Abstract:Objective To investigate the underlying neural mechanism of left hemiparalexia and left hemialexia in reading Chinese characters. Methods A patient with reading disorders caused by brain infarctions at the left ventralis medialis occipitotemporal lobe and the splenium of the corpus callosum was studied. A series of neuropsychological tests, such as reading Chinese characters presented in the central foveal field or in the left and right half of the foveal field, were conducted with the patient, and neuroimaging techniques including high spatial resolution 3D-MRI and diffusion tensor tractography (DTT) were used to examine whether or not there were lesions of the neural pathway. Results The patient showed left hemiparalexia, which was characterized by making substitution or omission mistakes, mostly in the left parts of Chinese characters, and also left hemialexia(alexia for characters presented in left visual field). 3D-MRI demonstrated infarctions in the left ventral mesial occipitotemporal area and in the left side of the splenium of the corpus callosum. The left lateral mid-fusiform cortex, which has been identified as the visual word form area(VWFA), was almost intact. DTT indicated the major forceps fibers running through the splenium were all disconnected due to the infarction of the left splenium. Conclusion As a result of disruption of the splemium-major forceps pathway, visual character information in the left visual field which is initially projected to the right occipital cortex cannot be transferred from the right visual cortex to the left VWFA. This mechanism of left hemiparalexia and left hemialexia in reading Chinese characters is similar to that in reading English words.

Abstract:The objective of the study was to further elucidate the potential mechanisms underlying left hemiparalexia induced by a splenium lesion in corpus callosum. A patient KY, who had infarctions in the splenium and the left ventral medial occipitotemporal area, was examined with neuropsychological tests and fMRI. KY presented left hemiparalexia when he read aloud characters presented in central foveal field tachistoscopically as well as in free-view field. KY also showed left hemialexia for characters in left visual field, while no left hemiparalexia occurred when characters were presented in the right visual field. KY performed poorly in lexical decision tasks. He could judge the directions of Landolt’s rings gaps in the left or right visual field equally. The result of fMRI indicated that characters in the left visual field could not activate the visual word form area (VWFA), such as left mid-fusiform cortex. All the above neuropsychological and fMRI findings have provided evidences against the assumption of left hemineglect dyslexia. Instead, they support the mechanism of disconnection of visual word form processing pathway. In conclusion, the evidences suggested that the visual information transmission of characters in the left visual field from right occipital area to the VWFA in the left hemisphere was interrupted by the splenium lesion.
Representations of grapho-motor patterns unique to Chinese character writing: evidence from a patient with mirror writing

Abstract:This study investigated the grapho-motor patterns used in writing Chinese characters. A Chinese patient, CSC, who demonstrated post-brain-injury mirror writing, was recruited. In Experiment 1, non-mirrored writing responses were obtained when CSC was instructed to copy asymmetrical non-verbal symbols and pictures. Resembling the patterns observed in a patient's writing reported in a previous study, it was hypothesized that CSC's mirror writing was a result of untransformed preserved grapho-motor patterns. In Experiments 2 and 3, CSC was further instructed to copy real Chinese characters, pseudo-characters with authentic radicals and logographemes (i.e., stroke clusters that frequently occur in radicals), and Hangul characters with stroke clusters resembling the shapes of authentic logographemes. The results showed that CSC demonstrated mirror writing only when authentic Chinese orthographic units were involved. Non-mirrored writing responses were obtained from stimuli without authentic Chinese orthographic units. In sum, CSC's performance supported the existence of grapho-motor patterns of Chinese orthographic units represented in the brain. Theoretical implications were discussed.

Repetition suppression for mirror images of objects and not Braille letters in the ventral visual stream of congenitally blind individuals

Abstract:Mirror-invariance effect describes the cognitive tendency to perceive mirror-image objects as identical. Mirrored letters, however, are distinct orthographic units and must be identified as different. Mirror-invariance must be broken to enable efficient reading. Consistent with this phenomenon, a small, localized region in the ventral visual stream, the Visual Word Form Area (VWFA), exhibits repetition suppression to identical and mirror pairs of objects but only to identical pairs of letters. The ability of congenitally blind individuals to break mirror invariance for pairs of mirrored Braille letters has been demonstrated behaviorally. However, its neural underpinnings have not yet been investigated. Here, in an fMRI repetition suppression paradigm, congenially blind individuals (both sexes) recognized pairs of everyday objects and Braille letters in identical (p & p), mirror (p & q), and different (p & z) orientations. We found repetition suppression for identical and mirror pairs of everyday objects in the parietal and ventral-lateral occipital cortex, indicating that mirror-invariant object recognition engages the ventral visual stream in tactile modality as well. However, repetition suppression for identical but not mirrored pairs of Braille letters was found in the left parietal cortex and the lateral occipital cortex but not in the VWFA. These results suggest notable differences in reading-related orthographic processes between sighted and blind individuals, with the LOC region in the latter being a potential hub for letter-shape processing.

Abstract:In this study, we show that separate subregions of the visual word form area (VWFA‐1/VWFA‐2) have distinct patterns of resting‐state functional connectivity. VWFA‐2 shows strong connections with frontal text‐selective regions, suggesting a unique role in the language network. These findings shed light on the functional architecture of visual cortex. Reading entails transforming visual symbols to sound and meaning. This process depends on specialized circuitry in the visual cortex, the visual word form area (VWFA). Recent findings suggest that this text‐selective cortex comprises at least two distinct subregions: the more posterior VWFA‐1 is sensitive to visual features, while the more anterior VWFA‐2 processes higher level language information. Here, we explore whether these two subregions also exhibit different patterns of functional connectivity. To this end, we capitalize on two complementary datasets: Using the Natural Scenes Dataset (NSD), we identify text‐selective responses in high‐quality 7T adult data (N = 8), and investigate functional connectivity patterns of VWFA‐1 and VWFA‐2 at the individual level. We then turn to the Healthy Brain Network (HBN) database to assess whether these patterns replicate in a large developmental sample (N = 224; age 6–20 years), and whether they relate to reading development. In both datasets, we find that VWFA‐1 is primarily correlated with bilateral visual regions. In contrast, VWFA‐2 is more strongly correlated with language regions in the frontal and lateral parietal lobes, particularly the bilateral inferior frontal gyrus. Critically, these patterns do not generalize to adjacent face‐selective regions, suggesting a specific relationship between VWFA‐2 and the frontal language network. No correlations were observed between functional connectivity and reading ability. Together, our findings support the distinction between subregions of the VWFA, and suggest that functional connectivity patterns in the ventral temporal cortex are consistent over a wide range of reading skills.

Abstract:Introduction: Handwriting is a modality of language production whose cerebral substrates remain poorly known although the existence of specific regions is postulated. The description of brain damaged patients with agraphia and, more recently, several neuroimaging studies suggest the involvement of different brain regions. However, results vary with the methodological choices made and may not always discriminate between "writing-specific" and motor or linguistic processes shared with other abilities. Methods: We used the "Activation Likelihood Estimate" (ALE) meta-analytical method to identify the cerebral network of areas commonly activated during handwriting in 18 neuroimaging studies published in the literature. Included contrasts were also classified according to the control tasks used, whether non-specific motor/output-control or linguistic/input-control. These data were included in two secondary meta-analyses in order to reveal the functional role of the different areas of this network. Results: An extensive, mainly left-hemisphere network of 12 cortical and sub-cortical areas was obtained; three of which were considered as primarily writing-specific (left superior frontal sulcus/middle frontal gyrus area, left intraparietal sulcus/superior parietal area, right cerebellum) while others related rather to non-specific motor (primary motor and sensorimotor cortex, supplementary motor area, thalamus and putamen) or linguistic processes (ventral premotor cortex, posterior/inferior temporal cortex). Conclusions: This meta-analysis provides a description of the cerebral network of handwriting as revealed by various types of neuroimaging experiments and confirms the crucial involvement of the left frontal and superior parietal regions. These findings provide new insights into cognitive processes involved in handwriting and their cerebral substrates.

Abstract:Developmental dyslexia is characterized by persistent reading and spelling deficits. Partly due to technical challenges with investigating subcortical sensory structures, current research on dyslexia in humans by-and-large focuses on the cerebral cortex. These studies found that dyslexia is typically associated with functional and structural alterations of a distributed left-hemispheric cerebral cortex network. However, findings from animal models and post-mortem studies in humans suggest that developmental dyslexia might also be associated with structural alterations in subcortical sensory pathways. Whether these alterations also exist in developmental dyslexia in-vivo and how they relate to dyslexia symptoms is currently unknown. Here we used ultra-high resolution structural magnetic resonance imaging (MRI), diffusion MRI and probabilistic tractography to investigate the structural connections of the visual sensory pathway in dyslexia in-vivo. We discovered that individuals with developmental dyslexia have reduced structural connections in the direct pathway between the left visual thalamus (LGN) and left middle temporal area V5/MT, but not between the left LGN and left primary visual cortex (V1). In addition, left V5/MT-LGN connectivity strength correlated with rapid naming abilities - a key deficit in dyslexia [14]. These findings provide the first evidence of specific structural alterations in the connections between the sensory thalamus and cortex in developmental dyslexia. The results challenge current standard models and provide novel evidence for the importance of cortico-thalamic interactions in explaining dyslexia.

Preventing Children From Developing Dyslexia: A Premature Writing Hypothesis

Abstract:It has been argued that dyslexia may develop in strongly left eye dominant children through learning to write using ipsilateral, right hemisphere motor pathways. New light on this theory has been cast by recent findings of atypical enhanced corpus callosum white matter in children with dyslexia, reflecting right to left hemisphere communication that is resistant to intensive remedial reading intervention. Enhanced corpus callosum white matter is consistent with uninhibited right to left hemisphere ipsilateral mirror-motor innervation, manifested as frequent mirror-letter writing errors in children with dyslexia. Delaying writing instruction until 7–8 years of age may prevent these errors and as well as the development of dyslexia. During the 7–8-year age period, visual-proprioceptive integration enables a child to mentally map whole word visual images onto kinesthetic/proprioceptive letter engrams (memory representations). Hypothetically, this process is facilitated by anterior commissure activity involving interhemispheric transfer of ipsilateral mirror-to-non mirror-motor movement. This postulate, involving delayed writing instruction pending further maturation, also receives indirect support from the remarkable proficiency leap among second graders reading Hebrew as Hebrew involves a leftward orthography in which ipsilateral right to left hemisphere innervation is uninhibited. Additionally, and more directly, normal reading comprehension for learning English among children with agenesis of the corpus callosum suggests that letter-sound decoding is not the sole route to proficient reading comprehension. In this paper, I make recommendations for obtaining empirical evidence of premature writing as a cause of dyslexia.

Adaptation of feeding to a low fat yield in breast milk.

Abstract:The effect of a low milk fat yield was assessed in a blinded prospective study of healthy term infants and mothers encouraged to breast-feed. Fat yield index was calculated as milk volume collected by Egnell pump multiplied by the "creamatocrit." Two weeks after delivery mothers who had a relatively low fat yield index (less than or equal to 30th percentile) were matched with mothers with a higher fat yield index and with formula-feeding mothers. Between 2 and 6 weeks the low fat yield group had a marginally lower weight gain but similar growth in length and head circumference to that of the higher fat yield group. The low fat yield group spent more time per feeding and had more complete breast emptying, resulting in a fat yield index comparable with the higher fat yield group for the mean milk volume ingested by the infant at 6 weeks. A low fat yield had no adverse effect on maternal satisfaction or maternal-infant interaction during feeding. Formula feeders spent the least time in feeding and en face gazing. Adaptation of breast-feeding to a low fat yield sustains infant intake and growth, maternal-infant interaction, and maternal satisfaction. However, there is little reserve if milk production diminishes. Further study is needed to define the limits of adaptation and effective interventions if fat yield is inadequate.

Abstract:We present a case study detailing cognitive performance, functional neuroimaging, and effects of a hypothesis-driven treatment in a 10-year-old girl diagnosed with complete, isolated corpus callosum agenesis. Despite having average overall intellectual abilities, the girl exhibited profound surface dyslexia and dysgraphia. Spelling treatment significantly and persistently improved her spelling of trained irregular words, and this improvement generalized to reading accuracy and speed of trained words. Diffusion weighted imaging revealed strengthened intrahemispheric white matter connectivity of the left temporal cortex after treatment and identified interhemispheric connectivity between the occipital lobes, likely facilitated by a pathway crossing the midline via the posterior commissure. This case underlines the corpus callosum's critical role in lexical reading and writing. It demonstrates that spelling treatment may enhance interhemisphericconnectivity in corpus callosum agenesis through alternative pathways, boosting the development of a more efficient functional organization of the visual word form area within the left temporo-occipital cortex.

Abstract:Abstract Objectives We have previously demonstrated difficulties in written production in dementia with Lewy bodies (DLB) patients. We now aim to determine the neural correlates of writing production in DLB, combining clinical data and structural MRI measures. Method Sixteen prodromal to mild DLB patients were selected to participate in the study. The GREMOTS test was used to assess writing production. Using three‐dimensional T1 brain MRI images, correlations between the GREMOTS test and grey matter (GM) volume were performed using voxel‐based morphometry (VBM; SPM12, XjView and Matlab R2021b softwares). Results VBM analysis ( p < 0.001, uncorrected) revealed a positive and significant correlation between both left anterior insula and left supramarginal gyrus GM volumes and DLB patients' ability to write logatoms using the phonological route. The handwriting deficit was negatively and significantly correlated to the supplementary motor area. The parkinsonism‐like characteristics of agraphia were negatively and significantly correlated with both right anterior and right posterior cerebellum GM volumes. Our study also revealed a negative and significant correlation between grammatical spelling impairments and an area of the orbitofrontal gyrus, and a negative and significant correlation between supramarginal gyrus and general slowness in dictation tasks. Conclusion Writing disorders in early DLB patients appears to be GM decreases in several brain regions, such as the left anterior insula, the left supramaginal gyrus, as well as two areas of the right cerebellum.

Abstract:Learning to read is thought to involve the recruitment of left hemisphere ventral occipitotemporal cortex (OTC) by a process of "neuronal recycling", whereby object processing mechanisms are co-opted for reading. Under the same theoretical framework, it has been proposed that the visual word form area (VWFA) within OTC processes orthographic stimuli independent of culture and writing systems, suggesting that it is universally involved in written language. However, this "script invariance" has yet to be demonstrated in monolingual readers of two different writing systems studied under the same experimental conditions. Here, using functional magnetic resonance imaging (fMRI), we examined activity in response to English Words and Chinese Characters in 1st graders in the United States and China, respectively. We examined each group separately and found the readers of English as well as the readers of Chinese to activate the left ventral OTC for their respective native writing systems (using both a whole-brain and a bilateral OTC-restricted analysis). Critically, a conjunction analysis of the two groups revealed significant overlap between them for native writing system processing, located in the VWFA and therefore supporting the hypothesis of script invariance. In the second part of the study, we further examined the left OTC region responsive to each group's native writing system and found that it responded equally to Object stimuli (line drawings) in the Chinese-reading children. In English-reading children, the OTC responded much more to Objects than to English Words. Together, these results support the script invariant role of the VWFA and also support the idea that the areas recruited for character or word processing are rooted in object processing mechanisms of the left OTC.
Reserpine inhibition of coronary arteriosclerosis.

Abstract:A histological study of rabbit hearts from animals on a high-cholesterol diet (2% by weight) supplemented with vasopressin administration (2–4 units, three times a week) showed that after 7 weeks this regimen caused coronary arteriosclerotic lesions in three out of five animals. At the end of 10 weeks, 95% of the coronary arteries observed in five out of five animals had signs of arteriosclerosis. When, in addition to this treatment, the rabbits received 0.1 mg/kg reserpine per day, the incidence and severity of the lesions were greatly reduced. At 7 weeks only 10%, and at 10 weeks only 26% of the coronary arteries observed showed arteriosclerotic changes. These data indicate that reserpine has a protective action against atheromatous lesions which may be associated with its calcium-depleting effect.

Abstract:Objectives: The aim of this study was to identify the neural pattern activation during mirror therapy (MT) and explore any cortical reorganization and reducing asymmetry of hemispheric activity for upper limb rehabilitation in post-stroke patients. Methods: A box containing a mirror was placed between the arms of the patients to create the illusion of normal motion in the affected limb by reflecting the image of the unaffected limb in motion. We measured the cerebral hemodynamic response using near-infrared spectroscopy (NIRS). We enrolled ten right-handed stroke patients. They observed healthy hand movements in the mirror (MT condition) while performing various tasks (MT condition), and then repeated the same tasks with the mirror covered (N-MT condition). Results: Significant activation of some brain areas was observed in the right and left hemiparesis groups for the MT condition, while lower levels of activation were observed for the N-MT condition. The results showed significant differences in hemodynamic response based on oxygenated (HbO) concentrations between MT and N-MT conditions across all tasks in sensorimotor areas. These neural circuits were activated despite the motor areas being affected by the brain injury, indicating that the reflection of movement in the mirror helped to activate them. Conclusions: These results suggest that MT promotes cortical activations of sensory motor areas in affected and non-affected brain sides in subacute post-stroke patients, and it encourages the use of these tools in clinical practice.

Abstract:Brain imaging work aimed at increased classification of dyslexia has underscored an important relationship between anterior (i.e., the inferior frontal gyrus; IFG) and posterior (i.e., superior temporal gyrus and supramarginal gyrus) brain regions. The extent to which the three components of the inferior frontal gyrus, namely the pars orbitalis, triangularis, and opercularis, are differentially related to the posterior regions, namely the superior temporal gyrus and supramarginal gyrus, needs further elucidation. Information about the nature of the anterior–posterior connections would facilitate our understanding of the neural underpinnings associated with dyslexia. Adult participants (N = 38; 16 with dyslexia) took part in an MRI study, whereby high‐resolution structural scans were obtained. Volumetric asymmetry of the three regions of the IFG, the superior temporal gyrus, and the supramarginal gyrus was extracted. Significant differences were found for each of the three IFG regions, such that skilled readers had a greater leftward asymmetry of the orbitalis and triangularis, and greater rightward asymmetry of the opercularis, when compared to individuals with dyslexia. Furthermore, the pars triangularis was significantly associated with leftward asymmetry of the superior temporal gyrus for skilled but not dyslexic participants. For individuals with dyslexia, the cortical asymmetry of the IFG, and the corresponding connections with other reading‐related brain regions, is inherently different from skilled readers. We discuss our findings in the context of the print‐to‐speech framework to further our understanding of the neural underpinnings associated with dyslexia.

Abstract:Abstract It has been argued that university students with dyslexia compensate for their reading deficits by a neural re-organization of the typical reading network, where the lexical representations of words are (re-)structured according to semantic rather than orthographic information. To investigate the re-organization of neural word representations more directly, we used multivariate representational similarity analyses (RSA) to find out which brain regions of the reading network respond to orthographic and semantic similarity between 544 pairs of words and whether there were any differences between typical and dyslexic readers. In accordance with the re-organization hypothesis, we predicted greater similarity (i.e., correlation of neural dissimilarity matrices) in adult dyslexic than in typical readers in regions associated with semantic processing and weaker similarity in regions associated with orthographic processing. Our results did not confirm these predictions. First, we found sensitivity to semantic similarity in all three subparts of the fusiform gyrus (FG1, FG2, and FG3) bilaterally. Adults with dyslexia showed less (rather than more) sensitivity to semantic similarity in the posterior subpart of fusiform gyrus (FG1) in the left hemisphere. Second, in typical readers, sensitivity to orthographic information was not only found in the left fusiform gyrus (FG1, FG2, and FG3) but also in left inferior frontal gyrus (IFG). Adults with dyslexia, in contrast, did not show sensitivity to orthographic information in left IFG. However, they showed increased sensitivity to orthographic information in the right hemisphere FG1. Together, the results show abnormal orthographic processing in left IFG and right FG1 and reduced semantic information in left FG1. While we found evidence for compensatory re-organization in adult dyslexia, the present results do not support the hypothesis according to which adults with dyslexia rely more heavily on semantic information. Instead, they revealed atypical hemispheric organization of the reading network that is not restricted to the typical left language hemisphere.
