Abstract:Objective To evaluate the effect of separate consultation before surgery on the level of anxiety in the third molar extraction. Methods 52 patients were randomly divided into the experimental group and the control group,26 cases in each group. Patients in the experimental group were received the individual consultation on the relevant information of the third molar ex-traction a week before the extraction,while the control group were received the same advice immediately before surgery. The two groups of patients filled with anxiety and pain questionnaire and the doctors filled out the patient compliance and therapeutic effect assessment scale. The results were statistically analyzed. Results There was a significant correlation between the level of anxiety and pain after tooth extraction and treatment effect. The pain and anxiety of patients in the experimental group were significantly lower than that of the control group. Patients in the experimental group received a high degree of acceptance of separate consulta-tion,the compliance of anesthesia and surgery was good,and the overall treatment effect was better than that of the control group. And all of the differences were statistically significant(P<0. 05). Conclusion Oral doctors could consider the extraction of pa-tients for a separate consultation before surgery.

Analysis of the Effect of Dental Anxiety on Tooth Extraction in Patients with Cardiovascular Disease

Abstract:Objective To analyze the effect of dental anxiety on tooth extractionin patients with cardiovascular disease,provide guidance for clinical work. Methods A total of 157 cases were selected from patients with cardiovascular disease who underwent tooth extraction by electrocardiograph monitoring in stomatology department. The Modified Dental Anxiety Scale (MDAS) and Wong-Baker Facial Scale was used to assess preoperative anxiety, pain degree of operation. The heart rate and blood pressure were recorded by biological measurements, and the length of operation was recorded.Whether or not could hemostasis after the operation.According to the MDAS scoring standard, patients who scored more than or equal to 13 points were classified as anxiety. Results The age of the patient was (74.73±8.98) years,the anxiety score was (9.95±3.35) points, there was a negative correlation between age of patients and anxiety score (r=-0.383, P<0.05). There was a positive correlation between anxiety score and the degree of pain in the patient, the length of the operation (r=0.340-0.445, P<0.05). Forty-nine cases of anxiety patients and 108 cases of non anxiety patients. The diastolic blood pressure after anesthesia for 5 minutes was (80.24± 8.10) mmHg(1 mmHg=0.133 kPa) in the patients with dental anxiety, the diastolic blood pressure afteranesthesia for 5 minutes was (77.04 ± 9.15) mmHg in the patients without dental anxiety,there were statistic differences in diastolic blood pressure after anesthesia for 5 minutes between the patients of two groups (t=2.11, P<0.05). The effective hemostasis rate of the patients with dental anxiety was 77.551％(38/49) and the patients without dental anxiety was 92.592％(100/108), there were statistical differences in hemostatic effect after the operation between the two groups of patients (χ2=7.169, P<0.05).Conclusions Medical staff can assess dental anxiety condition in patients with cardiovascular diseases who underwent tooth extraction and analysis its effecton surgery, implement targeted and personalized health education, eliminate the negative emotions of patients, reduce the incidence of anxiety, thereby reduce the adverse effects of anxiety on surgery, and guarantee safety during operation.

Abstract:A patient experiencing moderate to severe anxiety often skips regular check-ups, choosing to visit only when necessary. As a result, their oral health worsens. Various variables can affect anxiety such as age, gender, culture, and previous experiences. One of the most frequent oral surgical procedures is extracting the third molar, which is accompanied by severe pain, tension, and anxiety, the use of invasive equipment, and local anesthetic. A combined effect was found of four factors, including the patient, dental employees, setting, and dental procedure, which can trigger and affect dental anxiety. Both pharmacological and non-pharmacological approaches were explored; as a non-drug approach, anxiety was managed by music therapy. Acupuncture, music, audio-visual aids, and lavender oil showed an effect that helped in reducing anxiety. Patients' dental anxiety can be reduced through behavioral management such as educating the patients before the surgical procedure, relaxation strategies, biofeedback, and exposure therapy. Medications include benzodiazepines, nitrous oxide (N2O), and midazolam. Effective treatment relies on collaboration between the patients and dentists; calm patients not only provide a manageable environment for the dental team but can also result in better treatment outcomes.

Abstract:Background: Surgery is an effective method for treating certain diseases. Factors such as disease, preoperative fear and tension, surgical stress, postoperative pain, and related complications directly affect the smooth progression and outcome of surgery. Patients may experience a series of psychological and physiological changes during the perioperative period, resulting in anxiety and depression, which may reduce the pain threshold and worsen their prognosis. Aim: To investigate the effects of a psychological intervention among perioperative patients, based on social cognitive theory (SCT). Methods: We enrolled 200 patients who underwent surgical care at The First People's Hospital of Lin'an District, Hangzhou between January and December 2023. They were categorized into a routine intervention group (n = 103) and a psychological intervention group (n = 97), based on the intervention strategies used. Various assessment tools, including the self-rating anxiety scale (SAS), the self-rating depression scale (SDS), and the Connor-Davidson Resilience scale, were used to measure patients' negative states and emotions. The pre- and post-intervention scores for these metrics in the two groups were then analyzed. Results: In the psychological intervention group, the SAS and SDS scores (31.56 ± 5.18 and 31.46 ± 4.57, respectively) were significantly reduced compared to the routine intervention group (P < 0.05). The visual analog scale pain scores at 12 and 24 hours after intervention (6.85 ± 1.21, 4.24 ± 0.72) were notably higher than those in the routine intervention group (P < 0.05). The psychological intervention group also demonstrated superior scores in perseverance (36.08 ± 3.29), self-reliance (22.63 ± 2.91), optimism (11.42 ± 1.98), and resilience (70.13 ± 5.37), compared to the routine intervention group (P < 0.05). Additionally, the psychological intervention group's confrontation score (23.16 ± 4.29) was higher (P < 0.05). This group also reported lower scores in avoidance (9.28 ± 1.94) and yielding (6.19 ± 1.92) (P < 0.05). Lastly, the Short Form 36 Health Survey scores were significantly higher in the psychological intervention group, indicating a better quality of life (P < 0.05). Conclusion: Psychological intervention measures based on SCT can effectively alleviate pain, anxiety, and depression in perioperative patients.

Abstract:OBJECTIVE:To evaluate whether midazolam with propofol target controlled infusion (TCI) intravenous sedation during the mandibular third molar extraction influences patients'perioperative anxiety.METHODS:The subjects were patients who planned to undergo the mandibular third molar extraction in Peking University School and Hospital of Stomatology, whose state anxiety inventory (SAI) scores were≥38 at the initial visit. They were divided into intravenous sedation group (IVS) and local anesthesia group (LA) on the basis of the planned intravenous sedation. Each group was divided into two subgroups according to the overall SAI scores at the initial visit: IVS-I, LA-I (SAI: 38-50) and IVS-II, LA-II (SAI: 51-80). The anxiety before and after the surgery was evaluated by the SAI scores at the initial visit (T1), before surgery (T2) and 7 days after surgery (T3). The anxiety during the surgery was evaluated by the heart rate, blood pressure and visual analogue scale (VAS) scores.RESULTS:There were no significant differences on SAI at T1, T2, and T3 in the two groups (P>0.05). The heart rate, blood pressure and VAS pain scores of IVS group were significantly lower than those of LA group during the surgery (P<0.001).CONCLUSION:Intravenous sedation with midazolam and propofol TCI was effective on the patients' anxiety during the third molar extraction, which successfully made the patients more comfortable and their heart rate, blood pressure and oxygen saturation more stable during the surgery. But there were no significant differences on the patients'anxiety at the initial visit (T1), before surgery (T2) and 7 days after surgery (T3) according to the SAI scores in the two groups.
Effect of High-quality Nursing Care Combined with Psychological nursing on Intraoperative Stress and Postoperative Negative Emotions of Patients Undergoing General Anesthesia

Abstract:Objective: To evaluate the efficacy of high-quality nursing with psychological nursing on intraoperative stress and postoperative negative emotions of patients undergoing general anesthesia. Methods: Ninety patients undergoing general anesthesia in our hospital between January 2019 and January 2020 were identified as study subjects and equally assigned to the control group and the study group based on the order of admission. The study group received high-quality nursing with psychological nursing, and the control group adopted conventional care. The relevant clinical indicators were compared between the two groups. Results: The study group obtained significantly better intraoperative stress indicators, higher scores of the MOS 36-item short form health survey (SF-36) and higher satisfaction than control group after the intervention (P<0.05). The study group outperformed the control group in the Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS) scores (P<0.05). The study group yielded superior outcomes to the control group in body temperature after intervention and at 1 hour during operation (P<0.05). Conclusion: High-quality nursing with psychological nursing for patients undergoing general anesthesia can effectively increase nursing satisfaction, relieve patients' stress, and eliminate negative emotions, which is worthy of promotion and application.

Abstract:Objective:To assess the intensity of anxiety during extraction of mandibular third molar with different operation process by monitoring patients' heart rate changes.Methods:The study included 60 patients who were extracted the mandibular third molar under local anesthesia.The heart rate was monitored continuously during treatment to measure the anxiety.Different operation processes,including narcotic injection,flap surgery,turbine using,hammer using,dental elevator using,dental forceps using,and suturing operation.Results:Significant increase of heart rate was observed during operation process of narcotic injection,hammer using,dental elevator using,and dental forceps using.It is more significant when the operation time of hammer using and dental elevator using was prolonged.Conclusion:Patients have higher levels of anxiety during the operation process of narcotic injection,hammer using,dental elevator using,and dental forceps using,which is positively correlated with the operating time.

Abstract:Objective: To explore the nursing care of patients undergoing extraction of the extraction of the impacted wisdom tooth after propofol anesthesia.Method: 60 patients have undergone painless extraction of impacted tooth preoperative preparetion and psychological care,closely operative cooperation,carefully postoperative nursing,comprehensive health education reasonable diet guidance.Result: All patients had normal mental status without panic memory and fearful sence of tooth extraction.The ratio of complete indolence in 60 patients reached 99%.Conclusion: Effective perioperative nursing and painic sence which is essential for the successful treatment.

Abstract:BACKGROUND Oral implant surgery is an effective procedure for artificial implants in missing tooth areas under local anesthesia. Because patients under local anesthesia are conscious during this procedure, compared with general anesthesia-related operations, they are more likely to experience negative emotions, such as anxiety and tension. These emotional reactions result in shivering and chills in the limbs, leading to poor doctor-patient cooperation and even avoidance of treatment. In traditional Chinese medicine, it is believed that acupoint massage regulates blood and Qi, dredge menstruation, and relieve pain, which is beneficial for patients’ emotional adjustment; however, there are few related clinical studies. AIM To observe the changes in anxiety and pain in patients with oral implant after acupoint massage combined with touch therapy. METHODS One hundred patients undergoing oral implantation in our hospital between May 2020 and May 2023 were randomly divided into control and study groups, according to a random number table, with 50 patients in each group. The control group received routine intervention, and the study group received acupoint massage combined with touch on the basis of the control group. Anxiety [assessed using the Modified Dental Anxiety Scale (MDAS)], pain severity, blood pressure, heart rate, and satisfaction were compared between the two groups. RESULTS Before intervention, the difference in MDAS score between the two groups was not significant (P > 0.05), while after the intervention, the MDAS scores decreased in both groups compared with those before the intervention (P < 0.05); the MDAS score of the study group was lower than that of the control group, with a statistically significant difference (P < 0.05). The degree of pain in the intervention group was significantly lower than that in the control group (P 0.05). The systolic and diastolic blood pressures and heart rate in the intervention group, during and after the intervention, were significantly lower than those in the control group (P < 0.05). The total degree of satisfaction in the study group was significantly higher than that in the control group (P < 0.05). CONCLUSION Acupoint massage combined with touch better relieves anxiety and pain in patients undergoing dental implant surgery, improving the perioperative comfort of these patients and ensuring safety and a smooth operation.

Effects of nitrous oxide inhalation on dental anxiety during impacted wisdom tooth extraction

Abstract:AIM: To evaluate the clinical effects of nitrous oxide inhalation on dental anxiety during extraction of impacted mandibular wisdom teeth. METHODS: 70 patients with impacted mandibular wisdom tooth extraction were included in this study. The patients were divided into the nitrous-oxide inhalation group( n = 30) and local anesthesia group( n = 40). Blood pressure,heart rate,breathing and variation of calm situation,degree of anxiety,pain score were recorded before,during and after tooth extraction. The parameters were statistically analysed by t- test. RESULTS: All teeth were successfully extracted and with no serious complications. Nitrous oxide inhalation group showed much less changes in heart rate and blood pressure; much lower dental anxiety score,VAS pain score and postoperative forgetfulness than local anesthesia group( P 0. 05). CONCLUSION: Nitrous oxide inhalation is effective in alleviating dental anxiety during impacted wisdom tooth extraction.

Abstract:Purpose: To investigate the impact of perioperative whole high-quality nursing care on psychological status, vital signs and anesthetic medication of patients undergoing painless gastrointestinal endoscopy, and also to provide a reference for reducing adverse risks and improving safety of painless gastrointestinal endoscopy. Methods: Databases such as Pubmed, Embase, Web of Science, and Cochrane Library were retrieved. Literature was selected based on established standards, and quality evaluations were performed to extract required data. Finally, 13 pieces of literature were included for meta-analysis of relevant data. Results: In a meta-analysis of 13 relevant randomized controlled trials (RCTs), patients receiving high-quality perioperative nursing care experienced significant improvements in self-reported anxiety and depression levels, vital signs indicators such as systolic and diastolic blood pressure, mean arterial pressure, and heart rate, as well as decrease in narcotic drug dosage. Diagnosis and treatment time also significantly decreased (p < 0.05). Additionally, the incidence of respiratory depression was reduced (p < 0.00001). Conclusion: This meta-analysis suggests that perioperative high-quality nursing care alleviates the psychological stress of patients with painless gastrointestinal endoscopy, reduces the amount of anesthesia, and effectively reduces application time of endoscopy

Abstract:Objective To investigate the influence of nursing intervention on perioperative psychological status and treatment compliance of patients undergoing breast augmentation. Methods 82 patients were investigated with self-rating anxiety scale (SAS), self-rating depression scale (SDS), self-designed mental activity questionnaire on patients with breast augmentation. And they were divided into the intervention group(42 patients) and the control group(40 patients) randomly. The control group was given routine treatment and nursing. The intervention group received systematic nursing intervention such as psychological needs,confidentiality, the relevant specialist knowledge and explanation and prevention o complications besides routine treatment and nursing care. The perioperative psychological status and treatment efficacy were compared between the two groups. Results The worrying degree about operation - related problems was lower in the intervention group compared with that of the control group, but SAS and SDS score showed no significant difference one day before operation between the two groups. SAS and SDS score of the intervention group were improved before discharge compared with those before operation and were also better than those of the control group. Conclusions Patients undergoing breast augmentation surgery subject to a variety of perioperative psychological barriers, systematic nursing intervention can significantly improve the psychological state and treatment compliance, make them to acquire the unification of perfect body shape and psychology with best physical and mental status during perioperative stage.
The Effects of Music Listening During Extraction of the Impacted Mandibular Third Molar on the Autonomic Nervous System and Psychological State

Abstract:Purpose: Pain, anxiety, and nervousness related to dental procedures can cause acute changes in the autonomic nervous system. Music is widely accepted as a relaxation method during dental treatment; however, its effects during dental treatment are unclear. The authors explored the effects of listening to music during extraction of the impacted mandibular third molar on the autonomic nervous system and the psychological state and hypothesized that listening to music would suppress sympathetic nervous activity and decrease anxiety. Materials and methods: In this prospective study, 40 patients scheduled for extraction of an impacted mandibular third molar were randomized into 2 groups: extraction without music (control group) and extraction while listening to music (music group). Heart rate variability was recorded during the experiment, and Modified Dental Anxiety Scale and State-Trait Anxiety Inventory (STAI) scores were recorded before and after the procedure. Descriptive and bivariate statistics were computed and the P value was set at .05. Results: An increased low-to-high frequency ratio was observed in the control group during incision and flap reflection, bone removal, and separation of the tooth crown; the ratio was significantly decreased in the music group during these time points (P < .05). Compared with the control group, the music group had a significantly greater decrease in postoperative STAI State Anxiety scores from preoperative levels (P < .05). Conclusions: This study suggested that listening to music while undergoing extraction of the impacted mandibular third molar suppresses activity of the sympathetic nerves during incision, flap reflection, bone removal, and separation of the tooth crown and relieves anxiety after treatment. Future studies will focus on the mechanisms involved and methods to prevent the onset of systemic incidents.
Clinical observation of submucosal injection with different doses of dexamethasone to improve postoperative symptoms of mandibular third molar extraction

Abstract:Objective To study the clinical effect of submucosal injection of dexamethasone to improve postoperative symptoms of mandibular third molar extraction.Methods Patients (156 cases) with mandibular third molar extraction in Zhongshan Hospital of Fudan University and Shanghai Jingan Dental Clinic from August 2016 to November 2016 were randomly divided into control group (53 cases), dexamethasone 4 mg group (51 cases) and dexamethasone 8 mg group (52 cases). Patients in the control group were submucosal injected with normal saline within 1 cm range of mandibular third molar. Patients in the dexamethasone 4 mg group were submucosal injected with 4 mg Dexamethasone Sodium Phosphate Injection within 1 cm range of mandibular third molar. Patients in the dexamethasone 8 mg group were submucosal injected with 8 mg Dexamethasone Sodium Phosphate Injection within 1 cm range of mandibular third molar. After treatment, the postoperative symptom score (PoSSe) was evaluated, and postoperative swelling and maximum mouth opening degree among three groups were compared. After treatment, the scores of diet, speech, swelling, pain, daily life and total scores in the dexamethasone 4 mg and dexamethasone 8 mg group were lower than those in the control group, and there were difference among three groups (P < 0.05). The observational indexes in the dexamethasone 8 m group were significantly better than those in the dexamethasone 4 mg group, with significant difference between two groups (P<0.05). After treatment, the 1 grade swelling percentage in dexamethasone 8 mg group was significantly higher than those in the control and dexamethasone 4 mg groups, but the 3 and 4 grade swelling percentage in dexamethasone 8 mg group was significantly lower than those in the control and dexamethasone 4 mg groups, and there was difference among three groups (P < 0.05). After 1 week of surgery, the maximum mouth opening degree in the dexamethasone 4 mg and dexamethasone 8 mg group were significantly bigger than that in the control group, and there was difference among three groups (P < 0.05).Conclusion Submucosal injection with dexamethasone in mandibular third molar can improve postoperative symptoms, can decrease postoperative swelling, increase maximum mouth opening degree after 1 week of surgery, and dexamethasone with dose of 8 mg is better than dexamethasone with dose of 4 mg, which has a certain clinical application value.
EFFECT OF NURSING INTERVENTION ON THE PERIOPERATIVE STRESS RESPONSE IN GLIOMA PATIENTS

Abstract:Objective: To study the effect of nursing intervention on perioperative period stress response of glioma patients. Methods: The clinical data of glioma patients undergoing surgery in our hospital were retrospectively analyzed and divided into two groups according to different nursing methods. The control group was given routine nursing. The observation group was given standardized nursing intervention on the basis of the control group. The clinical indicators, psychological status and pain scores were compared between the two groups. Results: The heart rate and systolic blood pressure in the observation group before and after the surgery were lower than those in the control group, and the differences were statistically significant. Conclusion: Nursing intervention has a positive effect on the perioperative stress response of glioma patients. It can maintain the stability of vital signs, improve the patient's psychological state and effectively reduce the pain, which is worthy of clinical promotion.

Abstract:In order to explore the clinical effect of psychological nursing intervention on postoperative chemotherapy for rectal cancer, 120 cases of rectal cancer patients were selected as the research subjects. The control group received conventional nursing treatment after operation, and the research group received comprehensive psychological nursing intervention on this basis. The self-rating anxiety scale (SAS) scores, self-rating depression scale (SDS) scores, hope level scores, nursing satisfaction, mental state changes, treatment compliance, and immune function of two groups were analyzed and compared. There was no significant difference between the two groups of patients in the preoperative SAS, SDS, and hope level scale scores. After the intervention, postoperative SAS and SDS scores and CD8+ value of the research group were significantly lower than those of the control group. In contrast, the postoperative hope level score, treatment compliance, and postoperative CD4+/CD8+ of the research group were significantly higher, and the nursing satisfaction was better than that of the control group. The application of psychological nursing intervention in postoperative chemotherapy for patients with rectal cancer can effectively relieve anxiety and depression of patients, promote patients to establish a healthy and coordinated mental state, improve treatment compliance, improve immune function, and promote disease recovery.

Abstract:Objective: To compare the psychological care in children's tooth extraction with/without care and to establish a nursing model.Methods: One hundred and ninety children required extraction were recruited in the study.95 of these cases were equally distributed into two groups: observation and control groups.The observation group was under all psychological care during tooth extraction,while the control group was under conventional extraction model.Results: Psychological care greatly reduced dental anxiety.The satisfaction proportion of the observation group was 95.8% while the control group was 80%.Conclusion: The implementation of psychological care in children's tooth extraction can effectively reduce dental anxiety and cause their high coordination and success rate.

Abstract: Background: Surgical removal of the impacted third molar is associated with inflammatory morbidities which include swelling, trismus, and pain. Pain is the most common postsurgical morbidity associated with third molar surgery. It remains an important factor in patients' perception of recovery after third molar surgery with dental anxiety reported to exert influence on its threshold. Objective: The aim of the study was to determine if preoperative dental anxiety has any significant role on postoperative pain perception after third molar surgery. Materials and Methods: This was a cohort study involving sixty consecutive adult patients requiring extraction of impacted mandibular third molars under local anesthesia. Modified Dental Anxiety Scale Questionnaire was administered to each participant in the waiting area before the surgery. The visual analog scale was also given to each participant to be completed once daily at approximately the same time as the surgery time until day 7 after the surgery. Data collected were analyzed using SPSS version 23. Results: Sixty participants who consented to third molar surgery took part in this study. Five participants were lost to follow-up. There were slightly more males (50.9%) than females (49.1%). Nineteen participants in this study had moderate dental anxiety (34.5%) and 6 participants (11%) had severe dental anxiety with more females having moderate-to-severe dental anxiety. The correlation between pain perception at different days and dental anxiety was not significant (P > 0.05). Conclusion: Preoperative dental anxiety may not significantly influence pain perception after third molar surgery.
Effects Of Comprehensive Nursing Interventions On Stress In Patients Undergoing Percutaneous Coronary Intervention

Abstract:Objective: To discuss the effects of comprehensive nursing interventions on stress in patients undergoing percutaneous coronary intervention (PCI). Methods: A total of 110 coronary heart disease (CAD) patients who received PCI in the Second Affiliated Hospital of Nanchang University between January 2017 and July 2017 were enrolled in the study. Their baseline characteristics were retrospectively reviewed and they were randomized into the observation group (n = 55) and the control group (n = 55). The control group received conventional nursing interventions, while the observation group received comprehensive nursing interventions. Patients' stress levels were compared between the two groups through the serum levels of malondialdehyde (MDA) and lipid peroxidase (LPO); the quality of life was compared between the two groups through the scores of The MOS 36-Item Short-Form Health Survey (SF-36); the mental state was compared between the two groups through the scores of Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS); and the self-care agency was compared between the two groups through the scores of Exercise of Self-Care Agency (ESCA). Any complication occurred during the surgery was recorded in two groups. Results: After nursing interventions, the serum levels of MDA and LPO in two groups were significantly reduced (all P < 0.001), and were significantly lower in the observation group than in the control group (all P < 0.01); the scores of SF-36 in two groups were significantly increased, while the scores of SAS and SDS in two groups were significantly decreased (all P < 0.001). Statistical differences were also found between the two groups in the scores of SF-36, SAS and SDS after nursing interventions (all P < 0.01). After nursing interventions, scores on four factors of ESCA, namely motivation, knowledge base, active vs. passive response to situations and sense of self-worth, were significantly increased in two groups (all P < 0.001), and were significantly higher in the observation group than in the control group (all P < 0.01); the observation group was significantly higher than the control group in patient satisfaction (P = 0.031). The incidence of postoperative complications was significantly lower in the observation group than in the control group (P = 0.023). Conclusion: Comprehensive nursing interventions could considerably relieve stress and negative emotions for patients undergoing PCI, such as anxiety and irritability. It could lower the incidence of complications and significantly improve patients' self-care agency and their quality of life.
