[Atropine use for progressive myopia in children and adolescents]

Abstract:The worldwide prevalence of myopia varies within the range of 20-50% among the adult population of Europe and the United States reaching 60-90% in Asian countries. Reduction of pediatric myopia rates is an important task of medicine. From many reported conservative methods for stabilization of myopia, those that involve pharmaceutical measures are worth paying attention to. This review covers publications dated 1964 or later that contain the results of atropine use at different concentrations in children and adolescents with a minimum follow-up period of 5 years. Atropine mechanisms of action and side effects at different concentrations of the drug are also analyzed. The authors point out potential health hazards for patients on atropine therapy. The principal conclusion: low-dose atropine (0.01%) makes a good compromise between potential negative effects and statistically significant slowing of myopia progression proved in numerous studies. It is recommended that children at the age of 8-13 years undergo at least a 2-year course of atropine therapy.

Abstract:The prevalence of myopia has rapidly increased over the last 30 years, with the World Health Organization estimating a worldwide incidence of 23%, projected to increase to 50% by 2050. The myopia epidemic has prompted a reincarnation in efforts to overcome this challenge. The exploration of atropine use in myopia was a result due to a lack of treatment in effect. This study aimed at reviewing the role of atropine in the management of myopia worldwide based on currently available findings. A literature search was conducted using PubMed/MEDLINE and Google Scholar for studies published up to April 2022 inclusive. Articles with high or medium clinical relevance were selected for this review. Multiple studies have demonstrated the relevance and efficacy rates of different concentrations of atropine, despite still insufficiently explained the exact site and mechanism of action of atropine in slowing myopia progression. Currently available findings highlight that topical atropine opened a new page in pharmacotherapy of myopia and have shown a high therapeutic effect on myopia progression in Asian and European child population, irrespective of ethnicity. There is potential for myopia control with fewer side effects using lower concentrations but still exists a room for improvement, underscoring the requirement of modified atropine topical preparations with increased bioavailability, potentially with nanoparticle formulations, to enable the effective management of myopia.

Abstract:Abstract Background The effect and safety of atropine on delaying the progression of myopia has been extensively studied, but its optimal dose is still unclear. Therefore, the purpose of this meta-analysis is to systematically evaluate the safety and effectiveness of atropine in controlling the progression of myopia, and to explore the relationship between the dose of atropine and the effectiveness of controlling the progression of myopia. Methods This work was done through the data searched from PubMed, MEDLINE, EMBASE, and the Cochrane Central Register of Controlled Trials. The Cochrane Handbook was also used to evaluate the quality of the included studies. In addition, a meta-analysis was performed using Revman5.3 software. Results A total of 10 randomized controlled trials (RCTs) were included. Myopia progression was mitigated greater in the atropine treatment group than that in the control group, with MD = − 0.80, 95% CI (− 0.94, − 0.66) during the whole observation period. There was a statistical difference among 0.05, 0.5, and 1.0% atropine ( P = 0.004). In addition, less axial elongation was shown, with MD = − 0.26, 95% CI (− 0.33, − 0.18) during the whole observation period. Conclusion The effectiveness of atropine in controlling the progression of myopia was dose related. A 0.05% atropine was likely to be the optimal dose.

Abstract:Methods of biospecific adsorption chromatography of phospholipase A2 obtained from porcine pancreas and Naja naja oxiana, Vipera ursini renardi, Vespa orientalis venoms were developed. Granulated polyamide with covalently linked phosphatidylethanolamine were used as an affinity adsorbent. Chemical inertness of linked phosphatidylethanolamine to the hydrolytic action of phospholipase A2 and its high affinity for biospecific complexes are shown. Forms of phospholipase A2 different in their affinity for an immobilized substrate was isolated by biospecific adsorption chromatography. The role of hydrophobic and electrostatic interactions in formation of enzyme-ligand complexes was studied.

Abstract:The performance of desorption electrospray ionization (DESI) in the analysis of a group of pharmaceuticals and their glucuronic acid conjugates is reported. The suitability of different sprayer solvents and different surfaces was examined. In the positive ion mode, water/methanol/trifluoroacetic acid performed best, whereas, in the negative ion mode, water/methanol/ammonium hydroxide was found to be the most suitable spray solvent. Of the surfaces investigated, polymethylmethacrylate (PMMA) was found to give the best performance in terms of sensitivity. Spray solution flow rate and the distance of the sprayer tip from the surface were also found to have significant effects on the signal intensity. Analytes with basic groups efficiently formed the corresponding protonated molecules in the positive ion mode, whereas acidic analytes, such as the glucuronic acid conjugates, formed intense signals due to the deprotonated molecules in the negative ion mode. Ionization of neutral compounds was less efficient and in many cases it was achieved through adduct formation with simple anions or cations.
Topical Atropine in the Control of Myopia

Abstract:Atropine has been used for more than a century to arrest myopia progression. Compelling evidence of its protective effect has been reported in well-designed clinical studies, mainly performed during the last two decades. However, its exact mechanism of action has not been determined. Experimental findings have shown that the mechanism is not related to accommodation, as was thought for decades. A review of the published literature revealed a significant amount of evidence supporting its safety and efficacy at a concentration of 1.0%, and at lower concentrations (as low as 0.01%).

Abstract:ABSTRACT DsbA is a periplasmic thiol:disulfide oxidoreductase which contributes to the process of protein folding by catalyzing the formation of disulfide bonds. In this study, we demonstrate that the dsbA gene is required for the expression of the type III secretion system under low-calcium inducing conditions, intracellular survival of P. aeruginosa upon infection of HeLa cells, and twitching motility. The diverse phenotypes of the dsbA mutant are likely due to its defect in the folding of proteins that are involved in various biological processes, such as signal sensing, protein secretion, and defense against host clearing. In light of its effect on various virulence factors, DsbA could be an important target for the control of P. aeruginosa infections.

Abstract:PURPOSE:To conduct a meta-analysis on the effects of atropine in slowing myopia progression and to compare Asian and white children and randomized controlled trials (RCTs) and observational studies. METHODS:Randomized controlled trials and observational studies that assessed the effects of all concentrations of atropine in slowing myopia progression in children were searched from MEDLINE, EMBASE, and the Cochrane Library up to April 2013. Jadad scoring was used to evaluate the quality of RCTs, and the Newcastle-Ottawa Scale was used for observational studies. RESULTS:Four RCTs and seven cohort studies (a kind of observational study) with 1815 children aged 5 to 15 years were included. The children had a baseline refraction of -0.50 to -9.75 diopters (D) and were followed up for 22.0 months (range, 12.0 to 36.5 months). The weighted mean differences in myopia progression in RCTs and cohort studies of Asian children were 0.55 D per year (p < 0.01) and 0.54 D per year (p < 0.001), respectively, and 0.35 D per year (p = 0.01) in cohort studies of white children. Compared with placebo, the risk of fast myopia progression (>1.0 D per year) using atropine was significantly decreased in both RCTs (odds ratio [OR], 0.14; p < 0.01) and cohort studies (OR, 0.08; p < 0.01), and the benefit of slow myopia progression (<0.50 D per year) using atropine was significantly increased in both RCTs (OR, 6.73; p < 0.01) and cohort studies (OR, 22.10; p < 0.01). CONCLUSIONS:Atropine could significantly slow myopia progression in children, with greater effects in Asian than in white children. Randomized controlled trials and cohort studies provided comparable effects.

Abstract:Myopia is the most common ocular disorder worldwide with an increasing prevalence over the past few decades. It is a refractive error associated with excessive growth of the eyeball. Individuals with myopia, especially high myopia, are prone to develop sight-threatening complications. Currently, atropine is the only drug that is used to slow myopia progression in clinical practice. However, there are still areas of uncertainty such as treatment strategy, optimal concentration when considering risk–benefit ratio and active treatment period. Since the prevalence of myopia is much higher in Asian countries, most of the research on myopia control has been conducted in Asia. Data on the efficacy and tolerability to atropine eye drops in the non-Asian population remains limited. In this review, we summarize the results of published clinical trials on the effectiveness and tolerability of atropine eye drops for myopia control in non-Asian regions. The efficacy was evaluated by the mean change in spherical equivalent (SE) or axial length (AL). The tolerability of atropine eye drops was analyzed based on patients complains and adverse events. The results of this review suggest that 0.01% atropine eye drops are effective in non-Asian regions achieving less side effects compared to 0.5% concentration.

Abstract:Background and Objectives: Myopia is the most widespread ocular disorder globally and its prevalence has been increasing over the past decades. Atropine eye drops stand out as the only pharmacological intervention used in clinical practice to control myopia progression. The aim of this study was to explore the effect of 0.01% atropine eye drops on myopia progression. Patients and Methods: Healthy children aged 6-12 years with cycloplegic spherical equivalent (SE) from -0.5 D to -5.0 D and astigmatism ≤1.5 D were included. Myopia progression was assessed by changes in SE and axial length (AL) over 1 year and SE changes 1 year before the study enrollment and during the 1-year follow-up. Adverse events were evaluated based on complaints reported by either parents or the children themselves during follow-up visits. Results: The analysis involved 55 patients in the 0.01% atropine eye drops group and 66 in the control group. After the 1-year follow-up, the change in SE was -0.50 (-2.25-0.50) D in the control group compared to -0.50 (-1.50-0.50) D in the 0.01% atropine group (p = 0.935); AL change was 0.31 (0.18) mm in the control group and 0.29 (0.18) mm in the 0.01% atropine group (p = 0.480). The change in SE was -0.68 (-2.0--0.25) D/year before the study and remained similar -0.50 (-2.25-0.25) D over the 1-year follow-up in the control group (p = 0.111); SE change was reduced from -1.01 (-2.0--0.25) D/year before the study to -0.50 (-1.5-0.5) D over the 1-year follow-up in the 0.01% atropine group (p < 0.001). In the 0.01% atropine group, ten (16.4%) children experienced mild adverse events, including blurred near vision, ocular discomfort, photophobia, dry eyes, and anisocoria. Conclusions: Compared to the control group, the administration of 0.01% atropine eye drops demonstrated no significant effect on changes in SE and AL over a 1-year follow-up. However, children in the 0.01% atropine group initially experienced higher myopia progression, which decreased with treatment over the course of 1 year. Future studies should explore the long-term effects, rebound effects, potential genetic associations, and efficacy of higher doses of atropine in managing myopia progression.

Abstract:Background: We conducted this study to evaluate the efficacy & safety of topical 0.01% atropine eye drops in slowing down the progression of myopia and axial length elongation. Methods: In this study had 50 patients ( 100 eyes) divided in two groups , 25 patients (50 eyes) in interventional groups and 25 patients (50 eyes) in control group. Results: Autorefrectometer (AR) readings change from base line 2.11±0.11D to after 12 month 2.09±0.12D so increases in AR reading was –0.02D in interventional group at one year follow up. AR reading changes from baseline 2.16±0.12D to after 12 month 2.22±0.18D so increases in AR reading was -0.06D in control groups. Subjective refraction changed from base line 2.01±0.06D to after 12 month 2.91±0.11D so increases in Subjective refraction was +0.9D in cases at one year follow up. Subjective refraction changed from base line 2.03±0.05D to after 12 month 3.23±0.18D so increases in subjective refraction were +1.2D in control group at one year follow up. Conclusion: Low-dose (0.01%) topical atropine eye drops treatment was a safe and effective method for slowing down the progression of myopia in children and adolescents because it has growth suppressing influence by acting on extraretinal muscarinic receptors possible in the retinal pigment epithelium, choroid and sclera. Analysis of Clinical Efficacy of Atropine on Juvenile Myopia Prevention and Treatment

Abstract:Objective To explore the effectiveness of atropine for treatment of juvenile myopia,safety and mechanism.Methods From January 2009 to February 2010 in Department of Ophthalmology,Qinzhou City Second People Hospital patients of 8 to 18 years diagnosed as nearsighted were chosen.162 cases(324 eyes)of spherical equivalent degree ≤6.00 D,astigmatism ≤1.00 D were randomly divided into treatment groups and control group,each of 81 cases(162 eyes).Treatment group was given 10 g/L of atropine sulfate eye gel,once a day,3 months as 1 stage,then the administration stopped for 1 month,then the application cycle was repeated,altogether for 4 stages.Visual acuity,refraction,axial length,intraocular pressure were tested and recorded before and after the treatment.Results At the end of treatment,the cure,progress,decline rate and other indicators of the control group were all obviously better than the control group,vision loss of the two groups were 14.81%,56.62% respectively;diopter of the control group increase-1.3D on average,axial length increased 0.81 mm on average;the differences between the groups were statistically significant(P0.05).The intraocular pressure was normal,no lens or retinal damage occurred.Conclusion Long and regular application of atropine on eye can effectively control and reduce the development of myopia,without adverse reactions,and with the advantages of low cost and easy application,which is worth promoting.The mechanism is related to the reduction,regulation,and slowing down the speed of axial growth.

Abstract:Background: To evaluate the long-term efficacy and safety of topical 1% atropine for retarding moderate myopia.Methods: A randomized, controlled study evaluating atropine and placebo in 660 Chinese children. Patients received drops q1month for 24 months, then q2month for 12 months, followed by no drops for 12 months. Spherical equivalent, axial length, intraocular pressure and atropine-related side effects were examined at 6, 12, 24, 36 and 48 months for all children.Results: Spherical equivalent, myopic progression, axial length augmentation, and progression rate were significantly reduced in the atropine group than those in the placebo group (all P<0.05), indicating that 1% atropine effectively retarded myopia. Moreover, myopic rebound and adverse effects of 1% atropine were eliminated by gradual withdrawal and elimination of 1% atropine. Furthermore, pupil size, near visual acuity, and amplitude of accommodation returned to pretreatment levels after withdrawal of atropine.Conclusion: Topical 1% atropine periodically and alternatively in phase I with gradual reduction in phase II and final withdrawal in phase III may effectively improve atropine efficacy, retard moderate myopia, reduce atropine side effects, minimize myopic rebound, and increase compliance of children simultaneously.
