Abstract:Background Medication therapy management is a patient-centered process that involves assessment and evaluation of a patient's complete medication regimen by a pharmacist to prevent medication-related morbidity and mortality. Methods This pilot study evaluated the feasibility of rural access to a psychiatric pharmacist for seniors to reduce adverse drug reactions , inappropriate use of antipsychotic medications, and anticholinergic burden in a frail elderly population. Results Rural access to a psychiatric pharmacist for seniors resulted in statistically significant reductions in total medications, psychotropic medications, and anticholinergic burden. Conclusions Ongoing medication therapy management imbedded into primary care practice can reduce the use of potentially inappropriate medications and anticholinergic burden as seen in other practice environments.
