Abstract:Background: Thyroglossal duct cysts (TGDC) are one of the most common congenital anomalies found in the anterior neck region of children. Sistrunk’s procedure, described in 1920, is still considered the gold standard. Cyst recurrence is the most feared complication. Our study aims to describe the epidemiological, clinical, and therapeutic features of our patients, as well as reviewing the literature to identify factors that may explain the occurrence of complications and recurrences. Methods: The data of 32 patients aged 0 to 16 years who underwent TGDC surgery at Rabat's University Children's Hospital Infant Surgery Department "A", between 2015 and 2020 were retrospectively analyzed. We focused on aspects of clinical presentation, intra- and postoperative treatment, and long-term follow-up. Results: This study included 32 cases, out of which five (15,6%) had in their history postoperative recurrences of TGDC (without excising the central part of the hyoid bone), 09 patients had per-operative symptoms of infection, and 4 out of them had intraoperative cyst rupture. Only one out of these four patients had recurrence (3.1%); the only significant risk factor of TGDC recurrence was the non-adherence to the Sistrunk technique. Conclusion: TGDC is the most common neck anomaly in children. This condition can be managed successfully without complications and with a low recurrence rate. Preoperative infection and cyst rupture did not influence the recurrence rates.
Diagnostic and Surgical Approach of Thyroglossal Duct Cyst in Children: Ten Years Data Review

Abstract:INTRODUCTION: Thyroglossal duct cysts are the most common congenital midline neck swelling in children.AIM: To evaluate the clinical features, treatment, incidence of complications and outcomes in children with congenital midline neck lesions and more specifically with thyroglossal duct cyst, treated in our department the last ten years. It is a retrospective study.MATERIALS AND METHODS: The aggregated data revealed 59 patients with congenital midline neck lesions, of which 33 patients were with thyroglossal duct cyst (TDC). The diagnosis of TDC was made by physical examination, ultrasound (US) in all cases, and for complicated cases a magnetic resonance imaging (MRI) was performed. In all cases followed histopathological conformation of thyroglossal duct cyst.RESULTS: Fifty nine patients were admitted with midline cystic neck mass and the histopathological evaluation revealed, 33 cases (55.9%) of thyroglossal duct cyst, 14 cases (23.7%) of dermoid cysts, 3 cases (5.1%) of second branchial anomalies, 4 cases (6.7%) of inflammation of unknown cause, 4 cases (6.7%) of lymph nodes and 1 case (1.7%) of capillary skin haemangioma. More specifically, 38 patients were admitted with preoperative diagnosis of thyroglossal duct cyst, but in 5 patients pathological investigation demonstrated the presence of dermoid cyst. The mean age of the 33 patients with TDC at the time of surgery was 6.125 years, ranging between 9 months and 13 years with 10 patients younger than 3 years. Of the 38 patients with initial diagnosis of thyroglossal duct cyst, only four patients (10.5%) had a simple cyst excision and complication rate was 25% (1 case with recurrence). In these patients, the histopathological examination showed that three of them had a dermoid cyst and one had thyroglossal duct cyst. The other 34 patients (89.4%) were treated by Sistrunk's procedure, with two cases (5.9%) were proven on histology to be non TDC. In this group the complication rate was 3.03% (1 case with wound infection).CONCLUSION: The inappropriate surgical approach due to misdiagnosis or the incomplete surgical procedure remains the impact factor for recurrence. The modification of Sistrunk's procedure remains the main surgical technique that can provides good results with low rate of complication (3.03%) and recurrence (0%) as shown of our collecting data.
Thyroglossal duct cyst surgery: A ten-year single institution experience

Abstract:Objective: 1. Review our institution's experience with thyroglossal duct cyst (TGDC) excision. 2. Determine if TGDC recurrence rate is influenced by surgical technique, preoperative infection, or surgeon specialty. Methods: We conducted a retrospective analysis of patients at a tertiary care children's hospital who underwent excision of neck mass with proven TGDC histopathology from 2005 to 2015. Patient demographics and presentation, preoperative workup, surgeon specialty, procedural details, lesion characteristics, and complication rates including recurrence were evaluated. Results: 108 patients (51% male, 49% female), aged 6 months to 20 years (mean 6 years) met inclusion criteria. 121 procedures were performed: 96 (79.3%) for primary disease and 25 (20.7%) for recurrence. 32 patients (27.8%) were infected preoperatively, resulting in a significant increase in complications and recurrence (46.9% and 28.1%, respectively). Lack of preoperative imaging was correlated with increased recurrence (p = 0.0002). Recurrence rate after treatment for primary (11.3%) versus secondary disease (24%) was not significant (p = 0.23). Total recurrence rate differed between ENT (9.1%) and Pediatric Surgery (PS) (27.3%) (p = 0.0172). Difference was not significant for recurrence in primary disease (10% ENT, 14.8% PS, p = 0.49), but was for secondary disease (5.6% ENT, 71.4% PS, p = 0.002). Modifications of Sistrunk's procedure did not result in increased rates of recurrence (p = 0.1273). Conclusion: Preoperative TGDC infection and lack of imaging led to a significant increase in postoperative complications, including recurrence. Surgical specialty significantly affected recurrence rates following excision overall and in secondary disease. Recurrence was not affected by surgical technique, age, sex, cyst size, or mucin rich histology.

Abstract:BACKGROUND: A thyroglossal duct cyst is the most common form of congenital anomaly in the neck. Surgical removal is very effective. However, in some cases, a cyst recurs. The purpose of this study was to identify factors that predispose to recurrence of a thyroglossal duct cyst.METHODS: A retrospective study was conducted of consecutive patients who underwent surgical resection for histologically confirmed thyroglossal duct cysts between 1998 and 2013 in a tertiary referral center.RESULTS: Two hundred seven patients were included. The overall recurrence rate was 9.7%. The most important factor predicting recurrence was the type of resection: recurrence rate was 5.3% after the Sistrunk procedure, and 55.6% after plain excision (p < .001). The only other factor that was significantly associated with chance of recurrence was postoperative infection.CONCLUSION: The Sistrunk procedure is the treatment of choice for thyroglossal duct cysts because it yields low recurrence and morbidity. Postoperative infections, rather than preoperative infections, are associated with recurrence.

Abstract:PURPOSE: To determine the rate of recurrence following surgical treatment of thyroglossal duct cysts (TDGCs) in children.METHODS: A search of electronic databases (Pubmed and MEDLINE) was performed in order to identify studies concerning surgical treatment of TGDCs in children published between 1980 and 2012. The following terms were used: "thyroglossal duct cysts", "recurrent", "recurrence", "children", "pediatric".RESULTS: Sixteen articles were selected with the majority published during the last decade. The total number of included subjects was 1233. The mean (95% confidence interval (CI)) rate of recurrences was 10.7% (9.1%-12.6%). Eleven studies involving 751 subjects reported the frequency of recurrences separately for primary cases. The mean (95% CI) rate of recurrences was 10.8% (8.7%-13.3%). Five studies reported separately results of surgery for secondary cases. The mean (95% CI) rate of recurrences was 20.0% (12.2%-30.8%). Eight of the sixteen studies reported data on risk factors for recurrence. The use of the Sistrunk technique is a protective factor, whereas children who experienced repeated episodes of infection prior to surgery are exposed to a higher risk of recurrence.CONCLUSION: Recurrences after surgery for TDGCs remain a clinically relevant issue. One of ten operated children experiences a recurrence. Available data support the use of the Sistrunk technique but further studies aimed at improving the clinical management of TDGCs are required.

Neuroprotective agent riluzole dramatically slows inactivation of Kv1.4 potassium channels by a voltage-dependent oxidative mechanism.

Abstract:The modulation of Kv1.4 K+ channels by the neuroprotective agent riluzole was studied in bovine adrenal zona fasciculata cells by using whole-cell patch clamp. At concentrations ranging from 1 to 100 microM, riluzole reversibly inhibited Kv1.4 channels (IC50 = 70 microM) and irreversibly slowed Kv1.4 inactivation. Riluzole (100 microM) increased the inactivation time constant (tau(i)) from a control value of 28.9 +/- 3.9 to 623 +/- 47.6 ms (n = 13). The slowing of bKv1.4 inactivation was not affected by substituting poorly hydrolyzable nucleotides for ATP in the pipette solution, or by the addition of cAMP. Riluzole-induced slowing of bKv1.4 inactivation was nearly eliminated by the presence of the antioxidant reduced glutathione (3 mM) or dithiothreitol (3-5 mM) in the recording pipette, or when cells were superfused with riluzole at a holding potential of -40 mV rather than -80 mV. These results are consistent with a model in which riluzole inhibits bKv1.4 currents and slows inactivation by separate mechanisms. Slowing of inactivation is independent of protein kinases, but probably involves oxidation of a cysteine in the N-terminal inactivation domain. Failure of riluzole to slow inactivation when applied to a depolarized cell suggests that this cysteine is protected in an inactivated Kv1.4 channel. The neuroprotective action of riluzole involves inhibition of glutamate release from presynaptic terminals within the central nervous system. Kv1.4 K+ channels are distributed throughout the brain in axons and nerve terminals, including those from which glutamate is released. The pronounced slowing of Kv1.4 inactivation by riluzole in these neurons could be an important mechanism underlying the inhibition of glutamate release and the therapeutic actions of this drug.
Experience of Sistrunk’s Operation for Paediatric Patients of Thyroglossal Duct Cyst at Multicenter Tertiary Care Hospitals

Abstract:Background: The Thyroglossal duct cyst (TGDC) is a congenital defect resulting from a persistent thyroglossal tract. It usually exhibits as a painless nodule in the neck midline. In physical evaluation, it moves with swallowing and on tongue protrusion. The physical examination and detailed history are adequate to make an accurate diagnosis preoperatively. Neck ultrasound and Fine needle aspiration cytology are additional valuable diagnostic tools. Though it is a congenital abnormality, it usually appears in the 1st and 2nd decades of life. Sistrunk operation is the treatment of choice for cysts of the thyroglossal duct. Aim: The purpose of this analysis was to govern the age at onset, site of cyst in relation to the hyoid bone, time of thyroglossal duct cysts presentation and to observe postoperative complications of Sistrunk’s operation. Study Design: A Retrospective study. Place and Duration: In different Departments like Paediatric Surgery, Otolaryngology, Faciomaxillary from January 2016 to January 2021. Methods: A total of 76 patients operated in 5 years were included. The study included patients treated with the Sistrunk procedure in whom the presence of the thyroglossal duct was confirmed after clinical examination and ultrasonography. Demographics, age at admission, wound infection, hematoma, abscess, airway injury, seroma, stitch sinus, wound dehiscence and recurrence were observed. Results: There was a slight high prevalence among men. Most of the thyroglossal cysts were seen in the pediatric inhabitants and below the hyoid bone and can recur. Conclusion: Thyroglossal duct cyst is a pediatric disease. The Sistrunk procedure is the treatment of choice for TDC. The risk of serious complications is minimal when the procedure is carried out with certain nuances which we have described. The complications are minor and concerned to the wound. Recurrent thyroglossal duct cysts: a clinical and pathologic analysis

Abstract:OBJECTIVE: To analyze, in detail, the clinical and pathological findings in a patient population that had undergone previous Sistrunk procedures for the removal of thyroglossal duct cysts, in order to identify any possible factors that could be related to recurrence after 'definitive' surgery.SETTING: A large pediatric tertiary care center.METHODS: A retrospective chart review was performed to include all patients treated at a single center for thyroglossal duct cysts with a Sistrunk procedure, between 1978 and 1992 inclusive.RESULTS: A total of 108 consecutive patients were analyzed. We noted that the presence of recent preoperative infection of the cyst was the only statistically significant (P < 0.05) clinical difference noted between the successful and the unsuccessful surgical groups. However, pathological analysis revealed that there was a substantially greater number of multiple thyroglossal duct tracts (P < 0.05) noted in the group that failed a Sistrunk procedure.CONCLUSIONS: We feel that multiple thyroglossal tracts may play an etiologic role in some recurrent cysts. Thus, a wide conservative excision, including the middle two thirds of the hyoid bone, is necessary in order to include any multiple tracts in the resection.

Abstract:<h3>Introduction</h3><p>Thyroglossal duct cysts (TGDCs) result from incomplete involution of the thyroglossal duct and are resected with a Sistrunk-procedure. We studied and graded severity of postoperative complications in children who underwent this procedure, with corresponding risk factors.</p><h3>Methods</h3><p>In our electronic health record system we reviewed the medical records of all patients aged <18 years, with surgically treated TGDC between 01-01-2005 and 31-12-2015 in two university hospitals. Risk factors (age, gender, recurrence at presentation, treatment hospital, cyst inflammation, cyst rupture, drain placement, antibiotics or postoperative infection) were studied by univariate analysis. The Clavien-Dindo surgical complication classification was used as postoperative surgical grading system.</p><h3>Results</h3><p>Of the ninety-one patients, with a mean age of 4.4 years, seven were referred from other hospitals with a recurrent TGDC. 24 patients (26.4%) had a complication. Hemorrhage and resection of thyroid cartilage were the most severe complications. Recurrence and wound infection (both n=7, 7.7%) were most common. We could not identify risk factors for these complications.</p><h3>Conclusions</h3><p>the treatment of children with thyroglossal duct cysts shows a considerable amount of complications. Pre-selected risk factors did not show any significant correlation with these complications.</p>

Abstract:Ear, Nose &Throat Journal, Ahead of Print. Objectives: The study aims to retrospectively summarize the clinical features of pediatric thyroglossal duct cyst (TGDC), investigate the efficacy of the modified Sistrunk (mSis) procedure, and analyze the recurrence risks. Methods: The clinical data of 391 children with TGDC admitted to Beijing Children's Hospital affiliated Capital Medical University and Baoding Children's Hospital from March 2012 to December 2021 were retrospectively analyzed. All patients underwent cervical ultrasound for preoperative evaluation. Twenty cases had magnetic resonance imaging and 8 cases had computed tomography for further evaluation. All patients underwent the standard mSis procedure, and clinical manifestations information, surgical information, complications, and prognosis were analyzed. Results: Among the 391 TGDC cases, 118 (30.2%) had a history of recurrent neck infection and 36 (9.2%) had undergone previous neck cyst and fistula resection surgeries, initially diagnosed as neck cyst (22 cases), TGDC (12 cases), or branchial fistula (2 cases), with only 6 cases having undergone partial hyoid bone resection in the previous operation. During the 15 to 156 months of follow-up, 10 children experienced local wound infection, but no other complications were reported. The recurrence rate was 2.30%, and the recurrence time ranged from 0.5 to 34 (average, 7.2) months post surgery. In the Poisson regression model examining factors related to recurrence, the P values of the 3 factors were 2 cm) were independent risk factors for recurrence in pediatric TGDC.Level of Evidence: IV

Management of suspected thyroglossal duct cysts

Abstract:Aim of study: The aim of this study was to evaluate management of children with an anterior midline neck swelling by establishing 1) whether a preoperative ultrasound scan (USS) was appropriately requested, performed and reported; 2) whether there was preoperative infection; 3) whether a Sistrunk procedure was performed; 4) the rate of thyroglossal duct cyst (TGDC) recurrence following simple excision vs. Sistrunk procedure. Methods: A single centre retrospective study of children who underwent surgery for anterior midline neck swelling between April 2000 and May 2015 at our institution was performed. These were identified using a clinical coding system, and data were collected from electronic medical records, radiology, and histopathology reports. Recurrence rates between simple excision and Sistrunk groups were compared using Chi-square test. Main results: 227 patients were identified (115 male, 112 female). 169 (74%) had a preoperative USS. The presence of a thyroid gland was stated in 79% of USS reports. This increased to 92% when the requesting surgeon had specifically asked about this. 48 (21%) patients underwent simple excision, while 175 (77%) had a Sistrunk procedure. Recurrence was significantly more likely following simple excision than a Sistrunk procedure (29% vs 6.9%; P<0.0001). Of 25 TGDC recurrences, 9 (36%) had an inconclusive or alternative histopathological diagnosis at first operation. Conclusion: Preoperative USS should be performed in all patients with an anterior midline neck swelling. Appropriate requesting increases likelihood of a report confirming (or otherwise) the presence of a thyroid gland. A Sistrunk procedure is the operation of choice in all children presenting with an anterior midline neck swelling. The surgeon cannot reliably differentiate a TGDC from alternative pathology intraoperatively. Level of evidence: Treatment study: level IV.
Thyroglossal duct cysts: presentation and management in children versus adults

Abstract:OBJECTIVES: To determine if the clinical presentation of thyroglossal duct cysts (TGDC) varies between children and adults and whether this knowledge helps optimize the surgical management.METHODS: We retrospectively identified all patients with TGDC managed in our department between 1992 and 2002. We reviewed the patients' charts and recorded their gender, age at diagnosis, clinical presentation, radiologic imaging, surgical management, post-operative complications, and recurrence rate and compared the variables between the children and adults.RESULTS: Twenty-one children and 41 adults were treated for TGDC. Of the children, 57% were male and 43% were female, whereas 49% of the adults were male and 51% were female (P = 0.53). The average age was 6 +/- 5 years in children and 45 +/- 16 years in adults, which demonstrates a bimodal distribution. Forty-three percent of children and 42% of adults presented with an infected neck mass (P > 0.99). Among our patients, 96% of the adults and 100% of the children underwent a Sistrunk operation. Four children developed a wound infection that resolved with antibiotics. One adult developed a haematoma and another developed a wound seroma. There was one recurrence among adults and one among children, both of whom were treated with a second Sistrunk procedure.CONCLUSIONS: There appears to be a bimodal distribution for age at presentation of TGDC. Since the differential diagnosis among adults is broader, the opportunity for misdiagnosis is greater. However, once the correct diagnosis is made, the surgical management and post-operative outcome between adults and children is the same.

Abstract:Introduction: Thyroglossal duct cyst (TGDC) is the most common form of congenital anomaly in the head and neck region with prevalence of 7%. Embryologically the thyroglossal duct tract gets atrophied by fifth to tenth week of gestation. If it does not atrophied it results in TGDC and operation is the treatment of choice. However, in spite of standard surgical treatment there is recurrence of cyst. Eight percent of thyroglossal duct cyst may reoccur after adequate surgical excision. Objectives: To identify the recurrence of thyroglossal duct cyst after standard sistrunk’s operation. Methodology: A retrospective chart review performed in all the patients who were diagnosed with thyroglossal duct cyst and were managed by Sistrunk operation in B.P Koirala Institute of Health sciences, department of ORL and Head and Neck surgery from 2013 January1to 2015 December 31.The study was conducted on 37 patients’ age ranging from 4years to 68years. All the patients who had undergone Sistrunk’s operation and histopathologically diagnosed with thyroglossal duct cyst were analysed through hospital records. All records were reviewed for age, sex, location of cyst in neck and with relation to hyoid bone, postoperative complication and recurrence. Results: There was male predominance 67.56% and female only 32.43%.The most of the patients were between 1-15 yrs age group (48.64%). Thirty two patients presented with infrahyoid thyroglossal cyst and five patients with suprahyoid thyroglossal cyst. The post operative complications were in the form of wound dehiscence, haematoma, seroma, infection and there were two cases with recurrence out of thirty seven patients after Sistrunk operation in the form of persistence discharging sinus. Conclusion: A Sistrunk’s operation is effective in treating primary thyroglossal duct cyst with a comparably low recurrence rate. The intraoperative rupture of cyst is the most important risk factors predicting postoperative recurrence of thyroglossal duct cyst. Meticulous dissection of the thyroglossal duct cyst is very essential for the better surgical outcome.

Thyroglossal duct cyst: Factors affecting cosmetic outcome and recurrence

Abstract:<h3 class="article-section__sub-title section1"> Background</h3><p>Thyroglossal duct cyst (TDC) is the most common congenital abnormality in the neck in children. The purpose of this study is to perform a comprehensive review of all cases of TDC operated at a single institution and to evaluate the factors that influence the rate of recurrence, and the aesthetic outcome of the surgery through patients' follow‐up.</p><h3 class="article-section__sub-title section1"> Methods</h3><p>All cases of TDC surgically treated at the Department of Pediatric Surgery at Meyer Hospital from January 2005 to December 2016 were selected. Charts from 248 patients were reviewed and risk factors for recurrences evaluated. A questionnaire was submitted to patients' parents to determine which and if long‐term post‐operative complications were present. Standardized neck pictures were asked for, to evaluate the cosmetic result of the procedure. Microsoft Office Excel 2007 for Windows and Graphpad Prism 6 were used for data management and statistical analysis.</p><h3 class="article-section__sub-title section1"> Results</h3><p>Simple cyst excision and post‐inflammatory fibrosis (<i>p</i><0.05) were assessed as important risk factors for the recurrence of TDC. Recurrence rate through Sistrunk's procedure was 5%. Variables such as post‐inflammatory fibrosis before surgery (<i>p</i><0.001), the positioning of a drain (<i>p</i><0.01) and the development of recurrence (<i>p</i><0.001), negatively influenced the cosmetic result. No thyroglossal duct cyst carcinoma and no long‐term post‐operative complications were observed. </p><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Recurrence rates were higher in patients who underwent simple cyst excision instead of Sistrunk's procedure, as already reported in literature. Presence of post‐inflammatory fibrosis and positioning of drain at surgery were associated to higher rates of recurrence, as well as worse cosmetic outcome.</p><p>This article is protected by copyright. "Extended" Sistrunk procedure in the treatment of recurrent thyroglossal duct cysts: a 10-year experience

Abstract:Objective: We report a 10-year experience of children with recurrent thyroglossal duct cysts (TGDCs) who have been treated using the "extended" Sistrunk procedure. Methods: We performed a retrospective review of TGDC surgery from 2004 to 2013. Sistrunk operation was the procedure of choice in all patients. Seven children had TGDC recurrence. All of them underwent "extended" Sistrunk procedure. Follow up ranged from 6 months to 8 years. Results: There were no gender differences, all recurrences presented within 12 months follow-up in the same location of the primary cyst. Five out of 7 (71%) patients have been treated for preoperative and 2/7 (29%) for postoperative infection at the time of primary surgery. Pathological examination of the surgical specimens showed a single tract in 2 children (29%) and multiple tracts in 5 (71%). We did not observe postoperative complications or further recurrences. Conclusion: Our experience suggest that recurrent TGDCs are equally common in both sexes, develop in the same location of the primary cyst and recur more commonly after perioperative infections. The "extended" Sistrunk procedure is highly effective and safe in treating recurrent TGDCs also if multiple duct tracts are detected.
Optimal age of surgery for children with thyroglossal duct cysts: A single-institution retrospective study of 340 patients

Abstract:Objective: The recommended age of surgery for thyroglossal duct cysts (TGDCs) in children is inconclusive. This study aimed to explore the optimal age of surgery by analyzing the natural history of the disease and the association between the age of surgery and postoperative complications. Methods: All TGDC patients who underwent a modified Sistrunk procedure at our hospital between March 2010 and May 2022 were reviewed retrospectively. The evaluation focused on the age of preoperative symptomatic cystic infection, pathological inflammation, postoperative wound infection, and recurrence. Results: Of the 340 patients included in the study, the median age of surgery was 47.5 months (IQR, 24.1–61.6). Preoperative symptomatic cystic infection and pathological inflammation frequencies were 27.1% ( n = 92) and 48.5% ( n = 165), respectively. The cumulative hazard of symptomatic cystic infection and pathological inflammation increased steadily with age. The ages of 50% cumulative incidence of symptomatic cystic infection and pathological inflammation were 97 months and 71 months, respectively. Postoperative wound infection was higher in patients of younger age (OR = 0.96, 95% CI, 0.93–0.98, P < 0.001) and with symptomatic cystic infection (OR = 8.16, 95% CI, 2.54–36.86, P = 0.002). There was no significant association between the age of surgery and recurrence. Conclusion: Although wound infection was weakly associated with younger age, the symptomatic cystic infection increasing with age has a more remarkable impact on wound infection after the Sistrunk procedure. The recurrence rate did not increase in young patients receiving surgery. Therefore, the Sistrunk procedure was safe and effective at a young age, and prompt operation in children with TGDC once diagnosed was reasonable.
