Spiritual care needs and their attributes among Chinese inpatients with advanced breast cancer based on the Kano model: a descriptive cross-sectional study

Abstract:Numerous previous research have established the need for spiritual care among patients with cancer globally. Nevertheless, there was limited research, primarily qualitative, on the spiritual care needs of Chinese inpatients with advanced breast cancer. Furthermore, the need for spiritual care was rarely explored using the Kano model. To better understand the spiritual care needs and attributes characteristics of inpatients with advanced breast cancer, this study examined the Kano model.

### Research Background

Globally, breast cancer is the most common type of cancer among women. According to the 2023 global cancer statistics report, the incidence of breast cancer accounts for 31.0% of female cancers and 15.0% of all cancer-related deaths. In China, there are approximately 420,000 new cases of breast cancer each year, accounting for 19.9% of all female cancer cases. Most cases are advanced breast cancer. Treatments such as surgery, radiotherapy, chemotherapy, and targeted therapy inevitably produce side effects, leading to various health problems and affecting the quality of life. Therefore, spiritual care is particularly important for patients with advanced breast cancer, as it can help them overcome suffering, find meaning in life, and gain support.

### Research Methods

This study adopted a descriptive cross-sectional design and conducted surveys in the oncology departments of three tertiary hospitals in China. Data collection included demographic characteristics questionnaires, the Nurse Spiritual Therapeutic Scale (NSTS), and the Nurse Spiritual Therapeutic Attributes Scale based on the Kano model (K-NSTAs). Data were collected using convenience sampling and analyzed using the Kano model, descriptive statistics, two independent samples t-test, and one-way ANOVA.

- **Overall Spiritual Care Needs Score**: 31.16 ± 7.85 points.
- **Highest Average Score Dimension**: "Creating a Good Atmosphere" (3.16 ± 0.95).

- **Distribution of 12 Spiritual Care Needs Attributes**:
- Attractive Attributes: 3 items (located in Retention Area IV).
- One-Dimensional Attributes: 5 items (3 items in Dominant Area I, 2 items in Improvement Area II).
- Must-Be Attributes: 2 items (located in Secondary Improvement Area III).
- Indifference Attributes: 2 items (located in Improvement Area II).

### Discussion

The study shows that the spiritual care needs of hospitalized patients with advanced breast cancer in China are at a moderate level and need further improvement. The Kano model can accurately define, rank, classify, and optimize the attributes of spiritual care needs. Specifically, "Creating a Good Atmosphere" and "Sharing Self-Perception" are the main one-dimensional and must-be attributes, while the dimension related to "Helping with Religious Practices" is mainly an attractive attribute.
