Abstract:Statement of problem: The change in fit after ceramic layering of additively manufactured cobalt-chromium alloy frameworks has not been evaluated extensively. Purpose: The purpose of this in vitro study was to compare the fit of cobalt-chromium alloy fixed dental prostheses fabricated by different techniques before and after ceramic layering. Material and methods: A cobalt-chromium alloy master model was prepared to receive a 3-unit fixed dental prosthesis. Sixty cobalt-chromium alloy frameworks (N=60) were manufactured by using 3 manufacturing techniques (n=20): selective laser melting (Mediloy S-Co), soft milling (Ceramill Sintron), and conventional casting as the control group (Girobond NB). The replica technique was used to measure the marginal and internal discrepancies. The frameworks were then layered with ceramic, and the same fit measuring procedure was repeated. The results were compared before and after ceramic layering within each group. The data were analyzed using the Levene, ANOVA, and paired-samples t tests (α=.05). Results: A significant difference was found within groups before and after ceramic layering for selective laser melting (P=.006) and soft milling (P=.009) but not for conventional casting (P>.05). No statistical difference was reported in the marginal region for conventional casting group (P=.155) in contrast with the soft milling and selective laser melting groups (P=.003). Soft milling showed increased gap values in the marginal (P=.006) and occlusal regions (P=.004). Conclusions: Ceramic layering increased the discrepancy of the laser-sintered and milled frameworks, increasing the marginal discrepancy.

Abstract:The antitumor agent sulofenur (LY186641), which has shown promising activity against a wide range of cancers, causes hemolytic anemia and methemoglobinemia at dose-limiting toxicities. The antitumor and toxicological mechanism(s) of action of the drug is (are) not well understood, but unlike other antineoplastic agents, sulofenur does not interfere with DNA, RNA, or protein synthesis, or with polynucleotide function. In the present study, we evaluated the hypothesis that sulofenur undergoes bioactivation in vivo to generate p-chlorophenyl isocyanate (CPIC), which could carbamoylate biological macromolecules directly or form a conjugate with glutathione (GSH) which would serve as a latent form of CPIC. The objectives of this study, therefore, were to determine if the GSH and N-acetylcysteine conjugates of CPIC were excreted into bile and urine, respectively, after an i.p. dose of sulofenur to rats. In addition, the chemical stability and thiol exchange properties of these S-linked conjugates were determined. The results of this study indicate that sulofenur does undergo metabolism in vivo to yield the GSH conjugate of CPIC, and that this conjugation reaction is reversible and subject to thiol exchange in buffered aqueous solution (pH 7.4, 37 degrees C). In contrast, sulofenur itself was stable under these same conditions, even in the presence of GSH and glutathione-S-transferase (GST), thus raising the possibility that bioactivation of sulofenur is necessary for liberation of CPIC. These findings suggest that the generation of this isocyanate in vivo and subsequent carbamoylation of biological macromolecules may play a role in the toxicity and/or antitumor activity of sulofenur and related diarylsulfonylureas.
Pilot Study of Organ Preservation Multimodality Therapy for Locally Advanced Resectable Oropharyngeal Carcinoma

Abstract:The purpose of this study was to determine the early efficacy and toxicity of a new multimodality organ-preservation regimen for locally advanced, resectable oropharyngeal squamous cell carcinoma (SCC). Patients with T3-4N0-3M0 or T2N2-3M0 oropharyngeal SCC were eligible for this Phase II study. Patients needed the physiologic reserve for surgery and technically resectable tumors. Induction carboplatin (area under the curve = 6) and paclitaxel (200 mg/m2) × 2 cycles (q21 days) were given. Objective responders received definitive radiotherapy (XRT), 70 Gy/7 weeks with concurrent weekly paclitaxel. Initially, the dose of paclitaxel was 50 mg/m2/week; because of mucosal toxicity it was reduced to 30 mg/m2/week. Patients with N2-3 disease received post-XRT neck dissection and 2 more cycles of “adjuvant” chemotherapy. In the first 22 patients, the neutropenic fever rate was 27%. Although there has been no grade IV-V toxicity from induction therapy, grade II-III toxicity resulted in an unacceptable delay in starting XRT in 14% of patients. The response rate to induction chemotherapy was 91%. Grade III mucositis occurred in all patients during concurrent chemoradiotherapy. One patient died of pneumonia during concurrent chemoradiotherapy after receiving 26 Gy and 3 doses of paclitaxel 50 mg/m2. No dose-limiting toxicity occurred in 15 patients treated with concurrent paclitaxel 30 mg/m2/week. Actuarial overall survival at 18 months is 82%; local–regional control is 86%. To date, distant metastases have not developed in any patients. This regimen has intense but acceptable acute toxicity. The maximum tolerated dosage of weekly paclitaxel during standard continuous-course XRT is confirmed to be 30 mg/m2/week. The treatment efficacy of this regimen (response rate and short-term local–regional and distant control) is encouraging. Accrual continues to obtain long-term toxicity, efficacy, and quality-of-life data.
Cobalt chromium alloys in fixed prosthodontics: Investigations of mechanical properties and microstructure

Abstract:Statement of problem: Cobalt chromium (Co-Cr) alloys possess beneficial mechanical properties because alloys, even in thin sections, can resist high mastication forces and exhibit an acceptable bond to the surface porcelain layer. Traditional manufacturing techniques of Co-Cr alloys such as casting have been replaced with newer fabrication techniques, such as milling, laser melting, and presintered milling. Despite scarce documentation, these new manufacturing techniques are being used to fabricate dental and implant constructions. Purpose: This in vitro study investigates the hardness, yield strength, elastic modulus, and microstructure of the most commonly used Co-Cr alloys for fixed prosthodontics based on manufacturing technique. In addition, this study investigates the effect of heat treatment on the mechanical properties and microstructure of these materials. Material and methods: Five Co-Cr alloys were included (dumbbell and rectangular shaped) based on four manufacturing techniques: cast, milled, laser melted, and presintered milled. Commercially pure titanium grade 4 and titanium-6 aluminum-4 vanadium ELI (extra low interstitial) were included for comparison, and yield strength and elongation after fracture were evaluated. The specimens were tested for hardness using the Vickers test and for elastic modulus using a nondestructive impulse excitation technique. The microstructure of selected specimens was analyzed using focused ion beam-scanning electron microscopy (FIB-SEM) and energy dispersive X-ray spectroscopy (EDS). Results: The mechanical properties depend on the manufacturing technique used; the laser-melted and presintered Co-Cr specimens demonstrated the highest mechanical properties, followed by the milled and cast groups. Both the laser-melted and the presintered milled Co-Cr specimens showed smaller grain size compared with the cast and milled Co-Cr specimens. The titanium-6 aluminum-4 vanadium ELI demonstrated higher hardness and yield strength compared to commercially pure titanium grade 4. No major differences were observed for the selected materials regarding the mechanical properties and microstructural appearance after heat treatment. Conclusions: The laser melting and presintered milling techniques produced higher mechanical properties compared with the cast and milled Co-Cr. These findings were confirmed through microstructural analysis with respect to the grain size, precipitation, and number of pores.

Abstract:Cobalt-chromium (Co-Cr) alloy is a widely used base material for dental fixed prostheses. These restorations can be produced through casting technique, subtractive or additive manufacturing technologies. However, limited information is available regarding the influence of manufacturing techniques on the properties of Co-Cr alloy since most studies used different chemical compositions of Co-Cr alloy for different manufacturing methods. This study compares the mechanical properties, metal-ceramic bond strength, and microstructures of specimens produced by casting, milling, and selective laser melting (SLM) from one single Co-Cr alloy composition. The mechanical properties of the alloy were investigated by tensile and Vickers hardness tests, and metal-ceramic bond strength was determined by three-point bending. Scanning electron microscopy (SEM) with backscattered electron (BSE) images and optical microphotographs were used to analyze the surface microstructures. Compared with the casting and milling techniques, SLM Co-Cr alloy specimens indicated enhanced mechanical properties and comparable metal-ceramic bond strength. Besides, the microstructures of the SLM specimens showed finer grains with more second phase particles than the casting and milling specimens. The results of our study indicate that SLM might be superior to traditional techniques for the manufacturing of fixed dental restorations.

Effect of lamination layer thickness and abutment preparation on the fit of Co-Cr multi-unit prostheses fabricated by additive manufacturing: An in vitro study

Abstract:Statement of problem: Selective laser melting has been increasingly used in the fabrication of metal-ceramic prostheses. The lamination layer thickness may influence the fit of the restoration. However, data regarding its impact remain scarce. Purpose: The purpose of this in vitro study was to evaluate the marginal and internal fit of cobalt-chromium (Co-Cr) 3-unit frameworks fabricated by additive manufacturing with different melting layer thicknesses on different abutments. Material and methods: The first right maxillary premolar and molar from a typodont were prepared with a circumferential 1.2-mm chamfer, 2-mm occlusal reduction, and total taper of 8 degrees to receive 3-unit fixed partial dentures. After framework design, the data were sent to a laser machine, and 30 specimens were fabricated from Co-Cr metal powder (Mediloy S-Co) by selective laser melting. Specimens were assigned to 3 groups (n=10) with different lamination layer thicknesses of 25 (L25), 50 (L50), and 100 (L100) μm. Marginal fit and internal fit were evaluated. Results were compared among layer thickness groups and abutments. Data were compared by using the Levene test, t test, and ANOVA (α=.05). Results: A statistical difference was found between different layer thickness groups (P=.016), but not between abutments (P=.429). On the abutment level, significant difference was found in the gap values for the molars (P=.049) between layer thickness groups, but not for the premolars (P=.126). Group L25 reported the smallest mean discrepancy values, and group L100 reported the highest, with statistical difference found between group L100 and groups L25 (P<.001) and L50 (P<.025), but not between groups L25 and L50 (P=.094). Conclusions: The lamination layer thickness affected the adaptation of the metal frameworks. Framework fit did not significantly differ between abutments.
Additive Manufacturing of Cobalt-Based Dental Alloys: Analysis of Microstructure and Physicomechanical Properties

Abstract:The limitations of investment casting of cobalt-based alloys are claimed to be less problematic with significant improvements in metal additive manufacturing by selective laser melting (SLM). Despite these advantages, the metallic devices are likely to display mechanical anisotropy in relation to build orientations, which could consequently affect their performance “in vivo.” In addition, there is inconclusive evidence concerning the requisite composition and postprocessing steps (e.g., heat treatment to relieve stress) that must be completed prior to using the devices. In the current paper, we evaluate the microstructure of ternary cobalt-chromium-molybdenum (Co-Cr-Mo) and cobalt-chromium-tungsten (Co-Cr-W) alloys built with direct metal printing and LaserCUSING SLM systems, respectively, at 0°, 30°, 60°, and 90° inclinations (Φ) in as-built (AB) and heat-treated (HT) conditions. The study also examines the tensile properties (Young’s modulus, E ; yield strength, R P0.2 ; elongation at failure, A t ; and ultimate tensile strength, R m ), relative density (RD), and microhardness (HV5) and macrohardness (HV20) as relevant physicomechanical properties of the alloys. Data obtained indicate improved tensile properties and HV values after a short and cost-effective heat-treatment cycle of Co-Cr-Mo alloys; however, the process did not homogenize the microstructure of the alloy. Annealing heat treatment of Co-Cr-W led to significant isotropic characteristics with increased E and A t (except for Φ = 90°) in contrast to decreased R P0.2 , R m , and HV values, compared to the AB form. Similarly, the interlaced weld-bead structures in AB Co-Cr-W were removed during heat treatment, which led to a complete recrystallization of the microstructure. Both alloys exhibited defect-free microstructures with RD exceeding 99.5%.

Abstract:Statement of problem Three-dimensional (3D) printing of cobalt chromium (Co-Cr) removable partial dentures (RPDs)by selective laser melting (SLM) has been claimed to be less challenging than by conventional casting and provides significant improvements. However, the adaptation and fit of the SLM framework and the optimum build orientation are still unclear. Purpose The purpose of this in vitro and clinical study was to evaluate the effect of different build orientations on the adaptation of removable partial denture frameworks fabricated by SLM technology in vitro and to compare the adaptation of the SLM and conventional RPD frameworks clinically. Material and methods A master model simulating a maxillary arch of Kennedy class III modification 1 was scanned and duplicated to create a virtual 3D cast and reference cast. Four groups (n=40) of Co-Cr RPD frameworks were fabricated. For the SLM groups, the Co-Cr framework was virtually designed and exported for SLM printing. The SLM printing was done in 3 different build orientations: 0-degree (n=10), 45-degree (n=10), and 90-degree (n=10) groups. Other Co-Cr frameworks were conventionally cast (n=10). All Co-Cr frameworks were scanned and virtually superimposed with the master model using a surface-matching software program. The gap under 9 selected points in the palatal major connectors was analyzed and calculated. A smaller gap indicates more surface adaptation and close contact between the palatal major connector and the master model. The data were analyzed using the Kruskal-Wallis and Dunnett T3 tests (α=.05). Three patients with a partially dentate maxillary arch were enrolled in the clinical part based on inclusion criteria. Two RPD frameworks were provided for each patient (conventional casting and SLM printing). The adaptation of each framework was assessed by measuring the gap between the palatal major connector of the framework and the palate with light-body silicone. The differences in adaptation between the conventional and SLM frameworks were compared by using independent t tests (α=.05). Results The in vitro study identified significant differences in the adaptation of the palatal major connector among the 4 groups ( P <.001), except for conventional and 0-degree SLM printing ( P =.999). The conventional and 0-degree SLM frameworks exhibited the best adaptation, with the lowest gap underneath the palatal major connector of the RPD framework (0.01 ±0.02 mm and 0.01 ±0.01 mm, respectively). In the clinical part of the study, no significant difference was found between the adaptation of SLM and the conventional framework ( P =.430) Conclusions The adaptation of SLM printing can be maximized with less gap under the palatal major connector of the RPD framework when using the 0-degree build orientation. Co-Cr frameworks produced with SLM printing technology were comparable with conventionally produced frameworks; hence, SLM printed frameworks can be an alternative for clinical applications when optimum SLM parameters are applied.

The Effect of Firing Cycles on the Fit of Cobalt-Chromium Multiunit Prostheses Fabricated by Additive Manufacturing

Abstract:Purpose: The effect of annealing and firing cycle on the adaptation of cobalt-chromium (CoCr) restorations fabricated by additive manufacturing has been poorly documented. This study compared the marginal and internal fit of CoCr three-unit fixed dental prostheses before and after firing cycles. Material and methods: The first right maxillary premolar (MP) and molar (MM) on a typodont model, were prepared with a circumferential 1.2 mm chamfer, 2 mm occlusal reduction, and total taper of 8 degrees, to receive 3-unit fixed dental prostheses (FDP). After the framework design, 20 framework specimens were fabricated using CoCr metal powder by selective laser melting. The replica technique was used for marginal and internal fit measurements in mesiodistal and buccolingual planes. The specimens were submitted to an annealing process, and to ceramic firing protocol. Measurements were repeated after each firing cycle. The data were compared by using Levene test, t-test, and analysis of variance (ANOVA) (α = 0.05). Results: A significant difference was found in the total intaglio surface discrepancy between the 3 treatments. The annealing treatment induced a statistical difference in the discrepancy value compared to the as printed state (p = 0.028). A significant difference was detected between the initial manufacturing phase and the ceramic layering firing cycle (p = 0.003). Conclusions: Firing cycles induced an alteration of the frameworks adaptation.

Abstract:Cobalt–chromium (Co–Cr) alloys are the most widely used materials for removable and fixed dental prosthetic frameworks. The fitting accuracy between these components in dental prosthetic frameworks assembles (DPFAs) is largely influenced by the manufacturing method. This study presents a novel manufacturing method that combined two common techniques for obtaining one single framework: casting of Co–Cr inserts on top of parts previously manufactured by selective laser melting (SLM) of Co–Cr powder (CoM). Horizontal (n = 4) and vertical (n = 3) surfaces were microscopically analyzed (n = 770 count sum). The results revealed a high precision of the process and high fitting accuracy between the hybrid frameworks. The average distance measured between the frameworks in joined position was 41.08 ± 7.56 μm. In conclusion, the manufacturing of Co–Cr alloys DPFA using the CoM method reduced the deformation of hybrid frameworks and improved the joining accuracy between them.

Abstract:This study assessed the influence of CAD/CAM milling and 3D-printing fabrication methods on mechanical properties of 3-unit interim fixed dental prosthesis (IFDPs) after thermo-mechanical aging. Forty 3-unit IFDPs were fabricated on a mandibular right second premolar and second molar of a typodont cast. Samples were fabricated from the following materials; auto-polymerized polymethyl methacrylate (conventional resin), CAD/CAM PMMA (milled resin) and two different CAD/CAM 3D-printed composite resins; digital light processing Asiga (DLP AS) and stereolithography NextDent (SLA ND). Mechanical properties were compared between the studied materials using Kruskal–Wallis test, followed by multiple pairwise comparisons using Bonferroni adjusted significance. There was a significant difference in flexural strength and microhardness between the studied materials (p < 0.001), with the highest mean ± SD reported in the milled IFDPs (174.42 ± 3.39, 27.13 ± 0.52), and the lowest in the conventional IFDPs (98.02 ± 6.1, 15.77 ± 0.32). Flexural strengths differed significantly between the conventional IFDPs and all materials except DLP AS. The highest elastic modulus was recorded in the milled group, and the lowest in the SLA ND group (p = 0.02). In conclusion, superior flexural strength, elastic modulus, and hardness were reported for milled IFDPs. SLA ND printed IFDPs showed comparable mechanical properties to milled ones except for the elastic modulus.

Abstract:Purpose: The aim of this comparative in vitro study was to assess the bond strength and mechanical failure of carbon-fiber-reinforced composites against cobalt–chrome structures with ceramic veneering. Materials and methods: A total of 24 specimens (12 per group) simulating dental prosthetic frameworks were fabricated. The experimental specimens were subjected to a thermocycling aging process and to evaluate bond strength. All specimens were subjected to a three-point bending test to fracture using a universal testing machine. Results: The cobalt–chrome/ceramic group yielded a bond strength value of 21.71 ± 2.16 MPa, while the carbon-fiber-reinforced composite group showed 14.50 ± 3.50 MPa. The failure assessment reported statistical significance between groups. Although carbon-fiber-reinforced composite group showed lower bond strength values, the chipping incidence in this group was as well lower. Conclusions: The chrome–cobalt/ceramic group showed greater bonding strength compared to the carbon-fiber-reinforced composite; most of the fractures within the cobalt–chrome/ceramic group, had no possibility of direct clinical repair.

Abstract:Multitargeting is a valuable concept in drug design for the development of effective drugs for the treatment of multifactorial diseases. This concept has most frequently been realized by incorporating two or more pharmacophores into a single hybrid molecule. Many such hybrids, due to the increased molecular size, exhibit unfavorable physicochemical properties leading to adverse effects and/or an inappropriate ADME (absorption, distribution, metabolism, and excretion) profile. To avoid this limitation and achieve additional therapeutic benefits, here we describe a novel multitargeting strategy based on the synergistic effects of a parent drug and its active metabolite(s). The concept of metabolism‐activated multitargeting (MAMUT) is illustrated using a number of examples.

Abstract:Statement of problem: The selective laser melting (SLM) manufacturing technique has been widely employed to produce Co-Cr dental metal frameworks. The selection of Co-Cr alloy powders has the potential to influence the microstructure and tensile properties, consequently impacting the bond strength of the metal-porcelain. However, limited information is available regarding the effect of Co-Cr alloy powder on these properties when all other factors remain consistent. Purpose: The purpose of this in vitro study was to assess how the choice of Co-Cr alloys during SLM manufacturing influences the microstructure, tensile properties, and bond strength of metal-ceramic combinations. Material and methods: Three different Co-Cr alloy powders, Co-Cr-Mo, Co-Cr-Mo-W, and Co-Cr-W were selected in this study. The powder characteristics and chemical compositions were analyzed using a scanning electron microscope (SEM) and energy dispersive X-ray (EDX) analysis, respectively. Subsequently, 12×12×15-mm cube specimens, cylindrical tensile test specimens, and 25×3×0.5-mm metal strips were fabricated using the SLM technique. Microstructural investigations of the cube specimens were conducted after metallographic preparation using SEM. The cylindrical tensile specimens (n=8) from each composition were subjected to tensile tests at a deformation rate of 0.5 mm/min. Following the application of ceramic to the metal specimens (n=10) in each group, the strength of the metal-ceramic bond was evaluated through a 3-point bend test conducted at a crosshead speed of 1 mm/min. Mechanical properties obtained from the tensile tests and bond strength values were statistically analyzed using a one-way ANOVA and Tukey post hoc comparison tests (α=.05). Results: Melt pool boundaries, columnar and equiaxed grains, and precipitates were observed in the microstructures of 3 different alloys produced by SLM. The Co-Cr-Mo-W alloy had more uniformly dispersed and finely distributed precipitates compared with other alloy compositions. The Co-Cr-Mo-W alloy had exhibited the highest yield strength (1068.0 ±41.2 MPa) and ultimate tensile strength (1263.4 ±10.7 MPa) while showing the lowest ductility under the tensile tests (6.1 ±0.9%) among all 3 alloys. Significant differences in the tensile mechanical properties were observed in the alloys except between the yield strength of the Co-Cr-Mo and Co-Cr-W alloys. The highest elongation (8.9 ±1.2%) was seen in the Co-Cr-Mo alloy. However, no significant differences were detected regarding the bond strength of all 3 groups (P>.05). The mean bond strength values were approximately 42 MPa for all the alloys. Conclusions: The results indicate that the selection of different Co-Cr alloy powders used in SLM production may influence both microstructure and tensile properties. However, the strength of the metal-ceramic bond of Co-Cr alloys remained unaffected by this selection.

Abstract:Stable infants with anaemia needing a transfusion with adult red blood‐cells were studied to elucidate changes in brain blood‐flow velocity. Within 24 hours and at five to six days following transfusion a substantial mean flow velocity reduction was observed. Haemodynamic factors contributing to the reduction were an increase in cerebrovascular resistance and an increase in whole blood viscosity, as reflected by a raised pulsatility index. Transfusion with adult red blood‐cells causes an elevation in haemoglobin concentration, thereby increasing the total oxygen‐carrying capacity of arterial blood; however, this lowers the concentration of fetal haemoglobin which possesses a higher affinity for oxygen. Since cerebral oxygen transport is equal to the product of cerebral blood‐flow and arterial oxygen content, this finding suggests the existence of a homeostatic mechanism for cerebral oxygen transport. The actual amount of cerebral oxygen transport was found to increase progressively as the percentage of fetal haemoglobin rose above 30 per cent. At higher fetal haemoglobin levels, appropriate elevations in cerebral blood‐flow occurred, causing an increase in the supply of oxygen to the brain.

Abstract:This study compared the effect of using milled fiber-reinforced resin composite and Co-Cr (milled wax and lost-wax technique) frameworks for 4-unit implant-supported partial fixed dental prostheses; and also, evaluated the influence of the connector's cross-sectional geometries on the mechanical behavior. Three groups of milled fiber-reinforced resin composite (TRINIA) for 4-unit implant-supported frameworks (n = 10) with three connectors geometries (round, square, or trapezoid), and three groups of Co-Cr alloy frameworks manufactured by milled wax/lost wax and casting technique, were analyzed. The marginal adaptation was measured before cementation using an optical microscope. Then, the samples were cemented, thermomechanical cycled (load of 100 N/2 Hz, 106 cycles; 5, 37, and 55 ᵒC, a total of 926 cycles at each one), and cementation and flexure strength (maximum force) analyzed. Analysis of stress distribution in framework veneered considering resin and ceramic properties for fiber-reinforced and Co-Cr frameworks, respectively, implant, and bone was by finite element analysis under three contact points (100 N) on the central region. ANOVA and Multiple paired test-t with Bonferroni adjustment (α = 0.05) were used for data analysis. Fiber-reinforced frameworks showed better vertical adaptation (mean ranged from 26.24 to 81.48 μm) compared to the Co-Cr frameworks (mean ranged from 64.11 to 98.12 μm), contrary to horizontal adaptation (respectively, means ranged from 281.94 to 305.38 μm; and from 150.70 to 174.82 μm). There were no failures during the thermomechanical test. Cementation strength showed three times higher for Co-Cr compared to fiber-reinforced framework, as well as flexural strength (P < .001). Regarding stress distribution, fiber-reinforced had a pattern of concentration in the implant-abutment complex. There were no significant differences in stress values or changes observed among the different connector geometries or framework materials. Trapezoid connector geometry had a worse performance for marginal adaptation, cementation (fiber-reinforced 132.41 N; Co-Cr 255.68 N) and flexural strength (fiber-reinforced 222.57 N; Co-Cr 614.27 N). Although the fiber-reinforced framework showed lower cementation and flexural strength, considering the stress distribution values and absence of failures in the thermomechanical cycling test, it can be considered for use as a framework for 4-unit implant-supported partial fixed dental prostheses in the posterior mandible. Besides, results suggest that trapezoid connectors mechanical behavior did not perform well compared to round or square geometries.
Evaluation of internal fit of press ceramic and porous structured cobalt-chromium crown fabricated by additive manufacturing

Abstract:Background: The purpose of this in vitro study is to fabricate a novel metal-ceramic prosthesis with a porous structure, to compensate for the disadvantages associated with the design of existing prostheses, and to measure the internal fit of this prosthesis. Materials and methods: In this in vitro study, the mandibular first molar was scanned from the dental computer-aided-design to design a 3 mm porous structure frame. The frame was produced using the lamination method and fired in a pressed ceramic. For comparison, pore-free specimens were fabricated by selective laser sintering (SLS) as described above, and porous specimens were fabricated by casting (total n = 30). The internal fit was then measured using a digital microscope (at 100× magnification), and the data were analyzed using one-way ANOVA (α = 0.05). Results: The total mean internal discrepancies for each group were 42.32 ± 22.50 μm for the porous structure SLS group (PS-group), 107.54 ± 38.75 μm for no-porous casting group (group), and 121.36 ± 50.19 μm for the no-porous SLS group (group), with significant differences (P < 0.05) among all groups. Conclusion: The internal discrepancies of porous structure crown fabricated by SLS were smaller than that of no-porous crown fabricated by casting and SLS. Based on these laboratory findings, further studies should be conducted to evaluate the feasibility of the newly designed porous structure and press ceramic prosthesis to determine whether they can be applied in clinical practice.
