Abstract:Objectives The purpose of this cone beam computed tomography (CBCT) study was to describe facial alveolar bone alterations and gray value (GV) changes around implants in aesthetic anterior maxilla with simultaneous guided bone regeneration (GBR) and to investigate potential influence of factors related to vertical alveolar bone stability, such as particle sizes of the deproteinized bovine bone mineral (DBBM) used. Materials and Methods A retrospective study design was adopted. Four facio-palatal cross-sectional CBCT images of 166 implants from 124 patients who had maxillary anterior implants were obtained. Measurements of the vertical facial alveolar bone level (VFBL), horizontal facial alveolar bone thickness (HFBT), and GV were identified of 1-3 years. Then, correlation coefficients for those parameters were calculated. Finally, linear mixed models were established to investigate potential factors influencing vertical alveolar bone resorption. Results Facial alveolar bone underwent fast bone modeling and remodeling during the first 6 months, with decreases of 1.00 +/- 1.19 and 0.74 +/- 0.86 mm in VFBL and HFBT in implant shoulder, respectively, and there was positive and significant correlation between the alterations in VFBL and HFBT1 (r(s) = .516, p = .000). Linear mixed models identified particle size of DBBM as a critical factor associated with vertical bone resorption (p = .000). The GV gradually increased during the follow-up period. Conclusions Rapid and unavoidable peri-implant bone resorption usually happened during the first 6 months after implant placement. GBR is a predictable treatment for maxillary anterior implants, since GV has an increasing trend. Large bone particles of DBBM help maintain vertical alveolar bone stability.

Abstract:© 2009 The Authors JEADV 2009, 23 , 954–982 Journal compilation © 2009 European Academy of Dermatology and Venereology about our published article: ‘Botulinum toxin type A for the treatment of inverse psoriasis’. We have done botulinurn toxin injections with needles of 5 mm in length with a grade of 30 ° on the cutaneous surface; this made possible a depth changeability of 1–2 mm that we thought insignificant for the local and systemic diffusion. There is no evidence in literature that the intradermic administration is more related to a clinical manifestation of botulinum-like syndrome than the intramuscular administration. The botulinum toxin therapy is a symptomatic therapy, and in the particular case of the psoriatic patients, the hypothesized modalities of the action are the blockage of the proalgogenic neuromediators release and the blockage of the cholinergie glandular activity, as described in the literature. The histological changes after the treatment were not to be foreseen because there weren’t any documented histological changes after the treatment in the patients affected with hyperhidrosis. The erythema area was defined with photographic controls and the subjective symptomatology (pain and itch) was evaluated on a visual analogue scale. We didn’t consider the Psoriasis Area Severity Index because of the limited extension of the lesions, and the score would have been extremely low to be significant for the evaluation of the seriousness of the inverse psoriasis. The patients treated with botulinum toxin for the district hyperhidrosis in extended areas, with the same modalities of the dilution and the dosage, never had a symptomatology similar to that of neuropathies with a damage of the little fibers. We underline that the botulinum toxin action is transitory, and it doesn’t damage the fibers but it reduces the neuromediators release. About the potential toxicity of the botulinum toxin, we did not find in the data of the literature, and of our experience in the observation of more than 4000 treatments of palmoplantar and axillary hyperhidrosis during the last 10 years, any severe adverse effect, cardiovascular, respiratory and neuromuscular alteration and any allergic reaction. It has to be underlined that the patients enlisted in our study were resistant to the traditional therapies and with an important local symptomatology. In these patients, the hypothesis of the application of the botulinum toxin therapy is still experimental; therefore, many studies are necessary to verify this and to compare the results of the traditional therapies.
Radiographic Assessment of Deproteinized Bovine Bone Mineral (DBBM) and Collagen-Stabilized DBBM for Transalveolar Sinus Floor Elevation: A 2-Year Retrospective Cohort Study

Abstract:Background: Theoretically, collagen-stabilized deproteinized bovine bone mineral (DBBM-C) has better operability compared with DBBM. DBBM-C avoids dispersing during the transalveolar sinus floor elevation (TSFE) because of its block shape. Purpose: To evaluate radiological changes of using DBBM-C in TSFE. Materials and Methods: Patients who received TSFE using DBBM (Bio-Oss (R)) or DBBM-C (Bio-Oss (R) collagen) with simultaneous implantation were recruited. Graft bone height apically (aGH), endo-sinus bone gain (ESBG), and crest bone level (CBL) were assessed through panoramic radiograph and cone beam computed tomography (CBCT). Results: A total of 138 patients (138 implants) were retrospectively enrolled. After 2 years of implantation, the incidence of postoperative complications was 4.2% (95% CI: 0.9%-11.7%) and 4.5% (95% CI: 0.9%-12.7%) for DBBM and DBBM-C groups, respectively. Measured in panoramic radiograph, Delta aGH of DBBM-C (1.8 mm, SD: 1.4, 95% CI: 1.2-2.4, P = 0.044) group was significantly higher than that of DBBM (1.2 mm, SD: 1.4, 95% CI: 0.7-1.7) after 24 months. No significant differences for Delta CBL were noted during the entire observation period. Measured through CBCT, ESBG was 5.0 (SD: 1.8, 95% CI: 4.1-5.8) mm in DBBM group and 4.6 (SD: 1.6, 95% CI: 3.9-5.3) mm in DBBM-C group 24 months after implantation. The aGH value of DBBM-C group was significantly higher compared with DBBM in CBCT (OR = 1.4, 95% CI: 1.1-1.9, P = 0.020). Conclusions: DBBM-C could achieve similar bone generation as DBBM in TSFE. Both materials could maintain aGH, ESBG, and CBL relatively stable 2 years after implantation.
Combined use of xenogeneic bone substitute material covered with a native bilayer collagen membrane for alveolar ridge preservation: A randomized controlled clinical trial.

Abstract:AimThe aim of this split-mouth randomized controlled study was to evaluate radiographic dimensional changes after tooth extraction in posterior sites treated with a ridge preservation technique or left for spontaneous healing. Materials and MethodsIn a total of 18 patients, tooth extraction in posterior sites of the upper and lower jaw was performed in a split-mouth design. The post-extraction sockets were randomly assigned to the following two treatment modalities: deproteinized bovine bone mineral (DBBM) with 10% collagen (DBBM-C) covered with a native bilayer collagen membrane (NBCM) (test group) and spontaneous healing (control group). Cone beam computed tomography (CBCT) scans were performed after extractions, 3 and 6months later. The following parameters were measured: the height of the buccal bone plate (BH), height of the palatal bone plate (PH), horizontal width of the extraction socket at 1mm, 3mm, and 5mm (HW-1, HW-3, HW-5), and the horizontal width (thickness) of the buccal bone plate at 1mm, 3mm, and 5mm (BHP-1, BHP-3, BHP-5). Statistical analysis was performed applying a nonparametric Wilcoxon signed-rank test. ResultsThe CBCT analysis showed a bone loss compared to baseline in test and control group. The measurements which have reached statistically significant differences at 6months were BH (test: -2.31% vs control: -13.11%), PH (test: -2.07% vs control: -15.32%), HW-1 (test: -17.14% vs control: -32.47%), and HW-3 (test: -11.65% vs control: -28.47%). ConclusionsThe posterior ridge preservation technique using DBBM-C covered with a NBCM is a valid approach reducing the amount of the radiographic loss in alveolar ridge dimensions.

Abstract:After tooth extraction, bone levels in the alveoli decrease. Using a bone substitute can help minimize this bone loss. The substitute can be sourced from a human or animal donor or synthetically prepared. In this study, we aimed to address the following PICOS question: In patients needing dental alveolar preservation for implant placement, how does alveolar preservation using a bovine hydroxyapatite bone xenograft with collagen compare to a xenograft without collagen in terms of changes in alveolar height and width, bone density, and the characteristics of the bone tissue observed in biopsies taken at 6 months? We evaluated two xenograft-type bone substitutes for preserving post-extraction dental sockets using tomography and microscopy to answer that question. A total of 18 dental alveoli were studied: 11 preserved with a xenograft composed of apatite (InterOss) and 7 with a xenograft composed of apatite–collagen (InterOss Collagen). Tomographic controls were performed at 1 and 6 months, and microscopic studies were performed on 13 samples. The biopsies were examined with scanning electron microscopy (SEM) and energy-dispersive spectroscopy (EDS). A Multivariate Analysis of Variance (MANOVA) was conducted in the statistical analysis, revealing a significant increase in bone density over time (p = 0.04). Specifically, bone density increased from an average of 526.14 HU at 30 days to 721.96 HU at 60 days in collagen-free samples. However, no statistically significant differences in height or width were found between groups. The MANOVA results indicated that the overall model had a low predictive ability for height, width, and density variables (R-squared values were low), likely due to sample size limitations and the complexity of bone tissue dynamics. On the other hand, FTIR analysis revealed the presence of phosphate groups, carbonates, and amides I, II, and III, indicative of inorganic (hydroxyapatite) and organic (type I collagen) materials in the xenografts. TGA and DSC showed high thermal stability, with minimal mass loss below 150 °C. Finally, both xenografts were influential in alveolar bone regeneration after extraction without significant differences. The trend of increasing collagen density suggests an effect that requires further investigation. However, it is recommended that the sample size be increased to enhance the validity of the results.

Abstract:The aim of this study was to explore the varieties of time and the amount of bone tissue formation involved in three areas (coronal, central, apical) of the extraction socket during the healing of bone remodeling. The third premolars in bilateral mandibular were extracted for 9 mongrel dogs. And these sockets were randomly assigned to three groups, with 6 sockets in each group. Group A was filled with the porous mineralized collagen plug and covered with the bilayer mineralized collagen GBR membrane; group B was filled with porous mineralized collagen plug and covered with Bio-Gide membrane; and group C did not fill any implantation. The bone healing capacity of these materials was assessed by X-ray Micro-Computed Tomography (XDM) analysis in the three extraction areas at 8, 16 weeks postoperatively respectively. Group A and Group B had greater amount of newly formed bone compared with group C (P < 0.01), whereas no statistical difference was discovered between this two groups (P > 0.05). In terms of the bone regeneration degree in different areas of alveolar socket, the coronal portion had undergone a faster healing process of bone regeneration in each group at 8 and 16 weeks postoperatively. Hence, the porous mineralized collagen plug covered with the bilayer mineralized collagen GBR membrane had a remarkably regenerative potential and a similar efficacy with the Bio-Gide membrane in preserving residual bone, and it could acquired more bone mass, especially in the coronal compartment of the extraction socket.
The impact of autologous concentrated growth factors on the alveolar ridge preservation after posterior tooth extraction: A prospective, randomized controlled clinical trial

Abstract:Background: Alveolar ridge preservation can effectively decrease alveolar ridge resorption following tooth extraction, but it can be limited by reducing new bone formation and residual bone graft material. Efforts to develop more efficacious approaches are thus an area of active research. Purpose: To assess the impact of autologous concentrated growth factors (CGF) on alveolar ridge absorption and osteogenesis following posterior tooth extraction. Materials and methods: Fifty patients were randomly assigned to have extraction sockets treated with CGF or no treatment. At 10 days, 1 month, and 3 months postextraction, soft tissue color and texture were examined and evaluated with healing score. Cone-beam computed tomography (CBCT) scans were performed before and 3 months after extraction, while radiographic analyses were used to assess vertical and horizontal bone changes. Bone samples were collected from the extraction sockets during implant placement, and micro-computed tomography (micro-CT) scans and histological analysis were performed to evaluate new bone formation. t-Test or Mann-Whitney U test was used to compare data and the level of statistical significance was set at 0.05 for all analyses. Results: Forty-six patients completed the trial. Sockets in the experimental group exhibited significantly better healing score on Day 10 postextraction relative to the control group, whereas comparable healing was observed in both groups at 1 and 3 months postextraction. Experimental group exhibited reduced vertical bone changes relative to the control (p < 0.05). Significant reductions were observed in ridge width changes at 1 and 2 mm apical to the crest (p < 0.05), although differences at 3 and 5 mm apical to the crest were not significant. Significant differences of bone mineral density (BMD) and microarchitecture of trabecular bone were observed via micro-CT analyses, and the experimental group had better results. Conclusion: CGF application following posterior tooth extraction may reduce vertical and horizontal bone resorption and promote new bone formation.

Abstract:This report presents a case in which autogenous bone grafts were harvested from the maxillary tuberosity for guided bone regeneration and dental implant placement, with long-term follow-ups and assessment at the clinical, tomographic, and histologic levels. Particulate and block autogenous bone grafts were covered with a resorbable collagen membrane. Advanced bone remodeling and good bone quality, enabling dental implant placement, were observed after a short healing time (3 months). The differences in buccal bone plate thickness in the grafted area between the period immediately after implant placement and 4 years thereafter ranged from +0.879 mm to -0.001 mm. The implants osseointegrated uneventfully, and alveolar bone regeneration remained stable with a satisfactory result after 4 years.

Abstract:Objective To compare histologically the percentage of bone formation 12-20 weeks post-ridge augmentation using two different techniques. Background Tooth loss is associated with three-dimensional bone remodeling and ridge atrophy. Ridge preservation procedures can prevent alveolar bone volume loss. Different techniques and materials are used to preserve the alveolar ridge. Materials & Methods Computer-generated randomization software was used to assign two ridge preservation techniques for 11 extraction sites. In group I (type I bovine Achilles tendon collagen plugs with bioactive resorbable calcium apatite crystals (CPCAC) and ingroup II, cortico-cancellous bone chips (CCBC) mix, and an expanded polytetrafluoroethylene (ePTFE) barrier membrane were placed. The histomorphometric studies were performed using a computer-based image analysis system (IMAGE-J 1.4, National Institute of Health, Bethesda, MD, USA) to calculate the pixel area of bone tissue and the remaining bone graft material. The histomorphometric data were analyzed using a Student's t-test to compare the measurements between the two experimental groups. This parametric statistical test was employed to determine if there were any statistically significant differences in the quantitative histological parameters between the groups. Results: The sockets that received CPCAC showed a lower (31.89%) percentage of native bone surface area compared to the CCBC group (43.87%). However, the difference was not statistically significant (P< 0.05). In addition, group CPCAC showed evidence of foreign body reaction. Conclusion The CCBC graft covered with an ePTFE barrier may induce more bone formation with minimal inflammation in an extraction socket compared to a collagen plug with calcium apatite crystals. In addition, histological analysis of the CPCAC graft showed evidence of foreign body reaction, which indicates a negative clinical impact.

Abstract:OBJECTIVES: The purpose of this study was to estimate the volumetric change of augmented autobone harvested from mandibular body cortical bone, using cone-beam computed tomography (CBCT) and three-dimensional reconstruction. In addition, the clinical success of dental implants placed 4 to 6 months after bone grafting was also evaluated.MATERIALS AND METHODS: Ninety-five patients (48 men and 47 women) aged 19 to 72 years were included in this study. A total of 128 graft sites were evaluated. The graft sites were divided into three parts: anterior and both posterior regions of one jaw. All patients included in the study were scheduled for an onlay graft and implantation using a two-stage procedure. The dental implants were inserted 4 to 6 months after the bone graft. Volumetric stability was evaluated by serial CBCT images.RESULTS: No major complications were observed for the donor sites. A total of 128 block bones were used to augment severely resorbed alveolar bone. Only 1 of the 128 bone grafts was resorbed by more than half, and that was due to infection. In total, the average amount of residual grafted bone after resorption at the recipient sites was 74.6%±8.4%.CONCLUSION: Volumetric stability of mandibular body autogenous block grafts is predictable. The procedure is satisfactory for patients who want dental implants regardless of atrophic alveolar bone.
Healing patterns of alveolar bone following ridge preservation procedures

Abstract:Objectives: Examine the histomorphometric bone composition, following alveolar ridge preservation techniques and unassisted socket healing. Materials and methods: Forty-two patients (42) requiring a single rooted tooth extraction were randomly allocated into three groups (n = 14 per group): Group 1: Guided Bone Regeneration (GBR) using deproteinised bovine bone mineral (DBBM) and a porcine collagen membrane; Group 2: Socket Seal (SS) technique using DBBM and a porcine collagen matrix; Group 3: Unassisted socket healing (Control). Trephined bone biopsies were harvested following a 4-month healing period. Forty-two samples underwent Back-Scattered Electrons -Scanning Electron Microscopy (BSE-SEM) imaging, with 15 samples examined using Xray Micro-Tomography (XMT) (n = 6 for each GBR/SS and n = 3 Control). Images were analysed to determine the percentage (%) of connective tissue, new bone formation, residual DBBM particles and direct bone to DBBM particle contact (osseointegration). Results: BSE-SEM analysis demonstrated that new bone formation was higher in the Control (45.89% ± 11.48) compared to both GBR (22.12% ± 12.7/p < .004) and SS (27.62% ± 17.76/p < .005) groups. The connective tissue percentage in GBR (49.72% ± 9), SS (47.81% ± 12.57) and Control (47.81% ± 12.57) groups was similar. GBR (28.17% ± 16.64) and SS (24.37% ± 18.61) groups had similar levels of residual DBBM particles. XMT volumetric analysis indicated a lower level of bone and DBBM particles in all test groups, when matched to the BSE-SEM area measurements. Osseointegration levels (DBBM graft and bone) were recorded at 35.66% (± 9.8) for GBR and 31.18% (± 19.38) for SS. Conclusion: GBR and SS ARP techniques presented with less bone formation when compared to unassisted healing. GBR had more direct contact/osseointegration between the DBBM particles and newly formed bone.

Abstract:INTRODUCTION: The aim of this observational study was to evaluate bone growth at the recipient site of autotransplanted teeth using cone-beam computed tomography (CBCT) images and to compare it to the control tooth.METHODS: Bone growth was measured on CBCT images obtained from the anterior region of the maxilla, which received a single transplant of immature premolars due to early loss of the central incisor. The results were compared to the bone growth of contralateral central incisor that served as control. The sample consisted of 13 autotransplanted teeth and 13 control teeth. It was determined the thickness of the buccal bone of the marginal ridge (TBBMR), the thickness of the palatal bone of the marginal ridge (TPBMR), the height of the maxillary bone (HMB), and the thickness of the maxillary bone at its mid-height (TMB). The measurements were analyzed statistically, adopting a significance level of 5%.RESULTS: The mean interval between the dental autotransplantation surgery and the CBCT scan was 16.2 years. The mean TBBMR, TPBMR, HMB, and TMB were, respectively, 0.85, 0.94, 19.6, and 8.85 mm for autotransplanted teeth and 0.79, 1.09, 18.88, and 8.5 mm for control teeth. There were no significant differences in the bone measurements between autotransplanted and control teeth (p > 0.05).CONCLUSIONS: The growth of maxillary bone in the region that received the autotransplanted tooth was proportional to that of maxillary bone that supports the contralateral tooth. Thus, autogenous transplantation is efficient in maintaining the bone architecture at the recipient site.

A 3D micro-CT assessment of composition and structure of bone tissue after vertical and horizontal alveolar ridge augmentation using CAD/CAM-customized titanium mesh

Abstract:Objectives: To date, no studies have exploited micro-CT in humans to evaluate bone morphology and structure after bone augmentation with CAD/CAM-customized titanium mesh, in mandible and maxilla. The aim of this study was to assess the composition and microstructure of bone biopsy through micro-CT analysis. Materials and methods: Bone augmentation at both maxillary and mandible sites was performed on 30 patients randomly treated with customized mesh, either alone (M-) or covered with resorbable membrane (M+), in both cases filled 50:50 with autogenous bone and xenograft. After 6 months, biopsies were taken and micro-CT was performed on consecutive 1-mm-thick VOIs from coronal to apical side, measuring tissue volumes, trabecular thickness, spacing, and number. Results: In both groups, irrespective of membrane use, bone tissue (M-: 29.76% vs. M+: 30.84%) and residual graft material (M-: 14.87% vs. M+: 13.11%) values were similar. Differences were site-related (maxillary vs. mandibular) with higher percentage of bone tissue and trabecular density of low-mineralized bone and overall bone in the mandible. Conclusions: The composition and structure of bone tissue, as assessed by micro-CT after alveolar ridge augmentation using CAD/CAM-customized titanium meshes, showed similar features regardless of whether a collagen membrane was applied.
Elevated End-Tidal Pco2 During Long-Duration Spaceflight.

Abstract:BACKGROUND Elevated ambient Pco2 in the International Space Station (ISS) has been cited as a potential contributor to the vision impairment intracranial pressure syndrome (VIIP), a significant health risk for astronauts during long-duration space missions. The elevation in ambient Pco2 is rather modest and normal respiratory compensation could minimize the impact on arterial Pco2. METHODS In nine male astronauts, breaths measured prior to a rebreathing maneuver were examined to assess inspired and end-tidal Pco2 during upright seated preflight and in-flight conditions. RESULTS Inspired Pco2 increased from preflight baseline (0.6 ± 0.1 mmHg) to in flight (3.8 ± 0.4 mmHg). End-tidal Pco2 also increased from preflight baseline (36.0 ± 3.2 mmHg) to in flight (42.1 ± 3.7 mmHg). The difference between end-tidal Pco2 comparing in flight to preflight (6.1 ± 1.6 mmHg) was greater than the difference between inspired Pco2 comparing preflight to in flight (3.3 ± 0.5 mmHg). DISCUSSION The greater increase in end-tidal vs. inspired Pco2 might reflect alveolar hypoventilation due to differences in ventilatory control with spaceflight. These data suggest that further studies should focus on arterial Pco2 and acid-base balance to determine if CO2 dilates cerebral and retinal vessels and might contribute to the incidence of VIIP in astronauts. The Assessment of Volumetric Changes for Alveolar Ridge Preservation or Reconstruction by Three-dimensional Analysis at Posterior Extraction Sites with Severe Bone Defects Using DBBM-C Collagen Membrane and PRF: A Prospective and Randomized Clinical Trial

Abstract:STATEMENT OF PROBLEM: Volumetric resorption of the alveolar ridge often occurs following tooth extraction in both horizontal and vertical directions. There is a specific lack of evidence for alveolar ridge reconstruction at molar and premolar sites with severe bone resorption.PURPOSE: This randomized and controlled trial aimed to use three dimensional and linear analyses to evaluate volumetric changes of the alveolar bone following alveolar ridge reconstruction (ARR) at molar and premolar sites with severe bone resorption as compared with non-assisted socket healing be implant placement.MATERIAL AND METHODS: A total of 31 patients (15 males and 16 females) with more than 50% of hard tissue loss in one or more socket walls were recruited and randomized into either a test group (ARR after extraction using deproteinized bovine bone mineral with 10% collagen (DBBM-C) and platelet-rich fibrin (PRF) with a resorbable collagen membrane) or a control group (natural healing after extraction). Then, the clinical, linear, volumetric implant-related and patient-reported outcomes were analyzed after a 4-month healing process.RESULTS: Linear bone assessments revealed significantly greater gains of ridge width in the test group (25% in the mesial, mid-facial and distal aspects) and less reduction of vertical bone ridge than in the control group (P<0.05). Furthermore, volumetric bone remodeling was significantly higher in the test group (ARR=35.1±34.9%, control=14.2±12.8%, P<0.05). Patient-reported discomfort and keratinized mucosal changes were comparable between groups.CONCLUSIONS: Alveolar ridge reconstruction with a combination of DBBM-C, PRF, and a resorbable membrane at posterior sites with severe socket wall deficiency (> 50% bone loss) is a safe and more capable therapeutic method when compared with natural healing and non-assisted sockets.CLINICAL IMPLICATIONS: Collectively, our analyses demonstrated that alveolar ridge reconstruction represents an efficient method to maintain and augment crestal bone at posterior extraction sites with severe bone defects when assessed after four months of healing.
