI(-1. 83,1. 94),P = 0. 95〕,DM patients intensely preferred insulin pen〔OR = 68. 84,95% CI(23. 21,204. 14),P 0. 000 1〕,thinking it easier to operate. No severe adverse reactions due to insulin pen were noted. Conclusion Insulin pen is a feasible treatment mode which is easy to be accepted by DM patients.
Availability, affordability and price components of insulin products in different-level hospital pharmacies: Evidence from two cross-sectional surveys in Nanjing, China

Abstract:The essential medicine——insulin cannot be easily accessed and afforded in many countries. To help address this issue, we evaluated the availability, affordability and price of insulin products in Nanjing, eastern China. Two cross-sectional studies were conducted in 2016 and 2018. A total of 56 hospital pharmacies were sampled, using a simplified and adapted World Health Organization/Health Action International (WHO/HAI) methodology. Prices were expressed as Median Price Ratios (MPRs) to Australian Pharmaceutical Benefit Scheme (PBS) prices. In addition, we investigated the price components of seven selected insulin products as a case study before and after the Online Centralized Procurement Policy for Hospital Drugs in May, 2018. Affordability was presented as the number of daily wages of the lowest paid unskilled government worker (LPGW) required to purchase 1000IU of insulin based on the average courses of treatment, approximately 30 days’ treatment. The availability of insulin products was very high in secondary hospitals and tertiary hospitals both in 2016 and 2018, but in community hospitals was very low. In 2018, the availability of prandial insulin products showed fluctuation compared to 2016. The availability of pre-mixed human insulin products was over 95% overall, and also very high (80%) in community hospitals in 2018. The prices of insulin products were much lower than PBS prices of Australian in this study, with the MPRs less than 1 (0.32 to 0.71 in 2016 vs. 0.30 to 0.68 in 2018) for all insulin types. But insulin products in Nanjing in 2016 and 2018 were considered unaffordable, because the number of daily wages of the LPGW needed to purchase for the 30 days treatment of insulin products ranged from 2.26 to 8.49 in 2016 and 1.88 to 7.09 in 2018. The manufacturers’ selling price contributed the main part (74.15% to 77.70% before and 74.86% to 91.51% after the implementation of the bidding policy) of the price components of target insulin brands. The availability of insulin products was high in secondary hospitals and tertiary hospitals, but lower in community hospitals. However, the affordability in community hospitals was better than other hospitals, but the insulin products were still unaffordable for patients on low incomes. Further improvements of the availability accessibility and affordability of medicines in advancing health insurance policies and lowering drug prices should be put forward.

Rejection of Insulin Therapy among Patients with Type 2 Diabetes in China: Reasons and Recommendations

Abstract:Background Insulin injection therapy is one of the most effective treatments for type 2 diabetes mellitus (T2DM). Many people with T2DM in western countries resist starting insulin therapy; whether the same is true in China is unknown. This survey-based study assessed acceptance and rejection of insulin therapy among individuals with T2DM in China and self-reported reasons for these therapy choices. It also examined what methods may be useful for increasing the rate of insulin acceptance. Methods A multi-center, cross-sectional survey was conducted between April and July 2010 to a convenience sample of inpatients and outpatients at 50 medical centers across 29 administrative divisions in China. Data were collected on sociodemographic and T2DM characteristics, therapy regime, and attitudes toward insulin therapy. Results A total of 6 043 patients were surveyed, and 5 961 complete questionnaires (98.6%) were used in the analysis. Just over half the respondents (3 460, 58.0%) reported negative attitudes to insulin therapy, including 2 508 of the 4 469 patients (56.1%) whose physicians had recommended it to them. Of the patients counseled to use insulin, 800 (17.9%) were unwilling to start therapy and cited the following reasons: inconvenience (64.3%); concerns over addiction (24.6%); pain (14.3%); side effects (14.1%); and high cost (13.6%). Logistic regression suggested that respondents would be more willing to undertake insulin therapy if they had received diabetes education, had positive attitudes to the treatment, had higher glycosylated hemoglobin level, or had suffered diabetes for a longer period or with more complications. Conclusions Patients with T2DM in China are often resistant to insulin therapy if they have been diagnosed with the disease for a relatively short time or if the disease has been relatively mild. Educating patients on the benefits of insulin therapy, not only at the initial diagnosis with T2DM but also when insulin therapy becomes necessary, will likely increase their willingness to undertake it.
