Abstract:Dystrophic epidermolysis bullosa (DEB) is a severe monogenic skin disorder resulting from mutations in the COL7A1 gene, which disrupts the synthesis of type VII collagen. This leads to impaired anchoring fibrils and results in dermoepidermal separation. The clinical manifestation of DEB varies significantly, ranging from localized blistering in milder forms to extensive blistering with subsequent severe complications such as vision loss and squamous cell carcinoma in more severe cases. Despite the recent approval of the first gene replacement therapy by the U.S. Food and Drug Administration, the majority of DEB patients still depend on palliative care, an indication of the continued unmet therapeutic needs. In the past two decades, there has been a rapid advancement of gene therapy techniques, extensive research efforts, and pre-clinical studies focusing on the correction of DNA, RNA, and protein defects specific to DEB. In this review, we provide a comprehensive update on the current state of gene engineering strategies for DEB, including gene replacement, pre-mRNA regulatory therapies, and gene editing techniques. In addition, this review critically evaluates the role and development of animal models in DEB research, which are crucial for the progression of therapeutic strategies. Our discussion aims to delineate the existing challenges and emphasize ongoing advancements in the gene therapy landscape for DEB, providing insights that may guide future research and clinical approaches.

Abstract:Epidermolysis bullosa (EB) is a group of rare genetic skin fragility disorders, which are hereditary. These disorders are associated with mutations in at least 16 genes that encode components of the epidermal adhesion complex. Currently, there are no effective treatments for this disorder. All current treatment approaches focus on topical treatments to prevent complications and infections. In recent years, significant progress has been achieved in the treatment of the severe genetic skin blistering condition known as EB through preclinical and clinical advancements. Promising developments have emerged in the areas of protein and cell therapies, such as allogeneic stem cell transplantation; in addition, RNA-based therapies and gene therapy approaches have also become a reality. Stem cells obtained from embryonic or adult tissues, including the skin, are undifferentiated cells with the ability to generate, maintain, and replace fully developed cells and tissues. Recent advancements in preclinical and clinical research have significantly enhanced stem cell therapy, presenting a promising treatment option for various diseases that are not effectively addressed by current medical treatments. Different types of stem cells such as primarily hematopoietic and mesenchymal, obtained from the patient or from a donor, have been utilized to treat severe forms of diseases, each with some beneficial effects. In addition, extensive research has shown that gene transfer methods targeting allogeneic and autologous epidermal stem cells to replace or correct the defective gene are promising. These methods can regenerate and restore the adhesion of primary keratinocytes in EB patients. The long-term treatment of skin lesions in a small number of patients has shown promising results through the transplantation of skin grafts produced from gene-corrected autologous epidermal stem cells. This article attempts to summarize the current situation, potential development prospects, and some of the challenges related to the cell therapy approach for EB treatment.

Abstract:The replacement of a defective gene with a fully functional copy is the goal of the most basic gene therapy. Recessive dystrophic epidermolysis bullosa (RDEB) is characterised by a lack of adhesion of the epidermis to the dermis. It is an ideal target for gene therapy as all variants of hereditary RDEB are caused by mutations in a single gene, COL7A1, coding for type VII collagen, a key component of anchoring fibrils that secure attachment of the epidermis to the dermis. RDEB is one of the most severe variants in the epidermolysis bullosa (EB) group of heritable skin diseases. Epidermolysis bullosa is defined by chronic fragility and blistering of the skin and mucous membranes due to mutations in the genes responsible for production of the basement membrane proteins. This condition has a high personal, medical and socio-economic impact. People with RDEB require a broad spectrum of medications and specialised care. Due to this being a systemic condition, most research focus is in the area of gene therapy. Recently, preclinical works have begun to show promise. They focus on the virally mediated ex vivo correction of autologous epithelium. These corrected cells are then to be expanded and grafted onto the patient following the lead of the first successful gene therapy in dermatology being a grafting of corrected tissue for junctional EB treatment. Current progress, outstanding challenges and future directions in translating these approaches in clinics are reviewed in this article.

Abstract:The monogenetic disease epidermolysis bullosa (EB) is characterised by the formation of extended blisters and lesions on the patient's skin upon minimal mechanical stress. Causal for this severe condition are genetic mutations in genes, leading to the functional impairment, reduction, or absence of the encoded protein within the skin's basement membrane zone connecting the epidermis to the underlying dermis. The major burden of affected families justifies the development of long-lasting and curative therapies operating at the genomic level. The landscape of causal therapies for EB is steadily expanding due to recent breakthroughs in the gene therapy field, providing promising outcomes for patients suffering from this severe disease. Currently, two gene therapeutic approaches show promise for EB. The clinically more advanced gene replacement strategy was successfully applied in severe EB forms, leading to a ground-breaking in vivo gene therapy product named beremagene geperpavec (B-VEC) recently approved from the US Food and Drug Administration (FDA). In addition, the continuous innovations in both designer nucleases and gene editing technologies enable the efficient and potentially safe repair of mutations in EB in a potentially permanent manner, inspiring researchers in the field to define and reach new milestones in the therapy of EB.

Abstract:Background/purpose Dystrophic epidermolysis bullosa (DEB), a rare genetic skin disease caused by loss-of-function mutations in COL7A1 , the gene encoding type VII collagen (COL7), is characterized by skin blistering, scarring, and extracutaneous manifestations that markedly reduce patient quality-of-life. Beremagene geperpavec-svdt ('B-VEC') is a gene therapy employing a non-integrating, replication-defective herpes simplex virus type 1 (HSV-1)-based vector encoding two copies of full-length human COL7A1 to restore COL7 protein after topical administration to DEB wounds. B-VEC was approved in the United States in 2023 as the first topical gene therapy and the first approved treatment for DEB. However, few providers have experience with use of this gene therapy.

Abstract:We describe a 41-year-old Asian male who presented with a left upper eyelid mass for one year associated with multiple pigmented maculopapules on his face, who was found to have Epidermodysplasia verruciformis (EV). The ophthalmic examination was unremarkable. The patient spontaneously started to have disseminated, bean-sized, pinkish flat papules on his forehead since childhood and had history of multiple non-melanoma skin carcinoma. He had been treated as possible xeroderma pigmentosum (XP) by dermatologists. The oculoplastic surgeon resected the lesion and submitted tissue for histopathological and genetic analysis. Eyelid biopsy was consistent with a squamous cell carcinoma (SCC). Next-generation Sequencing (Sanger sequencing) suggested the homozygous mutation of TMC8 gene which is indicative of EV. Our case is a rare report in eyelid SCC in the setting of EV. This patient highlights the importance of considering systemic genodermatosis rather than individual eyelid malignancy and the function of molecular diagnosis in clinical challenge case.

Abstract:Monogenic skin diseases with chronic wounds are an excellent target for non-viral gene therapy due to the ease of application to the site, large gene delivery capacity, potentially low immunogenicity and the easy cost effective manufacturing processes of the non-viral technology. Recessive Dystrophic Epidermolysis Bullosa (RDEB) is a chronic blistering skin disorder caused by a mutation in COL7A1. The main barrier to clinical translation of non-viral gene therapy is the development of delivery vectors that can achieve therapeutically relevant transfection efficiency whilst maintaining biocompatibility. Our aim is focused on the use of a cationic polymer based delivery platform. In-vitro screening in RDEB-TA4 keratinocytes facilitated the identification of the optimal polymer. Using the transfection reporter assays, green fluorescent protein and gaussia luciferase expression enabled the efficient identification of the optimal polymer/DNA weight ratio formulation which far exceeded the transfection efficiency of leading commercial reagents branched Polyethylenimine and Superfect. Moreover, biocompatibility of reagents was assessed in vivo, demonstrating minimal adverse events after repeated intradermal injections of this biodegradable polymer. Given the multiple parameters that enable successful gene delivery, numerous physicochemical properties of formulation were assessed that would be indicators of efficient and safe gene delivery, such as polyplex size, surface charge and morphology. The optimal polyplex formulation was able to self assemble into nanoparticles of around 100nm with a positive surface charge (+12mV) for efficient uptake into cells and excellent buffering capacity to escape from endosomes for successful gene delivery. This novel group of promising non-viral gene delivery vectors displays high transfection efficiency and safety in in-vivo studies and represents a potentially new delivery platform for the treatment of RDEB.

Abstract:Recessive dystrophic epidermolysis bullosa (RDEB) is an autosomal monogenic skin disease caused by mutations in COL7A1 gene and lack of functional type VII collagen (C7). Currently, there is no cure for RDEB, and most of the gene therapies under development have been designed as ex vivo strategies because of the shortage of efficient and safe carriers for gene delivery. Herein, we designed, synthesized, and screened a new group of highly branched poly(β amino ester)s (HPAEs) as non-viral carriers for the delivery of plasmids encoding dual single-guide RNA (sgRNA)-guided CRISPR-Cas9 machinery to delete COL7A1 exon 80 containing the c.6527dupC mutation. The selected HPAEs (named PTTA-DATOD) showed robust transfection efficiency, comparable with or surpassing that of leading commercial gene transfection reagents such as Lipofectamine 3000, Xfect, and jetPEI, while maintaining negligible cytotoxicity. Furthermore, CRISPR-Cas9 plasmids delivered by PTTA-DATOD achieved efficient targeted deletion and restored bulk C7 production in RDEB patient keratinocyte polyclones. The non-viral CRISPR-Cas9-based COL7A1 exon deletion approach developed here has great potential to be used as a topical treatment for RDEB patients with mutations in COL7A1 exon 80. Besides, this therapeutic strategy can easily be adapted for mutations in other COL7A1 exons, other epidermolysis bullosa subtypes, and other genetic diseases.

Abstract:Many genodermatoses in children present initially with lesions in the head and neck. These known inherited skin disorders have accompanying molecular changes which can be used for targeted therapy. In 2023, a topical treatment for dystrophic epidermolysis bullosa was approved as a herpes simplex virus type 1 vector-based gene therapy. Functional copies of the deficient COL7A gene, in the case of dystrophic epidermolysis bullosa promote wound healing and the production of collagen. The aim of this literature review is to review pediatric genodermatoses which affect the head and neck and review their genetic mutations in the context of proposed and novel gene therapy targets.

Abstract:We present Dystrophic Epidermolysis Bullosa Cell Therapy (DEBCT), a scalable platform producing autologous organotypic iPS cell-derived induced skin composite (iSC) grafts for definitive treatment. Clinical-grade manufacturing integrates CRISPR-mediated genetic correction with reprogramming into one step, accelerating derivation of COL7A1-edited iPS cells from patients. Differentiation into epidermal, dermal and melanocyte progenitors is followed by CD49f-enrichment, minimizing maturation heterogeneity. Mouse xenografting of iSCs from four patients with different mutations demonstrates disease modifying activity at 1 month. Next-generation sequencing, biodistribution and tumorigenicity assays establish a favorable safety profile at 1-9 months. Single cell transcriptomics reveals that iSCs are composed of the major skin cell lineages and include prominent holoclone stem cell-like signatures of keratinocytes, and the recently described Gibbin-dependent signature of fibroblasts. The latter correlates with enhanced graftability of iSCs. In conclusion, DEBCT overcomes manufacturing and safety roadblocks and establishes a reproducible, safe, and cGMP-compatible therapeutic approach to heal lesions of DEB patients.

Abstract:Recessive dystrophic epidermolysis bullosa (RDEB) is a rare autosomal inherited skin blistering disorder caused by mutations in the type VII collagen gene (COL7A1). The large size of COL7A1 is a big delivery challenge for both viral and non-viral vectors and seriously hindered the development of therapeutic gene replacement therapy for RDEB. Our group has developed a non-viral minicircle DNA mediated gene replacement strategy for RDEB, which achieved high transfection efficiency while safety is improved by avoiding the usage of viruses and the removal of bacterial backbone from the plasmid DNA. Here we further synthesized a new hyper branched aminated polyesters (HAPE) with greater DNA delivery ability, higher gene expression efficiency and better biocompatibility, which performed better than commercial reagents like lipofectamine 3000 and JetPEI. The HAPE delivered minicircle DNA coding full COL7A1 gene successfully restored type VII collagen expression in RDEB keratinocytes. This improved system developed here has a high potential for use as an efficient and safe non-viral topical treatment for RDEB patients in the clinic and can be adapted to other genetic diseases.

Abstract:EBS is an inherited, skin fragility disorder predominantly caused by dominant-negative mutations in genes encoding for the cytoskeletal proteins, Keratin5 and Keratin14 within the basal cell layer. To date, EBS is incurable with only symptomatic therapies currently available. The downregulation of these mutant genes would provide an excellent curative therapy for treating EBS. siRNA therapeutics have been identified as an attractive therapy option for EBS given the highly accessible nature of skin tissue. The biggest hurdle thus far has been the identification of an effective system to deliver siRNA's into the skin. Among non-viral delivery vector systems, polymers, in particular cationic polymers are some of the most extensively studied delivery systems for RNA payloads. Cationic polymers have the ability to electrostatically bind to the negatively charged RNA. The ideal delivery system would be an amalgamation of the structural flexibility and stability of highly branched cationic polymers with the membrane interaction profile of lipids to yield a delivery system that will be both safe and effective. Utilizing the tunable nature of our novel highly branched polymer based delivery system provides an excellent platform to screen and assess these siRNA delivery vectors. Thus far we have successfully encapsulated a siRNA into a lipid-like nanoparticle through conjugation with a highly branched polymer with promising preliminary data indicating a safe and efficient knockdown of a target gene of interest. Further comprehensive analysis of the delivery system shall involve assaying physicochemical properties identified as key indicators of in-vitro transfection success.

Abstract:R dystrophic epidermolysis bullosa (RDEB) has been defined as severe chronic skin fragility and caused by mutations in COL7A1, which encodes for the elastic structural protein type VII collagen (C7). The 8.9 Kb COL7A1 transcript is particularly a large sequence with many repeating units which makes it difficult to manipulate and package into viral systems. Therefore, the minicircle system is ideal for use with COL7A1, firstly to minimize the overall DNA construct size while secondly increasing the safety profile of the gene therapy. We successfully inserted COL7A1 into the parental plasmid MN512A1 and combined it with our highly efficient a non-viral vector (HPAE). HPAE-MC-COL7A1 polyplexes successfully produced significant levels of recombinant C7 with negligible cytotoxicity in RDEB-TA4 keratinocytes. Minimal effect was seen on primary keratinocyte metabolic health, even after multiple applications of HPAE polyplexes. Furthermore, in vivo transfection studies revealed that HPAE carrying MC-COL7A1 restores the expression of C7 along the basement membrane zone in a human RDEB graft mouse model after intradermal injection and topical application. Of the 7 animals treated, 5 were positive for recombinant C7, with all animals receiving 2 or more applications having strong positive signal. While further assessment is required to prove this approach is safe and well tolerated in the long term, HPAE-MC-COL7A1 polyplexes showed great promise as a potential therapeutic for RDEB.

Abstract:RDEB is one of the most severe subtypes of EB, caused by mutations in COL7A1, which encodes for type VII collagen (C7), defined by fragility of the skin and epithelial tissues. The 8.9Kb COL7A1 transcript is a particularly large sequence with many repeating units which makes it difficult to manipulate and package into viral systems. Among non-viral delivery systems, cationic polymer vectors offer significant advantages over virus’ owing to their ability to condense DNA into nanoparticles. HPAE-EB is a cationic polymer, which binds electrostatically with negatively charged DNA (COL7a1) to form nanoscale polyplexes. In vitro studies revealed that HPAE-EB+COL7A1 polyplexes are well tolerated by healthy primary human and RDEB keratinocytes and more efficient at delivery compared to commercial vectors. Safety studies in SKH1 mice did not show any toxicology difference between the application of the controls (untreated and buffer) and HPAE-EB+COL7A1 polyplexes or HPAE-EB alone in a single intravenous dose (n=3) or multiple intradermal doses (n=11). Polyplexes made with PEI (positive control for toxicity) were irritant and caused skin damage, no negative effects were seen after injection of the HPAE-EB+COL7A1 polyplexes. Thus far a pilot study has begun using a human RDEB graft mouse model (n=5) to assess the efficiency of multiple topical applications with further comprehensive analysis to evaluate long term C7 expression. HPAE-EB+COL7A1 polyplexes show great promise as a potential topical therapeutic for treating the as of yet incurable RDEB, in addition to being gentle on the skin thereby adding any additional pain to the patients. Moreover this non-viral gene delivery system has broad spectrum applicability and can be used as a platform delivery system for other monogenic disorders.

Abstract:Introduction: Epidermolysis bullosa is a severe genodermatosis in which pain and wound management dominate patients’ lives leaving a high demand for more effective treatments. At present, gene therapy is the only treatment with the potential to cure inherited diseases. A small number of individuals with junctional and dystrophic forms of epidermolysis bullosa have received gene replacement therapies involving ex vivo viral transduction of intact transgenes into skin stem cells followed by autologous grafting of corrected epidermal sheets. Corrected stem cells ensure permanent regeneration of stable skin at the graft sites. Areas covered: Alternative therapeutic options are either RNA-based or involve the latest preclinical developments in the area of gene editing using designer nucleases such as TALEN or CRISPR/Cas9, which enable precise targeting of any desired locus and a potential traceless repair of mutations. Due to low targeting efficiencies and safety considerations regarding off-target effects, research in this area focuses on ex vivo approaches involving selection of correctly modified keratinocyte clones. Expert opinion: Although there is currently no single optimal therapy for epidermolysis bullosa, cell and gene therapy technologies are advancing rapidly holding great potential for modifying disease severity and improving quality of life for people living with this devastating disease.

Abstract:Background: Gene editing in induced pluripotent stem (iPS) cells has been hailed to enable new cell therapies for various monogenetic diseases including dystrophic epidermolysis bullosa (DEB). However, manufacturing, efficacy and safety roadblocks have limited the development of genetically corrected, autologous iPS cell-based therapies. Methods: We developed Dystrophic Epidermolysis Bullosa Cell Therapy (DEBCT), a new generation GMP-compatible (cGMP), reproducible, and scalable platform to produce autologous clinical-grade iPS cell-derived organotypic induced skin composite (iSC) grafts to treat incurable wounds of patients lacking type VII collagen (C7). DEBCT uses a combined high-efficiency reprogramming and CRISPR-based genetic correction single step to generate genome scar-free, COL7A1 corrected clonal iPS cells from primary patient fibroblasts. Validated iPS cells are converted into epidermal, dermal and melanocyte progenitors with a novel 2D organoid differentiation protocol, followed by CD49f enrichment and expansion to minimize maturation heterogeneity. iSC product characterization by single cell transcriptomics was followed by mouse xenografting for disease correcting activity at 1 month and toxicology analysis at 1-6 months. Culture-acquired mutations, potential CRISPR-off targets, and cancer-driver variants were evaluated by targeted and whole genome sequencing. Findings: iPS cell-derived iSC grafts were reproducibly generated from four recessive DEB patients with different pathogenic mutations. Organotypic iSC grafts onto immune-compromised mice developed into stable stratified skin with functional C7 restoration. Single cell transcriptomic characterization of iSCs revealed prominent holoclone stem cell signatures in keratinocytes and the recently described Gibbin-dependent signature in dermal fibroblasts. The latter correlated with enhanced graftability. Multiple orthogonal sequencing and subsequent computational approaches identified random and non-oncogenic mutations introduced by the manufacturing process. Toxicology revealed no detectable tumors after 3-6 months in DEBCT-treated mice. Interpretation: DEBCT successfully overcomes previous roadblocks and represents a robust, scalable, and safe cGMP manufacturing platform for production of a CRISPR-corrected autologous organotypic skin graft to heal DEB patient wounds.

Abstract:Rare eye diseases encompass a broad spectrum of genetic anomalies with or without additional extraocular manifestations. Genetic eye disorders in pediatric patients often lead to severe visual impairments. Therefore, a challenge of gene therapy is to provide better vision to these affected children. In recent years, inherited retinal diseases, inherited optic neuropathies, and corneal dystrophies have dominated discussions to establish gene and cell replacement therapies for these diseases. Gene therapy involves the transfer of genetic material to remove, replace, repair, or introduce a gene, or to overexpress a protein, whose activity would have a therapeutic impact. For the majority of anterior segment diseases, these studies are still emerging at a preclinical stage; however, for inherited retinal disorders, translation has been reached, leading to the introduction of the first gene therapies into clinical practice. In the past decade, the first gene therapy for biallelic RPE65-mediated inherited retinal dystrophy has been developed and the FDA and EMA both approved ocular gene therapy. Other promising approaches by intravitreal injection have been investigated such as in CEP290-Leber congenital amaurosis. Various techniques of gene therapies include gene supplementation, CRISPR-based genome editing, as well as RNA modulation and optogenetics. Optogenetic therapies deliver light-activated ion channels to surviving retinal cell types in order to restore photosensitivity. Beyond retinal function, ataluren, a nonsense mutation suppression therapy, enables ribosomal read-through of mRNA containing premature termination codons, resulting in the production of a full-length protein. An ophthalmic formulation was recently evaluated with the aim of repairing corneal damage, pending new clinical studies. However, various congenital disorders exhibit severe developmental defects or cell loss at birth, limiting the potential for viral gene therapy. Therefore mutation-independent strategies seem promising for maintaining the survival of photoreceptors or for restoring visual function. Restoring vision in children with gene therapy continues to be a challenge in ophthalmology. © 2023 Published by Elsevier Masson SAS on behalf of French Society of Pediatrics.
