Abstract:Objective To evaluate the effects of portoenterostomy (Kasai surgery) on living donor liver transplantation (LDLT) for children with biliary atresia (BA). Methods A total of 150 children with BA, who were treated with LDLT in our center from September 2006 to September 2014, were retrospectively analysed. The children were categorized into Kasai group (90 cases, 60%) and non-Kasai (60 cases, 40%) group, based on whether they had previously undergone Kasai procedure pre-LDLT. Clinical data, incidence of complications and accumulated survival rates were compared between two groups. Results The ages of pediatric patients were 4.9-87.0 months. The patient age and height were significantly higher in Kasai group than those of non-Kasai group (P<0.05). The serum bilirubin level was lower before surgery in Kasai group than that of non-Kasai group (P<0.05). There were no significant differences in body weight, pediatric end stage of liver disease (PELD) score, graft to recipient body weight ratio (GRWR), operation time and blood loss between two groups ( P>0.05). Similarly, there were no significant differences in pulmonary infection, acute rejection, portal vein thrombosis, hepatic artery occlusion and biliary complications between the two groups (P>0.05). The overall complication rate of post-LDLT was 61.1%in Kasai group, which was higher than that in non-Kasai group (43.3%,χ2=4.580, P=0.032). Totally, there were 7 cases (4.7%) died on post-LDLT, in which there were 6 cases (4.0%) in Kasai group including 5 cases of multiple organ <br> failure and 1 case of severe pulmonary infection, and 1 case (0.7%) in non-Kasai group, who died of multiple organ failure due to preoperative gastrointestinal bleeding for emergency surgery. There were no serious complications and death in donors. The overall cumulative survival rates were 98.6%, 96.6%, 94.9%and 92.7%in 1 month, 1 year, 3 years and 5 years after LDLT, respectively. And there were no significant differences in survival rates in 1 month, 1 year, 3 years and 5 years between two groups (χ2=1.490, P=0.222) with the rates of 98.9%, 96.5%, 93.8%, 91.3%in Kasai group and 98.3%, 96.6%, 96.4%, 95.5% in non-Kasai group. Conclusion Performing Kasai procedure can acquire satisfied results to pediatric patients with BA pre-LDLT, without increasing the incidence of major complications and mortality post-LDLT. And the accumulated survival rate is not different in pediatric patients received Kasai surgery compared with that in non-Kasai patient. Besides that, Kasai surgery might postpone the time of receiving LDLT, benefit to the growth of children and reduce the jaundice of pre-LDLT.
Laparoscopy-Assisted Versus Open Hepatectomy for Live Liver Donor: Systematic Review and Meta-Analysis

Abstract:Objective . To assess the feasibility, safety, and potential benefits of laparoscopy-assisted living donor hepatectomy (LADH) in comparison with open living donor hepatectomy (ODH) for liver transplantation. Background . LADH is becoming increasingly common for living donor liver transplant around the world. We aim to determine the efficacy of LADH and compare it with ODH. Methods . A systematic search on PubMed, Embase, Cochrane Library, and Web of Science was conducted in May 2017. Results . Nine studies were suitable for this analysis, involving 979 patients. LADH seemed to be associated with increased operation time (WMD = 24.85 min; 95% CI: −3.01~52.78, P=0.08), less intraoperative blood loss (WMD = −59.92 ml; 95% CI: −94.58~−25.27, P=0.0007), similar hospital stays (WMD = −0.47 d; 95% CI: −1.78~0.83, P=0.47), less postoperative complications (RR = 0.70, 95% CI: 0.51~0.94, P=0.02), less analgesic use (SMD = −0.22; 95% CI: −0.44~−0.11, P=0.04), similar transfusion rates (RR = 0.82; 95% CI: 0.24~3.12, P=0.82), and similar graft weights (WMD = 7.31 g; 95% CI: −23.45~38.07, P=0.64). Conclusion . Our results indicate that LADH is a safe and effective technique and, when compared to ODH.

Abstract:We investigated the overall situation of LT for BA in mainland China and analyzed their survival outcomes based on data from CLTR. Between January 1996 and December 2013, 509 liver transplants for BA were performed in mainland China and were included in this study. Patients' median age was 9.6 months (range: 4.8-175.2 months). KP was previously performed in 194 cases (38.1%). Grafts from living donors were used in 380 cases (74.7%). Era I (1996.1-2010.12) and era II (2011.1-2013.12) comprised 151 cases (29.7%) and 358 cases (70.3%), respectively. Twenty-five centers had performed at least one liver transplant for children with BA. Centers from Shanghai (197 cases), Tianjin (143 cases) and Beijing (81 cases) involved 82.7% of the 509 cases. One-and five-yr graft survival rates were 84.7% and 72.6%, respectively. Split grafts, center volume <20, GRWR >= 4.0%, and steroid-free immunosuppression regimen were independent risk factors for graft survival. In conclusion, the dramatic expansion of pediatric liver transplant programs in mainland China has enabled improved survival for those children affected by this devastating disease. However, screening of BA in neonates should be emphasized throughout the country to enhance early referrals for KP.
Risk Factors and Prognostic Effects of Cholangitis after Kasai Procedure in Biliary Atresia Patients: A Retrospective Clinical Study.

Abstract:Purpose: Cholangitis after Kasai procedure has been previously shown to be related to poor prognosis in Biliary Atresia (BA). To investigate the risk factors and clinical outcomes of cholangitis, we did a retrospective study in post-Kasai BA patients. Methods: Two-year follow-up data of 180 type-III BA patients after Kasai procedure in 2016 in our hospital were analyzed, including 119 cholangitis patients (66.11%). Among the cholangitis group, patients were further divided into early vs late cholangitis and single vs recurrent cholangitis groups. Liver pathology, liver function, cholangitis occurrence and frequency, jaundice clearance, and survival rates were examined. Results: Higher gamma-glutamyl transferase level before Kasai is a risk factor for cholangitis (p = 0.0393). Older age and higher liver fibrosis score at Kasai are risk factors for recurrent cholangitis (p < 0.05). Shorter prophylactic intravenous antibiotics usagemay contribute to early cholangitis, which may lead to higher cholangitis frequency (p < 0.0001). Recurrent cholangitis is associated with earlier cholangitis onsets (p < 0.0001). Cholangitis patients have a relatively delayed jaundice clearance, while early and recurrent cholangitis may contribute to lower overall survival. Conclusions: Personalized treatment considering risk factors in individual BA patients is needed to prevent cholangitis, especially early onsets, and to improve postoperative outcomes. Sequential Operation of Kasai Portoenterostomy and Liver Transplantation is Recommended in Biliary Atresia Children for Better Outcomes: A Propensity Score Matching Analysis

Abstract:Abstract Background Kasai portoenterostomy (KPE) is widely accepted for biliary atresia (BA) when diagnosed timely. However, the outcomes for many patients were not satisfactory and required liver transplantation (LT) subsequently. The aim of our study was to determine whether KPE could improve the post-transplant outcomes of BA children. Methods In this retrospective cohort study from 2006 to 2019, 1647 BA children were included and divided into two cohorts: those who underwent primary liver transplantation (pLT) (n = 599) and those who received KPE followed by LT (n = 1048). Propensity score matching (PSM) was conducted in a 1:1 ratio to balance basic characteristics and minimize the selection bias. Results Of 954 recipients after PSM, pLT group (n = 477) showed poor survival rate than KPE + LT group (n = 477) (p < 0.001), with 1-year survival rate of 87.2% and 94.7%, 5-year survival rate of 82.4% and 91.9% respectively. 59.2% of deaths happened within post-operative day 90, especially the first month (41.7%) after LT. pLT group had a significant higher mortality rate than KPE + LT group (15.5% vs 6.1%, p < 0.001). Pulmonary infection and post-LT liver failure were leading causes of death in pLT group compared with KPE + LT group. Conclusions KPE could improve the post-transplant survival for BA children. If diagnosed within 3 months, it is advisable to try KPE before considering LT for BA children.

Abstract:Objective To retrospectively analyze the clinical characteristics,surgical treatment and outcome of biliary atresia(BA)during the last decade Methods From 1999 to 2008,the clinical data of 184 patients with BA were collected.Among them,34 patients gave up further treatment.The basic conditions,clinical manifestations,diagnosis,surgical treatment and outcome of the other 150 patients were retrospectively analyzed.Results The 150 patients with a mean age of 56.02±39.44 days included 86 males and 64 females.Fifty five patients were older than 90 days at the time of diagnose.Common complaints were presented with jaundice longer than 2 weeks,white stools and dark urine. To diagnose their diseases,the patients underwent ultrasonography(137/150,91.33%),hepatobiliary scintigraphy(80/150,53.33%),biochemical liver function tests(150/150,100%)0A)and imaging examination(88/150,58.66%).Kasai procedure was performed on 88 patients(58.66%)with an average age of 81.11±31.01 days.One hundred and sixteen patients(77.33%)were followed up for 0.75 to 8.2 years after operation(mean,23.77±16.4 months).Fifty six patients died,6 (5.17%)underwent liver transplantation and 30 patients(25.86%)had normal quality of life.Twenty four(20.6%)patients were complicated with portal vein hypertension.Conclusions BA is a common cause of cholestatic iaundice in neonates.From the experience at this center,(1)the aetiology of BA remains unknown.The patients with BA start having Acholie(white)stools on an average age of 22.33±20.3 days,which may be explained by the so called two-hit theory.(2)Current diagnostic methods are not specific and sensitive enough,and should be performed combined examinations.(3) In this study.mortality(48.27%)is higher than European BA Registries report.(4)The overall prognosis of BA patients has improved since the era of pediatric liver transplantation.Only 6 cases were performed in our group liver transplantation due to its high cost.
Evaluation of The Long-term Prognosis For Children With Biliary Atresia After Kasai Operation Report of Operating 63 Cases With Their Native Liver.

Abstract:Objective To define the long-term prognosis of children undergoing the Kasai operation for biliary atresia. Methods A retrospective study was carried out in 271 patients operated on between 1968 and 1983 in Bicêtre hospital. Results Twenty years after opreration, 63 (23%) were alive with their native liver. Serum bilirubin was normal in 21 of these patients; 12 also had normal serum transaminase and gammaglutamyltransferase activities; 44 had signs of portal hypertension, 19 had advanced stage bacterial cholangitis and 6 gallstones,66 showed the different degree of liver cirrhosis by liver puncture biopsy. After age 20, 2 patients died of liver failure and 10 underwent liver grafting; 4 cases are awaiting liver transplantation.Twenty-year survival with native liver was significantly better in children with biliary atresia restricted to the hepatic ducts or with cysts at the porta hepatis. Conclusion In the long-term, less than 23 % of infants with biliary atresia treated by Kasai operation may avoid liver transplantation, even those with native liver their various markers also not a satisfactory recovery. Therefore, closely concern for change of hepatic function, as much as possible early detected out complication,which have active significance for improvement of prognose and therapeutic outcome. [
Surgical outcome and etiologic heterogeneity of infants with biliary atresia who received Kasai operation less than 60 days after birth: A retrospective study.

Abstract:This study aimed to analyze the impact of etiologic heterogeneity and operation age on prognosis of infants with biliary atresia (BA) who received Kasai operation prior to 60 days of age.From 2004 to 2010, 158 infants received Kasai operation before turning 60 days old. According to Davenport 2012 classifications, 4 groups of BA were defined: cystic BA, syndrome BA, and associated malformation, cytomegalovirus (CMV)-associated BA, and isolated BA. Native (autologous) liver survival rates and incidence of cholangitis 2 years after operation, as well as jaundice clearance rates 3 months after operation, were recorded.Although infants who received the operation between 51 and 60 days of age had a better jaundice clearance 3 months after operation and lower incidence of cholangitis as compared with those under 40 or between 41 and 50 days of age, there was no significant difference in survival rates. Among types of BA, infants with cystic BA had the best prognosis. In the syndrome BA and associated malformations group, as well as CMV-associated group, infants who received the operation early (<40 days of age) had a worse outcome as compared with those who received the operation between 41 and 50 days or 51 and 60 days of age.Both clinical etiologic heterogeneity and operation age may influence BA prognosis.
Adjuvant steroid treatment following Kasai portoenterostomy and clinical outcomes of biliary atresia patients: an updated meta-analysis

Abstract:BackgroundIt is controversial whether adjuvant steroid treatment should be given to biliary atresia (BA) patients following a Kasai portoenterostomy (KPE). This study aimed to quantitatively and systematically evaluate the effect of adjuvant steroid therapy post-KPE in relation to major clinical outcomes of BA patients.MethodsWe systematically reviewed the literature in PubMed, Embase, the Cochrane Library, China Knowledge Resource Integrated Database, Wanfang Database, Scholarly and Academic Information Navigator and manually searched for relevant papers published before August, 2015. We extracted data on the effects of steroid treatment following KPE on clinical outcome, including jaundice free rate and native liver survival rate at 6 months, 1 or 2 years after KPE. The weighted overall relative risk (RR) and 95% confidence intervals (CIs) were calculated by using a random-effects model.ResultsEight cohort studies and two randomized controlled trials (RCTs) were identified (n=998). Of them, 6 cohort studies and 2 trials investigated the effect of steroid treatment as compared to non-users or placebo (n=566), and 2 cohort studies compared the effects of high-dose to low-dose steroid treatment (n=432). Steroid usage increased the clearance rates of jaundice at 6 months (pooled RR: 1.32; 95% CI: 0.995-1.76; I2=72.6%) and 1 year (pooled RR: 1.35; 95% CI: 1.12-1.61; I2=0.0%), but not 2 years (pooled RR: 0.82; 95% CI: 0.55-1.22; I2=0.0%) after KPE. There was no solid evidence supporting that steroid treatment would improve native liver survival rate at 6 months (pooled RR: 1.02; 95% CI: 0.90-1.15; I2=0.0%), 1 year (pooled RR: 1.10; 95% CI: 0.91-1.34; I2=35.2%) or 2 years (pooled RR: 1.00; 95% CI: 0.73-1.35; I2=57.4%) after KPE.ConclusionsAdjuvant steroid treatment following KPE may improve short-term (≤1 year) clearance rate of jaundice, but no significant effects on long-term (≥2 years) clearance rate of jaundice and native liver survival rate. Studies on doses and duration of steroids, and long-term follow-up studies are warranted.

Abstract:Objective To study the prognostic value of perioperative portal pressure at time of Kasai operation in patients with biliary atresia (BA).Methods All 127 patients with BA,born between 1991 and 1996,who underwent a Kasai operation in Bicetre hospital were studied. During the Kasai operation,the umbilical vein was catheterised,and a portal pressure index (PPI) was measured.During the period of the follow-up,survival rate with native liver (SNL),and that in the patients living with their native liver without significant portal hypertension (SwsPHT) were calculated.Potential prognostic factors were studied by univariate (logrank test) and multivariate (Cox model) analysis.Results The 5- and 10-year SNL in patients with normal PPI at Kasai operation was longer than those in patients with high PPI.Patients with high PPI had much lower 5-year SwsPHT.The type of biliary atresia and PPI at Kasai operation played a more important role than other factors in determining the prognostic.Conclusion [WT5”BZ]PPI can be regarded as an indicator to determine the prognosis of BA patients after Kasai operation.It seems to be better than others to some extent.

Abstract:This study aimed to analyze the impact of age at Kasai operation on the short-term outcome of type III biliary atresia (BA) and to discuss if pathological scoring can be the prediction of effect in the Kasai procedure for the older (≥ 90 days) infant.During the period 2004 through 2010, 452 infants with type III BA were reviewed. The relationship between ages at Kasai operation and jaundice clearance rates and two year native liver survival rates were assessed, retrospectively. Pathological slides were analyzed with a histological scoring system.All of the patients were divided into 3 groups according to their ages at operation (group A: aged 60 days or less (n=146), group B: age between 60 and 90 days (n=222) , and group C: age on or over 90 days (n=84)). The worst outcome of clearance of jaundice was found in group A but not in group C 2 weeks after the operation (P<0.05). Jaundice clearance rates showed no difference among the three groups either at 3-months or 6-months after operation. Moreover, in group C patients, the pathological scores showed no difference between the jaundice clearance group and jaundice persistence group 6 months after surgery. In group C, two year survival rate of patients with native livers was 36.1%.Some patients over 90 days of age at surgery can still benefit from a Kasai procedure. The pathological scoring system does not play a role in predicting jaundice clearance in patients over 90 days of age at surgery.
Incidence of cholangitis after liver transplantation in patients with biliary atresia

Abstract:Objective To explore the incidence of cholangitis in patients with biliary atresia (BA) after liver transplantation (LT).Methods From January 2013 to July 2016,a total of 219 BA children undergoing LT were recruited.They were divided into two groups according to Kasai operation pre-LT.Then those with a history of Kasai operation were divided into two groups according to cholangitis post-LT and types of Roux-en-Y loop respectively.The percentages of cholangitis post-LT were analyzed in different groups.Results Fifty-one (23.29％) patients had an onset of cholangitis post-LT.The incidence of cholangitis post-LT was higher in Kasai group than that in non-Kasai group (30.70％ vs 15.24％,(x2 =7.316,P =0.007).No significant differences existed in the incidences of cholangitis post-LT between cholangitis and non-cholangitis groups (28.57％ vs 32.76％,(x2 =0.235,P =0.628),reused and rebuilt groups (31.43％ vs 30.38％,(x2 =0.013,P =0.911) and old and new loop groups (33.33％ vs 24.14％,(x2 =0.579,P =0.447) Conclusions LT is efficacious for recurrent cholangitis after Kasai operation.Cholangitis post-LT is common and curable for BA children.For BA children after Kasai operation,a sufficient length of Roux-en-Y loop can effectively control the occurrence of cholangitis post-LT.
A morphometric study of hepatic artery branches in biliary atresia

Abstract:Objective To evaluate the wall thickness and inner diameter of arterial branches at portal area of biliary atresia (BA) and seek its clinical correlation.Methods From June 2012 to March 2014,49 BA children undergoing Kasai procedure at our hospital were included.And the characteristics of arterial branches were morphometrically evaluated.And the controls were patients with intrahepatic cholestasis (n =10).The follow-up period was at least 6 months.They were divided into excellent,favorable and unfavorable groups.According to the age at Kasai procedure,they were divided into ≤60 days,60-90 (includ 90) days and ＞90 days groups.The relevance of age during Kasai procedure,short-term prognosis and hepatic arterial branch morphology was analyzed statistically.Results The authors evaluated 182 arterial branches (152 in BA,30 in controls).The arterial wall thickness of portal area was (83.58 ± 28.30) and (72.13 ± 16.59) μm respectively.And the inner diameter of arterial branches was (136.32 ± 52.64) and (113.94 ± 41.94) μm respectively.The arterial wall thickness of excellent,favorable and unfavorable groups was (83.03 ± 29.42),(78.16 ± 25.06) and (87.00 ± 28.61) μm respectively.The inner diameters of arterial branches were (138.92± 52.96),(125.81 ± 45.04) and (138.88 ± 56.00) μm respectively.The arterial wall thickness of ≤60 days,60-90 (includ 90) days and ＞90 days groups was (87.02 ± 23.66),(81.07 ± 29.94) and (83.08 ± 31.60)μm respectively.The inner diameter of arterial branches was (148.27 ± 54.11),(126.13 ± 47.97) and (137.16± 56.07) μm respectively.The arterial wall of BA cases was thicker than that of infants with intrahepatic cholestasis (P =0.038).The branches of hepatic artery were dilated in BA cases (P =0.026).There was a significant positive linear correlation between arterial wall thickness and inner diameter of arterial branches at portal area.Short-term prognosis was not associated with morphological changes of hepatic arterial branches.No statistical difference existed between age during Kasai procedure and changes of arterial branches at portal area.Conclusions In BA,thickening of arterial wall and dilation of arteries may be found in periportal area.And a significant positive linear correlation exists between arterial wall thickness and inner diameter of arterial branches at portal area of BA.

Abstract:Biliary atresia is an important reason for neonatal jaundice, and it is also the primary cause of liver transplantation in children. Porto-jejunal anastomosis (Kasai procedure) is an effective method for treating biliary atresia. In this article, the complications after Kasai procedure and long-term outcome were introduced. It is emphasized that the Kasai procedure is the first choice for biliary atresia patients. In addition, this article analyzes the possible influencing factors for surgical outcome, describes the possible problems regarding the long-term survival of autologous liver after the operation for biliary atresia, and proposes the need for more attention to the comprehensive treatment after Kasai procedure and before liver transplantation.

Abstract:Objective:To explore the prognostic factors of biliary atresia (BA) with multicenter data.Methods:Retrospective analysis was performed for clinical data of 569 BA children undergoing Kasai procedure from June 2015 to June 2017 at 11 children's medical centers in China. Clinical data was retrieved and postoperative follow-up period was 2 years after operation. Gender, operative age, preoperative liver function, cytomegalovirus (CMV) infection (CMV-IgM in sera), postoperative cholangitis occurrence duration and frequency, postoperative liver function, bilirubin and native liver survival (NLS) were recorded. Chisquare or two independent sample t-test was employed for univariate analysis and Logistic regression for multivariate analysis.Results:Among them, there were 257 boys and 312 girls with an average operative age of (70.1±22.9)(9-180) days. There were 89 cases with bilirubin completely cleared(total bilirubin <34.2 μmol/L) within 1 month and 248 cases within 3 months postoperatively. And 216 cases (38.0%) had cholangitis postoperatively, including early cholangitis (n=84) and recurrent cholangitis (n=61). The rates of NLS at 1/3/6-month and 1/2-year postoperatively were 97.4%, 77.5%, 58.7%, 46.4% and 38.5% respectively. No significant difference existed in preoperative liver function parameters (ALT, AST, GGT, TBIL & DBIL) between children with NLS and those without NLS (all P>0.05). According to operative age, they were divided into ≤45 d, 46-60 d, 61-75 d, 76-90 d, 91-105 d, 106-120 d and >120 d groups. In each age group, boy vs girl group, cholangitis vs non-cholangitis group, recurrent cholangitis vs and non-recurrent cholangitis group, negative vs positive group of cytomegalovirus infection, 2-year NLS rates showed no difference of statistical significance (all P>0.05). The 2-year NLS rate postoperatively was significantly lower in early cholangitis group than that in non-early cholangitis group (28.6% vs. 40.2%)( χ2=4.094, P=0.043). There was statistical significance. The 2-year NLS rate at Month 3 post-operation was significantly higher in jaundice clearance group than that in jaundice non-clearance group [65.3%(162/248) vs 29.4%(57/194)] and the difference was statistically significant ( χ2=56.248, P<0.001). Multivariate logistic regression analysis indicated that early cholangitis ( OR=2.094, 95% CI: 1.172-3.739, P=0.013) and clearance of jaundice within 3 months postoperatively ( OR=0.214, 95% CI: 0.142-0.322, P<0.001) were independent influencing factors for 2-year NLS rate of BA. Conclusions:Early cholangitis and clearance of jaundice within 3 months after Kasai procedure are the independent influencing factors of BA. Early clearance of jaundice after Kasai procedure for BA and active prevention of early cholangitis may improve the prognosis.
Surgical Outcomes of Infants Undergoing Kasai Operation under 60 Days with Biliary Atresia

Abstract:Objective To analyze the impact of etiologic heterogeneity and operative age on short-term outcomes of biliary atresia (BA) undergoing Kasai operation under 60 days.Methods During the period of 2004-2010,a total of 476 BA infants were reviewed.According to the preoperative examinations,4 groups could be defined clinically as cystic BA,syndromic with associated malformations,cytomegalovirus (CMV)-associated and isolated.Results Among them,158 infants underwent Kasai operation under 60 days.There were cystic (n =19),syndromic with associated malformations (n =36),CMV-associated (n =35) and isolated (n =68).The best outcomes were obtained in infants undergoing Kasai operation between 51 to 60 days (62.1％,46/74) while those operated under 40 days had the worse prognosis (42.8％,12/28).Infants with cystic BA had better outcomes than other three groups (68.4％,P＜0.05).Furthermore,this group had favorable prognosis in any operative age.Significant differences existed among the groups of syndromic with malformations (55.9％),CMV-associated BA (57.1％) and isolated BA (52.9％,P＞0.05).Syndromic with malformations group,CMVassociated BA group and those with an early operation (＜40 days) had worse outcomes than those operated between 41-50 days and 51-60 days(P =0.041＜0.05,P =0.029＜0.05).However,in isolated group,infants operated under 40 days had the same outcome with those between 41-50 days (50.0 ％ vs 57.1％,P＞0.05),and also had no significance with those from 51-60 days (50.0％ vs 54.8％,P＞0.05).Conclusions To some extent,operative age has impact on outcome.When Kasai operation is performed under 60 days,infants operated from 51-60 days achieve better outcomes.Clinical etiologic heterogeneity may have prognostic significance.

Abstract:Background Open portoenterostomy (OPE) remains the mainstay in treatment of biliary atresia, while during the past several years, the laparoscopic portoenterostomy (LPE) has been widely introduced. However, safety of LPE remains a major concern. Thus, we conducted a systematic review and meta-analysis to review the currently available data comparing LPE and OPE. Design Systematic review and meta-analysis. Data sources Medline, Ovid, Elsevier, Google Scholar Embase, Cohrane library. Study selection Comparative cohort studies. Data extraction Two investigators independently assessed selected studies and extracted the following information: study characteristics, quality, outcomes data, etc. Results Eleven studies about the effectiveness of LPE compared with OPE were performed by meta-analysis. Meta-analysis found no significant difference between the two groups in operative time, hospital stay, intraoperative blood loss, early clearance of jaundice, cholangitis and variceal bleeding. In addition, the rate of 2-year survival with native liver was significantly high in OPE group than in LPE group. Conclusion Laparoscopic portoenterostomy could not replace open portoenterostomy and open Kasai portoenterostomy remains the gold standard in the treatment of biliary atresia.

Abstract:Purpose: To analyze the influence of perioperative complications in the management of biliary atresia (BA).Methods: A retrospective study was performed using a total of 422 BA patients who underwent Kasai portoenterostomy (KPE) in a single institution between February 2016 and May 2017. Data on patients' clinical characteristics, laboratory examinations, perioperative complications, and outcomes were collected. Unpaired two-tailed t-test and χ2 test were employed for the comparison between BA patients with and without perioperative complications. Cox regression analysis was used to screen the risk factors for 2-years NLS in BA, and their influence on the 2-years NLS was analyzed using Kaplan–Meier survival analysis as well as the log-rank test.Results: The incidence of perioperative complications, 6-months jaundice clearance (JC) and 2-years native liver survival (NLS) rate were 60.4, 59.5, and 56.6%, respectively. Patients with perioperative complications had lower serum albumin (ALB) level, but higher aspartate aminotransferase-to-platelet ratio index (APRI) and international normalized ratio (INR) levels when compared with those without perioperative complications (ALB, P < 0.05; APRI, P < 0.01; INR, P < 0.05). Moreover, perioperative complications were correlated with glucocorticoid administration (P = 0.002). Univariate Cox regression analysis showed no relationship between perioperative complications and 2-years NLS (P > 0.05). However, multivariate Cox regression analysis indicated 6-months JC was an independent protective factor for 2-years NLS [P < 0.0001, hazard ratio (HR) = 0.074, 95% confidence interval = 0.05–0.11], and concordance index of this prediction model including age, weight, APRI, glucocorticoid, and 6-months JC was 0.811.Conclusion: Although perioperative complication is common during and after KPE, it had no influence on the prognosis of BA. However, assessment of the serum level of total bilirubin after KPE may serve as an important predictor for the outcome in BA.
Correlation between age of Kasai portoenterostomy and early prognosis for biliary atresia

Abstract:Objective:To explore the correlation between age of Kasai portoenterostomy and early prognosis to provide references for timing of Kasai surgery in biliary atresia (BA) children.Methods:Retrospective review was conducted for clinical data of hospitalized BA children from January 2019 to December 2021.General profiles (gender & operative age) and parameters of liver function, coagulation function and blood ammonia were collected.According to the jaundice clearance at 3 months post-operation, they were assigned into two groups of non-jaundice clearance ( n=47) and jaundice clearance ( n=66). The cut-off value of operative age was calculated.Then two groups were divided according to operative age.The prognostic parameters of Kasai portoenterostomy at 3 months post-operation were compared, including alkaline phosphatase (ALP), alanine aminotransferase (ALT), aspartate aminotransferase (AST), direct bilirubin (DBIL), postoperative DBIL/ preoperative DBIL, gamma-glutamyltransferase (GGT), total bile acid (TBA), total bilirubin (TBIL), platelet count (PLT), aspartate aminotransferase-to-platelet ratio index (APRI) and international normalized ratio (INR). Results:A total of 113 cases were collected, including 49 males and 64 females. The operative age was 59.00 (40.50, 69.00) days. Univariate analysis of the preoperative indexes of the non-jaundice clearance group and the jaundice clearance group showed that the age of operation(64 (52, 79) d vs. 55 (33, 67) d), AST (202.00 (138.00, 332.00) U/L vs.147.50 (100.00, 242.00) U/L), indirect bilirubin (IBIL) ((70.60±20.31)μmol/L vs.(79.96±27.13)μmol/L), total protein(TP) ((55.88±6.59)g/L vs.(53.13±4.71)g/L) between the two groups were statistically significant ( P<0.05). The indexes with P<0.1 in univariate analysis [age of operation, albumin (ALB), ALT, AST, IBIL, TP, activated partial thromboplastin time(APTT)] were included in the multivariate Logistic regression analysis, and the results showed that the age of operation ( OR=0.974, 95% CI: 0.951-0.997, P=0.030) was an independent factor influencing the jnundice clearance at 3 months after Kasai protoentrostomy. According to the cut-off value (46.5d) of the operative age group, the results showed that the levels of ALP(365.00 (316.00, 514.75) U/L vs.492.00 (343.00, 684.00) U/L), ALT ((76.35±81.33) U/L vs.(134.09±97.45)U/L), AST (67.50 (41.50, 99.50) U/L vs.112.00 (79.00, 168.00) U/L), DBIL (2.55 (1.78, 6.95) μmol/L vs. 9.80 (3.60, 36.80) μmol/L), postoperative DBIL/ preoperative DBIL(0.05 (0.03, 0.09) vs.0.12 (0.05, 0.54)), TBA (45.50 (25.93, 92.93) μmol/L vs.106.20 (41.20, 187.50) μmol/L), TBIL (9.65 (6.98, 15.43) μmol/L vs.21.50 (10.40, 66.90) μmol/L) and APRI (0.32 (0.22, 0.48) vs.0.62 (0.41, 0.93)) in the operation age ≤46.5 d group were significantly lower than those in the operation age > 46.5 d group, the difference was statistically significant ( P<0.05). Conclusion:Early prognostic parameters are superior in group with operative age≤46.5 days to group with operative age >46.5 days.And early Kasai portoenterostomy may improve the prognosis of BA.
