Abstract:Objective: To evaluate the feasibility and efficacy of endoscopic ultrasonography (EUS) and endoscopic submucosal dissection (ESD) in treatment of upper digestive tract neuroendocrine tumor (NEN).Methods: A retrospective review of 26 cases of upper digestive tract NEN undergoing EUS and ESD between August 2010 and January 2014 was conducted. All patients were followed-up using endoscopy. The clinical features and outcomes including EUS features, pathological grade, complete resection rate, complications, recurrence and metastasis were evaluated.Results: EUS results showed that 26 patients had 31 NENs (18 originated from muscularis mucosa and 13 from submucosa; 2 located in esophagus, 24 in stomach and 5 in duodenal bulb). All 26 lesions were removed by ESD en bloc; the average maximum diameter of the lesions was 1.0 (0.3-3.1) cm; the complete resection rate was 93.5% (29/31). Two patients with positive basal surgical margin included one patient with pathologic grade 2 NEN in stomach (who then underwent an additional ESD) and one patient with pathologic grade 1 NEN in duodenal bulb (who was treated with argon plasma coagulation after rejecting surgery and ESD). Histological examination determined that 27 NEN lesions were pathologic grade 1 and 4 were pathologic grade 2. No patients underwent delayed bleeding or perforation after ESD. The follow-up rate was 93.5% (29/31). During a mean follow-up period of 21 months (range: 10-41 months), no lymph node metastasis or distant metastasis was observed.Conclusion: EUS is able to distinguish the origin of upper digestive tract NEN and give aid to determing the range and depth of ESD. ESD appears to be a safe and effective treatment for upper digestive tract NEN limited to submucosa. DOI:10.3781/j.issn.1000-7431.2015.33.029
Clinical analysis of endoscopic resection for submucosal tumors in the esophagogastric junction

Abstract:Objective: To investigate the clinical application of endoscopic dissection technique for submucosal tumors (SMTs) in the esophagogastric junction (EGJ). Methods: A total of 43 cases of SMTs in the EGJ were treated by endoscopic resection in the Endoscopy Center of the First Affiliated Hospital, College of Medicine, Zhejiang University between January 2010 and October 2013. The clinical and histopathologic features, surgical approach, en bloc resection rate, complications, and postoperative follow-up information were evaluated. Results: There were 19 males and 24 females with a mean age of 52.0±11.0 years old. The en bloc resection rate was 100%. The mean lesion size was 27.81±19.51 mm. The mean procedure time was 66.97±30.53 minutes and the mean bleeding volume during operation was 4.65±12.12 mL. Perforations occurred in 4 patients, pneumoperitoneum in 2 patients, and esophago-mediastinal fistula in 1 patient. All of them were successfully managed with conservational therapy. One patient with severe pneumothorax was treated with thoracic drainage. Pathologic examination showed that the lesions were leiomyoma (n = 19), gastrointestinal stromal tumor (n = 14), unknown pathological type of spindle cell tumor (n = 5) and intermuscular lipoma (n = 3), and mucinous cystadenoma (n = 1). No local recurrence and distant metastasis were noted during the follow-up. Conclusion: Various endoscopic resection methods based on endoscopic submucosal dissection seem to be safe and effective for SMTs in the EGJ. DOI:10.3781/j.issn.1000-7431.2014.04.008

Abstract:Upper gastrointestinal submucosal tumors(SMTs) is a kind of more common disease which originate from the submucosa have being divided benign tumor and malignancy tumor, the former is more common.Because of the overlying normal mucosa tissue, it is difficult for the conventional biopsy forceps to get to submucosa.tumorous types is difficult to judge, SMTs have being follow-up in the past,once the tumor getting bigger in short time or causing obvious symptoms,The surgery will be carried out.In recent years, due to the ultrasonic gastroscope(EUS) development, we can easily know where the SMTs originate from and what type is it.With the development of endoscopic mucosal resection(EMR), endoscopic submucosal dissection(ESD),we can not only get the complete tissue for biopsy but also resecting the lesions safety and efficiency, reducing the patients pain and economic burden, this paper is mainly reviewed the endoscopic diagnosis and treatment of upper gastrointestinal SMTs and endoscopic treatment technology.

Abstract:BACKGROUND AND AIMS:This retrospective study was designed to evaluate the safety and efficacy of endoscopic submucosal dissection (ESD) for the resection of gastric submucosal tumors (SMTs) originating from the muscularis propria (MP) layer at our institution.PATIENTS AND METHODS:From January 2010 to January 2014, a total of 49 patients underwent ESD for the resection of MP-originating gastric SMTs. The characteristics of the 49patients, clinical efficacy, safety of ESD, and post-ESD pathological diagnoses were evaluated retrospectively.RESULTS:ESD was successfully performed in 48 cases, and one case was converted to surgery. Pathological diagnosis included 43 gastrointestinal stromal tumors, 5 leiomyomas, and 1 schwannoma. During the procedure, 9 patients developed perforation. These patients recovered after conservative treatment. No residual or recurrent tumors were detected during the follow-up period.CONCLUSIONS:ESD seems to be a feasible, safe, effective and minimally invasive procedure, even for the resection of large gastric tumors originating from the MP. However, more studies should be carried out in the future.

Abstract:Objective Endoscopic submucosal tunnel resection (ESTD) is defined as the endoscopic operation finished in the tunnel established between the mucosa and muscle layer in the digestive tract. The purpose of this paper is to evaluate the application of ESTD in the upper gastrointestinal tumor resection. Methods Eligible clinical reports about ESTD for the treatment of upper gastrointestinal tumors were identified from the database (2007-2014). The operation techniques, clinical application and complications were systematically analyzed. Results A total of 16 studies involving 196 cases (33 with malignant lesions, 163 benign) were included. The longest diameters of most tumors were less than 3 cm. The complete resection rate and whole block excision rate were close to 100%. Perforation was one of the common complications. The incidences of intraoperative perforation fluctuated between 0 and 100%, depending on the tumor size, tumor location, etc. However, there was no postoperative continuous gastrointestinal perforation. Serious complications such as peripheral viscera injury, bleeding, severe mediastinum infection were not reported in the studies. Conclusions ESTD is an effective, safe and minimally invasive surgical technique in the resection of upper gastrointestinal submucosal benign lesions. It may yield similar outcome with surgical treatment when the longest diameter is less than 3 cm. Abstract:Submucosal tumor (SMT) is a disease that is commonly discovered during endoscopic examination. With advances in endoscopic ultrasonography (EUS) technology, this technique has become the primary screening method for the diagnosis of upper gastrointestinal SMTs. The present study summarized the clinical data of patients who were examined and diagnosed with upper gastrointestinal SMTs by EUS, underwent endoscopic therapy or surgical treatment, and received final pathological results in our hospital between January 2011 and September 2014. Our results show that endoscopic therapy has become the main approach for the treatment of upper gastrointestinal SMTs with the development and maturation of endoscopic technology in recent years. Our conclusion suggests that the selection of endoscopic methods, such as endoscopic mucosal resection (EMR), endoscopic submucosal dissection (ESD), and peroral submucosal tunneling endoscopic resection (STER), under the guidance of EUS is safe and effective for the treatment of upper gastrointestinal SMTs.

Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>Endoscopic submucosal tunnel dissection (ESTD), which is a new treatment for upper gastrointestinal superficial precancerous lesions and tumors has been proposed in recent years. However, compared with endoscopic submucosal dissection (ESD), the safety and efficacy of ESTD for treating these lesions are still uncertain. The aim of this meta‐analysis was to compare the efficacy and safety of these two endoscopic procedures for the treatment of superficial upper gastrointestinal superficial precancerous lesions and tumors.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>Electronic databases, including Embase, PubMed, Cochrane Library and Web of Science, were systematically searched. Related literature was retrieved until April 1, 2020. The primary outcomes were complete resection rate and en bloc resection rate. The secondary outcomes were operation time, dissection speed, complication rates and recurrence rates. The Newcastle‐Ottawa Quality Assessment Scale (NOS) was adopted to evaluate the quality of the studies.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Seven researches were included in the final meta‐analysis. The complete resection rate and en bloc resection rates between ESTD group and ESD group were similar for early superficial esophageal lesions. Compared with the ESD group, the ESTD group had a higher complete resection rate and en bloc resection rate for early superficial gastric lesions and a faster dissection speed in both superficial esophageal lesions and gastric lesions. For superficial gastric lesions, the operation time of the ESTD group was shorter than that of the ESD group, while the operation times were similar between the groups for superficial esophageal lesions. There were no obvious differences in the bleeding rate and perforation rate between the ESTD group and the ESD group.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusion</h3><p>As an endoscopic resection technique for early upper gastrointestinal superficial lesions, ESTD is equally as safe and more effective than ESD, especially for gastric lesion.</p><p>This article is protected by copyright. Value of endoscopic resection technique of serous preservation for upper gastrointestinal tumor originated from muscularis propria

Abstract:Objective To investigate the safety and efficacy of endoscopic subserosal dissection (ESSD)for patients with upper gastrointestinal submucosal tumors. Methods A retrospective study was performed on the data of 6 patients with submucosal tumor undergoing ESSD in Nanjing Drum Tower Hospital from October 2016 to February 2017. The surgery time, surgical success rate, postoperative pathology and complications were analyzed. Results All patients successfully completed surgery. One patient had serosal perforation. The mean diameter of tumor was 27.5±10.0 mm. The mean surgery time was 49±18 min. The postoperative pathology of lesions was very low risk stromal tumor. No serious complications occurred. Conclusion ESSD is safe and reliable for patients with tumor originated from muscularis propria.
Efficacy and Safety of Endoscopic Submucosal Dissection for Colorectal Submucosal Tumors

Abstract:Objective To evaluate the efficacy and safety of endoscopic submucosal dissection (ESD)for colorectal SMTs. Methods In this retrospective study, 412 consecutive patients with colorectal SMTs who underwent ESD at Zhongshan Hospital of Fudan University from January 2008 to July 2014 were enrolled. Tumor histopathology,resection rate and complications were analyzed.Results Complete resection was achieved in 358(86.9%)lesions. Adverse events occurred in 13(3.2%)patients including bleeding and perforation. Multivariate analysis showed that the SMTs in the colon(OR=0.460,95%CI:0.232-0.911, P=0.014)and number of ESD procedures for colorectal SMTs(OR=0.421, 95% CI:0.211-0.837, P=0.026)were independent risk factors for non-complete resection.Moreover,SMTs in the colon(OR=0.253, 95%CI:0.067-0.957,P=0.043),tumors in the muscularis propria(OR=5.459,95%CI:1.162-25.638, P=0.032)and number of ESD procedures for colorectal SMTs(OR=0.198, 95% CI:0.058-0.674, P=0.010)were independent risk factors for adverse events. Conclusion ESD is safe and effective for resection of colorectal SMTs. Tumor location and the experience of endoscopists may influence the complete resection rate and the development of adverse events.

Abstract:Objective To assess the effectiveness and safety of the technique of endoscopic submucosal dissection (ESD) in upper gastrointestinal lesions. Methods The patients with mucosal or submucosal lesions of upper gastrointestinal tract detected by gastroscopy were enrolled. Endoscopic ultrasonography and/ or biopsy were applied to confirm the size, location, and nature of the lesions. The procedure of ESD, involving the use of hook knife, insulation-tipped diathermic knife ( IT knife), and high-frequency electric coagulation, was as follows : first, identified the lesions by gastroscopy or mucosal staining and marked it with needle knife or argon plasma coagulation (APC) ; second, raised the submucosa through submucosal injection of indigo carmine and diluted epinephrine. Then, pre-cut the circumferential mucosa of the target lesion; finally, completely dissected along the submucosal layer. Acid-suppressing and mucosal protection agents were administered postoperatively. Follow-up endoscopy was taken at 1, 2, 6 months after ESD to evaluate the progression of the lesions. Results A total of 153 cases were enrolled from August 2006 to January 2008, of which 85 were mucosal lesions and 68 were submucosal ones. The mucosal lesions included 2 cases of ulcerating type, 48 of protruding type, and 35 of erosive type, with the diameter ranging from 0. 4 to 5. 0 cm (mean 2. 0 era). All the mucosal lesions were dissected through endoscopy, with the operation time ranging from 15 to 210 minutes (mean 55 minutes). One patient received emergency endoscopic hemostaais becauseof delayed hemorrhage, seven others were treated conservatively because of perforation. All the wound surfaces healed during the follow-up, including 4 cases of recurrence, of which 3 received second ESD, and 1 was referred to surgery. Among the submucosal lesions, 52 cases underwent endoscopic ultrasonography before ESD, and the others were diagnosed by postoperative histology. The diameter of the lesion ranged from 0. 4 to 4. 0 cm ( mean 1.2 cm). All the submucosal lesions were removed completely in 10 to 182 minutes(mean 41 min), except one patient was treated by endoscopic nylon ligation. Eleven patients were treated conservatively because of perforation, and one received surgery because of the uncontrolled bleeding. Conclusion As a minimally invasive method, ESD may present a novel and promising procedure in treating mucosal or submucosal lesions of upper gastrointestinal tract, with the advantage of achieving large en-bloc resections, low recurrence rate, and few complications.
Feasibility and safety of endoscopic full‐thickness resection for submucosal tumors in the upper gastrointestinal tract, including predominantly extraluminal submucosal tumors (with video)

Abstract:Objectives Endoscopic full‐thickness resection (EFTR) for submucosal tumors (SMTs) has been technically challenging. This retrospective study aimed to evaluate the feasibility, safety, and efficacy of EFTR for upper gastrointestinal (GI) SMTs, including extraluminal lesions. Methods We retrospectively investigated 232 patients with SMTs who underwent EFTR from January 2014 to August 2023. Clinicopathologic characteristics, procedure‐related parameters, adverse events (AEs), and follow‐up outcomes were assessed in all patients. Results The en‐bloc resection and en‐bloc with R0 resection rates were 98.7% and 96.1%, respectively. The average endoscopic tumor size measured 17.2 ± 8.7 mm, ranging from 6 to 50 mm. The resection time and suture time were 49.0 ± 19.4 min and 22.5 ± 11.6 min, respectively. In all, 39 lesions (16.8%) exhibited predominantly extraluminal growth. Gastrointestinal stromal tumors (GISTs) were the predominant pathology, accounting for 78.4% of the cases. Twenty‐one patients (9.1%) encountered complications, including pneumothorax (1/232, 0.43%), hydrothorax (1/232, 0.43%), localized peritonitis (3/232, 1.29%), and fever (16/232, 6.9%). Although the incidence of postoperative fever was notably higher in the predominantly extraluminal group (7/39, 17.9%) compared to the predominantly intraluminal group (9/193, 4.7%, P = 0.008), there were no significant differences in outcomes of the EFTR procedure. No instances of recurrence were observed during the mean follow‐up period of 3.7 ± 2.3 years. Conclusion EFTR was found to be feasible, safe, and effective for resecting upper GI SMTs, including lesions with predominantly extraluminal growth. Further validation in a prospective study is warranted.

Abstract:Abstract Objective: Endoscopy is the main method to treat gastrointestinal neuroendocrine tumors (GI-NETs), but the specific indications are still controversial. We aim to investigate the clinical outcomes of GI-NETs patients who experienced endoscopic submucosal dissection (ESD). Methods: We retrospectively reviewed the clinical features and prognosis of 65 GI-NETs patients who underwent ESD between 2008 and 2018. Results: A total of 65 patients diagnosed with GI-NETs pathologically, bearing 75 lesions, were found by endoscopy incidentally for other symptoms. The locations of these lesions were stomach (n = 24), duodenal bulb (n = 4) and rectum (n = 47). The diameter of 75 tumors were as follows: size ≤ 1 cm (78.7%), 1 cm < size ≤ 2 cm (17.3%), 2 cm < size ≤ 3.5 cm (4.0%). Endoscopic ultrasonography (EUS) suggested that 72 lesions were confined to submucosa and 3 lesions invaded into muscularis propria. The rates of en bloc resection and complete resection were all 100% and the rates of intraoperative bleeding and perforation were 2.7% and 1.3%, respectively. None of the 65 patients had lymph nodes and distant metastasis during the period of study. Conclusion: For GI-NETs without lymph nodes and distant metastases, the lesion confined to submucosa with the diameter ≤1 cm is absolute indication of ESD. For rectal neuroendocrine tumors limited in submucosa with the diameter between 1 and 2 cm, and Type 1 gastric neuroendocrine tumors predicted to be T2, ESD should be prioritized to preserve gastrointestinal volume and function at initial treatment.

Abstract:Objective To assess the clinical efficacy and safety of endoscopic submucosal dissection(ESD) for submucosal tumor(SMT) of the gastrointestinal(GI) tract.Methods A total of 19 patients with SMT of the GI tract diagnosed by endoscopy(6 in the esophagus,6 the stomach,1 the duodenum,1 the sigmoid,and 5 the rectum),were examined using endoscopic ultrasonography(EUS).Among the cases,18 lesions were within the submucosa and 1 in the muscularis propria.The SMTs were managed by ESD with a needle knife.After injection of physiological saline solution into the submucosal layer to separate the lesion from the muscle layer,the mucosa surrounding the lesion was pre-cut,and the connective tissues of the submucosa beneath the lesion were dissected.Then,the lesion was resected completely.Results The resected lesions sized 0.5-3.0 cm in diameter(mean,1.6 cm).Among the 19 lesions,18 were successfully resected with ESD.The mean ESD procedure time was 45 min(ranged from 15 to 105 min).Two patients had massive hemorrhage during ESD,which could not be controlled under a gastroscope.One of them was treated with balloon compression,and the other was managed surgically.None of the patients had delayed bleeding after ESD.Perforation of the GI tract occurred in 2 cases(the duodenal ball in one,and the gastric fundus in another) during the procedure,and was closed with endoclips without surgical treatment.One patient with rectal carcinoid developed subcutaneous emphysema after the operation owning to deep tearing of the muscle layer,and recovered after several days' conservative treatment.Histological evaluation showed that the tunica of the tumors was intact,and both the lateral and basal margins of the specimens were free of tumor cells.Conclusions ESD is an efficacious and safe procedure for the treatment of SMT of the GI tract.It is possible to completely resect submucosal lesions and provide sufficient pathological information.For the SMT originated from the muscularis propria,ESD should be performed cautiously.
The effect of endoscopic ultrasound on the precise selection of endoscopic treatment for submucosal tumors in the upper gastrointestinal tract

Abstract:Objective: To summarize and discuss the guiding role of endoscopic ultrasound (EUS) in selecting endoscopic treatments for submucosal tumors (SMTs) in the upper gastrointestinal tract. Methods: A retrospective investigation was conducted on 156 SMT patients who received endoscopic resection guided by EUS in the endoscopy center of the Second Affiliated Hospital of Guangzhou University of Chinese Medicine from May 2019 to September 2021. Next, the size, pathological type, and distribution of lesions were analyzed; the correlation of the tumor origin with distribution of lesions and selection of treatments was explored; and the consistency of preoperative EUS diagnosis and postoperative pathological diagnosis was summarized and analyzed. Results: The tumor diameters of the included SMT patients ranged from 0.3 to 4 cm, with a mean diameter of 0.95 cm; the lesions were mostly located in the esophagus, gastric fundus or fundic cardia and gastric body. As for the pathological types, liomyoma was the most common tumor in the esophagus, liomyoma and mesenchymoma were mainly located in the fundic cardia and gastric body, and heterotopic pancreas was mostly discovered in the gastric sinus. Among 38 esophageal SMT patients, some with lesions originating from muscularis mucosa and submucosa under EUS mainly underwent endoscopic submucosal dissection (ESD) and endoscope band ligation (EBL); while others with lesions originated from muscularis propria mainly received submucosal tunneling endoscopic resection (STER). Of 115 gastric SMT patients under EUS, some with lesion origins from the muscularis mucosa and submucosa mainly underwent endoscopic submucosal excavation (ESE), while others from muscularis propria mainly underwent ESE, ESD, and endoscopic full-thickness resection (EFTR). Besides, 3 duodenal SMT patients with lesion origins from submucosa and muscularis propria under EUS were given ESD and ESE, respectively. Additionally, 121 cases showed a consistency between the EUS diagnosis and the postoperative pathological nature, and the consistency rate was 84.6%. Conclusion: Clarifying the origin layer, size, growth pattern, and pathological nature of the lesion through preoperative EUS can guide the precise selection of endoscopic treatments, thereby ensuring a safe, effective, and complete surgical outcomes and reducing complications.

Abstract:Gastrointestinal (GI) carcinoid tumors less than 10 mm in diameter and confined to the submucosal layer have a low risk of lymph node involvement and distant metastasis. Thus, these tumors can be managed with endoscopic submucosal dissection (ESD).
