Abstract:The clinical assessment of microvascular pathologies (in diabetes and in inflammatory skin diseases, for example) requires the visualization of superficial vascular anatomy. Photoacoustic tomography (PAT) scanners based on an all-optical Fabry–Perot ultrasound sensor can provide highly detailed 3D microvascular images, but minutes-long acquisition times have precluded their clinical use. Here we show that scan times can be reduced to a few seconds and even hundreds of milliseconds by parallelizing the optical architecture of the sensor readout, by using excitation lasers with high pulse-repetition frequencies and by exploiting compressed sensing. A PAT scanner with such fast acquisition minimizes motion-related artefacts and allows for the volumetric visualization of individual arterioles, venules, venous valves and millimetre-scale arteries and veins to depths approaching 15 mm, as well as for dynamic 3D images of time-varying tissue perfusion and other haemodynamic events. In exploratory case studies, we used the scanner to visualize and quantify microvascular changes associated with peripheral vascular disease, skin inflammation and rheumatoid arthritis. Fast all-optical PAT may prove useful in cardiovascular medicine, oncology, dermatology and rheumatology.

Opto-Ultrasound Imaging in Vivo in Deep Tissue

Abstract:It is of keen importance of deep tissue imaging with high resolution in vivo. Here we present an opto-ultrasound imaging method which utilizes an ultrasound to confine the laser pulse in a very tiny spot as a guide star. The results show that the imaging depth is 2mm with a resolution of 10um. Meanwhile, the excitation power we used is less than 2mW, which indicates that our methods can be applied in vivo without optical toxicity and optical bleaching due to the excitation power.
LightSpeed: A Compact, High-Speed Optical-Link-Based 3D Optoacoustic Imager

Abstract:Wide-scale adoption of optoacoustic imaging in biology and medicine critically depends on availability of affordable scanners combining ease of operation with optimal imaging performance. Here we introduce LightSpeed: a low-cost real-time volumetric handheld optoacoustic imager based on a new compact software-defined ultrasound digital acquisition platform and a pulsed laser diode. It supports the simultaneous signal acquisition from up to 192 ultrasound channels and provides a hig-bandwidth direct optical link (2x 100G Ethernet) to the host-PC for ultra-high frame rate image acquisitions. We demonstrate use of the system for ultrafast (500Hz) 3D human angiography with a rapidly moving handheld probe. LightSpeed attained image quality comparable with a conventional optoacoustic imaging systems employing bulky acquisition electronics and a Q-switched pulsed laser. Our results thus pave the way towards a new generation of compact, affordable and high-performance optoacoustic scanners.

Abstract:Structural microvascular alterations and dysfunction serve as key disease indicators of cancer, diabetes, ischemic stroke, neurodegenerative disorders and many other conditions. In vivo visualization of the microvasculature has traditionally been restricted to millimeter scale depths accessible with optical microscopy. Optoacoustic imaging has enabled breaking through the barrier imposed by light diffusion to map functional hemodynamic parameters in deep-seated vessels, but diffraction and dispersion of ultrasound waves in heterogeneous living tissues prevents reaching capillary resolution. Herein, we demonstrate three-dimensional microangiography of deep mouse brain beyond the acoustic diffraction limit (<20$\mu$m resolution) through the intact scalp and skull via optoacoustic localization of sparsely-distributed highly absorbing microparticles. This was enabled by devising 5$\mu$m sized extremely absorbing dichloromethane microdroplets exhibiting four orders of magnitude higher optical absorption than red blood cells at near-infrared wavelengths, thus facilitating in vivo compatibility and single particle sensitivity in the presence of highly absorbing blood background. Accurate mapping of the blood flow velocity within microvascular structures was also facilitated by the high 3D frame rate of the optoacoustic tomography system. We further show that the detected optoacoustic signal intensities from the localized particles may serve for estimating the light fluence distribution within optically heterogeneous tissues, a long-standing quantification challenge in biomedical optics. Given the intrinsic sensitivity of optoacoustics to various functional, metabolic and molecular events in living tissues, this new approach paves the way for non-invasive deep-tissue microscopic observations with unrivaled resolution, contrast and speed.
The sound of drug delivery: Optoacoustic imaging in pharmacology

Abstract:Optoacoustic (photoacoustic) imaging offers unique opportunities for visualizing biological function in vivo by achieving high-resolution images of optical contrast much deeper than any other optical technique. The method detects ultrasound waves that are generated inside tissue by thermo-elastic expansion, i.e., the conversion of light absorption by tissue structures to ultrasound when the tissue is illuminated by the light of varying intensity. Listening instead of looking to light offers the major advantage of image formation with a resolution that obeys ultrasonic diffraction and not photon diffusion laws. While the technique has been widely used to explore contrast from endogenous photo-absorbing molecules, such as hemoglobin or melanin, the use of exogenous agents can extend applications to a larger range of biological and possible clinical applications, such as image-guided surgery, disease monitoring, and the evaluation of drug delivery, biodistribution, and kinetics. This review summarizes recent developments in optoacoustic agents, and highlights new functions visualized and potent pharmacology applications enabled with the use of external contrast agents.
Enabling the autofocus approach for parameter optimization in planar measurement geometry clinical optoacoustic imaging

Abstract:In optoacoustic (photoacoustic) tomography, several parameters related to tissue and detector features are needed for image formation, but they may not be known a priori. An autofocus (AF) algorithm is generally used to estimate these parameters. However, the algorithm works iteratively, therefore, it is impractical for clinical imaging with systems featuring planar geometry due to long reconstruction times. We have developed a fast autofocus (FAF) algorithm for optoacoustic systems with planar geometry that is much simpler computationally than the conventional AF algorithm. We show that the FAF algorithm required about 5 sec. to provide accurate estimates of the speed of sound in simulated data and experimental data obtained using an imaging system that is poised to enter the clinic. The applicability of FAF for estimating other image formation parameters is discussed. We expect the FAF algorithm to contribute decisively to the clinical use of optoacoustic tomography systems with planar geometry.

Abstract:The present chapter summarizes progress with optical methods that go beyond human vision. The focus is on two particular technologies: fluorescence molecular imaging and optoacoustic (photoacoustic) imaging. The rationale for the selection of these two methods is that in contrast to optical microscopy techniques, both fluorescence and optoacoustic imaging can achieve large fields of view, i.e., spanning several centimeters in two or three dimensions. Such fields of views relate better to human vision and can visualize large parts of tissue, a necessary premise for clinical detection. Conversely, optical microscopy methods only scan millimeter-sized dimensions or smaller. With such operational capacity, optical microscopy methods need to be guided by another visualization technique in order to scan a very specific area in tissue and typically only provide superficial measurements, i.e., information from depths that are of the order of 0.05-1 mm. This practice has generally limited their clinical applicability to some niche applications, such as optical coherence tomography of the retina. On the other hand, fluorescence molecular imaging and optoacoustic imaging emerge as more global optical imaging methods with wide applications in surgery, endoscopy, and non-invasive clinical imaging, as summarized in the following. The current progress in this field is based on a volume of recent review and other literature that highlights key advances achieved in technology and biomedical applications. Context and figures from references from the authors of this chapter have been used here, as it reflects our general view of the current status of the field.

Abstract:AbstractMiniaturised high-resolution imaging devices are valuable for guiding minimally invasive procedures such as vascular stent placements. Here, we present all-optical rotational B-mode pulse-echo ultrasound imaging. With this device, ultrasound transmission and reception are performed with light. The all-optical transducer in the probe comprised an optical fibre that delivered pulsed excitation light to an optical head at the distal end with a multi-walled carbon nanotube and polydimethylsiloxane composite coating. This coating was photoacoustically excited to generate a highly directional ultrasound beam perpendicular to the optical fibre axis. A concave Fabry-Pérot cavity at the distal end of an optical fibre, which was interrogated with a tuneable continuous-wave laser, served as an omnidirectional ultrasound receiver. The transmitted ultrasound had a −6 dB bandwidth of 31.3 MHz and a peak-to-peak pressure of 1.87 MPa, as measured at 1.5 mm from the probe. The receiver had a noise equivalent pressure <100 Pa over a 20 MHz bandwidth. With a maximum outer probe diameter of 1.25 mm, the probe provided imaging with an axial resolution better than 50 µm, and a real-time imaging rate of 5 frames per second. To investigate the capabilities of the probe, intraluminal imaging was performed in healthy swine carotid arteries. The results demonstrate that the all-optical probe is viable for clinical rotational ultrasound imaging.

Abstract:Imaging is an essential tool in research, diagnostics and the management of endocrine disorders. Ultrasonography, nuclear medicine techniques, MRI, CT and optical methods are already used for applications in endocrinology. Optoacoustic imaging, also termed photoacoustic imaging, is emerging as a method for visualizing endocrine physiology and disease at different scales of detail: microscopic, mesoscopic and macroscopic. Optoacoustic contrast arises from endogenous light absorbers, such as oxygenated and deoxygenated haemoglobin, lipids and water, or exogenous contrast agents, and reveals tissue vasculature, perfusion, oxygenation, metabolic activity and inflammation. The development of high-performance optoacoustic scanners for use in humans has given rise to a variety of clinical investigations, which complement the use of the technology in preclinical research. Here, we review key progress with optoacoustic imaging technology as it relates to applications in endocrinology; for example, to visualize thyroid morphology and function, and the microvasculature in diabetes mellitus or adipose tissue metabolism, with particular focus on multispectral optoacoustic tomography and raster-scan optoacoustic mesoscopy. We explain the merits of optoacoustic microscopy and focus on mid-infrared optoacoustic microscopy, which enables label-free imaging of metabolites in cells and tissues. We showcase current optoacoustic applications within endocrinology and discuss the potential of these technologies to advance research and clinical practice.

Abstract:Changes in hemodynamic parameters are directly linked to biological function and physiological activity. Characterization of hemodynamics is commonly performed by Doppler ultrasound, which provides accurate measurements of blood flow velocity. Multi-spectral optoacoustic tomography is rapidly undergoing clinical translation fostered by its unique and complementary capacity for label-free mapping of the blood volume and the distribution of oxy- and deoxy-hemoglobin in blood. Here we report on a hybrid optoacoustic and ultrasound imaging approach that enables multi-modal imaging of blood flow and oxygen state using a multi-segment detector array. We further demonstrate rendering of multi-modal pulse-echo ultrasound, multi-spectral optoacoustic tomography, and color Doppler images from carotid artery of a healthy subject.

Abstract:Monitoring brain responses to ultrasonic interventions is becoming an important pillar of a growing number of applications employing acoustic waves to actuate and cure the brain. Optical interrogation of living tissues provides a unique means for retrieving functional and molecular information related to brain activity and disease-specific biomarkers. The hybrid optoacoustic imaging methods have further enabled deep tissue imaging with optical contrast at high spatial and temporal resolution. The marriage between light and sound thus brings together the highly complementary advantages of both modalities toward high precision interrogation, stimulation, and therapy of the brain with strong impact in the fields of ultrasound neuromodulation, gene and drug delivery, or noninvasive treatments of neurological and neurodegenerative disorders. In this review, we elaborate on current advances in optical and optoacoustic monitoring of ultrasound interventions. We describe the main principles and mechanisms underlying each method before diving into the corresponding biomedical applications. We identify areas of improvement as well as promising approaches with clinical translation potential.

Abstract:Photoacoustic imaging is a novel imaging modality that utilizes pulsed laser to illuminate samples and ultrasonic transducers for detection of the laser absorption generated ultrasonic/photoacoustic waves. Benefited from its high optical contrast and acoustic resolution at depth, as well as the capability of acquiring optical spectral information, photoacoustic imaging allows for structural and functional imaging of the biological relevant information such as hemoglobin concentration, blood oxygen metabolism, and lipid with high specificity, and is able to capture the corresponding physiological feature dynamics with high sensitivity. In terms of the imaging system, photoacoustic imaging manifests a variety of embodiments catering the multi-scale biomedical imaging needs. The above mentioned features and capabilities enable extensive biomedical application potentials of this imaging modality in both clinical diagnosis and pathological analysis, such as malignant tumor detection, atherosclerotic plaque assessment, and microcirculation abnormality diagnosis. Therefore, it is attracting more and more attention and interests in a wide range of biomedical research areas. This article presented the recent progress of photoacoustic technology development and application studies in our group in the fields of microvascular imaging, intravascular imaging, gastrointestinal endoscopic imaging, and sentinel lymph node imaging.

Abstract:We analyze photo-acoustic signals from capillaries and theoretically demonstrate the quantitative relationship between vascular diameter and spectral slope in a low-frequency band. Phantom experiments validate the theoretical analysis. Based on this finding, spectral slope is proposed as the imaging parameter of a photo-acoustic microscopy. This system effectively quantifies the microvasculature with diameters of 60 and 150 mu m, which are smaller than the wavelength 342 mu m at the central frequency 4.39 MHz of ultrasound transducer. The low frequency also guarantees the imaging depth in the order of centimeters. The proposed scheme could be potential for noninvasive diagnosis of diseases related to abnormal vasoconstriction or angiectasis. (C) 2015 Optical Society of America
Spatiotemporal singular value decomposition for denoising in photoacoustic imaging with low-energy excitation light source

Abstract:Photoacoustic (PA) imaging is an emerging hybrid imaging modality that combines rich optical spectroscopic contrast and high ultrasonic resolution and thus holds tremendous promise for a wide range of pre-clinical and clinical applications. Compact and affordable light sources such as light-emitting diodes (LEDs) and laser diodes (LDs) are promising alternatives to bulky and expensive solid-state laser systems that are commonly used as PA light sources. These could accelerate the clinical translation of PA technology. However, PA signals generated with these light sources are readily degraded by noise due to the low optical fluence, leading to decreased signal-to-noise ratio (SNR) in PA images. In this work, a spatiotemporal singular value decomposition (SVD) based PA denoising method was investigated for these light sources that usually have low fluence and high repetition rates. The proposed method leverages both spatial and temporal correlations between radiofrequency (RF) data frames. Validation was performed on simulations and in vivo PA data acquired from human fingers (2D) and forearm (3D) using a LED-based system. Spatiotemporal SVD greatly enhanced the PA signals of blood vessels corrupted by noise while preserving a high temporal resolution to slow motions, improving the SNR of in vivo PA images by 1.1, 0.7, and 1.9 times compared to single frame-based wavelet denoising, averaging across 200 frames, and single frame without denoising, respectively. The proposed method demonstrated a processing time of around 50 \mus per frame with SVD acceleration and GPU. Thus, spatiotemporal SVD is well suited to PA imaging systems with low-energy excitation light sources for real-time in vivo applications.

Abstract:The energy of light exposed on human skin is compulsively limited for safety reasons which affects the power of photoacoustic (PA) signal and its signal-to-noise ratio (SNR) level. Thus, the final reconstructed PA image quality is degraded. This Letter proposes an adaptive multi-sample-based approach to enhance the SNR of PA signals and in addition, detailed information in rebuilt PA images that used to be buried in the noise can be distinguished. Both ex vivo and in vivo experiments are conducted to validate the effectiveness of our proposed method which provides its potential value in clinical trials.
Multi-scale volumetric dynamic optoacoustic and laser ultrasound (OPLUS) imaging enabled by semi-transparent optical guidance

Abstract:Major biological discoveries have been made by interrogating living organisms with light. However, the limited penetration of unscattered photons within biological tissues severely limits the depth range covered by optical methods. Deep-tissue imaging has been achieved by combining light and ultrasound. Optoacoustic imaging uniquely exploits optical generation of ultrasound to render high-resolution images at depths unattainable with optical microscopy. Recently, laser ultrasound has further been suggested as a means of generating broadband acoustic waves for high-resolution pulse-echo ultrasound imaging. Herein, we propose an approach to simultaneously interrogate biological tissues with light and ultrasound based on layer-by-layer coating of silica optical fibers with a controlled degree of transparency. We exploit the time separation between optoacoustic signals and ultrasound echoes collected with a custom-made spherical array transducer for simultaneous three-dimensional optoacoustic and laser ultrasound (OPLUS) imaging with a single laser pulse. OPLUS is shown to enable large-scale comprehensive anatomical characterization of tissues along with functional multi-spectral imaging of spectrally-distinctive chromophores and assessment of cardiac dynamics at ultrafast rates only limited by the pulse repetition frequency of the laser. The suggested approach provides a flexible and scalable means for developing a new generation of systems synergistically combining the powerful capabilities of optoacoustics and ultrasound imaging in biology and medicine.

Optical wavefront shaping in deep tissue using photoacoustic feedback

Abstract:Over the past decade, optical wavefront shaping has been developed to focus light through highly opaque scattering layers, opening new possibilities for biomedical applications. To probe light intensity deep inside soft scattering media such as biological tissues, internal guide-stars are required. Here, we give an overview of the main principles and describe in depth the use of a photoacoustic feedback signal for this purpose. We further present first principles calculations and simulations to estimate important experimental parameters, and detailed instructions on designing and conducting these experiments. Finally, we provide guidance towards selecting suitable equipment for building a typical experimental setup, paving the way for further innovative biomedical imaging and therapy applications.

Abstract:Photo-acoustic imaging, also known as opto-acoustic imaging, has become a widely popular modality for biomedical applications. This hybrid technique possesses the advantages of high optical contrast and high ultrasonic resolution. Due to the distinct optical absorption properties of tissue compartments and main chromophores, photo-acoustics is able to non-invasively observe structural and functional variations within biological tissues including oxygenation and deoxygenation, blood vessels and spatial melanin distribution. The detection of acoustic waves produced by a pulsed laser source yields a high scaling range, from organ level photo-acoustic tomography to sub-cellular or even molecular imaging. This review discusses significant novel technical solutions utilising photo-acoustics and their applications in the fields of biomedicine and life sciences.

Abstract:Optoacoustic (OA) imaging offers powerful capabilities for interrogating biological tissues with rich optical absorption contrast while maintaining high spatial resolution for deep tissue observations. The spectrally distinct absorption of visible and near-infrared photons by endogenous tissue chromophores facilitates extraction of diverse anatomic, functional, molecular, and metabolic information from living tissues across various scales, from organelles and cells to whole organs and organisms. The primarily blood-related contrast and limited penetration depth of OA imaging have fostered the development of multimodal approaches to fully exploit the unique advantages and complementarity of the method. We review the recent hybridization efforts, including multimodal combinations of OA with ultrasound, fluorescence, optical coherence tomography, Raman scattering microscopy and magnetic resonance imaging as well as ionizing methods, such as X-ray computed tomography, single-photon-emission computed tomography and positron emission tomography.
