Abstract:OBJECTIVETo summarize the principles and application of phototherapy consistent with the current 2022 American Academy of Pediatrics "Clinical Practice Guideline Revision for the Management of Hyperbilirubinemia in the Newborn Infant 35 or More Weeks of Gestation."METHODSRelevant literature was reviewed regarding phototherapy devices in the United States, specifically those that incorporate blue to blue-green light-emitting diode, fluorescent, halogen, or fiberoptic light sources, and their currently marketed indications.RESULTSThe efficacy of phototherapy devices varies widely because of nonstandardized use of light sources and configurations and irradiance meters. In summary, the most effective and safest devices have the following characteristics: (1) incorporation of narrow band blue-to-green light-emitting diode lamps (∼460–490 nm wavelength range; 478 nm optimal) that would best overlap the bilirubin absorption spectrum; (2) emission of irradiance of at least 30 μW/cm 2 /nm (in term infants); and (3) illumination of the exposed maximal body surface area of an infant (35% to 80%). Furthermore, accurate irradiance measurements should be performed using the appropriate irradiance meter calibrated for the wavelength range delivered by the phototherapy device.CONCLUSIONSWith proper administration of effective phototherapy to an infant without concurrent hemolysis, total serum or plasma bilirubin concentrations will decrease within the first 4 to 6 hours of initiation safely and effectively.

Abstract:The purpose of this study was to investigate the effect of blue light phototherapy on the expression of circadian genes in peripheral blood mononuclear cells (PBMC) and plasma melatonin levels in neonates. Real-time reverse-transcriptase polymerase chain reaction (RT-PCR) was used to determine the expression of Bmal1 and Cry1 in PBMC, and an enzyme-linked immunosorbent assay was used to determine plasma melatonin levels in 32 breast-milk jaundiced neonates before and after phototherapy, compared with 29 control neonates. The results showed that the expression of Bmal1 was decreased and Cry1 increased significantly after phototherapy. Plasma melatonin levels were decreased after phototherapy. There was no statistical difference in Bmal1 and Cry1 gene expression and plasma melatonin levels in the control group. In conclusion, phototherapy does affect the expression of the circadian genes Bmal1 and Cry1 in PBMC and plasma melatonin concentration in jaundiced neonates. Our results suggest that phototherapy should be timed according to circadian rhythms when treating jaundiced neonates.

Abstract:AIM: Limited information is available about how guidelines on phototherapy for neonatal jaundice are applied in practice and toxicity is a concern. We studied the use of phototherapy in relation to birthweight and gestational age (GA) in Norwegian neonatal intensive care units (NICUs).METHODS: The study population was all 5382 infants admitted to the 21 NICUs in Norway between September 1, 2013 and August 31, 2014. Data were recorded daily in the Norwegian Neonatal Network database and anonymised data on patient characteristics, diagnoses, duration, the ages at the start and discontinuation of phototherapy were analysed.RESULTS: More than a quarter (26.6%) of all infants admitted to Norwegian NICUs during the study period received phototherapy. The use of phototherapy was inversely related to GA and birthweight. More than 80% of the preterm infants under 28 weeks of GA received phototherapy. The duration was significantly longer in the lowest birthweight and GA groups and decreased with increasing birthweight and GA.CONCLUSION: Phototherapy is proved to be a strong candidate for the most common therapeutic modality in NICU infants. However, in the light of reported toxicity in the smallest, most vulnerable infants, we recommend increased emphasis on quality control.

Abstract:Purpose This project aims to explore healthcare professionals' perceptions of providing home phototherapy for neonatal jaundice as part of standard care. Design and Methods In-depth, semi-structured interviews were conducted with a purposive sample of nine healthcare professionals from the Neonatal Intensive Care Unit. The data was analyzed according to the Interpretive Phenomenological Analysis using six steps devised by Braun and Clarke for thematic analysis using NVivo Software. Results Six major themes were identified that encompassed healthcare professionals' perceptions of home phototherapy for neonatal jaundice. These themes included: baby factors for home phototherapy, better in own environment, parent factors for home phototherapy, perceived benefits for home phototherapy, perceived risks of home phototherapy, and system factors impacting home phototherapy. Conclusions Home phototherapy was perceived positively amongst the healthcare professionals. The healthcare professionals expressed many perceived benefits of home phototherapy such as family-centred care, improved bonding, improved establishment of breastfeeding, and decreased healthcare expenditure. Practice implications Enables understanding of the barriers to establishing home phototherapy programs and provides a scaffold for the development of these programs.
Bilirubin Measurement and Phototherapy Use After the AAP 2022 Newborn Hyperbilirubinemia Guideline

Abstract:BACKGROUND AND OBJECTIVES Guidelines for the management of neonatal hyperbilirubinemia have helped to reduce rates of significant hyperbilirubinemia. However, recent evidence suggesting overtreatment and potential harms of phototherapy have informed the American Academy of Pediatrics clinical practice guideline revision and the accompanying increase in phototherapy thresholds. These changes are predicted to safely reduce overuse; however, to date, the exact effect of these guidelines has not been established. METHODS We conducted a retrospective study of newborns born at ≥35 weeks’ gestation across a network of 8 hospitals between January 2022 and June 2023. Outcomes included rates of phototherapy and total serum bilirubin (TSB) measurements before and after guideline publication, as well as clinical outcomes, including length of stay, readmissions, and duration of phototherapy. RESULTS In our cohort of >22 000 newborns, we observed a 47% decrease in phototherapy utilization, from 3.9% to 2.1% (P < .001). TSB measurements were reduced by 23%, from 712 to 551 measurements per 1000 newborns (P < .001), without an increase in outpatient TSB measurements. We did not observe an increase in readmissions receiving phototherapy, and length of stay increased by only 1 hour (P < .001). CONCLUSIONS Our study reveals that the publication of the updated American Academy of Pediatrics 2022 hyperbilirubinemia guidelines has likely yielded a significant reduction in phototherapy use and serum bilirubin measurement. Dedicated quality improvement initiatives may help determine which implementation strategies are most effective. Further population-level studies are needed to confirm safety with ongoing guideline uptake.

Abstract:The study of medications among pediatric patients has increased worldwide since 1997 in response to new legislation and regulations, but these studies have not yet adequately addressed the therapeutic needs of neonates. Additionally, extant guidance developed by regulatory agencies worldwide does not fully address the specificities of neonatal drug development, especially among extremely premature newborns who currently survive. Consequently, an international consortium from Canada, Europe, Japan, and the United States was organized by the Critical Path Institute to address the content of guidance. This group included neonatologists, neonatal nurses, parents, regulators, ethicists, clinical pharmacologists, specialists in pharmacokinetics, specialists in clinical trials and pediatricians working in the pharmaceutical industry. This group has developed a comprehensive, referenced White Paper to guide neonatal clinical trials of medicines – particularly early phase studies. Key points include: the need to base product development on neonatal physiology and pharmacology while making the most of knowledge acquired in other settings; the central role of families in research; and the value of the whole neonatal team in the design, implementation and interpretation of studies. This White Paper should facilitate successful clinical trials of medicines in neonates by informing regulators, sponsors, and the neonatal community of existing good practice.

[Effect and Safety of Intensive Phototherapy in Treatment of Neonatal Hyperbilirubinemia].

Abstract:OBJECTIVE:To study the effect and safety of intensive phototherapy in the treatment of neonatal hyperbilirubinemia.METHODS:A total of 144 neonates with neonatal hyperbilirubinemia were randomly and prospectively divided into intensive phototherapy group and conventional phototherapy group, with 72 neonates in each group. The therapeutic effect and incidence of complications were compared between the two groups.RESULTS:Within 12 hours after phototherapy, the total serum bilirubin level in the intensive phototherapy group was significantly lower than in the conventional phototherapy group (P<0.05), and the intensive phototherapy group had a significantly greater reduction in serum bilirubin level than the conventional phototherapy group (P<0.05). The intensives phototherapy group had a significantly shorter time of phototherapy than the conventional phototherapy group (P<0.05). The incidence rates of fever, diarrhea, rash, and hypocalcemia and reductions in blood calcium and hemoglobin levels after phototherapy showed no significant differences between the two groups.CONCLUSIONS:During the initial stage of phototherapy, intensive phototherapy can quickly and effectively reduce the serum level of bilirubin in neonates with neonatal hyperbilirubinemia. It can also shorten the total phototherapy time, and does not increase the incidence of adverse events. Therefore, it is superior to conventional phototherapy.

Abstract:Since its discovery in 50 ́s of the last century, 1 phototherapy of neonatal jaundice has become the golden standard in the treatment of severe neonatal jaundice. 2 Exposure of the newborn to the light of a specific wavelength (blue or blue-green light) leads to photisomerization and photooxidation of unconjugated bilirubin (UCB), the products of which are more polar and excretable through the kidneys into the urine and the liver into the bile. In the last seven decades, phototherapy has become a modern, evidence-based therapeutic tool, with the development of treatment protocols to maximize its efficacy. Among the factors most importantly affecting the efficacy of phototherapy are irradiance intensity (depending on the skin area exposed), distance of the light source (various sources have been developed, from fluorescence tubes, halogen spotlights, fiberoptic pads/blankets, to light-emitting diodes), the exposed skin, intermittence/continuance of phototherapy, and the spectrum of light. 2 Blue light, such as that emitted from blue light-emitting diodes (LED ́s) within the narrow range of 450–470 nm, has long been believed to be the most efficient, 3 based on the fact that the absorption maximum of UCB in aqueous media is around 440 nm. 4 However, recent data suggest that wavelengths longer than those reflecting the absorption maximum of UCB are even more effective in lowering serum UCB concentrations in newborns suffering from severe neonatal jaundice. The first experimental study on efficient bilirubin oxidation with blue-green/turquoise green light was reported as early as 1981 5 followed by several clinical studies proving the efficacy of green light. 6,7,8,9,10,11,12,13,14 In the recent study, Ebbesen and colleagues confirmed a significantly higher efficacy in reducing UCB of blue-green LED light phototherapy with a peak emission of 478 nm compared to standard blue LED light with a peak emission of 459 nm, 15 which was supported by quantifying the production of the main individual bilirubin photooxidation products.

Abstract:The sick preterm infant monitoring is an intriguing job that medical staff in Neonatal Intensive Care Units (NICU) must deal with on a daily basis. As a standards monitoring procedure, preterm infants are monitored via sensors and electrodes that are firmly attached to their fragile and delicate skin and connected to processing monitors. However, an alternative exists in contactless imaging to record such physiological signals (we call it as Physio-Markers), detecting superficial changes and internal structures activities which can be used independently of, or aligned with, conventional monitors. Countless advantages can be gained from unobtrusive monitoring not limited to: (1) quick data generation; (2) decreasing physical and direct contact with skin, which reduces skin breakdown and minimizes risk of infection; and (3) reduction of electrodes and probes connected to clinical monitors and attached to the skin, which allows greater body surface-area for better care. This review is an attempt to build a solid ground for and to provide a clear perspective of the potential clinical applications of technologies inside NICUs that use contactless imaging modalities such as Visible Light Imaging (VLI), Near Infrared Spectroscopy (NIRS), and Infrared Thermography (IRT).
Risk of childhood cancer in infants treated with phototherapy for neonatal jaundice

Abstract:Phototherapy of neonatal jaundice was first introduced by Cremer et al. as early as 1958, 1 but further studies that focused on demonstrating its safety and efficacy by Lucey et al. were needed to become widely adopted. 2 Since then, phototherapy has become the gold standard in the treatment of neonatal jaundice in fact, approximately 8 million newborn infants are treated annually with phototherapy. 3 However, as early as 1975, Speck et al. had raised the first concern about the possible carcinogenic potential of phototherapy, when they demonstrated the mutagenicity of photoilluminated bilirubin on calf thymus DNA. 4 Illumination of DNA in the presence of bilirubin resulted in DNA alterations, while these changes were not observed when DNA was irradiated in the absence of bilirubin or when DNA mixed with bilirubin was kept in the dark (4), indicating that rather than phototherapy itself, the bilirubin photooxidation products were responsible for these effects. Importantly, these genotoxic effects were documented in a human study in lymphocytes from newborns treated with phototherapy. 5

Four studies (three randomized studies and one retrospective study) meet the final inclusion criteria. Seven hundred and sixteen neonates were included in the meta‐analysis. There was no difference in the treatment efficacy and total serum bilirubin (TSB), while there was a significant difference in phototherapy duration and side effects after treatment of intermittent phototherapy and continuous phototherapy for neonatal hyperbilirubinaemia.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>Intermittent phototherapy appeared to be as effective as continuous phototherapy for the treatment of neonatal hyperbilirubinaemia and is safer than continuous phototherapy. Clinical efficacy analysis of intermittent and continuous blue light irradiation treatments on neonatal hyperbilirubinemia

Abstract:Objective: To observe the efficacy of intermittent and continuous phototherapy treatments on neonatal unconjugated hyperbilirubinemia and their adverse effects on children.Methods: 180 cases of children were randomly selected and divided into two groups,namely,intermittent phototherapy group and continuous light therapy group.Analysis of the situation was made of the intermittent and continuous blue light treatments.Results: The difference between blu-ray irradiation of serum bilirubin before and after treatment in two groups was statistically significant(P<0.05).The adverse events of blu-ray irradiation after treatment in two groups were statistically significant difference(P<0.01).Conclusion: Intermittent blue light irradiation can reach the same effect of continuous blue light,but the intermittent blue light incidence of adverse reactions is significantly lower than that of the continuous blue light,worthy to be popularized.
Neonatal encephalopathy: a systematic review of reported treatment outcomes

Abstract:Background: Neonatal encephalopathy (NE) is a multi-organ condition potentially leading to death or long-term neurodisability. Therapeutic hypothermia is the standard treatment for NE; however, long-term impairments remain common. Studies of new treatments for NE often measure and report different outcomes. Core outcome sets (COSs), a minimum set of outcomes to be measured and reported in all studies for a condition, address this problem. This paper aimed to identify outcomes reported (primary, secondary, adverse events and other reported outcomes) in (1) randomised trials and (2) systematic reviews of randomised trials of interventions for the treatment of NE in the process of developing a COS for interventions for the treatment of NE. Methods: We completed a systematic search for outcomes used to evaluate treatments for NE using MEDLINE, Embase, Cochrane CENTRAL, the Cochrane Database of Systematic Reviews and the WHO International Clinical Trials Registry Platform. Two reviewers screened all included articles independently. Outcomes were extracted verbatim, similar outcomes were grouped and outcome domains were developed. Results: 386 outcomes were reported in 116 papers, from 85 studies. Outcomes were categorised into 18 domains. No outcome was reported by all studies, a single study reported 11 outcomes and it was not explicitly stated that outcomes had input from parents. Discussion: Heterogeneity in reported outcomes means that synthesis of studies evaluating new treatments for NE remains difficult. A COS, that includes parental/family input, is needed to ensure consistency in measuring and reporting outcomes, and to enable comparison of randomised trials.
Evaluation of efficacy of oral calcium phosphate as an adjunct to standard-of-care regular phototherapy in cases of neonatal jaundice: a hospital-based double-blind, randomised, placebo-controlled trial

Abstract:Objective: Neonatal jaundice is the most common cause of neonatal morbidity and rehospitalisation in the first week of life, affecting approximately 60% of term and 80% of preterm neonates, with 10% requiring phototherapy to prevent bilirubin-induced neurological dysfunction. Enterohepatic circulation contributes 10%-20% of the body's bilirubin load, and oral calcium-phosphate can inhibit this process by binding to unconjugated bilirubin and acting as a bilirubin-trapping agent in the gut. This study aimed to evaluate the efficacy of oral calcium phosphate as an adjunct to phototherapy in reducing phototherapy duration, improving bilirubin decline rate and lowering rebound hyperbilirubinaemia incidence. Methods: This double-blind, placebo-controlled randomised controlled trial with a 1:1 allocation ratio was conducted in the neonatal intensive care unit of a tertiary care hospital in Eastern India. The investigator and the analyst were blinded to the treatment assignments. Eligible neonates with neonatal jaundice requiring phototherapy as per the 'American Academy of Pediatrics or 'National Institute for Health and Care Excellence' guidelines were enrolled and randomly assigned to receive either oral calcium phosphate or placebo. Results: The total duration of phototherapy was significantly lower in the intervention group compared with placebo (18.8±5.63 hours vs 24.3±4.50 hours; mean difference=-5.55 (95% CI -7.82 to -3.28), p<0.001). The rate of fall of bilirubin (mg/dL/hour) was also significantly higher in the intervention group (0.186±0.0137 vs 0.116±0.0088; mean difference=0.0693 (95% CI 0.0642 to 0.0745), p<0.001). The intervention group showed a trend towards a decrease in the incidence of rebound hyperbilirubinaemia, with a relative risk of 0.30 ((95% CI 0.0891 to 1.01), p=0.066). Conclusion: The use of oral calcium phosphate results in a statistically significant reduction in phototherapy hours, an improvement in the rate of bilirubin decline and a decrease in rebound hyperbilirubinaemia incidence. This allows for shorter hospital stays and reduces the need for rehospitalisation, resulting in less mother-baby dyad separation, lower hospital resource consumption and reduced financial burden on parents. Neuromonitoring in neonatal intensive care units—an important need towards individualized neuroprotective care

Abstract:Neuromonitoring has been widely accepted as an important part in neonatal care. Amplitude-integrated EEG (aEEG) and near-infrared spectroscopy (NIRS) are often mentioned in this context, though being only a part of the fully array of methods and examinations that could be considered neuromonitoring. Within the broad array of medical conditions that could be encountered in a neonatal patient, it is important to be aware of the indications for neuromonitoring and especially which neuromonitoring technique to use best for the individual condition. aEEG is now a widely accepted neuromonitor in neonatology with its value in hypoxic events and seizures only rarely questioned. Other methods like NIRS still have to prove themselves in the future. The SafeBoosC-III trial showed that it still remains difficult for some of these methods to prove their value for the improvement of outcome. Bute future developments such as multimodal neuromonitoring with data integration and artificial intelligence analysis could improve the value of these methods.

Neonatal Hyperbilirubinemia: Evaluation and Treatment

Abstract:Neonatal jaundice due to hyperbilirubinemia is common, and most cases are benign. The irreversible outcome of brain damage from kernicterus is rare (1 out of 100,000 infants) in high-income countries such as the United States, and there is increasing evidence that kernicterus occurs at much higher bilirubin levels than previously thought. However, newborns who are premature or have hemolytic diseases are at higher risk of kernicterus. It is important to evaluate all newborns for risk factors for bilirubin-related neurotoxicity, and it is reasonable to obtain screening bilirubin levels in newborns with risk factors. All newborns should be examined regularly, and bilirubin levels should be measured in those who appear jaundiced. The American Academy of Pediatrics (AAP) revised its clinical practice guideline in 2022 and reconfirmed its recommendation for universal neonatal hyperbilirubinemia screening in newborns 35 weeks' gestational age or greater. Although universal screening is commonly performed, it increases unnecessary phototherapy use without sufficient evidence that it decreases the incidence of kernicterus. The AAP also released new nomograms for initiating phototherapy based on gestational age at birth and the presence of neurotoxicity risk factors, with higher thresholds than in previous guidelines. Phototherapy decreases the need for an exchange transfusion but has the potential for short- and long-term adverse effects, including diarrhea and increased risk of seizures. Mothers of infants who develop jaundice are also more likely to stop breastfeeding, even though discontinuation is not necessary. Phototherapy should be used only for newborns who exceed thresholds recommended by the current AAP hour-specific phototherapy nomograms.
Regenerative Retinal Laser and Light Therapies (RELITE): Proposal of a New Nomenclature, Categorization, and Trial Reporting Standard

Abstract:Objectives: Numerous laser and light therapies have been developed to induce regenerative processes in the choroid/retinal pigment epithelium (RPE)/photoreceptor complex, leaving the neuroretina undamaged. These therapies are applied to the macula for the treatment of various diseases, most prominently diabetic maculopathy, retinal vein occlusion, central serous chorioretinopathy, and age-related macular degeneration. However, the abundance of technologies, treatment patterns, and dosimetry protocols has made understanding these therapies and comparing different approaches increasingly complex and challenging. To address this, we propose a new nomenclature system with a clear categorization that will allow for better understanding and comparability between different laser and light modalities. We propose this nomenclature system as an open standard that may be adapted in future toward new technical developments or medical advancements. Methods: A systematic literature review of reported macular laser and light therapies was conducted. A categorization into a standardized system was proposed and discussed among experts and professionals in the field. This paper does not aim to assess, compare, or evaluate the efficacy of different laser or dosimetry techniques or treatment patterns. Results: The literature search yielded 194 papers describing laser techniques, 50 studies describing dosimetry, 272 studies with relevant clinical trials, and 82 reviews. Following the common therapeutic aim, we propose "regenerative retinal laser and light therapies (RELITE)" as the general header. We subdivided RELITE into four main categories that refer to the intended physical and biochemical effects of temperature increase (photothermal therapy, PTT), RPE regeneration (photomicrodisruption therapy, PMT), photochemical processes (photochemical therapy, PCT), and photobiomodulation (photobiomodulation therapy, PBT). Further, we categorized the different dosimetry approaches and treatment regimens. We propose the following nomenclature system that integrates the most important parameters to enable understanding and comparability: Pattern-Dosimetry-Exposure Time/Frequency, Duty Cycle/Irradiation Diameter/Wavelength-Subcategory-Category. Conclusion: Regenerative retinal laser and light therapies are widely used for different diseases and may become valuable in the future. A precise nomenclature system and strict reporting standards are needed to allow for a better understanding, reproduceable and comparable clinical trials, and overall acceptance. We defined categories for a systematic therapeutic goal-based nomenclature to facilitate future research in this field.

Abstract:Introduction There is insufficient evidence to determine if non-invasive transcutaneous bilirubin (TcB) measurement can replace serum bilirubin (SBR) in assessing rebound hyperbilirubinaemia after phototherapy. Objective To investigate if TcB can safely guide management of neonates after phototherapy. Subjects 100 well neonates ≥35 weeks’ gestation who had received inpatient phototherapy. Method Measurement of both helix (manufacturer’s recommendation) and earlobe TcB coincidentally with routine SBR 12 hours after cessation of phototherapy. All mothers gave written informed consent. Results Gestation ranged from 35 +0 to 41 +5 (median 37 +6 ) weeks; birth weight 2018–4566 (median 3230) g; age 55–222 (median 109) hours at testing. 86% neonates were Caucasian. Outcomes determined by SBR included restarting phototherapy (n=0), repeat SBR next day (n=29), no further routine follow-up (n=71). TcB and SBR measurements were unpredictably inconsistent. Helix TcB tended to underestimate SBR (mean difference 50.1 (95% CI 113.9 to -13.7) μmols/L); for earlobe TcB mean difference was -13.4 (95% CI 46.3 to -73.2) μmols/L (overestimate), but bias was greater over the range of mean differences. No demographic factor predicted consistency between TcB and SBR. TcB was 25% (helix) and 76% (earlobe) sensitive in predicting repeat phototherapy and/or repeat SBR; specificities were 92% and 58%, respectively. Adding a safety margin of 120 μmols/L to helix TcB value could have safely avoided invasive SBR measurement in 50/98 (51%) babies. Conclusions Consistency between TcB and rebound SBR is unpredictable in well neonates > 35 weeks’ gestation but adopting a wide safety margin has potential to reduce blood sampling. Recommencement of phototherapy is uncommon in this population.

Abstract:Abstract Introduction Light is the most potent signal for the circadian regulation of sleep. To quantify light’s biological impact via melanopic Equivalent Daylight Illuminance (EDI), photic intensity is weighted based on the spectral sensitivity of melanopsin. This metric for light and time-of-day specific threshold recommendations support development, standardization, and replication of studies of the physiological effects of light. Light measurement outside the lab poses many unique challenges (e.g. greater variability in timing, intensity, spectrum; individual factors; technology limitations). Thus, consistent methods for measurement and reporting are important for maximizing the utility of emerging tools. Methods A framework for assessing and reporting light in field studies was developed, incorporating: 1) factors that influence light exposure, 2) methods for capturing the most significant factors, 3) strategies for addressing unique tradeoffs and challenges, and 4) an initial step-by-step protocol for measuring and reporting lighting characteristics in field studies of the physiological effects of light. This framework was applied in multiple study protocols to inform development and refinement. Results All studies used both static and continuous measures, along with subjective assessments. In one study, lighting was not homogenous and therefore position and location in the space mattered (melanopic EDI seated vs standing, 133.84 vs 221.91 lux; between workstations (seated), 133.84 vs 62.29 lux). In a second study, lighting was more homogenous but varied by time-on-shift; thus, continuous measures with a stationary meter were used. In a third study, subjective assessment of behaviors that influence photic exposure patterns over the past month indicated individuals likely did not meet nighttime light recommendations (melanopic EDI < 1 lux): 73% used their phone in bed often or always before sleep; and 20% often or always checked their phone if awakened from sleep. Conclusion When measuring and reporting light in the field, a one-size-fits-all approach is impossible, but some level of standardization is necessary. The proposed multi-level framework expands on recent lab-based guidelines by making adaptations for field studies, facilitating the evidence-based translation of our fundamental understanding of the effects of light on sleep and circadian health.
