Abstract:The safety and health of sailors offshore is of major concern. World Sailing (WS) and International Maritime Health Association (IMHA) are taking seriously the potential dangers to the safety and health at sea. By the nature of their sport, the sailors racing in offshore racing environment can be exposed to injuries and other health problems that can endanger their lives. Being aware of the potential dangers caused by the distance from onshore health facilities and lack of professional help on board, IMHA and WS decided to support the activities that are leading to the enhancement of safety and health protection on board. With common initiative, joint Workgroup on Medical Support in Offshore Racing has been formed and the series of workshop organised. The WS/IMHA Workgroup on Medical Support for Offshore Yacht Races previously reached consensus on the common competences and learning outcomes for medical training for offshore racing. In addition, the Workgroup has also set standards for required medical kit inventory for yachts par- ticipating in the various categories of offshore yacht races. Documents were both approved by WS Medical Commission and the IMHA Board. Fourth workshop on Medical Support for Offshore Yacht Races was held in London, United Kingdom, 1-2 December 2018 and workgroup reached consensus on the standards for availability of Telemedical Advice Services (TMAS) for the various categories of offshore yacht races held under the authority of WS. This position paper sets out how the TMAS should be integrated with the practical usage of medicines and medical equipment on board offshore racing yachts. In addition, this position paper also sets out how the level of medical training integrates with appropriate use of the TMAS. Overall, the three WS/IMHA position papers on the triad of medical inventories, medical training and TMAS, are aimed at providing the best possible medical care on offshore racing yachts, by fully integrating each part of the triad of medical support.

Abstract:Individuals with rare skeletal disorders like Multiple Osteochondromas and Ollier Disease often experience physical and psychological burdens. Adventure therapy, with activities like sailing in outdoor settings, promotes personal growth and psychological well-being, potentially improving health-related quality of life (HRQoL). This study aimed to evaluate the impact of a sailing program on health-related quality of life and participant satisfaction in individuals with Multiple Osteochondromas and Ollier Disease. A quasi-experimental one-group pre-post design was employed, with HRQoL assessed using the EQ-5D® instrument and participant satisfaction measured via a feedback survey. Data were collected before and after the five-day sailing program conducted in the Mediterranean Sea in 2022 and 2023, involving participants diagnosed with Multiple Osteochondromas and Ollier Disease. Statistical analyses were performed using the Wilcoxon signed-rank test and McNemar's test for paired data. A significance level of p < 0.05 and p < 0.10 was considered. A total of 25 participants, predominantly male (52%), with a median age of 16 years (ranking from 11 to 31), were included in the study. The sailing program had mixed impact on HRQoL. Specifically, individuals who were female ( p = 0.03), aged 16 and older ( p = 0.04), with higher educational attainment ( p = 0.10) or stronger self-management ( p = 0.09), resilience ( p = 0.01) and self-engagement ( p = 0.09) skills experienced enhanced HRQoL. Conversely, other participants exhibited an increase in self-care difficulties ( p = 0.02) and a trend towards worsening pain/discomfort ( p = 0.38). Overall satisfaction with the program was high, with 90% of participants expressing satisfaction.

Abstract:Background: Medical emergencies and on-going medical conditions on board may seriously impair seafarers' health and safety, and also negatively impact on future work prospects for seafarers. When a seafarer gets ill or injured on a ship, medical treatment often relies on the competences on his colleagues on board. The aim of this project was to establish a consensus-based minimum standard for medical education for seafarers, in order to ensure competency for adequate management of ill-health on board. Materials and methods: International Maritime Health Foundation (IMHF) conducted a workshop on medical training of seafarers. A research-based approach to gain consensus on core learning outcomes/competences developed by the Tuning Project, has been used. This method was used by Tuning (Medicine) to gain consensus on core learning outcomes for primary medical degrees (Master of Medicine) across Europe. Results: The result of the project is a set of learning outcomes/competences in medical training for merchant seafarers. Conclusions: The project resulted in a set of learning outcomes/competences on medical training of the seafarers that will be submitted to the relevant bodies of International Maritime Organization (IMO) in the process of the development of model courses 1.13, 1.14 and 1.15.
Field study of anthropomorphic and muscle performance changes among elite skippers following a transoceanic race

Abstract:Background: Ocean racing has become increasingly demanding, both physically and psychologically. The aim of the study was to assess global changes after a transoceanic race. Materials and methods: Eight male sailors were evaluated pre- and post-race through anthropometric measurements (weight, skinfold, girth at different level and estimated body fat percentage), multifrequency tetrapolar bioelectrical impedance, muscular performance, visual analogic scale for perceived fatigue and Critical Flicker Fusion Frequencies for cerebral arousal. Results: Compared to pre-race values, a significant decrease in body weight (-3.6 ± 1.4%, p = 0.0002) and body composition with reduction of body fat percentage (-15.1 ± 3.5%, p < 0.0001) and fat mass (-36.4 ± 31.4%, p = 0.022) was observed. Muscle performance of the upper limb was preserved. In the lower limb, monohulls skippers showed a significant reduction of jump height (-6.6 ± 4.8%, p = 0.022), power (-11.7 ± 7.3%, p = 0.011) and speed (-14.6 ± 7.4%, p = 0.0006) while a multihulls skipper showed a gain in speed (+0.87%), power (+8.52%), force (+11%) resulting in a higher jump height (+1.12%). These changes were inversely correlated with sea days (Pearson r of -0.81, -0.96 and -0.90, respectively, p < 0.01). Conclusions: Changes in body weight and composition are consistent with previous data indicating a probable negative energy balance. The main finding demonstrates a difference in muscular conditioning between upper and lower limbs that might be explained by differential workload related to boat architecture (trampolines) or handling.

Abstract:A topical collection on "Advances in Search and Rescue at Sea" has appeared in recent issues of Ocean Dynamics following the latest in a series of workshops on "Technologies for Search and Rescue and other Emergency Marine Operations" (2004, 2006, 2008 and 2011), hosted by IFREMER in Brest, France. Here we give a brief overview of the history of search and rescue at sea before we summarize the main results of the papers that have appeared in the topical collection. Navy En-Route Care in Future Distributed Maritime Operations: A Review of Clinician Capabilities and Roles of Care

Abstract:Objective: As the United States Navy transitions from Operation Iraqi Freedom/Operation Enduring Freedom to preparing for a near-peer competition, an increasing focus of wartime strategy relies upon a network of distributed naval assets for total sea control, known as Distributed Maritime Operations (DMO). Historically, embedded medical personnel have provided care at sea in times of war. Recent reviews of shipboard and evacuated mass casualty incidents have alluded to weaknesses in the existing Navy Medicine approach that will require advances in care provision to sustain high-quality care that would benefit from industry and civilian academic collaboration. To gain input from civilian prehospital expertise and insight, the current DMO and Navy En-Route Care (ERC) systems must be plainly described for non-Navy military and civilian leaders, clinicians, and researchers to understand. Methods: N/A. Results: In this review, we translate US Navy structure and vernacular into common civilian and non-Navy language, describe the maritime role-tiered ERC system, elucidate the medical assets on each naval warship, and discuss clinician levels and capabilities while deployed to help communicate the inherent challenges of US Navy maritime medical care during routine operations, casualty treatment, stabilization, and evacuation. Conclusions: We describe the roles of care, clinician levels, and medical assets within the Navy ERC system for researchers and military leaders who aim to mitigate the inherent challenges of future maritime trauma care in the age of Distributed Maritime Operations. This paper lays the framework of the Navy deployed medical system to enable research in maritime en-route care, and prompt inclusion of identified solutions into common use in the US Navy.

Abstract:The 274th ENMC workshop for optimizing bone strength in neuromuscular disorders (NMDs) was held on January 19-21, 2024. The group of participants included experts in the fields of bone health and neuromuscular medicine along with the patient voice. Bone strength represents a crucial aspect of the management of pediatric and adult patients with NMDs. Bone strength may be compromised due to different pathophysiologic mechanisms, including disrupted bone-muscle "cross-talk", loss of biomechanical loading, nutritional insufficiency, inadequate weight-bearing physical activity, muscle weakness and/or immobility, and drug treatment. While for Duchenne muscular dystrophy recommendations for evaluation and treatment of bone strength have been published, evidence on bone strength in other hereditary and acquired NMDs is scarce. Enhanced knowledge is needed to understand the development and maintenance of bone strength in patients with NMDs. This workshop aimed to develop a strategy to improve bone strength and thus prevent fractures in patients with NMDs.
Beach Sprints Rowing: Injury and Illness Prevalence at the 2022 World Championships

Abstract:Background: Injury and illness surveillance helps establish the infrastructure to provide adequate medical support at regattas and is the foundation for developing prevention strategies. Objectives: To assess the prevalence and characteristics of injuries in the 4 weeks before the start of the 2022 World Rowing Beach Sprints Finals (WRBSF) (the 'Prevalence Survey') and describe the incidence and nature of new-onset injuries and illnesses incurred during the WRBSF (the 'Incidence Survey'). Methods: Athletes completed: (1) a prevalence survey recording injuries the 4 weeks before the WRBSF and (2) a recording of injuries and illnesses that occurred during the 3-day regatta. Results: Fifty-nine of 152 eligible WRBSF athletes completed the prevalence injury survey. Twenty-three (38.9%) reported experiencing at least one injury within the 4 weeks before the WRBSF. The most prevalent anatomical injury sites were the forearm (11.86%), lumbar spine (10.17%), knee (8.47%), ankle (6.78%) and hand/fingers (6.78%). During the competition, only two illness occurrences were reported; both were respiratory infections. Two athletes reported injuries: a hamstring strain and a concussion. Conclusion: Rowers presenting to the WRBSF described injuries leading up to the event that were similar to those common in classic rowing. Rowers at the event suffered injuries of the lower limb that were different from classic rowing and may be related to the addition of running to this event. An event concussion should be considered as a more likely injury in this type of rowing and future events should be prepared to manage such an injury.

Efficacy of Medical Operations and Layout Planning Onboard Nontraditional US Navy Vessels at High Seas

Abstract:Introduction: Attempting to expedite delivery of care to wounded war fighters, this study aimed to quantify the ability of medical and surgical teams to perform lifesaving damage control and resuscitation procedures aboard nontraditional US Navy Vessels on high seas. Specifically, it looked at the ability of the teams to perform procedures in shipboard operating and emergency rooms by analyzing motion of personnel during the procedures. Methods: One hundred and twelve damage control and resuscitation procedures were performed during a voyage of the US Naval Ship Brunswick in transit from Norfolk, Virginia, to San Diego, California. The ability of personnel to perform these procedures was quantified by the use of motion link analysis designed to track the movement of each participant as they completed their assigned tasks. Results: The link analysis showed no significant change in the number of movements of participants from the beginning to the end of the study. However, there was a learning effect observed during the study, with teams completing tasks faster at the end of the study than at the beginning. Conclusion: This shows that the working conditions aboard the US Naval Ship Brunswick were satisfactory for the assigned tasks, indicating that these medical operations may be feasible aboard nontraditional US Navy vessels.

Abstract:The aims of the study are: 1) to replace the urine glucose test for diabetes with more than 50% false negatives, with an accurate screening for type 2 diabetes and hypertension in the mandatory biannual fit-for-duty medical examinations of seafarers; 2) to produce data driven ‘Green Ship’ health promotion in the ships. A new health promotion and disease prevention public health intervention program integrated in the fit-for-duty medical examinations for seafarers is being developed. The lack of an accurate diagnosis of type 2 diabetes is replaced by accurate HbA1c and/or fasting glucose tests and the test for hypertension in various disease stages is based on the International Associations’ Guidelines. A ‘Green Ship’ health promotion program is proposed for all on board, not only for diseased crew members. A protocol for an accurate biannual screening for diabetes and hypertension is presented. Educational programs for medical doctors and seafarers on the management of hypertension and diabetes on board will be developed. Presuming that all crew members are potentially on their way to be pre-diseased or are diseased, the ‘Green Ship’ health promotion program is implemented for the whole crew. The International Labour Organization and the National Maritime Authorities are prompted to revise the International and the National Guidelines for Seafarers Medical Examinations, respectively. Concerted actions are requested to implement public health promotion projects in shipping. Abstract:Background: Current injury prevention programmes in football are limited by a one-size-fits-all approach, which predominantly focuses on preventive exercise programmes while ignoring differences in risk profiles between individuals and teams. Objective: To address this gap, we developed a new data-driven, customisable approach based on the principles of risk management. We collaborated with key stakeholders to identify focus areas for injury and illness prevention and determine their priorities. Setting: The team medical and coaching staff included members from 17 professional football clubs, the national team and a youth football academy in Qatar. Methods: In 2015, we launched a series of annual workshops under the Aspetar Sports Injury and Illness Prevention Programme. The workshops included club medical personnel and fitness coaches in a process to develop team-specific programmes for injury and illness prevention based on the principle of risk management. Over 2 years, workshops refined focus areas through discussions, surveys and small-group presentations, culminating in the creation a novel programme for football injury prevention. Results: Out of 44 focus areas first identified, 23 were selected as priorities for inclusion in multimodal injury and illness prevention programmes. The identified focus areas represent a variety of aspects, including social/behavioural/lifestyle, exercise programmes/training, load management, recovery and equipment. The top priorities included communication, the Nordic hamstring exercise, training load, recovery strategies, nutrition, sleep, warm-up, the Copenhagen adduction exercise and core and dynamic stability. Conclusion: We have developed a comprehensive framework for preventing injuries and illnesses in football grounded in the general principles of risk management. This framework has proven feasible and led to the creation of a new multicomponent programme, The Aspetar IP2 (Injury and Illness Prevention for Performance) NetWork, focusing on a range of areas beyond preventive exercise programmes only.
Remote Training of Neurointerventions by Audiovisual Streaming : Experiences from the European ESMINT-EYMINT E-Fellowship Program

Abstract:Background: Remote access of trainees to training centers via video streaming (tele-observership, e‑fellowship) emerges as an alternative to acquire knowledge in endovascular interventions. Situational awareness is a summary term that is also used in surgical procedures for perceiving and understanding the situation and projecting what will happen next. A high situational awareness would serve as prerequisite for meaningful learning success during tele-observerships. We hypothesized that live perception of the angiographical procedures using streaming technology is feasible and sufficient to gain useful situational awareness of the procedure. Methods: During a European tele-observership organized by the European Society of Minimally Invasive Neurological Therapy (ESMINT) and its trainee association (EYMINT), a total of six neurointerventional fellows in five countries observed live cases performed by experienced neurointerventionalists (mentors) in six different high-volume neurovascular centers across Europe equipped with live-streaming technology (Tegus Medical, Hamburg, Germany). Cases were prospectively evaluated during a 12-month period, followed by a final questionnaire after completion of the course. Results: A total of 102/161 (63%) cases with a 1:1 allocation of fellow and mentor were evaluated during a 12-month period. Most frequent conditions were ischemic stroke (27.5%), followed by embolization of unruptured aneurysms (25.5%) and arteriovenous malformations (AVMs) (15.7%). A high level of situational awareness was reported by fellows in 75.5% of all cases. After finishing the program, the general improvement of neurointerventional knowledge was evaluated to be extensive (1/6 fellows), substantial (3/6), and moderate (2/6). The specific fields of improvement were procedural knowledge (6/6 fellows), technical knowledge (3/6) and complication management (2/6). Conclusion: Online streaming technology facilitates location-independent training of complex neurointerventional procedures through high levels of situational awareness and can therefore supplement live hands-on-training. In addition, it leads to a training effect for fellows with a perceived improvement of their neurointerventional knowledge.

Abstract:Abstract This presentation is an analytic review of our research work among fishermen of coastal sector (FCS) in Morocco which focused on the assessment of occupational safety and health (OSH), stress, burnout, sleep and alertness disorders, risk-taking behaviors (ordalisme, sexually transmitted infections), consumption of psychoactive substances, musculoskeletal disorders and skin diseases. The dangerousness of the work of fishermen have been known since antiquity. The human fragility at sea was formulated by the Greek philosopher Anacharsis, who declared that “there are three sorts of people: those who are alive, those who are dead, and those who are at sea”. The relentless struggle of FCS and the sea continues for centuries, and the maritime environment remains dangerous to those who work there especially in developing countries. Working at sea is associated with a number of occupational risks and challenges. The FCS carry out manual activities, characterized by heavy work-load, very intense rhythms and inconvenient working- hours in a hostile environment (bad weather, promiscuity in dilapidated boats, etc.). FCS constitute a particular target for physical, chemical and psychosocial risks (PSR) because they are exposed to high relational and organizational constraints linked to difficult working conditions and concerns about their daily earnings. The fishermen profession is universally known as being the most accident-prone. Indeed, the rates of occupational accidents and deaths are respectively three times and 44 times higher among fishermen than among land-based workers. PSR can be induced by the activity itself or generated by the organization and work relationships. They correspond to work situations where fatigue, stress, harassment are present. The combination of an unsafe work environment, job strain and socio-economic difficulties is a major risk factor for chronic stress that can lead to burnout syndrome. The harmful effects of PSR on the psychological and physical health of workers are well established: psychosomatic manifestations, multiple chronic illnesses (cardiovascular, musculoskeletal, metabolic, etc.), compensatory behavior (consumption of psychoactive substances), etc. The risks in this sector are universal, however, our research has the merit of raising and discussing a number of questions specific to fishermen in Morocco. Indeed, if difficult and dangerous working conditions play a main role in the genesis of risks, modulators can intervene to strengthen or weaken this influence. These have been grouped into three sections: - the intrinsic and specific factors of the working environment in this sector (the activity of the FCS, interpersonal relations and the relationship between the fishermen and the fishing sector), - para-professional economic, social, ethnic and cultural environmental factors, - individual factors and personality traits of FCS. Working conditions at sea, low levels of education and ordalisme make it difficult to implement programs able to improve OSH. However, it is a need for introducing of specific strategies, which should be adapted and effective. Health promotion and education initiatives should be carried out to raise fishermen’s awareness of occupational risks and their consequences. An integrated prevention management system, linked to real conditions, must promote appropriate, planned and prioritized preventive measures to improve quality of life and working conditions.

Deep Sea Research Part II Topical Studies in Oceanography

Abstract:An international conference on the oceanography of the Ross Sea was held in Naples, Italy in July 2023. A total of 75 abstracts were presented orally, and 20 presented as posters. Researchers from 11 countries attended, with conference support from a variety of public and private firms. The conference was held under the patronage of the United Nations Decade of Ocean Science for Sustainable Development, the Southern Ocean Observing System, the Ministero dell’Università e della Ricera, the Programma Nazionale di Recerche in Antartide, and the Commissione Oceanografica Italiana, and was organized by Parthenope University of Naples and Marche Polytechnic University of Ancona. The papers included were either presented at the meeting or were closely related to the conference's goals and program. The conference maintained Italy's long tradition of hosting engaging conferences on oceanographic research in the Ross Sea.

Abstract:Access to health care always has been one of the most critical issues for offshore crews. Therefore, telemedicine has a long and outstanding tradition in the maritime environment. Since 1931 Cuxhaven medical center (Medico Cuxhaven) operates as a hospital-based radio medical advice center (RMA) for ships worldwide providing one of the first routine telemedical services worldwide. For a long time this task was performed on a honorary basis by the hospital's physicians. In 1994 Germany accepted the IMO/ILO (International Maritime Organisation and International Labour Organisation) solution 164. Therefore, in 1998 a formal contract of the German Ministry of Transport officially installed Medico Cuxhaven as the TMAS Center for Germany. According to IMO/ILO solution 164, the RMA provides an expert level of care 24 h/day, 365 days/year. Cuxhaven hospital is a busy general hospital with departments of anesthesia and intensive care, internal medicine, surgery, gynecology and obstetrics, urology, pediatrics, ENT, radiology, and pain medicine. All physicians directly responding to TMAS calls have practical experience in maritime routine and emergency medicine. All incoming emergency calls are primarily handled by board-certified anesthesiologists, experienced in martime emergency medicine (e. g., staffing coastal SAR helicopters, working on rescue boats, carrying out repatriation services for sailors worldwide). If needed, other medical specialists are included. Today, Medico Cuxhaven takes an average workload of one to two emergency calls (response time < 20 s), two to three follow-up calls and approximately one informative call per day. Since 2000 transmission of digital photos has been used for routine RMA. Recently, a system for online and trenddata telemetry of twelve-lead ECG, NIBP (noninvasive blood pressure measurement), CO(2), SaO(2), pulse and respiration rates including the transmission of video images was developed. This system allows worldwide communications as it is capable of all possible means of telecommunication such as GSM, Inmarsat-ISDN or Iridium-Satellite-Transmission. So far, this system has been installed on board several German SAR rescue boats.However, radio telephone and the simple transmission of digital photography remain the basis of maritime radio medical advice service.
Accidents, diseases and health complaints among seafarers on German-flagged container ships

Abstract:Background: For seafarers on the high seas health hazards are various and due to the setting also specific. The spectrum of job-related health impairments and accidents is mainly influenced by the maritime characteristics. The aim of this study is to assess the kind of accidents and the frequency of diseases and health complaints among seafarers on German container ships by evaluating medical log books. Methods: A systematic analysis of 14,628 medical entries from 95 medical log books of 58 container ships under German flag from 1995 to 2015 was performed. This monocentric retrospective and descriptive study used information on accidents, diseases and health complaints among different occupational groups and medical treatment procedures for the analysis and evaluation. Results: The analysis showed that more than one third of all consultations with the Health Officer on board are related to internal (33.7%) and surgical (31.3%) symptoms. Almost twenty percent of consultations were due to respiratory infections (19.6%) and accidents (17.9%). Accidents represented the most frequent reason for unfitness for sea service (31.2%). Based on occupational categories, most injuries occurred among deck crew (22.5%), followed by ratings working in the engine room (18.9%). In 106 cases, telemedical contact with a physician ashore was necessary. In total, 15 seafarers had to be evacuated from the ship for further medical treatment onshore. Medicine/drug application was the most common therapeutic measure on board, accounting for 77% of all consultations. Conclusions: The high proportion of health complaints and accidents among seafarers shows that there is a need to optimize medical care at sea and accident prevention, e.g. by standardized treatment algorithms or improving the medical training of Health Officers. The development and introduction of a digital patient file to record medical treatments on the vessels could also improve medical documentation on board.
Lifestyle interventions in the maritime settings: a systematic review

Abstract:Background: Evidence on workplace health promotion interventions at sea is scattered and includes different methodological approaches. The continued focus on lifestyle and health promotion on land-based industries makes it pertinent to evaluate available data from maritime settings to gain systematic knowledge on the field. Methods: In this systematic review, we systematically searched PubMed and NLM Gateway (for MEDLINE), Institute of Scientific Information/Web of Science (ISI/WOS), and SCOPUS up to January 2019 using standard keywords including lifestyle interventions in the maritime setting. Two independent reviewers assessed papers and extracted the data. The quality of included studies was assessed using the Cochrane Risk of Bias tool. Due to significant heterogeneity between studies, the effectiveness of interventions was presented as a qualitative synthesis. Results: After the initial search and refinement based on a total of 4432 records, ten articles met eligibility criteria and were included in the final review. Six studies originated from US maritime settings, 3 studies were conducted on Danish seafarers and one study came from Finland. The main focus of 6 studies was educational interventions including stress management, healthy eating, anti-smoking and anti-drinking sessions, sexual behavior program, and advice about preventive strategies. Four studies described the implementation of interventions, including micro-nutrient supplementation, physical activity, and pharmacotherapy. Follow-up assessments occurred over a time period ranging from 80 days to 2 years. Three studies found a positive though limited effect of structural and/or education interventions in maritime settings. The quality of all included studies was modest. Conclusion: Results of this systematic review show that a limited number of studies of lifestyle interventions in the maritime setting exist and that the quality of them is generally modest. Also, most of the interventions identified have failed to demonstrate substantial health benefits for seafarers. Systematic review registration number in PROSPERO: CRD42019134533.
