Abstract:OBJECTIVE To explore the risk factors and intervention methods of periprosthetic infections in patients with total hip arthroplasty.METHODS A total of 306 patients with total hip arthroplasty in the hospital from Jan.2010 to Dec.2016 were enrolled in this study.The clinical symptoms,such as body temperature,joint pain and changes of inflammatory markers were compared with the use of antibacterials,gender,time of surgery,age,history of hormone use,obesity,immunological diseases,diabetes and so on.RESULTS There were 19 cases of periprosthetic infections in 306 patients,and the incidence rate was 6.21％.Totally 23 strains of gram-positive bacteria were cultured and isolated,mainly Staphylococcus aureus.The use of antibacterial drugs,operation time,immunological disease,diabetes and hormone were the factors associated with infections (P＜0.05).The neutrophil count,patients after treatment (Harris),erythrocyte sedimentation rate,visual analogue scale (VAS),Creactive protein and white blood cell counts of patiens after treatment were (57.20 ± 5.13) ％,(93.06 ± 4.98)points,(17.24±4.53)mm/h,(4.99±2.08)points,(10.28±1.34)mg/L and (8.06±2.05)×109/L,which had significant differences compared with those before treatment (P＜0.05).After treatment,4 cases were improved and 15 cases were cured.CONCLUSION The clinical symptoms of periprosthetic infections after artificial joint replacement are obvious,and were related to the patient's own diseases,the use of antibiotics and the time of operation.In the treatment,patients should be combined with their own situations to reduce the drug treatment and operation time,especially to take control measures on critically ill patients to reduce the incidence of periprosthetic infections.
Partial Two?stage Exchange for Periprosthetic Joint Infection after Total Hip Arthroplasty

Abstract:Objective To evaluate the early stage effects of partial two?stage exchange (PTE) for infection after total hip arthroplasty (THA) in order to improve surgical treatment outcomes. Methods Twelve patients (7 males and 5 females) of infect?ed THA treated by PTE between September 2000 and February 2016 were included with an average of 59 years old (range, 40-74 years old). The femoral stem prosthesis was preserved when we ensured that it was not infected and with well fixation during opera?tion. The acetabular prosthesis was totally replaced. Furthermore, the secondary surgery was performed on patients who were free from infection for at least 3 months. At the follow?up duration, the pelvic X?ray was examined to access the presence of loosening of the prosthesis. The inflammatory index was recorded. Hip function was assessed by the Harris hip score, and the visual analogue score (VAS) and patient's subjective satisfaction were recorded by the questionnaire. Results All of the 12 patients were fol?lowed up for a mean of 33.3 months (range, 24-48 months). We achieved a 92% (11/12) success rate. One patient resuffered infec?tion, which was controlled after the second debridement. The preoperative Harris score of 12 patients was 23-57, with an average of 40.83±10.62. All patients had significant improvement in hip function after PTE. The Harris score reached 59.58±4.34 at one month after surgery, and reached 64.58±9.08 after three months and 86.75±4.58 at the last follow?up. There was significantly dif?ferent when compared with the preoperative Harris score. At the last follow?up, the length difference between the lower limbs was less than 1cm in all patients. Two patients did not complain of pain, while other 10 patients got a VAS score of less than 2 points. The patient's subjective satisfaction rating was 100%. Conclusion PTE could be a valuable alternative treatment for infection af?ter THA. The infection control rate and clinical outcomes are comparable to those of two?stage exchange.
Debridement, Antibiotics, and Implant Retention for Acute Periprosthetic Joint Infection

Abstract:Objective: To report on our clinical outcomes and on the experience of managing acute periprosthetic joint infection (PJI) with debridement, antibiotics, and implant retention (DAIR). Methods: We performed a retrospective review of all patients who were diagnosed with acute PJI after primary hip or knee replacement surgeries and who were managed with DAIR in our prospective joint replacement registry from 2008 to 2019. The diagnosis of PJI was made according to the 2011 Musculoskeletal Infection Society (MSIS) criteria. The symptom onset duration, inflammatory marker levels (i.e. C-reactive protein [CRP], erythrocyte sedimentation rate [ESR], white cell count [WBC], and synovial WBC count), functional scores including the Knee Society Score (KSS), the KSS functional score and the Harris Hip Score (HHS), bacteriology, and surgical outcomes of the patients were tracked and recorded. A paired sample of joint fluid and tissues was also sent for a metagenomic next-generation sequencing (mNGS) test. A paired-samples t-test was used to compare the differences in the inflammatory markers and functional scores before and after surgery. Results: A total of 24 patients with 7 infections after hip replacements and 17 infections after knee replacements were included. A total of 21 patients exhibited early postoperative infections, and 3 exhibited late acute hematogenous infections. During a mean follow-up time of 29.2 ± 15.1 months, 22 patients were successfully treated, whereas 2 patients were unsuccessfully treated and required repeated DAIR. The overall success rate of DAIR was 91.7%. For staphylococcal infections, DAIR had a 100% success rate. Five patients who presented with symptoms between 4 and 8 weeks also achieved a 100% success rate. At the last follow-up, the mean CRP level decreased from 52.6 ± 34.0 to 5.4 ± 3.5 (P < 0.001), and the mean ESR level decreased from 72.3 ± 34.3 to 20.2 ± 12.1 (P < 0.001). The mean KSS score increased from 44.8 ± 12.2 to 81.4 ± 9.2 (P < 0.001), and the mean KSS functional score increased from 38.1 ± 3.5 to 73.9 ± 23.0 (P < 0.001), and the mean HHS score increased from 34.4 ± 6.9 to 84.1 ± 15.1 (P < 0.001). The overall pathogen identification rate was 91.7% (22/24 cases). The success rates for Staphylococcus, Streptococcus, and the other pathogens were 100% (9/9 cases), 71.4% (5/7 cases), and 100% (6/6 cases), respectively. Conclusion: Debridement, antibiotics, and implant retention has a high success rate for the treatment of acute PJI and may be performed in selected patients whose symptoms have been sustained for over 4 weeks. A high rate of success for staphylococcal infections was reported with the use of DAIR.
Debridement with Implant Retention and Postoperative Continuous Irrigation in the Treatment of Acute Prosthetic Joint Infection after Arthroplasty

Abstract:[Objective]To assess the efficiency of debridement with implant retention and postoperative continuous irrigation in the treatment of acute prosthetic joint infection,and investigate the factors which may have influence on the result of the management. [Methods]We retrospectively collected data from 11 cases of acute prosthetic infection that were managed with debridement with implant retention and postoperative continuous irrigation from June 2003 to March 2009.Six cases were periprosthetic infections after total hip arthroplasty,and 5 cases were prosthetic joint infectione after total knee arthroplasty.In addition,there were 7 cases of early postoperative infection and 4 cases of hematogenous infection.Before the implant retention surgery,erythrocyte sedimentation rate,CRP and aspirated fluid culture were routinely conducted.When the debridement was carried out in hip,the joint was dislocated and all grossly infected and necrotic tissue was oberliterated.In the procedure of knee surgery,tibial polyethylene component was removed and replaced with a new one.Of the 11 cases,3 implant retention surgeries were performed at one week after onset of symptoms associated with acute infection.[Results]The mean follow-up was 41.4 months.There were 4 cases of early postoperative prosthetic infection and 2 of acute hematogenous infection that retained the prosthesis without further evidence of infection.The success rate was 54.5%.[Conclusion]Debridement with implant retention and operative continuous irrigation has a good therapeutic effect in the treatment of early postoperative prosthetic infection and acute hematogenous periprosthetic infection.If the surgery can be done within four weeks after onset of symptoms,some cases of infection may be controlled.The types and virulence of causative organisms have been implicated as a significant predictor of outcome.

Abstract:Abstract Background The debridement, antibiotics, and implant retention (DAIR) procedure is an established therapeutic option for periprosthetic knee infections (PKI). However, the efficacy and the indication for this procedure are still controversial. Methods All the relevant literatures were systematically reviewed and analyzed. The present study aimed to assess the success rate of DAIR in the management of PKI, identify the factors associated with prognosis of DAIR, and establish a simple algorithm for predicting a high success rate of DAIR. Results Totally, 33 studies with 1266 cases were included. The overall success rate following DAIR in the management of PKI was 57.11%. In the subgroup analyses, the factors of “the time from symptoms to debridement was < 3 weeks” and “the bacterial species other than methicillin-resistant Staphylococcus aureus” significantly improved the success rate of DAIR and thus were defined as the major criteria. The statistically insignificant factors of “the open debridement and liner exchange” and “the comorbidity of rheumatoid arthritis” were set as the minor criteria. The success rate of DAIR for PKI meeting all the major criteria and no less than one minor criterion was 80.98%, which was significantly higher than the overall success rate of DAIR (p < 0.05). Conclusion PKI cases meeting two major criteria and no less than one minor criterion may confer a high success rate of DAIR. This simple algorithm may contribute to identifying the appropriate PKI patient for DAIR treatment and predicting the prognosis of DAIR.
Acute Delayed or Late Infection of Revision Total Hip Arthroplasty Treated with Debridement/Antibiotic-loaded Cement Beads and Retention of the Prosthesis

Abstract:PURPOSE: The treatment of infected revision total hip arthroplasty (THA) is very challenging due to retained revision prosthesis, poor bone stock and soft tissue condition derived from previous revision surgeries, and comorbidities. The purpose of this study was to investigate the effectiveness and short-term outcomes of aggressive debridement and use of antibiotic-loaded cement beads with retention of the prosthesis for acute delayed or late infection of revision THAs.MATERIALS AND METHODS: Ten consecutive patients with symptoms or signs of less than one-week evolution and well-fixed prostheses, were treated with this procedure and a postoperative course of organism-specific antibiotics for a minimum of 6 weeks. All hips presented with acute delayed or late infection of revision THAs. Patients with a mean age of 68.1 years (range, 59-78 years) underwent an average of 1.9 previous revision THAs (1-4) before the index surgery. The minimal follow-up was 2 years with a mean of 46.2 months (range, 24-64 months).RESULTS: There were 8 cures (80.0%) and 2 failures with no mortality during the study period. The 2 failures involved the same and resistant bacteria implicated in the primary infection (methicillin-resistant <i>Staphylococcus aureus</i> and <i>Prevotella oralis</i>, respectively). The mean Harris hip score was 65.2 (range, 26-83) and the mean visual analogue scale was 2.6 (range, 1-4) at final follow-up.CONCLUSION: With a favorable success rate and no mortality, our procedure may be considered a safe and effective alternative for the treatment of acute delayed or late infection of revision THAs with well-fixed prostheses.

Abstract:Introduction Periprosthetic joint infection (PJI) is a severe complication following total joint arthroplasty (TJA). This study aimed to investigate the dynamics of synovial fluid markers following single-stage exchange arthroplasty or debridement combined with antibiotics and implant retention (DAIR) with topical antibiotic infusion for PJI. Methods This retrospective study analyzed patient records at a tertiary hospital from March 1, 2018, to May 1, 2023. Patients who received single-stage exchange arthroplasty or DAIR followed by intra-articular antibiotic infusion for PJI were included. Basic demographic details, comorbidities, Charlson Comorbidity Index (CCI) scores, microorganism profile, presence of sinus tract, and antibiotic treatment type were collected. Synovial fluid samples were collected preoperatively and postoperatively every two days for 14 days to quantify synovial white blood cell (WBC) count and polymorphonuclear cell percentage (PMN%). Results The study included 140 patients who had a mean age of 63 years and a mean body mass index (BMI) of 25. The results showed a steady decrease in synovial WBC count from preoperative levels to Day 14 postoperative. Patients who had successful outcomes had significantly higher preoperative WBC counts compared to those who had a treatment failure. The synovial PMN% initially increased postoperatively, peaking at days one to two, and then gradually declined. Patients who had successful outcomes showed a faster decline in PMN% compared to those who had persistent infections. Different bacteria exhibited varying preoperative synovial WBC counts and PMN%, but these differences were not statistically significant. Conclusion Monitoring synovial WBC count and PMN% can help distinguish between normal postoperative inflammation and persistent infection. Higher preoperative synovial WBC counts are associated with successful outcomes, suggesting their potential role in predicting treatment success. Future research with larger sample sizes is necessary to further validate these findings and improve the management and diagnosis of PJI.

Methods and probability of success after early revision of prosthetic joint infections with debridement, antibiotics and implant retention

Abstract:Prosthetic joint infection (PJI) is a rare-and dreaded-complication of arthroplasty requiring multidisciplinary care. Given the dual goal of treating the infection and maintaining satisfactory function, it is preferable to determine how and when the implanted components can be retained. Bacteria and fungi organize themselves into biofilms that shield them from antibiotics and the immune system. This biofilm is in place after 15 days of active infection. Some antibiotics have a better activity on biofilms. The following factors have a negative impact on the probability of a successful debridement, antibiotics and implant retention (DAIR) intervention: fracture or revision, use of cement, bacteremia, kidney and/or liver failure, immunosuppression and elevated CRP. Hematogenous infections have a worse prognosis than early postoperative infections. Using a decision algorithm increases the chances of DAIR being successful. The KLIC score applies to early postoperative infections (<4 weeks postoperative and<3 weeks from the first signs) while the CRIME-80 score applies to hematogenous infections (<3 weeks from the first signs). Arthroscopic treatments have no role here, whereas DAIR through an arthrotomy is well standardized. Wide spectrum antibiotic therapy, secondarily adapted to the causative microorganism, is indicated for a total of 3 months. The results against the infection are mixed, although following a decision algorithm resolves the infection in about 75% of cases. The functional outcomes and quality of life are close to those of patients who have undergone primary joint replacement. It is not recommended to carry out a second DAIR if the first one fails. It is logical to apply the principles set out for the hip and knee to other joint replacements, and to use the same algorithm. For the upper limb, and especially for reverse shoulder arthroplasty, one must be careful about Cutibacterium acnes infections as they are hard to diagnose. Surgeons should not hesitate to contact a referral center for any PJI, although it is preferable that early infections be treated at the facility that performed the implantation.
Observation of prevention strategy for conversion total hip arthroplasty:the outcome with at least two years follow-up

Abstract:Objective To observe the mid-term outcome of treatment procedures for preventing periprosthetic joint infection（PJI） in conversion total hip arthroplasty（cTHA）. Methods Clinical data of 54 patients underwent cTHA managed with treatment procedures for preventing PJI and followed-up for at least 2 years in Division of Sports Medicine and Adult Reconstructive Surgery, Department of Orthopedic Surgery, Nanjing Drum Tower Hospital between January 2018 and June 2020 were retrospectively analyzed. There were 29 males and 25 females with an age of（68.3±12.1）（43-82）years. Reason for cTHA included femoral head avascular necrosis after internal fixation with cannulated screws for femoral neck fracture in 31 cases and after intramedullary nail fixation of intertrochanteric fractures in 8 cases, non-union of femoral neck fracture in 4 cases and secondary osteoarthritis after internal fixation for acetabular fracture in 11 cases. The type of previous operation and preoperative erythrocyte sedimentation rate（ESR） and C-reactive protein（CRP） were compared between patients with and without positive finding in intraoperative tissue culture. Results The follow-up time was（35.0±8.7）（24-54）months. Bacteria was identified by culture of joint fliud in 2 patients before cTHA, while in 9 patients by intra-operative tissue biopsy. The ESR and CRP of the 11 patients were higher than that of patients with negative intra-operative tissue culture, although the difference was not statistically significant. PJI occurred in 3 patients（5.6%） after cTHA within 6 months, and treated with primary revision. The pre-operative CRP was elevated and more than 12 mg/L in all the 3 patients. Patients showed significantly higher Harris score at last follow-up（87.4±9.3） when compared to the pre-operative one（47.3±16.4）（P＜0.001）. Conclusion Patients underwent cTHA would confront higher risk of PJI. Our algorithm by optimizing the diagnosis of infection before and during operation, the usage of antibiotics and the nutrition support therapy could reduce the development of PJI.

Abstract:Background: Periprosthetic joint infection (PJI) represents a challenge following hip or knee arthroplasty, demanding immediate intervention to prevent implant failure and systemic issues. Bacterial biofilm development on orthopedic devices worsens PJI severity, resulting in recurrent hospitalizations and significant economic burdens. The objective of this retrospective cohort study is to evaluate the efficacy of this novel antiseptic solution, never previously evaluated in vivo, in managing early post-operative or acute hematogenous PJI following primary hip and knee joint replacements. Methods: The inclusion criteria consist of patients with total hip arthroplasty (THA) or knee arthroplasty diagnosed with acute PJI through preoperative and intraoperative investigations, in accordance with the MSIS ICM 2018 criteria. The minimum required follow-up was 12 months from the cessation of antibiotic therapy. This novel antiseptic lavage solution is composed of ethanol, acetic acid, sodium acetate, benzalkonium chloride and water. Data included demographic characteristics, diagnostic criteria, surgical techniques, post-operative treatment and follow-up outcomes. Results: A total of 39 patients treated with Debridement, Antibiotics Pearls and Retention of the Implant (DAPRI) procedures using this solution between May 2021 and April 2023 were analyzed. At a mean follow-up of 24.6 ± 6.4 months, infection recurrence-free survival rates were 87.2%, with no local allergic reactions or relevant systemic adverse effects detected. Persistent PJI necessitated two-stage revision surgery. Conclusions: This novel antiseptic lavage solution shows promise as an adjunctive tool in the treatment of PJI, demonstrating support in infection control while maintaining a favorable safety profile.

Validation of a Classification System for Optimal Application of Debridement, Antibiotics, and Implant Retention in Prosthetic Joint Infections following Total Knee Arthroplasty: A Retrospective Review

Abstract:Introduction: Periprosthetic joint infection (PJI) remains a serious complication after total knee arthroplasty (TKA). While debridement, antibiotics, and implant retention (DAIR) are considered for acute PJI, success rates vary. This study aims to assess a new scoring system's accuracy in predicting DAIR success. Methods: 119 TKA patients (2008–2019) diagnosed with PJI who underwent DAIR were included for analysis. Data were collected on demographics, laboratory values, and clinical outcomes. This was used for validation of the novel classification system consisting of PJI acuteness, microorganism classification, and host health for DAIR indication. Statistical analysis was carried out using SPSS programming. Results: Mean follow-up was 2.5 years with an average age of 65.5 ± 9.1 years, BMI of 31.9 ± 6.2 kg/m2, and CCI of 3.04 ± 1.8. Successful infection eradication occurred in 75.6% of patients. The classification system demonstrated 61.1% sensitivity, 72.4% specificity, and 87.3% positive predictive value (PPV) when the DAIR cutoff was a score less than 6. For a cutoff of less than 8, sensitivity was 100%, specificity was 37.9%, and PPV was 83.3%. Conclusions: To date, no consensus exists on a classification system predicting DAIR success. This novel scoring system, with high PPV, shows promise. Further refinement is essential for enhanced predictive accuracy.

Abstract:Objective To investigate risk factors for prosthetic joint infections after primary hip and knee arthroplasty ,and to provide the basis for prevention form prosthetic joint infections in the early stage.Methods A retrospective analysis was carried out on the clinical information of patients who underwent the primary hip and knee arthroplasty in the Center of Diagnosis and Treatment for Joint Disease of Nanjing Drum Tower Hospital from Mar .2013 to Mar.2016 and were divided into case group and the control group.The patients who suffered from postoperative PJI were attributable to the case group .A total of 28 cases,among them,13 hips(11 hips were total hip arthroplasty and two were hemiarthroplasty ) and 15 knees(one case was bilateral total knee arthroplasty ) were assigned to the case group.According to 1:2 ratio,in the same period 56 patients without prosthetic joint infections were chosen as the control group . The data were compared between the two groups using SPSS 17.0 software.The measurement data of the two group were compared by t-test,and the enumeration data of the two group were compared by chi-square test.The single factors treated as independent variable with significance were analyzed by the logistic regression analysis .Results A total of 10 pathogen strains were detected in the patients with prosthetic joint infections,mainly gram-positive bacteria.Staphylococcus epidermidis caused 38.4％ PJI in hip arthroplasty ,and staphylococcus aureus accounted for 37.3％ PJI in knee arthroplasty .Body mass index (BMI),operation time,volume of drainage,hospital stay,operation history of the incision,long-term use of steroids or immuno-suppressor ,preoperative hypo-albuminemia ,asymptomatic bacteriuria ,superficial infection were risk factors for PJI after primary hip and knee arthroplasties ( t=4.73、8.27、5.913、3.461、7.524、3.021、7.721、4.519、2.918 and all P <0.05 ).The logistic regression analysis showed that advanced BMI ,long operation time ,large volume of drainage ,long hospital stays ,past-operation history of incision,long-term use of steroids or immunosuppressor ,preoperative hypoalbuminemia ,asymptomatic bacteriuria,superficial infection were independent risk factors for prosthetic joint infections (P<0.05). Conclusions The consequences suggest that pathogens of prosthetic joint infections after primary arthroplasty are mainly gram-positive bacteria .Advanced BMI ,long operation time ,bigger volume of drainage,long hospital stays,past-operation history of incision,long-term use of steroids or immunosuppressor ,preoperative hypoalbuminemia ,asymptomatic bacteriuria ,superficial infection are remarkably inclined to increase the incidence of periprosthetic infections .

Abstract:The onset and duration of clinical relaxation of atracurium and vecuronium were studied in patients on chronic nifedipine therapy and compared to similar groups of patients not on such a therapy. Neuromuscular block was monitored by recording the integrated electromyogram of the hypothenar muscles (using a 'Datex Relaxograph') following stimulation of the ulnar nerve with a train-of-four stimulation. Neither the time to onset of maximum block (3.0-3.4 min) nor the duration of clinical relaxation (36-40 min) differed significantly between the groups indicating no significant interaction in humans between atracurium and vecuronium and chronic nifedipine therapy.

Abstract:BackgroundTwo-stage exchange arthroplasty is a feasible treatment for chronic PJI (periprosthetic joint infection) of total hip arthroplasty (THA). However, there are large numbers of patients who are unable to ultimately receive reimplantation after resection arthroplasty owing to uncontrolled infection and multiple comorbidities. The purpose of the current study was to identify patient-related risk factors and reasons for being unable to undergo revision THA.MethodsIndividuals undergoing resection arthroplasty for chronic PJI treatment from 2013 to 2020 at our institution were retrospectively reviewed. A variety of patient comorbidities, laboratory data, isolated pathogens, and follow-up status were collected for analysis.ResultsA total of 84 patients (46 men and 38 women) with a 2.7 ± 2.1-year follow-up were analyzed. Thirty-eight (45.2%) patients eventually underwent revision THA, while the other 46 (54.8%) did not receive reimplantation during follow-up. The patients without receiving reimplantation had higher Charlson comorbidity index (CCI) score (3.1 ± 2.9 versus 1.2 ± 1.5; p = 0.001). Lower cumulative incidence of receiving reimplantation was observed in patients with chronic kidney disease (log-rank test, p = 0.019), anemia (p = 0.011), presence of initial fever (p = 0.030), and oxacillin-resistant strain infection (p = 0.030). The most common reasons for not receiving reimplantation were uncontrolled infection, unstable medical conditions, and death. The patients without reimplantation had a relatively higher mortality rate (log-rank test, p = 0.002).ConclusionChronic hip PJI with poor medical conditions or oxacillin-resistant strain infection decreased the chance of undergoing revision surgery. These patients had unfavourable outcomes and a higher mortality rate after resection arthroplasty.

Abstract:Background: Periprosthetic joint infection (PJI) following total knee arthroplasty is a serious complication lacking evidence-based diagnostic and treatment protocols, particularly in ruling out persisting infection before reimplantation. Methods: This retrospective analysis assessed the mid-term outcomes of 66 patients undergoing septic two-stage knee revision surgeries from 2007 to 2013, diagnosed as per the Musculoskeletal Infection Society criteria. After implant removal and antibiotic treatment, reimplantation decisions were based on either joint aspiration, blood counts, and clinical examination (group A) or an open biopsy (group B). Both groups underwent meticulous debridement and spacer exchange during the interim period. Results: Late re-infection occurred in 12.1% of all patients. In group A, 13.8% experienced late re-infection, with 14.3% in subgroup A1 and 13.3% in subgroup A2. In group B, 10% had a late re-infection. No significant difference in re-infection or complication rates was found between the groups. Conclusions: The study did not demonstrate the superiority of group B's approach of open biopsy over group A's joint aspiration, clinical examination, and blood counts in preventing re-infection or reducing complications.
