Abstract:Background: In recent years, research data warehouses moved increasingly into the focus of interest of medical research. Nevertheless, there are only a few center-independent infrastructure solutions available. They aim to provide a consolidated view on medical data from various sources such as clinical trials, electronic health records, epidemiological registries or longitudinal cohorts. The i2b2 framework is a well-established solution for such repositories, but it lacks support for importing and integrating clinical data and metadata. Objectives: The goal of this project was to develop a platform for easy integration and administration of data from heterogeneous sources, to provide capabilities for linking them to medical terminologies and to allow for transforming and mapping of data streams for user-specific views. Methods: A suite of three tools has been developed: the i2b2 Wizard for simplifying administration of i2b2, the IDRT Import and Mapping Tool for loading clinical data from various formats like CSV, SQL, CDISC ODM or biobanks and the IDRT i2b2 Web Client Plugin for advanced export options. The Import and Mapping Tool also includes an ontology editor for rearranging and mapping patient data and structures as well as annotating clinical data with medical terminologies, primarily those used in Germany (ICD-10-GM, OPS, ICD-O, etc.). Results: With the three tools functional, new i2b2-based research projects can be created, populated and customized to researcher's needs in a few hours. Amalgamating data and metadata from different databases can be managed easily. With regards to data privacy a pseudonymization service can be plugged in. Using common ontologies and reference terminologies rather than project-specific ones leads to a consistent understanding of the data semantics. Conclusions: i2b2's promise is to enable clinical researchers to devise and test new hypothesis even without a deep knowledge in statistical programing. The approach presented here has been tested in a number of scenarios with millions of observations and tens of thousands of patients. Initially mostly observant, trained researchers were able to construct new analyses on their own. Early feedback indicates that timely and extensive access to their "own" data is appreciated most, but it is also lowering the barrier for other tasks, for instance checking data quality and completeness (missing data, wrong coding).

Abstract:HL7 Fast Healthcare Information Resources (FHIR) is rapidly becoming the de-facto standard for the exchange of clinical and healthcare related information. Major EHR vendors and healthcare providers are actively developing transformations between existing EHR databases and their corresponding FHIR representation. Many of these organizations are concurrently creating a second set of transformations from the same sources into integrated data repositories (IDRs). Considerable cost savings could be realized and overall quality could be improved were it possible to transformation primary FHIR EHR data directly into an IDR. We developed a FHIR to i2b2 transformation toolkit and evaluated the viability of such an approach.

This framework can accelerate scientific progress by enhancing collaboration and knowledge transfer throughout the intermediate steps of data creation.

Modulation of ovarian LH receptor and serum hormone levels in rats with hyperprolactinemia induced by administration of ovine prolactin or sulpiride.

Abstract:Hyperprolactinemia was experimentally produced in rats by administration of ovine prolactin (oPRL) and sulpiride, and tried to evaluate the effect of hyperprolactinemia on ovarian receptor for luteinizing hormone (LH) as well as that on serum gonadotropin and steroid hormone levels. Wistar-Imamichi strain mature female rats showing 4-day estrous cycles were treated with various doses of oPRL or sulpiride twice a day for 4 days from diestrus. They were killed on the fifth day. Binding of ovarian LH receptors was reduced by a small dose of oPRL (0.1 IU) or sulpiride (0.25 mg) and restored to normal by larger doses of oPRL. However, larger doses of sulpiride (50 or 100 mg) increased the receptor bindings beyond the control level (4.39 +/- 0.40 ng/mg homogenate protein). Serum prolactin levels decreased in rats treated with larger doses of oPRL, but increased with larger doses of sulpiride. Serum LH levels increased with both agents. Although the ovaries treated with either oPRL or sulpiride suggested the lack of ovulation, there were no significant changes of steroid hormones in oPRL groups. In contrast, sulpiride treatment resulted in a reduction of estradiol and an increase of progesterone secretion, suggesting the prolonged effect of the drug. Thus, prolactin appeared to act on the rat ovarian LH receptors in two different manners in hyperprolactinemia, depending on the amount of this hormone or a ratio of prolactin to LH.

Abstract:Plants have a complex chemo-diversity and represent a reservoir of potential new therapeutic agents. Within a Swiss research project, six scientific research groups from different disciplines are collaborating to investigate a collection of more than 17’000 unique dried plant extracts. It aims to find new bioactive molecules and their modes of action, with for example anti-infective or pro-metabolic activities. One of the main challenges of this enterprise is the management, integration and sharing of the highly heterogeneous data that are produced by the different research groups. Among these we find (i) massive high-resolution mass spectrometry data, (ii) the numerical results of innovative chemo-informatics methods, (iii) bioassay results from experimental models of tuberculosis and obesity, and (iv) organic synthetic chemistry. Additionally, requirements for data management plan and open-source science with the FAIR principles must be met. We have established an agile pipeline to capture and structure this heterogeneous data into an RDF graph. The data content's gradual expansion and evolution throughout the project presented considerable challenges, particularly in terms of data modeling. Additionally, despite many collaborators not being RDF experts, most were technically adept at producing RDF triples relevant to their contributions. We have deployed multiple instances of a triplestore and developed an in-house custom tool (i.e. KGSteward) to synchronize their content, based on a configuration file, which is centrally managed and version-controlled using Git. This strategy gave us the flexibility required to address global project challenges in common data management effectively.

Abstract:Background: Modern data-driven medical research provides new insights into the development and course of diseases and enables novel methods of clinical decision support. Clinical and translational data warehouses, such as Informatics for Integrating Biology and the Bedside (i2b2) and tranSMART, are important infrastructure components that provide users with unified access to the large heterogeneous data sets needed to realize this and support use cases such as cohort selection, hypothesis generation, and ad hoc data analysis. Objective: Often, different warehousing platforms are needed to support different use cases and different types of data. Moreover, to achieve an optimal data representation within the target systems, specific domain knowledge is needed when designing data-loading processes. Consequently, informaticians need to work closely with clinicians and researchers in short iterations. This is a challenging task as installing and maintaining warehousing platforms can be complex and time consuming. Furthermore, data loading typically requires significant effort in terms of data preprocessing, cleansing, and restructuring. The platform described in this study aims to address these challenges. Methods: We formulated system requirements to achieve agility in terms of platform management and data loading. The derived system architecture includes a cloud infrastructure with unified management interfaces for multiple warehouse platforms and a data-loading pipeline with a declarative configuration paradigm and meta-loading approach. The latter compiles data and configuration files into forms required by existing loading tools, thereby automating a wide range of data restructuring and cleansing tasks. We demonstrated the fulfillment of the requirements and the originality of our approach by an experimental evaluation and a comparison with previous work. Results: The platform supports both i2b2 and tranSMART with built-in security. Our experiments showed that the loading pipeline accepts input data that cannot be loaded with existing tools without preprocessing. Moreover, it lowered efforts significantly, reducing the size of configuration files required by factors of up to 22 for tranSMART and 1135 for i2b2. The time required to perform the compilation process was roughly equivalent to the time required for actual data loading. Comparison with other tools showed that our solution was the only tool fulfilling all requirements. Conclusions: Our platform significantly reduces the efforts required for managing clinical and translational warehouses and for loading data in various formats and structures, such as complex entity-attribute-value structures often found in laboratory data. Moreover, it facilitates the iterative refinement of data representations in the target platforms, as the required configuration files are very compact. The quantitative measurements presented are consistent with our experiences of significantly reduced efforts for building warehousing platforms in close cooperation with medical researchers. Both the cloud-based hosting infrastructure and the data-loading pipeline are available to the community as open source software with comprehensive documentation.
Offset voltage control scheme for modular multilevel converter operated in nearest level control

Abstract:This paper proposes an offset voltage control scheme for the modular multilevel converter (MMC) operated in nearest level control (NLC) to improve the total harmonic distortion (THD) of AC phase voltages. Through the offset voltage control, the magnitude of each AC pole voltage maintains constant value with N+1 level in the range of whole modulation index (MI). The validity of the proposed scheme was confirmed by computer simulations for the MMC with 22.9kV/25MVA and experimental works for the scaled MMC with 380V/10kVA. It was confirmed that the proposed control scheme can generate constant pole voltages regardless the value of MI, and form phase voltages with improved THD.

Abstract:Biomedical research depends upon increasingly high throughput instruments and sophisticated data analytics. In spite of the significant overhead of handling research data, there is little support for researchers to manage and organize data for purposes of exploration, analysis, and ultimately publication. Shared file systems with metadata coded into directory hierarchies and spreadsheets are the common practice. In this paper, we present a digital asset management approach and system for streamlining data operations and reducing data management overheads for biomedical researchers. It consists of data management tasks including storage, archival, annotation, search, cataloging, publication, and collaboration. We present a preliminary performance evaluation of a key component of the system, and we demonstrate the utility of this approach in a pilot deployment and user study.

Abstract:The National Research Data Infrastructure for Personal Health Data (NFDI4Health) uses Local Data Hubs (LDHs) to manage locally research studies, documents and sensitive personal data to support controlled data sharing. While research data management (RDM) systems facilitate the storage and preparation of data and metadata as well as organizational access, they often lack support for interoperability standards of the application domain. To support the exchange with external registries of research studies, we chose 17 attributes to characterize the most relevant aspects of clinical trials (in the following named "metadata profile"). We implemented the metadata profile in the RDM system FAIRDOM SEEK using core attributes and SEEK's extended metadata feature and created a mapping conforming to the Health Level 7 Fast Healthcare Interoperability Resources (FHIR) standard version R4. Finally, we implemented a prototype application interface for exports in FHIR-JSON format. We plan to extend the interface to serve central registries and support specific FHIR Implementation Guides from various use cases.

Abstract:We report ab intio total energy and electronic structure calculations for benzene physisorbed on graphite and MoS 2 . Our results constitute the first detailed ab initio investigation of the states through which tunneling occurs in the scanning tunneling microscope (STM) through an insulating molecule. At low voltages the density of states near the Fermi energy some way from the surface is dominated by a weak admixture of molecular states into the substrate states; the resulting STM images reflect the details of the molecule-substrate interaction. Abstract:From 2012 to 2015 together with other Linked Data community members and experts from the social, behavioral, and economic sciences (SBE), we developed diverse vocabularies to represent SBE metadata and tabular data in RDF. The DDI-RDF Discovery Vocabulary (DDI-RDF) is designed to support the dissemination, management, and reuse of unit-record data, i.e., data about individuals, households, and businesses, collected in form of responses to studies and archived for research purposes. The RDF Data Cube Vocabulary (QB) is a W3C recommendation for expressing data cubes, i.e. multi-dimensional aggregate data and its metadata. Physical Data Description (PHDD) is a vocabulary to model data in rectangular format, i.e., tabular data. The data could either be represented in records with character-separated values (CSV) or fixed length. The Simple Knowledge Organization System (SKOS) is a vocabulary to build knowledge organization systems such as thesauri, classification schemes, and taxonomies. XKOS is a SKOS extension to describe formal statistical classifications. To ensure high quality of and trust in both metadata and data, their representation in RDF must satisfy certain criteria - specified in terms of RDF constraints. In this paper, we evaluate the data quality of 15,694 data sets (4.26 billion triples) of research data for the social, behavioral, and economic sciences obtained from 33 SPARQL endpoints. We checked 115 constraints on three different and representative SBE vocabularies (DDI-RDF, QB, and SKOS) by means of the RDF Validator, a validation environment which is available at http://purl.org/net/rdfval-demo.

Abstract:BackgroundStandards and technical specifications have been developed to define how the information contained in Electronic Health Records (EHRs) should be structured, semantically described, and communicated. Current trends rely on differentiating the representation of data instances from the definition of clinical information models. The dual model approach, which combines a reference model (RM) and a clinical information model (CIM), sets in practice this software design pattern. The most recent initiative, proposed by HL7, is called Fast Health Interoperability Resources (FHIR). The aim of our study was to investigate the feasibility of applying the FHIR standard to modeling and exposing EHR data of the Georges Pompidou European Hospital (HEGP) integrating biology and the bedside (i2b2) clinical data warehouse (CDW).ResultsWe implemented a FHIR server over i2b2 to expose EHR data in relation with five FHIR resources: DiagnosisReport, MedicationOrder, Patient, Encounter, and Medication. The architecture of the server combines a Data Access Object design pattern and FHIR resource providers, implemented using the Java HAPI FHIR API. Two types of queries were tested: query type #1 requests the server to display DiagnosticReport resources, for which the diagnosis code is equal to a given ICD-10 code. A total of 80 DiagnosticReport resources, corresponding to 36 patients, were displayed. Query type #2, requests the server to display MedicationOrder, for which the FHIR Medication identification code is equal to a given code expressed in a French coding system. A total of 503 MedicationOrder resources, corresponding to 290 patients, were displayed. Results were validated by manually comparing the results of each request to the results displayed by an ad-hoc SQL query.ConclusionWe showed the feasibility of implementing a Java layer over the i2b2 database model to expose data of the CDW as a set of FHIR resources. An important part of this work was the structural and semantic mapping between the i2b2 model and the FHIR RM. To accomplish this, developers must manually browse the specifications of the FHIR standard. Our source code is freely available and can be adapted for use in other i2b2 sites.

Abstract:In the era of translational research, data integration and clinical data warehouses are important enabling technologies for clinical researchers. The OMOP common data model is a wide-spread choice as a target for data integration in medical informatics. It's portability of queries and analyses across different institutions and data are ideal also from the viewpoint of the FAIR principles. Yet, the OMOP CDM lacks a simple and intuitive user interface for untrained users to run simple queries for feasibility analysis. Aim of this study is to provide an algorithm to translate any given i2b2 query to an equivalent query which can then be run on the OMOP CDM database. The provided algorithm is able to convert queries created in the i2b2 webclient to SQL statements which can be executed on a standard OMOP CDM database programmatically.

Abstract:To understand the loss of species, a benchmark is needed, e.g. the status of biodiversity in 1992 when the Convention on Biological Diversity recognized biodiversity crisis to compare to its status in the successive year. Though we are far from knowning how many species there are on planet Earth, we keep track of their descriptions and number through the information kept in our libraries. Each species discovered is represented therein by at least one taxononic treatment. The library includes an estimated 500 million pages and is updated daily with an estimated 17–18,000 new species annually and over 100,000 treatments augmenting the knowledge of existing species. In reality, we do not know how many species exist. We know that the catalogue of life is incomplete and basic knowlege of known species is often lacking and not even updated regularly. On the other hand, the scientific standard to cite previous works and facts is at the same time an ideal prerequisit to build a knowledge graph in the era of digital knowledge. TreatmentBank (TB) and the Biodiversity Literature Repository (BLR), two European Research Infrastructures, provide a service to convert unstructured biodiversity data from scholarly publications into semantically enhanced, digital accessible knowledge (Fawcett et al. 2022) making it findable, accessible, interoperable, reusable (FAIR, Wilkinson et al. 2016) and providing long term access in collaboration with Zenodo. Data sources are either legacy publications or active journals using an XML workflow such as Zookeys or the Biodiversity Data Journal, published by the pioneer of taxonomically enhanced Taxpub XML publishing, Pensoft. As part of the Horizon 2020-funded project Biodiversity Community Integrated Knowledge Library (BiCIKL), bidirectional linking of taxonomic names with the Catalogue of Life, DNA-sequences with the International Nucleotide Sequence Database Collaboration (INSDC) and material citations with their respective natural history collection or their depositions in the Global Biodiversity Information Facility (GBIF) is targeted. As input to allow queries including text and data mining for traits and biotic interactions, the treatments are uploaded to the Swiss Institute of Bioinformatics (SIB) Library Service (SIBiLS) using TaxPub and schema based on Journal Article Tag Suite (JATS) widely used in life science publishing and PubMed Central, allowing the use of the advanced text and data mining tools in the life science community. Another research infrastructure that ensures an RDF representation and conversion into Linked Open Data (LOD) is OpenBiodiv allowing users to explore DAK as part of growing linked open biodiverstiy and related data. All the data fit for GBIF use is reused by GBIF representing close to 60% of all published data sets, which makes TreatmentBank and BLR, for example, a sole provider of data for approximately 90,000 species not recorded in GBIF by other publishers. The long-term strategy is to build up the momentum to be an infrastructure relevant for global biodiversity conservation and research. Long-term support will be achieved by negotiations with respective agencies and publishers, and through building up a global user community using the biodiversity digital acccessible knowledge.

Abstract:With the ever-growing availability of so-called complex data, especially on the Web, decision-support systems such as data warehouses must store and process data that are not only numerical or symbolic. Warehousing and analyzing such data requires the joint exploitation of metadata and domain-related knowledge, which must thereby be integrated. In this paper, we survey the types of knowledge and metadata that are needed for managing complex data, discuss the issue of knowledge and metadata integration, and propose a CWM-compliant integration solution that we incorporate into an XML complex data warehousing framework we previously designed.

Abstract:A pervasive scheme which normalizes and reuses datasets is needed to consolidate the utilities of heterogeneous datasets in distinct systems to achieve highly integrated information retrieval. A flexible metadata standard was developed for a typical system distribution scenario with an integration scheme which standardizes the original datasets, establishes a gateway for all the metadata, and provides query services through a single entry point. Extra work in the original systems is minimized and users get transparent access to all the original data resources. The scheme has been implemented in a medical data sharing project with excellent results.
Integrating row level security in i2b2: segregation of medical records into data marts without data replication and synchronization

Abstract:Objective: i2b2 offers the possibility to store biomedical data of different projects in subject oriented data marts of the data warehouse, which potentially requires data replication between different projects and also data synchronization in case of data changes. We present an approach that can save this effort and assess its query performance in a case study that reflects real-world scenarios. Material and methods: For data segregation, we used PostgreSQL's row level security (RLS) feature, the unit test framework pgTAP for validation and testing as well as the i2b2 application. No change of the i2b2 code was required. Instead, to leverage orchestration and deployment, we additionally implemented a command line interface (CLI). We evaluated performance using 3 different queries generated by i2b2, which we performed on an enlarged Harvard demo dataset. Results: We introduce the open source Python CLI i2b2rls, which orchestrates and manages security roles to implement data marts so that they do not need to be replicated and synchronized as different i2b2 projects. Our evaluation showed that our approach is on average 3.55 and on median 2.71 times slower compared to classic i2b2 data marts, but has more flexibility and easier setup. Conclusion: The RLS-based approach is particularly useful in a scenario with many projects, where data is constantly updated, user and group requirements change frequently or complex user authorization requirements have to be defined. The approach applies to both the i2b2 interface and direct database access.
