Abstract:As a typical multiple criteria comprehensive evaluation (MCCE) problem, healthcare insurance audits (HIA) play a vital role in governmental audits. To handle the HIA in an autonomous region in China, a systematic methodology in the framework of MCCE is presented. An evaluation system is constructed at first based on the specific auditing objective. The group of individual preferences is collected by a probabilistic linguistic preference relation (PLPR). Its consistency and prioritization are handled based on the idea of stochastic analysis and the widely acknowledged techniques of linguistic preference relations (LPRs). Specifically, the consistency degree is measured by the probability of stochastically generated LPRs being with acceptable consistency, and then improved by revising the probabilistic distributions involved in the PLPR. The priority is derived by the expected value of priorities of the stochastically generated LPRs. Moreover, the comprehensive evaluation is conducted based on aspiration-based utility functions. The proposed methodology enables decision makers to determine parameters intuitively, and presents interpretable and admissible results. It is effective even if the original individual preferences are with low consistency degree. Finally, the focused HIA problem is solved based on the proposed methodology.

Abstract:As an important major public health emergency, COVID-19 broke out more than two years. At present, China has entered the post-epidemic era. However, it is still necessary to study the medical health resource allocation in public health emergencies. Therefore, the evaluation of medical health resources allocation is important. Firstly, we use two kinds of linguistic preference orderings (LPOs) to represent experts' opinions when evaluating the medical health resources allocation in public health emergencies. Then, a novel ORESTE method with LPOs is developed to solve multiple criteria decision-making (MCDM) problems. Additionally, we apply the proposed ORESTE method to solve a practical MCDM problem involving the medical health resources allocation in public health emergencies. Finally, some comparative analyses among the proposed ORESTE method and some existing methods under a double hierarchy linguistic environment are set up, and some discussions are summarized to show the validity and applicability of the proposed novel ORESTE method.

Abstract:Many critical or even life-saving healthcare resources such as cadaveric donor organs are scarce. Upon procurement of such resources, some priority rule is applied to make the allocation decisions. In this paper, we consider the problem of optimally designing a single-score based priority rule to rank patients for each unit of available resource in real-time. We address the cases where multiple potentially conflicting objectives are simultaneously considered and the optimality principles on these objectives are in the expectation sense. We thus propose a multiobjective stochastic genetic algorithm approach to obtain Pareto-optimal policies, i.e., determining the weights placed on different prioritization criteria. To accommodate the stochastic nature, we adapt a ranking-and-selection procedure to construct an elite chromosome set in each generation of the genetic algorithm, and use the elite chromosome set to improve the offspring generation. To ensure sufficient diversity in the population, we apply clustering to identify representative elite chromosomes. We use cadaveric liver allocation policy optimization as a proof-of-the-concept study, for which we consider both pre-transplant and post-transplant survival rates as the objectives. We incorporate a self-developed discrete-event simulation model into our optimization algorithm framework. To tune the algorithm parameters efficiently, we use a response-surface-based surrogate model to evaluate each candidate solution. We then obtain a set of Pareto-optimal solutions based on the simulation evaluations. Our study justifies the viability of the approach, i.e., efficiently developing good single-score based priority rules with respect to multiple system outcomes.
Identification and evaluation of potential anti-cancer drugs on human neuroendocrine tumor cell lines.

Abstract:UNLABELLED The aim of this study was to investigate drug sensitivity in neuroendocrine tumor cell lines. MATERIALS AND METHODS In vitro drug sensitivity screening was performed using the fluorometric microculture cytotoxicity assay in one human pancreatic carcinoid and two human bronchial carcinoid cell lines. In addition, a normal human retinal pigment epithelial cell line was used for comparison. A total of 18 drugs with different mechanisms of action were tested. RESULTS The most active agents were brefeldin A, emetine, bortezomib and idarubicin, having IC5o values <1 microM in all four cell lines. In addition, the three tumor cell lines showed sensitivity for sanguinarine, Bayll-7085, mitoxantrone, doxorubicin, beta-lapachone, NSC 95397 and CGP-74514A. CONCLUSION The cell lines were sensitive for several drugs acting in different ways, covering a broad spectrum of mechanisms of action. Some of these compounds may possibly be used in clinical trials and show therapeutic effect in patients with neuroendocrine tumors.
Application of fuzzy TOPSIS for prioritization of patients on elective surgeries waiting list - A novel multi-criteria decision-making approach

Abstract:Prioritizing patients is a growing concern in healthcare. Once resources are limited, prioritization is considered an effective and viable solution in provision of healthcare treatment to awaiting patients. Prioritization is a preferred approach that helps clinicians to apportion scarce resources fairly and transparently. In this study, a novel methodology of prioritizing the patient is formulated using fuzzy Technique for Order of Preference by Similarity to Ideal Solution (TOPSIS). The objective is based on actual hospital conditions in Pakistan. The proposed methodology has two contributions: objective scoring mechanism that translates the patient’s condition given in human linguistic terms; and second methodology to prioritize patients according to corresponding scores. To validate the proposed methodology, simulation was carried out on actual data collected in real-time by surgeons, while providing consultations to their patients. The proposed methodology outperforms the traditional methodology by reducing average waiting time by 34% (from 4.246 to 2.810 days), minimize wait time and delays by 46.7% (from 15 to 8 days), and number of surgery days by 18%. The majority of the previously presented researched methodologies prioritize the patients subjectively. This study presents an objective methodology to prioritize the patients and decrease wait-times while ensuring transparency and equity.

Abstract:Walk-in patients can take a large portion of outpatient visits in comprehensive hospitals. In current common practice, an even number of appointments is assigned to each slot and patients with appointment are given a strictly higher priority than walk-in patients. As a result, walk-in patients usually wait longer and incur noticeable dissatisfaction. To improve their satisfaction, we propose an integrated optimization approach for patient-centred outpatient scheduling. This approach aims to allocate appointments in line with the temporal variation of patient walk-ins, and further develop a time-invariant threshold-based prioritization scheme to balance the service priorities between the two types of patients in real time. To substantiate this approach, we develop a prospect theory-based model to quantify each patient's real-time satisfaction. We apply discrete-event simulation-based optimization to maximize the average patient satisfaction over the two types of patients. To verify our integrated optimization approach, we conduct a pilot in the endocrine department of Shanghai Sixth People's Hospital. Our results suggest that our approach substantially decreases the waiting time of walk-in patients without much compromise on the satisfaction of patients who made advance appointment.

Abstract:The hierarchical medical treatment system is an efficient way to solve the problem of insufficient and unbalanced medical resources in China. Essentially, classifying the different degrees of diseases according to the doctor's diagnosis is a key step in pushing forward the hierarchical medical treatment system. This paper proposes a framework to solve the problem where diagnosis values are given as picture fuzzy numbers (PFNs). Point operators can reduce the uncertainty of doctor's diagnosis and get intensive information in the process of decision making, and the Choquet integral operator can consider correlations among symptoms. In order to take full advantage of these two kinds of operators, in this paper, we firstly define some point operators under the picture fuzzy environment, and further propose a new class of picture fuzzy point⁻Choquet integral aggregation operators. Moreover, some desirable properties of these operators are also investigated in detail. Then, a novel approach based on these operators for multiattribute decision-making problems in the picture fuzzy context is introduced. Finally, we give an example to illustrate the applicability of the new approach in assisting hierarchical medical treatment system. This is of great significance for integrating the medical resources of the whole society and improving the service efficiency of the medical service system.

Abstract:Picture fuzzy numbers (PFNs) with three degrees of memberships can be used to accurately describe the uncertainty of cognitive information. However, picture fuzzy multi-criteria decision-making (MCDM) methods need to be further studied. This paper describes an extended picture fuzzy multi-objective optimization by ratio analysis and a full multiplicative form (MULTIMOORA) method based on the prospect theory (PT) to handle MCDM. By adopting this process, decision-makers (DMs) can provide fuzzy linguistic terms to evaluate relevant criteria. The evaluation information can be transformed into PFNs based on transformation scales. Then, the corresponding weights of criteria can be calculated according to picture fuzzy entropy. Moreover, the PT, which is considered an important tool for describing the psychological cognition of DMs, can be used to obtain a prospect decision matrix. Here, the MULTIMOORA method, which involves the simultaneous application of the picture fuzzy ratio system, the picture fuzzy reference point, and the picture fuzzy multiplicative form methods, was utilized to determine the final rankings of candidate alternatives. We hence propose an extended picture fuzzy MULTIMOORA method based on the PT, the MULTIMOORA method, and picture fuzzy Dice distance measures, which can be applied to MCDM problems where weight information is completely unknown. The feasibility and validity of the proposed method were verified by applying it to medical institution selection. Sensitivity and comparative analyses demonstrated the superiority of this method compared to the existing ones.

A Novel Multi-Attribute Group Decision-Making Method and Its Application in Solving the Downward Referral Problem in the Hierarchical Medical Treatment System in China

Abstract:China has made many achievements in health care industry since the establishment of hierarchical medical treatment system, but also exposed a lot of problems, such as the problem of “easy to upward-referral, hard to downward-referral”, which occurred in the implementation process of two-way referral. Recently, the appearance of “hard to downward-referral” have gotten more and more attention from scholars. The key step in solving the problem of “downward-referral” is to classify hospitals at different levels evaluated by the professional doctors. This paper proposes a framework to solve the problem where evaluation values of the hospital are given as q-rung orthopair fuzzy uncertain linguistic variables (q-ROFULVs). Considering Schweizer-Sklar t-conorm and t-norm (SSTT) can make the information aggregation process more flexible, and Hamy Mean operator can consider the correlation between hospital indicators. In order to take full advantage of these two kinds of operators, firstly, we extend SSTT to q-ROFULVs and define Schweizer-Sklar operational rules of q-ROFULVs. Secondly, we combine the Hamy Mean operator with Schweizer-Sklar t-conorm and t-norm, and propose the q-rung orthopair fuzzy uncertain linguistic Schweizer-Sklar Hamy mean aggregation operators and q-Rung orthopair fuzzy uncertain linguistic Schweizer-Sklar weighted Hamy Mean respectively. Afterwards, q-rung orthopair fuzzy uncertain linguistic Schweizer-Sklar dual Hamy mean operator and its weighted form are also developed. Then, we have studied in detail some of the ideal properties of these operators. Furthermore, a novel multi-attribute group decision-making approach based on proposed operators is introduced. Finally, we apply the new approach to solve the problem about the downward conversion in the hierarchical medical treatment system. This is of great significance for integrating the medical resources of the whole society and improving the service efficiency of the medical service system.

Abstract:Since makeshift hospitals have strong ability in blocking the spread of the virus, how to design some methods to select the reasonable sites of makeshift hospitals is vitally important for containing COVID-19. This paper investigates an efficiency-based multi-criteria group decision making (MCGDM) method by combining the best-worst method (BWM) and data envelopment analysis (DEA) in trapezoidal interval type-2 fuzzy (TrIT2F) environment. This MCGDM method is called TrIT2F-BWM-DEA, where the TrIT2F-BWM is used to determine the weights of criteria and decision-makers, and the TrIT2F-DEA is employed to rank alternatives by measuring their overall efficiencies. Based on cut set theory, the expectation and average expectation (AE) of TrIT2FSs are successively defined. To solve three key issues in the development of the TrIT2F-BWM, this paper proposes a flexible ranking relation of TrIT2FSs to transform the TrIT2F constraints, initiates an efficient theorem to normalize the TrIT2F weights, and designs an input-based consistency ratio to check the reliability of the determined weights. A fully TrIT2F-DEA model is originally built to measure the TrIT2F efficiencies of alternatives. The alternatives are finally ranked according to the AEs of alternatives' TrIT2F efficiencies. A site selection case of Fangcang hospitals and some comparative analyses are provided to confirm the validity and merits of the proposed TrIT2F-BWM-DEA.

Doctors ranking through heterogeneous information: The new score functions considering patients' emotional intensity

Abstract:With the popularity of the Internet and the growing complexity of COVID-19, more and more patients tend to consult doctors online. With the difficulty of doctor selection caused by a massive amount of information, this study proposes a hybrid multi-criteria decision-making framework, which can model patients' emotional intensity through heterogeneous information and rank doctors. Firstly, online reviews (ORs) are transformed into probabilistic linguistic term sets through sentiment analysis. Then, new score functions are proposed considering the nonlinear influence of doctors' information and the patients' negative bias toward ORs. Next, a method of weight determination combining the Term Frequency Inverse Document Frequency and the Decision-making Trial and Evaluation Laboratory method is proposed. Finally, the proposed score functions are applied to the Combined Compromise Solution (CoCoSo) method to aggregate information and rank doctors. The proposed method is verified in a case study on haodf.com. The results show that considering the emotional intensity of heterogeneous information will make the recommendations more realistic. Comparative analysis and sensitivity analysis are further performed to illustrate the availability and effectiveness of the proposed method.

Abstract:The imbalanced development among different levels of healthcare facilities has become a major social issue in China's urban healthcare system, which has raised the irrational patient flow distribution on the levels of both intra-hospital and inter-healthcare facilities. In this research, we develop a methodology to find the optimal macrolevel patient flow distribution in terms of multidimension inputs and outputs for the two-level healthcare system. The proposed method integrates the discrete-event simulation (DES), the multiobjective optimization and the simulation budget allocation together to comprehensively improve the overall system performances by finding the approximate Pareto patient flow distribution in the hierarchical healthcare system. The multiobjective optimal computing budget allocation (MOCBA) is applied to improve the efficiency, where the nondominated probability is functioned as the fitness measurement to each design. A case study based on the real data is carried out to validate and implement the proposed method. The results of the case study show the recommended Pareto optimal patient flow distribution can improve the overall hierarchical system performances and our methodology are qualified as a quantitative decision tool for decision makers.
Multicriteria Optimization Techniques for Understanding the Case Mix Landscape of a Hospital

Abstract:Various medical and surgical units operate in a typical hospital and to treat their patients these units compete for infrastructure like operating rooms (OR) and ward beds. How that competition is regulated affects the capacity and output of a hospital. This article considers the impact of treating different patient case mix (PCM) in a hospital. As each case mix has an economic consequence and a unique profile of hospital resource usage, this consideration is important. To better understand the case mix landscape and to identify those which are optimal from a capacity utilisation perspective, an improved multicriteria optimization (MCO) approach is proposed. As there are many patient types in a typical hospital, the task of generating an archive of non-dominated (i.e., Pareto optimal) case mix is computationally challenging. To generate a better archive, an improved parallelised epsilon constraint method (ECM) is introduced. Our parallel random corrective approach is significantly faster than prior methods and is not restricted to evaluating points on a structured uniform mesh. As such we can generate more solutions. The application of KD-Trees is another new contribution. We use them to perform proximity testing and to store the high dimensional Pareto frontier (PF). For generating, viewing, navigating, and querying an archive, the development of a suitable decision support tool (DST) is proposed and demonstrated.

Abstract:The multicriteria group decision-making method in fuzzy environment has become the focus of many scholars. The purpose of this paper is to extend the traditional ORESTE (organisation, rangement et Synthese de donnees relarionnelles) method to solve the multi-criteria group decision-making problem under IT2FS environment. The main contributions of this paper are to define a new similarity measure and a global preference score function based on the new similarity measure. First, a new similarity measure of two IT2FSs is defined. Then, some properties of the new similarity measure are investigated, and a comparison between the new similarity measure and other existing similarity measures is provided. Subsequently, a global preference score function is defined based on the new similarity measure. The average preference score is derived using the power average operator, and the weak order of the alternatives is obtained according to the average preference score. Furthermore, a PIR (P: preference; I: indifference; R: incomparability) structure is constructed to reach a strong ordering of alternatives. Finally, the feasibility and effectiveness of the proposed method is illustrated by a practical case of selecting the best supplier involving comparisons with the ELECTRE-based outranking method and IT2FS-TOPSIS method. And the new approach proposed in this paper can be applied to e-commerce, medical decision making, product recommendation, and fault diagnosis.

Abstract:Ranking key quality characteristics (KQCs) which are widely utilized to evaluate the product quality level and selecting the most important one are necessary for improving the quality of mechanical and electrical product. Many classical ranking methods have been proposed to rank KQCs, e.g., the classical quality function deployment (QFD) method, the classical ORESTE method. However, some drawbacks in constructing ranking framework, rating KQCs/influence factors (IFs) and ranking KQCs are existed in these methods. To solve this problem, we develop a three-dimensional house of quality (HoQ) model-based interval-valued spherical fuzzy-ORESTE (IVSF-ORESTE) method to rank KQCs. First, the three-dimensional HoQ models which add meta-action units (MAUs) dimension are constructed to provide a ranking framework, and interval-valued spherical fuzzy sets (IVSFSs) are utilized to rate KQCs/IFs. Secondly, some new distance and weight distance measures based on the Dice and the Jaccard similarity are proposed to extend the traditional distance measure between IVSFSs. Furthermore, the IVSF-QFD method is used to calculate the importance degrees of IFs and the KQCs are ranked with the IVSF-ORESTE method. Finally, a real case vis-à-vis a NC rotary table is illustrated to explain the implementation process of the proposed method, and discussion results indicate that the proposed ranking method is rational and valid.

Generalized Ordinal Priority Approach for Multi-Attribute Decision-Making under Incomplete Preference Information

Abstract:The Ordinal Priority Approach (OPA) is a multi-attribute decision-making (MADM) method to determine the relative importance (weights) of experts, attributes, and alternatives. This study formally establishes the fundamental properties of OPA, including solution efficiency, analytical solution expression, the decomposability of optimal decision weights, and its relationship with rank-based surrogate weights. Building on these properties, we propose a Generalized Ordinal Priority Approach (GOPA) based on an "estimate-then-optimize" contextual optimization framework for MADM when preference information is incomplete. In the first stage, we derive utility distributions for ranked alternatives in discrete and continuous prospects by minimizing cross-entropy utility under partial preference information, including weak order relations, absolute differences, ratio scales, and lower bounds. Rank-based surrogate weights and risk preference utility functions serve as the global utility structure for discrete and continuous prospects, respectively. The elicited utility information is then introduced into the second-stage problem to simultaneously optimize the weights of experts, attributes, and alternatives within a normalized weight space. Metrics for validating the group decision outcomes of GOPA, including percentage standard deviation, correlation coefficient, and confidence level measurement, are proposed. Theoretical analysis reveals several advantageous properties of GOPA, including model generalizability, analytical solvability, and risk preference independence. Furthermore, this study provides a lower bound reference for transforming the general optimization-based weight elicitation problems into optimization problems with stochastic dominance constraints. The applicability of GOPA is demonstrated through an improvisational emergency supplier selection problem.

Abstract:Owing to the limitations of Pythagorean fuzzy and intuitionistic fuzzy sets, scientists have developed a distinct and successive fuzzy set called the q-rung orthopair fuzzy set (q-ROFS), which eliminates restrictions encountered by decision-makers in multicriteria decision making (MCDM) methods and facilitates the representation of complex uncertain information in real-world circumstances. Given its advantages and flexibility, this study has extended two considerable MCDM methods the fuzzy-weighted zero-inconsistency (FWZIC) method and fuzzy decision by opinion score method (FDOSM) under the fuzzy environment of q-ROFS. The extensions were called q-rung orthopair fuzzy-weighted zero-inconsistency (q-ROFWZIC) method and q-rung orthopair fuzzy decision by opinion score method (q-ROFDOSM). The methodology formulated had two phases. The first phase 'development' presented the sequential steps of each method thoroughly.The q-ROFWZIC method was formulated and used in determining the weights of evaluation criteria and then integrated into the q-ROFDOSM for the prioritisation of alternatives on the basis of the weighted criteria. In the second phase, a case study regarding the MCDM problem of coronavirus disease 2019 (COVID-19) vaccine distribution was performed. The purpose was to provide fair allocation of COVID-19 vaccine doses. A decision matrix based on an intersection of 'recipients list' and 'COVID-19 distribution criteria' was adopted. The proposed methods were evaluated according to systematic ranking assessment and sensitivity analysis, which revealed that the ranking was subject to a systematic ranking that is supported by high correlation results over different scenarios with variations in the weights of criteria.

Abstract:Traditional Chinese medicine (TCM) has gradually played an indispensable role in people's health maintenance, especially in the treatment of chronic diseases. However, there is always uncertainty and hesitation in the judgment and understanding of diseases by doctors, which affects the status recognition and optimal diagnosis and treatment decision-making of patients. In order to overcome the above problems, we lead into probabilistic double hierarchy linguistic term set (PDHLTS) to accurately describe language information in traditional Chinese medicine and make decisions. In this paper, a multi-criteria group decision making (MCGDM) model is constructed based on the MSM-MCBAC (Maclaurin symmetric mean-MultiCriteria Border Approximation area Comparison) method in the PDHL environment. Firstly, a PDHL weighted Maclaurin symmetric mean (PDHLWMSM) operator is proposed to aggregate the evaluation matrices of multiple experts. Then, combined with the BWM and maximizing deviation method, a comprehensive weight determination method is put forward to calculate the weights of criteria. Furthermore, we propose PDHL MSM-MCBAC method based on the Multi-Attributive Border Approximation area Comparison (MABAC) method and the PDHLWMSM operator. Finally, an example of a selection of TCM prescriptions is used and some comparative analyses are made to verify the effectiveness and superiority of this paper.

Abstract:With the number of patients with chronic diseases increasing, a hierarchical diagnosis and treatment system is a feature of long-term treatment. During the treatment, if patients with chronic diseases are relieved, they can be referred to community hospitals for rehabilitation treatment, and to promote the rational allocation of hospital medical resources. Therefore, hospitals need referral standards to encourage patients to refer down. Existing ranking research which is mainly based on referral criteria,has not considered the low reliability due to different opinions within each group when making referral standard. In view of this, we propose a probabilistic linguistic-multiple criteria decision-making (PL-MCDM) method based on the decision-making reliability of multi-group for patients with chronic diseases requiring downward referral. First, the decision information of different groups on referral criteria is expressed as probabilistic linguistic preferences, and then the reliability levels of groups are measured when setting standards respectively. After that, the decision information and referral standards are re-adjusted for the least reliable group to improve the whole reliability. Secondly, the patients requiring downward referral are ranked by the similarity measure based on statistics of probability preference to acquire the formal referral standards. Finally, compared with the methods without reliability improvement, the real case of coronary heart disease in First Hospital of Qinhuangdao is used to verify the effectiveness of the proposed method, which illustrates its superiority.

Abstract:Surgical rescheduling is necessary for adjusting initial schedules on surgery day after emergency demand is realized. While people-oriented medical service has been emphasized these years, the traditional rescheduling scheme which is only in pursuit of a great cost-related performance is no longer desirable since patients and medical staff are also highly involved in rescheduling and their preferences should not be ignored. In order to provide a satisfactory and people-centered rescheduling plan, this study considers the preferences of three involved participants (i.e. the operating room manager, medical staff, and elective patients) while designing a rescheduling plan. Based on prospect theory, we introduce three functions to evaluate three participants' dissatisfaction about rescheduling schemes in terms of their respective preferences. Then a multi-objective rescheduling model is established with multiple resource constraints, emergency lead-time target constraints, and the objective of minimizing the dissatisfaction of three participants caused by rescheduling. A hybrid particle swarm optimization (HPSO) algorithm with two improved strategies—an initial population construction strategy and a local search strategy, is then developed to solve the proposed problem. Several numerical experiments are carried out by leveraging data reported in existing case studies in conjunction with simulated data. The results demonstrate the effectiveness of two improved strategies and show that the proposed HPSO algorithm can provide better Pareto solutions for our problem in comparison with the basic non-dominated sorting genetic algorithm.
