Heart Rate Variability Biofeedback: A Complimentary Option for Pain Management in Fibromyalgia

Abstract:Aim The aim of our study was to determine the feasibility and acceptability of a 20 minute, twice-daily heart rate variability biofeedback protocol in a group of Veterans with fibromyaglia. Background Fibromyalgia (FM) is associated with debilitating pain and decreased emotional adaptability and resistance to stress. Heart rate variability biofeedback (HRVB) may reduce FM-related symptoms as well as improve physical functioning and quality of life. Method In a multi-method feasibility and acceptability study at a Veterans' outpatient clinic, we enrolled 7 women and 3 men age 33 to 68 years with a diagnosis of FM. Participants were followed weekly and instructed on a HRVB protocol for seven weeks. Following the intervention period, we conducted an end of study focus group. Results Most participants expected improvement in their pain and felt HRVB would reduce their FM-related pain. The mean daily practice frequency rate was 0.80 and duration was 19.36 minutes. Pain scores did not vary greatly pre- and post-intervention. The mean total FIQR scores post-intervention improved by 18.1 points. Lastly, three common themes emerged: intervention implementation, protocol adherence, and self-awareness. Conclusions Our findings suggest twice-a-day practice protocol is not feasible. However, 20-minute sessions were feasible and acceptable. FIQR scores post-treatment suggest HRVB may improve functional status and QOL. Results also suggest difficulties operating the HRVB device and scheduling challenges interfered with HRVB implementation. However, veterans reported self-awareness of the benefits of HRVB, positive physiological and improved psychological effects. Future studies require a larger sample size to provide a deeper insight. Clinical implications: HRVB is a promising complimentary treatment for FM which health care providers can implement into their treatment protocol.
