Abstract:Background: There is a need to integrate informatics education into medical training programs given the rise in demand for health informaticians and the call on the Accreditation Council for Graduate Medical Education (ACGME) and the body of undergraduate medical education (UGME) for implementation of informatics curricula. Objectives: This report outlines a 2-year longitudinal informatics curriculum now currently in its seventh year of implementation. This report is intended to inform United States (US) Graduate Medical Education (GME) program leaders of the necessary requirements for implementation of a similar program at their institution. Methods: The curriculum aligns with the core content for the subspecialty of clinical informatics (CI) and is led by a multidisciplinary team with both informatics and clinical expertise. This educational pathway has a low direct cost and is a practical example of the academic learning health system (aLHS) in action. The pathway is housed within an internal medicine department at a large tertiary academic medical center. Results: The curriculum has yielded 13 graduates from both internal medicine (11, 85%) and pediatrics (2, 15%) whose projects have spanned acute and ambulatory care and multiple specialties. Projects have included Clinical Decision Support (CDS) tools, of which some will be leveraged as substrate in applications seeking extramural funding. Graduates have gone on to CI board certification and fellowship, as well as several other specialties, creating a distributed network of clinicians with specialized experience in applied CI. Conclusions: An informatics curriculum at the GME level may increase matriculation to CI fellowship and more broadly increase development of the CI workforce through building a cadre of physicians with HIT expertise across specialties without formal CI board certification. We offer an example of a longitudinal pathway which is rooted in aLHS principles. The pathway requires a dedicated multidisciplinary team and departmental and IT leadership support.

Abstract:Background In 2011, the American Board of Medical Specialties established clinical informatics (CI) as a subspecialty in medicine, jointly administered by the American Board of Pathology and the American Board of Preventive Medicine. Subsequently, many institutions created CI fellowship training programs to meet the growing need for informaticists. Although many programs share similar features, there is considerable variation in program funding and administrative structures. Objectives The aim of our study was to characterize CI fellowship program features, including governance structures, funding sources, and expenses. Methods We created a cross-sectional online REDCap survey with 44 items requesting information on program administration, fellows, administrative support, funding sources, and expenses. We surveyed program directors of programs accredited by the Accreditation Council for Graduate Medical Education between 2014 and 2021. Results We invited 54 program directors, of which 41 (76%) completed the survey. The average administrative support received was $27,732/year. Most programs (85.4%) were accredited to have two or more fellows per year. Programs were administratively housed under six departments: Internal Medicine (17; 41.5%), Pediatrics (7; 17.1%), Pathology (6; 14.6%), Family Medicine (6; 14.6%), Emergency Medicine (4; 9.8%), and Anesthesiology (1; 2.4%). Funding sources for CI fellowship program directors included: hospital or health systems (28.3%), clinical departments (28.3%), graduate medical education office (13.2%), biomedical informatics department (9.4%), hospital information technology (9.4%), research and grants (7.5%), and other sources (3.8%) that included philanthropy and external entities. Conclusion CI fellowships have been established in leading academic and community health care systems across the country. Due to their unique training requirements, these programs require significant resources for education, administration, and recruitment. There continues to be considerable heterogeneity in funding models between programs. Our survey findings reinforce the need for reformed federal funding models for informatics practice and training. Preparing Future Pediatric Care Providers with a Clinical Informatics Elective

Abstract:Background Clinical informatics (CI) has reshaped how medical information is shared, evaluated, and utilized in health care delivery. The widespread integration of electronic health records (EHRs) mandates proficiency among physicians and practitioners, yet medical trainees face a scarcity of opportunities for CI education. Objectives We developed a CI rotation at a tertiary pediatric care center to teach categorical pediatric, pediatric–neurology, and medicine–pediatric residents foundational CI knowledge and applicable EHR skills. Methods Created in 2017 and redesigned in 2020, a CI rotation aimed to provide foundational CI knowledge, promote longitudinal learning, and encourage real-world application of CI skills/tools. Led by a team of five physician informaticist faculty, the curriculum offers personalized rotation schedules and individual sessions with faculty for each trainee. Trainees were tasked with completing an informatics project, knowledge assessment, and self-efficacy perception survey before and after rotation. Paired t -test analyses were used to compare pre- and postcurriculum perception survey. Results Thirty-one residents have completed the elective with their projects contributing to diverse areas such as medical education, division-specific initiatives, documentation improvement, regulatory compliance, and operating plan goals. The mean knowledge assessment percentage score increased from 77% (11.6) to 92% (10.6; p ≤ 0.05). Residents' perception surveys demonstrated improved understanding and confidence across various informatics concepts and tools (p ≤ 0.05). Conclusion Medical trainees are increasingly interested in CI education and find it valuable. Our medical education curriculum was successful at increasing residents' understanding, self-efficacy, and confidence in utilizing CI concepts and EHR tools. Future data are needed to assess the impact such curricula have on graduates' proficiency and efficiency in the use of CI tools in the clinical workplace. Imaging Informatics Education in Clinical Informatics Programs: Perspective from Imaging and Clinical Informatics Professionals

Abstract:Background Imaging and Clinical Informatics are domains of biomedical informatics. Imaging Informatics topics are often not covered in depth in most Clinical Informatics fellowships. While dedicated Imaging Informatics fellowships exist, they may not have the same rigor as ACGME (Accreditation Council for Graduate Medical Education) accredited Clinical Informatics fellowships and they do not provide a direct path toward subspecialty board certification. Objectives We compared published curricula and test content between Clinical and Imaging Informatics fellowship programs. We then highlighted differences between training programs and identified overlapping topics and opportunities for additional education for each type of trainee. Methods Published consensus curricula and topics were extracted for each specialty. Two informaticists compared topics as shared or not shared between specialties. Next, test content outlines were compared for each specialty exam, extracted, and classified as shared or not shared content. A Venn diagram was created to highlight areas unique to each specialty as well as areas of overlap. Results There were 139 Clinical Informatics topics compared with 97 Imaging Informatics topics. Of the 139 Clinical Informatics topics, 115 (83%) were covered in the Imaging Informatics curriculum. Of the 97 Imaging Informatics topics, 74 (76%) were covered in the Clinical Informatics curriculum. When using test content outline data, 170 out of 397 (43%) Imaging Informatics topics matched to 64 out of 139 (46%) Clinical Informatics topics. We describe examples of overlapping topics and those unique to each program to identify potential areas to expand. Conclusion Imaging Informatics and Clinical Informatics fellowship programs have some overlap with areas unique to each. Our review may help guide those seeking informatics education and potential certification. As enterprise imaging evolves, these differences may become more important and create knowledge gaps, if not systematically evaluated. Continuing Board Certification: Seeing Our Way Forward

Abstract:Background: Traditionally the role of certifying boards has been to hold physicians accountable for demonstrating standards of competence. In recent years, the authority of continuing board certification has been challenged, due to multiple factors that have shifted the dynamics. The breadth and depth of new information, combined with the pressures of system barriers and administrative burdens, can make it challenging for clinicians stay current and maintain their own competency. Absent feedback about their performance, physicians presume they're practicing effectively. The resulting gap between confidence and competence can also lead physicians to make errors of which they may be unaware. In this environment, assessment and accountability are more important than ever. Four key areas: The authors present four key areas to address to move forward with a board certification system that is effective, relevant, and respected. First, boards should set and communicate the specific expectations of specialists. Second, boards should use technology to create practice-relevant assessments. Third, they should collaborate with educators, while maintaining their distinct role as assessors. Fourth, boards need to establish and meet standards for professionalism and ethics that reflect their position as regulatory bodies. Conclusion: Boards have a critical role in professional self-regulation. They should not compromise on their primary responsibility to set and evolve standards for competence and to conduct rigorous assessments of physicians. The methods boards use for assessments should evolve to meet the changing needs of physicians. Collaboration between educators and assessors provides more educational choice, relieves burdens, and supports physicians' commitment to lifelong learning. By working together with physicians, educators and assessors advance their shared goal of supporting physicians to work at the top of their capability and ultimately, optimize patient care.
The Need for Expanded Educational Opportunities in Clinical Informatics for Pediatric Trainees

Abstract:Clinical informaticians use their skills to design, implement, and evaluate clinical information systems and tools.[1] Whether the initiative is transformational or incremental, focus on improving health information systems and tools has been shown to improve patient safety and clinical effectiveness; we expect health information technology (IT) will continue to play a vital role in improving patient safety and quality improvement in the future.[2] [3] [4] [5] [6] [7] [8] As with many things in pediatrics, there are special requirements for pediatric health information systems compared with those used in adult medicine.[9] As recently as 2015, the majority of pediatric providers used an electronic health record (EHR) that lacked important pediatric-specific capabilities, such as: (1) immunization management including immunization decision support, (2) growth tracking including specialized growth curves utilized in neonatology and specialty clinics such as those that care for children with Down syndrome and Noonan syndrome as an example, (3) medication dosing including clinical decision support to assist with dosing by weight and age, (4) laboratory and vital sign interpretation which changes with patient age, and (5) privacy and confidentiality concerns.[10] [11] Privacy and confidentiality considerations for adolescent and young adult patients have become increasingly complex with the uptake of patient portals, the need for institutions to navigate proxy access, and the release of sensitive information.[12] [13] [14] [15] [16] [17] While many of the large commercial EHR vendors have products that address these issues, often the vendor-provided content needs extensive modifications and customization to fit local culture and regulation requiring the expertise of a pediatric clinical informaticist.[18] The absence of these pediatric-specific capabilities may also contribute to physician burnout and frustration.[19] A robust community of pediatric-focused informaticians is required to support the material complexity associated with pediatric-specific EHR management.[9] [20] [21] However, evidence suggests that there are not enough pediatricians equipped to drive the informatics work required to deliver optimal patient care.[22] [23] As of 2018, only 320 of 1,851 total clinical informaticians are board-certified pediatricians.[22] [23] This number may underestimate the total number of pediatricians practicing as clinical informaticians as some providers may not have opted to become board-certified or may list an alternative specialty in the database. The number of pediatric informaticists is unlikely to increase in the coming years as only 31 individuals who completed clinical informaticist fellowships between 2016 and 2024 listed pediatrics as their primary specialty.[22] This concern is amplified when considering the geographic distribution of clinical informaticians. Eighteen states had 10 or fewer clinical informaticians and 33 states had 2 or fewer practicing informaticians per 1,000 active physicians.[23] Given that pediatricians make up less than 20% of clinical informaticians overall, it is a reasonable assumption that at least several states in the United States do not have a single pediatrician who is board-certified in Clinical Informatics (CI). Abstract:Background: The Vanderbilt Clinical Informatics Center (VCLIC) is based in the Department of Biomedical Informatics (DBMI) and operates across Vanderbilt University Medical Center (VUMC) and Vanderbilt University (VU) with a goal of enabling and supporting clinical informatics research and practice. VCLIC supports several types of applied clinical informatics teaching, including teaching of students in courses, professional education for staff and faculty throughout VUMC, and workshops and conferences that are open to the public. Objectives: In this paper, we provide a detailed accounting of our center and institution’s methods of educating and training faculty, staff, students, and trainees from across the academic institution and health system on clinical informatics topics, including formal training programs and informal applied learning sessions. Methods: Through a host of informal learning events, such as workshops, seminars, conference-style events, bite-size instructive videos, and hackathons, as well as several formal education programs, such as the Clinical Informatics Graduate Course, Master’s in Applied Clinical Informatics, Medical Student Integrated Science Course, Graduate Medical Education Elective, and Fellowship in Clinical Informatics, VCLIC and VUMC provide opportunities for faculty, students, trainees, and even staff to engage with Clinical Informatics topics and learn related skills. Results: The described programs have trained hundreds of participants from across the academic and clinical enterprises. Of the VCLIC-held events, the majority of attendees indicated through surveys that they were satisfied, with the average satisfaction score being 4.63/5, and all events averaging a satisfaction score of greater than 4. Across the 20 events VCLIC has held, our largest audiences are DBMI, HealthIT operational staff, and students from the medical and nursing schools. Conclusions: VCLIC has created and delivered a successful suite of formal and informal educational events and programs to disseminate clinical informatics knowledge and skills to learners across the academic institution and healthcare system.

Engaging the next generation of physician-informaticians through early exposure to the field: successes and challenges associated with starting a novel clinical informatics interest group

Abstract:Clinical informatics remains underappreciated among medical students in part due to a lack of integration into undergraduate medical education (UME). New developments in the study and practice of medicine are traditionally introduced via formal integration into undergraduate medical curricula. While this path has certain advantages, curricular changes are slow and may fail to showcase the breadth of clinical informatics activities. Less formal and more flexible approaches can circumvent these drawbacks. Interest groups (IGs), which are organized through the Association of American Medical College Careers in Medicine (CiM) program, exemplify the informal approach. CiM IGs are student-led groups that provide exposure to different specialty options, acting as an adjunct to the traditional medical curriculum. While the primary purpose of these groups is to assist students applying to residency programs, we took a novel approach of using an IG to increase student exposure to an area of medicine that had not yet been formally integrated at our institution. IGs provide unique advantages to formal integration into a curriculum as they can be more easily setup and can quickly respond to student interests. Furthermore, IGs can act synergistically with UME, acting as proving grounds for ideas that can lead to new courses. We believe that the lessons and takeaways from our experience can act as a guide for those interested in starting similar organizations at their own schools.
Training digital natives to transform healthcare: a 5-tiered approach for integrating clinical informatics into undergraduate medical education

Abstract:Abstract Expansive growth in the use of health information technology (HIT) has dramatically altered medicine without translating to fully realized improvements in healthcare delivery. Bridging this divide will require healthcare professionals with all levels of expertise in clinical informatics. However, due to scarce opportunities for exposure and training in informatics, medical students remain an underdeveloped source of potential informaticists. To address this gap, our institution developed and implemented a 5-tiered clinical informatics curriculum at the undergraduate medical education level: (1) a practical orientation to HIT for rising clerkship students; (2) an elective for junior students; (3) an elective for senior students; (4) a longitudinal area of concentration; and (5) a yearlong predoctoral fellowship in operational informatics at the health system level. Most students found these offerings valuable for their training and professional development. We share lessons and recommendations for medical schools and health systems looking to implement similar opportunities.

Clinical informatics training in medical school education curricula: a scoping review

Abstract:Abstract Objectives This scoping review evaluates the existing literature on clinical informatics (CI) training in medical schools. It aims to determine the essential components of a CI curriculum in medical schools, identify methods to evaluate the effectiveness of a CI-focused education, and understand its delivery modes. Materials and Methods This review was informed by the methodological guidance of the Joanna Briggs Institute. Three electronic databases including PubMed, Scopus, and Web of Science were searched for articles discussing CI between January 2010 and December 2021. Results Fifty-nine out of 3055 articles were included in our final analysis. Components of CI education include its utilization in clinical practice, ethical implications, key CI-related concepts, and digital health. Evaluation of educational effectiveness entails external evaluation by organizations external to the teaching institute, and internal evaluation from within the teaching institute. Finally, modes of delivery include various pedagogical strategies and teaching CI using a multidisciplinary approach. Discussion Given the broad discussion on the required competencies, we propose 4 recommendations in CI delivery. These include situating CI curriculum within specific contexts, developing evidence-based guidelines for a robust CI education, developing validated assessment techniques to evaluate curriculum effectiveness, and equipping educators with relevant CI training. Conclusion The literature reveals that CI training in the core curricula will complement if not enhance clinical skills, reiterating the need to equip students with relevant CI competencies. Furthermore, future research needs to comprehensively address current gaps in CI training in different contexts, evaluation methodologies, and delivery modes to facilitate structured training.

A Standard Approach to Project-Based Learning in a Clinical Informatics Fellowship

Abstract:Background The Accreditation Council for Graduate Medical Education suggests that Clinical Informatics (CI) fellowship programs foster broad skills, which include collaboration and project management. However, they do not dictate how to best accomplish these learning objectives. Objectives This study aimed to describe a standard approach to project-based learning for CI, to share its implementation, and to discuss lessons learned. Methods We created a standard approach to project-based learning based on concepts from adult learning theory, the project life cycle framework, the Toyota Production System, and Improvement Science. Results With this standard approach in place, we learned how best to support fellows in its use. In addition to this approach to supporting needs assessment, risk/change management, implementation, and evaluation/improvement skills, we found the need to develop fellow skills in collaboration, leadership, and time management/managing up. Supported by project-based learning using this standard approach, and with targeted project selection to meet topic-based learning objectives, fellows reached the ability to practice independently in 15 to 21 months. Discussion Fellows are uniquely positioned to ensure the success of projects due to their increased availability and protected time compared with attendings. They are readily available for project teams to draw upon their expertise with clinical workflows and understanding of technological solutions. Project-based learning addressing organizational priorities complements fellow project management coursework and improves fellows' ability to function successfully in large, complex, and dynamic organizations. Exposing fellows to contemporary problems, then addressing them through projects, provides fellows with up-to-date applied informatics knowledge. Conclusion Project-based learning can ensure that many general CI learning objectives are supported inherently. It reinforces project management teachings, while providing fellows with a marketable project portfolio to aid with future job applications. Having projects tightly aligned with organizational priorities supports ongoing investment in fellowship programs. This manuscript does not involve Human or Animal Subjects research. Abstract:Abstract Objective: To assess and refine competencies for the clinical research data management profession. Materials and Methods: Based on prior work developing and maintaining a practice standard and professional certification exam, a survey was administered to a captive group of clinical research data managers to assess professional competencies, types of data managed, types of studies supported, and necessary foundational knowledge. Results: Respondents confirmed a set of 91 professional competencies. As expected, differences were seen in job tasks between early- to mid-career and mid- to late-career practitioners. Respondents indicated growing variability in types of studies for which they managed data and types of data managed. Discussion: Respondents adapted favorably to the separate articulation of professional competencies vs foundational knowledge. The increases in the types of data managed and variety of research settings in which data are managed indicate a need for formal education in principles and methods that can be applied to different research contexts (ie, formal degree programs supporting the profession), and stronger links with the informatics scientific discipline, clinical research informatics in particular. Conclusion: The results document the scope of the profession and will serve as a foundation for the next revision of the Certified Clinical Data ManagerTM exam. A clear articulation of professional competencies and necessary foundational knowledge could inform the content of graduate degree programs or tracks in areas such as clinical research informatics that will develop the current and future clinical research data management workforce.

Abstract:ObjectiveThis article reviews the current status and recent trends in the American Board of Psychiatry and Neurology (ABPN) psychiatric subspecialties and discusses the implications of those trends as well as several key questions whose answers may well determine subspecialty viability.MethodsData are presented on specialty and subspecialty programs; graduates; and ABPN certification candidates and diplomates drawn from several sources, including the records of the ABPN, the websites of the Accreditation Council for Graduate Medical Education and the American Medical Association, and the annual medical education issues of JAMA.ResultsFewer than half of psychiatry graduates pursue sub-specialty training. While most recent specialty graduates attempt to become certified by the ABPN, many subspecialists elect not to do so. There have been recent decreases in the number of fellowship programs and trainees in geriatric psychiatry and addiction psychiatry. The pass rates for fellowship graduates are superior to those for the “grandfathers” in all of the newer psychiatric subspecialties. Lower percentages of subspecialists than specialists participate in maintenance of certification, and maintenance of certification pass rates are high.ConclusionThe initial interest in training and certification in some of the ABPN subspecialties appears to have slowed, and the long-term viability of those subspecialties may well depend on the answers to a number of complicated social, economic, and political questions in the new health care era.

The Value of Board Recertification Among Physicians

Abstract:An ongoing challenge among healthcare certifying organizations is identifying an effective manner of evaluating a physician's competency. The medical field is constantly changing, with new technology, research and pharmacology available, and physicians must be kept up to date in order to properly care for their patients. Maintenance of certification and specifically, recertification exams, are used to verify that a high standard of care is consistently met across all medical specialties. However, different countries have different structures in place to ensure physicians are kept abreast of the latest medical knowledge. For instance, American physicians must recertify themselves every ten years by passing a standardized exam to maintain their credentials. In comparison, Canadian physicians are not required to pass a formal examination. This contrast puts into question the value of the recertification exam in medical professional development programs. We outline the rectification situation in both Canada and the US and recommend MOC programs similar to Canada's, which does not require a recertification examination.

Abstract:Strengthening the field of imaging informatics by further defining standards and advocating for continuous education are the cornerstones of the American Board of Imaging Informatics (ABII). ABII is the non-profit organization that governs the Imaging Informatics Professional certification program. ABII is responsible for awarding the Certified Imaging Informatics Professional (CIIP) designation to candidates who meet specified educational and experience-based criteria and pass a qualifying exam (1). For this paper, we analyzed Quality Improvement (QI) projects submitted to ABII for satisfaction of the 10-year requirements in 2017–2021. The project reports demonstrated a variety of interventions undertaken to ultimately improve patient care. A retrospective review of these reports exemplifies the critical role the Certified Imaging Informatics Professionals have in delivery of high quality, safe healthcare and their vital contributions to the healthcare industry and practice of medicine.

Applied Clinical Informatics

Abstract:Background Leaders in Informatics, Quality, and Systems (LInQS) is a non-ACGME (Accreditation Council for Graduate Medical Education)-accredited 2-year training program developed to enhance training in the fields of health care delivery, quality improvement (QI), clinical informatics, and leadership. Methods This single-institution 2-year longitudinal training program grounded in QI and informed by leadership and clinical informatics includes didactics, coaching, and mentorship, all centered around individualized QI projects. The program has been available to sub-specialty fellows, advanced practice providers, and physicians. Results From 2019 to 2023, 32 fellows have been accepted into the program with 13 graduates and 16 currently enrolled. Fellows have been predominately female, physicians, and from multiple specialties but predominantly hospital medicine. Fellows' evaluations of the fellowship are highly positive, rating the didactics and mentorship aspects of the curriculum most favorably. Most fellows' projects utilized informatics solutions including clinical decision support tools to increase quality of care, improve patient outcomes, and reduce costs of care resulting in manuscript publications, national presentations, and a national specialty society award. Since matriculation, 50% of fellows received certification as Epic Physician Builders and 34% received leadership positions in clinical informatics, quality, and education. Conclusion Our experience supports the need to provide health care providers more expansive training in the areas of QI, clinical informatics, and leadership for improving health care delivery. Additional in-depth knowledge and experience in these fields may produce and benefit leaders in these fields. The program evaluation meets the criteria for nonhuman subjects research designation by the Colorado Multiple Institutional Review Board (COMIRB). Abstract:Health Informatics Journal, Volume 30, Issue 4, October-December 2024. Background: Healthcare Informatics leaders are the bridge between the information systems and clinical departments. The role has grown to where Health Informatics leaders analyze, design, develop, implement, and evaluate information and communication systems that enhance individual and population health outcomes, improve patient care, and strengthen the clinician-patient relationship. Their role spans across administration, information technology into the clinical realm. Objective: The primary objective of this study was to identify the past, current roles, and responsibilities of Health Informatics leaders while the secondary outcome was to identify the future trends in the responsibilities and roles. Methods: A thirteen-question survey was emailed to recipients through listservs. Results: Respondents cited their roles have evolve in respect to EMR to improve patient outcome and efficiency, overseeing compliance with federal and state regulations, design and customization of information technology systems, quality improvement management, healthcare data analytics (machine learning and artificial intelligence), population health management. Conclusion: The role of CMIO and healthcare informatics leaders have begun to be distinctly delineated, and their positions have had a tremendous impact on the integration of health information technology in healthcare. The role of the CMIO will continue to evolve as technology changes.

Predictive validity of certification by the American Board of Internal Medicine

Abstract:Study objective: To determine the predictive validity of the American Board of Internal Medicine (ABIM) certification process. Design: Prospective measurement of the knowledge, skills, and attitudes of 185 ABIM-certified and 74 noncertified internists by a written examination; evaluation by professional associates; a patient questionnaire assessing satisfaction with care, physician's counseling role, and preventive care; and review of records of patients with common illnesses. Subjects: Practicing internists who completed training or received ABIM certification 5 to 10 years previously. Setting: Office-based practices in six western states. Results of data analysis: Physicians certified by the ABIM had significantly higher scores on the written examination than the noncertified physicians, and scores on our examination correlated highly with the ABIM certification examination (r = 0.73). Ratings of clinical skills by professional associates were significantly higher for certified internists and also correlated highly with ABIM examination scores (r = 0.53 to 0.59). Regression analysis showed that ABIM certification status was the major variable affecting performance on these measures of clinical competence. Results from other measures did not show many differences between certified and noncertified physicians in the care of patients with common illnesses, but modest differences in preventive care and a few differences in outcome favored the certified physicians. Conclusions: Comparison of findings from the written examination and the professional associate ratings with certification status and original ABIM certification examination scores shows predictive validity of ABIM certification. Further studies are needed to determine if certification status predicts important differences in the care of patients with complex illnesses.
