Abstract:Primary hemifacial spasm with few exceptions is due to the vascular compression of the facial nerve that can be evidenced with high resolution MRI. Microvascular decompression is the only curative treatment for this pathology. According to literature review detailed in chapter "conflicting vessels", the compression is located at the facial Root Exit Zone (REZ) in 95% of the cases, and in 5% distally at the cisternal or the intrameatal portion of the root as the sole conflict or in addition to one at brainstem/REZ. Therefore, exploration has to be performed on the entire root, from the ponto-medullary fissure to the internal auditory meatus. Because microvascular decompression is functional surgery, the procedure should be as harmless as possible and with a high probability of permanent efficacy. Besides facial palsy, main complications are hearing loss, tinnitus and gait disturbances. Causes are cochlea/labyrinth ischemia due to manipulations of their nutrient arteries and/or stretching of the eight nerve complex. To minimize the latter, the approach should not be with lateral-to-medial retraction of the cerebellar hemisphere, but along an infra-floccular trajectory, from below. In fact, most of the neurovascular conflicts are situated ventro-caudally to facial REZ at the brainstem, particularly those from a megadolicho-vertebrobasilar artery and its posterior inferior-cerebellar branch. Also, care should be taken not to cause any injury of the manipulated vessels or stretching of their perforators to brainstem. Heating from bipolar coagulation must be avoided. The inserted material used to maintain the offending vessel(s) away must not be neo-compressive. Intraoperative neuromonitoring is considered to be useful for achieving safe surgery at least until the learning curve has reached an optimal level, particularly BrainstemAuditory Evoked Potentials recordings. Increase in latency and/or decrease in amplitude of wave V warn excessive stretching or damage to the cochlear nerve, and decrease in amplitude of wave I signals possible ischemia of the cochlea. Free-running EMG of the facial muscles may warn against excessive manipulation of the facial nerve. Recording of the lateral spread responses - which are a sign of hyperexcitabilty of the facial motor system - may provide information on completeness of the decompression.

Abstract:This article describes the anatomical approach and operative points of microvascular decompression for hemifacial spasm based on the author's personal experience with 1,000 cases. As vascular compression of the facial nerve is often quite caudal, it is necessary to ensure that the compressing arteries in this area are moved. Neither the vessels nor the prosthesis used to move them should not touch the root exit zone of the nerve(vascular transposition nerve isolation technique). It should also be emphasized that every effort should be made to avoid complications such as hearing impairment and postoperative subcutaneous spinal fluid accumulation.
Microscopic measurement of the facial nerve root exit zone from central glial myelin to peripheral Schwann cell myelin

Abstract:Object: The authors have attempted to define the exact borders of the root exit zone (RExZ) of the facial nerve, measure the distribution of myelin histologically, and examine the relationship between contact vessels and the RExZ. Methods: Seventy-five facial nerves were obtained from brainstems excised from 44 adult patients at autopsy. The arteries and veins associated with the facial nerve were counted and measured. The facial nerves, associated vasculature, and adjoining portions of the brainstem were then removed en bloc. These tissues were serially sectioned and stained, and a photomicrograph of each section was obtained. The distribution of myelin on each section was measured from the upper edge of the supraolivary fossette, and the relationship between contact vessels and the RExZ examined. The lateral transitional zone of the facial nerve began 8 mm distal to the upper edge of the supraolivary fossette (root exit point [RExP]) and had a mean length of 1.9 mm. The root detachment point (RDP) of the facial nerve at the medial side was located very close to the beginning of the medial transitional zone. In more than 80% of the nerves that were examined, vascular structures compressed the central glial myelin of the nerve. Conclusions: The authors propose the use of the terms "RExP," "RDP," and "transitional zone," instead of RExZ, which cannot be well defined. The RDP appears to be a good landmark for use during microvascular decompression.
Infranuchal infrafloccular approach to the more vulnerable segments of the facial nerve in microvascular decompressions for the hemifacial spasm

Abstract:Objective: We investigated the locations of compressing vessels in hemifacial spasm. To approach compression sites, we described and evaluated the efficacy of the infranuchal infrafloccular (INIF) approach. Methods: A retrospective review of 31 consecutive patients who underwent microvascular decompression (MVD) through INIF with a minimum follow-up of 1 year was performed. Along the intracranial facial nerve, we classified the compression sites into the transitional zone (TRZ), the central nervous system (CNS) segment and the peripheral nervous system (PNS) segment. The INIF approach was used to inspect the CNS segment and the TRZ. Subdural patch graft technique was used in order to achieve watertight dural closure. The cranioplasty was performed using polymethylmethacrylate. The outcome and procedure-related morbidities were evaluated. Results: Twenty-nine patients (93%) showed complete disappearance of spasm. In two patients, the spasm was resolved gradually in 2 and 4 weeks, respectively. Late recurrence was noted in one patient (3%). The TRZ has been identified as the only compression site in 19 cases (61.3%), both the TRZ and CNS segment in 11 (35.5%) and the CNS segment only in 1 (3.2%). There was no patient having a compressing vessel in the PNS segment. Infection as a result of cerebrospinal fluid leak occurred in one patient (3%). Delayed transient facial weakness occurred in one patient. Conclusion: The TRZ and the CNS segment were more vulnerable area to the compression of vessels. We suggest that surgical avenue with the INIF approach provides early identification of this area.c.
Intermediate Nerve Neuralgia Can Be Diagnosed and Cured by Microvascular Decompression

Abstract:Here, we present a case of a 55-year-old woman with a 10-year history of hemifacial spasm accompanied by 1-month ipsilateral paroxysmal otalgia. Magnetic resonance imaging revealed the presence of vessels around the facial nerve root. Surgical exploration via suboccipital retromastoid craniotomy showed converging compression of the facial nerve root and intermediate nerve from both sides by an anterior inferior cerebellar artery loop. The patient's hemifacial spasm and ipsilateral otalgia were completely relieved after microvascular decompression of the facial nerve root and intermediate nerve. Intraoperative findings and the postoperative result of this case confirmed that vascular compression of the intermediate nerve was the exclusive cause of paroxysmal otalgia. The presence of ipsilateral hemifacial spasm, combined with preoperative neuroimaging studies, contributed to the diagnosis of intermediate nerve neuralgia. Microvascular decompression should be considered for the management of patients with intermediate nerve neuralgia.

Abstract:Background: Hemifacial spasm (HFS) is most often caused by blood vessels touching a facial nerve. In particular, responsible vessels compress the root exit zone (REZ) of the facial nerve. Although we recognize these causes of HFS, it is difficult to evaluate the findings of precise lesion in radiological imaging when vessels compress REZ. Hence, we tried to obtain precise images of pre- and postoperative neuroradiological findings of HFS by creating a fusion image of MR angiography and the REZ of facial nerve extracted by magnetic resonance imaging (MRI) diffusion tensor image (DTI). Case description: A 52-year-old woman had a 2-year history of HFS on the left side of her face. It was confirmed that the left vertebral artery and anterior inferior cerebellar artery were presented near the facial nerve on MRI. REZ of the facial nerve was visualized using DTI and fusion image was created with vascular components, making it possible to recognize the relationship between compression vessels and REZ of the facial nerve in detail. She underwent microvascular decompression and her HFS completely disappeared. We confirmed that the REZ of the facial nerve was decompressed by MRI imaging, in the same way as before surgery. Conclusion: We describe that the REZ of facial nerve and compressive vessels was delineated in detail on MRI and this technique is useful for pre- and postoperative evaluation of HFS.
Microanatomy of the central myelin portion and transitional zone of the oculomotor and abducens nerves

Abstract:Background: The microanatomy of the central myelin portion and transitional zone of several cranial nerves including trigeminal, facial, vestibulocochlear, glossopharyngeal, and vagus nerves have been clearly demonstrated to provide information for neurovascular compression syndrome such as trigeminal neuralgia and hemifacial spasm. However, the study of oculomotor and abducens nerve is limited. Materials and methods: Oculomotor and abducens nerves were harvested with a portion of brainstem and embedded in paraffin. Longitudinal and serial sections from ten of each cranial nerve were stained and a photomicrograph was taken to make the following observations and measurements: 1) patterns of central myelin portion, 2) length of central myelin portion, and 3) depth of central myelin- peripheral myelin transitional zone. Results: For oculomotor nerve, the longest central myelin bundle was always seen on the first nerve bundle and that the length of central myelin decreased gradually. For abducens nerve, morphological patterns were classified into four types based on number of nerve rootlets emerging from the brainstem and number of nerve bundles in each rootlet. Length of central myelin portion was between 0.36-6.10 mm (2.75 ± 0.83 mm) and 0.13-5.01 mm (1.66 ± 1.39 mm) for oculomotor and abducens nerves, respectively. The oculomotor nerve transitional zone depth was 0.07-058 mm (0.23 ± 0.07 mm), while for abducens nerve, depth was 0.05-0.40 mm (0.16 ± 0.07 mm). Positive weak correlations between central myelin and depth of TZ were found in oculomotor nerve (r +0.310, p < 0.05) and abducens nerves (r +0.413, p < 0.05). Conclusions: Detailed microanatomy of the central myelin and transitional zone might be beneficial for locating the site of compression in neurovascular conflicts at oculomotor and abducens nerves.
Feature Integration, Multi-image Queries and Relevance Feedback in Image Retrieval

Abstract:In this paper, we explore the effect of feature integration, multi-image queries, and relevance feedback in enhancing the performance of an image retrieval system. Weighted integration of structure, color and texture features is studied. In addition, we propose a methodology of retrieval consisting of multiple query images, as opposed to the traditionallyused model of a single query image. Two different mechanism of relevance feedback are also proposed and analyzed. Integration of features and feedback significantly improves the performance of the retrieval system.
Indication for a skull base approach in microvascular decompression for hemifacial spasm

Abstract:Background: A thorough observation of the root exit zone (REZ) and secure transposition of the offending arteries is crucial for a successful microvascular decompression (MVD) for hemifacial spasm (HFS). Decompression procedures are not always feasible in a narrow operative field through a retrosigmoid approach. In such instances, extending the craniectomy laterally is useful in accomplishing the procedure safely. This study aims to introduce the benefits of a skull base approach in MVD for HFS. Methods: The skull base approach was performed in twenty-eight patients among 335 consecutive MVDs for HFS. The site of the neurovascular compression (NVC), the size of the flocculus, and the location of the sigmoid sinus are measured factors in the imaging studies. The indication for a skull base approach is evaluated and verified retrospectively in comparison with the conventional retrosigmoid approach. Operative outcomes and long-term results were analyzed retrospectively. Results: The extended retrosigmoid approach was used for 27 patients and the retrolabyrinthine presigmoid approach was used in one patient. The measurement value including the site of NVC, the size of the flocculus, and the location of the sigmoid sinus represents well the indication of the skull base approach, which is significantly different from the conventional retrosigmoid approach. The skull base approach is useful for patients with medially located NVC, a large flocculus, or repeat MVD cases. The long-term result demonstrated favorable outcomes in patients with the skull base approach applied. Conclusions: Preoperative evaluation for lateral expansion of the craniectomy contributes to a safe and secure MVD.

Abstract:A correlation between high plasma serotonin levels and total pulmonary resistance was reported in more than 80% of pulmonary hypertensive patients. When submitted to chronic hypoxia (10% O2 for more than 3 weeks), wildtype mice develop lung vascular remodeling and pulmonary hypertension. We previously reported that, by contrast, the development of these hypoxiadependent alterations is totally abolished in mice with permanent (genetic) or transient (pharmacologic) inactivation of the serotonin 5-HT2B receptor. In the present study, we asked whether 5-HT2B receptors could be involved in the control of plasma serotonin levels. Further investigating the chronic-hypoxic-mouse model of pulmonary hypertension, we first show that in wildtype mice, plasma serotonin levels and 5-HT2B receptors expression were significantly increased after chronic exposure to hypoxia. This increase appeared before significant changes in remodeling factors could be detected and persisted when the pathology was established. Conversely, in mice with either genetically or pharmacologically inactive 5-HT2B receptors, plasma serotonin levels were not modified by chronic hypoxia. We then confirmed that 5-HT2B receptors can control plasma serotonin levels by providing in vivo evidence that an acute agonist stimulation of 5-HT2B receptor triggers a transient increase in plasma serotonin that is serotonin transporter dependent and blocked by 5-HT2B receptor selective antagonist or genetic abalation. Our data support the notion that a 5-HT2B receptor-dependent regulation of serotonin uptake is implicated in the control of plasma serotonin levels. Fully endoscopic microvascular decompression for hemifacial spasm: a clinical study and analysis

Abstract:Fully endoscopic microvascular decompression (MVD) of the facial nerve is the main surgical treatment for hemifacial spasm. However, the technique presents distinct surgical challenges. We retrospectively analyzed prior cases to consolidate surgical insights and assess clinical outcomes. Clinical data from 16 patients with facial nerve spasms treated at the Department of Neurosurgery in the First Affiliated Hospital of Bengbu Medical College, between August 2020 and July 2023, were retrospectively examined. Preoperatively, all patients underwent magnetic resonance angiography to detect any offending blood vessels; ascertain the relationship between offending vessels, facial nerves, and the brainstem; and detect any cerebellopontine angle lesions. Surgery involved endoscopic MVD of the facial nerve using a mini Sigmoid sinus posterior approach. Various operative nuances were summarized and analyzed, and clinical efficacy, including postoperative complications and the extent of relief from facial paralysis, was evaluated. Fully endoscopic MVD was completed in all patients, with the offending vessels identified and adequately padded during surgery. The offending vessels were anterior inferior cerebellar artery in 12 cases (75%), vertebral artery in 3 cases (18.75%), and posterior inferior cerebellar artery in 1 case (6.25%). Intraoperative electrophysiological monitoring revealed that the lateral spread response of the facial nerve vanished in 15 cases and remained unchanged in 1 case. Postoperative facial spasms were promptly alleviated in 15 cases (93.75%) and delayed in 1 case (6.25%). Two cases of postoperative complications were recorded-one intracranial infection and one case of tinnitus-both were resolved or mitigated with treatment. All patients were subject to follow-up, with no instances of recurrence or mortality. Fully endoscopic MVD of the facial nerve is safe and effective. Proficiency in endoscopy and surgical skills are vital for performing this procedure.

Abstract:This review article discusses the clinical significance of intraoperative neurophysiological monitoring (IONM), provides recommendations for monitoring protocols, and considers the interpretation of results in microvascular decompression (MVD) for hemifacial spasm (HFS). The lateral spread response (LSR) is an important monitoring parameter during MVD. It helps to identify the responsible blood vessel and confirms its thorough decompression from the facial nerve. The disappearance of the LSR during surgery is associated with favorable clinical outcomes. Standard and revised monitoring protocols and the confirmation of LSR persistence and disappearance are also discussed. The blink reflex and other facial nerve monitoring modalities, such as free-running electromyography, facial motor evoked potentials, F-waves, and the Z-L response, are further considered.
[Prevention of perforating plantar ulcers: trial directed by a mobile team].

Abstract:AIMS to prevent the appearance of plantar ulcerations and then mutilations, by going in the field in order to: make suitable footwear, educate the patients, train the paramedical staff. Means: 1 fitted lorry, 1 physiotherapist, 1 shoemaker, and 1 educator. RESULTS In one year, 206 patients have been provided with shoes and followed up. Advice have been taken exactly in the leper villages in which the attendance rate is of 98%; this rate varies between 47 to 70% in the all-purpose dispensaries. Paramedical workers of all-purpose Health Centers did not take great interest in this action. After 6-12 months under observation: 84% of good results for the feet without deformity or slightly deformed; 51% for the deformed feet. The results are quite satisfactory for the feet without plantar ulcer at the beginning, and that whatever the food deformity stage. But for the feet wounded by plantar ulcer at the beginning, 33% of recovery have been reported after wearing these shoes.
Postural Changes of the Dural Sac in the Lumbar Spines of Asymptomatic Individuals Using Positional Stand-Up Magnetic Resonance Imaging

Abstract:Study Design. Positional magnetic resonance imaging (MRI) study of control subjects. Objectives. To determine dimensional changes in the lumbar dural sac as a function of posture, and to establish changes between the supine, erect and seated positions. Summary of Background Data. Studies using computerized tomography and MRI were done to determine the mechanical effects on the lumbar spinal canal in the different positions. There has been no consecutive study, however, in which normal individuals were investigated for positional changes of the dural sac, including true standing position. Methods. Thirty-two male asymptomatic volunteers were recruited. The examination was performed using a new MRI system. All subjects were examined with sagittal T2 and axial T1-weighted spin-echo images. The subjects were studied in the supine, standing, and sitting positions. The measurements were made using OSIRIS software (Digital Imaging Unit University Hospital of Geneva, Geneva, Switzerland). On axial images, dural sac cross-sectional area and anteroposterior (AP) dural sac diameter were measured at the level of the L3/4, L4/5, and L5/S1 discs. On midsagittal images, AP dural sac diameter and the upper-endplate angles of L1 and S1 were measured. Results. We found a disc degeneration or disc protrusion in 41% (12/29) of the subjects, but there was no obvious compression of the dural sac. Depending on the postures, the mean dural sac cross-sectional area and AP dural sac diameter changed. At all levels, mean dural sac cross-sectional area in the supine position was significantly smaller than in other postures. The dural area decreased most at the L5/S1 level due to positional change from standing to supine. The largest dural area at the L5/S1 level was in sitting extended. AP dural sac diameter on axial and midsagittal images showed a similar tendency. Conclusions. A significant posture-dependent difference of the dural sac cross-sectional area at the level of intervertebral disc in asymptomatic volunteers has been demonstrated. When the posture changed from supine to standing position, lumbar dural sac volume expanded by the increased pressure of cerebrospinal fluid, and the dural sac cross-sectional area increased. The smallest values were found in the supine position.

Abstract:Indications corridor and limits of exposure: This video demonstrates our endoscope-assisted microvascular decompression (MVD) technique in hemifacial spasm. A 2-cm lower retrosigmoid approach is used to reach the facial nerve exit zone. The additional use of endoscopy serves to overcome the microscopes linear axis of view on the compression site. Anatomic essentials need for preoperative planning and assessment: A neurovascular conflict in the facial nerve exit zone is to be identified on CISS-MRI. A CT scan helps assessing the approach. Acoustic evoked potentials and facial nerve neuromonitoring including lateral spreads are required. 1. Essential steps of the procedure: The patient is positioned supine with 45° head rotation to the contralateral side. In addition, the operating table is tilted 30° to facilitate optimal cerebellar retraction by gravity avoiding the need for cerebellar spatula. The dura is incised parallel to the sigmoid sinus. With the operating microscope, the arachnoid is dissected exposing the vestibulocochlear nerve and the lower cranial nerves. The lower cranial nerve group is exposed up to the exit from the brain stem, enabling a subfloccular approach to the facial nerve exit zone. The endoscope is used to inspect the facial nerve and to identify the compressing vessel. Microscopically, the vessel is mobilized and the nerve decompressed by shredded Teflon. 2. Pitfalls/avoidance of complications: Jugular vein compression by excessive head rotation must be avoided. Teflon placed directly onto the nerve can cause spasms itself. Opened mastoid cells are carefully sealed. Variants and indications for their use: Transposition is favored over interposition. Besides shredded Teflon, a Teflon-Bridge or Teflon-Sling can be placed. 3-5The patient consented to the procedure and to the publication of her image.

Abstract:Objective To study the anatomy relationship among the vessles, nerves and bone landmarks in the region of CPA and provide operation-related anatomy knowledge for microvascular decompression of trigeminal neuralgia and hemifacial spasm. Method 16 cadaver specimens were used. 32 sides of CPA region were dissected with retrosigmoid approach. The optimal bone window center for Ⅶ,Ⅷ cranial nerves was localized and the relationship of nerves and vessels were observed and measured. Results (1)The bone window was 2 cm under the midpoint of superior nuchal line superiorly, 1 cm above superior margin of foramen magnum inferiorly, 3 cm lateral to occipital sinus medially, 0.5 cm medial to mamillary process base laterally. The center of the bone window was properly above the Ⅶ,Ⅷ cranial nerves. (2)The widest distance between the Ⅶ,Ⅷ cranial nerves and trigeminal nerve was 10.20～12.18 mm, mean (11.19±0. 12)mm in the cistern. (3)The distance between Ⅶ,Ⅷ cranial nerve and petrosal vein is 9.10±11.22 mm, mean(10.05±0.11)mm,and this interspace can be used to perform microvascular decompression without cutting off petrosal vein. Conclusions The bone window for microvascular decompression in CPA can be optimized to a special location. The locations of trigeminal nerve and Ⅶ,Ⅷ cranial nerves are relatively constant. Therefore these nerves can be used as intraoperative landmarks. The microvascular decompression operations can be performed in the interspace between nerves and vessles without cutting off petrosal vein.

Abstract:Objective: The surgical anatomy of the nervus intermedius (NI) is highly variable. The aim of this study was to describe the anatomy of the NI during endoscope-assisted microvascular decompression (MVD) in hemifacial spasm (HFS), and the involvement of the nerve in the vascular conflict. Methods: The authors reviewed a prospectively maintained database for MVDs performed between 2002 and 2022 and extracted clinical data including patient demographics, symptoms, and offending vessel(s). Operative videos and photographs were analyzed retrospectively in an attempt to identify the NI. Results: Endoscopic identification of the NI was possible in 139 of 435 MVDs. The anatomy is very variable. In 79 (56.8%) patients, a single-bundle pattern was detected, whereas a multiple-bundle pattern was identified in 60 (43.2%) patients. Overall the most common pattern was a single-bundle type A (49.7%). In 20.1%, a multiple-bundles type A was identified. In 4.3%, a single-bundle type B was detected. In 2.9% a single-bundle type C was found, and in just 0.7% a multiple-bundles type C was detected. A multiple-origin pattern (type D) was found in 31 patients (22.3%). The NI was frequently involved in the neurovascular conflict (approximately 85%). The type of NI or vascular compression pattern did not affect the results regarding the outcome or recurrence of HFS. Conclusions: The anatomy of the NI is for the first time evaluated endoscopically in MVD for HFS. The nerve had various anatomical patterns that were clearly identified. Further studies to evaluate the compression patterns in relation to NI neuralgia are warranted.
