Abstract:A simple, highly-automated instrument system used for on-site detection of foodborne pathogens based on fluorescence was designed, fabricated, and preliminarily tested in this paper. A corresponding method has been proved effective in our previous studies. This system utilizes a light-emitting diode (LED) to excite fluorescent labels and a spectrometer to record the fluorescence signal from samples. A rotation stage for positioning and switching samples was innovatively designed for high-throughput detection, ten at most in one single run. We also developed software based on LabVIEW for data receiving, processing, and the control of the whole system. In the test of using a pure quantum dot (QD) solution as a standard sample, detection results from this home-made system were highly-relevant with that from a well-commercialized product and even slightly better reproducibility was found. And in the test of three typical kinds of food-borne pathogens, fluorescence signals recorded by this system are highly proportional to the variation of the sample concentration, with a satisfied limit of detection (LOD) (nearly 102–103 CFU·mL−1 in food samples). Additionally, this instrument system is low-cost and easy-to-use, showing a promising potential for on-site rapid detection of food-borne pathogens.

Abstract:The quantitative detection of pathogens in milliliters of beverage sample requires complex preprocessing. To achieve rapid and ultrasensitive quantification of pathogens in large volume food sample, we developed a filtration-based interfacial digital LAMP (idLAMP) system, which consists of a nanoporous membrane for filtration and nanoporous hydrogel for digital amplification. Digital counting of single bacteria at the membrane surface under nanoconfinement could be achieved. The idLAMP successfully accomplished the quantitative detection of Escherichia coli in 100 mL water samples within 30 min, with wide dynamic range from 0.09 to 900 cells/mL. This technique could also be well applied to the quantification of Escherichia coli and Salmonella typhi in real beverage samples (juice, tea drinks, carbonated drinks and alcoholic drinks) without tedious sample pre-treatments. With facile operation, higher specificity and sensitivity and better end-point analysis capabilities, the system has great potential in quantitative counting of single bacteria in large-volume food samples.

Abstract:Conventional methods for pathogen detection in water rely on time-consuming enrichment steps followed by biochemical identification strategies, which require assay times ranging from 24 hours to a week. However, in recent years, significant efforts have been made to develop biosensing technologies enabling rapid and close-to-real-time detection of waterborne pathogens. In previous studies, we developed a plastic optical fiber (POF) immunosensor using an optoelectronic configuration consisting of a U-Shape probe connected to an LED and a photodetector. Bacterial detection was evaluated with the immunosensor immersed in a bacterial suspension in water with a known concentration. Here, we report on the sensitivity of a new optoelectronic configuration consisting of two POF U-shaped probes, one as the reference and the other as the immunosensor, for the detection of Escherichia coli. In addition, another methos of detection was tested where the sensors were calibrated in the air, before being immersed in a bacterial suspension and then read in the air. This modification improved sensor sensitivity and resulted in a faster detection time. After the immunocapture, the sensors were DAPI-stained and submitted to confocal microscopy. The histograms obtained confirmed that the responses of the immunosensors were due to the bacteria. This new sensor detected the presence of E. coli at 104 CFU/mL in less than 20 min. Currently, sub-20 min is faster than previous studies using fiber-optic based biosensors. We report on an inexpensive and faster detection technology when compared with conventional methods.

Abstract:There is a constant need for the development of easy‐to‐operate systems for the rapid and unambiguous identification of bacterial pathogens in drinking water without the requirement for time‐consuming culture processes. In this study, we present a disposable and low‐cost lab‐on‐a‐chip device utilizing a nanoporous membrane, which connects two stacked perpendicular microfluidic channels. Whereas one of the channels supplies the sample, the second one attracts it by potential‐driven forces. Surface‐enhanced Raman spectrometry (SERS) is employed as a reliable detection method for bacteria identification. To gain the effect of surface enhancement, silver nanoparticles were added to the sample. The pores of the membrane act as a filter trapping the bodies of microorganisms as well as clusters of nanoparticles creating suitable conditions for sensitive SERS detection. Therein, we focused on the construction and characterization of the device performance. To demonstrate the functionality of the microfluidic chip, we analyzed common pathogens (Escherichia coli DH5α and Pseudomonas taiwanensis VLB120) from spiked tap water using the optimized experimental parameters. The obtained results confirmed our system to be promising for the construction of a disposable optical platform for reliable and rapid pathogen detection which couples their electrokinetic concentration on the integrated nanoporous membrane with SERS detection.This article is protected by copyright. Abstract:The monitoring of pathogenic microorganisms in water is important for public health and disease outbreaks prediction. Recently, optical detection techniques have drawn much attention due to the advantages of rapid response, security and high sensitivity. In this paper, a fluorescence spectrometer based on 375 nm exciting laser and the microchannel liquid sample flow technology is proposed. The 4 × 4 narrowband filter array was coupled to a Silicon Photomultiplier (SiPM) array with single-photon sensitivity. B500 fluorescent microspheres and Escherichia coli were used for performance evaluation of the spectrometer. As a result, it is feasible to use random particle counting method to detect the bacteria concentration level in water even low to several CFU/mL. In addition, based on Python tools and neural network algorithm models, the fluorescence spectra of different kinds of substances (biotic and abiotic) can be classified with an accuracy of more than 97%. The method was successfully applied to tap water samples. The results suggest that the proposed method is applicable for on-site bacteria detection.

Abstract:The real-time monitoring of water quality parameters using optical sensors has become widespread. However, the price of commercial sensors and the ability to integrate them into customized sensing networks can limit their application in research and monitoring programs. This study introduces the design, verification, and validation of an innovative, low-cost, portable, low-power fluorometer–nephelometer device, employing long-range wide area network technology. The developed Internet of Things capable device can measure temperature, turbidity, phycocyanin fluorescence (a proxy for cyanobacteria biomass), and chlorophyll-a(Chl-a) fluorescence [a proxy for phytoplankton (cyanobacteria plus algae) biomass] in aquatic ecosystems. The fluorometer–nephelometer structure employs one digital thermistor probe, three distinct light-emitting diodes (LEDs) that are amber (590 nm) to excite phycocyanin pigments of cyanobacteria, blue (465 nm) to excite Chl-a pigments of phytoplankton, and near-infrared (NIR) (870 nm) to measure turbidity through light scattering. Two orthogonal silicon photodiodes are used as detectors set in line with long-pass filters of 830 nm for turbidity and 630 nm for phytoplankton. A peristaltic pump circulates water through a polymethylmethacrylate cuvette within a 3-D-printed fluorometer assembled in a weatherproof box. The activation by personalization long range wide area network (LoRaWAN) protocol is utilized for real-time wireless transmission of water quality data, while a micro-SD card is employed for storing the data locally. The optical sensor tests are conducted in the laboratory using standard turbidity solutions and pigments. The turbidity, phycocyanin, and Chl-a sensing ranges are 3–200 FTU, 0.025–2.5 mg-PC/L, and 1– $50 \, \mu $ g-chl/L, respectively. In repeated laboratory tests, the relative percent difference is consistently less than 10%.

Abstract:In medical treatment, the detection of pathogens at an early stage of diseases is a key step to set up an appropriate diagnosis. To reach this goal, several techniques have been elaborated for point-of-care diagnostic applications. One of the state-of-the art methods is the application of biosensor devices. Label-free versions of them ensure an appropriate detection of pathogens from fluid samples by their relative sensitivity, rapidity and portability, thus offering a feasible and affordable alternative to the traditional diagnostic techniques. The aim of the present study is to fulfill these requirements with a cheap construction of an electro-optical biosensor, for application as a rapid test in clinical diagnostics. Hence, an integrated microsystem consisting of dielectrophoretic surface-electrodes, a rib waveguide and a microfluidic channel was created for label-free optical detection of bacteria from fluid samples. To model the efficiency of the sensor, we carried out quantitative measurements by observing the light scattered by living Escherichia coli cells located in the vicinity of the waveguide. A significant change in the scattered light pattern was observed even when objectives of moderate magnification (x10, x4.7) were used, implying that such type of sensing of the cells can be achieved by low-cost cameras, as well. The optimal frequency utilized in the process of dielectrophoretic cell-collecting was also established. With this novel system, a detection limit of ca. 102 CFU × mL−1 was achieved, which is relevant to characteristic pathogen concentrations in body fluids, e.g., urine. Our further plan is to utilize this cell-gathering method in other, highly sensitive integrated optical sensor constructions, as well. The working principle of this dielectrophoretically enhanced detection of Escherichia coli cells from their suspensions gives us a low-cost and rapid-sensing alternative to routinely used, but time- and money-consuming other methods. Hence, we expect it to be readily applicable in point-of-care diagnostics as a basis of rapid tests to identify general pathogens from various body fluids.

A Photonic Immunosensor Detection Method for Viable and Non-Viable E. coli in Water Samples

Abstract:Detection and enumeration of coliform bacteria using traditional methods and current molecular techniques against E. coli usually involve long processes with less sensitivity and specificity to distinguish between viable and non-viable bacteria for microbiological water analysis. This approach involves developing and validating an immunosensor comprising ring resonators functionalized with specific antibodies surrounded by a network of microchannels as an alternative method for detecting and indirectly enumerating Escherichia coli in samples of water for consumption. Different ELISA assays were conducted to characterize monoclonal and polyclonal antibodies selected as detection probes for specific B-galactosidase enzymes and membrane LPS antigens of E. coli. An immobilization control study was performed on silicon nitride surfaces used in the immunosensor, immobilized with the selected antibodies from the ELISA assays. The specificity of this method was confirmed by detecting as few as 10 CFU/mL of E. coli from viable and non-viable target bacteria after applying various disinfection methods to water samples intended for human consumption. The 100% detection rate and a 100 CFU/mL Limit of Quantification of the proposed method were validated through a comprehensive assessment of the immunosensor-coupled microfluidic system, involving at least 50 replicates with a concentration range of 10 to 106 CFU/mL of the target bacteria and 50 real samples contaminated with and without disinfection treatment. The correlation coefficient of around one calculated for each calibration curve obtained from the results demonstrated sensitive and rapid detection capabilities suitable for application in water resources intended for human consumption within the food industry. The biosensor was shown to provide results in less than 4 h, allowing for rapid identification of microbial contamination crucial for ensuring water monitoring related to food safety or environmental diagnosis and allowing for timely interventions to mitigate contamination risks. Indeed, the achieved setup facilitates the in situ execution of laboratory processes, allowing for the detection of both viable and non-viable bacteria, and it implies future developments of simultaneous detection of pathogens in the same contaminated sample.

Abstract:Since microbial detection is an important aspect for the prevention and control of foodborne diseases, an ideal detection system with high sensitivity, strong specificity, and timeliness is needed. Here, we proposed a fluorescent and opt-electric recording bacterial identification device (FORBID) for fully automatic real-time photoelectric sensing analysis of microbials by integrating the metabolic characteristics of microbial and selective substrate catalysis. It simplifies the testing process (one-step) and decreases the need of professional technicians. Besides, the system exhibits ultrasensitive (1 CFU/mL) and specific detection (99%) in both microbials, Escherichia coli and Pseudomonas aeruginosa. More importantly, the timeliness of this system is even better than that of the traditional culture methods. Abstract:Molecular imprinting has advanced towards synthesizing whole-cell imprints of microorganisms such as bacteria on various sensor surfaces including wire electrodes, quartz crystal microbalances, and microparticles (MPs). We recently introduced cell-imprinted polymers (CIPs) coated on MPs, called CIP-MPs, for bacteria recovery from water. In this paper, we have advanced towards rapid fluorometric Escherichia coli (E. coli) detection by applying fluorescent magnetic CIP-MPs, which were captured by soft ferromagnetic microstructures integrated into a microfluidic channel. The shape of ferromagnetic microstructures was optimized numerically to enhance the magnetophoretic accumulation of CIP-MPs in the microchannel. The device was then fabricated and the flow rate required to enhance bacteria capturing efficiency by CIP-MPs was determined. Decreasing the flow rate reduced the flow-induced drag and increased the interactions between the bacteria and imprinted CIP cavities. Fluorescent imaging of the accumulated CIP-MPs was done before and after bacteria capturing to quantify the changes in the fluorescence intensity as E. coli cells were captured by CIP-MPs in the microchannel. The dose-response curve of the sensor at 0-109 CFU/mL bacterial counts was obtained. Using the 3- and 10-sigma methods on the dose-response curve, the limits of detection (LOD) and quantification (LOQ) of the sensor were determined to be 4 × 102 and 3 × 103 CFU/mL, respectively, within a dynamic range of 102-107 CFU/mL. Lastly, the specificity of the CIP-MPbased sensor towards E. coli was tested and confirmed using Sarcina as a non-specific target bacterium. In summary, our developed sensor offers a promising approach for rapid and low-cost detection of bacteria in water and is suitable for development of portable and durable all-polymeric sensors for point-of-need detection.

Abstract:Background: Public health events caused by foodborne pathogens have brought substantial economic and social burdens. Conventional pathogen detection relies on a time-consuming culturing assay, laying back for the in-spection requirements of the modern food industry. With portability, automation, high throughput, and cost-effectiveness, microfluidic systems provide a solution for advancing the diagnosis of foodborne pathogens. Scope and approach: This review systematically summarizes the advances in microfluidic systems for detecting foodborne pathogens in the theme of substrates (i.e., glass/silicon, thermoplastics, hydrogels, paper, threads, the combination of various materials) enabled functions. We further discuss the extensions of microfluidic systems in the era of "big data" by integrating the Internet of Things (IoT) for in-time food safety inspection and warning. Additionally, we explore the future direction of microfluidic technology in detecting and monitoring foodborne pathogens. Key findings and conclusions: Towards the goal of applications, the fabrication of microfluidic devices has been gradually developed as low-cost, portable, and easy-to-use, from silicon/glass, and polymers, to hydrogel, paper, thread, and the combination of various materials. The materials with distinct physicochemical properties empower microfluidic systems with various advances for rapidly detecting foodborne pathogens. Microfluidic systems integrated with IoT have opened new opportunities for real-time food safety inspecting and warning.

Abstract:Water quality monitoring is of great significance for human health, which involves a large number of distinct targets detection, such as pathogens, heavy metal ions, and toxins. Traditional detection methods usually carried out in well-equipped central labs are subjected to time, labor, and expense consumption, thus not suitable for implementing water quality monitoring at point of care, where the detection implemented on a multiplexed, miniaturized, and simplified device with rapid and sensitive answer out is desired. To this end, we developed a paper disc relying on upconversion fluorescence signal and aptamer recognition probes for multiplex detection of water contaminants with high sensitivity and specificity. Finally, several different typical kinds of water contaminants have been successfully detected on our paper disc with limit of detection of 115 cfu/mL for Salmonella, 3 ng/mL for Ochratoxin A and Microcystin-LR, 20 nM for Hg2+ and 4 nM for Pb2+, respectively. Additionally, we designed a compact smartphone-based reading device to enable water quality monitoring at point-of-care. Furthermore, this paper disc-based detection platform can be extended to apply to multiplex detection for disease diagnosis and food safety monitoring.

Abstract:Point-of-care testing (POCT) devices for continuous low-cost monitoring of critical patient parameters require miniaturized and integrated setups for performing quick high-sensitivity analyses, away from central clinical laboratories. This work presents a novel and promising laser-induced fluorescence platform for measurements in direct optical test formats that leads towards such powerful POCT devices based on fluorescence-labeled immunoassays. Ultimate sensitivity of thin film photodetectors, integrated with microfluidics, and a comprehensive optimization of all system components aim at low-level signal detection in the targeted biosensor application. The setup acquires fluorescence signals from the volume of a microfluidic channel. An innovative sandwiching process forms a flow channel in the microfluidic chips by embedding laser-cut double-sided adhesive tapes. The custom fit of amorphous silicon based photodiode arrays to the geometry of the flow channel enables miniaturization, fully adequate for POCT devices. A free-beam laser excitation with line focus provides excellent alignment stability, allows for easy and reliable swapping of the disposable microfluidic chips, and therewith greatly improves the ease of use of the resulting integrated device. As a proof-of-concept of this novel in-volume measurement approach, the limit of detection for the dye DY636-COOH in pure water as a model fluorophore is examined and found to be 26 nmol .

Abstract:Development of sensitive methods for the determination of E. coli bacteria contamination in water distribution systems is of paramount importance to ensure the microbial safety of drinking water. This work presents a new sensing platform enabling the fast detection of bacteria in field samples by using specific antibodies as the biorecognition element and dark field microscopy as the detection technique. The development of the sensing platform was performed using non-pathogenic bacteria, with the E. coli DH5α strain as the target, and Bacillus sp. 9727 as the negative control. The identification of the captured bacteria was made by analyzing the dark field microscopy images and screening the detected objects by using object circularity and size parameters. Specificity tests revealed the low unspecific attachment of either E. coli over human serum albumin antibodies (negative control for antibody specificity) and of Bacillus sp. over E. coli antibodies. The system performance was tested using field samples, collected from a wastewater treatment plant, and compared with two quantification techniques (i.e., Colilert-18 test and quantitative polymerase chain reaction (qPCR)). The results showed comparable quantification capability. Nevertheless, the present method has the advantage of being faster, is easily adaptable to in-field analysis, and can potentially be extended to the detection of other bacterial strains.

Methods A 12-week pilot study randomized 42 participants recruited in person from a safety-net ambulatory care clinic in Los Angeles into one of the 3 study arms: control, phone messaging (PM), and phone messaging plus social support from family members and friends (PM+FF). All participants were prompted to set PA goals and to self-monitor PA behavior using pedometers and walking logs. PM and PM+FF participants received STVMs with reminders to review goals and self-monitor, PA behavior change education, and feedback on performance. Participants in the PM+FF arm also had their family members and friends receiving STVMs with suggestions for how they could support the participant’s PA behavior change efforts. Participants completed semistructured assessments in person at baseline, 6 weeks, and 12 weeks. Outcomes were PA (steps/day) and perceived social support from family members and friends. Results Among PM and PM+FF participants, those who opted to receive text messages (short message service, SMS) responded to 62.7% (128/204) of SMS text messages requiring a response while those who opted to receive voice messages responded 30% (12/40) of the time. Participants perceived guidance in self-regulation as useful, particularly self-monitoring, goal setting, self-instruction, feedback, and social support. All participants increased PA at 6 weeks, but only the PM and PM+FF arms increased PA at 12 weeks. All study arms experienced an increase in perceived social support from family members and friends at 6 weeks, but only those in the PM+FF arm had an increase in the perception of social support at 12 weeks. Conclusion Designing an STVM intervention based on self-regulation techniques is feasible and perceived as useful by participants. The STVM intervention has the potential to improve PA in terms of daily steps and perceived social support from family members and friends. Abstract:OBJECTIVE To compare survival in Acute Promyelocytic Leukemia (APL) patients treated with or without All-Trans Retinoic Acid (ATRA). STUDY DESIGN Longitudinal, comparative study. PLACE AND DURATION OF STUDY The Armed Forces Bone Marrow Transplant Centre (AFBMTC), Rawalpindi, Pakistan from May 2001 to April 2007. METHODOLOGY All consecutive newly diagnosed patients of acute promyelocytic leukemia, treated at Armed Forces Bone Marrow Transplant Centre, Rawalpindi, Pakistan, between May 2001 and April 2007, were included and given chemotherapy according to availability of ATRA. Diagnosis was confirmed on morphology/ karyotyping/ molecular analysis. Eligibility criteria included confirmed morphologic diagnosis and/or by demonstration of t(15;17) and/or PML/RAR proportional to re-arrangement, no prior chemotherapy, normal hepatic and renal function, Eastern Cooperative Oncology Group (ECOG) performance status of 0 - 2 and no contraindications to ATRA (history of sensitivity to Vit. A or other retinoids). All patients having history of cardiac failure (LVEF < 50) and arrhythmias, ECOG performance status 3 and 4, relapse / refractory disease, ALT twice normal values, serum creatinine > 150 micromol/L and pregnancy were excluded from this study. Survival was calculated from the date of chemotherapy to death or last follow-up according to Kaplan-Meier and Cox (Proportional hazard) regression analysis methods. RESULTS During the 6 years study period, 31 newly diagnosed patients with acute promyelocytic leukemia received treatment at AFBMTC. Seventeen patients received anthracycline-based remission induction and consolidation chemotherapy, while 14 received ATRA-based remission induction, consolidation and by two years maintenance therapy. Overall Survival (OS), Disease Free Survival (DFS) and mortality were 29.4%, 29.4% and 70.6% respectively in 17 patients who received anthracycline based chemotherapy, whereas in patients who received ATRA-based chemotherapy OS, DFS and mortality was 71.4%, 64.2% and 28.6% respectively. Major causes of mortality were septicemia and chemotherapy related toxicity. CONCLUSION Response to ATRA-based chemotherapy in patient cohort was better as compared with anthracycline based chemotherapy (71.4% vs. 29.4%) in terms of survival and mortality.
Simultaneous SERS-decoding detection of multiple pathogens in drinking water with home-made portable double-layer filtration and concentration device

Abstract:The engineering of a home-made portable double-layer filtration and concentration device with the common syringe for rapid analysis of water samples is reported. The core elements of the device were two installed filtration membranes with different pore sizes for respective functions. The upper filtration membrane was used for preliminary intercepting large interfering impurities ( interception membrane ), while the lower filtration membrane was used for collecting multiple target pathogens ( enrichment membrane ) for determination. This combination can make the contaminated environmental water, exemplified by surface water, filtrated quickly through the device and just retained the target bacteria of Escherichia coli O157:H7, Staphylococcus aureus , and Listeria monocytogenes on the lower enrichment membrane. Integrating with surface-enhanced Raman spectra (SERS) platform to decode the SERS-Tags (SERS-Tag CVa , SERS-Tag R6G , and SERS-Tag MB ) already labeled on each of the enriched bacteria based the antibody-mediated immuno-recognition effect, fast separation, concentration, and detection of multiple pathogenic bacteria from the bulk of contaminated environmental water were realized. Results show that within 30 min, all target bacteria in the lake water can be simultaneously and accurately measured in the range from 10 1 to 10 6 CFU mL −1 with detection limit of 10.0 CFU mL −1 without any pre-culture procedures. This work highlights the simplicity, rapidness, cheapness, selectivity, and the robustness of the constructed method for simultaneous detecting multiple pathogens in aqueous samples. This protocol opens a new avenue for facilitating the development of versatile analytical tools for drinking water and food safety monitoring in underdeveloped or developing countries.
Biliary Sludge and Hyperbilirubinemia Associated with Ceftriaxone in an Adult: Case Report and Review of the Literature

Abstract:Ceftriaxone is a commonly used third‐generation cephalosporin that has antimicrobial activity against many gram‐positive and gram‐negative organisms. Generally, ceftriaxone is a safe antibiotic; however, symptomatic biliary sludge has been reported in rare instances, most of which have involved children. It is uncommon for ceftriaxone to cause increases in laboratory indexes, such as bilirubin levels. We describe the case of a 53‐year‐old man who was treated with intravenous ceftriaxone 2 g every 12 hours. After 7 days of therapy, the patient's liver function test results, including total, direct, and indirect bilirubin levels, increased significantly from baseline, and the patient became jaundiced. A right upper quadrant ultrasound examination revealed biliary sludge and cholelithiasis without sonographic evidence of cholecystitis. Ceftriaxone was thought to be the responsible agent, and it was discontinued. The patient's jaundice subsided, and his liver function test results improved, returning to baseline within 14 days. Clinicians need to be aware of the association of ceftriaxone and biliary pseudolithiasis and hyperbilirubinemia, and monitor accordingly.
