Abstract:Background: Suicide is the fourth most common cause of death for the 15-29 age group. Research on the impact of suicide on parents is scarce and, therefore, poorly understood. Aim: To explore parents' experiences who have lost a son or a daughter due to suicide and their experience of the services available to them. Methods: This phenomenological study involved 1-2 interviews with ten parents aged 40-65, seven mothers and three fathers in all 13 interviews. The age range of their sons and daughters was 17-37 years when they died. Results: For the parents, losing a son or a daughter to suicide is an overwhelming life experience characterised by Excruciating existential suffering and complicated grief where they are confronted with deep meaning-making and existential questions without answers since the person who can answer most of them is no longer alive. They, therefore, felt stuck in their grief for up to 4 years. The initial experience was an immense paralysing shock and sense of unreality. The subsequent period was a blur, and they were numb. Then, their psyche and bodies collapsed, and for a long time, they felt no grief processing was taking place. They sorely needed long-term professional trauma-informed support and felt that, in too many cases, they had to reach out for help themselves. They would have liked to see the healthcare system embrace them with more open arms, offer help and be met with information and individualised support. Conclusions: Standard operating procedures must be installed to support suicide-bereaved parents better. Long-term professional support and trauma-focused care are required following such major trauma, and providing such support could help to reduce their adverse health impacts. Nurses and other health professionals must be better educated on existential suffering in this context.

Abstract:The purpose of this study was to examine the experience process, meaning, and the essential structure of interactions in parents bereaved by their children’s suicide. To this end, 12 parents bereaved by child suicide were recruited by purposive sampling and semi-structured interviews were conducted. The data collection period is a total of 6 months from August 2022, and 1-4 interviews were conducted per participant. Data analysis was analyzed by applying the grounded theory method. As a result of the study, 100 concepts, 44 subcategories, and 17 categories were derived. The central phenomenon was that ‘even the pain of longing buried in guilt and shame.’ The core category was derived as ‘expanding altruistic view for youth while burying the pain of longing in the heart.’ The process of experience in parents bereaved by child suicide was divided into six stages: ‘shock and collapse stage’, ‘avoidance stage’, ‘struggle stage’, ‘acceptance stage’, and ‘re-establishment stage’. This study is significant in that it provided basic data for counseling and therapeutic intervention for parents bereaved by child suicide.

Abstract:People bereaved by suicide have an increased risk of suicide and suicide attempt, yet report receiving less support than people bereaved by other sudden deaths. Reductions in support may contribute to suicide risk, yet their nature is unclear. We explored the impact of suicide bereavement on the interpersonal relationships of young adults in the UK using an online survey to collect qualitative data. We conducted thematic analysis of free-text responses from 499 adults to questions capturing the impact of bereavement on relationships with partners, close friends, close family, extended family, and other contacts. We identified four main themes describing the changes in relationships following the suicide: (1) Social discomfort over the death (stigma and taboo; painfulness for self or others to discuss; socially prescribed grief reactions); (2) social withdrawal (loss of social confidence; withdrawal as a coping mechanism); (3) shared bereavement experience creating closeness and avoidance; (4) attachments influenced by fear of further losses (overprotectiveness towards others; avoiding attachments as protective). These findings contribute to understanding deficits in support and pathways to suicidality after suicide bereavement. Such disrupted attachments add to the burden of grief and could be addressed by public education on how to support those bereaved by suicide.

Abstract:Objective: The death of one's child may negatively affect parental health. Existing models exploring parental bereavement do not provide sufficient frameworks to understand the complexities of health outcomes. The purpose of this study was to examine the health status of bereaved parents. Method: Cross-sectional retrospective study of bereaved parents whose children died at this hospital between 2002 and 2019. A study-specific demographic questionnaire, the National Institutes of Health Roadmap Patient-Reported Outcomes Measurement Information System (PROMIS) initiative. Depressive symptoms were assessed using the PROMIS v1.0 Emotional Distress–Depression—Short Form 8a (Pilkonis et al., 2011). PROMIS measures (depressive symptoms, anxiety, applied cognitive functioning, physical functioning, sleep disturbance), and a measure of posttraumatic growth were mailed and included a link to self-enter data via Research Electronic Data Capture (REDCap, n.d.) or return paper copies. Results: One hundred fifty surveys were returned (22% response rate). Mean participant age was 41.6 years (SD = 10.3 years; 83% female). Participants' health history responses show greater prevalence of depressive symptoms, anxiety, and sleep disorders after their child's death. The largest difference on PROMIS measures was poorer physical function in the bereaved cohort than in the general population. There were no differences in PROMIS scores based on participant gender or on the presence of surviving siblings. Among those reporting smoking or alcohol consumption, qualitative data suggest that although some might increase use, such use might decrease over time. Conclusions: Bereaved parents manifest poorer psychological and cognitive functioning, and their physical functioning was substantially poorer than the general population mean. Bereaved parents may benefit from proactive guidance normalizing potential challenges to health status and behavior, including appropriate psychoeducational resources. Abstract:Introduction The death of a child has a tremendous impact on parents' lives. The experience of parents who have lost a child to cancer may differ from other bereavement experiences, including other childhood and adulthood causes of death, because of the uncertainty of the prognosis, the aggressive treatment, and the potential for regret about treatment decisions. Bereavement care remains scarce, and effective interventions to meet the diverse needs of parents have not been defined. Objective To provide insights on bereaved parents' experiences, their needs, and wishes of support following the loss of their child to cancer. Methods We conducted 18 qualitative, in‐depth, semi‐structured interviews with 23 bereaved parents (seven fathers, 16 mothers), and used reflexive thematic analysis to analyze the data. Results Parents received both informal and professional support. Regarding informal support, parents expressed a high level of ambivalence rooted in grief illiteracy. Parents also recognized their own struggles to express what forms of informal support they would have liked to receive. Support provided by healthcare professionals, institutions, and organizations involved in the children's care or in bereavement was inconsistent due to personnel time constraints, interpersonal relationships, or disruptions due to the COVID‐19 pandemic. These factors could result in parents not having access to certain forms of support or not receiving long‐term support. Conclusion Improving grief literacy may strengthen informal support and make discussions of grief and death less taboo. Institutional policies, training, and networking may help to ensure that support provided by healthcare professionals, institutions, and organizations is less vulnerable to inconsistencies.

Abstract:Background People bereaved by suicide are at increased risk of suicide. Potential explanations include changes in the cognitive availability of suicide after suicide bereavement, but this has been under‐investigated. This study aimed to investigate how suicide bereavement influences thoughts about suicide, including methods considered. Method We interviewed 20 UK‐based adultswho reported having been preoccupied by the suicide of a close contact, analyzing qualitative data using reflexive thematic analysis. Results We identified four main themes: divergent changes in views about suicide as an option; impact of the method used on consideration of own potential method of suicide (including an aversion to the same method); experience of suicidal ideation as a means of understanding the deceased's state of mind; and thoughts related to reunion with the deceased. Conclusions Our findings suggest that the trauma of exposure to a close contact's suicide can modify the cognitive availability of suicide in divergent ways, including suicide being perceived as a more or less acceptable option, and a tension between the two. These insights assist clinicians in sensitive exploration of suicide bereavement and in risk mitigation. They suggest revisions to existing models of cognitive availability and the potential for psychological interventions that modify the cognitive availability of suicide.

Abstract:Background: Parents often suffer reduced mental health after their child’s death; however, the trajectory and risk factors are not well described. Objective: Describe the change in complicated grief, depression, and post-traumatic stress symptoms among parents between 6 and 13 months after their child’s death in a pediatric intensive care unit (PICU), and factors associated with 13-month symptoms. Methods: Parents whose children died in 1 of 8 PICUs affiliated with the Collaborative Pediatric Critical Care Research Network completed surveys 6 and 13 months after their child’s death. Surveys included the Inventory of Complicated Grief (ICG), the Patient Health Questionnaire-8 (PHQ-8) for depression, and the Short Post-Traumatic Stress Disorder Rating Interview (SPRINT). Parents provided sociodemographics. Charts were reviewed for child characteristics. Results: One-hundred and fifty seven parents of 104 deceased children completed surveys at both time points. Mental health symptoms declined over time (mean (SD)): ICG (33.8 (15.4) vs. 30.5 (15.2), p < 0.001), PHQ-8 (9.0 (6.4) vs. 7.3 (5.8), p < 0.001), and SPRINT (14.1 (8.3) vs. 12.0 (8.2), p < 0.001). After controlling for 6-month scores, higher 13-month ICG was independently associated with sudden unexpected death; higher PHQ-8 with Black race, insecure attachment style, and sudden unexpected death; and higher SPRINT with having a high school level of education (compared to college degree or higher). Conclusion: Mental health symptoms improve among parents during the first 13 months after their child’s death; however, symptoms persist for many. Black parents and those whose children die suddenly may be high risk for poor adjustment during bereavement.

Abstract:Background: When a parent of dependent children (<18 years old) is at end of life from cancer, this has a profound impact on the family. Children less prepared for the death of a parent are more susceptive to poorer psychosocial adjustment in later life. There is a lack of understanding from the literature surrounding what support parents require, and how they navigate this end of life experience. Aim: To explore bereaved parents’ experience and needs for families when a parent is at end of life from cancer with dependent children. Design: In-depth, semi-structured qualitative interviews were conducted with 21 bereaved mothers and fathers, identified from the general public, a family support service and hospice. Data were analysed thematically. Results: Parents often live in ‘parallel worlds’ throughout the end of life period. In one world, ‘living in the moment’, cherishing the ordinariness of family life, remaining hopeful treatment will prolong life, whilst adapting as the illness unfolds. The other world presents as ‘intermitted glimpses that death is approaching’, shadowed with painful emotional concerns surrounding their children and the future. At the end, death rapidly approaches, characterised as suddenly ‘falling off the cliff’; placing significant demands on the well-parent. Conclusions: Amidst challenges, clinicians should provide parents with clear information surrounding a poor prognosis, so families can plan and prepare for parental death. There is a need for healthcare professionals to engage, encourage and equip parents, as they prepare their children throughout the end of life experience for the inevitable death of a parent.

Abstract:Abstract Background Suicide bereavement entails profound social stressors, including stigma and communication barriers, which can impair social support for suicide loss survivors (SLS). Despite recognized benefits of empathetic interactions, social support, and self-disclosure in mitigating adverse mental health outcomes after suicide loss, we lack a comprehensive understanding of the factors influencing perceived social support among SLS within their broader social environments. To address this gap, our study explores the diverse social experiences of SLS beyond their immediate circles. Specifically, we identify characteristics that define both supportive and non-supportive social experiences of SLS, as well as the facilitators and barriers to social support in the context of suicide bereavement. Methods In 2022, we conducted structured online individual interviews with a diverse sample of 18 SLS in Germany. We analyzed these interviews using qualitative content analysis. Results We examined the social experiences of SLS across three phases and social contexts: (1) the immediate aftermath of the loss; (2) during bereavement practices; and (3) over time. Our findings show that proactive responses and personalized mourning rituals significantly enhance SLS’ sense of community support, while encounters characterized by avoidance or intrusive curiosity lead to feelings of isolation. Over time, supportive interactions often emerge from peers with similar experiences, promoting openness and shared vulnerability. Conversely, superficial engagement, along with experiences of others depersonalizing and avoiding conversations about the loss, contribute to a sense of marginalization. Conclusions Our findings highlight the importance of proactive engagement and open dialogue, calling for societal and communicative shifts toward inclusive and compassionate approaches in addressing suicide loss. This study underscores the need for comprehensive strategies that enhance both suicide and grief literacy and address the taboo and stigma surrounding suicide, ultimately fostering supportive social environments for SLS.

Abstract:Parental suicide in childhood increases the risk of mental ill-health, substance use and premature mortality, particularly through suicide. Postvention supports tailored to the well-being and functioning of suicide-bereaved children and their remaining parents are thus of critical importance to counteract negative development. This explorative cross-sectional study seeks clinically relevant knowledge by investigating posttraumatic stress (PTS), sense of coherence (SOC) and family functioning among children ( n = 22), adolescents ( n = 18) and parents ( n = 40) before their attendance at a family-based grief support program. The results demonstrate critical health outcomes for children and parents, and in particular for adolescents. Clinically relevant symptoms of PTS were found in 36% of children, 65% of adolescents, and 37% of parents. All groups showed lower SOC than the norm. Adolescents reported dysfunctional family functioning for the dimensions Communication and Affective Responsiveness. Psychoeducational and trauma-informed support is recommended where family communication and meaning construction of suicide is given special attention.

Three Sibling Survivors' Perspectives of their Father's Suicide: Implications for Postvention Support

Abstract:This qualitative case study describes three adult siblings' experiences and their perceptions of support connected with the time before and after their father's suicide. At the time of the suicide, participants were ages 1, 5, and 8 years old. We considered commonalities and disparities among the three survivors' perceptions. We also considered how their reported experiences compared to extant literature on child survivors of parent suicide. Our findings suggest that, although the siblings experienced the same traumatic event, each had unique perceptions of the parent's suicide. Immediately prior to closing each interview, to deescalate from the intense topic of suicide, participants offered their impressions of potentially therapeutic children's books and how bibliotherapy may or may not support surviving children. Participants' perceptions of selected children's picture books offer insights about opening communication and addressing challenges specific to a parent's suicide. Implications for teachers, parents, and school-based mental health professionals are provided. We conclude that postvention must consider and monitor each child's perceptions and provide individualized interventions that encourage open communication and support adaptive coping to navigate the intense grief associated with a parent's suicide.

Abstract:This phenomenological study aimed to deeply understand the perceptions, experiences, and coping mechanisms of families who have experienced completed suicide in the pre- and post-suicide periods. In this scope, in-depth interviews were conducted with a family member who had experienced completed suicide. Colaizzi's analysis method was used for data analysis. The main theme of the pre-suicide process included sub-themes of traumatic experiences, unmanageable genetic inheritance , and alcoholism ; the main theme of the approaching period included sub-themes of clear signals and accelerators ; the main theme of the post-suicide process included sub-themes of difficulty in creating meaning, stigma and guilt , and unmet support needs ; the main theme of perceptions of preventability included the sub-theme not everyone who is sick has to die and ultimately.

Cause of death and the quest for meaning after the loss of a child

Abstract:This study examined patterns of making meaning among 155 parents whose children died from a variety of violent and non-violent causes. Findings indicated 53% of violent loss survivors could not make sense of their loss, as compared to 32% of non-violent loss survivors. Overall, there was overlap in sense-making strategies across different causes of death, with many parents invoking spiritual and religious meanings and the cultivation of empathy for the suffering of others. Nonetheless, violent loss survivors described the imperfection of the world and brevity of life more frequently in their narrative responses than parents who lost a child to natural causes, who in turn were more likely to find benefit in the loss in terms of personal growth. Violent loss survivors--and especially those losing a child to homicide--also reported enhanced appreciation of life more frequently than survivors of non-violent losses, and surviving a child's suicide was specifically associated with a change in priorities in the sample. Findings are discussed in terms of common and distinctive themes in meaning making that clinicians may encounter when working with parental bereavement, and the implications these carry for finding spiritual and secular significance in a traumatic loss.
Experiences of Bereaved Families by Suicide in South Korea: A Phenomenological Study

Abstract:When an individual commits suicide, family members frequently experience feeling of hurt, denial, shock and anger, resentment, shame, and guilt. These negative emotions experienced by family survivors make their life suffer and impede the grieving process. If left untreated, they become physically and psychologically vulnerable and the risk of suicidal ideation is high, so professional intervention is needed. This study aimed to explore the experiences of suicidally bereaved families in South Korea. This research was designed to a qualitative phenomenological study conducted by using Colaizzi's methods. Participants were seven individuals who had lost a family member to suicide in Changwon City, South Korea. The Data were collected through in-depth and individual interviews with participants from June to December 2018, and the interviews took place 1 to 15 months after their loss. A total of 25 meaning units, 12 themes, and 5 theme clusters emerged from the analysis. The 5 themes of south Korean bereaved families' experience were: (1) an absurd breakup that came without notice, (2) a life trapped in pain, (3) family isolation by themselves, (4) uncontrolled mind in daily life, and (5) liberating from the bondage of pain. The findings of this study provide insight regarding how suicide loss affects bereaved families and could inform the development of evidence-based programs to prevention suicide thought experienced by bereaved families.

Abstract:Abstract Background Bereaved parents experience higher rates of depressive and post-traumatic stress symptoms after the stillbirth of a baby than after live-birth. Yet, these effects remain underreported in the literature and, consequently, insufficiently addressed in health provider education and practice. We conducted a participatory based study to explore the experiences of grieving parents during their interaction with health care providers during and after the stillbirth of a baby. Methods This community-based participatory study utilized four focus groups comprised of twenty-seven bereaved parents (44% fathers). Bereaved parents conceptualized the study, participating at all stages of research, analyses, and drafting. Data were reduced into a main theme and subthemes, then broad-based member checked to ensure fidelity and nuances within themes. Results The major theme that emerged centered on provider acknowledgement of the baby as an irreplaceable individual. Subthemes reflected 1) acknowledgement of parenthood and grief, 2) recognition of the traumatic nature of stillbirth, and 3) acknowledgement of enduring grief coupled with access to support. It was important that providers realized how grief was experienced within health care and social support systems, concretized by their desire for long-term, specialized support. Conclusions Both mothers and fathers feel that acknowledgement of their baby as an individual, their parenthood, and their enduring traumatic grief by healthcare providers are key elements required in the process of initiating immediate and ongoing care after the stillbirth of a baby.

Abstract:Despite the high prevalence of suicide and fatal overdose, and the morbidity associated with these forms of bereavement, little is known about the loss-related needs of adults bereaved by these two causes, including whether survivors' needs differ from those losing significant persons to other forms of sudden death and how these needs may relate to psychiatric symptomatology. Using a newly validated measure (the Sudden Bereavement Needs Inventory) and an online survey design, the purpose of this study was to examine the needs of adults ( N = 403) who have experienced the suicide or fatal overdose of a close other within the past five years, to compare these needs with those of individuals bereaved by a sudden natural death, and to explore how the importance of specific need categories relates to psychiatric symptomatology. Results showed great overlap between adults bereaved by suicide and fatal overdose across all bereavement need categories, with notable differences between these two groups and those bereaved by a sudden-natural loss (e.g., cardiac arrest). Further, among the suicide and overdose bereaved, the self-rated importance of specific need categories accounted for much of the variation in symptoms of prolonged grief, posttraumatic stress, and depression, with relational needs and meaning-based needs emerging as most strongly related to psychiatric symptoms. Study results may suggest the importance of addressing specific needs in therapy among overdose and suicide bereaved individuals to promote adaptive outcomes.

Feelings of guilt and self-stigma influence help-seeking behaviour among suicide-bereaved individuals. WHAT THE PAPER ADDS TO EXISTING KNOWLEDGE?: Coping mechanisms adopted by suicide-bereaved individuals mediate the impact of suicide on their family, and especially on the quality of relationships among them. Supporting others in need can help alleviate guilt and self-blame for the suicide while it enables the bereaved to fulfil their need to keep a non-traumatizing, or even positive bond with the deceased. WHAT THE IMPLICATIONS FOR PRACTICE ARE?: Nursing interventions to facilitate suicide-bereaved family members' participation in self-help support groups and promote their engagement in supporting others in need are important. Mental health nurses need to facilitate the replacement of dysfunctional coping strategies, such as substance use or self-blame with more adaptive ones focused on the personal needs of the bereaved, in order to help them embrace a non-traumatizing memory of the deceased while being in peace with the social environment. Screening for mental health problems and management of shame, self-stigma and guilt during the grieving period needs to be a priority in nursing interventions. Abstract: INTRODUCTION: Losing a family member to suicide is a traumatic experience which includes guilt and self-stigma. Yet, there is lack of data synthesis on the survivors' experience. Aim: A meta-synthesis of qualitative data on the interpretation of loss in suicide-bereaved family members, their coping strategies and the effects on family. Method: A meta-ethnographic synthesis following a systematic literature search and evaluation of the methodological quality of the selected studies was applied. Results: The narratives of 326 individuals (parents/siblings/children/spouses) reported in sixteen studies were analysed. Trying to achieve a balance between keeping alive a non-traumatizing memory of the deceased, destigmatizing and liberating themselves from self-blame, self-criticism and guilt while being able to transform this experience into support towards others in need, was identified as the essence of the experience of the bereaved. Discussion: Although suicide within a family is a traumatic experience, spiritual and existential implications among the bereaved have been reported; their coping mechanisms mediate the impact of suicide on family sustainability. Implications for practice: Nursing interventions to facilitate adoption of coping strategies centred on keeping a non-traumatizing memory of the deceased among the bereaved and promote their participation in self-help groups and activities to support others in need are important.
"Resisting social identity threat and maintaining resilience": A qualitative study of Chinese parents following the loss of an only child

All versions of this article have been corrected.] Objective: Parents who lose an only child in China are stressed and traumatized due to social identity threat (SIT). This qualitative study aimed to interpret their experience to inform culturally and socially sensitive intervention strategies. Methods: Using a phenomenological approach, 17 bereaved parents who lost an only child were interviewed. The transcripts were analyzed using Colaizzi's method. Results: Three themes were identified, namely, "assuming a new social identity," "triggering social identity threat," and "resisting social identity threat and maintaining resilience." The study showed that SIT initially began with identity reconstruction, where self-identity and social identity occurred 1 after another. Once labeled with such social identity, the bereaved parents suffered social identity threat triggered by inner inferiority and external stigmatization. The bereaved parents undertook a variety of coping strategies to resist the threat and to maintain resilience; of these strategies, 4 patterns depicting resilience and threat were interpreted. Conclusion: The findings offer an understanding of the multifaceted bereavement dilemma and lay a foundation for developing intervention strategies. Promoting or maintaining resilience and alleviating SIT are 2 important ways that help parents move on. To help them with identity reconstruction, the development of culturally sensitive resilience-based programs and the linking of social resources to solve practical problems are recommended. Community health professionals should encourage parents to maintain good health management to prevent their predicament from worsening. Raising economic assistance, building an elderly care support system, and promoting social acceptance are strategies that could be considered by policymakers. Abstract:Suicide bereavement is known to be highly distressing and to negatively affect the mental health of the bereaved. The role of religion and spirituality after suicide bereavement remains understudied. We aimed to specifically explore the role of spirituality after suicide bereavement by conducting a qualitative study through semi-structured interviews with fifteen people bereaved by suicide. The grounded theory-informed thematic analysis identified five main themes, namely "continuing bonds with the deceased," "lightening the burden," "grounded spirituality," "perceptual and conceptual shift" and "the movement of giving." For many participants, spirituality was a resource that facilitated the process of accepting their loss by offering a larger entity to which to hand over their suffering. Moreover, the continuing bonds with the deceased helped them to get through this ordeal. Our results can enable mental health professionals to better understand how spirituality can facilitate post-traumatic growth in people bereaved by suicide.

Abstract:Adolescents bereaved by suicide and other traumatic death may experience strong grief reactions and increased risks of mental health problems and suicidal behaviour. As timely access to professional help can be critical, it is essential to understand how counselors perceive suicide bereavement in adolescents and how they work with this population. This study aimed to examine the perspectives of counselors (N = 34). Eleven participated in an individual semi-structured interview and 23 others in group interviews. Thematic analysis yielded three themes: (1) Building a relationship with the bereaved adolescent, (2) Offering support tailored to the needs of the grieving adolescent, and (3) Offering strengths-based and sustainable support. Counselors' skills, attitudes, content-related expertise, and approaching the adolescent's grief within their developmental context were deemed essential for building a therapeutic relationship and offering viable support. The findings may inform good practices in counseling bereaved adolescents to facilitate positive mental health outcomes.
