Abstract:TO THE EDITOR: Pneumatosis intestinalis (PI), also known as Pneumatosis cystoides intestinalis, is a rare condition defined as the presence of gas in the intestinal wall in the form of variably sized cystic spaces; it can be found anywhere in the gastrointestinal tract, but it is most common in the small and large intestine [1]. The histopathologic diagnosis of PI is easily made on resection specimens, in which the presence of submucosal or subserosal empty spaces lined by histiocytes and giant cells, presents little diagnostic dilemma [2], but the diagnosis, as in the case herein presented, is more challenging in biopsy material. Apreviously healthy 50-year-oldman complained of recurrent abdominal pain. The physical examination was unremarkable, and laboratory investigations showed no significant abnormalities. Colonoscopy revealed numerous sessile polyps, extending from the cecum to the left flexure, which was confirmed by the barium enema. Pathologic examination of the biopsy taken from one polyp confirmed the diagnosis of tubular adenoma. There was no familial history of familial adenomatosis polyposis and therewere noother polyps found through upper digestive endoscopy. The patient was referred to our hospital for surgery; he underwent subtotal colectomy with ileosigmoidien anastomosis. Four days later, he experienced an episode of fever (38.5 ) and acute abdominal pain. CT scan showed a pelvic abscess surrounding the anastomosis, drained surgically. An ileostomy was performed. Cross section of these “polyps” revealed cystic, sponge like spaces ranging in size from 0.2–1.5 cm within the submucosa (Figure 1A). Microscopic study showed unlined air-filled submucosal spaces, surrounded by inflammatory cells; including foreign body giant cells (Figure 1B). There were no dysplastic glands in the colonic mucosa covering those spaces. The diagnosis of pneumatosis coli was made. Pneumatosis intestinalis remains a challenging diagnosis on biopsy material, since the changes seen on mucosal biopsies (abnormal crypt branching, crypt dilation, cryptitis, crypt abscesses) are a secondary phenomenon and can overlap with other conditions such as inflammatory bowel disease or polyposis. Furthermore, the endoscopy, as in our case, may not suggest the diagnosis; the endoscopist describes the lesions as polyps, frequently multiple, or raised mucosal folds [2]. The initial slides were not reviewed in our case, but the over diagnosis of “adenoma” was probably caused by an altered mucosal architecture, related to the presence of submucosal cysts, which probably were not present in the biopsy material. Around15%of cases are primary and found, as inour patient, most frequently in the ileocecal and ascending colon regions, the restoccurs ina settingofpredisposing conditions (inflammation, emphysematouspulmonary, transplantion;;;) [1]. The prognosis of PI varies with the underlying illness.
