Abstract:Purpose: Outdoor physical activities (OPA) can not only improve the physical fitness and health of autistic preschool children, but also provide an indispensable opportunity for their social learning and interaction. However, parents’ intentions of providing OPA for their autistic children are complex due to reasons such as potential risk. Theory of Planned Behavior(TPB)has conceptualized that attitude (AT), subjective norms (SN), and perceived behavioral control (PBC) are influential factors of behavioral intention. Perceived risk and perceived usefulness are suggested to associate with one’s intention, therefore, the purpose of this study was to test an extended TPB model on parent’s intention of providing OPA among parents of autistic children. Methods: This was a cross-sectional study. A convenient sample of parents of 301 autistic preschoolers (mean age = 3.45±1.10 years old; boys=219; girls = 68) were recruited. Parents reported to questionnaires assessing their AT, SN, PBC, and intention (Ajzen, 1991; Yang, 2017) as well as perceived risk and perceived usefulness of providing OPA for their autistic children (Davis et al., 1989; Yu, 2016). Structural Equation Modeling (SEM) using SmartPLS software was employed to investigate the extended TPB model by including perceived risk and perceived usefulness (exogenous variables) with AT, SN, PBC to synergistically predict intention (endogenous variable). Results: The results revealed that autistic preschooler parents’ perceived usefulness ( p < 0.01, f 2 = 0.087)had a significant and positive prediction on intention in addition to the significant contributions from AT ( p < 0.01, f 2 =0.053), SN ( p < 0.01, f 2 =0.047), and PBC ( p < 0.01, f 2 =0.060). However, perceived risk ( p < 0.01, f 2 = 0.070) had a significant and negative prediction on intention. Besides, perceived usefulness ( p < 0.01, f 2 = 0.108) was positively correlated with AT and AT partially mediated the association between perceived usefulness and intention ( Variance Accounted For = 23.2%). In general, perceived usefulness, PBC, AT, SN and perceived risk had a high explanatory power to intention ( R 2 =0.536). Conclusion: The extended TPB model demonstrated to be a useful framework to explain autistic preschooler parents’ intention of providing OPA for their autistic children. Perceived risk and perceived usefulness are critical to parents’ intention in addition to PBC, AT and SN, which should also be the targets of intervention in practice. This finding also suggests practitioners may help parents’ understand the usefulness of OPA to help autistic preschoolers’ parents form positive attitude of OPA, then boosting their intention to engage their autistic children in OPA.

Abstract:Background Autistic children can derive numerous benefits from engaging in physical activity (PA), necessitating parental support for active participation. However, parents often lack the foundational knowledge to effectively facilitate PA for their children. Despite the significance of this issue, limited research has explored the outcomes of parent-mediated PA interventions for autistic children. Objective The purpose of this study was to examine the attitudes and intentions of parents to include their autistic children in PA after participating in a 12-week PA intervention. Methods Employing the Theory of Planned Behavior as a framework, we conducted a descriptive-qualitative investigation. Fifteen parents participated in semi-structured interviews, with the data transcribed and analyzed using thematic line-by-line analysis. Results Three themes emerged, elucidating how parents fostered PA opportunities for their autistic children were constructed: (a) Awareness and reinforcement of PA benefits, (b) Keep the momentum going, and (c) From intentions to behaviors. Conclusions The study findings indicate a positive impact on parental attitudes and intentions toward engaging in PA with their autistic children after participating in the intervention. his study highlights the significance of parent-mediated interventions in fostering PA for autistic children and suggests avenues for future research and interventions.

Abstract:Introduction: In Luxembourg, competency-based practices (CBP), differentiated instruction (DI), and formative assessment (FA) have been imposed by the 2009 school law. Referring to the Theory of Planned Behavior (TPB), this study examined factors influencing the implementation of these practices in classrooms. Methods: Teachers participated in an online survey assessing their attitudes, subjective norm, perception of behavioral control, intention, and pedagogical practices regarding CBP, DI, or FA. Measurement models were used in structural equation models testing the TPB. Results: If the main relationships postulated by the theory were confirmed, some inconstancies were observed depending on the targeted practices. Structural equation TPB models controlling for gender, experience, teaching level, and socio-economic level of the school population explained between 20 and 45% of the variance in teachers' practices, and between 65 and 75% of the variance in teachers' intention to use these practices. Discussion: The relevance of the TPB for studying teaching practices and implications for professional training are discussed.

Injury hospitalizations: using the nationwide inpatient sample.

Abstract:BACKGROUND The aims of this study are to develop estimates of discharge rates and frequencies for all injury-related hospitalizations in the United States for the year 2000 and to characterize patterns of hospitalized injury and anatomic region using a modified Barell Matrix. The utility of the Nationwide Inpatient Sample (NIS) for providing national estimates of hospitalized injuries will be discussed. METHODS This study is a retrospective analysis of hospital discharge data using the Nationwide Inpatient Sample. All hospital discharges with a primary diagnosis of injury were selected. Total number of hospitalizations, annual discharge rates, and 95% confidence intervals were calculated by body region, nature of injury, and injury mechanism. Number of injuries by age, sex, body region, and nature of injury were also calculated for falls and motor vehicle crashes. RESULTS In 2000, there were an estimated 1,690,780 hospital discharges with a primary injury diagnosis. Discharge rates were highest for the oldest age groups. Falls and motor vehicle crashes were the leading causes of hospitalization. Fracture was the most common diagnosis and lower extremity injury was the most common anatomic region. CONCLUSIONS Hospital discharge data adds another dimension to our understanding of the total injury burden. The Nationwide Inpatient Sample may be useful in providing national estimates of hospital discharges.
Self-Directed Telehealth Parent-Mediated Intervention for Children With Autism Spectrum Disorder: Examination of the Potential Reach and Utilization in Community Settings

Abstract:BACKGROUND: There is a significant need for strategies to increase access to evidence-based interventions for children with autism spectrum disorder (ASD). One novel approach is to train parents to use evidence-based interventions for their child with ASD via telehealth. Pilot work examining the efficacy of one such program, ImPACT Online, demonstrated a high rate of parent program engagement, low attrition, and associated gains in parent learning and child social communication.OBJECTIVE: The objective of this study was to conduct an open trial of ImPACT Online to better understand its dissemination potential.METHODS: We examined the reach and representativeness of families who registered (n=36) compared to families who were referred (n=139) to the open trial for one referral site. We then compared the demographics of all families who enrolled in the open trial (n=112) to families who enrolled in one of two controlled trials of the same program (n=50). We also examined metrics of program engagement for the open and controlled trials, the relationship between program engagement and changes in parents' intervention knowledge, and program evaluation for the participants in the open trial.RESULTS: In total, 25.8% (36/139) of the parents who were given information about the program at their child's diagnostic feedback session registered with the program. The parents who enrolled in the open (OT) and controlled trials (CT), respectively, were similar in gender (OT: 84.8% (95/112); CT: 88% (44/50), female), marital status (OT: 80.4% (90/112) ; CT: 69.6% (32/46), married), education (OT: 58.0% (65/112); CT: 54.0% (27/50), college degree or higher), and employment status (OT: 58.0% (65/112); CT: 65.3% (32/49), employed outside the home). The child participants were similar in terms of gender (OT: 83.0% (93/112); CT: 76.0% (38/50), male) and race and ethnicity (OT: 38.4% (43/112); CT: 24.0% (12/50), minority). However, the mean chronological age of the child participants in the open trial group was significantly higher (Mean=60.0 months) than in the controlled trial group (Mean=43.0 months), with t160=5.22, P<.001. Parents in the open trial engaged with the program at a significantly lower rate than the controlled trial, F3,81=21.14, P<.001. Program engagement was significantly associated with gains in parent intervention knowledge across both the groups, beta=.41, t=2.43, P=.02. Participants in the open access trial evaluated the program highly, but several barriers were noted.CONCLUSIONS: These data suggest that additional strategies may need to be developed to support families in using telehealth-based parent-mediated intervention in community settings.

Abstract:Early Childhood Special Education teachers select practices to use to promote social communication development for their students with autism spectrum disorder. Understanding what evidence-based practices teachers select and why can inform the development of dissemination and implementation supports at the critical Adoption-Decision stage of implementation. The researchers used discrete choice analysis to examine and test the effects of Early Childhood Special Education teachers’ beliefs (self-efficacy, attitude, subjective norms; Theory of Planned Behavior) about key evidence-based practices on their practice selections. To assess malleable determinants of practice selection, this study addressed these aims: (1) assess Early Childhood Special Education teachers’ beliefs (attitudes, subjective norms, and self-efficacy) about key evidence-based practices, (2) describe which evidence-based practices teachers selected, and (3) examine whether teachers’ beliefs predicted practice selections. A web-based survey gathered responses from 222 Early Childhood Special Education teachers. Participants had the most favorable beliefs about naturalistic intervention and the least about discrete trial teaching and scripting. These beliefs aligned with the frequencies with which these practices were selected, and teachers’ beliefs predicted which practices they selected. Together, these findings suggest that beliefs serve as determinants of Early Childhood Special Education teachers’ practice selections that can be acted on to tailor pre-implementation supports and improve implementation from the start. Lay abstract Preschool special educators’ are more likely to choose an educational practice to teach a young child with autism a social communication skill if they have positive beliefs about it. We asked preschool special educators to read a description of an autistic student and their social communication goal and imagine they were the student’s teacher. We then asked them to pick one of five practices to teach the student. We also asked them questions to understand their attitudes about, confidence in their ability to use, and their perception of their coworkers’ support of each practice. There are many research-based practices that a teacher could use to help children learn, and preschool teachers often make these decisions for their students. Teachers’ beliefs varied in how supportive they were of each practice, and research shows people are more likely to do something that their beliefs support. In this study, they had more supportive beliefs and were more likely to use some practices, like naturalistic intervention, than other practices, like discrete trial teaching. By knowing this, researchers can help teachers use practices that their beliefs support and help change teachers’ beliefs to be supportive of a practice they may need to use.

Abstract:The diagnosis and clinical management of diabetes are ba ed upon the detection and correction of an abnormal glucose-tolerance test. However, as this test is merely indicative of altered carbohydrate metabolism, 'true' control of the diabetic state can only be achieved by the accurate definition and treatment of the aetiological agents reponsible in each individual patient. The atal Indian diabetic, for example, conforms to a pattern which is highly suggestive of adrenocortical hyperactivity. Thus these patients rarely develop ketosis, are relatively resistant to insulin, are frequently aglycosuric in the fasting state, and have hirsutes and trunk obesity. As it is generally accepted that certain diseases have a racial prevalence, the following study was instituted with the object of comparing: (a) The excretion pattern of the 17-hydroxycorticoids (17 OHCS) and the 17-ketosteroids (oxosteroids or 17-KS) in pregnant non-diabetic Indian females during the third trimester of pregnancy with similar values in the pregnant atal Indian diabetic. (b) The effect (if any) of diabetic 'control' on glucocorticoid and ketosteroid activity as measured by their excretory products.
Room to Improve: Predictors of Attendance and Outcomes Within Mindfulness-Enhanced Behavioral Parent Training

Abstract:Abstract Objectives Parents across a broad sociodemographic spectrum struggle with their children’s difficult behaviors and need effective and engaging interventions. We developed a mindfulness-enhanced behavioral parent training group program (MeBPT) aiming to be more inclusive and enable positive outcomes. This study investigated whether MeBPT can meet diverse needs across child, parent, family, and program-delivery factors, and explored whether improvements in parent-rated child behavior are predicted by changes in mindful parenting, parenting approach, or both. Method Participants were 338 parents of 3- to 12-year-old children presenting with externalizing problems to a university clinic in regional Australia. Factors that significantly correlated with improvements in parent and child outcomes or attendance were included in regression analyses. Results Improved parent-rated child behavior was predicted by change in mindful parenting ( p = 0.019) but not parenting approach ( p = 0.305). Attendance was high across all factors, and particularly for older parents. Improvements in parent well-being, approach, mindfulness, and child behavior were similarly large for families across child and family characteristics. Low-income families exhibited less improvement in parental well-being ( p = 0.012), although demonstrated similar improvements in child behavior, parenting approach, and mindful parenting. Families with more severe problems at baseline showed largest gains ( p < 0.002). Parental self-awareness and acceptance were important change agents. Conclusions MeBPT appears effective across various sociodemographic backgrounds and is particularly beneficial for families presenting with more severe behavioral problems. Controlled research with ethnically diverse families is needed to confirm the benefits of MeBPT and to allow examination of mediators and moderators.

Abstract:Background: Social ABCs is a caregiver-mediated Naturalistic Developmental Behavioral Intervention for toddlers with confirmed/suspected Autism Spectrum Disorder (ASD), with evidence in controlled research settings. Information is lacking on implementation in community settings. We reported on the treatment effectiveness of this program within a community setting, and the current paper describes the implementation phase of this work. Distinguishing between treatment and implementation effectiveness is critical for transporting interventions from laboratory to community. Objectives: Describe the implementation of Social ABCs through a large public autism service, supported by a research-community partnership. Methods: We describe this project through the Exploration, Preparation, Implementation, Sustainment (EPIS) framework as it focuses on implementation of evidence-based practices in publicly funded services. We apply this framework to the reporting stage. This project took place in the context of a 3-year government-funded pilot at a hospital-based publicly funded autism service. Participants: Program developers; Autism Service team; toddlers with suspected/confirmed ASD aged 14–34 months ( M = 25.18 months) and their caregivers. Training/supervision: Provided by program developers at tapering intensity. Evaluation: Caregivers completed the Caregiver Diary and satisfaction surveys. We explored training processes, intervention uptake, acceptability, adaptations to fit community context, appropriateness, perceived impact, and facilitators/barriers. Results: Six coaches were trained to fidelity, and three of these were further trained as Site Trainers. 183 clinically referred families enrolled and 89.4% completed the 12-week program. Caregivers reported increases in adherence and competence, high satisfaction and perceived benefits for their children. Coaches reported high satisfaction. Toddlers were appropriately identified to receive the intervention. Referral processes improved, including decreased referral age, and increased family readiness for diagnostic assessment and subsequent services. Conclusions: Social ABCs was successfully implemented in a community service through a research-community partnership. The program was feasible, acceptable, and appropriate within a community context. Drivers of success included funding, institutional support, shared decision-making, adaptations to fit context, leadership support, perceived positive impact, and commitment to evaluation.

Abstract:Background: Autistic children can benefit from physical activity (PA) in a variety of ways. However, autistic children tend not to meet PA recommendations and, consequently, may not experience the associated benefits. Parental PA support can facilitate PA participation among autistic children, but parents of autistic children may lack the skills to help their child engage in PA. Few studies, to date, have examined the outcomes of parent-mediated PA interventions for autistic children. The purpose of this study was to explore parents' perceived behavioral control (PBC) to support their autistic children in PA after their participation in a PA intervention. Methods: The theory of planned behavior served as the framework for this descriptive–qualitative investigation. Fifteen parents (each with 1 autistic child in the intervention) participated in semistructured interviews (3 wk after the intervention), which were transcribed and then analyzed using thematic line-by-line analysis. Results: Three themes characterized the changes to parents' PBC after completing the PA intervention. Those themes were: (1) I learned by son! (2) You are my coach! and (3) I can do it! Conclusions: The results showed that by participating in a parent-mediated PA intervention, parents experienced improved confidence and awareness of their child's abilities, thus enhancing their PBC. Future research is needed to examine how these improvements in PBC may influence the actual PA behaviors of autistic children.

Abstract:Paediatric Autism Communication Therapy is a parent-mediated, video-aided, communication-focussed intervention for young children with autism spectrum disorder. It has been shown in a UK randomised controlled trial to lead to improvements in parent–child communication and family quality of life, together with a sustained reduction in child autism symptom severity. This qualitative study examined parental perceptions of their participation in Paediatric Autism Communication Therapy within the context of the randomised controlled trial. Semi-structured interviews were carried out with 18 parents within 12 months of completion of the therapy. The thematic analysis provides insights into parents’ hopes, expectations, and learning processes when working with Paediatric Autism Communication Therapy therapists and carrying out daily practice in the home. Parents reported positive changes in their interaction and relationship with their child and improvements to their child’s communication and interaction. Some also highlighted poignant realisations and emotional challenges associated with taking part in this post-diagnostic therapy. Practical difficulties were also emphasised. Implications for the clinical practice of parent-mediated interventions with young children with autism spectrum disorder are discussed. Lay abstract Paediatric Autism Communication Therapy is an intervention for young children with autism spectrum disorder that focuses on parent–child communication. In Paediatric Autism Communication Therapy, the therapist and parent watch videos of the parent and child playing together. The therapist coaches the parent to carefully observe the child’s communication and to interact with their child in a more sensitive and responsive way. Parents are encouraged to use the strategies with their child at home. Paediatric Autism Communication Therapy has been shown to lead to long-term improvements in parent–child communication and family quality of life. This study aimed to explore parents’ perceptions of their participation in Paediatric Autism Communication Therapy. Interviews were carried out by an independent researcher with 18 parents. Parents discussed the learning processes they went through when working with Paediatric Autism Communication Therapy therapists and carrying out home practice. Some parents described initial doubts about the approach and hesitations about being videoed and analysing video material. In time, most parents came to really value the therapy and their relationship with the therapist. They reported positive changes in their interaction and relationship with their child and improvements to their child’s communication and interaction. Some also highlighted poignant realisations and emotional challenges associated with taking part in this post-diagnosis therapy. Practical difficulties were also emphasised, including the time commitment, accessibility of therapy venues and difficulties in occupying the child during therapist–parent discussion. Implications for the clinical practice of parent-mediated interventions are discussed.

Abstract:Background/Objectives: The prevalence of autism has increased substantially among Latine children; however, few service systems are prepared to provide culturally relevant services. Parents Taking Action (PTA) is a culturally informed parent-mediated psychoeducation program designed to meet the informational needs of Latine families with children with autism. The purpose of this study was to pilot a hybrid model of intervention by including direct parent coaching through pivotal response training (PRT) along with PTA among Latina mothers of children with autism. Methods: Ten Latina mothers of children eight years of age with autism participated in this study. The mothers received PTA from two bilingual trained promotoras (community health workers). PTA provided 14 2 h psychoeducation sessions. The mothers also received four one-hour sessions of PRT from bilingual/bicultural coaches. Results: Paired samples t-tests indicated significant increases in the families' outcomes, their self-efficacy in using the intervention strategies, and their frequency of using strategies, from pre- to post-test. Conclusions: This study suggests that culturally informed parent-mediated autism intervention coupled with parent coaching positively affects family outcomes among Latine families of children with autism.

Abstract:Topics in Early Childhood Special Education, Ahead of Print. We examined adult behavioral skills training (BST) for home-based Therapeutic Support Staff (TSS) using a naturalistic developmental behavioral intervention (NDBI) with young children with Autism Spectrum Disorder (ASD) derived from Project ImPACT. We taught three TSS to use interactive strategies with online modules, in-vivo training and ongoing feedback to a predetermined frequency criterion with young children with ASD in the home setting. The TSS increased strategy use to criterion and generalized the strategies to other settings. Child spontaneous communication increased in frequency and complexity from mostly eye gaze and gestures to eye gaze, vocalizations, and words. Our results extend what we know about training direct service providers and measuring social communication components.

Abstract:Introduction Although adolescents make treatment gains in psychiatric residential treatment (RT), they experience significant difficulty adapting to the community and often do not sustain treatment gains long term. Their parents are often not provided with the necessary support or behaviour management skillset to bridge the gap between RT and home. Parent training, a gold standard behaviour management strategy, may be beneficial for parents of these youth and web-based parent training programmes may engage this difficult-to-reach population. This study focuses on a hybrid parent training programme that combines Parenting Wisely (PW), a web-based parent training with facilitated discussion groups (Parenting Wisely for Residential Treatment (PW RT )). This study aims to: (1) establish the feasibility and acceptability of PW RT , (2) evaluate whether PW RT engages target mechanisms (parental self-efficacy, parenting behaviours, social support, family function) and (3) determine the effects of PW RT on adolescent outcomes (internalising and externalising behaviours, placement restrictiveness). Methods and analysis In this randomised control trial, parents (n=60) will be randomly assigned to PW RT or treatment as usual. Each week for 6 weeks, parents in the PW RT condition will complete two PW modules (20 min each) and attend one discussion group via Zoom (90 min). Adolescents (n=60) will not receive intervention; however, we will evaluate the feasibility of adolescent data collection for future studies. Data from parents and adolescents will be collected at baseline, post intervention (6 weeks post baseline) and 6 months post baseline to allow for a robust understanding of the longer-term effects of PW RT on treatment gain maintenance. Ethics and dissemination The study has been approved by The Ohio State University Institutional Review Board (protocol number 2022B0315). The outcomes of the study will be shared through presentations at both local and national conferences, publications in peer-reviewed journals and disseminated to the families and organisations that helped to facilitate the project. Abstract:Clinical Case Studies, Ahead of Print. Practical functional assessment and skill-based treatment (PFA/SBT) is increasingly recognized as a safe and efficient behavioral assessment and treatment approach for severe challenging behaviors commonly exhibited by children with autism spectrum disorder (ASD). Few studies, however, have evaluated the safety, efficiency and acceptability of implementing PFA/SBT procedures by coaching caregivers via telehealth, particularly for caregivers with no prior experience and training. In this pilot single-case investigation, the mother and father of a 6-year-old autistic girl who exhibited severe challenging behaviors were individually coached in weekly one-hour sessions via telehealth to implement PFA/SBT procedures with their child over 12 weeks in their home. The child's father was subsequently coached to implement a treatment extension with the child in weekly one-hour telehealth sessions conducted over an additional 12-week period. Neither parent had prior training or experience implementing behavioral interventions with their child. Results showed that both parents implemented the intervention with high fidelity and that dangerous forms of challenging behavior rarely occurred during the treatment period. Both parents rated the intervention as highly acceptable. These results demonstrate that parents with limited knowledge and experience of behavioral principles can be successfully coached to implement PFA/SBT procedures via telehealth safely, efficiently, and with high fidelity.

Abstract:This study identified patterns of therapist delivery of evidence-based intervention strategies with children with autism spectrum disorder within publicly funded mental health services and compared patterns for therapists delivering usual care to those trained in AIM HI (“An Individualized Mental Health Intervention for ASD”). Data were drawn from a randomized community effectiveness trial and included a subsample of 159 therapists (86% female) providing outpatient or school-based psychotherapy. Therapist strategies were measured via observational coding of psychotherapy session recordings. Exploratory factor analysis used to examine patterns of strategy delivery showed that among therapists in the usual care condition, strategies loaded onto the single factor, General Strategies, whereas for therapists in the AIM HI training condition, strategies grouped onto two factors, Autism Engagement Strategies and Active Teaching Strategies. Among usual care therapists, General Strategies were associated with an increase in child behavior problems, whereas for AIM HI therapists, Active Teaching Strategies were associated with reductions in child behavior problems over 18 months. Results support the effectiveness of training therapists in evidence-based interventions to increase the specificity of strategies delivered to children with autism spectrum disorder served in publicly funded mental health settings. Findings also support the use of active teaching strategies in reducing challenging behaviors. Findings support the use of active teaching strategies in reducing challenging behaviors in children with autism spectrum disorder and provide support for the effectiveness of training therapists in evidence-based interventions to promote the delivery of targeted, specific intervention strategies to children with autism spectrum disorder in mental health services.

Abstract:Purpose: This study evaluated the theory of planned behavior (TPB) as a framework for understanding exercise adherence during phase II cardiac rehabilitation (CR). Methods: A total of 215 patients completed a baseline questionnaire that included the TPB constructs and past exercise. Exercise adherence was measured via program attendance during phase II CR. Results: Hierarchic regression analyses indicated that attitude, subjective norm, and perceived behavioral control (PBC) explained 30% of the variance in exercise intention, with attitude, subjective norm, and PBC each making significant unique contributions to intention. Furthermore, exercise intention explained 12% of the variance in exercise adherence. Finally, the behavioral, normative, and control beliefs provided novel information concerning why patients in phase II CR hold certain attitudes, subjective norms, PBC, and exercise intentions. Conclusion: Results of the present study provide evidence that the TPB is a useful framework for understanding exercise intentions and adherence during phase II CR.

Abstract:Objective: To examine predictors of implementation and perceived usefulness of four empirically supported strategies for treating externalizing behavior in youths with ASD. Method: Participants were 557 providers in the United States with experience treating externalizing behavior in youths with ASD. Generalized estimating equations were used to determine whether self-reported use and usefulness of four empirically supported intervention strategies (functional communication training, functional behavior analysis, visual tools/supports, token economy) were predicted by key provider characteristics: professional discipline, experience, and practice specialization (across three indices) in ASD. Post-hoc contrasts were performed to identify provider groups reporting the greatest use and usefulness of the four strategies. Results: Strategies were most often used by providers with behavioral backgrounds, though perceived usefulness of strategies varied by providers' professional discipline. Compared to providers with more than 10 years of experience, less experienced providers endorsed the highest average use and usefulness of almost all strategies. Regarding ASD practice specialization, a lower volume of ASD cases, treating fewer youths with ASD over a 5-year period, and having a higher proportion of practice time working with youths with ASD reported were associated with greater use and usefulness of the strategies. Conclusions: Empirically supported strategies are widely used by and perceived as useful by providers who treat youths with ASD and co-occurring externalizing behaviors. Use and usefulness varies based on provider discipline, experience, and ASD practice specialization.

Abstract:Improving child behavior and promoting family well-being is a key objective of evidence-based parenting programs, such as the Triple P–Positive Parenting Program. To achieve this goal, parenting programs are delivered using a multidisciplinary workforce. Previous researchers have collectively examined the entire workforce of parenting practitioners to determine the factors that influence program delivery, primarily using self-report measures. However, these findings did not highlight the unique factors relevant to specific practitioner disciplines. Educators are one practitioner discipline that play an integral role in delivering parenting programs through schools and early childhood learning settings. This study aimed at exploring the facilitators and barriers that impact frequency of program use for educator practitioners using both qualitative and quantitative analyses. Data from 404 Triple P educator practitioners were extracted from a larger dataset of 1202 practitioners from English-speaking countries who completed self-report questionnaires and responded to three open-ended questions. Hierarchical multiple regressions were conducted using eight independent variables (with participant characteristics as control variables), revealing seven positive and one negative predictor for frequency of use. A thematic analysis was then conducted on the qualitative responses, producing 11 themes and 28 subthemes. The quantitative analysis revealed organisational support, perceived usefulness, and practitioner self-regulation were the most important positive predictors. The qualitative analysis supported these findings and revealed novel barriers including Covid-19/work from home, online delivery, parent factors, and specific organisational factors. These findings highlight the need for online resources, reliable virtual delivery methods, improved ways to reach and engage families, and additional trained education practitioners to distribute high workloads.

Abstract:Background/Objective: Autism spectrum disorder (ASD) is a neurodevelopmental condition characterized by lifelong impacts on functional social and daily living skills, and restricted, repetitive behaviors (RRBs). Applied behavior analysis (ABA), the gold-standard treatment for ASD, has been extensively validated. ABA access is hindered by limited availability of qualified professionals and logistical and financial barriers. Scientifically validated, parent-led ABA can fill the accessibility gap by overcoming treatment barriers. This retrospective cohort study examines how our ABA treatment model, utilizing parent behavior technicians (pBTs) to deliver ABA, impacts adaptive behaviors and interfering behaviors (IBs) in a cohort of children on the autism spectrum with varying ASD severity levels, and with or without clinically significant IBs. Methods: Clinical outcomes of 36 patients ages 3–15 years were assessed using longitudinal changes in Vineland-3 after 3+ months of pBT-delivered ABA treatment. Results: Within the pBT model, our patients demonstrated clinically significant improvements in Vineland-3 Composite, domain, and subdomain scores, and utilization was higher in severe ASD. pBTs utilized more prescribed ABA when children initiated treatment with clinically significant IBs, and these children also showed greater gains in their Composite scores. Study limitations include sample size, inter-rater reliability, potential assessment metric bias and schedule variability, and confounding intrinsic or extrinsic factors. Conclusion: Overall, our pBT model facilitated high treatment utilization and showed robust effectiveness, achieving improved adaptive behaviors and reduced IBs when compared to conventional ABA delivery. The pBT model is a strong contender to fill the widening treatment accessibility gap and represents a powerful tool for addressing systemic problems in ABA treatment delivery.
