Corneal manifestations in bariatric surgery-related hypovitaminosis

Abstract:We read with interest the article titled "Incidence of ocular pathology following bariatric surgery for with morbid obesity across a large United States National Database" by Russell et al. [1], reporting the association between bariatric surgery (BSx) and reduced risk of ocular morbidity and mortality post-surgery. However, it is worth highlighting that BSx can also be indirectly associated with potentially sight-threatening ocular manifestations. Indeed, nowadays in developed countries, BSx represents one of the main causes of hypovitaminosis and, in particular, of vitamin A deficiency (VAD) [2]. The latter can be associated with various ocular manifestations, such as nyctalopia, ocular surface alterations (including corneal ulcerations), and retinopathy [2, 3]. In these cases, an integrative holistic approach is crucial for differential diagnosis and patient's management. We report of a 62-year-old woman, affected by retinitis pigmentosa (RP), presenting with 3-year nyctalopia and recent onset of bilateral blurry vision and photophobia due to a perforated corneal ulcer (3.2 × 3.0 mm) with athalamia and active intraocular inflammation in the right eye, and corneal ulcer (2.5 × 2.7 mm) in the left. The patient had a history of BSx in 2003 and hospitalization for acute pancreatitis, hypocalcaemia, anaemia, and vascular encephalopathy in 2022. Based on the clinical findings and the self-discontinuation of multivitamin supplementation, patient was investigated for nutrient deficiencies and autoimmune diseases. Blood tests showed deficiencies of vitamins A, D, D3 and E. Patient was treated with tectonic keratoplasty in the right eye, topical antibiotics in the left eye, oral doxycycline, and was restarted on targeted multivitamin supplementation by a nutritionist. From baseline to 10-month follow-up, visual acuity improved from light perception to 20/25 and from 20/32 to 20/20 in the right and left eye, respectively.
