Abstract:Objective \r\nTo study the relationship between the in situ expression of infiltrating T lymphocyte subsets and leukocyte adhesion molecules in psoriatic patients.\r\n\r\n\r\nMethods \r\nImmunohistochemical technique was employed to study the expression of infiltrating T lymphocyte subsets (CD3, CD4, CD8) and leukocyte adhesion molecules (ICAM-1, ELAM-1, VCAM-1) in the lesional and non lesional psoriatic skin.\r\n\r\n\r\nResults \r\nThere was a significant increase of T lymphocyte subsets (CD3, CD4 and CD8) and adhesion molecules (ICAM-1, ELAM-1, VCAM-1) in the lesional skin than those in non lesional skin in psoriasis, and that there is a significant positive correlation between the density of infiltrating T lymphocyte subsets and the intensity of in situ expression of adhesion molecules. In comparison to the materials from normal controls, the upregulation expression of adhesion molecules was observed in non-lesional skin and normal appearance skin after short duration topical treatment with corticosteroid.\r\n\r\n\r\nConclusion \r\nIncreased upregulation expression of adhesion molecules is closely associated with the infiltration of T lymphocytes in lesional skin of psoriasis and may be one of the factors for the recurrence of psoriasis.\r\n\r\n\r\nKey words: \r\nPsoriasis; Cells,endothelium vascular; Cell adhesion molecules; T lymphocytes
Sarcopenia, and Chronic Pain in Patients with Pseudoexfoliation Syndrome

Abstract:Introduction: We aimed to investigate whether the rate of sarcopenia is higher in patients with pseudoexfoliation syndrome and if an association exists between pseudoexfoliation syndrome, sarcopenia parameters, and chronic musculoskeletal pain. Materials and Method: A total of 96 enrolled patients were divided into two equal groups: “pseudoexfoliation syndrome group” and “no pseudoexfoliation syndrome group”. The variables were demographic characteristics, sarcopenia parameters (SARC-F, hand-grip strength, chair-rise test, gait speed), and pain parameters (having any chronic musculoskeletal pain, pain regions, and Visual Analog Scale-pain). Results: Comparison of sarcopenia and pain parameters between the two groups showed that SARC-F (all groups:p<0.001, 65-74 years:p<0,001, 75-84 years:p=0,015), chair rise test (all groups:p<0.001, 65-74 years:p=0,002, 75-84 years:p=0,003), and Visual Analog Scale-pain (all groups:p<0.001, 65-74 years:p=0,007, 75-84 years:p=0,003) scores were statistically significantly higher, while the gait speed (all groups:p<0.001, 65-74 years:p=0,004, 75-84 years:p=0,007) score was significantly lower in “pseudoexfoliation syndrome group” than in “no pseudoexfoliation syndrome group”. 60.4% of patients with pseudoexfoliation syndrome had probable sarcopenia, and 83% had chronic musculoskeletal pain. A comparison of the two groups showed that the rate of sarcopenia (all groups:p<0.001, 65-74 years:p<0,001, 75-84 years: p=0,014) and the rate of having chronic musculoskeletal pain (all groups, 75-84 years: p=0.002) was significantly higher in patients with pseudoexfoliation syndrome. Conclusion: Our study results showed that most patients with pseudoexfoliation syndrome had chronic musculoskeletal pain and probable sarcopenia. Although pseudoexfoliation syndrome and sarcopenia are problems of aging, further research is needed to explain the pathogenetic mechanisms underlying the high rate of sarcopenia and chronic pain in patients with pseudoexfoliation syndrome. Abstract:Objective: To evaluate the relationship of serum Vitamin B12 and serum Homocysteine levels with Pseudo Exfoliation Syndrome. Study Design: Cross-sectional study. Place and Duration of Study: Department of Ophthalmology, Armed Forces Institute of Ophthalmology, Rawalpindi Pakistan, from Jun to Nov 2022. Methodology: We included 55 individuals with unilateral Pseudo Exfoliation Syndrome without glaucoma and 50 healthy individuals as controls. All participants were subjected to the assessment of serum Homocysteine levels by high-performance liquid chromatography and Vitamin B12 levels by chemi-luminescent enzyme immunoassay. Results: Thirty-two (58.18%) individuals out of 55 with unilateral Pseudo Exfoliation Syndrome had increased serum Homocysteine levels (>5umol/L). Of these 32, 25(78.12%) individuals had moderately high serum Homocysteine levels, while 7(21.87%) had severely high serum Homocysteine levels. Twenty-six (47.27%) out of 55 individuals with Pseudo Exfoliation Syndrome had decreased serum Vitamin B12 levels, while the rest 29(52.72%) had normal-range serum Vitamin B12 levels. In comparison, 7(14%) had reduced serum Vitamin B12 levels, while the rest, 43(86%), had normal serum Vitamin B12 levels. Conclusion: Our study showed that raised Hcy levels were present in individuals with PXS without glaucoma and reduced serum Vitamin B12 levels. Assessment of Hcy level in individuals presenting with PXS may be undertaken as it is a known risk factor for vasuclopathies.
Pseudoexfoliation Syndrome and Antidepressant Drug Use

Abstract:Objectives: To investigate the relationship between pseudoexfoliation syndrome (PES) and the use of antidepressant medications. Materials and methods: This population-based, cross-sectional study included 2,017 of 2,356 invited subjects who were randomly selected from the Turkish Statistical Institute database (www.turksat.gov.tr/) as part of an epidemiologic study which specifically aimed to detect the prevalence of PES in the province of Eskişehir. During the examination, a detailed questionnaire was administered to determine physician-diagnosed systemic disease and drug use. Results: Eight of the 2,017 participants in the study were excluded for various reasons (2 with posterior synechiae, 2 with corneal opacity, 1 uncooperative patient, 1 eviscerated patient, 1 with active adenoviral keratoconjunctivitis, and 1 with a history of angle closure). PES was detected in 100 (5%) of the 2,009 patients whose data were analyzed. The percentage of subjects with concurrent antidepressant drug use in the PES and non-PES non-glaucoma groups were 4.1% (n=3) and 1.1% (n=21), respectively. The difference between the two groups was statistically significant (p=0.024). In addition, the use of medications for hypertension (p<0.001) and coronary artery disease (p=0.009) was also higher in the PES group. Conclusion: The higher prevalence of antidepressant drug use in patients with PES may be related to the processes of vascular damage and inflammation common to the pathogenesis of both PES and depression, as well as the high rate of chronic systemic comorbidities in these patients.
Corneal Clarity and Tomographic Patterns in Pseudoexfoliation Syndrome: A Severity-Based Investigation

Abstract:Purpose: The aim of the study was to investigate the corneal and anterior segment characteristics in pseudoexfoliation syndrome (PXS), depending on the severity of the disease. Materials and methods: This cross-sectional study was conducted at a tertiary care hospital, in the ophthalmology department, in Turkey. Pseudoexfoliation syndrome group was classified into three groups. Pseudoexfoliative material on the part of the pupillary border was classified as group 1, on the entire pupillary border was classified as group 2, and pseudoexfoliative material on the entire pupillary border and iris surface was classified as group 3. The control group was composed of age and gender-matched individuals who had undergone a completely normal ophthalmologic examination. Scheimpflug tomography was performed to record tomographical data. Results: The PXS groups 1, 2, and 3, along with the control group, consist of 29, 32, 33, and 42 participants, respectively. There was no significant difference in age-gender distribution between patients and the control group (P>0.05). Group 3 showed significant differences in central corneal thickness compared with the control group (P=0.001). In addition, anterior chamber angle was found to be significantly lower in group 3 than the control group (P<0.001). In corneal densitometry (CD) values, significant increases compared with the control group were observed in groups 2 and 3 across all anterior annuli (P<0.001). In group 1, significant increases observed in the anterior 6-10 mm and 10-12 mm annuli (P<0.001). In the middle 6-10 mm annuluses, only group 3 showed a significant increase compared with the control group (P<0.001). In the posterior 0-2, 2-6, and 6-10 mm annuli, significant increases compared with the control were observed only in groups 2 and 3 (P=0.001, P=0.001, P<0.001, respectively). Conclusions: Corneal densitometry values, even without an increase in intraocular pressure, have a tendency to increase in PXS depending on the severity of the disease. While minimal changes were observed in the middle layer, it was particularly noted that the effects of the disease were more prominent in the peripheral cornea.
Retinal nerve fiber layer and ophthalmic artery blood flow parameters in pseudoexfoliation syndrome and pseudoexfoliative glaucoma

Abstract:Objectives: To investigate the relationship between ophthalmic artery (OA) blood flow parameters and retinal nerve fiber layer (RNFL) thickness in eyes with pseudoexfoliation (PEX). Methods: We compared PEX eyes without glaucoma (group A, n=53) and those with glaucoma (group B, n=18) with control eyes (group C, n=44). Subsequently, eyes in groups A and B were compared. Finally, OA color Doppler imaging measurements were recorded, and peripapillary RNFL analysis was performed. Results: The total RNFL measurements differed significantly among the groups (P=0.012), being thicker in group C than in group A (P=0.010) and significantly different between group B and groups A and C (both P=0.001). The peak systolic velocity (PSV) and end diastolic velocity (EDV) measurements of groups A and B were lower than those of group C (PSV: P=0.001 and P=0.001, respectively; EDV: P=0.001 and P=0.001, respectively). No significant difference was noted in resistive index (RI) measurements (P=0.370). In group B, significant negative correlations were noted between total RNFL and PSV (r=-0.743; P=0.001) and between total RNFL and EDV (r=-0.691; P=0.001), but not between total RNFL and RI measurements (P=0.548). Conclusions: Pseudoexfoliation syndrome (PXS) with or without glaucoma was associated with a decrease in the PSV and EDV values of the OA. An extensive study may be needed to further explore the role of PXS in OA blood flow parameters. Total RNFL thickness values were lower in eyes with PEX than in those without.
Analysis of Epithelial–mesenchymal Transition Markers in Psoriatic Epidermal Keratinocytes

Abstract:Psoriasis is similar to endpoints of epithelial–mesenchymal transition (EMT), a process of epithelial cells transformed into fibroblast-like cells. The molecular epithelial and mesenchymal markers were analysed in psoriatic keratinocytes. No obvious alteration of epithelial markers E-cadherin (E-cad), keratin 10 (K10), K14 and K16 was detected in psoriatic keratinocytes. However, significantly increased expression of Vim, FN, plasminogen activator inhibitor 1 (PAI-1) and Slug was seen. IL-17A and IL-13 at 50 ng ml−1 strongly decreased expression of K10, Vim and FN. TGF-β1 at 50 ng ml−1 promoted the production of N-cad, Vim, FN and PAI-1. Slug was decreased by dexamethasone (Dex), but E-cad was upregulated by Dex. Silencing of ERK partially increased E-cad and K16, but remarkably inhibited K14, FN, Vim, β-catenin, Slug and α5 integrin. Moreover, inhibition of Rho and GSK3 by their inhibitors Y27632 and SB216763, respectively, strongly raised E-cad, β-catenin and Slug. Dex decreased Y27632-mediated increase of β-catenin. Dex at 2.0 µM inhibited SB216763-regulated E-cad, β-catenin and slug. In conclusion, EMT in psoriatic keratinocytes may be defined as an intermediate phenotype of type 2 EMT. ERK, Rho and GSK3 play active roles in the process of EMT in psoriatic keratinocytes.
Assessing systemic inflammatory markers in psoriasis: A retrospective study

Abstract:Background Psoriasis is a chronic inflammatory disease often associated with serious cardiovascular comorbidities. The aim of this study was to investigate the systemic inflammatory burden in psoriasis by examining various inflammatory markers and to assess the relationship between these markers and the severity of the disease. Methods This retrospective study was conducted on medical records of patients who visited the dermatology outpatient clinic between 1 January 2016 and 31 December 2022. The study included patients with psoriasis vulgaris and healthy volunteers. Demographic data, Psoriasis Area and Severity Index score, C‐reactive protein, monocyte‐high‐density lipoprotein cholesterol ratio, neutrophil‐to‐lymphocyte ratio, platelet‐to‐lymphocyte ratio, monocyte‐to‐lymphocyte ratio, systemic immune‐inflammation index, and Systemic Inflammation Response Index were analysed and compared. Results A total of 278 psoriasis patients and 90 healthy volunteers were analysed. Compared to the control group, psoriasis patients showed significantly higher systemic immune‐inflammation index, Systemic Inflammation Response Index, neutrophil‐to‐lymphocyte ratio, monocyte‐high‐density lipoprotein cholesterol ratio, serum C‐reactive protein levels, neutrophil count, monocyte count, body mass index, and waist circumference (p 10), systemic immune‐inflammation index, neutrophil‐to‐lymphocyte ratio, platelet‐to‐lymphocyte ratio, and serum C‐reactive protein levels were significantly higher compared to patients with mild/moderate psoriasis (Psoriasis Area and Severity Index ≤10). In the ROC curve analysis, the optimal cut‐off (AUC, sensitivity, specificity) values for neutrophil‐to‐lymphocyte ratio, systemic immune‐inflammation index, and platelet‐to‐lymphocyte ratio were found to be 2.11 (0.592, 62%, 57%), 552.9 (0.579, 61%, 58%), and 111.9 (0.578, 64%, 46%), respectively. The inflammatory parameters that showed correlation with Psoriasis Area and Severity Index were systemic immune‐inflammation index, Systemic Inflammation Response Index, neutrophil‐to‐lymphocyte ratio, monocyte‐high‐density lipoprotein cholesterol ratio, monocyte‐to‐lymphocyte ratio, and C‐reactive protein. Conclusion The findings of this study suggest that systemic immune‐inflammation index, Systemic Inflammation Response Index, neutrophil‐to‐lymphocyte ratio, monocyte‐high‐density lipoprotein cholesterol ratio, and C‐reactive protein values have the potential to serve as simple and cost‐effective markers for assessing the inflammatory burden in individuals with psoriasis.

Abstract:Objective To explore the variability characteristics of routine blood,serum biochemical and immunological indicators and evaluate the clinical value on diagnosis,classification of disease subtypes and prognostic on psoriasis patient.Methods 292 psoriasis patients(including subtypes of 225 vulgaris,23 erythrodermic,44 arthritis) and 57 healthy controls were enrolled in this study.The routine blood examination,serum biochemical and immunological indicators were detected in 292 cases of psoriasis patients and healthy volunteers and statistical analysis.Results The white bloodcell(WBC),neutrophil(NEUT),eosinophil(EO),platelet(PLT),mean platelet volume(MPV),platelet distribution width(PDW),erythrocyte sedimentation rate(ESR),serum C reactive protein(CRP),uric acid(UA),Triglycerides(TG),Low density lipoprotein(LDL),Apolipoprotein B(ApoB),lipoprotein(a) and Immunoglobulin G(IgG) of the psoriasis patients were significantly higher than that of healthy controls.Meanwhile,the levels of serum high density lipoprotein(HDL),ApolipoproteinA1(ApoA1),calcium(Ca),total protein(TP) and albumin(ALB) of psoriasis patients were obviously lower than that of healthy controls.The WBC,NEUT,monocytes(Mono),PLT,PDW,ESR,CRP,ESR,UA levels of erythrodermic psoriasis group were higher than that of psoriasis vulgaris group,while the red blood cell(RBC),hemoglobin(Hgb),Hematocrit(HCT),serum total cholesterol(TCH),ApoA1,TP and ALB concentrations were significantly lower than that of psoriasis vulgaris group.The PDW,CRP,ESR,serum IgG levels in arthritis psoriasis group were obviously higher and RBC,Hgb,MPV,serum LDL and ALB were significantly lower than that of vulgaris group patients.The NEUT and serum UA were higher and the ESR,serum TP,IgG levels in erythrodermic psoriasis group were lower than that of arthritis psoriasis group.Conclusion The psoriasis patient have obviously abnormal on routine blood,serum biochemical and immunological indicators,the changes of those indicators in different subtypes psoriasis patient have its own features.To understand the abnormalities of these indicators above could be great helpful in the clinical diagnosis,classification,treatment,prognosis and outcome evaluation of the psoriasis.
Evaluation of the effects of environmental factors and eating habits on exfoliation syndrome and glaucoma in a Turkish population

Abstract:European Journal of Ophthalmology, Ahead of Print. PurposeThis study aimed to assess the possible effect of environmental factors and eating habits on patients with the exfoliation syndrome (XFS) and exfoliative glaucoma (XFG) in a Turkish population.MethodsA questionnaire was applied to 1,000 individuals, including 290 patients with XFS, 210 patients with XFG, and 500 age- and sex-matched healthy control participants. Sociodemographic characteristics, home type and warming methods, living and working conditions (indoor–outdoor), dietary habits based on the the guidance of Turkey Dietary Guidelines, (Determination of Nutritional Status Report and Turkey National Nutrition and Health Survey) and using sunglasses were evaluated. The chi-square test, Student t test, and analysis of variance were used for the statistical analysis with SPSS v. 23.0 software.ResultsThe case–control groups were matched during data collection, and age and sex distribution in groups was analyzed; however, no difference was found. The average years and hours spent outdoor were statistically different between case and control groups (P < 0.05). The disease risk among individuals wearing sunglasses was 2.74 times less than that among those who did not wear sunglasses. Individuals born in the city had 1.46 times lower risk. Also, living out of the city until the age of 12 years increased the risk of getting the disease 1.36 times. In addition, while living in an apartment decreased the risk of disease, using a stove increased. The control groups had more healthy eating habits than the case groups.ConclusionThis case–control study showed that time spent outdoors, wearing sunglasses, home type, heating method, and eating habits might be associated with XFS and XFG.

Systemic Vascular Parameters in Ocular Pseudoexfoliation

Abstract:Background Pseudoexfoliation syndrome (PEX) is characterized by a dandruff-like substance in the anterior chamber, composed of various glycoproteins that have an unclear origin. Its deposition is observed on the pupillary margin, lens zonules, and trabecular meshwork. Proteomic studies have identified numerous proteins in the affected individuals, suggesting associations with systemic conditions like heart disease, stroke, and Alzheimer's disease. However, the systemic associations of PEX remain inconclusive, particularly in regions like southern India. Materials and methods A cross-sectional study was conducted on 114 participants. Pseudoexfoliation was graded as mild, moderate, and severe as per standard photographic grading. Systemic examinations included blood pressure measurements, electrocardiography (ECG), and blood investigations for serum lipid profile, fasting and postprandial blood sugar levels, and serum C-reactive protein levels. Small incision cataract surgery was performed for all the patients. Intraoperative complications and postoperative status were recorded. Results Thirty-eight patients (33.3%) had mild PEX, 44 (38.6%) had moderate PEX, and 32 (28.1%) had severe PEX. Hypertension was present in 54 participants (47.4%), diabetes in 21 (18.4%), coronary artery disease in nine (7.9%), and cerebrovascular accidents in three (2.6%). The mean systolic blood pressure was 140.39 mmHg and the mean diastolic blood pressure was 90.37 mmHg. Systolic blood pressure exceeded 140 mmHg in 29 participants (90.6%) with severe PEX, while diastolic blood pressure surpassed 90 mmHg in 26 participants with severe PEX, both with a p-value of 0.001. Mean fasting and postprandial blood sugar levels were 103.80 ± 31.81 mg/dl and 131.72 ± 48.24 mg/dl, respectively. Serum lipid profiles showed mean low-density lipoprotein (LDL), very low-density lipoprotein (VLDL), cholesterol, and triglyceride levels of 103.00 ± 34.49 mg/dl, 29.04 ± 15.51 mg/dl, 172.73 ± 43.34 mg/dl, and 129.33 ± 64.65 mg/dl respectively. Electrocardiographic results indicated that 54 participants (47.37%) had abnormal ECG including rate abnormality in 13.2%, conduction defects in 12.3%, ischemic changes in 10.5%, and structural defects in 11.4%. Eighty-seven percent of patients had non-dilating pupils and iris atrophy, 13.2% had zonular dialysis and intraoperatively, 78% had capsulorhexis extension, 49.12% had difficult nucleus prolapse, and 28.95% had posterior capsular rent. Conclusion This study highlights the significantly elevated parameters of systemic vascular diseases in PEX patients, like elevated blood pressure and more frequent cardiac anomalies, emphasizing the need for comprehensive systemic evaluation and careful preoperative assessment for ocular comorbidities.
Evaluation of Dry Eye Severity and Ocular Surface Inflammation in Patients with Pemphigus and Pemphigoid

Abstract:Purpose: To evaluate ocular surface involvement, tear cytokine levels, and histopathological changes in pemphigus and pemphigoid patients. Methods: A total of 22 patients (15 pemphigus and 7 pemphigoids) and 21 non-diseased controls were enrolled in our study. All participants underwent ocular surface evaluation, which included ocular surface disease index test, slit lamp observation, dry eye-related examination, tear multicytokine analysis, and conjunctival impression cytology. Results: Pemphigus and pemphigoid patients presented much more severe conjunctivochalasis, corneal epithelial defects, corneal opacity, symblepharon and dry eye. Severe ocular surface squamous metaplasia and a significant increase of tear macrophage inflammatory protein-1beta, tumor necrosis factor-alpha, interleukin (IL)-1β, IL -6, and IL-8 occurred in pemphigus and pemphigoid patients. Conclusions: Our results revealed that ocular surface inflammation and dry eye persist in most pemphigus and pemphigoid patients, and do not occur in parallel with the systemic course. Regular ophthalmological examinations and local anti-inflammatory should be provided for pemphigus and pemphigoid patients.
Comprehensive analysis of pseudoxanthoma elasticum: epidemiological, genetic, and clinical findings from the leading Italian center

Abstract:Background: Pseudoxanthoma elasticum (PXE) is a rare genetic autosomal recessive metabolic disease characterized by progressive mineralization and fragmentation of elastic fibers from soft connective tissues. The objective of our study was to analyze the epidemiological, genetic, cutaneous, and extracutaneous clinical data from the largest Italian monocentric cohort of PXE patients. Methods: We included all patients diagnosed with PXE and referred to Neurocutaneous Rare Diseases at Umberto I Polyclinic Hospital (Rome, Italy) between January 1983 and February 2024. A retrospective analysis of their data was performed. Results: We enrolled 86 patients (77.9% women), revealing compound heterozygosity in 19.8% of cases and homozygosity in 5.8%. Missense (34.9%), non-sense (5.8%), splice-site (5.8%), deletion (4.7%), and frameshift (2.3%) mutations were disclosed. Cutaneous alterations were noted in the neck (69.7%), axilla (33.7%), inguinal (17.5%), and cubital folds (11.7%). The most common ocular findings were angioid streaks (64.0%) and choroidal neovascularization (18.6%), with blindness reported in 5.8% of cases. Thicker intima-media was observed around the mid-fifties in the supra-aortic trunks (40.7%), lower limb arteries (32.6%), and renal arteries (4.7%). Regurgitation was more common in atrioventricular valves (48.8%) than in semilunar ones (10.5% and 9.3%). Dyslipidemia (19.8%), hypertension (18.8%), and fatty liver disease (12.8%) were prevalent, with calcifications found in the kidneys (25.6%), liver (15.1%), spleen (11.6%), and testicles (8.1% of males). Autoimmune diseases and depression were observed in 11.6% and 4.7% of cases, respectively. Conclusions: Enhanced understanding of PXE can improve patients' quality of life and facilitate the development of more effective therapeutic strategies.

Abstract:Objective: Pseudoexfoliation syndrome (PEX) occurs as a result of aggregation of extracellular matrix material in ocular tissues and visceral organs and it is known as a systemic disease. The purpose of our study is to investigate the activities of carbonic anhydrase (CA), glutathione reductase (GR), aldose reductase (AR), paraoxonase (PON) enzymes in aqueous and serum samples, also serum selenium levels. Methods: Serum and aqueous samples from 33 PEX and 33 control subjects were collected. Enzyme activities were studied in both serum and aqueous samples by biochemical methods. Results: According to the results, serum CA hydratase enzyme activity levels were higher in the samples with PEX compared to the control samples (p 0.05). Conclusions: Based on the relationship of carbonic anhydrase with aqueous production and glaucoma, increased CA hydratase activity may help to understand the etiopathogenesis and mechanism of conversion to glaucoma of PEX. In the literature, we did not find a study on activity of CA in aqueous and serum samples in patients with PEX. Therefore, we believe that this study can be a reference.
Association between systemic immunity-inflammation index and psoriasis among outpatient US adults

Abstract:Background: Psoriasis is a chronic dermatological condition characterized by a complex pathogenesis that impacts approximately 3% of adults in the United States and brings enormous social burdens. For many diseases, the systemic immune-inflammatory index (SII), defined as neutrophils × platelets/lymphocytes, has been recognized as a prognostic indicator. Therefore, we conducted a cross-sectional study to assess the association between SII and psoriasis among outpatient US adults. Methods: In this cross-sectional study, we used data on the US adults 20 to 59 years of age from the National Health and Nutrition Examination Survey (NHANES) spanning 2003–2006 and 2009–2014. Sample-weighted logistic regression and stratified analysis of subgroups were used. Results: Among the 16,831 adults, there were 8,801 women and 8,030 men, with a psoriasis prevalence rate of 3.0%. A fully adjusted model revealed a positive association between a SII higher than 479.15 × 10 9 /L and a high risk of psoriasis. According to subgroup analysis and interaction testing (p for interaction > 0.05), age, sex, alcohol drinking status, marital status, and body mass index (BMI) were not significantly correlated with this positive association. Conclusion: Our findings suggested that SII higher than 479.15 × 10 9 /L was positively associated with a high risk of psoriasis among outpatient US adults. To the best of our knowledge, this is the first cross-sectional study using NHANES data focused on the risk of higher SII on psoriasis among outpatient US adults. The outcomes of this cross-sectional serve to supplement previous research, indicating a need for larger-scale prospective cohorts for further validation.

Abstract:Purpose: To study the point prevalence of pseudoexfoliation syndrome (PXS) among Yemeni patients in different governorates with age-related cataract scheduled for surgery. Settings: Eye camps organized by the Nibras Health Society to perform cataract surgeries during the years 2002-2006. All patients aged 40 years and above were included in the study. Materials and methods: A total of 2535 eyes of 2535 patients from 13 governorates, scheduled for cataract surgery in eye camps, were included. All eyes underwent complete eye examination before the surgery and were evaluated for the signs of pseudoexfoliation material in the pupil, iris and lens capsule on dilated slit lamp examination. Results: The study found 495 of the 2535 eyes (19.53%) with PXS with males more commonly affected than females (55.2 and 44.8%, respectively). The mean age of patients with PXS was 66.2 years while it was 64.6 years in non-PXS patients. The prevalence of pseudoexfoliation syndrome increased with age (10.1% in the age group of 41-50 years that increased to 28.8% in the age group of more than 81 years old). The rate of PXS detection in camps in 13 governorates ranged from 13.33 to 24.22% with an overall rate of 19.53%. The lowest rate was noticed in Sana'a and the highest in Al-Dhale governorate. Conclusion: This pilot study confirms that PXS was common in patients undergoing cataract surgery in Yemen with an increased detection rate with age. This study also highlights the prevalence of an ocular disease that is associated with systemic and ocular complications; however, further studies based on population studies are needed.

Abstract:<span class="a-plus-plus abstract-section id-a-sec1"><h3 class="a-plus-plus">Purpose</h3><p class="a-plus-plus">This cross-sectional study compared the peripapillary vessel density and retinal nerve fiber layer (RNFL) thickness in patients with exfoliation syndrome (XFS) and healthy controls for evaluation of the early structural and vascular alterations in XFS.</p></span><span class="a-plus-plus abstract-section id-a-sec2"><h3 class="a-plus-plus">Methods</h3><p class="a-plus-plus">One eye was included from 75 patients with XFS and 54 healthy controls. The patients with XFS were matched the controls for age, intraocular pressure and axial length. The vascular density of the radial peripapillary capillaries (RPCs) and the peripapillary RNFL thickness were evaluated with optical coherence tomography angiography.</p></span><span class="a-plus-plus abstract-section id-a-sec3"><h3 class="a-plus-plus">Results</h3><p class="a-plus-plus">The mean peripapillary RNFL thicknesses of the groups were similar in all sectors (<em class="a-plus-plus">p</em> > 0.05 for all). However, eyes with XFS demonstrated lower mean peripapillary vessel densities in all areas (<em class="a-plus-plus">p</em> < 0.05 for all) except for the nasal sector (<em class="a-plus-plus">p</em> = 0.68) compared to the controls. The gradual age correlated decline in the peripapillary RNFL thickness and the RPC vessel density observed in the healthy eyes was absent in XFS (<em class="a-plus-plus">r</em> = − 0.14 <em class="a-plus-plus">p</em> = 0.65 and <em class="a-plus-plus">r</em> = − 0.23 <em class="a-plus-plus">p</em> = 0.05).</p></span><span class="a-plus-plus abstract-section id-a-sec4"><h3 class="a-plus-plus">Conclusions</h3><p class="a-plus-plus">Alterations in the peripapillary vascular density despite a preserved RNFL thickness in XFS supports the hypothesis that vascular alterations may precede structural alterations and have an important role in the pathogenesis of XFS. XFS may have different effects on the microvasculature of different peripapillary areas, with the nasal sector being mostly preserved.</p></span>

Research on Psoriasis As a Psychosomatic Disease

Abstract:In 1988 and 1998, We carried on SAS and SDS evaluations on 201 psoriatic patients and 147 healthy subjects. In 1996, we studied autonomic nervous function with the 34 vulgar psoriasis patients and 30 normal subjects. In 1997 we discovered that there was higher content of neuro immune protein in psoriasis patients' serum. In the same year, we also found abnormal expression of heat shock proteins, nerve growth factor and receptor on psoriatic lesions. These studies showed systematic imbalance of regulatory system functions aside from the abnormality of psoriatic lesions. In 1999, we studied the treatment mechanism of biofeedback method and found that it could improve regulatory function of autonomic nerves. This research showed that the improvenment of systematic condition could help lesions healing. The aforesaid series of studies shows that psoriasis is a psychosomatic disease.
Evaluation of the inflammatory parameters as potential biomarkers of systemic inflammation extent and the disease severity in psoriasis patients

Abstract:The disease severity of psoriasis is mainly assessed subjectively via psoriasis area and severity index (PASI) and body surface area (BSA), while an optimal measure of cutaneous response, may overlook systemic inflammation in psoriasis patients. The neutrophil to lymphocyte ratio (NLR), platelet to lymphocyte ratio (PLR), monocyte to lymphocyte ratio (MLR), monocyte to high density lipoprotein ratio (MHR), and systemic immune-inflammation index (SII) exhibit notable associations with the inflammation severity in multiple diseases. The aim of this retrospective study was to explore the associations between inflammatory parameters and the skin lesions' severity of psoriasis. After analysis, we found that patients with psoriasis had higher NLR, MLR, PLR, MHR, and SII levels compared to the control group. At baseline, the parameters of NLR (r = 0.124, P = 0.003), MLR (r = 0.153, P < 0.001), MHR (r = 0.217, P < 0.001) and SII (r = 0.141, P = 0.001) had a positive correlation with PASI in psoriasis patients. At the same time, we analyzed the patients who received different systemic therapy. We observed a significant decrease in NLR, PLR, MLR, and SII in psoriasis patients after treatment. Notably, TNF-α inhibitors and IL-17A inhibitors subgroups showed a more significant reduction than IL-23/IL-12/23 inhibitors and MTX medication. Additionally, we found the change of NLR (r = 0.194, P < 0.001), PLR (r = 0.104, P = 0.014), MLR (r = 0.191, P < 0.001), MHR (r = 0.106, P = 0.012), and SII (r = 0.228, P < 0.001) had a positive correlation with the change of PASI in psoriasis patients. In conclusion, this study suggests that NLR, MLR, and SII may serve as useful biomarkers for assessing systemic inflammation extent and disease severity in psoriasis patients.

Abstract:Purpose To investigate changes in choroidal thickness in patients diagnosed with pseudoexfoliation syndrome (PEX) compared to healthy controls, using optical coherence tomography (OCT). Methods PubMed and Scopus databases were systematically searched for published articles comparing choroidal thickness between patients with PEX and healthy controls. Standardized Mean Difference (SMD) with 95% confidence interval (CI) was computed to compare continuous variables. Revman 5.4 was used for the analysis. Subgroup analyses were performed according to OCT devices used. Results 12 studies were included in our analysis. Subfoveal choroidal thickness was decreased in patients with PEX compared to healthy controls. Subgroup analysis confirmed this finding in studies that used Heidelberg or Optovue OCT Devices. Conclusion Our meta-analysis showed that choroidal thickness was decreased in patients with PEX compared to controls. Increased heterogeneity and small case-control studies are the main limitations of the meta-analysis. Further studies are needed to evaluate the clinical significance of reduced subfoveal choroidal thickness in PEX.
