Abstract:Vegetarian diets are plant-based diets including all the edible foods from the Plant Kingdom, such as grains, legumes, vegetables, fruits, nuts, and seeds. Dairy and eggs can be added in small amounts in the lacto-ovo-vegetarian subtype, or not at all in the vegan subtype. The abundance of non-processed plant foods-typical of all well-planned diets, including vegetarian ones-can provide the body with numerous protective factors (fiber, phytocompounds), while limiting the intake of harmful nutrients like saturated fats, heme-iron, and cholesterol. The beneficial effects on health of this balance have been reported for many main chronic diseases, in both observational and intervention studies. The scientific literature indicates that vegetarians have a lower risk of certain types of cancer, overall cancer, overweight-obesity, type 2 diabetes, dyslipidemia, hypertension, and vascular diseases. Since the trend of following a vegetarian diet is increasing among citizens of developed countries, the knowledge in the field will benefit from further studies confirming the consistency of these findings and clarifying the effects of vegetarian diets on other controversial topics.

Abstract:The concept of vegetarianism includes different alimentary habits which have various effects on health. Today, the main reasons to adopt these practices are related to weight loss, reducing the risk of chronic degenerative diseases or contributing to their treatment. However, restrictive or imbalanced vegetarian diets may lead to nutritional deficiencies, particularly during periods of high metabolic demand. Some micronutrients are at greater risk of being deficient, since they are found in animal products, which are scarcely consumed. Added to this lower intake, their absorption may be affected by the high contents of phytates, oxalates and fiber which distinguish these diets. Health professionals should be informed about the benefits and potential risks of being vegetarian in order to design a well-balanced nutritional plan, as well as using fortified foods or specific micronutrient supplements to avoid deficiencies.

Abstract:A vegetarian diet generally includes plenty of vegetables and fruits, which are rich in phytochemicals, antioxidants, fiber, magnesium, vitamins C and E, Fe3+, folic acid and n-6 polyunsaturated fatty acid (PUFA), and is low in cholesterol, total fat and saturated fatty acid, sodium, Fe2+, zinc, vitamin A, B-12 and D, and especially n-3 PUFA. Mortality from all-cause, ischemic heart disease, and circulatory and cerebrovascular diseases was significantly lower in vegetarians than in omnivorous populations. Compared with omnivores, the incidence of cancer and type 2 diabetes was also significantly lower in vegetarians. However, vegetarians have a number of increased risk factors for non-communicable diseases such as increased plasma homocysteine, mean platelet volume and platelet aggregability compared with omnivores, which are associated with low intake of vitamin B-12 and n-3 PUFA. Based on the present data, it would seem appropriate for vegetarians to carefully design their diet, specifically focusing on increasing their intake of vitamin B-12 and n-3 PUFA to further reduce already low mortality and morbidity from non-communicable diseases. Abstract:Introduction In recent years, vegetarian and vegan diets became increasingly important as they are associated with beneficial health outcomes. Therefore, the NuEva study compares the impact of flexitarian, vegetarian, or vegan diets with omnivorous nutritional habits on nutrient intake and risk factors for non-communicable diseases. Methods A dietary protocol was kept over five days and blood and 24h urine samples were collected to examine the impact of dietary habits [omnivores, n = 65 (Median/Interquartile range: 33/17 yrs.), flexitarians, n = 70 (30/17 yrs.), ovo-lacto vegetarians, n = 65 (28/14 yrs.), vegans, n = 58 (25/10 yrs.)] on nutrient intake, nutrient concentrations in plasma, serum or 24h urine, body composition, and blood lipids. Results The increased exclusion of animal based foods in the diet (omnivores < flexitarians < vegetarians < vegans) is associated with a decreased intake of energy, saturated fat, cholesterol, disaccharides, and total sugar as well an increased intake of dietary fibers, beta carotene, vitamin E and K. The combined index of the B12 status (4cB12 score) in vegetarians (0.02/0.75) was lower compared to omnivores (0.34/0.58; p ≤ 0.05) and flexitarians (0.24/0.52; p ≤ 0.05). In omnivores vitamin A, vitamin E, ferritin, and the urinary excretion of selenium, iodine, and zinc were higher than in vegans ( p ≤ 0.05). In contrast, vegans had the highest concentrations of biotin, folate, and vitamin C. Flexitarians, vegetarians, and vegans had a lower body weight, BMI, and body fat percentage in comparison to omnivores ( p ≤ 0.05). In omnivores the concentrations on total cholesterol, total cholesterol/HDL cholesterol ratio, LDL cholesterol, LDL cholesterol/HDL cholesterol ratio, apolipoprotein B, and apolipoprotein B/ apolipoprotein A1 ratio were higher than in vegetarians and vegans ( p ≤ 0.05). Conclusion The NuEva study confirms the position of the Academy of Nutrition and Dietetics that adequately planned vegetarian diets are healthy, nutritionally adequate, and may provide health benefits in the prevention and treatment of non-communicable diseases. Nevertheless, critical nutrients were identified for all groups studied. This highlights the need to develop individual nutritional concepts to ensure an adequate nutrient intake.

Nutrient values and sustainability of omnivorous, vegetarian and vegan choices in a hospital canteen

Abstract:Abstract Background A plant-based diet has a lower environmental impact than an omnivorous diet and, when balanced, may reduce the risk of certain non-communicable diseases. The aim of our work is to evaluate the nutritional profile and sustainability of omnivorous, vegan and vegetarian choices in an Italian hospital canteen. Methods We photographed the midday meals of users in the hospital canteen of Palmanova (Italy) for 5 consecutive days in September 2022. We examined the recipes provided by the canteen and divided the meal trays in omnivorous, vegan and vegetarian. For each tray we estimated the carbon and water footprint using the SU-EATABLE LIFE database and the energy and nutrient content using the Italian Food Composition Database for Epidemiological Studies. Results We analysed 228 trays (167 omnivorous, 7 vegan, 54 vegetarian). Median energy content was higher (p < 0.05) in the omnivorous trays (942 kcal/tray) than the plant-based trays (813 kcal/tray). The omnivorous trays contained more fats (42 %E) and less carbohydrates (36 %E) than the plant-based trays (fats: 35 %E; carbohydrates: 44 %E) (p < 0.05). The carbon and water footprint were higher (p < 0.05) for the omnivorous meals (1202 gCO2eq/tray, and 1370 LH2O/tray) than for the plant-based meals, which had a median value of 687 gCO2eq/tray and 907 LH2O/tray. Conclusions Vegan and vegetarian meals were more in line with the Italian dietary reference values and more sustainable than omnivorous meals. Nutrition education is needed to improve the sustainability and quality of workers’ lunches and to ensure an adequate choice of plant-based meals. Key messages • Even if plant-based meals were more sustainable and nutrionally balanced than the omnivorous ones, they were less chosen. • Improving food choices can help achieve the Sustainable Development Goals.

Abstract:Consumer awareness of food safety and nutrition is a major issue in relation to healthy lifestyles and disease prevention. Improper consumer food management has been implicated in a large number of cases of foodborne illnesses. To reduce the risk of foodborne illness, consumers must be willing to change behaviours that are not consistent with safe food storage and preparation practices. Change in such behaviours is strongly related to consumer knowledge of proper food handling practices. At the same time there is public consciousness of the role of diet in contributing to the health status, but this awareness has not led to sufficient improvement of eating habits. What people buy and eat and the way they manage food depends not only on the individual but also on social, cultural, economic, and environmental factors. Identifying an effective strategy to improve consumers’ behaviour is a concern for politicians and health promoters. Non-targeted spreading of generic information is one strategy, but seems to be relatively unsuccessful. This paper describes an innovative and effective methodology to improve the level of consumer awareness towards healthier food choices and safer behaviours based on a holistic approach. This strategy could be adopted to better disseminate information regarding healthy foods on a large scale and at a relatively low cost, obtaining significant benefits in terms of primary prevention and contributing to the amelioration of food management at the consumer level.

Abstract:Vegetarian diets have been associated with a lower incidence of several chronic diseases. The benefits of plant-based diets are related mainly to the improvement of metabolic parameters that can indicate risk for such diseases. Some metabolic factors, such as oxidative balance, lipid profile, and glucose homeostasis, can be improved directly by diet, but paradoxically, some characteristics of vegetarian diets may promote a negative scenario that increases the risk of certain chronic diseases. Additionally, many benefits of a vegetarian diet are mediated by the gut microbiota, members of which not only have taxonomic and functional differences but also produce diverse, specific metabolites that vary according to whether the host consumes an omnivorous or a vegetarian diet. This review examines the modulation of human metabolism and gut microbiota by vegetarian and omnivorous dietary patterns and explores how this modulation may affect the risk of cardiovascular disease.

Abstract:A large body of evidence supports the notion that incorrect or insufficient nutrition contributes to disease development. A pivotal goal is thus to understand what exactly is appropriate and what is inappropriate in food ingestion and the consequent nutritional status and health. The effective application of these concepts requires the translation of scientific information into practical approaches that have a tangible and measurable impact at both individual and population levels. The agenda for the future is expected to support available methodology in nutrition research to personalize guideline recommendations, properly grading the quality of the available evidence, promoting adherence to the well-established evidence hierarchy in nutrition, and enhancing strategies for appropriate vetting and transparent reporting that will solidify the recommendations for health promotion. The final goal is to build a constructive coalition among scientists, policy makers, and communication professionals for sustainable health and nutritional policies. Currently, a strong rationale and available data support a personalized dietary approach according to personal variables, including sex and age, circulating metabolic biomarkers, food quality and intake frequency, lifestyle variables such as physical activity, and environmental variables including one's microbiome profile. There is a strong and urgent need to develop a successful commitment among all the stakeholders to define novel and sustainable approaches toward the management of the health value of nutrition at individual and population levels. Moving forward requires adherence to well-established principles of evidence evaluation as well as identification of effective tools to obtain better quality evidence. Much remains to be done in the near future.
A Comprehensive Examination of Vegan Lifestyle in Italy

Abstract:The popularity of veganism and plant-based diets is rapidly increasing worldwide, including in Italy, where more individuals and families are adopting this lifestyle. However, medical and health professionals often lack the necessary knowledge and are skeptical about this diet despite the scientific evidence. It is important for them to provide support and expertise to those following this diet. The survey evaluated various aspects of the lifestyle of Italian vegans living in Italy and abroad, including food frequency, vitamin and mineral supplementation, relationship with medical and health professionals, and perceived difficulties in daily life. The emphasis was on potentially critical aspects for those following this dietary choice. A cross-sectional survey was conducted in Italy between March and April 2022. A questionnaire was distributed through social media platforms such as Instagram, Facebook, and Telegram, and 2180 Italian adults who follow a vegan diet completed it. The survey found that most of the vegan population surveyed were female, showed a greater sensitivity to ethical issues, were aware of the need for vitamin B12 supplementation, and followed healthy-eating guidelines. It is evident that despite the increasing popularity of plant-based diets, many medical and health professionals remain cautious and hesitant to recommend them.

Abstract:In the field of nutritional science, the importance of embracing plant-based dietary patterns has gathered significant attention over the last decades (Willett et al. Citation 2019 ). Supported by a robust body of scientific evidence showing their impact on human health and environmental preservation and corroborated by endorsements of international organisations, the integration of diverse plant-based foods and beverages into daily diets stands as a cornerstone of preventive medicine and sustainable food systems (Grosso et al. Citation 2020 ). Central to the health-promoting properties of plant-based foods lies their richness in vitamins, antioxidants and fibre, all nutritional elements indispensable for optimal health and well-being (Satija and Hu Citation 2018 ). While the precise mechanisms through which these bioactive compounds exert their effects remain only partially elucidated, emerging research emphasises the pivotal role of the gut microbiota and the immune system in mediating the health benefits conferred by plant-based diets (Gentile and Weir Citation 2018 ; Grosso et al. Citation 2022 ). As technological advancements propel the frontier of nutritional epidemiology forward, traditional methodologies are gradually giving way to more sophisticated approaches, facilitating a deeper understanding of the intricate interplay between diet, health outcomes and the underlying molecular mechanisms (Brennan and Hu Citation 2019 ). At the vanguard of this paradigm shift lie omic-based methodologies, encompassing genomics, metabolomics and other "omic" sciences, which hold the promise of unravelling the multifaceted tapestry of associations between dietary exposures and health outcomes with unprecedented precision (Ramos-Lopez et al. Citation 2022 ). Emerging research highlights the potential of metabolomic analyses in elucidating the intricate nexus between dietary patterns and metabolic pathways, offering invaluable insights into the biochemical underpinnings of health and disease (Karczewski and Snyder Citation 2018 ). By interrogating the "omic" signatures of individuals following plant-based dietary regimens, researchers can glean profound insights into the physiological responses elicited by these dietary factors, paving the way for personalised nutritional strategies tailored to individual needs (Laddu and Hauser Citation 2019 ). Through the integration of multi-omic data sets, researchers can construct intricate networks delineating the molecular pathways underpinning the health-promoting effects of plant-based diets, offering a holistic perspective on the complex interplay between diet and health (Phillips et al. Citation 2019 ). Within the pages of this issue of the International Journal of Food Sciences and Nutrition , a series of studies have further delved into these pivotal subjects, shedding light on various facets of the relationship between plant-based diets and health outcomes.

Abstract:The Mediterranean diet (MD) concept as currently known describes the dietary patterns that were followed in specific regions of the area in the 1950s and 1960s. The broad recognition of its positive effects on the longevity of Mediterranean populations also led to the adoption of this diet in other regions of the world, and scientific interest focused on revealing its health effects. MD is not only linked with eating specific nutritional food products but also with social, religious, environmental, and cultural aspects, thus representing a healthy lifestyle in general. However, modern lifestyles adhere to less healthy diets, alienating people from their heritage. Therefore, considering the increasing evidence of the beneficial health effects of adherence to the MD and the ongoing transitions in consumers’ behavior, the present review focuses on updating the scientific knowledge regarding this diet and its relevance to agrobiodiversity. In addition, it also considers a sustainable approach for new marketing opportunities and consumer trends of the MD.

Abstract:Introduction and purpose Recently, there has been a significant increase in interest in the dynamically developing field of science, nutripsychiatry, which has contributed to an increase in the number of studies conducted assessing the relationship between the use of a vegetarian nutritional model and its potential therapeutic impact on holistically understood mental health. Material and methods For the purposes of this work, a review of the literature available in the PubMed and Google Scholar search engines, was performed using the following keywords: vegetarian diet; depression; psychiatry. Works published before 2015 were excluded from the analysis. During the review of the literature of available scientific texts, attempts were made to comprehensively present the state of current knowledge. State of knowledge Numerous clinical studies support the beneficial effect of a vegetarian diet on mood by improving well-being and lower levels of anxiety and depression. However, there are also studies that did not show a relationship between the diet and the mood of the subjects, as well as studies indicating possible dangers associated with the use of this nutritional model. Conclusions The use of a vegetarian dietary pattern as a potential intervention is a promising method of non-pharmacological support. The amount of available scientific data confirming the beneficial effect of a vegetarian diet on mental health is insufficient to recommend it as a standard form of supporting the treatment of depression. Before implementing a vegetarian nutritional model, it is necessary to assess its safety in the patient and individualize dietary recommendations.

Abstract:Dietary proteins are indispensable to human nutrition. In addition to their tissue-building function, they affect body composition and regulate various metabolic pathways, as well as satiety and immune system activity. Protein use can be examined from a quantitative or qualitative viewpoint. In this scoping review, we compare animal- and plant-based protein sources in terms of their effects on human health and the environment. We conclude that the consumption of vegetable protein sources is associated with better health outcomes overall (namely, on the cardiovascular system) than animal-based product use. The healthier outcomes of vegetable protein sources dovetail with their lower environmental impact, which must be considered when designing an optimal diet. Indeed, the health of the planet cannot be disjointed from the health of the human being. Future research will clarify the mechanisms of action underlying the health effects of plant-based protein sources when compared with animal sources, fostering better agronomic practices and influencing public health in a direction that will benefit both the planet and its inhabitants.

Abstract:There is growing interest in the adoption of a more sustainable diet, and this has led to the development of plant-derived alternative products that are used as a substitute for products of animal origin. This is a promising way to improve the sustainability of the agricultural food industry; however, there are increasing concerns regarding the nutritional profile of plant-based products, with emerging evidence that many are not nutritionally equivalent to conventional animal products. In this study, we provide a narrative review focusing on the nutritional quality of plant-based alternative products. First, we summarize the available literature examining consumers' and healthcare professionals' perceived healthfulness and nutritional concerns regarding plant-based foods. Then, we compare the nutritional composition (macro- and micronutrients) of plant-based alternatives to their conventional animal counterparts. Moreover, we outline the potential impact of these nutritional differences on overall diet quality and provide a summary of the differences in the digestibility and bioavailability of nutrients from plant and animal sources. We highlight the role of product development and innovations in food processing to support the nutritional composition of plant-based products and outline the priorities for more comprehensive research on the nutrient density of these products and the sustained effect of modern plant-derived diets on long-term health.

Abstract:Plant-based diets, defined here as including both vegan and lacto-ovo-vegetarian diets, are growing in popularity throughout the Western world for various reasons, including concerns for human health and the health of the planet. Plant-based diets are more environmentally sustainable than meat-based diets and have a reduced environmental impact, including producing lower levels of greenhouse gas emissions. Dietary guidelines are normally formulated to enhance the health of society, reduce the risk of chronic diseases, and prevent nutritional deficiencies. We reviewed the scientific data on plant-based diets to summarize their preventative and therapeutic role in cardiovascular disease, cancer, diabetes, obesity, and osteoporosis. Consuming plant-based diets is safe and effective for all stages of the life cycle, from pregnancy and lactation, to childhood, to old age. Plant-based diets, which are high in fiber and polyphenolics, are also associated with a diverse gut microbiota, producing metabolites that have anti-inflammatory functions that may help manage disease processes. Concerns about the adequate intake of a number of nutrients, including vitamin B12, calcium, vitamin D, iron, zinc, and omega-3 fats, are discussed. The use of fortified foods and/or supplements as well as appropriate food choices are outlined for each nutrient. Finally, guidelines are suggested for health professionals working with clients consuming plant-based diets.

Abstract:It is the position of the American Dietetic Association and Dietitians of Canada that appropriately planned vegetarian diets are healthful, nutritionally adequate, and provide health benefits in the prevention and treatment of certain diseases. Approximately 2.5% of adults in the United States and 4% of adults in Canada follow vegetarian diets. A vegetarian diet is defined as one that does not include meat, fish, or fowl. Interest in vegetarianism appears to be increasing, with many restaurants and college foodservices offering vegetarian meals routinely. Substantial growth in sales of foods attractive to vegetarians has occurred, and these foods appear in many supermarkets. This position paper reviews the current scientific data related to key nutrients for vegetarians, including protein, iron, zinc, calcium, vitamin D, riboflavin, vitamin B-12, vitamin A, n-3 fatty acids, and iodine. A vegetarian, including vegan, diet can meet current recommendations for all of these nutrients. In some cases, use of fortified foods or supplements can be helpful in meeting recommendations for individual nutrients. Well-planned vegan and other types of vegetarian diets are appropriate for all stages of the life cycle, including during pregnancy, lactation, infancy, childhood, and adolescence. Vegetarian diets offer a number of nutritional benefits, including lower levels of saturated fat, cholesterol, and animal protein as well as higher levels of carbohydrates, fiber, magnesium, potassium, folate, and antioxidants such as vitamins C and E and phytochemicals. Vegetarians have been reported to have lower body mass indices than nonvegetarians, as well as lower rates of death from ischemic heart disease; vegetarians also show lower blood cholesterol levels; lower blood pressure; and lower rates of hypertension, type 2 diabetes, and prostate and colon cancer. Although a number of federally funded and institutional feeding programs can accommodate vegetarians, few have foods suitable for vegans at this time. Because of the variability of dietary practices among vegetarians, individual assessment of dietary intakes of vegetarians is required. Dietetics professionals have a responsibility to support and encourage those who express an interest in consuming a vegetarian diet. They can play key roles in educating vegetarian clients about food sources of specific nutrients, food purchase and preparation, and any dietary modifications that may be necessary to meet individual needs. Menu planning for vegetarians can be simplified by use of a food guide that specifies food groups and serving sizes.

Abstract:The difference in gut microbiota composition between individuals following vegan or vegetarian diets and those following omnivorous diets is well documented. A plant-based diet appears to be beneficial for human health by promoting the development of more diverse and stable microbial systems. Additionally, vegans and vegetarians have significantly higher counts of certain Bacteroidetes-related operational taxonomic units compared to omnivores. Fibers (that is, non-digestible carbohydrates, found exclusively in plants) most consistently increase lactic acid bacteria, such as Ruminococcus, E. rectale, and Roseburia, and reduce Clostridium and Enterococcus species. Polyphenols, also abundant in plant foods, increase Bifidobacterium and Lactobacillus, which provide anti-pathogenic and anti-inflammatory effects and cardiovascular protection. High fiber intake also encourages the growth of species that ferment fiber into metabolites as short-chain fatty acids (SCFAs), including acetate, propionate, and butyrate. The positive health effects of SCFAs are myriad, including improved immunity against pathogens, blood-brain barrier integrity, provision of energy substrates, and regulation of critical functions of the intestine. In conclusion, the available literature suggests that a vegetarian/vegan diet is effective in promoting a diverse ecosystem of beneficial bacteria to support both human gut microbiome and overall health. This review will focus on effects of different diets and nutrient contents, particularly plant-based diets, on the gut microbiota composition and production of microbial metabolites affecting the host health.
