Abstract:Background: Worksite-based nutrition interventions can serve as access points to facilitate healthy eating and translate existing knowledge of cardiometabolic disease prevention. We explored perceptions, facilitators, and barriers for healthy eating in a cafeteria at a large worksite in Mexico City. Methods: We conducted an exploratory qualitative study in a large department store in Mexico City with ~ 1500 employees. We conducted eight focus group discussions (FGD) with 63 employees stratified by job category (sales, maintenance, shipping, restaurant, cafeteria, administrative staff, and sales managers). Employees were invited to participate in the FGD if they were at the store at the day and time of the FGD for their job type. FGDs were audio-recorded, transcribed verbatim and analyzed using the thematic method. This process involved the researches´ familiarizing themselves with the data, generating initial codes, searching for themes, reviewing the themes, defining and naming themes, and then interpreting the data. Results: Employees defined healthy eating as eating foods that are fresh, diverse, and prepared hygienically. The most commonly reported facilitators of healthy eating at the worksite were availability of affordable healthy food options and employees' high health awareness. Major barriers to healthy eating included unavailability of healthy foods, unpleasant taste of food, and preference for fatty foods and meat. For lower-wage workers, affordability was a major concern. Other barriers included lack of time to eat work and long working hours. Conclusion: A broad range of factors affect healthy eating at the cafeteria, some related to nutrition and some related to the employees type of job. Availability of healthy, hygienic, and tasty food at an affordable price could lead to healthier food choices in the worksite cafeteria. These strategies, along with work schedules that allow sufficient time for healthy eating, may help improve dietary behaviors and health of employees.
Intervention Effects on Nutrition Knowledge Attitude Behavior among Employees of an Enterprise in Pudong New Area of Shanghai City

Abstract:[Objective]To understand the intervention effects on nutrition knowledge,attitude and behavior(KAP) among employees of a shipbuilding enterprise in Pudong New Area of Shanghai City,and provide evidence for nutritional intervention on employees.[Methods]Totally 843 employees from 2 working groups eating in different canteens were selected and divided into intervention group and control group randomly.Nutrition interventions were conducted on intervention group.After 6 month,intervention effects were evaluated by nutrition surveys.[Results]After nutrition intervention,nutritional KAP of intervention group was significantly improved and was significantly better than control group on most indicators.[Conclusion]Nutrition intervention can improve employees' nutrition knowledge,reinforce their positive attitudes and optimize nutrition practice.
Multiplicity and complexity of food environment in China: full-scale field census of food outlets in a typical district

Abstract:Community food environment is closely associated with residents’ health status, and it has received substantial research attention in recent years. However, such studies are sparse in China. This study aims to reveal the characteristics of food environment in China. A ground-truthing field census of food outlets was conducted in Xi Hu district, Hangzhou. A novel categorizing system was developed to better characterize food outlets in the study area. Geocoding and food environment analysis were performed by Arc-GIS software. A total of 9274 food outlets were identified, of these, 6161 (66.43%) were food service places and 3113 (33.57%) were food stores. Fast-food restaurants (2692) and convenience stores (1764) were the two most numerous outlet types. The density of food outlets was 29.72/km2 and 113.97/10,000 people. Availability of BMI-unhealthy food outlets was four times greater than that of BMI-healthy ones. Further, 41.86% of total food outlets provided food-delivery services; among them, fast-food restaurants were the highest (65.53%) providers. In the urban area, the average density of food outlets was 104/km2. While, it was only 29/km2 in the rural area. Availability of food outlets was excellent in our study area, though it showed regional imbalance. Food environment tends to be obesogenic and high prevalence of food delivery service might increase this trend. Thus, policy makers should guarantee the smooth development of catering industry in China, so as to ensure a balanced nutrition environment.

Abstract:China is confronted with the challenge of increasing childhood obesity. Although interventions for childhood obesity have been developed, their effectiveness and implementation can vary considerably across nations. In this paper, we review and consolidate the evidence on childhood obesity intervention strategies aimed at multiple levels and components across life, in China. School-based interventions, primarily delivered with group-based dietary and physical education, have shown potential efficacy and can be enhanced by integrating food environment reform and family support. However, most interventional studies were done in eastern and urban areas of China, which highlights the need for attention in regions with insufficient health resources where policies and initiatives are less accessible. In addition, the sparse reporting on implementation design and delivery might hinder the scalability of potentially effective strategies. Amid rapid social and economic development over the past few decades, China faces unique challenges on urbanisation, physical inactivity, and nutritional transitions, necessitating an updated policy agenda for primary prevention of childhood obesity. Structural longitudinal evaluations of public initiatives led by the government and organisations along with inclusive and equitable interventions targeting children of different ages, urban–rural regions, and different ethnic and socioeconomic groups are warranted. Tackling childhood obesity in China needs a cohesive approach that integrates social, economic, cultural, and environmental strategies, combining school-based, family-based, and individual-based approaches, and concerted efforts from multiple sectors and entities within China.
Effectiveness of a Worksite-Based Lifestyle Intervention on Employees’ Obesity Control and Prevention in China: A Group Randomized Experimental Study

Abstract:Background: This study was to culturally adapt a lifestyle intervention for employees’ obesity control and prevention using a participatory process, and evaluate the effectiveness of the project at worksites. Methods: A group randomized experimental study included four worksites (two intervention, two control) in the Yangtze River Delta in China was conducted. A total of 388 participants (216 in the intervention worksites and 172 in the control worksites) were finally recruited from 955 employees at the four worksites (464 in the intervention worksites and 491 in the control worksites). The final evaluation was completed by two hundred and seventy-eight employees (159 in the intervention worksites and 119 in the control worksites, respectively). Data of demographic information, weight, BMI, waist circumference, hip circumference and weight-related behaviors including diary behaviors and physical activities were collected before and after a 12-month intervention and analyzed using descriptive statistics, t-test, chi-square test, linear mixed regression and logistic mixed regression. Results: Although the intervention worksites had a reduction in body mass index (23.21 to 22.95, p < 0.01), hip circumference (95.97 to 95.28, p = 0.03) and waist-to-height ratio (0.49 to 0.48, p = 0.01), the differential changes compared to those of the control group were not statistically significant. The frequency of sweet beverages (−1.81, 95%CI: −0.52, −3.11), frequency of vegetable intake (5.66, 95%CI: 1.59, 9.74), daily servings of vegetables (0.53, 95%CI: 0.24, 0.82), frequency of fruit intake (3.68, 95%CI: 1.25, 6.12), daily servings of fruit (0.26, 95%CI: 0.44, 0.92), daily servings of vegetables and fruit (0.79, 95%CI: 0.43, 1.16), daily steps (863.19, 95%CI: 161.42, 1564.97) and self-efficacy to change physical activity (OR = 1.91, 95%CI: 1.02,3.60) were more improved in the intervention group than were those measures in the control group. Conclusions: The worksite-based lifestyle intervention project for obesity control and prevention improved several employees’ dietary behaviors and physical activities at worksites in China in a short time. Long-term intervention with larger samples in more worksites should be further examined.

Abstract:Background: The Chinese government is piloting canteen services for older adults, but few studies have explored the influence of canteen services on the health of these older adults. This study aimed to investigate the impact of canteen services on older adults' general mental health, nutritional status, satisfaction with life, and social capital in rural areas. Methods: This study used a cross-sectional design. We selected 14 villages in Jinhua City, Zhejiang Province, China, including seven villages with canteen services and seven villages without canteen services. Participants were 284 senior older adults (aged 75~98), including 140 residing in villages with canteen services (Canteen Group [CG]) and 144 residing in villages without canteen services (Non-Canteen Group [NCG]). We also divided the CG into two sub-groups according to the funding sources (one receiving government support only, the other receiving government support plus enterprise donations). We used a self-designed questionnaire, including sociodemographics, diet-related items (e.g., satisfaction with the meals, diet expenditure, self-evaluation of meal nutrition), and the four scales including the Chinese version of the 12-item General Health Questionnaire (GHQ-12), the Chinese version of the Mini Nutritional Assessment Short-Form (MNA-SF), the Satisfaction with Life Scale (SWLS), and the Social Capital Questionnaire (SCQ). Results: The overall mental health, satisfaction with life, and social capital of the CG were better than the NCG (P < 0.05). The nutritional status of these two groups did not show a significant difference. Participants in the CG with financial support from local government and donations from an enterprise with a better dietary diversity exhibited a better nutritional status (P < 0.05); the average satisfaction with diet and self-evaluation of food nutrition of the CG were higher than the NCG (P < 0.05); the ratio of having a diet on time in the CG was higher than that of the NCG (P < 0.05). Conclusion: The free canteen services provided by the government can improve older adults' satisfaction with life and diet, and mental health status and also enrich their social capital, but this still needs to be future evidenced. More financial support for canteen services is an essential component in promoting successful aging in China.
Personal Prioritization of Healthy Eating among Chinese Adults and Its Association with Dietary Behaviors: Findings from the China Health and Nutrition Survey

Abstract:Objective: China faces a growing non-communicable disease (NCD) burden linked with diet. The link between attitudes towards healthy diets and specific eating behaviors has been underexplored in Chinese settings. The aim of this study is to evaluate disparities in personal healthy eating prioritization among Chinese adults and its association with specific dietary patterns. Methods: Data from the 2011 China Health and Nutrition Survey (CHNS) was used to examine healthy eating prioritization (how important “eating a healthy diet” was to participants) and its association with socio-demographic, interpersonal, and physical health variables, along with plant-based and healthy eating dietary patterns, and consumption of specific food groups. Results: Of the 13,653 participants with healthy eating prioritization data, 94% believed eating a healthy diet to be important. Odds of believe healthy eating to not be important was negatively associated with increased age (AOR:0.98, 95%CI:0.98-0.99), higher educational attainment (AOR:0.12, 95%CI: 0.07-0.19), living in urban environments (AOR:0.76, 95%CI:0.63-0.83), living with both parents (AOR:0.69, 95%CI:0.48-0.98), and past diagnosis of NCDs (AOR:0.76, 95%CI:0.59-0.96). Men and those who were single had higher odds of believing eating a healthy to not be important (AOR:1.36, 95%CI:1.08-1.71; AOR:1.36, 95%CI:1.08-1.71). Compared to those who believed eating a healthy diet to be important, those who did not had a lower odds of higher vegetable intake (AOR:0.77, 95%CI:0.62-0.96) and higher meat intake (AOR:0.80, 95%CI:0.63-1.00). Conclusion: Findings provide novel insights on how interventional and observational dietary attitude research in China may need to explore the unique role vegetable and meat intake play in the diet attitude-behavior link.
Assessing food availability and healthier options in an urban Chinese university: a case study using the Chinese Nutrition Environment Measurement Survey for Stores (C-NEMS-S)

Abstract:Abstract Young adults (18–24 years) in universities are frequently exposed to an environment that promotes unhealthy eating behaviors. Using a validated tool, the Chinese Nutrition Environment Measurement Survey for Stores (C-NEMS-S), we assess the food availability and healthier options in a large, urban Chinese university. We employed C-NEMS-S for scoring criteria and weighting. A total of 52 on-campus canteen outlets were audited in an urban university located in Shijiazhuang City, China. General food outlets ( n 43) and self-served food outlets ( n 7) were further categorized into eight subtypes. Beverage outlets ( n 2) were discussed separately from food outlets. C-NEMS-S scores were significantly different across food outlet types ( P = 0.0024), especially between noodle and rice outlets ( P = 0.0415). Food availability scores for starchy tubers ( P < 0.001), dry beans ( P < 0.001), vegetables ( P = 0.0225), and fruits ( P < 0.001) were significantly different across food outlet subtypes. Healthier options were scarce and only appeared in “grains” ( n 2) and “meat and poultry” ( n 2) categories. Further research on improving the accustomed audit tool and assessing university student diet quality is warranted.

The Relationship among Perceived Stress, Self-efficacy and Health-related Dietary Behaviors of Enterprise Staff

Abstract:Objective To examine the status of health-related eating behaviors and their influential factors of enterprise staff, and explore the relationship among perceived stress, self-efficacy and eating behaviors.Methods A self-administered questionnaire survey including demographic characteristics, perceived stress, self-efficacy and health-related eating behaviors were conducted in 388 employees from four enterprises in Hangzhou, Jiaxing and Shanghai.Canonical correlation analysis and Logistic regression were used to explore the association of perceived stress, self-efficacy with eating behaviors.Results Demographic characteristics, perceived stress and self-efficacy had effect on eating behaviors of enterprise staff (P＜0.001).The higher stress levels, the more possible the stuff eat food while doing other things (P＜0.001).When the self-efficacy of dietary control declined, enterprise staff tended to take more fried food, sweetened beverages, sweets, and eat while detracted (P＜0.05).Conclusion Perceived stress and self-efficacy are associated with eating behaviors of enterprises staff.Therefore we need to take the stress coping strategies and self-efficacy enhancement in account when we conduct the health promotion projects in worksite.
Development and evaluation of a food environment survey in three urban environments of Kunming, China

Abstract:Background: Given the rapid pace of urbanization and Westernization and the increasing prevalence of obesity, there is a need for research to better understand the influence of the built environment on overweight and obesity in world's developing regions. Culturally-specific food environment survey instruments are important tools for studying changing food availability and pricing. Here, we present findings from an effort to develop and evaluate food environment survey instruments for use in a rapidly developing city in southwest China. Methods: We developed two survey instruments (for stores and restaurants), each designed to be completed within 10 minutes. Two pairs of researchers surveyed a pre-selected 1-km stretch of street in each of three socio-demographically different neighborhoods to assess inter-rater reliability. Construct validity was assessed by comparing the food environments of the neighborhoods to cross-sectional height and weight data obtained on 575 adolescents in the corresponding regions of the city. Results: 273 food establishments (163 restaurants and 110 stores) were surveyed. Sit-down, take-out, and fast food restaurants accounted for 40%, 21% and 19% of all restaurants surveyed. Tobacco and alcohol shops, convenience stores and supermarkets accounted for 25%, 12% and 11%, respectively, of all stores surveyed. We found a high percentage of agreement between teams (>75%) for all categorical variables with moderate kappa scores (0.4-0.6), and no statistically significant differences between teams for any of the continuous variables. More developed inner city neighborhoods had a higher number of fast food restaurants and convenience stores than surrounding neighborhoods. Adolescents who lived in the more developed inner neighborhoods also had a higher percentage of overweight, indicating well-founded construct validity. Depending on the cutoff used, 19% to 36% of male and 10% to 22% of female 16-year old adolescents were found to be overweight. Conclusions: The prevalence of overweight Chinese adolescents, and the food environments they are exposed to, deserve immediate attention. To our knowledge, these are the first food environment surveys developed specifically to assess changing food availability, accessibility, and pricing in China. These instruments may be useful in future systematic longitudinal assessments of the changing food environment and its health impact in China.
Effectiveness of a school-based nutrition and food safety education program among primary and junior high school students in Chongqing, China

Abstract:Background: Health behavioral patterns, especially eating patterns, established in childhood often carry over into adulthood, and some of the unhealthy ones are later associated with adult morbidity and mortality. Recently, a few nutrition and food safety education programs have been implemented in primary and junior high schools in China. Purpose: This study aims to examine the effectiveness of a school-based nutrition and food safety education program among primary and junior high school students in China. Methods: A mixed study design incorporating an intervention study and a quantitative survey was conducted for this research. With stratified cluster sampling, students from the 5 th and 6 th grade in one primary school and the 7 th and 8 th grade in one junior high school in Chongqing, China, were all selected and separated randomly into an intervention group ( n = 501) and a control group ( n = 522). Effectiveness evaluation investigations were performed at the initial time and nine-month follow-up ( n = 472), respectively. Effectiveness of pre-/post-intervention and nine-month follow-up changes in scores of nutrition knowledge and food safety was assessed using a two-tailed t-test and analysis of variance. Results: Nutrition knowledge scores for the intervention group were mean 9.03, SD±2.75 at the baseline, and 14.70±3.28 after intervention. There was a significant improvement ( t = 29.78, p < 0.01). The nine-month follow-up knowledge scores of the intervention group were 12.35±2.89, which were lower than the immediately after the intervention group ( t = 12.40, p<0.01 ), but higher than those of the baseline level ( t = 18.04, p < 0.01). Food safety scores of the post-intervention were higher ( p < 0.01) than that of the control group in both pre-intervention and nine-month follow-up. The control group had no significant change in the pre-post intervention. Conclusions: It is feasible and effective to improve nutrition and food safety knowledge among primary and junior high school students through school-based nutrition and food safety education programs.

An exploratory social media intervention for perception and behavior on eating away from home: A cluster randomized trial in Chongqing, China

Abstract:A modern diet with high energy density is harmful to health and negatively affects the environment. Eating out is particularly prevalent among young adults and has steadily increased in many countries over the last few decades. This study aims to assess the impact of nutrition education interventions on social media on cognitive and behavioral changes in the diet of university students. A cluster-randomized trial with an intervention group (n = 853) and a no-intervention control group (n = 846) was conducted in Chongqing. A two-month educational intervention was conducted through social media (WeChat official account, Sina Weblog, and Tencent Weibo) with participants of the intervention group. Self-reported eating-out-related knowledge and behaviors were assessed at baseline and six and eight months after randomization. Multilevel mixed-effects logistic regression and linear regression analyses accessed the outcomes between the intervention and control groups. The proportion of participants in the high-level knowledge and attitude group in the intervention group increased by 13% and 8.5% compared with the control group (p < 0.05). The effects of the intervention on the outcomes significantly changed over time, as demonstrated by the achieved statistical significance for knowledge and attitude score (p for group and time interaction <0.001) in the interaction between time and randomization groups. The high-frequency eating away from home/school at weekends decreased from 29.6% to 27.3% in the intervention group. In the control group, the high-frequency eating away from home/school at weekends in the increase was increased (26.3%–27.2%). A total of 73.6% of respondents in the intervention group reported that they consciously reduced their frequency of eating away from home/school. The possibility exists for narrowing the perception and behavior disparity gap through the effective use of social media, such as WeChat and Microblog, which could be a sustainable platform to provide health education or new food policies to promote healthy eating and reduce food waste among university students.

Understanding Motives for Food Choice of Chinese Young People in Mainland China

Abstract:With increasing prevalence of early-onset dietary-related diseases in Chinese young people, understanding their motives for food choice and the associated factors is particularly important for further behavioral interventions and food policy decisions. In our study, the food choice questionnaire was adopted, to measure Chinese young people's motives for food choice at an administrative division level in mainland China, and to understand physiological and socio-economic effects on the motives. Compared to ethical concern which was rated least important, sensory appeal, convenience and health were three most important motives ranked by people from Southwest, Southcentral, East, Northeast and North regions, while people from Northwest ranked familiarity as one of their most important motives. Canonical discriminant analysis indicated clear similarities and differences for each region of residence. Analysis of variance showed socio-economic factors played independent roles in food choice motives. However, physiological factors, including body mass index, waist-hip ratio, self and caregivers'health conditions didn't show any independent effects or interactions with socio-economic factors. Cluster analysis indicated that Chinese young people from six experimental regions were mainly grouped in five different motive-oriented clusters. Nearly 40 percent of young people were basically health-oriented food consumers. Together, it would be possible to alter motives for food choice towards health by changing some socio-economic factors, like level of education, monthly expenses. These data are important for further studies on health conditions and eating behaviors of Chinese young people distributed in different areas.
Eating Out-of-Home in Adult Residents in Shanghai and the Nutritional Differences among Dining Places

Abstract:Background: With the rapid development of Shanghai’s economy, diet habits have undergone great changes. The study aimed to examine the situation of out-of-home (OH) eating in Shanghai adults and the nutrition characteristics of eating in different dining places, and to assess the social demographic determinants of eating OH. Method: Data was sourced from the Shanghai Diet and Health Survey (SDHS) involving people aged 18 years or older in 2012–2013. The food frequency questionnaire (FFQ) and three-day 24-h dietary recall (24-HDR) were used to collect dietary intake data on how people eat out in a cross-sectional study of 1689 adults. OH food refers to the food prepared or consumed away from home. We define that people who eat at least one meal prepared away from home in each survey have a habit of eating outside. The multiple linear and logistic regression methods were used for statistical analysis. Results: The prevalence of eating OH and at restaurants was only 55.1% and 31.8%, respectively. There was an increase in energy, protein, carbohydrate, fat, and iron intake while eating OH. Restaurant and company/school canteen consumption were both associated with an increase in daily total energy intake of 140 kcal and 91 kcal, and fat intake of 6.0 g and 4.3 g, respectively. However, eating at restaurants was associated with higher intake of 548 mg of sodium. However, no significant association was observed between eating at canteens and higher sodium intake. Conclusions: Eating OH related to a poor diet quality, and the diet quality was different between restaurant and canteen food. There may be a need for interventions to target residents’ overall dining-out behavior, particularly focusing on the consumption of restaurant food.
Intestinal metaplasia at the gastro-oesophageal junction:Helicobacter pylori gastritis or gastro-oesophageal reflux disease?

Abstract:Background—Intestinal metaplasia, whether in the cardia or the distal oesophagus, has been uniformly defined as specialised columnar epithelium, suggesting a relation with Barrett’s oesophagus. It is, however, not clear whether the risk factors associated with intestinal metaplasia are identical at both sites. Aims—To investigate biopsy specimens obtained below the squamocolumnar junction (SCJ) in relation to endoscopic aspect, gastric histology, and clinical presentation. Patients and methods—In 423 patients investigated the endoscopic aspect of the SCJ was classified as unremarkable (group I, n=315) or suggestive of Barrett’s oesophagus (group II, n=108). Standardised biopsy specimens from the antrum, corpus, and directly below the SCJ were investigated. Results—Intestinal metaplasia was detected at the SCJ in 13.4% of group I patients, where it was significantly associated with gastric intestinal metaplasia (odds ratio (OR) 6.96; confidence interval (CI) 2.48 to 19.54) andH pylori (OR 7.85; CI 2.82 to 21.85), and in 34.3% of group II patients where it was significantly associated with reflux symptoms (OR 19.98; CI 6.12 to 65.19), erosive oesophagitis (OR 12.16; CI 3.86 to 38.24), and male sex (OR 6.25, CI 2.16 to 18.14), but not with H pylori or gastric intestinal metaplasia. Conclusion—This study suggests that the pathogenesis of intestinal metaplasia at the SCJ is not uniform: at an endoscopically unremarkable SCJ it is a sequela ofH pylori gastritis, but coexisting with endoscopic features of Barrett’s oesophagus it is associated with male sex and gastro-oesophageal reflux disease.

Abstract:Copper and zinc containing-superoxide dismutase (Cu,Zn-SOD), and manganese containing-superoxide dismutase (Mn-SOD) were purified form human placenta with immunoadsorbent columns using anti-human SOD-antibody conjugated Sepharose 4B. The final preparation has a high purity with a good recovery. The Cu,Zn-SOD purified by an immunoadsorbent column had less zinc content and was more labile than the native Cu,Zn-SOD, but this difference disappeared completely by dialyzing the enzyme obtained by immunoadsorbent column against the buffer containing 1mM ZnCl2. Other enzymological properties of two SODs were identical with that of native SODs.
Family Food Environments and Their Association with Primary and Secondary Students' Food Consumption in Beijing, China: A Cross-Sectional Study

Abstract:Family is the most fundamental and proximal context for children, and children's eating behavior occurs mostly in the home or together with family members. With this study, we aimed to evaluate the distribution of family food environment dimensions and their relationship with healthy and unhealthy food consumption in primary and secondary students in order to provide evidence in the Chinese context and explore environmental solutions for improved child nutrition. Multi-stage stratified cluster sampling was used to conduct a cross-sectional survey among students in Beijing, China, from September 2020 to June 2021. Family food environment (FFE) was measured by the validated Family Food Environment Questionnaire for Chinese School-age Children, which was self-administered by the children's caregivers. The students were asked to answer questions about food consumption frequencies in the past 7 days. Binary logistic regression models were used to investigate the relationships between food consumption frequency and FFE, and odds ratios (ORs) with 95% confidence intervals (CI) were computed for independent variables. Among the study population, 9686 students in grades 3-12 and their caregivers completed the survey. The mean score of FFE was 65.7 (+/- 8.4) out of a total possible score of 100, with 76.6% of students categorized as relatively healthier according to their FFE score (>= 60). Compared with the reference group, students in primary schools and those cared for principally by parents were more likely to be in a healthy FFE category (p < 0.05). Daily consumption of fruits and vegetables was reported by 62.6% and 71.6% of students, respectively, and weekly sugared soft drink consumption was reported by 70.9% of the students. Students with a healthier FFE score (>= 60) were more likely to consume fruits (OR = 1.578, 95% CI: 1.428 similar to 1.744) and vegetables (OR = 1.402, 95% CI: 1.263 similar to 1.556) but less likely to consume sugared soft drinks (OR = 0.592, 95% CI: 0.526-0.667). Family food availability (beta = 0.137), caregivers' nutritional literacy (beta = 0.093), meal practices (beta = 0.079) and food rules (beta = 0.050) were positively correlated with food consumption behavior (p < 0.05). The students with healthier FFE scores (OR = 1.130, 95% CI: 1.014 similar to 1.258) and whose caregiver was obese (OR = 2.278, 95% CI: 1.973 similar to 2.631) were more likely to be overweight. The family food environment plays an important role in shaping food consumption in children. Provision of healthy foods instead of unhealthy foods, positive meal practices and food rules, and nutrition education for parents can promote healthy eating in children.
The roles of health culture and physical environment in workplace health promotion: a two-year prospective intervention study in China

Abstract:Background To understand the potential influencing factors on the effectiveness of workplace health promotion interventions and examine whether workplace health culture and physical environment can mediate the relationship between workplace health promotion and intervention effectiveness. Methods A total of 719 participants from 10 Chinese government agencies were recruited for a prospective self-controlled trial. Questionnaires, qualitative interviews, and direct observation were used for the baseline evaluation, process evaluation, and effectiveness evaluation. Based on the results of the need assessment and risk assessment at each workplace, a two-year comprehensive health intervention was conducted by each workplace. Health outcomes including self-rated health (SRH) and mental health were measured at baseline and 24 months. Health culture was measured at 24 months. Physical environment and intervention implementation were measured at 12 months and 24 months. Results Compared with the baseline, the means of SRH and mental health increased significantly by 0.302 and 2.698, respectively. The SRH scores were different before and after intervention; furthermore, the differences varied by workplace. Health culture mediated the relationship between intervention implementation and intervention effectiveness, including SRH and mental health improvement, but physical environment did not. Physical environment quality was significantly negatively correlated with SRH improvement and mental health improvement. Under the relatively high-quality interventions with scores higher than 4.047 or 4.151 (out of 5), better health culture may led to greater SRH and mental health improvements. Conclusions Health culture may mediate the relationship between intervention implementation and intervention effectiveness, whereas physical environment does not seem to mediate this relationship. Under relatively high-quality interventions, a better health culture may lead to more positive improvements in SRH and mental health. Future studies will need to examine the physical environment as a moderating effect rather than mediating effect. Preliminary Insights on Barriers to and Facilitators of Healthy Eating for Rural Residents Emerging from Extreme Poverty: A Qualitative Study in Dafang, China

Abstract:The purpose of this study was to examine the dietary views and practices and to identify associated barriers to and facilitators of healthy eating of rural residents emerging from poverty in the context of rapid socio-economic development. A qualitative design with semi-structured interviews was utilized to collect detailed insights into the dietary behaviors of 22 rural residents from 10 villages in Fengshan town, Dafang county, Guizhou province, China. Thematic analysis was applied to interpret the interview data, guided by the socio-ecological model. Four themes were identified: traditional eating patterns, factors influencing food choice, limited knowledge of healthy diet, and lack of nutritional guidance. Barriers to healthy eating included traditional but unhealthy foods, limited nutritional knowledge, inadequate understanding of nutritional requirements, overwhelming nutritional information, and limited professional guidance. Facilitators encompassed healthy traditional eating patterns, household composition, home gardening, preference for traditional bean and soy products, improved food supply and availability owing to poverty alleviation efforts, and being open to receiving professional dietary guidance. As a preliminary investigation into the dietary views and practices of this population, the study highlights a significant gap in the nutritional knowledge and guidance available to rural residents in China, emphasizing the need for comprehensive strategies that address the complex socio-ecological factors influencing dietary behaviors.

Abstract:Dietary change is a win-win opportunity to address the nexus of health and the environment. To prevent city dwellers from developing non-communicable diseases, in 2013, China updated the 2000 version of nutrition-based dietary reference intake (DRI) guidelines. However, whether the DRI guidelines have a positive effect on the environment is not well understood. Here, we explored the systematic effects of urbanization on China's health and environmental nexus based on survey data. Then, we optimized the diets of 18 age-gender groups to reduce carbon emissions, water consumption, and land use while meeting the healthy nutrition goals of both DRI guidelines. The results showed that the optimal diets based on the DRI 2013 outperformed these on DRI 2000 in improving China's environmental sustainability, although these diets did not always perform better at an individual scale. Our findings suggest that dietary changes can reduce carbon, water, and ecological footprints by 24%, 15%, and 22% in 2050, respectively; however, the differences in age-specific and gender-specific health goals cannot be neglected.
