Abstract:Purpose To assess the influence of Botulinum neurotoxin A (BoNT-A) injection on meibomian gland function and dry eye in patients diagnosed with Blepharospasm (BPS) and Hemifacial spam (HFS). Methods Adult patients aged 18 years or older who suffer from periocular dystonia, and were treated with BoNT-A injections, were recruited in this interventional prospective study between 2023 and 2024. Each patient was followed up for a period of three months. The following parameters were compared at baseline, 14 and 90 days post BoNT-A injections: Visual acuity, meibography, tear break up time (TBUT), Schirmer test, meibum expression, tear meniscus height, fluorescein corneal staining, meibomian gland dysfunction (MGD) grading, and ocular surface disease index (OSDI). In addition, patients underwent subjective quality of life questionnaire for BPS and HFS. Results Thirty-six eyes of twenty-six patients were included in this study. The mean age ± SD was 59 ± 17.33 years. There was a significant statistical difference in vascular changes on the lid margin, TBUT, corneal and conjunctival fluorescein staining and meibum expression (P<0.001 in all). The differences were statistically significant when compared to baseline vs visit 2, and visit 2 vs visit 3 (p-value<0.05). Jankovic rating scale and HFS score improved significantly between the time periods (P<0.001 in both).There was no significant difference in meibomian gland loss, tear meniscus height, Schirmer test, and OSDI. Conclusion BoNT-A injection is an effective treatment for periocular dystonia, but it worsens ocular surface and dry eye disease. Moreover, it affects the meibomian glands by disturbing their secretion.

Correlation Between Blepharospasm and Psychological Diseases: the Anxiety, Depression and Sleep Disorder Study

Abstract:AIM: To investigate the relationship between benign essential blepharospasm (BEB) symptoms and depression/anxiety/sleep disorder in a prospective manner and to determine whether treatment the BEB with botulinum toxin type A (BoNT/A) can impact psychological symptoms. METHODS: This prospective interventional case series recruited 61 adults with evidence of BEB. Patients were administered the Jankovic Rating Scale (JRS), the Blepharospasm Disability Index (BSDI), Personal Health Questionnaire Depression Scale (PHQ-8), Generalized Anxiety Disorder 7-item scale (GAD-7) and the Athens insomnia scale (AIS) to evaluate the severity of BEB symptoms, depression, anxiety and sleep disorder before and 1wk, 1, 3mo after the BoNTA treatment. Statistical analysis was performed to assess the relationships between changes in the survey scores. RESULTS: The mean score for JRS, BSDI, PHQ-8, and GAD-7 improved significantly (P<0.0001), respectively, compared to the initial visit at follow-up. At baseline, worse BSDI scores were correlated with worse GAD-7 and PHQ-8, but not with worse AIS. At 1mo follow-up visit, there was no correlation between change in BSDI and PHQ-8/AIS, the change in GAD-7 showed a mild association with change in BSDI. The change in BSDI was correlated with the change in both PHQ-8 and GAD-7 in the subgroup of patients without a prior diagnosis of depression or anxiety. Patient satisfaction with BoNT/A treatment reached the highest at 1mo of follow-up (83.6%, 51/61). CONCLUSION: BEB may lead to psychological diseases. BoNT/A can significantly improve motor and non-motor symptoms of BEB, which emphasize the effectiveness of BoNT/A and therefore pave the way for its use in the field of psychiatry. However, further research is needed to confirm these findings and understand the underlying mechanisms.

Abstract:Intracranial foreign bodies due to nonmissile intracranial penetrations occur rarely. Most of these penetrating injuries result from industrial accidents or criminal assaults. The complications which cause mortality in early stage are intracerebral hemorrhage, contusion, major vascular injury and meningitis. In case of such injuries, foreign bodies near the major vascular structures should not be attempted to taken out. Total excision of the foreign body via craniotomy should be planned and possible dural and vascular injuries should be repaired during surgery. Urgent surgery should be performed as there is 53% morbidity in case of late surgery and 62% morbidity in nonoperated cases. We herein report a 20-year old man who attempted suicide by introducing a nail into his brain and review the related literature.
Exploration of Effect and Dosage of Botulinum Toxin Type A for Long-Term Treatment of Idiopathic Blepharospasm

Abstract:AIM: To explore the relationship between the efficacy and the dosage of botulinum toxin type A (BTXA) for long-term and repetitive treatment of blepharospasm. METHODS: 178 patients repetitively treated with BTXA were followed up for 7 a. The dosage, effect, effect duration, and adverse reations of 6 sessions were compared among themselves. The severity of blepharospasm was gauged by Cohen scale. SAS software package was administered for statistic analysis. RESULTS: 178,138,112,83,63, and 48 patients were enrolled in the consequently 6 sessions in 7 a. The withdraw rates were 14.0 %(25/178) (P0.05),5.1 %(7/138), 5.4 %(6/112),6 %(5/83) and 6 %(4/63) since the second session. The effective rate of each session was 89.9 %(160/178),97.1 %(134/138),96.4 %(108/112),95 %(79/83),95 %(60/63), and 98 %(47/48), respectively. The average BTXA dosage was about 70 U, and the mean duration of improvement was 15-16 wk. Though the lower effective rate in the first session compared with the second and third sessions, there were no significant differences among other sessions in the effective rate, mean dosage and duration of improvement. The incidence rates of adverse reactions were 36.5 %(65/178),31.9 %(44/138),31.3 %(35/112),31 %(26/83),30 %(19/63) and 29 %(14/48) respectively in each session, and there were no significant differences in statistic analysis. The adverse reactions of focal weakness occurred at 2-3 d after effect emerged and lasted 2-6 wk. The most concerned complications were ptosis, blurred vision and diplopia. The ptosis occurred 10.1 % in the first session, which had statistic difference (P0.05) compared with the second and third sessions (3.6 %-3.6 %). The adverse reaction rate was 30 % on the whole. CONCLUSION: Without increasing dose, the long-term therapeutic effect and duration of improvement can be maintained by repeated topical BTXA injection. The incidence of adverse reactions such as ptosis can be reduced.
Ocular Surface Alterations in Blepharospasm Patients Treated with Botulinum Toxin a Injection

Abstract:Purpose To evaluate ocular surface changes secondary to periocular botulinum toxin A injection in patients with essential blepharospasm. Methods Thirteen eyes of 13 patients with essential blepharospasm who underwent periocular botulinum toxin A injection were included in this prospective study. Patients were evaluated prior to and at 2-week and 1-, 3-, and 6-month time points following injections. Ocular surface tests were carried out in the order of tear break-up time (TBUT), lissamine green (LG) staining, Schirmer I test with anesthesia, and ocular surface disease index (OSDI) questionnaire for all patients. The Friedman test with Conover post hoc method was used for statistical comparisons of values at different time points. Results The TBUT was found to be increased at 1 month after the injection (8.5 ± 2.1; p = 0.018) and decreased below baseline levels (6.4 ± 2.1) at the 6-month visit (5.7 ± 2.0; p = 0.018). None of the Schirmer test values at follow-up visits were significantly different as compared to baseline levels (11.3 ± 5.5), although the 2-week measurement (14.3 ± 5.6) was significantly higher as compared to that at the 6-month follow-up visit (9.6 ± 4.9; p = 0.034). There was also a significant decrease in LG staining scores at 2-week (0.6 ± 0.4; p = 0.012) and 1-month (0.6 ± 0.4; p = 0.012) time points compared to the baseline levels (1.1 ± 0.6). The OSDI scores improved at 2-week (5.4 ± 6.8; p<0.001), 1-month (3.2 ± 5.1; p<0.001), 3-month (2.5 ± 4.4; p<0.001), and 6-month (5.5 ± 5.4; p<0.001) time points as compared to baseline levels (11.6 ± 8.5). Conclusions Botulinum toxin A injection appears to have a positive but temporary effect on ocular surface parameters in patients with blepharospasm.

Treatment Effects of Botulinum Toxin Injection and Acupuncture on Blepharospasm Assessed by the Change in Lower Eyelid Tension

Abstract:Objective: To investigate the treatment effects of botulinum toxin-A (BTA) injection and acupuncture on blepharospasm (BP) evaluated by the change in lower eyelid tension (LET). Methods: A series of 30 patients (male: 8, female: 22) aged between 37 and 83 years (63.80 ± 10.96 yrs) who met the eligibility criteria of BP were recruited in this study, who were randomly assigned to BTA injection group (BTA group, n = 15) and acupuncture treatment group (Acupuncture group, n = 15). BTA injections were administered to the patients in BTA group while patients in acupuncture group received the acupuncture treatment. The LET was measured by a tensiometer in both groups at baseline and at post-treatment. Results: A significant decrease in LETs over 8 weeks was found in acupuncture group (812.76 ± 193.95 Pa at baseline, 549.69 ± 150.04 Pa at 4 weeks, and 510.96 ± 150.66 Pa at 8weeks, respectively; F = 31.127, p << 0.001). There was a significant decrease in LET from 858.61 ± 190.54 Pa at baseline to 414.45 ± 63.38 Pa at 2 weeks after treatment (Z = -4.542, p << 0.01) in BTA group. At the endpoint of the study, a significant difference in LET was found between the acupuncture group (301.80 ± 181.77 Pa) and the BTA group (444.16 ± 193.44 Pa) (t = -2.077, p = 0.047). Conclusions: BP patients have an increased LET. Both BTA and acupuncture are effective in decreasing the LET. Close monitoring of LET holds promise in planning the treatment strategy for Blepharospasm.
Acute changes in blood flow in pigs infused with beta-adrenergic agonists.

Abstract:Previous results indicate that clenbuterol decreases carcass adipose tissue accretion when administered to pigs but does not appear to stimulate the adipose tissue beta-adrenergic receptor. Clenbuterol increases plasma free fatty acid concentration when acutely infused in vivo, suggesting an indirect affect. One possible indirect effect is that clenbuterol could change blood flow to adipose tissue. Blood flow was measured with radiolabeled microspheres in tissues from pigs before and after infusion of a beta-adrenergic agonist for 30 min. High probability levels (up to P less than .2) were used to indicate trends due to extreme variability. Infusion of isoproterenol increased heart rate, plasma free fatty acid concentration and blood flow at many adipose tissue sites and at a few skeletal muscle sites. Infusion of isoproterenol decreased blood pressure. Infusion of clenbuterol increased heart rate and tended to increase blood flow to several skin and adipose tissue sites slightly. The results suggest that increased adipose tissue blood flow may contribute to the accelerated release of free fatty acids when clenbuterol is infused acutely in vivo.
Impact of Botulinum Toxin Injections on Quality of Life and Self-Esteem in Patients with Blepharospasm

Abstract:To determine the effects of botulinum toxin type A (BoNT-A) injections on self-esteem and quality of life (QOL) in blepharospasm patients, we enrolled 87 patients into the study. Patients completed the WHOQOL-BREF questionnaire, the Self-Esteem Scale (SES), and the Ocular Surface Disease Index (OSDI) before treatment on the date of enrollment and again after 4 weeks of treatment with BoNT-A. The overall QOL, general health status, the physiological and psychological domain scores of the WHOQOL-BREF, and the SES score significantly improved after treatment. The motor symptoms improved, and the OSDI score increased after treatment. Overall QOL and SES scores were correlated with disease duration, while psychological domain scores were correlated with mean duration of therapeutic effect. All variables correlated with OSDI scores. In conclusion, BoNT-A injections improved QOL and self-esteem in blepharospasm patients.
Depressive symptoms and quality of life in patients with benign essential blepharospasm under long-term therapy with botulinum toxin

Abstract:Regular and long-term injections of botulinum toxin (BoNT) are considered the first line therapy for essential blepharospasm (BEB), but no data exists on the long-term effect of this therapy on depressive symptoms and quality of life. This study aims to prospectively evaluate the long-term effects of BoNT therapy on depressive symptoms as well as on daily activities, emotional well-being and quality of life using validated questionnaires (BEB-scale, Beck`s Depression Inventory (BDI)). 86 patients diagnosed with BEB were followed up for a median of 4 years. Clinical symptoms improved significantly after BoNT-injections. Everyday activities and subjective assessment of the overall situation improved gradually under long-term BoNT therapy. Significant correlations ( p < 0.0001; r-values between 0.498 and 0.706) were found between the BDI and items of the BEB-scale. No significant antidepressive effect of long-term BoNT therapy was found with a low median BDI total score (5/max. 63), but up to 31.3% of BEB patients had a BDI score ≥ 11, indicating clinically relevant depressive symptoms. Of these, 65.4% had no known history of depression. Although, several studies reported an antidepressant effect of botulinum toxin injections in patients with major depression, this effect does not seem to be present in patients with BEB despite clinical improvement of symptoms. A high prevalence of previously undetected depressive symptoms was found in BEB patients. As this may influence BoNT therapy success, identifying potential depressive symptoms at the time of BEB diagnosis and initiating appropriate treatment seems important.

Grey Matter Microstructural Integrity Alterations in Blepharospasm Are Partially Reversed by Botulinum Neurotoxin Therapy

Abstract:Objective: Benign Essential Blepharospasm (BEB) and hemifacial spasm (HFS) are the most common hyperkinetic movement disorders of facial muscles. Although similar in clinical presentation different pathophysiological mechanisms are assumed. Botulinum Neurotoxin (BoNT) is a standard evidence-based treatment for both conditions. In this study we aimed to assess grey matter microstructural differences between these two groups of patients and compared them with healthy controls. In patients we furthermore tracked the longitudinal morphometric changes associated with BoNT therapy. We hypothesized microstructural differences between the groups at the time point of maximum symptoms representation and distinct longitudinal grey matter dynamics with symptom improvement. Methods: Cross-sectional and longitudinal analyses of 3T 3D-T1 MRI images from BEB, HFS patients prior to and one month after BoNT therapy and from a group of age and sex matched healthy controls. Cortical thickness as extracted from Freesurfer was assessed as parameter of microstructural integrity. Results: BoNT therapy markedly improved motor symptoms in patients with BEB and HFS. Significant differences of grey matter integrity have been found between the two patients groups. The BEB group showed lower cortical thickness at baseline in the frontal-rostral, supramarginal and temporal regions compared to patients with HFS. In this group BoNT treatment was associated with a cortical thinning in the primary motor cortex and the pre-supplementary motor area (pre-SMA). Contrary patients with HFS showed no longitudinal CT changes. A decreased cortical thickness was attested bilaterally in the temporal poles and in the right superior frontal region in BEB patients in comparison to HC. Patients in the HFS group presented a decreased CT in the left lingual gyrus and temporal pole. Conclusions: Although patients with BEB and HFS present clinically with involuntary movements of facial muscles, they exhibited differences in cortical thickness. While BoNT therapy was equally effective in both groups, widespread changes of cortical morphology occurred only in BEB patients. We demonstrated specific disease- and therapy-dependent structural changes induced by BoNT in the studied hyperkinetic conditions.
Lid wiper epitheliopathy in patients with blepharospasm and/or hemifacial spasm

Abstract:Objective: To evaluate the presence of wiper epitheliopathy in patients with blepharospasm and/or hemifacial spasm before and 4 weeks after routine treatment with botulinum toxin. Methods: Prospective study comprising 31 eyes of 20 patients with neurological diagnosis of hemifacial spasm (9 eyes of 9 patients) and essential blepharospasm (22 eyes of 11 patients). Various ocular surface parameters were assessed before and 4 weeks after infiltration with botulinum toxin using the OSDI questionnaire, Schirmer's test, tear break-up time (BUT), fluorescein and lissamine green staining assessed with the Oxford test and the degree of involvement of the palpebral wiper. Results: 100% of the patients had palpebral wiper involvement before (30% mild and 70% moderate) and after toxin treatment (100% mild). 75% of patients had mild-normal OSDI before treatment, after treatment it was 80%. The BUT was 7.2 ± 0.2 sg before and 7.5 ± 0.7 sg after treatment. Schirmer's test was 11.4 ± 5.5 and 12.5 ± 5.5 mm before and after treatment. The Oxford test was initially pathological in 69.3% of patients, after 4 weeks it was pathological in only 54%. Conclusion: Wiper epitheliopathy is present in 100% of patients with blepharospasm and/or hemifacial spasm. The main pathophysiological mechanism that triggers it in these patients is the increase in the coefficient of friction, as tear volume and stability are normal.

Abstract:Objective To observe the efficacy of using Botox-A in the treatment of blepharospasm(BS) and hemifacial spasm(HFS).Methods A total of 150 patients with BS and HFS were involved in the present study.Fifty patients who had the disease for less than six months were divided into two groups:one group(25 cases) was treated with a local ice-bag application and the other group(25 cases) was used for the control.One hundred patients who had a history of more than 6 months were managed with a local injection of Botox-A.Results In the 25 patients treated with ice bags,symptoms completely disappeared in 23 patients and the remaining 2 patients presented with relieved spasm.Among the 25 cases used as the controls,the symptoms of 15 patients spontaneously disappeared 1 to 2 months later,7 patients presented with relieved spasm and the conditions of 3 patients remained unchanged.Ninety-eight of the 100 cases with BS and HFS whose history lasted more than 6 months had a complete or obvious remission of symptoms after receiving a local injection of Botox-A(BTA),with no effect seen after the treatment in the remaining 2 cases.No systemic toxic or side effects were found in any patients treated with a Botox-A injection.Conclusion Local ice-bag application contributes to the relief of BS and HFS in the early stages.A BTA local injection,which is easy to administer,is a safe,effective and reliable treatment for patients whose spasm has lasted for more than 6 months.
Pilot study of topical acetyl hexapeptide‐8 in the treatment for blepharospasm in patients receiving botulinum toxin therapy

Abstract:BACKGROUND AND PURPOSE: Injectable botulinum neurotoxin (BoNT) is the principal effective treatment for blepharospasm (BSP). This trial explores the safety and efficacy of topical acetyl hexapeptide-8 (AH8), a competitive SNAP25 inhibitor, as a potential new therapy in BSP.METHODS: Double-blind, placebo-controlled, randomized trial of daily topical application of AH8 in 24 patients with BSP. The primary outcome was time to return to baseline Jankovic Blepharospasm Rating Scale (JBRS) after a BoNT injection simultaneously with the initiation of AH8. Patients displaying a strictly regular pattern of response to 3-monthly injections of BoNT were included.RESULTS: There were no significant adverse events. There was a trend for longer time until return to baseline JBRS after injection in the active group compared to placebo (3.7 months vs. 3.0 months), and for better scores in the active group. One-third (4/12) of the patients in the active group had a considerable extension of symptom control after BoNT (range: 3.3-7.1 months).CONCLUSIONS: Topical AH8 is safe and promising for extending the duration of action of BoNT therapy for BSP.

Abstract:Objective To investigate the clinical effect of subcutaneous injecting the compound anisodine besides the superficial temporal artery combined with danhong iontophoresis in treatment of blepharospasm with dry eye.Methods The clinical data of 64 patients (75 eyes) with blepharospasm and dry eye treated in The Second People′s Hospital of Lianyungang were collected from September 2014 to June 2016.According to the digital table method, the patients were randomly divided into control group and treatment group.Among them, there were 30 cases (34 eyes) in the control group and 34 cases (41 eyes) in the treatment group.30 cases (34 eyes) enrolled in the control group were treated with oral mecobalamin capsule and sodium hyaluronate eye drops.On the basis of the control group′s treatments, 34 cases (41 eyes) enrolled in the treatment group were treated by injecting the compound anisodine besides the superficial temporal artery combined with Danhong iontophoresis.Before and after treatment,two groups of patients were examined in classification of blepharospasm, SchirmerⅠtest (SIt), breakup time (BUT), and corneal fluorescent staining, respectively.SIt wetted length, BUT, and corneal fluorescein staining scores were presented as mean ± standard deviation(±s), and compared by independent samples t-test between two groups, and compared by paired samples t-test before and after treatment.The intensity of blepharospasm was divided into four grades, and the results were compared by Kruskal-Wallis H rank sum test.The therapeutic effect of blepharospasm was expressed as the number of cases and percentage (%), and the total effective rate was compared by chi square test.Results Before the treatment, the results of SIt, BUT and corneal fluorescent staining in the control group were (2.43±1.17) mm/5 min, (2.67±1.03) s and (5.10±1.71) points;after treatment, they were (10.00±2.99) mm/5 min, (9.10±2.44) s and (1.67±1.15) points, respectively.The results of SIt, BUT and corneal fluorescent staining were compared in the control group before and after treatment, and the differences were statistically significant (t=9.12, 10.26, 5.48;P<0.05).Before the treatment, the results of SIt, BUT and corneal fluorescent staining in the treatment group were (2.15±1.02) mm/5 min, (2.59±0.86) s and (5.12±1.55) points;after treatment, they were (13.02±3.22) mm/5 min, (12.00±2.20) s and (1.09±0.90) points, respectively.The results of SIt, BUT and corneal fluorescent staining were compared in the treatment group before and after treatment, and the differences were statistically significant (t=9.10, 10.50, 4.67;P<0.05).Before the treatment, comparing the results of SIt, BUT and corneal fluorescent staining between the treatment group and control group, and there were no statistically significant differences (t=1.05, 0.33, 0.04;P>0.05).However, comparing the results of SIt, BUT and corneal fluorescent staining between the treatment group and control group, after the treatment, and the differences were statistically significant (t=3.88, 5.00, 2.25;P<0.05).Before the treatment, there was no significant difference in the intensity distribution of blepharospasm between the two groups before treatment (Hc=0.10, P>0.05).But after the treatment, there was statistical difference in the total effective rate which was 94.12% in the treatment group and 80.00% in the control group (χ2=6.94, P<0.05).Conclusion Subcutaneous injecting the compound anisodine besides the superficial temporal artery combined with Danhong iontophoresis is efficient in the treatment of blepharospasm and dry eye.
Long-term stable efficacy of botulinum toxin A in facial movement disorders with no need for increasing dose

Abstract:Botulinum toxin A is considered an effective treatment for involuntary facial movements. We examined whether treatment efficacy maintained or changed over time with two products, Botox and Dysport, in patients with hemifacial spasm, facial synkinesis and benign essential blepharospasm.We retrospectively investigated 87 consecutive patients (51 women, 36 men) who had undergone treatment for ≥6 years. Long-term effects, as well as side effects of Botox or Dysport local injections were evaluated. The first three treatments were considered the titration period and not taken into account when testing for dose changes.Mean treatment duration was 10 years (range 6-11, SD 1.0), 2441 treatments were administered, 1162 with Botox and 1279 with Dysport, the two brands were interchanged as needed. Good to full improvement was seen in 90% of patients both with both brands. Injection doses and treatment responses were consistent during the study with both drugs. No major side effects were reported, and relatively few minor adverse events were reported, with clear reduction from the titration period (6.1%), to the remainder of the study (3.9%).Botulinum toxin (BTX-A) is a satisfactory long-term treatment without need for dose increase over. Both Botox and Dysport were effective when used interchangeably.
The long-term response to botulinum toxin injections in patients with blepharospasm undergoing upper eyelid surgery

Abstract:Background: Upper eyelid surgery (UES) is a therapeutical strategy used for those patients affected by blepharospasm (BSP) who either do not respond or experience a gradual decrease in responsiveness to botulinum toxin (BoNT) injections. Nevertheless, most of them need to restart with BoNT despite the intervention. Aim: To evaluate the long-term post-surgical response to BoNT in patients with BSP and to identify predictive factors associated to treatment outcome. Methods: We collected data of 60 BS patients, divided into two groups - blepharoplasty YES (8) and NO (52), collecting demographic - age, sex - and clinical data -disease duration, duration of the treatment with BoNT. Respective responses to injections - evaluated through the differences of both Jancovic Rating Scale and the Blepharospasm Disability Index pre and post BoNT (delta JRS and delta BSDI) just before their periodic three-month injection and after 1 month from it - were compared. Finally, clinical and demographics variables were included in multivariate regression and correlation analyses to assess their impact on the long-term response to injections. Results: Patients who underwent UES had significantly lower delta at both scales, showing a poorer outcome after BoNT treatment. No variable was found to be associated with the response. Discussion: Our data seem to suggest that surgery does not improve response to BoNT injections on the long run. As such, UES could be considered as an efficacious treatment in BSP just if evaluated soon after its performing. Long-term BSP management seems still difficult to be performed adequately and new therapeutical approaches are still needed.

Abstract:Purpose. To report on an improved botulinum toxin injection with conjunctival microincision for beginners, and to determine the effectiveness of botulinum toxin A (BTXA) in the treatment of patients with acute acquired comitant esotropia (AACE). Methods. Medical records of 29 AACE patients were retrospectively analyzed. BTXA was injected into the unilateral or bilateral medial rectus muscle with conjunctival microincision without electromyographic guidance. Success was defined as total horizontal deviation ≤10 prism diopters (PD) and evidence of binocular vision. Results. Twenty-nine patients were included, of whom 22 were male and 7 were female. The mean age at onset was 14.2 ± 7.4 (range, 4–34) years. The mean time from onset of AACE to injection was 18.4 ± 20.3 (range, 1–96) weeks. All patients completed at least 6 months of follow-up, and the mean follow-up after BTXA injection was 12.3 ± 4.8 months (range, 7–24 months). Neurological evaluation and brain magnetic resonance imaging (MRI) were unremarkable in all patients. The mean spherical equivalent refraction was −1.22 ± 2.85D and −0.97 ± 2.80D in the right and left eyes, respectively. Mean preinjective esotropia was 38.4 ± 18.9 PD (range, +10–+80 PD) at near and 40.2 ± 17.7 PD (range, +20–+80 PD) at far distance. The mean angle of deviation at 6 months after injection was 0.6 ± 4.1 PD (range, −3–+15 PD) at near and 3.0 ± 5.9 PD (range, 0–+20 PD) at far distance. There was significant difference in the angle of deviation at near and far fixation between pre-BTXA and post-BTXA 6 months ( p < 0.001 , p < 0.001 , resp.). There was no significant difference in the angle of deviation at near and far fixation between post-BTXA 6 months and post-BTXA at final follow-up ( p = 0.259 and 0.326, resp.). Mean stereoacuity improved from 338 to 88 arc seconds. During the follow-up period, 5 of 29 patients had recurrent esotropia. Two patients refused all further treatment, and the other 3 patients required incisional strabismus surgery. The success rates were 86.2% (25/29) at 6 months and 82.8% (24/29) at final follow-up. Conclusion. Conjunctival microincision injection of botulinum toxin is a practical and safe method for beginners to locate an extraocular muscle, which is as effective as the traditional methods. Botulinum toxin injection can be preferred as the first-line treatment for AACE patients with potential binocular vision.

Alexithymia Is Associated with Delayed Treatment Seeking in Acute Myocardial Infarction

Abstract:sations. Alexithymic trait scores were characterized using established classification scores (scores 6 61). Statistical analyses were carried out in a 2-step logistic regression approach. All possible factors were entered simultaneously in a single block. All 2-way interactions between the possible risk factors were included. To select significant interactions, a backward elimination procedure with a threshold of p ! 0.20 was used. The final sample included 83 patients, with 73.5% males. The mean time to treatment was 176.75 8 239.65 min. In the analyses, 36 patients (43.4%) were allocated to the cases, 47 patients (56.6%) to the controls. Table 1 lists the frequencies of factors concerning the awareness of the disease and care-seeking behavior. None of the possible confounding factors taken into account differed between cases and controls in a statistically significant manner. However, the association between alexithymia (OR for cases 3.7, 95% CI 1.3–10.1) and contact with primary care prior to hospitalization (OR for cases 3.5, 95% CI 1.3–9.3) was statistically significant. No difference was found between cases and controls with regard to diagnosis of lifetime anxiety and depression. The multivariate analysis confirms the differences indicated by the univariate model but the two factors did not show any interaction, since the copresence of the two risk factors produced a cumulative risk without significant amplification (OR approx. 15.5, 95% CI 2.85–84.6; p ! 0.0001). The study shows that low emotional awareness, measured by alexithymia, and having had contact with primary care were associated with increased time to treatment in STEMI. No interaction was found between the two factors. An early study of Kenyon et al. [6] found evidence that the presence of alexithymia predicted a delay in seeking treatment in AMI. A study of O’Carroll et al. [7] , 10 years later, found some differences in alexithymia when comparing a group of patients with AMI who were prompt care seekers ( ! 4 h), and a group of late AMI care seekers ( 1 4 h) but these differences did not reach the threshold for statistical significance. Even so the results of such studies are partially in agreement with our data; the extension of their study sample might have biased the results since STEMI is the severest form of AMI and the only in which the delay in medical intervention is currently known to be of great danger [2] . On the other hand, ours is the first report in the literature exploring alexithymia as a potential risk factor for delay in seeking care in STEMI and also the first study to divide cases and controls with the delayed time known to be associated with a poor outcome (2 h) [2] . Sociodemographic and clinical variables did not independently appear to explain the delay. These results partially confirm the previous findings [8] . However, there appears to be some contradictory findings concerning consultation with a primary care The elapsed time between a person experiencing pain or discomfort due to an acute myocardial infarction (AMI) with ST segment elevation (STEMI) and his/her arrival at a hospital seeking care is crucial for survival. In fact, the degree of myocardial necrosis is related to the length of the ischemic episode [1] , and survival depends on the early provision of reperfusion techniques. If reperfusion occurs within 2 h, some preservation of myocardial function is achieved; after this period, life expectancy or survival decreases drastically [2] . The most important factor impacting on longer time to treatment is the patient’s decision to delay seeking care [1] . Thus, psychological variables such as alexithymia influencing the decision may be relevant. Unfortunately many of the findings with respect to psychosocial factors in delayed care seeking are still controversial [3] . We sought to explore psychological factors that might explain the delay in seeking care in STEMI with particular attention to alexithymia. Consecutive patients aged 18–85 years, admitted to a Cardiology Ward at Cagliari University Hospital, from April 2010 to June 2011, diagnosed with STEMI according to the European Society of Cardiology guidelines [2] were eligible for this study. The study sample was divided based on ‘time to treatment’, defined as the interval between the onset of symptoms and the first diagnostic ECG: controls = early care-seeking patients ( ! 2 h); cases = late care-seeking patients ( 1 2 h).
