Abstract:Objective: To evaluate the effect of an intelligent Internet-based information system upon optimising the management of patients diagnosed with type 2 diabetes mellitus (T2DM). Methods: In 2015, a T2DM information system was introduced to optimise the management of T2DM patients for 1 year in Fangzhuang community of Beijing, China. A total of 602 T2DM patients who were registered in the health service centre of Fangzhuang community were enrolled based on an isometric sampling technique. The data from 587 patients were used in the final analysis. The intervention effect was subsequently assessed by statistically comparing multiple parameters, such as the prevalence of glycaemic control, standard health management and annual outpatient consultation visits per person, before and after the implementation of the T2DM information system. Results: In 2015, a total of 1668 T2DM patients were newly registered in Fangzhuang community. The glycaemic control rate was calculated as 37.65% in 2014 and significantly elevated up to 62.35% in 2015 ( p < 0.001). After application of the Internet-based information system, the rate of standard health management was increased from 48.04% to 85.01% ( p < 0.001). Among all registered T2DM patients, the annual outpatient consultation visits per person in Fangzhuang community was 24.88% in 2014, considerably decreased to 22.84% in 2015 ( p < 0.001) and declined from 14.59% to 13.66% in general hospitals ( p < 0.05). Conclusion: Application of the T2DM information system optimised the management of T2DM patients in Fangzhuang community and decreased the outpatient numbers in both community and general hospitals, which played a positive role in assisting T2DM patients and their healthcare providers to better manage this chronic illness.

Effect Of Standardized Management In Patients With Type 2 Diabetes Mellitus In Community

Abstract:Objective To evaluate the effects of the standardized management on blood glucose control in patients with type 2 diabetes mellitus (T2DM) in the community. Method Zheda-yubi community, which carries out the standardized management in patients with T2DM, was chosen as the study group. A community with similar demographic characteristics was chosen as the matched group. The general information and follow-up information of diabetic patients in two groups before and after standardized management were investigated and analyzed. Results (1) With continue of management time, the management patients increased and the follow-up rate dropped in both groups. The follow-up rate in study group was still higher than 90%, while that of the control group declined from 78.1% to 44.4% (with a significant difference of chi(2) = 22.224, P < 0.01). (2) The control ratio of fasting blood glucose (FBG) in the study group increased from 50.0% to 63.5%, while in the control group it decreased from 31.7% to 27.6% (with a significant difference of chi(2) = 22.564, P < 0.01). Conclusion Standardized management of T2DM in the community plays an important role in maintaining the continued follow-up and controlling blood glucose in patients with T2DM.
Web-Based TangPlan and WeChat Combination to Support Self-management for Patients with Type 2 Diabetes: Randomized Controlled Trial.

Abstract:Background: China has the largest number of patients with type 2 diabetes mellitus (T2DM) in the world. However, owing to insufficient knowledge of self-management in patients with diabetes, blood glucose (BG) control is poor. Most diabetes-related self-management applications fail to bring significant benefits to patients with T2DM because of the low use rate and difficult operation. Objective: This study aims to examine the effectiveness of the combination of the self-designed web-based T2DM management software TangPlan and WeChat on fasting BG (FBG), glycated hemoglobin (HbA(1c)), body weight, blood pressure (BP), and lipid profiles in patients with T2DM over a 6-month period. Methods: Participants were recruited and randomized into the TangPlan and WeChat or control groups. Participants in the control group received usual care, whereas the TangPlan and WeChat participants received self-management guidance with the help of TangPlan and WeChat from health care professionals, including BG self-monitoring; healthy eating; active physical exercise; increasing medication compliance; and health education during follow-ups, lectures, or web-based communication. They were also asked to record and send self-management data to the health care professionals via WeChat to obtain timely and effective guidance on diabetes self-management. Results: In this study, 76.9% (120/156) of participants completed the 6-month follow-up visit. After the intervention, FBG (mean 6.51, SD 1.66 mmol/L; P=.048), HbA(1c) (mean 6.87%, SD 1.11%; P<.001), body weight (mean 66.50, SD 9.51 kg; P=.006), systolic BP (mean 127.03, SD 8.00 mm Hg; P=.005), diastolic BP (mean 75.25, SD 5.88 mm Hg; P=.03), serum low-density lipoprotein cholesterol (mean 2.50, SD 0.61 mmol/L; P=.006), and total cholesterol (mean 4.01, SD 0.83 mmol/L; P=.02) in the TangPlan and WeChat group were all significantly lower, whereas serum high-density lipoprotein cholesterol (mean 1.20, SD 0.25 mmol/L; P=.01) was remarkably higher than in those in the control group. Compared with the baseline data, significance was found in the mean change in FBG (95% CI -0.83 to -0.20; P=.002), HbA(1c) (95% CI -1.92 to -1.28; P<.001), body weight (95% CI -3.13 to -1.68; P<.001), BMI (95% CI -1.10 to -0.60; P<.001), systolic BP (95% CI -7.37 to -3.94; P<.001), diastolic BP (95% CI -4.52 to -2.33; P<.001), triglycerides (95% CI -0.16 to -0.03; P=.004), serum low-density lipoprotein cholesterol (95% CI -0.54 to -0.30; P<.001), and total cholesterol (95% CI -0.60 to -0.34; P<.001) in the TangPlan and WeChat group but not in the control group (P=.08-.88). Conclusions: Compared with usual care for patients with T2DM, the combination of TangPlan and WeChat was effective in improving glycemic control (decrease in HbA(1c) and BG levels) and serum lipid profiles as well as reducing body weight in patients with T2DM after 6 months.

Abstract:Objective To explore the application of the remote blood glucose control system for type 2 diabetes mellitus (T2DM) management in community. Methods A total of 363 newly diagnosed but poorly controlled T2DM patients from Shichahai Community Health Service Center were selected in Xicheng District of Beijing from June 2012 to May 2013. They were randomly divided into the control group (n=184) and the experimental group (n=179) by random number table. Patients in the control group received conventional blood glucose management while the community health service center carried out remote blood glucose control on patients in the experimental group. Patients in both groups were followed up for one year. Results of laboratory examinations before the intervention and 3, 6 and 12 months after the intervention were compared between two groups, which included fasting blood glucose (FPG), glycosylated hemoglobin (HbA1c), total cholesterol (TC), triglyceride (TG), high density lipoprotein cholesterol (HDL-C), low density lipoprotein cholesterol (LDL-C), creatinine (Cr), systolic blood pressure (SBP), diastolic blood pressure (DBP) and a seven-day diet control. Results Test results of FPG, TC, HDL-C, LDL-C, Cr and the seven-day diet control differed significantly in two groups (P<0.05). These results differed significantly between two groups at different time points (P<0.05). Intervention methods and different time points had an interaction on the results of FPG, TC, HDL-C, LDL-C, Cr and the seven-day diet control (P<0.05). There were no significant differences in HbA1c, TG and SBP between two groups (P>0.05), but they showed significant differences in the same group at different time points (P<0.05). Intervention methods and different time points had an interaction on the results of HbA1c, TG, and SBP (P<0.05). The DBP results showed no significant differences between two groups (P>0.05) or at different time points (P>0.05). There was no interaction on DBP between intervention methods and different time points (P>0.05). Conclusion Remote blood glucose control system has positive effects on type 2 diabetes mellitus management in community and improves patients'metabolism.

Abstract:Objective . The Chronic Care Model, based on core elements of team-centered care in chronic diseases, has widely been accepted. This study was aimed at evaluating the effectiveness of the Chronic Care Model in type 2 diabetes management. Methods . A group randomized experimental study was conducted. Twelve communities of the Zhaohui Community Health Service Center in Hangzhou, China, were randomly assigned into an intervention group ( n = 6 ) receiving the Chronic Care Model-based intervention and a control group ( n = 6 ) receiving conventional care. A total of three hundred patients, twenty-five for each community, aged ≥18 years with type 2 diabetes for at least 1-year duration, were recruited. Data of health behaviors, clinical outcomes, and health-related quality of life (Short-Form 36-item questionnaire) were collected before and after a 9-month intervention and analyzed using descriptive statistics, t -test, chi-square test, binary logistic regression, and linear mixed regression. A total of 258 patients (134 in intervention and 124 in control) who completed the baseline and follow-up evaluations and the entire intervention were included in the final analyses. Results . Health behaviors such as drinking habit ( OR = 0.07 , 95% CI: 0.01, 0.75), physical activity ( OR = 2.92 , 95% CI: 1.18, 7.25), and diet habit ( OR = 4.30 , 95% CI: 1.49, 12.43) were improved. The intervention group had a remarkable reduction in glycated hemoglobin (from 7.17% to 6.60%, P < 0.001 ). The quality of life score changes of the role limitation due to physical problems ( m e a n = 9.97 , 95% CI: 3.33, 16.60), social functioning ( m e a n = 6.50 , 95% CI: 2.37, 10.64), role limitation due to emotional problems ( m e a n = 8.06 , 95% CI: 2.15, 13.96), and physical component summary score ( m e a n = 3.31 , 95% CI: 1.22, 5.39) were improved in the intervention group compared to the control group. Conclusion . The Chronic Care Model-based intervention helped improve some health behaviors, clinical outcomes, and quality of life of type 2 diabetes patients in China in a short term.

Abstract:Background: Levels of self-management behaviors (SMB) and quality of life (QoL) are suboptimal in Chinese adults with type 2 diabetes (T2D), especially in rural China. Integrated health management within a county medical consortium, featuring multi-level teams of doctors, nurses, and other professionals offering follow-up services such as check-ups, assessments, treatment, and health education, is promising in improving this. This study aimed to assess the effect of integrated health management within a county medical consortium on the SMB and QoL of rural T2D patients in China. Methods: Based on a survey conducted on the county medical consortium in Eastern China, this study initially employed propensity score matching (PSM), a nonparametric technique, to precisely estimate the average treatment effect on the treated (ATT) of integrated health management on SMB and QoL in rural T2D patients. Subsequently, quantile regression was also performed to estimate the relationship between the implementation of integrated health management, sociodemographic factors, follow-up services (offered during integrated health management) and both SMB and QoL. Results: The ATT values for SMB and QoL, representing the net effect of integrated health management within a county medical consortium on SMB and QoL, were significantly positive. They ranged from 4.34 to 4.67 for SMB and from 0.89 to 1.06 for QoL, respectively, based on the four different PSM modalities. The results of quantile regression also revealed a statistically significant positive association between the implementation of integrated health management and both SMB (coef. = 4.15) and QoL (coef. = 1.54). These findings suggest that integrated health management within a county medical consortium can effectively improve SMB and QoL in rural T2D patients. Furthermore, frequency of follow-up service and health behavior guidance were positively associated with SMB and QoL. Conversely, on-call follow-up services, medication guidance and follow-up services at medical institutions were negatively correlated with SMB or QoL. Conclusions: The study highlights the effectiveness of integrated health management within a county medical consortium in improving SMB and QoL among individuals with T2D in rural China. The findings offer invaluable insights for the advancement of chronic disease management in rural areas of developing countries.
Improving the management of type 2 diabetes in China using a multifaceted digital health intervention in primary health care: the SMARTDiabetes cluster randomised controlled trial

Abstract:Background: There is limited evidence, mainly from high-income countries, that digital health interventions improve type 2 diabetes (T2DM) care. Large-scale implementation studies are lacking. Methods: A multifaceted digital health intervention comprising: (1) a self-management application ('app') for patients and lay 'family health promotors' (FHPs); and (2) clinical decision support for primary care doctors was evaluated in an open-label, parallel, cluster randomized controlled trial in 80 communities (serviced by a primary care facility for >1000 residents) in Hebei Province, China. People >40 years with T2DM and a glycated haemoglobin (HbA1c) ≥7% were recruited (∼25/community). After baseline assessment, community clusters were randomly assigned to intervention or control groups (1:1) via a web-based system, stratified by locality (rural/urban). Control arm clusters received usual care without access to the digital health application or family health promoters. The primary outcome was at the participant level defined as the proportion with ≥2 "ABC" risk factor targets achieved (HbA1c < 7.0%, blood pressure < 140/80 mmHg and LDL-cholesterol < 2.6 mmol/L) at 24 months. Findings: A total of 2072 people were recruited from the 80 community clusters (40 urban and 40 rural), with 1872 (90.3%) assessed at 24 months. In the intervention arm, patients used FHPs for support more in rural than urban communities (252 (48.6%) rural vs 92 (21.5%) urban, p < 0.0001). The mean monthly proportion of active app users was 46.4% (SD 7.8%) with no significant difference between urban and rural usage rates. The intervention was associated with improved ABC control rates (339 [35.9%] intervention vs 276 [29.9%] usual care; RR 1.20, 95% CI 1.02-1.40; p = 0.025), with significant heterogeneity by geography (rural 220 [42.6%] vs 158 [31.0%]; urban 119 [27.9%] vs 118 [28.6%]; p = 0.022 for interaction). Risk factor reductions were mainly driven by improved glycaemic control (mean HbA1C difference -0.33%, 95% CI -0.48 to -0.17; p = 0.00025 and mean fasting plasma glucose difference -0.58 mmol, 95% CI -0.89 to -0.27; p = 0.00013). There were no changes in blood pressure and LDL-cholesterol levels. Interpretation: A multifaceted digital health intervention improved T2DM risk factor control rates, particularly in rural communities where there may be stronger relationships between patients and doctors and greater family member support. Effects of Hospital-Community Integrated Management Based on the Joint Diagnosis and Treatment Center for Chronic Diseases in Patients with Type 2 Diabetes Mellitus

Abstract:Objective To study the clinical effects of hospital-community integrated management based on the joint center of diagnosis and treatment for chronic disease in patients with type 2 diabetes mellitus. Methods A total of 122 type 2 diabetes mellitus patients were randomly divided into the control group (n=62) and treatment group (n=60). Patients in the treatment group were managed by the joint diagnosis and treatment center for chronic disease after they were established management files, while patients in the control group were managed according to the previous diabetes management mode. The levels of glycated hemoglobin (HbA1c), fasting blood glucose, 2 h postprandial blood glucose, blood pressure, blood lipid, urine microalbumin/createnine and body mass index (BMI) were detected. And the standard reaching rate of above indicators were compared between before and 24 months after the management. Results The levels of HbA1c [(6.6 ± 3.0)% vs. (7.7 ± 1.4)%, t=2.792, P=0.031], fasting blood glucose [(6.7 ± 1.2) mmol/L vs. (8.4 ± 2.4) mmol/L, t=5.134, P=0.005], 2 h postprandial blood glucose [(9.5 ± 2.3) mmol/L vs. (11.0 ± 2.8) mmol/L, t=3.286, P=0.008] and total cholesterol [(5.1 ± 1.4) mmol / L vs. (5.9 ± 1.9) mmol / L, t = 2.527, P = 0.012] in the treatment group before the management were much lower than those after the management, and the standard reaching rate of HbA1c (47/60 vs. 33/60, χ2=7.350, P=0.007), fasting blood glucose (49/60 vs. 39/60, χ2=4.261, P=0.039), 2 h postprandial blood glucose (49/60 vs. 29/60, χ2 = 14.650, P<0.001), blood pressure (45/60 vs. 33/60, χ2=5.167, P=0.038) and triglyceride (33/60 vs. 21/60, χ2=4.089, P=0.044) in the treatment group before the management were much higher than those after the management. However, the above indicators in the control group all showed no significant differences between before and 24 months after the management (all P<0.05). Conclusion The therapeutic effect of hospital-community integrated management based on the joint diagnosis and treatment center for chronic disease in patients with type 2 diabetes mellitus is remarkable.
Effectiveness of the Lilly Connected Care Program in Improving Glycemic Management Among Patients With Type 2 Diabetes in China: Retrospective Real-world Study

Abstract:Background: Type 2 diabetes mellitus (T2DM) is a worldwide public health concern. Mobile health management platforms could be a potential way to achieve effective glycemic control. Objective: This study aimed to evaluate the real-world effectiveness of the Lilly Connected Care Program (LCCP) platform in glycemic control among patients with T2DM in China. Methods: This retrospective study included Chinese patients with T2DM (aged ≥18 years) from April 1, 2017, to January 31, 2020, for the LCCP group and from January 1, 2015, to January 31, 2020, for the non-LCCP group. Propensity score matching was used to match the LCCP and non-LCCP groups to reduce confounding, with covariates including age, sex, the duration of diabetes, baseline hemoglobin A 1c (HbA 1c ), and the number of oral antidiabetic medication classes. HbA 1c reduction over 4 months, the proportions of patients achieving an HbA 1c reduction of ≥0.5% or ≥1%, and the proportions of patients reaching to target HbA 1c level of ≤6.5% or <7% were compared between the LCCP and non-LCCP groups. Multivariate linear regression was used to assess factors associated with HbA 1c reduction. Results: A total of 923 patients were included, among whom 303 pairs of patients were well matched after propensity score matching. HbA 1c reduction during the 4-month follow-up was significantly larger in the LCCP group than the non-LCCP group (mean 2.21%, SD 2.37% vs mean 1.65%, SD 2.29%; P =.003). The LCCP group had a higher proportion of patients with an HbA 1c reduction of ≥1% (209/303, 69% vs 174/303, 57.4%; P =.003) and ≥0.5% (229/303, 75.6% vs 206/303, 68%; P =.04). The proportions of patients reaching the target HbA 1c level of ≤6.5% were significantly different between the LCCP and non-LCCP groups (88/303, 29% vs 61/303, 20.1%; P =.01), whereas the difference in the proportions of patients reaching the target HbA 1c level of <7% was not statistically significant (LCCP vs non-LCCP: 128/303, 42.2% vs 109/303, 36%; P =.11). LCCP participation and higher baseline HbA 1c were associated with a larger HbA 1c reduction, whereas older age, longer diabetes duration, and higher baseline dose of premixed insulin analogue were associated with a smaller HbA 1c reduction. Conclusions: The LCCP mobile platform was effective in glycemic control among patients with T2DM in China in the real world.

Abstract:BACKGROUND Type 2 diabetes (T2D) is a widespread chronic disease with high rates of morbidity and mortality worldwide. Managing risk factors can effectively prevent acute and chronic complications, improve quality of life, and reduce mortality. Therefore, implementation of a diabetes health management strategy is urgently needed. Emerging medical technologies have strengthened communication between patients and clinicians. The establishment and improvement of the graded diagnosis and treatment system has promoted the prevention, treatment, and management of diabetes. METHODS A total of 300 patients diagnosed with T2D in the Health Management Center at the Affiliated Hospital of Nantong University were randomly divided into two groups: an internet plus graded diagnosis and treatment strategy group, and a control group. After 6 months, the physiological parameters, management indices, and complications were compared between the groups. RESULTS Physiological indicators, such as body mass index (BMI), waist circumference, triglycerides, low-density lipoprotein, systolic and diastolic blood pressure, and blood glucose were significantly alleviated in the Internet plus graded diagnosis and treatment strategy group. Management indicators (such as blood glucose monitoring compliance rate, diet control compliance rate, and exercise compliance rate) also improved substantially. The incidence of hypoglycemia was notably increased compared to the control group. CONCLUSIONS The new health management strategy for diabetes can improve lifestyle, ameliorate physiological indicators, reduce the complication rate, and form a virtuous cycle. This provides a positive impact on the entire life-cycle health management of diabetes, and is worthy of further promotion.

Abstract:Purpose: The aim of the study is to examine the impact of an Internet supported diabetes management program and to analyze the influencing factors of the improvement. Methods: The Internet Diabetes Shared Care Program (IDSC) in Peking University Shougang Hospital was an application of DSMES (Diabetes Self-Management Education and Support) with Internet technologies. Interdisciplinary care team including diabetologist, nurses, dietitians and diabetes educators provide individualized consultation in hospital every 3 months, and continuous online lifestyle coaching via smartphone applications. Change of (HbA1c), BP, LDL-c, BMI at baseline and at last visit were compared. Multiple linear regression analysis were applied to analyze the data. Results: Total of 349 patients (female 51%) were enrolled from 2018 to 2022 with average age 56.98±10.61 years, duration of DM 9.59±8.23 years. Patients were followed in the IDSC program for 1.78±0.90 years. At baseline, 41.8% used insulin. After the program. the HbA1c decreased 1.29±2.22%(8.41±2.23% vs 7.12±1.30%, t=10.32, P=0.000), BMI decreased 0.43±2.50 kg/m2(25.98±3.99kg/m2 vs 25.56±3.83 kg/m2, t=3.10, P=0.002). Multiple linear regression found and patient age (β=-0.036, P=0.001), followed duration (β=-0.349, P=0.004), baseline HbA1c achieved ≤7% (β=-2.477, P=0.000) and weekly communication with diabetes educators (β=0.828, P=0.013)were the impact factors for glycemic improvement. Conclusion: The Internet supported diabetes management program significantly improved the glycemic control of DM patients, in which age, weekly communication and continuous care are essential factors for glycemic improvements. The short- and long-term effects of community-family-doctor-based type 2 diabetes self-management interventions

Abstract:Objectives The popularity of contracted family doctor services in China has been growing in recent years, but community-family-doctor-based type 2 diabetes mellitus (T2DM) intervention programs have yet to be adequately studied. This study was to evaluate the short- and long-term effects of community-family-doctor-based self-management interventions for T2DM and to explore strategies for long-term glycemic control. Study design This was a randomized controlled trial. Methods A total of 144 eligible participants were randomly assigned to intervention and control groups. The control group received only routine community diabetes care, and the intervention group received community-family-doctor-based interventions involving the same standard of care. The interventions lasted for 3 months, and the follow-up was continued for 15 months. Intention-to-treat analysis and generalized estimation equations were then used to determine the short- and long-term effects of the interventions on glycated hemoglobin (HbA 1c ), diabetes self-management, and medication adherence. Results There were statistically significantly greater improvements in all aspects of the intervention group after 3 months of intervention. Compared with baseline, changes in the attitude ( β = 0.384, 95% confidence interval [CI; 0.194, 0.574], P < 0.001), practice ( β = 1.751, 95% CI [0.762, 2.739], P = 0.001), and knowledge, attitudes, practice total scores ( β = 2.338, 95% CI [0.682, 3.995], P = 0.006) of patients in the intervention group were statistically significant after 15 months, and the HbA 1c (8.19 ± 1.73%), knowledge (16.42 ± 3.21), and medication adherence (5.53 ± 1.76) scores were slightly better than those at baseline, although not statistically significant ( P > 0.05). Conclusions T2DM self-management interventions based on community family doctors improved patients' HbA 1c , diabetes self-management, and medication adherence, did not do so significantly in the long term.

Management Status of Type 2 Diabetes Mellitus in Tertiary Hospitals in Beijing: Gap Between Guideline and Reality.

Abstract:BACKGROUND Diabetes has become one of the most common chronic diseases and the third leading cause of death in China. Many programs have been initiated at national and local levels to address the illness. However, the effect of these programs in daily outpatient clinics is still unclear. The objective of this study was to investigate the management status of type 2 diabetes mellitus (T2DM) and factors associated with it in diabetes clinics of tertiary hospitals in Beijing. METHODS A cross-sectional survey was conducted in six tertiary hospitals in Beijing. Control criteria were defined based on 2007 China guideline for type 2 diabetes (CGT2D). RESULTS A sample of 1151 patients, age (60.8 ± 9.2) years, and with a median disease duration of 7.3 years was included. The hemoglobin A1c (HbA1c) mean level was (7.15 ± 1.50)%, the percentage of patients achieving the targets for HbA1c was 37.8%, blood pressure 65.6%, triglyceride (TG) 48.8%, high-density lipoprotein (HDL) 59.2%, low-density lipoprotein (LDL) 34.0%, and total cholesterol (TC) 42.0%. The factors independently associated with glycemic control were diabetes duration (odds ratio (OR) = 0.95; 95% confidence interval (CI): 0.919 - 0.982, P < 0.01), body mass index (BMI) (OR = 0.914, 95%CI: 0.854 - 0.979, P = 0.01) and smoking (OR = 0.391, 95%CI: 0.197 - 0.778, P < 0.01). The factors independently associated with blood pressure control were BMI (OR = 0.915, 95%CI: 0.872 - 0.960, P < 0.01) and male gender (OR = 0.624, 95%CI: 0.457 - 0.852, P < 0.01). The factor independently associated with LDL control was education level (OR = 1.429, 95%CI: 1.078 - 1.896, P = 0.013). CONCLUSIONS The management status of T2DM patients in tertiary hospitals in Beijing has improved remarkably. However, there is still room for further improvement to reach the guideline target. Long diabetes duration, high BMI, smoking, male gender and low education level were independently associated with poor metabolic control.

Utilization of National Essential Public Health Services and Its Relationship with Management Effect in Chinese Type 2 Diabetic Patients

Abstract:BackgroundThe national essential public health services (NEPHS) , which have been implemented since 2009, may be the largest population-based intervention practice for Chinese patients with diabetes currently. It is important to understand the utilization and management effect of such services in diabetic population over this period of more than 10 years of development.ObjectiveTo understand the utilization and management effect of NEPHS as well as their association in Chinese type 2 diabetics.MethodsBy use of multi-stage stratified sampling, 1 527 type 2 diabetics (≥35 years old) were selected from 20 community (township) health centers in 10 districts (counties) of 5 cities in eastern, central and western China during November to December 2019. Sociodemographic characteristics, utilization and management effect of NEPHS in these patients were collected by face-to-face surveys with a self-designed questionnaire.ResultsAccording to the survey, patients' self-reported rates of creating health records, use of health records, undergoing standardized blood glucose tests, and receiving standard follow-ups were 90.34% (1 375/1 522) , 52.80% (725/1 373) , 83.69% (1 262/1 508) , and 90.18% (1 377/1 527) , respectively. Household follow-ups and hospital follow-ups accounted for 29.24% (443/1 515) and 61.06% (925/1 515) of the total last follow-ups, respectively. The analysis of management effect showed that patients' self-reported rates of home-based self-monitoring blood glucose and regular medication in the past 6 months were 53.57% (818/1 527) , and 89.26% (1 363/1 527) , respectively. The rates of patients who were satisfied with glycemic control, and overall medical services assessed in the last follow-up were 65.23% (996/1 527) , and 95.15% (1 453/1 527) , respectively. In those≥65 years old, the prevalence of home-based self-monitoring blood glucose differed significantly by the creation of health records and Chinese medicine services (P<0.05) . The prevalence of regular medication differed significantly by number of follow-ups (P<0.05) . The level of overall satisfaction with services differed significantly by the access to personal medical records at any time, Chinese medicine services, number of home-based self-monitoring blood glucose, and type of follow-up (P<0.05) . In those aged from 35 to 64, the prevalence of home-based self-monitoring blood glucose differed significantly by the creation of health records, access to personal medical records at any time, Chinese medicine services, number of blood glucose testing, and number of follow-ups (P<0.05) . The prevalence of regular medication differed significantly by the type of follow-up (P<0.05) . The satisfaction rate of blood glucose control differed significantly by access to personal medical records at any time (P<0.05) . The overall service satisfaction rate differed significantly by follow-up type and creation of health records (P<0.05) .ConclusionNEPHS have influenced community-based management of type 2 diabetic patients, the standard implementation of which has enhanced the management effect and the overall service satisfaction in these patients.

Abstract:Background Xiamen is a pilot city in China for hierarchical diagnosis and treatment reform of non-communicable diseases, especially diabetes. Since 2012, Xiamen has implemented a program called the “three-in-one”, a team-based care model for the treatment of diabetes, which involves collaboration between diabetes specialists, general practitioners, and health managers. In addition, the program provides financial incentives to improve care, as greater accessibility to medications through community health care centers (CHCs). The aim of this study was to evaluate the effectiveness of these policies in shifting visits from general hospitals to CHCs for the treatment of type 2 diabetes mellitus (T2DM). Method and materials A retrospective observational cohort study was conducted using Xiamen’s regional electronic health record (EHR) database, which included 90% of all patients registered since 2012. Logistic regression was used to derive the adjusted odds ratio (OR) for patients shifting from general hospitals to CHCs. Among patients treated at hospitals, Kaplan-Meier(KM) curves were constructed to evaluate the time from each policy introduction until the switch to CHCs. A k-means clustering analysis was conducted to identify patterns of patient care-seeking behavior. Results In total, 89,558 patients and 2,373,524 visits were included. In contrast to increased outpatient visits to general hospitals in China overall, the percentage of visits to CHCs in Xiamen increased from 29.7% in 2012 to 66.5% in 2016. The most significant and rapid shift occurred in later periods after full policy implementation. Three clusters of patients were identified with different levels of complications and health care-seeking frequency. All had similar responses to the policies. Conclusions The “three-in-one” team-based care model showed promising results for building a hierarchical health care system in China. These policy reforms effectively increased CHCs utilization among diabetic patients.

Abstract:Objective: To assess the effect of use frequency of SMBG real-time decision support in a diabetes app on glycemic control in adult with T1D. Methods: T1D users from TangTangQuan® App meeting the following criteria will be selected: physician diagnosed T1D with aged≥18 years old, T1D duration ≥1 year. Frequency of uploading SMBG was be calculated in 2018. Patients were divided into two groups according to frequency of uploading SMBG (high engagement group (HE, ≥15 days/month; and control group<15 days/month). The changes in HbA1c, incidence of hyper- and hypo-glycemia from baseline to 12 months after using app were compared. Results: Of 3474 users over 1 year, 562 eligible patients had complete set of HbA1c records. At baseline, the mean age was 26.6±7.3 years, 33.5% were male, the mean HbA1c was 6.5±0.9%. Change in HbA1c from baseline improved in the HE group than that in control [-0.3 (-0.6, 0.2) vs. -0.1 (-0.8, 0.5) %, p=0.023]. Furthermore, Incidence of hyperglycemia at 12 months was significantly decrease in HE group than control, while no difference in Hypoglycemia between two groups (Table 1). Engagement intensity was significantly and independently associated with changes in HbA1c after adjustment by multiple regression analysis [-0.4 (-0.7, -0.0), p=0.036]. Effectiveness evaluation of the mobile health patients management mode on treatment compliance and glycemic control for type 2 diabetes patients using basal insulin treatment for 12 weeks

Abstract:Objective To assess the effectiveness of a novel mobile health patient management system involving doctors, nurses, and patients (" TRIO" mode) on glycemic control and self-monitoring of blood glucose (SMBG) compliance among the type 2 diabetes mellitus ( T2DM) patients using basal insulin standardization treatment in China. Methods From April 2014 to April 2015, 416 hospitals in 110 cities of 30 provinces, municipalities, and autonomous regions across China were selected to participate in the program. A Online-to-Offline ( O2O) integrated mobile health patients management system with mobile terminals for the doctors, nurses, and patients was applied in the program for patient education, follow-up, and data collection. For all the newly recruited patients, the baseline information was collected and a first-day patient education program were provided by a designated nurse. In the 12-week follow-up period, data of basal insulin doses and fasting plasma glucose ( FPG) values were collected from the patients by text messages or tracking phone call by the nurse. The patients also received timely standardized patients health education and glycemic control guidance by participating in the " patient education forum" and " doctors' hotline" in order to help them achieve the glycemic control goals. Results A total of 102 524 patients using basal insulin treatment were eligible and enrolled in the program. 64 420 patients completed the 12 weeks follow-up and provided at least one FPG value at all five follow-ups. In total, 62. 6% (40 334 / 64 420) of the patients reached the FPG control target(FPG≤7. 0 mmol/ L) at the end of follow-up period. The weekly average FPG for patients with complete SMBG data decreased from 10. 58 mmol/ L to 6. 91 mmol/ L while the FPG control rates increased from 13. 4% to 69. 2% . The weekly average FPG for the patients provided incomplete SMBG data decreased from 10. 54 mmol/ L to 7. 13 mmol/ L while the FPG control rates increased from 13. 6% to 62. 2% . The FPG control rates for the patients provided complete SMBG were 1. 74 times higher than those patients provided incomplete SMBG. Based on a GEE model, the average decline of the FPG and the increase of the FPG control rates were significantly better for patients who provided complete SMBG as compared to the patients with incomplete SMBG data. The results of the multivariate logistic regression analysis showed that factors such as receiving the first-day education, participating in the follow-up patient education forum, and the doctors' hotline were significantly associated with the improvement of the SMBG compliance, the treatment adherence, and the FPG control rates. The SMBG compliance and the treatment adherence for patients who completed first-day education were 1. 68 times and 1. 22 times higher, respectively. For the patients who participated in follow-up education activities, their SMBG compliance and treatment adherence were 3. 17 times and 3. 36 times higher, respectively. Conclusion The innovative " TRIO"mobile health patient management mode was feasible and effective for better managing the type 2 diabetes patients initiated on basal insulin treatment in China. Active participation in the first-day education program and the follow-up patient education activities can effectively improve the SMBG compliance and the treatment adherence, and therefore play an important role in helping patient achieving FPG control in a faster manner.
Evaluation of the effectiveness of diabetes self-management program based on Chinese local patients in Nanjing community

Abstract:Objective To evaluate the diabetes self-management program based on Chinese local patients in Nanjing community. Methods From April 2014 to June 2014, diabetes patients were recruited through health records system screening in the community health service centers, letter invitation, poster announcements at communities, and telephone notification. A total of 53 self-management groups were established. Nanjing diabetes self-management program included six 1-1.5 hours sessions scheduled on consecutive weeks, based on the blueprint of Shanghai Chronic Disease Self-Management Program (CDSMP) developed at Stanford University. Baseline and three-month later interviews were conducted respectively. Results A total of 636 patients were recruited and agreed to enter CDSMP; 603 completed the 6-session activities, with the response rate being 94.8%. Compared to baseline, nine of the patients' the awareness rate of diabetes-related knowledge, six of self-management behaviors, the scores of quality of life in physical component summary [(47.51 ± 9.47) vs. (49.10 ± 8.27) points, t=6.170, P=0.000] and mental component summary [(47.09±11.95) vs. (49.13±10.74) points, t=5.157, P=0.000] were all higher after three months (all P values<0.05). Three months after implementation, the level of systolic blood pressure, diastolic blood pressure, fasting plasma glucose and total cholesterol decreased respectively by (1.42±0.52) mmHg (1 mmHg=0.133 kPa), (0.98 ± 0.34) mmHg, (0.66 ± 0.16) mmol/L, (0.15 ± 0.56) mmol/L,the differences were statistically significant (tpaired values were 3.935, 2.030, 4.889, 4.899, all P values<0.05). Conclusion The diabetes self-management program based on Chinese local patients for Nanjing may improve patients' awareness rate of diabetes-related knowledge, self-management behavior, the quality of life, and health status. CDSMP could be applied effectively in Nanjing.

Abstract:Objective. We aim to evaluate the impact of diabetes management shared care clinic (DMSCC) on glycated hemoglobin A1c (HbA1c) compliance and self-management abilities in patients with type 2 diabetes mellitus (T2DM). Methods. This study was a prospective cohort study of patients with T2DM participating in the DMSCC. At baseline and after management, the HbA1c levels were measured, the HbA1c compliance rate were calculated, and the Summary of Diabetes Self-Care Activities-6 (SDSCA-6), Diabetes Empowerment Scale-DAWN Short Form (DES-DSF), and Problem Areas in Diabetes Scale—Five-item Short Form (PAID-5) were completed. These pre- and post-management data were compared. Results. A total of 124 eligible patients were enrolled. After the diabetes management of DMSCC, the average HbA1c decreased and the HbA1c compliance rate increased significantly ( P < 0.01). SDSCA-6 showed significant improvement in physical activity, glycemic monitoring, smoking ( P < 0.01), and taking medication ( P < 0.05). DES-DSF suggested a greater willingness to try to effectively treat diabetes ( P < 0.05). PAID-5 indicated significant improvement in diabetes-related emotional distress. Conclusion. DMSCC can help patients with T2DM reduce HbA1c, increase HbA1c compliance, improve diabetes self-management behaviors, empowerment, and diabetes-related emotional distress and serve as an effective exploration and practice of diabetes self-management education and support.
