Abstract:Abstract Impulse oscillometry (IOS) is a variant of forced oscillation technique described by Dubois 50 years ago, which allows us to measure the reactance of the airways and the resistance of the small and large airways during tidal breathing. It requires minimal patient cooperation from subjects who are unable to perform spirometry, like elders, children and patients with neurologic disorders. IOS can outline the diagnosis of obstructive airway disease, differentiate small airway obstruction from large airway obstruction. It is more sensitive than spirometry for peripheral airway disease in determining the severity of the disease, the exacerbations and evaluate the therapeutic response. Other applications include early evaluation of transplant rejection, cystic fibrosis, vocal cord disorder, bronchiectasis, hypersensitivity pneumonitis, obstructive sleep apnea.

Abstract:Impulse oscillometry (IOS) is a variant of forced oscillation technique, described by Dubois over 50 years ago, which permits passive measurement of lung mechanics. In this method, sound waves are superimposed on normal tidal breathing, and the disturbances in flow and pressure caused by the external waves are used to calculate parameters describing the resistance to airflow and reactive parameters that mostly relate to efficient storage and return of energy by the lung. It requires minimal patient cooperation and can be done easily in subjects who are unable to perform spirometry. Importantly, IOS can differentiate small airway obstruction from large airway obstruction and is more sensitive than spirometry for peripheral airway disease. It has been used to study various respiratory disorders, especially asthma and is suitable for measuring bronchodilatory response as well as bronchoprovocation testing. IOS parameters seem to be able to pick up early changes in lung functon such that they are superior to spirometry in predicting loss of control in asthmatic patients and possibly in identifying early airway disease in smokers. Such comparisons, especially for chronic obstructive pulmonary disease, are made difficult by widespread use of spirometric parameters as the diagnostic gold standard. Here, we discuss the principles and technique of IOS and review its application in obstructive airway diseases.

Abstract:OBJECTIVE:To evaluate the clinical application of lung function analysis in diagnosis of children respiratory diseases.METHODS:Respiratory function was evaluated with portable lung function detector in 6 261 children with respiratory diseases. FEV1, FVC and PEFR were measured in 268 children with cough variant asthma (CVA) and 147 children with asthma (AS), and the results were self-compared during follow-up.RESULT:There were no significant differences in above parameters between children with cough variant asthma and those with asthma (P>0.05). Lung functions of asthma children were improved evidently by administration of glycocorticoid inhalator (P<0.001).CONCLUSION:Lung function detection is essential for diagnosis of asthma and it can be used as a reliable index for therapeutic effect of asthma children.

Abstract:Heterochelamon tessellatum n. sp. and H. castanea n. sp. are described trom the Guangxi Zhuang Autonomous Region, southern China. Heterochelamon tessellatum n. sp. and H. castanea n. sp. are morphologically most similar to H. yangshuoense Türkay & Dai, 1997, and H. guangxiense Türkay & Dai, 1997, respectively. The new species can be differentiated from these allied species by differences in the shape of external orbital tooth, epibranchial tooth, and male first gonopod. The present study brings the number of Heterochelamon species to five. A key to species of the genus Heterochelamon is provided.

Abstract:Background In preschoolers, performing an acceptable spirometry and measuring bronchodilator response (BDR) is challenging; in this context, impulse oscillometry (IOS) represents a valid alternative. However, more studies on the standardization of BDR for IOS in young children are required. Objective The objective of the study was to identify optimal thresholds to define a positive BDR test with IOS in preschoolers with suspected asthma. Methods Children aged 3–6 years with suspected asthma and their lung function investigated with both IOS and spirometry pre‐ and post‐BDR were retrospectively analyzed. The spirometric BDR was defined as positive when the change of FEV1 was ≥12% or ≥200 mL. The oscillometric BDR was defined as positive in case of change of at least −40% in R5, +50% in X5, and −80% in AX. Results Among 72 patients, 36 (age 5.2 ± 1 years; 64% boys) were selected for the subsequent analysis according to ATS/ERS quality criteria of measurements; specifically, 19 patients did not meet IOS and 36 did not meet spirometry criteria. The spirometric BDR was found positive in seven subjects (19.4%); conversely, a positive oscillometric BDR was identified in four patients (11.1%). No patient presented a positive BDR response with both methods. In IOS, the mean decrease in R5 and AX was 19.9% ± 10% and 44% ± 22.1%, and the mean increase in X5 was 23.3% ± 17.8%, respectively. A decrease in R5 of 25.7% (AUC 0.77, p = .03) and an increase in X5 of 25.7% (AUC 0.75, p = .04) showed the best combination of sensitivity and specificity to detect an increase of FEV1 ≥ 12% and/or ≥200 mL. Conclusion The IOS represents a valid alternative to spirometry to measure BDR in preschool children and should be the gold standard in this age group. We are considering a decrease of 26% in R5 and an increase of 26% in X5 as diagnostic threshold for BDR.

Abstract:Diagnosing asthma typically relies on clinical history, physical examination, and objective lung function tests. Spirometry, the gold standard for assessing lung function, is universally recommended for the diagnostic, assessment and monitoring of asthma. It requires maximal respiratory effort that limits cooperation in certain populations and has limitations in detecting small airway dysfunction. Oscillometry requires minimal patient effort, is non-invasive, and provides valuable information about both large and small airways. Recent task force reports have suggested oscillometry as an alternative or a complementary lung function test to spirometry in the management of individuals with asthma. This review explores the scientific evidence and psychometric properties regarding oscillometry in three main purposes of lung function testing in asthma, namely diagnosis, assessment of control and future risk, longitudinal monitoring including clinical utility. it addressed challenges and facilitators to implementation and future directions regarding its positioning as lung function testing in individual with asthma.

Abstract:Background Asthmatic symptoms often start during early childhood. Impulse oscillometry (IOS) is feasible in preschool children who may be unable to reliably perform spirometry measurements. Objective We evaluated the utility of IOS in a multicenter multiethnic high-risk asthma cohort entitled the Vitamin D Antenatal Asthma Reduction Trial (VDAART). Methods The VDAART recruited pregnant women whose children were followed from birth to age 8 years. Lung function was assessed with IOS at ages 4, 5, and 6 years and spirometry at ages 5, 6, 7, and 8 years. Asthma status, respiratory symptoms, and medication use were assessed with repeated questionnaires from birth to age 8 years. Results In total 220 children were included in this secondary analysis. Recent respiratory symptoms and short-acting β2-agonist (SABA) use were associated with increased respiratory resistance at 5 Hz (R5) at age 4 years (β 2.6; 95% CI 1.0, 4.4; P=0.002 and β 3.4; 95% CI 0.7, 6.2; P=0.015, respectively). Increased R5 at age 4 years was also associated with decreased lung function from ages 5 to 8 years (β -0.3; 95% CI -0.5, -0.1; P<0.001 for FEV 1 at 8 years) and active asthma at age 8 years (β 2.0; 95% CI 0.2, 3.8; P=0.029). Conclusion Increased respiratory resistance in preschool IOS is associated with frequent respiratory symptoms as well as school-age asthma and lung function impairment. Our findings suggest that IOS may serve as a potential objective measure for early identification of children who are at high risk of respiratory morbidity.

Application of Impulse Oscillometry in Adult Asthmatic Patients With Preserved Lung Function

Abstract:PURPOSE: It is difficult to assess airway obstruction using spirometry in adult asthmatic patients with preserved lung function. Impulse oscillometry (IOS) can detect not only airway resistance but also reactance. Therefore, IOS may be useful in assessing pulmonary function in such patients. We investigated the applicability of IOS for asthma patients with preserved lung function.METHODS: Between 2015 and 2018, 1,248 adult asthmatic patients suspected of having asthma who visited the Allergy and Asthma Center of Severance Hospital underwent both spirometry and IOS. Consequently, 784 patients had asthma, 111 had chronic obstructive lung disease (COPD) or asthma-COPD overlap, and 7 had parenchymal lung disease. The remaining 346 patients had chronic cough without underlying lung or airway disease. Among the 784 asthmatic patients, 191 with decreased lung function (predicted forced expiratory volume in 1 second [FEV1] < 80%) were excluded. Propensity score matching was performed to adjust baseline characteristics between 346 non-asthmatic and 593 asthmatic patients with preserved lung function. Subsequently, we compared the spirometry and IOS parameters between the 329 asthmatic and 329 non-asthmatic patients.RESULTS: Multiple logistic regression analysis showed that the area of reactance (AX) was associated with asthma with preserved lung function. In receiver operating characteristic (ROC) curve analysis, the area under the curve (AUC) of AX (AUC = 0.6823) for asthma was not significantly different from that of FEV1 (AUC = 0.6758). However, the AUC of a combination of AX and FEV1 (AUC = 0.7437) for asthma was significantly higher than that of FEV1 alone. The cutoff value of AX was 0.51 kPa/L in univariate ROC analysis.CONCLUSIONS: AX is associated with adult asthma with preserved lung function. Performing spirometry together with IOS is more beneficial than performing spirometry alone for diagnosing asthma in adult patients with preserved lung function.
Clinical Value of Impulse Oscillometry in Chronic Obstructive Pulmonary Disease: A Systematic Review and Meta-Analysis

Abstract:Introduction: Impulse oscillometry (IOS) is an effortless test compared to spirometry. Numerous studies explored the role of IOS in spirometry-based chronic obstructive pulmonary disease (COPD), but most of them had limited sample sizes with poor statistical power. This systematic review and meta-analysis aimed to pool the individual data and quantitatively analyze the clinical value of IOS in COPD. Methods: PubMed, Web of Science, Ovid, Cochrane Library, China National Knowledge Internet, and Wanfang were searched for studies with comparisons of IOS indicators between COPD patients and healthy controls, including respiratory resistance at 5 Hz (R5) and 20 Hz (R20), difference between R5 and R20 (R5-R20), respiratory reactance at 5 Hz (X5), resonant frequency (Fres), and area of reactance (Ax). Meta-analyses were conducted to calculate the weighted mean differences (WMDs) and 95% confidence intervals (CIs). Results: 39 eligible studies were enrolled, involving 6,144 COPD patients and 4,611 healthy controls. Relative to healthy controls, COPD patients had significantly higher R5 (WMD: 0.17, 95% CI: 0.14, 0.20), R5-R20 (WMD: 0.13, 95% CI: 0.11, 0.15), Fres (WMD: 9.04, 95% CI: 7.66, 10.42), Ax (WMD: 1.24, 95% CI: 0.86, 1.61), and lower X5 (WMD: -0.15, 95% CI: -0.18, -0.11), and such differences became even greater as the Global Initiative for Chronic Obstructive Lung Disease (GOLD) stage escalated. Pooled correlation coefficients presented that R5, R5-R20, Fres, and X5 were significantly related to post-bronchodilator forced expiratory volume in the first second (FEV1)/forced vital capacity ratio (meta r = -0.37, -0.45, -0.53, and 0.42, respectively) and FEV1 as a percentage of predicted value (meta r = -0.43, -0.54, -0.59, and 0.56, respectively). Conclusion: IOS may be a supplement to spirometry in diagnosing and assessing COPD, especially when spirometry is inappropriate. More well-designed, large sample-sized, prospective studies are warranted to establish an IOS-based criterion for COPD management.

Abstract:Preschool children with asthma present a challenge in lung function testing, as they cannot readily cooperate with spirometry. The forced oscillation technique (FOT) measures passive pressures and flows delivered by a loudspeaker to a facemask, at frequencies much higher than those occurring physiologically. This in turn allows for rapid collection of data from a spontaneously breathing child in a timespan of seconds. However, at very rapid oscillatory flow rates, the mechanical properties opposing flows into and out of the respiratory system (collectively termed the respiratory system impedance, and comprised of elastic, resistive and inertial components) are very different from at normal breathing frequencies, with elastic properties being less important and inertial properties being more important. An understanding of how the respiratory system behaves at high frequencies is essential to understanding the physiological basis of this technique. This article presents a way to understand these oscillatory mechanics of the respiratory system. It then describes studies of the FOT in normal preschool children and in children with asthma. The technique can also measure the separate contributions of the central and peripheral airways, as well as assess for changes after bronchodilator administration. The FOT holds promise for the objective measurement of lung function in children who are too young to reliably perform spirometry.

Abstract:Objective While coronavirus disease 2019 (COVID‐19) is generally considered to exhibit a less severe clinical course in children than in adults, studies have demonstrated that respiratory symptoms can endure for more than 3 months following infection in at least one‐third of pediatric cases. The present study evaluates the respiratory functions of children aged 3–15 years within 3–6 months of their recovery from COVID‐19 using impulse oscillometry (IOS) and compares them with the values of healthy children. Methods Included in this prospective cross‐sectional study were 63 patients (patient group) aged 3–15 years who contracted COVID‐19 between December 2021 and May 2022, as well as 57 healthy children as a control group, matched for age and sex. The demographic, clinical, and laboratory data of the patients were recorded, and respiratory function was assessed based on airway resistance (zR5, zR20, R5–20) and reactance (zX5, zX20, reactance area [AX], resonant frequency [Fres]) using an IOS device. Results There were no significant differences in the age, weight, height, and body weight z score values of the two groups (p > .05). While the zR5 and R5–20 levels of the patient group were higher (p = .008 and p .05). Abstract:Background: Asthma is defined as a chronic inflammatory disorder of the airways, characterized by bronchial hyper-responsiveness and variable airflow obstruction, that is often reversible either spontaneously or with treatment. Impulse oscillometry is a newer diagnostic modality for asthma. It is based on the measurement of sound waves reflected by airway resistance.Objectives: The aim of this article is to study the role of impulse oscillometry in diagnosis and follow-up of bronchial asthma.Methods: Fifty-five clinically diagnosed bronchial asthma patients were evaluated with spirometry and impulse oscillometry before and after 3 months of inhaled treatment. The sensitivity to diagnose and follow-up was compared using proper statistical tests.Results: Impulse oscillometry was superior to spirometry in diagnosing bronchial asthma and also in accessing the treatment response after 3 months.Conclusion: Impulse oscillometry is superior in predicting bronchial asthma and its parameters are also more sensitive in accessing treatment response. It can replace spirometry as it is easy to perform and effort independent.

Abstract:Impulse oscillometry (IOS) is the most commonly used type of forced oscillation technique in clinical practice, although relatively little is known about its application in COPD. Resistance at 20 Hz (R20) is unrelated to COPD severity and does not improve with bronchodilatation or bronchoconstriction, inferring a lack of large airway involvement in COPD. Peripheral airway resistance expressed as frequency dependent heterogeneity between 5 Hz and 20 Hz (R5-R20), and peripheral airway compliance as area under the reactance curve (AX), are both closely related to COPD severity and exacerbations. Both R5-R20 and AX markedly improve in response to long acting bronchodilators, while AX appears to be more sensitive than R5-R20 in response to bronchoconstriction. Future studies may be directed to assess if IOS in combination with spirometry is more sensitive at predicting future exacerbations. Perhaps AX might also be useful as a screening tool in early stage disease or to monitor long term decline in COPD.

Abstract:Purpose of review Oscillometry is a noninvasive pulmonary function test that has gained significant interest in the evaluation of lung disease. Currently, oscillometry is primarily a research tool, but there is a growing body of evidence supporting its clinical use. This review describes the recent work evaluating the role of oscillometry in the diagnosis and treatment of asthma. Recent findings A large body of observational data supports the ability of oscillometry to distinguish healthy individuals from those with respiratory symptoms or lung disease. Oscillometry may not be as useful as an isolated diagnostic test in asthma, but the combination with other pulmonary function tests may improve its diagnostic ability. Oscillometry can detect peripheral airways dysfunction in asthma, which is associated with symptoms and the risk for exacerbations. To help guide future research, minimal clinically important differences for specific oscillometry variables have been developed. Oscillometry may be useful in monitoring the response to biological therapy and has potential for personalizing treatment for individual patients. Oscillometry also has potential in uncovering unique aspects of the pathophysiology of asthma in obesity. Summary Oscillometry is a promising tool in the diagnosis and management of asthma. More research is needed to support its routine clinical use.

Abstract:Impulse oscillometry (IOS) is a method for measuring respiratory impedance based on the forced oscillation technique (FOT). The IOS measurement requires only quiet tidal breathing, which is simple, suitable for a wide range of people and provides various respiratory physiological parameters. For promoting the standardization and clinical application of IOS measurement in China, based on the relevant research evidence and the updated technical standards by European Respiratory Society, this article provided a brief introduction to the background of IOS technique and the current situation of its clinical application in China, as well as the recommendations and interpretations on the indications and contraindications, device calibration, testing protocols, quality control requirements including acceptability and repeatability criteria, measurement parameters and graphs, testing results interpretation, references values and normal limits, report template, clinical application including assessment of airway patency, airway reversibility and responsiveness, small airway function and so on. It also offered recommendations on the relevant thresholds of diagnosis and evaluation.

Abstract:Abstract Developing countries face a huge challenge in managing obstructive lung disorders due to frequency, severity, management, and economic impact. Spirometry has been the popular choice to assess the severity and reversibility of airflow obstruction, but the success of evaluation depends on the practitioner’s skills and the patient’s cooperation. Impulse oscillometry (IOS), a variant of forced oscillation technique is preferred over spirometry since it can be performed without forced respiratory maneuvers. The objective of this review is to evaluate IOS techniques and weigh its benefits against spirometry. Publications from international electronic databases such as PubMed, Scopus, and Web of Science were searched using relevant keywords for 5 years (2018–2023). A total of 416 articles were filtered, and of these, 70 were selected which specifically discussed the principles, procedures, parameters, and interpretations of spirometry and oscillometry. From the full-text articles included, we have reflected on the history, defined, explained, compared, and contrasted spirometry and oscillometry methods to establish the versatility of oscillometry technique to identify small airway functions and its application in special clinical conditions. This review presents an easily comprehensible synthesis of oscillometry and spirometry bringing out the principles, advantages, and limitations that are suitable for a larger spectrum of healthcare and research community.

Abstract:Measuring lung function is an important component in the decision making process for patients with obstructive airways disease (OAD). Not only does it help in arriving at a specific diagnosis, but it also helps in evaluating severity so that appropriate pharmacotherapy can be instituted, it helps determine prognosis and it helps evaluate response to therapy. Spirometry is currently the most commonly performed lung function test in clinical practice and is considered to be the gold standard diagnostic test for asthma and COPD. However, spirometry is not an easy test to perform because the forceful expiratory and inspiratory manoeuvres require good patient co-operation. Children aged <5 years, elderly people and those with physical and cognitive limitations cannot perform spirometry easily.

Design This retrospective observational study was conducted using patient data from between January 2014 and December 2015. Setting Five public hospitals in China: West China Hospital, Nuclear Industry 416 Hospital, Suining Central Hospital, Affiliated Hospital, Medical College of Chengdu University and 363 Hospital. Participants The study included 6307 participants aged>40 years, comprising 2109 COPD patients and 4198 general non-COPD individuals, according to the Global Initiative for Obstructive Lung Disease (GOLD) spirometry standard. Participants with lung cancer, pulmonary tuberculosis, pneumonia or those who underwent lung resection were excluded from the study. Outcome measures and analysis Demographic data, spirometry results and IOS results were collected. Spearman's correlation analysis was used to examine the correlation between the IOS and spirometry parameters. Receiver operating characteristic curve analysis was used to evaluate the IOS performance in COPD diagnosis and severity staging. Results Patients with COPD exhibited significant increases in Z5, R5, R20, R5–R20, Fres and Rp, but a decrease in X5 compared with non-COPD subjects (p<0.0001). IOS parameters, including Z5, R5–R20, Fres, Rp and X5, varied with the GOLD stages, with mild-to-moderate correlations with MMEF 25%–75% , forced expiratory volume in one second (FEV 1 )/forced vital capacity and FEV 1% , respectively. However, the combination of these five IOS parameters did not exhibit ideal performance in diagnosing COPD (area under the curve (AUC) 0.78; sensitivity 63.68%; specificity 80.09%), differentiating GOLD stage 1 patients from the general non-COPD population (AUC 0.71; sensitivity 54.71%; specificity 77.49%) or identifying GOLD stages 3 and 4 patients among those with COPD (AUC 0.75; sensitivity 69.51%; specificity 70.32%). Conclusion IOS parameters, while showing good correlation with spirometry in patients with COPD, did not perfectly substitute for spirometry in diagnosing COPD, especially in the early and advanced stages of the disease.

Clinical Application Value Of Impulse Oscillometry In Geriatric Patients With Copd

Abstract:Background: The diagnosis and assessment of COPD rely mainly on the use of spirometry, which is an effort-dependent test and requires good patient cooperation. Impulse oscillometry (IOS) is a non-volitional method that requires less effort and cooperation and presents advantages for geriatric patients. However, the clinical application value of IOS in geriatric patients with COPD remains unclear.Aim: The aim of this study was to investigate the clinical application value of IOS in geriatric patients with COPD.Subjects and methods: A total of 234 subjects were retrospectively enrolled in this study, including 133 patients with COPD and 101 healthy volunteers. All the participants underwent IOS and spirometry examination. The data were collected and analyzed in the overall group, the geriatric group (aged. 65 years), and the advanced elderly group (aged >= 80 years).Results: 1) In COPD patients, a significant increase in respiratory impedance (Z5), resonant frequency (Fres), and respiratory resistance (R5, R20, R5-R20) and a decrease in respiratory reactance (X5) were observed in the overall group, the geriatric group, and the advanced elderly group compared with the healthy control subjects. 2) The IOS parameters correlated well with spirometry in COPD. In particular, R5-R20 showed the best correlation with forced expiratory volume in 1 second (FEV1) in the different age groups. 3) Fres and R5-R20 had the best diagnostic efficiency for COPD. The area under the curve (AUC) values for Fres, expressed by the receiver operating characteristic (ROC) curve, were 0.905, 0.909, and 0.914, for the different age groups, respectively. 4) The optimal cutoff values for Fres to diagnose airflow obstruction from ROC curves was 17.715 in the COPD patients. Its sensitivity and specificity were 0.789 and 0.931, respectively, and the cutoff values were similar in geriatric and advanced elderly patients.Conclusion: IOS demonstrated good relevance compared with spirometry for geriatric patients with COPD. IOS may serve as an alternative method for spirometry in elderly subjects for the evaluation of the state of COPD.

Abstract:Background: Small airway dysfunction (SAD) in asthma can be measured by impulse oscillometry (IOS). Usually, the reactance should decrease with decreases in frequency oscillation. Sometimes an upward shift of the curve at low frequencies can be observed together with lower than expected reactance values. The actual value of the reactance at 5 Hz (X5) is calculated by the Sentry Suite application of the Jaeger Master screen iOS system™, providing the corrected X5 parameter (CX5). Our hypothesis is that correction of X5 is common in persistent asthma and it correlates better than X5 with the IOS parameters for evaluating SAD. Methods: In this transversal study, we evaluated 507 children (3-18 years old) using IOS-spirometry (Sentry Suite, Vyntus®). Resistance of all airways (R5), reactance area (AX), resonant frequency (Fres), X5, CX5, difference between R5 and R20 (D5-20), and spirometry parameters were analyzed. Reactance inversion and CX5 prevalence by age range was determined. The mean IOS-Spyrometry values in children with and without CX5 were compared, and correlations with each IOS-spirometry parameter in the age groups were performed. Results: CX5 was found in 83.5% of preschool children, 66.2% of schoolchildren, and 43.3% of adolescents (p < 0.001). The means of R5, AX, and D5-20 were significantly higher and FEV1 was significantly lower in children with CX5 (p < 0.05). In all ages, CX5 correlated better than X5 with IOS-spirometry parameters. Conclusion: Reactance inversion and CX5 are frequent in asthmatic children, decrease with age, and correlate more closely than X5 with other IOS-spirometry parameters for evaluating SAD.
