Abstract:Objective To evaluate the internal fixation with dual incisions and mini plates for calcaneal fractures of Sanders type Ⅲ.Methods We treated 23 patients with closed calcaneal fracture of Sanders type Ⅲ between May 2012 and May 2014.They were 17 men and 6 women,aged from 20 to 62 years (average,39.8 years).All of them underwent internal fixation with dual incisions and mini plates.X-ray examinations and CT scans were performed before and after surgery and at the final follow-up to evaluate the height,length and width of the calcaneal joint surface,Bthler angle,Gissane angle and the joint surface step.Complications and American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot score were also recorded.Results Of this series,20 patients completed a follow-up of 18 to 30 months (average,21.5 months).Delayed wound healing of the medial incision occurred in one case,sural nerve impairment in one,and impairment of tibial nerve branch in 2.No superficial or deep wound infection was noted.Loss of reduction happened in only one case due to too early weight bearing.Bone union was achieved after an average of 11.3 weeks (from 10 to 12 weeks).The mean B(o)hler angle (28.3°± 10.1°),Gissane angle (120.4°±4.9°),calcaneal width (40.3 ± 9.8 mm),calcaneal height (44.8 ± 3.8 mm) at the final follow-up were significantly improved compared with the preoperative values (-18.2°± 18.2°,97.1 °± 14.1 °,53.1 ± 10.0 mm,and 31.5 ±6.1 mm,respectively) (P ＜ 0.05).The calcaneal length at the final follow-up (78.6 ± 6.0 mm) was similar to the preoperative value (77.5 ±6.5 mm) (P ＞ 0.05).The joint surface step was significantly improved from the preoperative 5.8 ±3.1 mm to 0.5 ±0.7 mm (t =7.557,P ＜ 0.01).The AOFAS scores averaged 82.9 points (from 45 to 95 points),resulting in 5 excellent cases,9 good ones,5 fair ones and one poor case.Signs of traumatic arthritis were observed in 6 cases.Conclusion Internal fixation with dual incisions and mini plates is an effective treatment for calcaneal fractures of Sanders type Ⅲ,because it leads to satisfactory reduction,stable fixation,and limited soft tissue complications.

Surgical treatment of displaced intraarticular calcaneal fractures by a minimally invasive technique using a locking nail: A preliminary study

Abstract:Background: The aim of this retrospective study was to evaluate the outcome of patients with intra-articular calcaneal fractures treated using a minimally invasive locking nail (Calcanail®). Methods: Between January 2016 and April 2017, 15 patients (9 men and 6 women) with a calcaneal fracture were consecutively treated with Calcanail®. The Böhler angle was recorded on standard X-rays pre- and post-operatively. The articular reduction of the posterior facet was evaluated with the Goldzak index in a CT scan 3 months post-operatively. The mean age of the patients was 53 years (range, 24-78). Mean final follow-up was 18 months (range, 12-24). Results: Six fractures were classified as Sanders II, 7 as Sanders III and 2 as Sanders IV. In 13 out of the 15 patients treated, the post-operative Böhler angle was of more than 20°. Goldzak index was deemed as excellent in 73.5% of the cases (11 patients), good in 20% of cases (3 patients), and poor in 6.5% (1 patient). Post-operative mean AOFAS score was 85 (range, 60-96). Conclusions: The Calcanail® provides good restoration of the subtalar joint and the calcaneal angles with the advantages of a minimally invasive approach. It was effectively used in Sanders types II and III, even in the presence of poor cutaneous conditions.

Abstract:Introduction: Surgical treatment of displaced intra-articular calcaneal fractures using a standard extended lateral approach for plate fixation is frequently associated with substantial wound healing complications and infections. This matched pairs analysis compares the new minimally-invasive calcaneal interlocking nailing technique to standard locking plate fixation in terms of reduction capacity, complication rates, and functional outcomes. Methods: Forty feet in 38 patients (mean patient age 52.5 years, range 27-78) were treated with calcaneal locking nail (LN)(n=20) or locking plate (LP)(n=20) and followed clinically and radiologically for 20 months. Follow-up included radiological assessments and functional evaluations including the American Orthopaedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot Score and the Foot Function Index (R-FFI). Results: Times for surgery were similar for both techniques (LN 93min, range 34 to 175 vs. LP 101min, range 53 to 305). Length of hospital stay was significantly lower in the LN group (7.6 days, range 3 to 14 vs. LP 11 days, range 6 to 37). Time away from work was shorter by 3.5 days in LN group. LP group had superior range of motion in plantarflexion, eversion, and inversion at follow-up, while LP patients were superior in demonstrating a secure heelstrike (79% vs. LN 58%) and fewer needed permanent walking aids. AOFAS and FFI scores revealed no differences for both groups. Radiographs indicated an adequate reconstruction in both groups, followed by a slight loss of reduction at follow-up (LP 4.7° postoperative vs. follow-up; LN 5.1°). The overall complication rate between groups was significant with 5% complications in the LN group vs. 50% in the LP group (5% deep infection, 35% impaired wound healing, 5% hardware breakage, 5% superficial wound infection; LN: 5% superficial wound infection). Conclusions: The new calcaneal interlocking nail shows promising results in terms of reduction capacity and safety. Use of the minimally invasive locking nail technique minimized the risk of postoperative wound complications and achieved superior functional outcomes compared with standard locking plate fixation via the extended lateral approach.

Calcaneous interlocking nail treatment for calcaneous fracture: a multiple center retrospective study

Abstract:Background: Minimally invasive treatments for calcaneous fractures have the same outcomes and fewer complications. However, they are technically demanding, and there are a lack reduction tools. To overcome these problems, a calcaneous interlocking nail system was developed that can make reduction and fixation minimally invasive and effective. We retrospectively studied the calcaneous fracture variables intraoperatively and followed up to evaluate the outcomes of patients treated with the calcaneous interlocking nail system. Methods: All patients in 7 institutions between October 2020 and May 2021 who had calcaneous fractures treated with calcaneous interlocking nails were retrospectively analyzed. The patient characteristics, including age, sex, injury mechanism, Sanders type classification, smoking status, and diabetes were recorded. The calcaneous interlocking nail and standard surgical technique were introduced. The intraoperative variables, including days waiting for surgery, surgery time, blood loss, incision length, and fluoroscopy time, were recorded. The outcomes of complications, AOFAS scores and VAS scores were recorded and compared with other similar studies. Results: Fifty-nine patients were involved in this study; 54 were male; 5 were female; and they had an average age of 47.5 ± 9.2 years (range 25-70). 2 of these fractures were Sanders type I, 28 of these fractures were Sanders type II, 27 of these fractures were Sanders type III, and 2 of these were Sanders type IV. The surgery time was 131.9 ± 50.5 (30-240) minutes on average. The blood loss was 36.9 ± 41.1 (1-250) ml. The average incision length was 3.5 ± 1.8 (1-8) cm; 57 were sinus tarsi incisions; and 2 were closed fixations without incisions. The average fluoroscopy time was 12.3 ± 3.6 (10-25) seconds during the surgery. The VAS score of patients on the day after surgery was 2.4 ± 0.7 (1-3). The AOFAS ankle-hindfoot score in patients who had a follow-up of at 12 months was 93.3 ± 3.6(85-99). During the follow-up, all patients' functional outcomes were good. One patient had a superficial infection. The rate of complications of the 59 patients was 1.7% (1/59). Conclusion: The calcaneous interlocking nail system can have satisfactory reduction and fixation in calcaneous fractures, even in Sanders type IV. The outcomes of follow-up showed good function. The calcaneous interlocking nail could be an alternative method for minimally invasive calcaneous fracture fixation.
Locking Nail versus Plate Fixation in Calcaneal Fractures: Brief report on a Retrospective Analysis of Treatment Characteristics and Radiographic Correction Potential

Abstract:PURPOSE OF THE STUDY Minimal and limited access techniques are gaining increasing interest for the treatment of displaced intra-articular calcaneal fractures. The ideal treatment method is however still debated and largely based on individual case factors and surgeon experience. Aim of this study was thus to compare the treatment characteristics and radiographic correction potential of a locking nail system with a sinus tarsi approach to plate fixation via an extended lateral approach. MATERIAL AND METHODS We retrospectively reviewed 39 cases of patients with calcaneal fractures that received primary fracture treatment for displaced intra-articular calcaneal fractures between July 2017 and March 2020. Patient characteristics, time to surgery, time to discharge, OR time and the correction achieved were analyzed and comparative statistics performed. RESULTS In total 19 patients treated with the locking nail and 20 patients treated with plate fixation were analyzed. Patient age and fracture severity according to the Sanders classification were comparable between the groups. Overall surgical time, as well as the achieved reduction was equal between both groups. Time to surgery, as well as time from surgery to discharge was significantly shorter in the locking nail group. 2 additional soft tissue procedures were necessary in the extended lateral approach group. DISCUSSION AND CONCLUSIONS The results with the locking calcaneus nail and sinus tarsi approach suggest, that similar treatment results can be achieved as with plate osteosynthesis and an extended lateral approach. Soft tissue management, as well as pre- and postoperative timing and discharge management can be improved with the nail. Further controlled trials comparing the longterm outcome between the treatment options are needed. Key words: calcaneus fracture, sinus tarsi approach, calcaneal nail, C-Nail.

Abstract:The increased alkaline phosphatase (EC 3.1.3.1) activity of reconstituted lyophilized control sera has recently been the focus of considerable interest because of its possible implications for quality-control procedures. We confirm that these reconstituted materials show a temperature-dependent increase in alkaline phosphatase activity, but could show no alteration in activity of fresh sera. The rate of increase in activity was unaffected by dialysis of the reconstituted material, and occurred more rapidly in dilute solutions. Studies with acrylamide gel electrophoresis and Sephadex G-200 gel filtration showed that, immediately after reconstitution, a high-molecular-weight alkaline phosphatase component predominated; during subsequent spontaneous activation this component decreased, and there was a concomitant increase in a low-molecular-weight alkaline phosphatase component. The results obtained when the reconstituted material was extracted with butanol suggest that the observed changes in alkaline phosphatase activity may be attributed to the breakdown of a complex between alkaline phosphatase and lipoprotein.

Abstract:Names and name-binding are useful concepts in the theory and practice of formal systems. In this thesis we study them in the context of dependent type theory. We propose a novel dependent type theory with primitives for the explicit handling of names. As the main application, we consider programming and reasoning with abstract syntax involving variable binders. Gabbay and Pitts have shown that Fraenkel Mostowski (FM) set theory models a notion of name using which informal work with names and binding can be made precise. They have given a number of useful constructions for working with names and binding, such as a syntax-independent notion of freshness that formalises when two objects do not share names, and a freshness quantifier that simplifies working with names and binding. Related to FM set theory, a number of systems for working with names have been given, among them are the first-order Nominal Logic, the higher-order logic FM-HOL, the Theory of Contexts as well as the programming language FreshML. In this thesis we study how dependent type theory can be extended with primitives for working with names and binding. Our choice of primitives is different from that in FM set theory. In FM set theory the fundamental primitive for working with names is swapping. All other concepts such as α-equivalence classes and binding are constructed from it. For dependent type theory, however, this approach of constructing everything from swapping is not ideal, since it requires us to make strong assumptions on the type theory. For instance, the construction of α-equivalence classes from swapping appears to require quotient types. Our approach is to treat constructions such as α-equivalence classes and name-binding directly, turning them into primitives of the type theory. To do this, it is convenient to take freshness rather than swapping as the fundamental primitive. Based on the close correspondence between type theories and categories, we approach the design of the dependent type theory by studying the categorical structure underlying FM set theory. We start from a monoidal structure capturing freshness. By analogy with the definition of simple dependent sums Σ and products Π from the cartesian product, we define monoidal dependent sums Σ∗ and products Π∗ from the monoidal structure. For the type of names N, we have an isomorphism ΣN ∼= ΠN generalising the freshness quantifier. We show that this structure includes α-equivalence classes, name binding, unique choice of fresh names as well as the freshness quantifier. In addition to the set theoretic model corresponding to FM set theory, we also give a realizability model of this structure. The semantic structure leads us to a bunched type theory having both a dependent additive context structure and a non-dependent multiplicative context structure. This type theory generalises the simply-typed αλ -calculus of O’Hearn and Pym in the additive direction. It includes novel monoidal products Π∗ and sums Σ∗ as well as hidden-name types H for working with names and binding. We give examples for the use of the type theory for programming and reasoning with abstract syntax involving binders. We show that abstract syntax can be handled both in the style of FM set theory and in the style of Weak Higher Order Abstract Syntax. Moreover, these two styles of working with abstract syntax can be mixed, which has interesting applications such as the derivation of a term for the unique choice of new names.

Abstract:Calcaneal fracture can lead to long-term disability and have a considerable economic effect. Most calcaneal fractures are intraarticular fractures involving the posterior facet of the subtalar joint. Treating displaced intraarticular calcaneal fractures is complicated because of the lack of an optimal treatment option. Internal fixation typically involves screw-and-plate implants, which can be unfavorable owing to the lack of an anatomic design and the intraoperative bending required for the plate to contour to the irregular surface of the calcaneus. We assessed the outcomes of 30 patients treated using innovative, anatomically designed calcaneal locking plates and the perceived advantages for surgeons. Postoperative computed tomography images of the affected feet were obtained, and the functional performance was recorded. The mean average Böhler angle had increased significantly from 16.8° ± 14.9° to 28.5° ± 9.4° (p < .001). The mean average maximal fracture gap and maximal step-off in the posterior facet of the subtalar joint in the coronal computed tomography images also decreased significantly from 2.8 ± 3.7 mm to 0.8 ± 1.3 mm (p < .01) and from 3.3 ± 2.8 mm to 0.8 ± 1.2 mm (p < .001), respectively. The mean average American Orthopaedic Foot and Ankle Ankle-Hindfoot scale score was 93.9 ± 7.1 at the final follow-up visit. In addition, the surgical time was reduced because bending the plate was not required and the quality of reduction could be assessed easily by examining the gap between the cortex and the plate. The results were promising, revealing that the anatomic locking plate can be used effectively in the treatment of displaced intraarticular calcaneal fractures using simple reduction techniques with a potentially shortened operating time.

Abstract:BackgroundIntramedullary calcaneal nailing is used to treat displaced intra-articular calcaneal fractures.The main goal of the study was to assess the reduction of tomography and secondary goals were patient functional scores and complication rates.MethodsThe functional outcome and restoration of the radiographic parameters were evaluated post-operatively, at three months, at one year, and at the last follow-up. The morphology of the posterior facet was evaluated post-operatively, at one year and at the last follow-up by CTs.ResultsTwenty-six patients were included. The mean follow-up was 2.8 years. The mean AOFAS-AHS was 79 at the last follow-up. The mean calcaneal height index and length rose respectively from 0.44 to 0.86 and 83 to 87 mm, and the width decreased from 50 to 46 mm.ConclusionThe radiographic parameters were restored. The AOFAS-AHS was comparable with other series. This study confirms the efficiency of this procedure with lower rate of complications.

Abstract:The purpose of the present study was to examine the clinical results of surgery for intra-articular calcaneal fractures using a calcaneal locking plate and a bone spreader as a reduction tool. Ten patients with intra-articular calcaneal fractures were treated. An extended lateral approach was used. The reduction of the intra-articular fragments of the posterior facet was temporarily held using a bone spreader and Kirschner wires. Internal fixation was achieved with a locking calcaneal plate. Böhler's angle and Preiss' angle were assessed on the day of injury and the day of the final follow-up examination. The step off and gap of the posterior facet were assessed on the day of injury and the first week after surgery. The functional outcome was evaluated using the American Orthopaedic Foot and Ankle Society ankle hindfoot scale score. The mean Böhler's angle ranged from 2.1° ± 11.0° to 30.4° ± 5.0° (p < .0001), the mean Preiss' angle ranged from 23.5° ± 4.5° to 15.5° ± 2.9° (p < .0001), the mean step off ranged from 4.0 ± 1.9 mm to 0.1 ± 0.2 mm (p = .0002), and the mean gap ranged from 2.6 ± 1.0 mm to 1.2 ± 0.6 mm (p = .0035). The mean American Orthopaedic Foot and Ankle Society Ankle-Hindfoot scale score was 89.2 (range 85 to 100) at a mean of 14.3 months after surgery. Our results suggest that a locking calcaneal plate can be used to restore and reduce an intra-articular calcaneal fracture and achieve good clinical results.

Abstract:BackgroundTibiotalocalcaneal arthrodesis is commonly considered a salvage option for a variety of hindfoot disorders involving the ankle and subtalar joint mostly with concomitant deformity. Retrograde interlocking nailing represents one of the biomechanically most stable fixation modes after reduction of hindfoot geometry. The considerable complications, non-union and revision rates at a moderate outcome reported even with modern retrograde nails underscore the idea that the development of an optimized retrograde may be warranted.MethodsA novel circular shaped nail was designed with a perfect fit to the reamed canal and the implant geometry which respects physiologic hindfoot anatomy and alignment. The present clinical series reports the experience and the clinical and radiographic outcomes after application of the implant in the first 30 consecutive patients. Assessments included validated versions of the AOFAS Ankle/Hindfoot Score and the Foot Function Index.ResultsAnatomical alignment was achieved and maintained in 29/30 cases with a single case of later loss of reduction due to a technical error which led to surgical reintervention two months after the index procedure. The overall union rate reached 93%. Two non-unions (one ankle, one subtalar joint) were observed without necessitating further surgery. Three superficial surgical site infections were registered which made a local flap coverage necessary in two patients due to local skin break-down. No deep infection occurred. There were 2 implant removals, one was not related to hindfoot nailing. At the time of follow-up, the AOFAS Ankle/Hindfoot Score was 57 (median) from a maximum of 86 points. The self-assessment via the Foot Function Index improved from preoperative 155 points to 62.5 postoperatively (median values, p < 0.001, Wilcoxon test).ConclusionOn the basis of our results, which saw a high rate of successful fusion and patient satisfaction and where the hindfoot reduction was maintained until definite healing in the vast majority of cases, the novel circular arc nail represents a viable and safe option for tibiotalocalcaneal arthrodesis with a low complication rate.

Abstract:OBJECTIVE This paper reports comprehensive data on psychiatric symptoms and disorders and medical problems of first- and second-degree biological relatives of prepubertal children who have contemplated or attempted suicide. METHOD Standard family study and family history interview techniques were used to obtain information about psychopathology and medical illness in 488 first- and 1,062 second-degree relatives of 25 child psychiatric inpatients who reported suicide attempts, 28 child psychiatric inpatients who contemplated suicide, 16 nonsuicidal child inpatients, and 54 normal children. RESULTS Suicidal behavior in children was associated with suicidal behavior in their families, although no first-degree relatives committed suicide. More first-degree relatives of child suicide attempters, compared to first-degree relatives of normal children, had antisocial personality disorder, assaultive behavior, and substance abuse. Mood disorders in first-degree relatives were not associated with child suicidal behavior. No significant associations were identified for psychopathology of second-degree relatives and child suicidal behavior. CONCLUSIONS The results suggest the importance of evaluating familial psychopathology during assessments of suicidal children. Self-directed and externally directed violence, antisocial personality disorder, and substance abuse of relatives of suicidal children should be studied to elucidate the etiology of youth suicidal behavior.

Short-Term Radiographic Outcomes of Calcaneus Fractures Treated With 2-Incision, Minimally Invasive Approach

Abstract:Background: The treatment of displaced, intra-articular calcaneus fractures is controversial. The extensile lateral approach has been historically preferred because it provides excellent exposure and visualization for fracture reduction. However, soft tissue complications with this approach can lead to poor outcomes for patients. Recently, there has been an interest in the minimally invasive treatment of calcaneus fractures. The purpose of the present study was to determine the radiographic reduction of displaced, intra-articular calcaneus fractures and the rate of complications using a 2-incision, minimally invasive approach. Methods: A dual-incision, minimally invasive approach with plate and screw fixation was utilized for the treatment of 32 patients with displaced, intra-articular calcaneus fractures. Preoperative and postoperative calcaneal measurements were taken to assess fracture reduction. Additionally, a retrospective chart review was performed to assess for complications. Results: The mean preoperative Bohler's angle measurement was 12.9 (range, -5 to 36) degrees and the final postoperative Bohler's angle was 31.7 (range, 16-40) degrees. One patient (3.1%) had postoperative numbness related to the medial incision in the calcaneal branch sensory nerve distribution. Two patients (6.2%) had a wound infection treated with local wound care and oral antibiotics, while 1 patient (3.1%) had a deep infection that required a secondary surgery for irrigation and debridement. Two patients (6.2%) returned to the operating room for removal of symptomatic hardware. Conclusion: Operative fixation of displaced, intra-articular calcaneus fractures treated with a 2-incision, minimally invasive approach resulted in acceptable fracture reduction with a minimal rate of complications.
