Abstract:Superficial fungal diseases of the skin and nails are an increasingly common occurrence globally, requiring effective topical treatment to avoid systemic adverse effects. Polymeric nanoparticles have demonstrated sustained and effective drug delivery in a variety of topical formulations. The aim of this project was to develop polymeric antifungal nanospheres containing terbinafine hydrochloride (TBH) to be loaded into a hydrogel formulation for topical nail drug delivery. A quality by design (QbD) approach was used to achieve optimized particles with the desired quality target product profile (QTPP). Polyvinyl alcohol (PVA) at 2% w/v and a drug to polymer ratio of 1:4, together with a robust set of processes and material attributes, resulted in nanoparticles of 108.7 nm with a polydispersity index (PDI) of 0.63, 57.43% recovery, and other desirable characteristics such as zeta potential (ZP), particle shape, aggregation, etc. The nanospheres were incorporated into a carbomer-based gel, and the delivery of TBH through this formulation was evaluated by means of in vitro drug release testing (IVRT) and ex vivo nail permeation study. The gel containing the TBH nanospheres demonstrated a slower and controlled drug release profile compared with the control gel, in addition to a more efficient delivery into the nail. These antifungal nanospheres can be utilized for topical therapy of a multitude of superficial fungal infections.

Abstract:The topical formulation of antifungal agents has demonstrated prominent success in treating fungal infection. However, traditional topical cream or solution limited ability to penetrate the nail plate and target the infected area contributes to the inefficacy of antifungals in the treatment of onychomycosis. In this work, the problem of topical administration reaching only the skin's surface was addressed by dissolving microneedles (MN) to pierce the skin and deliver drugs directly to the site of infection. Terbinafine (TH) was encapsulated into polydopamine (PDA) nanoparticles (TH@PDA) to increase the antifungal efficacy of TH. The results of checkerboard microdilution assays showed that the antifungal efficacy of TH@PDA was increased up to 2–8 times in comparison to free TH. In animal experiment, TH@PDA was incorporated into MN to target the infected site and increase the therapeutic effect. Our results demonstrated TH@PDA MN significantly decreased the fungal burden in the nails and relieved inflammation at the site of infection when compared to TH MN, TH cream, and oral TH. These results suggested that the TH@PDA MN exhibited enhanced antifungal effects and can serve as a potentially promising approach for onychomycosis treatment. Abstract:Topical therapy of superficial fungal infections allows the prevention of systemic side effects and provides drug targeting at the site of disease. However, an appropriate drug concentration in these sites should be provided to ensure the efficacy of such local treatment. The enhancement of intra- and transdermal penetration and accumulation of antifungal drugs is an important aspect here. The present overview is focused on novel nano-based formulations served to improve antimycotic penetration through the skin. Furthermore, it summarizes various approaches towards the stimulation of drug penetration through and into the stratum corneum and hair follicles, which are considered to be promising for the future improvement of superficial antifungal therapy as providing the drug localization and prolonged storage property at the targeted area.

Lecithins-Zein nanoparticles for antifungal treatment: Enhancement and prolongation of drug retention in skin with reduced toxicity

Abstract:Fungal infections are one of the major skin healthcare issues and cause significant morbidity. Ketoconazole (KC) as a broad-spectrum antifungal drug is widely used to treat skin fungal diseases. However, its therapeutic effects are limited by low concentration, short duration of drug efficacy in the skin and severe systemic toxicity. Here, the ketoconazole loaded Lecithins-Zein nanoparticles (KLZ-NPs) with core-shell structure were designed to resolve above problems. In vitro penetration test confirmed that the ketoconazole concentration of the KLZ-NPs group in the stratum corneum and deeper layers increased significantly (2.98-fold, 1.51-fold higher to free ketoconazole, respectively). Meanwhile, follicular closing technique showed the formed nanoparticles via follicle pathway into the skin had been significantly enhanced, and the results of the visual fluorescent images also confirmed it. Additionally, in the in vivo imaging experiment, the fluorescence intensity of the signal applying of the DiR-LZ-NPs was higher than that of the thrice usage of the free DiR. More importantly, the results also indicated that the accumulation of nanoparticles in the liver and spleen was significantly reduced. Hence, Lecithins-Zein nanoparticles are a promising strategy to enhance the drug concentration, prolong efficacy and reduce systemic toxicity in the topical administration for antifungal treatment.

[Expression and purification of telomerase resever transcriptase motifs in E.coli].

Abstract:Our project is designed to clone a 1.3kb gene fragment of telomerase catalytic subunit gene which contains seven reverse transcriptase motifs and specific region with conserved sequence termed "T motif". The gene fragment was amplified by PCR and was inserted into expression vector pET28-b. The recombinant plasmid was induced by IPTG for 4h and a 52KD recombinant protein was produced. Amount of hTRT recombinant protein expression was 20% of total bacterial protein in the form of inclusion. Inclusion was dissolved in 8 mol/L urea and 10 mmol/L DTT and carried out affinity purification under denaturing condition. The purified hTRT recombinant protein was conformed by Western-blot successfully.
In Silico Drug Screening Based Development of Novel Formulations for Onychomycosis Management

Abstract:Onychomycosis is a prominent fungal infection that causes discoloration, thickening, and mutilation leading to the separation of the nail from the nail bed. Treatment modalities for onychomycosis may include oral, topical, or combination therapy with antifungals and at times may require chemical or surgical intervention. The burden of side effects of antifungals is enormous, and therefore using molecular docking-based drug selection in context with the target keratin protein would ensure better disease management. Ciclopirox, Amorolfine HCl, Efinaconazole, Tioconazole, and Tavaborole were submitted for assessment, revealing that Amorolfine HCl is the best fit. Consequently, two formulations (Nail lacquer and nanoemulgel) were developed from Amorolfine HCl to validate the in silico screening outcomes. The formulations were further fortified with over-the-counter ingredients vis-a-vis with vitamin E in nail lacquer and undecylenic acid in nanoemulgel for their prominent roles in improving nail health. Both the formulations were systematically designed, optimized, and characterized. Amorolfine HCl containing nanoemulgel (NEG) was developed using undecylenic acid as an oil phase and thioglycolic acid as a penetration enhancer. The quality parameters evaluated were particle size, the zeta potential for nanoemulsion (NE) (78.04 ± 4.724 nm and −0.7mV, respectively), in vitro cumulative drug release (96.74% for NE and 88.54% for NEG), and transungual permeation (about 73.49% for NEG and 54.81% for NE). Nail lacquer was evaluated for the drying time, non-volatile content, and blush test. In vitro cumulative drug release of the developed nail lacquer and comparator marketed formulations were around 81.5% and 75%, respectively. Similarly, the transungual drug permeation was 6.32 μg/cm2 and 5.89 μg/cm2, respectively, in 24 h. The in silico guided preparation of both formulations containing Amorolfine HCl and over the counter ingredients is amenable for therapeutic use against onychomycosis and will be evaluated in the in vivo model.

Abstract:Psoriasis is a common and highly relapsing skin disease, for which topical treatments are used by approximately 88% of people with psoriasis as their primary therapy. However, in practice, the low convenience and side effects such as skin irritation of current topical treatments limit the application of the therapy. To address these issues, we calcined Phellodendri Chinensis Cortex (PCC) to prepare Phellodendri Chinensis Cortex Nanoparticles (PCC-NPs) which were packed into dissolvable microneedles (MNs) for the treatment of psoriasis. In this study, we revealed that the trace amounts of PCCNPs delivered by MNs could exert therapeutic effects therapeutic effects in the affected skin comparable to those of standard drugs, accompanied with the suppressed psoriasis-like inflammation without significant hepatic or renal toxicity or allergic reactions. These results indicate that dissolvable PCC-NPs MNs may serve as an innovative topical therapy for the inhibition of psoriatic inflammation.

Abstract:A lipase-triggered drug release nanoplatform (PGL-DPP-FLU NPs) for multi-modal antifungal therapy is developed. The lipases secreted by C. albicans can accelerate FLU release. The ROS and heat produced by PGL-DPP-FLU NPs make C. albicans more vulnerable to FLU, thereby PGL-DPP-FLU NPs exhibit high performance for combating azole-resistant C. albicans biofilms and wound infection.
Nanomaterial-based therapeutics for enhanced antifungal therapy

Abstract:The application of nanotechnology in antifungal therapy is gaining increasing attention. Current antifungal drugs have significant limitations, such as severe side effects, low bioavailability, and the rapid development of resistance. Nanotechnology offers an innovative solution to address these issues. This review discusses three key strategies of nanotechnology to enhance antifungal efficacy. Firstly, nanomaterials can enhance their interaction with fungal cells via ingenious surface tailoring of nanomaterials. Effective adhesion of nanoparticles to fungal cells can be achieved by electrostatic interaction or specific targeting to the fungal cell wall and cell membrane. Secondly, stimuli-responsive nanomaterials are developed to realize smart release of drugs in the specific microenvironment of pathological tissues, such as the fungal biofilm microenvironment and inflammatory microenvironment. Thirdly, nanomaterials can be designed to cross different physiological barriers, effectively addressing challenges posed by skin, corneal, and blood-brain barriers. Additionally, some new nanomaterial-based strategies in treating fungal infections are discussed, including the development of fungal vaccines, modulation of macrophage activity, phage therapy, the application of high-throughput screening in drug discovery, and so on. Despite the challenges faced in applying nanotechnology to antifungal therapy, its significant potential and innovation open new possibilities for future clinical antifungal applications.
Why Is Urological Laparoscopy Minimally Invasive?

Abstract:Objectives: Laparoscopic procedures have been developed and established with the view that a similar operative effect can be achieved with less traumatization, especially as far as systemic stress response is concerned. We report a prospective, controlled, nonrandomized animal and patient study to determine the systemic response to laparoscopic and open surgical procedures. Methods: In the animal study, 26 female pigs underwent either a laparoscopic bilateral varix ligation followed by bilateral nephrectomy (group I), sole introduction of trocars (group II) or sole establishment of an open surgical approach (group III). In the patient study, 145 patients underwent various laparoscopic procedures (nephrectomy, renal cyst marsupialization, varix ligation), open surgical procedures (nephrectomy, inguinal orchiectomy) or extracorporeal shockwave lithotripsy (ESWL). The serum parameters interleukin (IL)–6, IL–10 and C–reactive protein (CRP) were measured before, during and after the operative procedure. Results: In animals and patients, laparoscopy resulted in significantly lower serum levels of CRP during and after the operative procedure. Animals in group I showed a 5–fold elevation, in group II a 3–fold elevation and in group III a 9–fold elevation of CRP. In patients, the increase of CRP was twice as high after open unilateral nephrectomy than after laparoscopic unilateral or bilateral nephrectomy. IL–6 showed less marked elevation during laparoscopy, ESWL and minor operative procedures like laparoscopic varix ligation or inguinal orchiectomy when compared to an open unilateral nephrectomy. The parameter IL–10 showed no significant differences among the patient groups. Conclusions: The extent of the acute phase reaction to the operative trauma correlates much more convincingly to the approach than to the extent of the procedure. Only larger operations like nephrectomy trigger a systemic acute phase reaction, which can be limited by the laparoscopic access. For minor operative procedures like varix ligation or exploration of cryptorchidism, laparoscopy offers technical advantages rather than minimal invasiveness.
Thermosensitive Tri-Block Polymer Nanoparticle-Hydrogel Composites as Payloads of Natamycin for Antifungal Therapy Against Fusarium Solani

Abstract:Purpose: Fusarium Solani is the principal pathogen associated with fungal keratitis. As a sensitive drug to F. Solani, natamycin (NAT) was limited by the poor penetration and low bioavailability in clinical application. The aim of this study was to develop a new type of tri-block polymer nanoparticle-gel complex (Gel@PLGA-PEI-PEG@NAT) for delivering NAT and evaluate its physicochemical properties, antifungal activity, safety, penetrability, adhesion, and efficacy in treating fungal keratitis.Methods: PLGA-PEI-PEG@NAT was prepared and characterized with a nano-particle size analyzer, transmission electron microscopy (TEM), scanning electron microscopy (SEM), and Fourier transform infrared spectroscopy (FTIR). The minimum inhibitory concentration (MIC), cytotoxicity, penetrability of NAT (Natacyn® 5% ophthalmic suspension; Alcon) and PLGA-PEI-PEG@NAT with different concentrations were assessed. The eye surface retention time, ocular irritation, and curative effect of the NAT ophthalmic suspension and Gel@PLGA-PEI-PEG@NAT on a rabbit fungal keratitis model were evaluated.Results: PLGA-PEI-PEG@NAT had a particle size of 150 nm, a positive surface charge, and a sustained-release effect. The MIC for F. Solani was 2 μg/mL. A cytotoxicity test and ocular irritation test showed that PLGA-PEI-PEG@NAT and Gel@PLGA-PEI-PEG@NAT had good biocompatibility and no obvious irritation for rabbit corneas. Penetration experiments confirmed that PLGA-PEI-PEG@NAT can successfully enter corneal epithelial cells and through the cornea to enter the anterior chamber. Compared with NAT ophthalmic suspension, Gel@PLGA-PEI-PEG@NAT had stronger cornea permeation at the same concentration. The therapeutic effect and precorneal retention ability of the NAT ophthalmic suspension and Gel@PLGA-PEI-PEG@NAT on the fungal keratitis rabbit model were compared. Gel@PLGA-PEI-PEG@NAT achieved a better therapeutic effect at a lower drug concentration, and its eye surface retention time was significantly longer than that of the NAT ophthalmic suspension.Conclusion: Gel@PLGA-PEI-PEG@NAT was shown to be a safe and effective nanodrug delivery system for NAT. It has great potential to improve the cure rate of fungal keratitis, reduce the administration frequency during the treatment process, and improve patient compliance.

Abstract:A study was undertaken to determine the ability to characterize benign and malignant masses with unenhanced and contrast material‐enhanced fast lowangle shot and fat‐suppressed spin‐echo magnetic resonance (MR) imaging. Thirty patients with adrenal masses detected at computed tomography (CT) underwent MR imaging within 14 days after CT. CT and MR images were interpreted in a prospective, blinded fashion. Sixteen patients had 20 benign adrenal masses, and 14 patients had 18 malignant masses. Quantitative measurements included percentage of contrast enhancement on immediate postcontrast dynamic images and periphery ‐ center signal‐to‐noise ratio (S/N) on gadolinium‐enhanced fat‐suppressed images. Qualitative evaluation included determination of the regularity of lesion margins, homogeneity of signal intensity, and local extension. MR imaging depicted all adrenal masses discovered at CT examinations. Lesions ranged in diameter from 1 to 15 (mean, 4.4) cm. No significant difference was observed in percentage of contrast enhancement between benign (90.5% ± 59.0 [standard deviation]) and malignant (110.5% ± 116.4) masses. A difference was observed between periphery ‐ center S/N for benign (−.05 ± 1.5) and malignant (7.7 ± 9.8) masses; overlap between the two, however, occurred. Qualitative evaluation allowed correct characterization of 32 of 38 masses, comparing favorably with CT, which allowed characterization of 30 lesions.

Abstract:Even though fungal keratitis is a severe corneal disease that can lead to permanent vision loss, its therapeutic management is still problematic. This limitation is in part attributable to the fact that tear film renewal curtails exposure time of drug containing eye drops to the ocular surface. Accordingly, anti-microbial therapy needs to be performed repeatedly. Efforts to resolve this hindrance include developing formulations that prolong drug retention on the ocular surface which can in turn improve therapeutic efficacy as well as reduce administration frequency. To extend drug residence time on the ocular surface, we designed a novel organic-inorganic hybrid consisting of a PEG-PPG copolymer modified with polyhedral oligomeric silsesquioxane (POSS) groups. Its tailored POSS-micelles possess both good biocompatibility and bio-adhesiveness, which improve ocular delivery of poorly soluble drugs. We describe here use of mouse fungal keratitis model to determine the effects of this formulation encapsulating amphotericin B (AMB) on this condition. The results show that POSS-micelles significantly prolonged AMB retention time on the ocular surface, leading to improved resolution of this disease. Moreover, this treatment strategy had no toxic effects during ocular administration either in vitro or in vivo. Therefore, this POSS formulation bearing AMB has the potential to prolong drug retention time and improve its therapeutic efficacy in managing fungal keratitis.

Abstract:Fungal keratitis is challenging to diagnose and treat and remains a significant cause of blindness worldwide. The easiest and most common method of drug delivery for patients with fungal keratitis is eye drop administration. However, the therapeutic effect of traditional eye drops is unsatisfactory, largely due to the intrinsic nature of the ocular barriers, which limit drug absorption; the rapid decrease in the drug concentration caused by tears; and the side effects induced by the uncontrolled release of ocular drugs. Oxidative stress and inflammation are the main causes of corneal tissue necrosis in fungal keratitis, and reducing reactive oxygen species (ROS) and the inflammatory response are important goals in developing drugs for fungal keratitis. In the current study, we developed a ROS-responsive and controllable nanocarrier (GC-EB) that efficiently delivered a clinically used antifungal drug, voriconazole (VOR), to treat fungal keratitis. In vitro and in vivo results demonstrated that the developed GC-EB-VOR exhibited high penetration through corneal barriers, good retention in the cornea and controllable drug release under low concentrations of ROS. As a result, ROS were effectively depleted and the inflammatory response was inhibited; thus, GC-EB-VOR shows promising antifungal efficacy. This work may provide a new strategy for developing nanodrugs to improve the therapeutic effect of eye drop instillation on fungal keratitis and reduce the risk of blindness.

Abstract:Fungi infection is a serious threat to public health, but an effective antifungal strategy remains a challenge. Herein, a biomimetic nanocomposite with multifunctionalities, including fungi diagnosis, antifungal adhesion, precise fungi elimination, and cytokine sequestration, is constructed for battling Candida albicans ( C. albicans ) infection. By screening a range of cells, we find that the polarized macrophage cells have the strongest binding tendency toward C. albicans . Thus, their membranes were exfoliated to camouflage UCNPs and then decorated with photosensitizers (methylene blue, MB) and DNA sensing elements. The resulting nanocomposite can tightly bind to fungal surfaces, promote DNA recognition, and squeeze pro-inflammatory cytokines to relieve inflammation. Consequently, this nanocomposite can detect C. albicans with enhanced sensitivity and precisely eliminate fungal cells through photodynamic therapy with minimal phototoxicity because of its switchable fluorescence behavior. The developed nanocomposite with good biocompatibility achieves a satisfactory diagnostic and therapeutic effect in a C. albicans -infected mouse model, which offers a unique approach to fight fungi infection.

Abstract:Fungal keratitis is a globally blinding eye disease caused by fungi, with more than 1.05 million cases diagnosed annually, mainly in Asia. It is the most common form of infectious keratitis, accounting for approximately 40–50% of microbial keratitis cases. The disease is difficult to treat and the prognosis is often poor. Conventional antimicrobial therapies, which are the mainstay of treatment, face problems of off-target toxicity, low bioavailability, and drug-resistant fungi. In recent years, novel drug delivery systems have emerged as a promising alternative. These systems improve drug stability, extend drug residence time, control drug release, target specific tissues and cells, and reduce toxic side effects. Nanoparticle-based drug delivery systems are particularly compelling. The aim of this paper is to elucidate the mechanisms and advances of nanoparticle-based drug delivery systems in antifungal therapy and to provide new perspectives on the treatment of fungal keratitis.

Preparation of Topical Itraconazole with Enhanced Skin/Nail Permeability and In Vivo Antifungal Efficacy against Superficial Mycosis

Abstract:In this study, a stable and highly skin-permeable topical delivery system for itraconazole (ITZ) was designed to provide effective treatment against superficial mycosis. Herein, ITZ was incorporated into a solution composed of ethanol, benzyl alcohol, hydrochloric acid, Transcutol P, and cyclomethicone as a delivery vehicle, solubilizer, protonating agent, permeation enhancer, and spreading agent, respectively. At 72 h, the optimal topical ITZ formulation (ITZ–TF#11) exhibited 135% enhanced skin permeability, which led to increases in drug deposition in the stratum corneum, epidermis, and dermis of 479%, 739%, and 2024%, respectively, compared with the deposition of 1% ITZ in ethanol (control). Moreover, on day 7, ITZ–TF#11 demonstrated 2.09- and 2.30-fold enhanced nail flux and drug deposition, compared with the control. At a dose of 40 mg/kg/day, ITZ–TF#11 showed 323% greater lesion recovery, a 165% lower mean erythema severity score, and a 37% lower mean logarithm of viable fungal cells in skin in the treated area, compared with mice that received oral ITZ at the same dose. Overall, the findings imply that ITZ–TF#11 is a superior alternative to oral ITZ for treatment of superficial mycosis.
