131. Assessment of Cognitive Functions, Gait Affaction and Fatigue in Relapsing- remitting Multiple Sclerosis Patients

Abstract:Background/Objective(s) Multiple sclerosis (MS) is one of the most common disabling diseases of the central nervous system (CNS) especially during early adult life. In 70% of cases, this inflammatory, demyelinating, and neurodegenerative disease becomes clinically apparent between the ages of 20 and 40 years. It affects more than 2.3 million persons worldwide and is currently an incurable disease.Along with impaired walking, cognitive impairment occurs in about half of people with multiple sclerosis. Furthermore, fatigue is the most reported symptom. Material(s) and Method(s) A cross sectional descriptive study was conducted on 45 patients with relapsing- remitting multiple sclerosis (RRMS) who were assessed for cognitive functions, gait and fatigue who didn't develop relapses in their last 3 months using different functional scales. Result(s) Regarding Demographics and clinical characteristics of recruited patients, the age of patients was (32.76 ± 6.58 years, gender (73% females and 26.7% males) and education years (13.49 ± 2.47) years. Moreover, regarding age of onset it was (26.04 ± 5.82) and duration of illness (6.84 ± 3.10). Regarding number of attacks in last 2 years, it ranges, from 0-3 relapses and onset of last relapse ranged from (4-48) months. The recruited patients excluded to have moderately severe depression according to mood assessment and severity of PHQ9.Regarding gait assessment, walking speed affected in 100% of patients that was beyond cut off value of normal, disability by EDSS showed median 4.5 and balance parameters using berg balance test showed affection of 48.9% of patients. There was affection of cognitive functions in different domains. Regarding processing speed, 60% of patients were beyond the Egyptian cut off normal value, verbal memory affected in 57.8% of patients, while visuospatial domain affected in 6.7% of sample. Regarding scoring system of MOCA test, 53.3% of patients have moderate cognitive impairment, while frontal lobe functions using FAB scale showed affection in 15.6% of sampled patients. The enrolled patients reported subjective fatigue in its different domains in 97.8% using MFIS scale. Conclusion(s) The current study showed evidence of early affection in balance, cognition and fatigue in RRMS patients even though patients are on DMD management who may need further symptomatic treatment.
