High Risk Factors of Persistent Pulmonary Hypertension in Newborn Infants

Abstract:Objective To understand high risk factors of persistent pulmonary hypertension(PPHN) in newborn infants,and the relationship between gestational age,primary diseases and prognosis.Methods From Jan.2003 to Jun.2007,there were 146 newborn infants with PPHN admitted in neonatal intensive care unit.The sex,gestational age,primary diseases(including anoxia,infection,lung diseases,conge-nital heart diseases,congenital diaphragmatic hernia,et al),prognosis(recovery,improvement,invalidation,dead) and the relativity of these patients were analyzed.Results In the 146 PPHN newborn infants,including 90 male cases and 56 female cases,of which 105 cases(71.9%) who were term-birth and 41 cases(28.1%) who were preterm-birth.There were 74 cases(50.7%) associated with pulmonary diseases[including 26 cases with wet lung syndrome,26 cases with aspiration pneumonitis/meconium aspiration syndrome,17 cases with respiratory distress syndrome(RDS) and 5 cases with pneumothorax],43 cases(29.45%) with anoxia,9 cases(6.16%) with congenital heart diseases,8 cases(5.48%) with infection,and 7 cases(4.79%) with congenital diaphragmatic hernia,respectively.Wet lung syndrome of the newborn infant was the most common cause in term-births with PPHN and RDS in preterm-births,respectively.In these newborn infants,75 cases were cured,34 cases improved,37 cases invalid or dead.There were 23 cases with wet lung syndrome and 8 cases with RDS in term-births,while there were 3 cases with wet lung syndrome and 9 cases withe RDS in preterm-births.According to different initial problems,the mortality was 7.7% in group with wet lung disease,15.4% in group with aspiration pneumonitis,more than 20% in other groups,respectively.Conclusions An increased risk for PPHN of the newborn was associated with male gender,term birth,primary diseases with lung diseases and anoxia.However,the prognosis of the baby with PPHN caused by wet lung syndrome or aspiration pneumonitis is better than others.
