Abstract:Whole brain radiotherapy (WBRT) remains reference treatment in patients with brain metastasis (BM), especially with multiple lesions. Hippocampus avoidance in WBRT (HA-WBRT) offers the feasibility of less impaired cognitive function than conventional WBRT and better intracranial control than SBRT. Oncological safety is critical in defining the proper role of HA-WBRT. The study aims to investigate the frequency of intracranial substructure involvement based on large series of radiological data and to optimize the margin definition in treatment planning. Consecutive patients with diagnosis of BM from enhanced MRI between 03/2011 and 07/2016 in RuiJin Hospital were analyzed. Lesions of each patient were confirmed by a senior radiologist and the closest distances from tumor to the hippocampal area were measured. A total of 226 patients (male 115/226, female 111/226) with 1080 metastatic sites are reviewed. The distribution of the primary tumors were: 72.6% from lung cancer (164/226), 19.9% from breast cancer (45/226) and 7.5% from other malignancies (17/226). Seventy-one patients had BM before or simultaneously with their primary malignancy, among them, 64 were with primary lung cancer. In the other 155 patients, the average and median time from the diagnosis of primary disease to the diagnosis of BM were 14 m and 9 m in patients with lung cancer (100/155), 59 m and 34 m in patients with breast cancer (42/100). In total, 758/1080(70.2%) lesions were situated beyond the tentorium. The average maximum diameters of single lesion were 13.82mm ±14.67mm and median diameters were 10mm(1.2mm-162mm).The distribution of the lesions were as follows: 322(29.8%) in the cerebellum, 268(24.8%)in the frontal lobe,168(15.6%)in the temporal lobe,128(11.9%) in the parietal lobe, 131(12.1%)in the occipital lobe, 45(4.2%)in the thalamus and 18(1.6%) in the brainstem. After measuring the closest distance from lesions to the Hippocampus area in each patients, the definition and the frequency of Hippocampus involvemt can be classifed into 3 catogories: 7/226(3.1%) within 5mm or less, 13/226(5.7%) within 10mm or less and 19/226(8.4%) within 20mm or less. 18 of 45 patients who had complete radiological follow-up after WBRT in the same hospital were founded progress of BM. The median time of follow-up is 11 months. There was only one new lesions in area of Hippocampus (less than 5mm). With complete radiological diagnosis, HA-WBRT can be delivered with oncological safety. Proper margin definition of HA in delineation is to be confirmed with individual technique.
