Abstract:Providers often underestimate the influence of patient religious and spiritual (R&S) needs. The current study sought to determine the influence of R&S beliefs on treatment decision making among patients and providers in the context of cancer care. We conducted a systematic review of the literature using web-based search engines and discipline-specific databases. Search terms included a combination of the following Medical Subject Headings and key terms: "cancer,""spirituality,""religion," and "decision making." We used Covidence to screen relevant studies and extracted data into Microsoft Excel. Among 311 screened studies, 32 met inclusion/exclusion criteria. Most studies evaluated the patient perspective ( n = 29), while 2 studies evaluated the provider perspective and 1 study examined both. In assessing patient R&S relative to treatment decision making, we thematically characterized articles according to decision-making contexts, including general ( n = 11), end-of-life/advance care planning ( n = 13), and other: specific ( n = 8). Specific contexts included, but were not limited to, clinical trial participation ( n = 2) and use of complementary and alternative medicine ( n = 4). Within end-of-life/advance care planning, there was a discrepancy regarding how R&S influenced treatment decision making. The influence of R&S on general treatment decision making was both active and passive, with some patients wanting more direct integration of their R&S beliefs in treatment decision making. In contrast, other patients were less aware of indirect R&S influences. Patient perception of the impact of R&S on treatment decision making varied relative to race/ethnicity, being more pronounced among Black patients. Most articles focused on R&S relative to treatment decision making at the end of life, even though R&S appeared important across the care continuum. To improve patient-centered cancer care, providers need to be more aware of the impact of R&S on treatment decision making.

Abstract:This study aims to explore the influence of religious beliefs on health decision-making among diverse faith communities. The research seeks to understand how religious doctrines, practices, and community dynamics shape health behaviors, medical interventions, mental health approaches, and end-of-life decisions. A qualitative research design was employed, utilizing semi-structured interviews to gather in-depth insights from participants. A purposive sample of 30 individuals representing various religious traditions, including Christianity, Islam, Hinduism, Buddhism, Judaism, and other minor faiths, was selected. Interviews were conducted in person or via video calls, recorded, and transcribed verbatim. Data analysis was performed using NVivo software, following a thematic approach to identify and categorize key themes and subthemes. Theoretical saturation was achieved when no new information emerged from the interviews. The study identified several key themes: the influence of religious beliefs on health, the role of religious communities, and the interplay between religion and medicine. Participants reported that their religious beliefs significantly influenced dietary practices, medical interventions, mental health approaches, and end-of-life decisions. Religious communities provided crucial emotional and financial support, while religious leaders played a significant role in guiding health decisions. The compatibility and conflicts between religious beliefs and medical advice were evident, with participants employing various strategies to negotiate these differences within medical contexts. Religious beliefs profoundly shape health decision-making across diverse faith communities. Healthcare providers must recognize and respect these beliefs to provide culturally competent and holistic care. Training programs that enhance cultural and religious competence, collaborative approaches involving religious leaders, and institutional policies that accommodate religious practices can improve patient satisfaction and health outcomes. Future research should explore these dynamics with larger, more diverse samples and mixed-methods approaches.

Abstract:Background Spiritual care is a key domain of quality palliative care as defined by national palliative care guidelines. Aims The aim of this project was to synthesize data and research experience by the authors over 30 years related to spirituality in patients with cancer. The research objectives for this analysis were to: Describe spirituality in patients with cancer. Determine the outcomes of palliative care interventions on spirituality and related variables in patients with cancer in these studies conducted by the authors. Summarize methodological issues in conducting research related to spirituality in oncology patients. Methods A synthesis of 15 prior studies conducted by the authors was completed and also compared to current literature regarding spirituality in cancer and other serious illness. Results Findings demonstrate the broad scope of spirituality, key spiritual concerns in cancer and methodological approaches to assessing spirituality. Conclusion Spirituality is a major concern for patients, yet limited training has been provided for clinicians who deliver this care. Additional studies are needed to advance this important aspect of palliative care.

Abstract:We review decision making along the cancer continuum in the contemporary context of informed and shared decision making in which patients are encouraged to take a more active role in their health care. We discuss challenges to achieving informed and shared decision making, including cognitive limitations and emotional factors, but argue that understanding the mechanisms of decision making offers hope for improving decision support. Theoretical approaches to decision making that explain cognition, emotion, and their interaction are described, including classical psychophysical approaches, dual-process approaches that focus on conflicts between emotion versus cognition (or reason), and modern integrative approaches such as fuzzy-trace theory. In contrast to the earlier emphasis on rote use of numerical detail, modern approaches emphasize understanding the bottom-line gist of options (which encompasses emotion and other influences on meaning) and retrieving relevant social and moral values to apply to those gist representations. Finally, research on interventions to support better decision making in clinical settings is reviewed, drawing out implications for future research on decision making and cancer.

Abstract:Objective This article investigates the intricate interplay between spirituality, religion, healthcare services, and cultural diversity concerning non-communicable diseases. Methods The authors thoroughly searched scholarly articles in databases like CINAHL Database, PubMed, and MEDLINE. This search utilised English keywords. A review of these articles involved three approaches: a thorough investigation, a content analysis and historical context. Results There is a clear deficiency in cancer patients' access to spiritual and religious healthcare treatments. This is not just a gap but extends to a failure to meet the all-encompassing requirements of individuals grappling with the multifaceted challenges of cancer. The lack of dedicated services deprives patients of crucial support for their emotional and spiritual well-being, impacting the holistic nature of their care. Recognising the profound psychological and spiritual implications of cancer, tailored spiritual and religious services have the potential to provide comfort, encouragement, and a sense of purpose. Conclusions The analysis emphasises the interconnectedness of spiritual and religious healthcare services with cultural diversity. It comprehensively depicts the varied cultural dimensions of delivering these services to individuals battling cancer.
Can a cancer program-sponsored spiritual event meet with acceptance from patients and other attendees?

Abstract:Background: While many cancer patients derive strength from spiritual or religious faith, concern often remains regarding how different patient subgroups and other community members might react to faith-based services when sponsored by a secular health care organization. Methods: "A Sacred Gathering for Those Touched by Cancer" was presented in 2 Catholic and 2 Protestant churches. The service included key themes (surrendering fear, peace, hope, community support, and God's love) reinforced by Scripture, music, ritual, and prayer. Patients, clergy, and staff participated. Questionnaires evaluating attendee characteristics, emotional response to the service, and satisfaction with service components were distributed. Results: Attendees (women: 80%; Catholic: 71%; half older than 50 years) returned 450 questionnaires. Most found the service very (83%) or somewhat (14%) helpful. Multivariate regression of perceptions indicated (1) the opinion that the service was helpful was associated with the perception that the service made the respondent feel hopeful (P < .0001), that respondents found inspirational messages important (P = .058), and that the respondent was a current patient (P = .018) and (2) an angry response reported by respondents was associated with current patient status (P = .0044). Men tended to feel less loved by God (P = .012) and people (P = .034) and less hopeful (P = .057) than women did. Men liked music less (P = .048), liked Scripture and prayers concerning community less (P = .040), and found prayer (P = .0035) less important. However, men felt the gatherings were as helpful as women did. Past patients felt less sadness than did others (P = .0084). Increased perceived helpfulness of the service was associated in a multivariate analysis with current patient status, feeling hopeful as a result of the service, increased appreciation of the service's inspirational message, and the perception that the service was not too long. Conclusions: While almost all attendees found the service somewhat or very helpful, distinct preferences and reactions to the service were noted for gender, patient status, and religious affiliation. This evaluation will help tailor future events to better meet the spiritual needs of cancer patients and their loved ones.

Abstract:This paper aims to discover and discuss the place of spirituality in the meaning-making process for patients with life-threatening diseases like cancer. Does the search for meaning serve a specific role, or is it purely subjective? Finding constructive coping strategies can improve the quality of life. Meaning-making might be an essential coping strategy. Research has confirmed that spirituality is an integral part of meaning-making for many patients. To understand the findings of studies and their relevance for oncology and healthcare in general, definitions of religion, religiosity, and spirituality are presented. The main theoretical framework is shown in theories of the psychology of meaning. The search for meaning is discussed conceptually in psychoanalytic terms alongside developing a supporting super-ego. It aims to clarify the psychological effects and implications of faith, religion, religiosity, and spirituality on individual, interpersonal and social levels. The influence of illness on life and the impact of life on illness have to be considered, as cultural/societal challenges influence change processes. Internalization of a supportive super-ego, adequate self-regulation, and subjective illness perception/ "truth" influence (social) behavior (un)consciously. Therefore, a systems-theoretical view of the ability to accept boundaries should accompany the supporting super-ego in the process of the search for meaning. A bio-psycho-social model integrates all aspects of human life which are involved in dealing with a disease. The interest in the bio-psycho-social model is neither outdated nor new. The model is often supplemented by the factor of spirituality, which has been part of human experience throughout human history.

Abstract:Spirituality is among the resources that many turn to as they deal with a diagnosis of advanced cancer. Researchers have made much progress in exploring and understanding spirituality’s complex and multifaceted role in the midst of metastatic disease. As a result, spirituality is seen as an important aspect of a holistic and respectful approach to clinical care for patients and their loved ones. In this article, we provide a systematic review of the literature related to the interface between spirituality and metastatic cancer. We included articles published from January 2013 to June 2014. Twenty-two articles were reviewed, consisting of clinical intervention trials, association studies, surveys, qualitative studies, and review articles. The articles discussed efforts to improve patients’ spiritual well-being, with relevant measurement scales; the associations of spirituality and end of life treatment practices; and efforts to better understand and meet the spiritual needs of patients and caregivers.

Abstract:The complex process of health has, until recently, been understood devoid of a spiritual component. The present article offers a model of health inclusive of spirituality with implications for the health communication field. Amending the assumptive non-relevance of spirituality to individual health, a growing body of scholarship in various disciplines recognizes the ways in which spirituality connects to overall wellness. As a whole, this literature equates spirituality with seeking, striving, and forward movement. Given the potential for health communication scholars to make significant contributions at the forefront of this research, this article proposes a dynamic model of health inclusive not only of the physical and mental, but of the spiritual as well. Recognizing its centrality to wellbeing, the model locates the spiritual self at the center. Specifically, the spiritual self is described as engaging action, hope, and connection to self, others, and/or the universe.

Abstract:Although religion/spirituality (R/S) is important in its own right for many cancer patients, a large body of research has examined whether R/S is also associated with better physical health outcomes. This literature has been characterized by heterogeneity in sample composition, measures of R/S, and measures of physical health. In an effort to synthesize previous findings, a meta-analysis of the relation between R/S and patient-reported physical health in cancer patients was performed. A search of PubMed, PsycINFO, the Cumulative Index to Nursing and Allied Health Literature, and the Cochrane Library yielded 2073 abstracts, which were independently evaluated by pairs of raters. The meta-analysis was conducted for 497 effect sizes from 101 unique samples encompassing more than 32,000 adult cancer patients. R/S measures were categorized into affective, behavioral, cognitive, and 'other' dimensions. Physical health measures were categorized into physical well-being, functional well-being, and physical symptoms. Average estimated correlations (Fisher z scores) were calculated with generalized estimating equations with robust variance estimation. Overall R/S was associated with overall physical health (z = 0.153, P < .001); this relation was not moderated by sociodemographic or clinical variables. Affective R/S was associated with physical well-being (z = 0.167, P < .001), functional well-being (z = 0.343, P < .001), and physical symptoms (z = 0.282, P < .001). Cognitive R/S was associated with physical well-being (z = 0.079, P < .05) and functional well-being (z = 0.090, P < .01). 'Other' R/S was associated with functional well-being (z = 0.100, P < .05). In conclusion, the results of the current meta-analysis suggest that greater R/S is associated with better patient-reported physical health. These results underscore the importance of attending to patients' religious and spiritual needs as part of comprehensive cancer care.

Abstract:The purpose of this descriptive, cross-sectional, qualitative study was to describe the spiritual needs experienced in living with cancer from the perspective of patients with cancer and family caregivers. The sample included 28 African American and Euro-American patients with cancer and family caregivers receiving care from inpatient and outpatient units at two metropolitan hospitals in the southwestern United States. In-depth, tape-recorded, semistructured interviews were analyzed using the process of data reduction, data display, and verification. Seven categories of identified spiritual needs included needs associated with relating to an Ultimate Other; the need for positivity, hope, and gratitude; the need to give and receive love; the need to review beliefs, the need to have meaning; and needs related to religiosity and preparation for death. Informants responded with varying levels of awareness of personal spiritual needs. Caregivers were observed to have spiritual needs similar to those of patients. The findings of this study will inform nurses as they assess and document spiritual needs.

Abstract:ABSTRACTWe examine how to sensibly integrate spirituality into the care of older adult medical and psychiatric patients from a multi-cultural perspective. First, definitions of spirituality and spiritual integration are provided. Second, we examine the logic that justifies spiritual integration, including research that demonstrates an association between religious/spiritual (R/S) involvement and health in older adults and research that indicates widespread spiritual needs in later life and the consequences of addressing or ignoring them. Third, we describe how and when to integrate spirituality into the care of older adults, i.e. taking a spiritual history to identify spiritual needs and then mobilizing resources to meet those needs. Fourth, we examine the consequences of integrating spirituality on the well-being of patients and on the doctor-patient relationship. Finally, we describe boundaries in addressing R/S issues that clinicians should be cautious about violating. Resources will be provided to assist with all of the above.

Abstract:Journal of Black Psychology, Ahead of Print. The meaning-making model proposes that distress results from discrepancies between one's global meaning system and one's situational appraisals of negative life events. This model suggests a preventative function for religion when religious global beliefs inform situational appraisals in ways that minimize distress. Using consensual qualitative research, we evaluated the initial reactions to a cancer diagnosis of 29 Black Christian women. We examined how the women appraised the cancer diagnosis and how this appraisal was related to reported distress. Our results indicated that lower levels of distress were related to believing that God was faithful, while higher levels of distress were related to beliefs in retributive justice and cancer as a death sentence. Lower distress was further related to spiritual surrender, a religious coping practice that combines entrusting the outcome of one's cancer experience to God and actively pursuing treatment. We discuss spiritual surrender as a collaborative religious coping strategy and highlight its importance in informing culturally sensitive psychological interventions.

Abstract:Introduction: For patients with newly diagnosed hematological cancer spirituality constitutes an important aspect. Spirituality refers to a person's attachment to what inspires and grounds him/her as well as to associated beliefs, values, and existential experiences, be they religious or not. However, these aspects are not systematically and regularly determined and integrated into the care of patients with hematological cancer. Objective: To describe the perception of spirituality among patients with newly diagnosed hematological malignancies and their loved ones. Method: This qualitative descriptive study employed semi-structured individual interviews with patients and their loved ones and the completion of three questionnaires concerning spiritual well-being, quality of life and symptom burden. Results: A total of 10 patients and six family members were included in our study. Three types of spirituality emerged from the interviews: religious, connecting, and non-religious spirituality. These perceptions are part of the backdrop of this disease whose sudden onset requires the mobilization of resources such as family support, coping strategies, resilience, and hope. Discussion: Beyond the physical consequences, hematological malignancies induce profound changes in patients and their loved-ones regarding their values and beliefs. Encouraging moments of self-expression such as addressing notions of spirituality and supporting patients in their personal cultural practices are important supportive measures that nurses can adopt into their practice.

Abstract:The investigation of spiritual/religious factors in health is clearly warranted and clinically relevant. This special section explores the persistent predictive relationship between religious variables and health, and its implications for future research and practice. The section reviews epidemiological evidence linking religiousness to morbidity and mortality, possible biological pathways linking spirituality/religiousness to health, and advances in the assessment of spiritual/religious variables in research and practice. This introduction provides an overview of this field of research and addresses 3 related methodological issues: definitions of terms, approaches to statistical control, and criteria used to judge the level of supporting evidence for specific hypotheses. The study of spirituality and health is a true frontier for psychology and one with high public interest.

Abstract:Relationships between faith communities and international multi-lateral organizations can be complicated. While there is potential for synergy between the two, different values often characterize the approach of each. The history of these relationships is illustrative. This review describes collaboration between the World Health Organization (WHO) and faith-based organizations (FBOs) in the implementation of primary health care, the role of spirituality in health, community responses to the HIV pandemic, and definitions of Quality of Life containing spiritual dimensions. However, important gaps persist in the appreciation and measurement of the contribution of faith communities to health assets on the part of governments and the WHO. FBOs can still draw from the nine points developed in the 1960s as a time-tested viable agenda for current and future operations.
