Abstract:Background: This study aimed to evaluate the early and mid-term outcomes of drug-coated balloon (DCB) use in patients who underwent intervention for transplant renal artery stenosis (TRAS). Material/Methods: We retrospectively reviewed the records of TRAS patients who received endovascular therapy with DCB in our institution from March 2016 to January 2017. Statistical analysis of pre-/postoperative levels of serum creatinine (Scr), systolic blood pressure (SBP), and renal artery peak systolic velocities (PSV) were performed. Results: Fourteen patients presenting with TRAS, which were mostly located at the anastomosis (n=9) and transplanted artery proximal portion (n=2), were treated with DCB. Three TRAS patients with in-stent restenosis (ISR) were also included in the series. The procedure technique success rate was 100%. The mean follow-up time was 8.6 months. The Scr level decreased from 481.8 mu mol/L (208.5-746.2 mu mol/L) pre-operation to 154 mu mol/L (89.1-301.2 mu mol/L, p<0.01) at 1 month post-intervention. The SBP varied from 161.4 mmHg (152-173 mmHg) to 144.8 mmHg (136-154 mmHg, p<0.01). Renal artery PSV decreased from 364.1 cm/s (217.6-511.9 cm/s) to 134.9 cm/s (79.8-184.2 cm/s, p<0.01). Eleven patients finished mid-term (>6 months) follow-up. The statistical results were not significant compared to those at 1 month, although they all slightly decreased. No re-intervention was performed. Conclusions: The endovascular approach to TRAS with DCB was a safe and effective treatment for restore and maintain the artery flow and renal function in short-term follow-up.

Abstract:Background: To report the incidence, etiology, and treatments of diabetes insipidus-like state that complicate percutaneous transluminal renal stenting (PTRS) for transplant renal artery stenosis (TRAS). Methods: Data from 7 patients on whom PTRS for TRAS was performed between October 2008 and March 2012 were reviewed retrospectively. The parameters investigated included blood flow velocity, blood pressure, and creatinine levels before and after the intervention. Results: The procedural success rate was 100%. Three cases developed a diabetes insipidus-(l)ike state in the immediate postprocedural period. Urine output returned to normal within 2 weeks after treatment. The median blood flow velocity was significantly reduced from 4.51 m/sec (4.31-4.61 m/sec) at the time of TRAS diagnosis to 1.33 m/sec (1.31-1.51 m/sec) at the most recent follow-up of the group with a diabetes insipidus-like state. The ratio of median blood flow velocity before and after stenting in the group with a diabetes insipidus-like state was significantly higher than that in the group without a diabetes insipidus-like state (3.39 vs. 1.93). Conclusions: Diabetes insipidus-like state that complicates PTRS for TRAS is not an uncommon event, but appears to be underreported in the medical literature. A high ratio of pre- and poststenting median blood flow velocity may be a predictor for a postprocedural diabetes insipidus-like state. The most probable cause may be the marked increase in renal arterial flow. Early recognition of the condition is essential to avoid dehydration and electrolyte imbalance.
Proximal Renal Artery Stenosis Presenting as Uncontrolled Hypertension

Abstract:Presentation: We present a case of 50-year-old lady who present with 1-month history of uncontrolled hypertension. Diagnosis: Magnetic resonance angiography showed stenosis of the left renal artery just beyond the ostium extending over approximately 7mm in length. Computed tomography angiography show focal narrowing of the proximal renal artery just distal to vessel origin, approximately 60% stenosis. Treatment: Anti-hypertensive medication was initiated in the ward. She was referred to vascular surgeon for renal bypass. Discussion: Renal artery stenosis is common cause of hypertension but may go unrecognised. The focus of this case is on the evaluation and necessity for a complete evaluation of the patient who is presenting with uncontrolled hypertension. To rule out renal artery stenosis, patient should be examined using CT-angiography or, if possible, arteriography.

Abstract:Hypertension, Ahead of Print. BACKGROUND:Renal denervation is a recognized adjunct therapy for hypertension with clinically significant blood pressure (BP)-lowering effects. Long-term follow-up data are critical to ascertain durability of the effect and safety. Aside from the 36-month follow-up data available from randomized control trials, recent cohort analyses extended follow-up out to 10 years. We sought to analyze study-level data and quantify the ambulatory BP reduction of renal denervation across contemporary randomized sham-controlled trials and available long-term follow-up data up to 10 years from observational studies.METHODS:A systematic review was performed with data from 4 observational studies with follow-up out to 10 years and 2 randomized controlled trials meeting search and inclusion criteria with follow-up data out to 36 months. Study-level data were extracted and compared statistically.RESULTS:In 2 contemporary randomized controlled trials with 36-month follow-up, an average sham-adjusted ambulatory systolic BP reduction of −12.7±4.5 mm Hg from baseline was observed (P=0.05). Likewise, a −14.8±3.4 mm Hg ambulatory systolic BP reduction was found across observational studies with a mean long-term follow-up of 7.7±2.8 years (range, 3.5–9.4 years;P=0.0051). The observed reduction in eGFR across the long-term follow-up was in line with the predicted age-related decline. Antihypertensive drug burden was similar at baseline and follow-up.CONCLUSIONS:Renal denervation is associated with a significant and clinically meaningful reduction in ambulatory systolic BP in both contemporary randomized sham-controlled trials up to 36 months and observational cohort studies up to 10 years without adverse consequences on renal function.

Abstract:Renal artery stenosis in its early phase is now considered a treatable cause of hypertension. The most common causes of renal artery stenosis (RAS) are atherosclerosis and fibromuscular dysplasia. It is important to be familiar with the various modalities for diagnosis as well as treatment of RAS, especially due to the increasing availability of minimally invasive therapies. These diagnostic modalities include ultrasound, computed tomography (CT), magnetic resonance imaging (MRI) and nuclear medicine scans. RAS if not treated leads to progressive development of renal insufficiency and renal failure. The minimally invasive therapies for treatment of RAS include percutaneous balloon angioplasty and stent placement. The objective of this article is to provide an overview of the diagnosis of renal arterial stenosis using newer imaging technology, as well as to familiarize the readers with the advances made in the realm of endovascular treatment.

Abstract:Background Randomized sham-controlled trials have confirmed the efficacy and safety of catheter-based renal denervation in hypertension. Data on the very long-term effects of renal denervation are scarce. Aims This study evaluates the 10-year safety and efficacy of renal denervation in resistant hypertension. Methods This prospective single-center study included patients with resistant hypertension undergoing radio-frequency renal denervation between 2010 and 2012. Office blood pressure, 24-h ambulatory blood pressure, antihypertensive medication, color duplex sonography, and renal function were assessed after 1-, 2- and 10-years. Results Thirty-nine patients completed the 10-year follow-up (mean follow-up duration 9.4 ± 0.7 years). Baseline office and 24-h ambulatory systolic blood pressure were 164 ± 23 mmHg and 153 ± 16 mmHg, respectively. After 10 years, 24-h ambulatory and office systolic blood pressure were reduced by 16 ± 17 mmHg ( P < 0.001) and 14 ± 23 mmHg ( P = 0.001), respectively. The number of antihypertensive drugs remained unchanged from 4.9 ± 1.4 to 4.5 ± 1.2 drugs ( P = 0.087). The estimated glomerular filtration rate declined within the expected range from 69 (95% CI 63 to 74) to 60 mL/min/1.73m 2 (95% CI 53 to 68; P < 0.001) through 10-year follow-up. Three renal artery interventions were documented for progression of pre-existing renal artery stenosis in two patients and one patient with new-onset renal artery stenosis. No other adverse events were observed during the follow-up. Conclusion Renal denervation was safe and sustainedly reduced ambulatory and office blood pressure out to 10 years in patients with resistant hypertension.

Abstract:AimRenal denervation (RDN) has beneficial effects on cardiac remodelling and function in resistant hypertension. We aimed to investigate the impact of RDN on cardiac angiogenesis during prolonged pressure overload.MethodsCardiac pressure overload was reproduced by transverse aorta constriction (TAC) procedure in adult Sprague Dawley male rats (n = 35). RDN/sham-RDN procedure was performed in surviving rats at 5 weeks after TAC.ResultsFive weeks post-TAC, transthoracic echocardiography revealed that myocardial hypertrophy occurred in TAC rats, with ejection fraction and fractional shortening not significantly changed. At the end of 10 weeks, cardiac systolic function was preserved in RDN group, but not in sham group. CD31 immunohistochemical staining showed that RDN-treated rats had higher cardiac capillary density than sham rats. However, no significant between-group difference was observed in the kidneys. A decreased protein expression of left ventricle vascular endothelial growth factor (VEGF) was observed in sham group, while RDN attenuated this decrease. Compared with sham, RDN resulted in a higher protein expression of VEGF receptor 2 (VEGFR2) and phosphorylated endothelial nitric oxide synthase (p-eNOS) in the heart.ConclusionRenal denervation benefits cardiac angiogenesis during sustained pressure overload, involving regulation of VEGF and VEGFR2 expression as well as activation of eNOS.

Abstract:Renal artery stenosis (RAS) refers to the presence of a lesion that narrows the vessel lumen by at least 60%, the hemodynamic significance of which can be further confirmed by determining the pressure gradient with endovascular methods [1] . It can occur in up to 5–10% of the general population, and its main cause is the presence of atherosclerotic plaque. Thus, the prevalence of the disease increases with age and correlates with the presence of cardiovascular risk factors: hypertension, dyslipidemia, smoking, male sex, diabetes, chronic kidney disease or a family history. In 20%, it occurs bilaterally or involves the single functioning kidney. Rarer causes include fibromuscular dysplasia (FMD), a history of or active vasculitis including the presence of Takayasu arteritis (type 3, 4 or 5 disease). FMD occurs in a younger population, is nine times more common in women and is more common in people without cardiovascular risk factors, except for smoking, which contributes to its occurrence. It develops in less than 1% of the general population and is usually diagnosed in the fifth decade of life, often incidentally during diagnostic imaging performed for other clinical indications, but cases diagnosed in childhood have also been described. The pathogenesis of FMD is not fully understood. There are suggestions that it is related to the presence of genetic and environmental factors, smoking and estrogens [1,2] . In 10%, the disease occurs in a family member. FMD takes the form of narrowing or widening (ectasias, aneurysms) of medium-sized arteries, and the sudden dissection of the vessel at the site of the primary lesion may occur. The condition most often affects the arteries of the central nervous system and renal arteries, less often other locations in the abdomen and vessels of the extremities. RAS, regardless of the cause, can lead to a number of adverse consequences. Reducing the flow behind the stenosis results in increased secretion of renin in the affected kidney, which leads to the activation of the renin-angiotensin-aldosterone system (RAAS) and the sympathetic nervous system and consequently the development of secondary hypertension and increased cardiovascular risk. RAS is the most common form of renovascular hypertension and one of the most common (5–28%) of secondary hypertension in general [3] . Progressive renal ischemia may contribute to the deterioration of its excretory function and lead to chronic kidney disease and, in advanced cases, to loss of function and atrophy of the organ. During RAS, there may also be recurrent pulmonary oedema with abrupt onset (flash pulmonary edema), and in those cases, exclusion of structural disease in the heart suggests the presence of RAS. Other conditions that raise suspicion of renal artery disease include onset of hypertension less than 30 years of age, rapid acceleration of previously well controlled blood pressure, onset of resistant hypertension, hypertensive crisis (acute heart failure, hypertensive encephalopathy), unexplained renal atrophy or disproportion in kidney size, unexplained kidney failure, worsening of kidney function after starting an angiotensin-converting enzyme inhibitor (ACEi) or an angiotensin-receptor blocker (ARB).

Abstract:<h2>Abstract</h2><p>Atherosclerotic renal artery stenosis is the leading cause of secondary hypertension and may lead to resistant (refractory) hypertension, progressive decline in renal function, and cardiac destabilization syndromes (pulmonary edema, recurrent heart failure, or acute coronary syndromes) despite guideline-directed medical therapy. Although randomized controlled trials comparing medical therapy with medical therapy and renal artery stenting have failed to show a benefit for renal artery stenting, according to comparative effectiveness reviews by the Agency for Healthcare Research and Quality, the trials may not have enrolled patients with the most severe atherosclerotic renal artery stenosis, who would be more likely to benefit from renal stenting. Because of limitations of conventional angiography, it is critical that the hemodynamic severity of moderately severe (50% to 70%) atherosclerotic renal artery stenosis lesions be confirmed on hemodynamic measurement. The authors review techniques to optimize patient selection, to minimize procedural complications, and to facilitate durable patency of renal stenting. The authors also review the current American College of Cardiology and American Heart Association guidelines and the Society for Cardiovascular Angiography and Interventions appropriate use criteria as they relate to renal stenting.</p>
Renal denervation restores biomechanics of carotid arteries in a rat model of hypertension

Abstract:The prevalence of hypertension increases with aging and is associated with increased arterial stiffness. Resistant hypertension is presented when drug treatments fail to regulate a sustained increased blood pressure. Given that the mechanisms between the sympathetic nervous system and the kidney play an important role in blood regulation, renal denervation (RDN) has emerged as a therapeutic potential in resistant hypertension. In this study, we investigated the effects of RDN on the biomechanical response and microstructure of elastic arteries. Common carotid arteries (CCA) excised from 3-month, 8-month, and 8-month denervated rats were subjected to biaxial extension-inflation test. Our results showed that hypertension developed in the 8-month-old rats. The sustained elevated blood pressure resulted in arterial remodeling which was manifested as a significant stress increase in both axial and circumferential directions after 8 months. RDN had a favorable impact on CCAs with a restoration of stresses in values similar to control arteries at 3 months. After biomechanical testing, arteries were imaged under a multi-photon microscope to identify microstructural changes in extracellular matrix (ECM). Quantification of multi-photon images showed no significant alterations of the main ECM components, elastic and collagen fibers, indicating that arteries remained intact after RDN. Regardless of the experimental group, our microstructural analysis of the multi-photon images revealed that reorientation of the collagen fibers might be the main microstructural mechanism taking place during pressurization with their straightening happening during axial stretching.

Abstract:Abstract Increasing studies strongly prove that renal denervation, a minimally invasive surgery, is a promising new non-drug treatment method that can effectively control blood pressure in patients with resistant hypertension, but the evaluation of the long-term blood pressure control effect of renal denervation for resistant hypertension is still lacking. Here, we critically review current long-term follow-up data about the use of renal denervation for RH to comprehensively evaluate the effectiveness of renal denervation for RH, and to provide practical guidance for practitioners who are establishing a renal denervation service. Limited by the current research, many problems need to be solved before renal denervation is applied to RH. In addition, ambulatory blood pressure should be the first choice for the evaluation of blood pressure. Finally, the continuous antihypertensive effect of renal denervation in different renal denervation systems also needs to be strictly compared.

Ten-Year Outcomes in Patients Undergoing Simultaneous Coronary and Renal Angiography—Does Renal Artery Stenosis Matter?

Abstract:Background: We aimed to characterize the population of consecutive patients undergoing coronary angiography with simultaneous renal artery angiography and assess prognostic factors at a 10 year follow-up. Methods: The KORONEF study was a prospective, single-center, observational, and descriptive study with 492 patients included. We analyzed several baseline demographics, clinical and periprocedural characteristics, and laboratory data, and we assessed the results of coronary angiography and renal artery angiography. Results: The study population consisted of 37.2% women, and the mean age was 64.4 ± 9.9 years (min. 30 years, max. 89 years). Angiography revealed significant renal artery stenosis (RAS) in 35 (7.1%) patients. Among patients with significant RAS (≥50%), we observed more women (57.1% vs. 35.7%, p = 0.011), and patients were older (69.1 ± 10.4 years vs. 64.0 ± 9.7 years, p = 0.005). In the whole population, all-cause death was reported in 29.9% of patients, myocardial infarction (MI) rate—in 11.8%, and stroke—in 4.9%. In the multivariable analysis, independent predictors of death were age 65–75 years (HR 2.88), age > 75 years (HR 8.07), diabetes (HR 1.59), previous MI (HR 1.64), chronic kidney disease (HR 2.22), unstable angina (HR 0.37), and left ventricular ejection fraction > 60% (HR 0.43). Conclusions: Over a 10 year follow-up, the all-cause death rate was 29.9%, showing no statistically significant differences between patients with and without significant RAS.

Endovascular treatment for transplant renal artery stenosis: ten years' experience from a single center

Abstract:Purpose Transplant renal artery stenosis (TRAS) is a common post-renal transplant complication. Although endovascular treatment is widely used to treat TRAS, previous research has been limited by small sample sizes. This article aims to present the clinical outcomes of endovascular treatment for TRAS in a large sample. Method Between January 2010 and December 2019, this study included patients with TRAS who were admitted to our center. All patients' clinical symptoms, comorbidities, imaging data, treatment, and follow-up results were reviewed retrospectively. Result 72 patients participated in this study. The median time between renal transplantation and TRAS was 5.25 months. Out of 72 patients, 55 (76.4%) received balloon dilatation in conjunction with stent deployment, 10 (13.9%) received drug-coated balloon dilatation alone, and 7 (9.7%) received balloon dilatation alone. The median follow-up period was 27 months. Primary patency rates were 100%, 81.8%, 74.5%, 64.6%, and 61.8% at 1, 3, 6, 12, and 24 months. A total of 23 patients were found to have restenosis during follow-up, with 6 (26.1%) requiring reintervention and none remaining restenosis after the second treatment. In the subgroup analysis of the three types of stenosis, patients with transplant renal stenosis at the anastomosis had a significantly higher rate of primary patency. Among endovascular treatments, the primary patency rate, postoperative creatinine clearance, and mean systolic blood pressure did not differ significantly. Conclusion Endovascular treatment resulted in favorable short-term patency as well as effective relief of renal dysfunction and renal hypertension in TRAS patients.

Abstract:Objectives To evaluate the long-term effectiveness of renal artery intervention on blood pressure and renal function in patients with renal artery stenosis(RAS).Methods A total of 120 in-hospital patients with serious RAS undergoing renal artery intervention in Department of Cardiology of Shenyang General Hospital of PLA from January 1998 to June 2006 were analyzed for outcomes.Blood pressure and renal function in 72 hours and 5 years after intervention were observed.Effect of renal artery intervention on renal hypertension and renal inadequacy was evaluated.Results Narrowing rate of the 120 patients was 76.1%±19.4%,diameter of stents was(5.8±1.2) mm,length of stents was(17.1±4.2) mm,residual stenosis rate was 2.1%±0.5%,and success rate of intervention was 100%.Blood pressure in 72 hours was(128.1±14.5/75.1±10.1) mm Hg and(140.2±18.7/84.4±13.2) mm Hg 5 years after intervention,decreased significantly comparing with the preoperative(178.7±28.4/ 100.2±17.6) mm Hg(P<0.05).Kind of antihypertensive drugs was decreased significantly after intervention(2.7±1.0 vs.3.5±1.0,P<0.05).There was no significant difference in estimated glomerular filtration rate(eGFR) in 72 hours(61.16±23.36) mL/(min·1.73 m2) and 5 years after intervention(64.12±23.30) mL/(min·1.73 m2) comparing with the preoperative(65.50±24.41) mL/(min·1.73 m2)(P>0.05).For the 27 cases with RAS combined with renal function abnormalities,preoperative eGFR was(35.43±11.66) mL/(min·1.73 m2),eGFR in 72 hours after intervention was(33.86±12.51) mL/(min·1.73 m2),and eGFR in 5 years after intervention was(39.10±12.69) mL/(min·1.73 m2).eGFR value was declined after 5 years comparing with that of preoperative,but it had no significant difference(P>0.05).Conclusions Renal artery intervention may significantly reduce blood pressure and kind of antihypertensive drugs in renal hypertensive patients,but no significant improvement on renal function.
Successful Endovascular Treatment of Chronic Renal Artery Occlusion: a Preliminary Retrospective Case Series Including 15 Patients

Abstract:PURPOSE:To evaluate in a retrospective, single-center, case series if percutaneous transluminal renal angioplasty/stenting of chronic renal artery occlusion is beneficial to renal function and blood pressure control, which remains controversial.METHODS:Data from 15 consecutive patients with renal artery stump who underwent successful percutaneous transluminal renal angioplasty/stenting only for unilateral chronic renal artery occlusion at our center from January 2007 to February 2018 and completed follow-up were retrospectively evaluated. Of the 15 patients, 14 (93.3%) were treated with stenting and 1 with only balloon angioplasty. Estimated glomerular filtration rate (eGFR) was calculated using the Modification of Diet in Renal Disease (MDRD) formula corrected for Chinese, and changes in blood pressure and need for antihypertensive medications were recorded.RESULTS:At median 1.5-year (interquartile range 0.5-5.0) follow-up, restenosis rate was 20.0%. Renal function improved or remained stable in 26.7% and 53.3% of patients, respectively, and blood pressure normalized or improved in 13.3% and 40.0% of patients, respectively. Young patients with Takayasu's arteritis or fibromuscular dysplasia appeared to benefit the most from revascularization.CONCLUSIONS:In this preliminary retrospective series of select patients with renal artery stump, endovascular treatment of chronic renal artery occlusion appeared to preserve renal function and improve blood pressure.
FOLLOW-UP OF RENAL DENERVATION IN PATIENTS WITH RESISTANT ARTERIAL HYPERTENSION

Abstract:Objective: Aim. Determining the long-term efficacy and safety of RDN the results of a 10-year follow-up Design and method: We selected 1146 patients with resistant arterial hypertension (RAH) who received 3 or more antihypertensive drugs. 16 patients were selected for renal artery denervation. RDN was performed in 8 patients (0.70%). 8 patients refused the procedure. The age was 52.13 ± 3.88 years (40–66), the duration of hypertension was 16.75 ± 2.17 (12–25) years. The number of antihypertensive drugs was 5.5 ± 0.18 (patients was in refractory hypertension). Office SBP/DBP on optimization of therapy was 156.88/95.00 ± 2.98 mm Hg and heart rate - 68.88 ± 3.28 beats/min. 24SBP/DBP was 149.16 ± 5.40/86.98 ± 5.08 mm Hg. 24HR was 70.12 ± 2.94 beats/min. All patients tolerated the procedure well. Results: The decrease in office SBP/DBP in the dynamics of 1 month, 12 months, 2 years, 5 years and 10 years were -12,59/-10,0, -11,17/-2,57, -20,31/-11,71, -27,71/-13,33 and -30,21/-16,67 mm Hg, respectively. The office heart rate (HR) decreased, its dynamics were -3.74 bpm, -4.68 bpm, -1.45 bpm, - 2.13 bpm. -4.21 bpm, - 3.88 bpm., respectively. The decrease in 24SBP/DBP was -14,45/-6,95, -8,70/-10,01, -19,45/-12,37, -19,91/-11,24, -29,93/-13,61 and -26,13/-23,35 mm Hg. (p 0.05. One patient died of thyroid cancer 2 years later. All other patients are alive and have not had any cardiovascular event or death during 10 years. Conclusions: Long-term results of renal artery denervation showed no cardiovascular events or cardiovascular death over 10 years. RDN contributed to a reduction in the number of antihypertensive drugs from 5.5 to 3.67 after 10 years of therapy. All patients achievement of the target BP level.

Abstract:Because previous randomized clinical trials have failed to show any benefits of renal artery stenting (RAS), clinicians have been confused regarding the optimal management of patients with atherosclerotic renal artery stenosis (ARAS). Here, we report a notable case with ARAS whose clinical course could help clinicians identify patients suitable for RAS. A 66-year-old woman with ARAS and some parameter abnormalities on renal duplex ultrasound was admitted to our hospital with severe hypertension (>250 mmHg). After the initiation of antihypertensive intravenous treatment, the patient manifested acute worsening of renal function over 6 days and required dialysis. Because renal failure continued to deteriorate over more than 4 weeks, we performed stenting to the severe ostial stenosis of the right renal artery. As a result, the patient achieved dramatic improvement in renal function and successful withdrawal from dialysis and has maintained stable control of blood pressure without additional events for more than 2 years since discharge. This case suggests that careful analysis of the patient's profile, such as history of rapidly deteriorating renal failure, presence of bilateral ARAS, or a solitary functional kidney, in addition to various parameters of renal duplex ultrasound, may help predict the efficacy of RAS in patients with ARAS. <Learning objective: Because several randomized studies have reported negative results for renal artery stenting (RAS), an individual-based approach is required for the treatment of atherosclerotic renal artery stenosis (ARAS). To determine whether RAS could be effective in patients with ARAS, it is important to assess patient-specific characteristics such as progression of renal failure (e.g. acute worsening), presence of bilateral ARAS, or the existence of a solitary functional kidney, as well as parameters of related examinations such as the renal arterial resistive index.>.

Abstract:Reports from recent trials indicate little additional benefit from stent supported revascularization in patients with atherosclerotic renal artery stenosis. These data have been questioned, particularly on the basis of including subjects with modest occlusive disease and reports of clinical benefits to patients with episodic congestive heart failure. Nonetheless, these data have moved the pendulum away from renal revascularization as a primary maneuver to one reserved for refractory hypertension and/or progressive loss of renal function and circulatory congestion. Recent data emphasize the limits of the kidney adaptation to reduced blood flow, the eventual development of widespread renal hypoxia with activation of inflammatory and fibrogenic pathways. Experimental data now support developing adjunctive measures to support angiogenesis and anti-inflammatory renal repair mechanisms, such as those observed with cell-based therapy with mesenchymal stem/stromal cells.

Abstract:Atherosclerotic renal artery stenosis(ARAS) is the most common cause of renal artery stenosis now, which not only causes hypertension, but also ischemic renal disease. Surgical revascularization is associated with significant perioperative morbidity and mortality and is no longer preferred after the 90 era. Percutaneous transluminal renal angioplasty with stent implantation has become the predominant intervention for ARAS since then. There are several randomized trials comparing medical management with angioplasty or renal artery(RA) stenting. It showed that angioplasty was not superior to medical therapy in blood pressure control, but had a significant drug-saving effect. Two large recent trials showed that RA stenting had no benefit over medical therapy in outcomes of blood pressure, renal function preservation, and mortality. Moreover, in the angioplasty and stenting for renal artery lesions(ASTRAL) trial that included 804 patients, a subgroup analysis was done based on baseline glomerular filtration rate(GFR), degree of stenosis, and renal size, and no difference in outcome was found between the groups. Studies showed that statins had protective effects on kidney beyond lowering lipidemia. Recent nonrandomized trials in ARAS showed that renin angiotensin aldosterone system(RAAS) blockade offers significant benefit in terms of renal function preservation, as well as survival. Agents that block the RAAS improve outcomes and should be a part of the medical regimen in ARAS. Concern of acute kidney injury in ARAS with impaired GFR has been a barrier to the use of these agents, but it can be easily detected early and reversed by discontinuing or adjusting the dose of the drug.
