Abstract:The management of inoperable lung cancer remains a challenge. It has been proven that computed tomography (CT)-guided iodine-125 (125I) seed implantation is a safe and efficient method for treating lung cancer. Computed tomographic fluoroscopy (CTF) is superior to traditional CT for percutaneous management of lung lesions, due to the real-time guidance and accurate localization of the lesions. The aim of the present prospective study was to evaluate the feasibility, safety and efficacy of CTF-guided percutaneous permanent implantation of 125I seeds for the treatment of selected patients with inoperable stage T1-3N0M0 non-small-cell lung cancer (NSCLC). A total of 24 patients with resectable but inoperable stage T1-3N0 NSCLC, with a total of 28 lesions, underwent CTF-guided percutaneous implantation of radioactive 125I seeds. A prescription dose of 100-120 Gy was delivered to each lesion. The complications and local tumor control rates were documented. Survival was estimated using the Kaplan-Meier method. All the patients successfully completed the procedure, with a mean procedure duration of 45.7 min (range, 30-75 min). No severe complications occurred. Small asymptomatic pneumothorax with lung volume compression of <10% and minor hemorrhage along the needle track without hemoptysis occurred immediately after the procedure in 3 (12.5%) and 4 (16.7%) of the 24 patients, respectively. At a median follow-up of 31.5 months (range, 8-46 months), the local control rate (LCR) of the lesions was 78.6% (22/28). The 1-, 2- and 3-year overall survival rate was 95.8, 78 and 55%, respectively. In conclusion, CTF is the favourable imaging guidance method for the percutaneous implantation of 125I seeds. CTF-guided brachytherapy with implantation of 125I seeds is a safe, feasible and effective modality for the treatment of inoperable early-stage NSCLC and may be considered an alternative option in selected patients with medically inoperable NSCLC.

Abstract:BACKGROUND:Primary lung cancer and metastatic lung cancer are common malignant tumors of the lung and are the main cause of cancer-related death. Advanced lung cancer and lung metastatic cancer are mainly treated by systemic therapy, and local treatment is also an effective treatment for the refractory or recurrent lesions in the lungs after systemic treatment. ¹²⁵I radioactive particle implantation, as an efficient conformal radiotherapy, has a certain control effect on localized lung cancer. The purpose of this study was to investigate the clinical efficacy and safety of computed tomography (CT) guided percutaneous puncture of lung tissue with ¹²⁵I radioactive particle implantation in the treatment of advanced lung cancer and lung metastatic cancer.METHODS:Continuouslycollectedthe clinical and pathological data of 105 patients with advanced lung cancer and metastatic lung cancer treated by ¹²⁵I radioactive seed implantation in the Department of Thoracic Surgery, First Affiliated Hospital of Zhejiang University School of Medicine from January 1, 2014 to November 30, 2018. The patients were followed until March, 2019. The clinical efficacy and complications of seedimplantation were analyzed.RESULTS:A total of 105 patients were included in the study, 78 patients with advanced lung cancer and 27 patients with lung metastases. The median survival time after seed therapy was 395 days. The 1-year survival rate was about 78.1%, and the 2-year survival rate was about 56.1%. Seed implantation for advanced lung cancer is equivalent to lung metastasis. Seed combined with radiofrequency ablation, microwave ablation, and chemotherapy did not improve seed treatment. However, particle combined with external radiation therapy has a significant survival disadvantage compared with simple seed therapy.CONCLUSIONS:The CT-guided ¹²⁵I radioactive seed implantation has controllable complications and can be used as a safe and effective treatment for advanced lung cancer and lung metastases.

Abstract:Brachytherapy was the earliest technique of radiotherapy for the treatment of cancer and still is an important part of modern radiation oncology.It has been widely used as one of the modalities for gynecological cancer as well as head and neck cancer.In recent years,radioactive seed implantation has been proved to be one of the standard treatments for early stage prostate cancer,and brachytherapy alone is emerging as the major treatment for early stage breast cancer after lumpectomy.This article reviewed the current status of brachytherapy in clinical application as reported during the 2005' annual ASTRO meeting and its prospected for future development.
Brachytherapy by Percutaneous Paracentesis Implantation of Iodine-125 Seeds for Lung Cancer

Abstract:Objective To evaluate the efficacy and safety of brachytherapy by percutaneous paracentesis implantation of iodine 125 seeds for lung cancer. Methods Twenty patients with peripheral lung cancer were undergone brachytherapy by percutaneous paracentesis implantation of iodine 125 seeds. The effectiveness was evaluated by periodically chest film or CT. The complications were analyzed. Results Eighteen of the twenty patients were observed for more than 2 months. CR, PR and NC were seen in 27.8% (5/18), 66.7% (12/18) and 5.6%(1/18) respectively of the patients. Pneumothorax, hemoptysis and pleural cavity bleeding were seen in 20%(4/20),25%(5/20) and 5%(1/20)respectivly of the patients. Conclusion Brachytherapy by percutaneous paracentesis implantation of iodine 125 seeds for lung cancer is quite effective. The complications are easy to treat.

Abstract:ObjectiveTo explore the application of a new radioactive seed template retainer in the learning curve of CT-guided 125I seed brachytherapy (CTISBT) for advanced non-small cell lung cancer (NSCLC).MethodsThis study retrospectively analyzed the medical records of 60 patients who underwent CTISBT for advanced NSCLC by a single physician between January 2018 and December 2019. The data were sorted in order of admission and divided into three groups according to the order of surgery, group A (cases 1–20), group B (cases 21–40), and group C (cases 41–60). All patients underwent preoperative planning and postoperative dosimetry verification, and the operation time, intraoperative CT scans, postoperative hospital stay, and postoperative complications were compared among the three groups. The quality of life (QOL) score and tumor volume were compared before and 2 months after surgery.ResultsThere were no statistically significant differences among the three groups in QOL scores, tumor volume, and tumor site before CTISBT assisted by the new radioactive seed template retainer. However, the surgical time differed significantly between the three groups (P < 0.01). The operation time was longer in group A than that in groups B and C (P < 0.01). There was no significant difference between groups B and C. There were no significant differences in the number of CT scans among the three groups and the length of postoperative hospital stay. The follow-up QOL and tumor volume were significantly reduced at 2 months after surgery compared with those before surgery (P < 0.01).ConclusionsThe short-term efficacy of the new radioactive seed template retainer-assisted CTISBT was definitive for advanced NSCLC. After the physician had accumulated experience with 20 cases of a new type of radioactive seed template retainer-assisted CTISBT surgery, the follow-up operation time was significantly shortened and the learning curve entered the plateau stage.

Abstract:Purpose: The purpose is to evaluate the clinical efficacy and safety of computerized tomography (CT)-guided 125I seed implantation in the treatment of recurrent non-small cell lung cancer (NSCLC) after chemoradiotherapy. Materials and Methods: We retrospectively analyzed the data of 30 recurrent NSCLC patients in our institute from January 2016 to June 2020. According to the preoperative Treatment planning system plan, CT was used to guide 125I seed implantation into 30 evaluable lesions in the lungs. Clinical response rate, quality of life score, and adverse reactions were observed at postoperative follow-up. Results: The postoperative follow-up duration was 13 (1–24) months, of which the disease control rate at months one, three, and six were 96.67%, 93.1%, 85.18%, respectively and the objective response rate was 53.33%, 48.27%, and 48.14%, respectively. The postoperative 1-year and 2-year survival rates were 76.66% (23/30) and 53.33% (16/30), respectively. Median overall survival was 18 (1–24) months. The postoperative 1-year and 2-year progression-free survival (PFS) rates were 63.33% (19/30) and 40% (12/30), respectively. The median PFS was 14.5 (1–24) months. Adverse reactions include radiation-related pulmonary reactions in four patients (13.33%); skin reactions in four patients (13.33%); radiation-related esophageal reactions in two patients (6.67%), and leukopenia in three patients (10%). Other radiation-related adverse reactions did not occur. Conclusion: We conclude that 125 I seed implantation is an effective and safe treatment for recurrent NSCLC.

Abstract:Lung cancer is the second most common, and the deadliest, disease globally. Because it is often diagnosed late, surgical resection is not a viable treatment option for ∼75 % of patients, often resulting in a poor prognosis. Of the available treatments, radioactive iodine-125 (125-I) seed implantation therapy, or brachytherapy, has emerged as a promising option. In this procedure, small radioactive seeds are implanted inside tumor cells to produce sustained effects. Because of the short radial distance of this radiation, 125-I brachytherapy selectively and efficiently kills cancer cells while minimizing injury to adjacent cells. The present review describes the mechanism of 125-I seed implantation in the treatment of lung cancer, its efficacy and safety, and its combination with other therapies. We conclude that radioactive 125-I seed implantation and its use in combination with other therapies are good options for the management of local tumor growth, pain palliation, and improving the life span of patients suffering from lung cancers. This technique can enhance the clinical efficacy of treatment and improve the overall survival of patients with lung cancers. However, standardized dosage regimens and other procedures are still required to achieve treatment homogeneity and provide guidance for the clinical implementation of this technique.

Abstract:BACKGROUND:Microwave ablation and radioactive seed implantation are therapeutic options for patients with advanced lung cancer and lung metastases lesion who are not surgical candidates. However, reports on therapeutic effectiveness and safety of the two combination treatments methods have been limited to small case series. This research was to assess the clinical outcome and safety of radioactive seed implantation combined with computed tomography (CT)-guided microwave ablation for the treatment of advanced lung cancer and lung metastases lesion.METHODS:The clinical data of 21 advanced lung cancer and lung metastases patients who received treatment at author hospital during the period from May 2018 to December 2018 were retrospectively analyzed. The patients were divided into two groups by whether received extra percutaneous microwave thermal ablation therapy. The short-term effectiveness and safety were analyzed.RESULTS:The efficacy of control group was 28.75%, and the efficacy of experiment group was 14.28%. The rate of adverse reaction between two groups has no significantly difference (P>0.05).CONCLUSIONS:Seed implantation combined with CT-guided microwave ablation for advanced lung cancer and lung metastases is safety and effective.

Abstract:As an interstitial brachytherapy,radioactive seed implantation could provide high doses in the local site and minimal doses at surrounding normal tissues.It has become one of the best choice for early stage prostate carcinoma.Radioactive seeds were implanted under the guidance by ultrasound,CT and MRI,featured with surgical and interventional treatment.Based on dosimetry for target and organs at risk,radioactive seed implantation is multiple disciplinary.In order to acquire the accurate and high quality seed implantation,it is necessary to set up a team including surgeons,radiation oncologists,interventional doctors and nuclear medicine doctors.The content of the consensus is as follows:radiation physics and dosimetry,indications,side-effects and 3 D-printing template work-follow.Despite the benefit of radioactive seed implantation for solid carcinoma,there still a compelling need for prospective randomized and stage Ⅲ clinical trials from multiple centers,so as to upgrade the evidencebased level,above all confirm the role of radioactive seed implantation in the comprehensive treatment of tumors.

Abstract:Targeted drug therapy plays an important role in the clinical application of non-small cell lung cancer, especially adenocarcinoma. However, for patients with advanced disease, drug resistance after targeted therapy, unclear target, and other reasons that cannot or do not want surgery, the combination of chemotherapy, radiotherapy, immunity, etc. is often used. The synergistic effect of targeted drugs and radiotherapy in non-small cell lung cancer has shown good clinical efficacy. This article reviews the clinical progress of targeted drug therapy combined with radiotherapy in advanced non-small cell lung cancer in recent years, in order to provide new ideas for further clinical research of this treatment mode.
Clinical outcomes of radioactive seed brachytherapy and microwave ablation in inoperable stage I non-small cell lung cancer

Abstract:This study assessed the efficacy and safety of radioactive iodine-125 seed ablation brachytherapy (RSABT) in comparison to microwave ablation therapy (MWAT) for treating inoperable stage I non-small cell lung cancer (NSCLC). We conducted a retrospective analysis of data from stage I NSCLC patients who underwent CT-guided RSABT or MWAT. The primary outcomes measured were progression-free survival (PFS), overall survival (OS), and the occurrence of adverse events. Of the patients included in the study, 71 underwent RSABT and 105 received MWAT. The median follow-up time for these groups was 47.4 months and 60 months, respectively. The PFS rates at 1-year, 3-year, and 5-year for the RSABT group were 87.3%, 72.6%, and 65.8%, while for the MWAT group, they were 89.5%, 69.3%, and 43.7%, respectively (P = 0.011). The OS rates at 1-year, 3-year, and 5-year for the RSABT group were 97.2%, 78.1%, and 66.1%, and for the MWAT group, they were 99%, 75.8%, and 55%, respectively (P = 0.112). Upon multivariate analysis, the treatment modality was identified as an independent predictor of PFS (P = 0.008). Additionally, both sex and T stage were found to be independent predictors of both PFS and OS (P < 0.05). Adverse events, such as pneumothorax, occurred in 50% of the MWAT group and 39% of the RSABT group (P = 0.313). The incidence of pleural effusion was 44% in the MWAT group compared to 14% in the RSABT group (P < 0.001). Needle bleeding was observed in 32% of the RSABT group and 5% of the MWAT group (P < 0.001). We conclude RSABT demonstrates promising efficacy and safety in the treatment of stage I NSCLC. However, further studies are essential to validate these preliminary findings.

Abstract:Advanced non-small cell lung cancer (NSCLC) with MET amplification primarily relies on MET inhibitors for treatment, but once resistance occurs, the available treatment options are limited and the prognosis is typically poor. A 57-year-old man with advanced NSCLC and C-MET amplification was initially treated with crizotinib but developed progressive disease. After the antirotinib treatment, he achieved a partial response for a year. Genetic testing showed high PD-L1 expression, and he was treated with pembrolizumab and chemotherapy for 3 months, with partial response. Maintenance therapy with pembrolizumab and local I-125 seeds brachytherapy (ISB) was given after the lung lesion progressed but other lesions remained stable. The therapy resulted in significant resolution of the right upper lung lesion. It demonstrates the effectiveness of ISB-ICI combination in treating MET amplification advanced NSCLC. Ongoing research and treatment innovation are important in managing advanced NSCLC with complex genetic aberrations. To explore the candidate mechanism of ISB therapy response, we download public genetic data and conduct different expression Lncrnas analysis and pathway analysis to discover radiotherapy related sensitive or resistance lncRNAs and pathways, we found that AL654754.1 is a key lncRNA with radiotherapy response, and it also include in classical p53 and Wnt signaling pathway. Overall, the clinical case reports, combined with the exploration of underlying mechanisms, provide positive guidance for the precise treatment of lung cancer.

Abstract:Radiation therapy has been used for over a century in the treatment of tumors, with interstitial tissue treatment using radioactive seeds playing a key role in this approach. Iodine-125 (I-125) seeds are the most commonly used radioactive sealed source for permanent interstitial brachytherapy. In recent decades, significant advancements have been made in the field of interstitial radiation therapy. The development of three-dimensional (3D)-printed personalized templates, treatment planning systems, and robot-assisted systems have significantly improved the accuracy of I-125 brachytherapy. This review summarizes the advances in technology, radiobiology, physics, and immunology of I-125 brachytherapy. These advancements have improved the accuracy of dose delivery and increased the effectiveness of I-125 interstitial brachytherapy. In particular, the utilization of 3D-printed personalized templates has allowed for customized treatment planning and more precise dose delivery. Robot-assisted systems have also made significant contributions by assisting in the precise placement of radioactive seeds during treatment. However, several challenges persist within the field of interstitial I-125 brachytherapy. One of the current issues is the difficulty in accurately predicting the biological response to radiation therapy in individual patients. Addressing this challenge represents an important area for further research, as it has the potential to improve treatment outcomes and minimize side effects. In addition, there is a need for more research into the utilization of immunotherapy in conjunction with interstitial brachytherapy, as this combination has demonstrated promise in preclinical studies. Overall, this review provides a comprehensive overview of the advances and challenges associated with interstitial brachytherapy using I-125 seeds. These advancements offer a theoretical basis for achieving precise and remote medical care in brachytherapy. As technology continues to evolve, it is likely that interstitial brachytherapy will emerge as an even more effective treatment option for cancer patients.

Abstract:Radioactive iodine-125 (I-125) is the most widely used radioactive sealed source for interstitial permanent brachytherapy (BT). BT has the exceptional ability to deliver extremely high doses that external beam radiotherapy (EBRT) could never achieve within treated lesions, with the added benefit that doses drop off rapidly outside the target lesion by minimizing the exposure of uninvolved surrounding normal tissue. Spurred by multiple biological and technological advances, BT application has experienced substantial alteration over the past few decades. The procedure of I-125 radioactive seed implantation evolved from ultrasound guidance to computed tomography guidance. Compellingly, the creative introduction of 3D-printed individual templates, BT treatment planning systems, and artificial intelligence navigator systems remarkably increased the accuracy of I-125 BT and individualized I-125 ablative radiotherapy. Of note, utilizing I-125 to treat carcinoma in hollow cavity organs was enabled by the utility of self-expandable metal stents (SEMSs). Initially, I-125 BT was only used in the treatment of rare tumors. However, an increasing number of clinical trials upheld the efficacy and safety of I-125 BT in almost all tumors. Therefore, this study aims to summarize the recent advances of I-125 BT in cancer therapy, which cover experimental research to clinical investigations, including the development of novel techniques. This review also raises unanswered questions that may prompt future clinical trials and experimental work.

Abstract:Objective To explore the efficacy of CT-guided radioactive seed implantation for recurrent rectal cancer.Methods Thirty patients with recurrent rectal cancer after surgery and radiotherapy underwent CT-guided radioactive seed implantation.Treatment planning system was applicated to confirm the seed number and the needle placement.The seeds were implanted at 0.5—1.0 cm apart.Postoperative quality evaluation was performed for patients as well.Results The number of seeds ranged from 9—136,and the radioactive activity was 0.50—1.60 mCi.The matched peripheral dose ranged from 110—160 Gy.The follow-up ranged from 3 to 75 months.The local control rate was 70.00%(21/30).The median local control time was 7.5 months,median survival time was 9.0 months.The 1-,2-and 3-year local control rates and survival rates were 62.7%,47.1%,47.1% and 30.9%,13.3%,6.6%,respectively.Numerical rating scales of pre-and postoperation were 6.32±2.15 and 2.77±1.77(P0.001).Pain relief rate was 100%(22/22).Conclusion As a salvage therapy,CT-guided brachytherapy is safe,minimal invasive and effective for recurrent rectal cancer.
Image-guided Permanent I-125 Seed Implantation for Refractory Head-and-neck Carcinoma in China: A Comprehensive Review of Contemporary Technical and Clinical Aspects

Abstract:Permanent I-125 seed implantation is one of the most commonly used brachytherapy (BT) forms, potentially applicable to almost any solid tumor as a standalone anti-tumor therapy or in combination with external beam radiotherapy (EBRT). The main advantage of I-125 seed implantation BT is its ability to deliver a high radiation dose within target volumes while maintaining a sharp dose drop-off profile, thereby protecting adjacent normal tissue. The clinical use of I-125 seed implantation in China has significantly increased in the past decades due to the application of an image-guided BT treatment planning system with reverse optimization capabilities and 3D printing individual template (3D-PIT) assistance. Computed tomography-guided I-125 seed implantation became a widely common modality of BT and has been used in many clinical settings for a variety of refractory cancer in China since 2002. The application has been expanded significantly not only for prostate cancer as monotherapy but also in combination with EBRT for the salvage treatment for refractory/recurrent head-and-neck carcinoma (HNC). With the assistance of 3D-PIT, since 2015, the quality control of I-125 seed implantation has been significantly adapted with high precision and real-time dose optimization. This review aims to summarize the developments and status of 3D-PIT-based I-125 seed implantation BT for the treatment of refractory HNC and standardized workflow in China.

Abstract:Lung cancer is one of the most malignant health-threatening tumors.In China,CT-guided iodine-125 seed implantation has been applied in the treatment of lung cancer,and has shown significant efficacy.However,the techni-cal procedure is still not normalized.Northern China Radioactive Seeds Multi-center Cooperative Group (Lung Cancer Group)summarized the technical procedure of CT-guided iodine-125 seed implantation in the treatment of lung cancer, illustrating the surgical steps with graphs,hoping to provide guidance and reference for future work.
