Abstract:The conventional approach to gauging mortality convergence in Europe relies on life expectancy estimates (e0 ) at the national level. However, mortality can differ within countries significantly. To better apprehend whether Europe's mortality patterns have been converging or diverging over recent decades, we must shift our focus to regional mortality data. Using data from statistical offices, we present annual e0 estimates for 420 regions in 16 EU countries from 1995 to 2019. In our empirical analysis, we examined whether regions with initially high mortality levels caught up with low‐mortality regions, and we investigated changes in the standard deviation of Europe's regional e0 distribution over time. Indeed, mortality variation has generally decreased from 1995 to 2019 due to larger gains in e0 for regions with initially high mortality levels. The convergence phase took place mostly during the first half of the entire time period analyzed. Over more recent periods, however, we observe more heterogeneity in the development of e0 . Some advantaged regions realized further gains in e0 , even as e0 improvements slowed for more disadvantaged regions. In conclusion, our analysis underscores the importance of addressing widening health inequalities. Policies should target disadvantaged regions to retard mortality divergence across Europe.

Abstract:Abstract Background Male excess mortality is mostly related to non-biological factors, and is thus of high social- and health-policy concern. Previous research has mainly focused on national patterns, while subnational disparities have been less in the focus. This study takes a spatial perspective on subnational patterns, covering seven European countries at the crossroad between Eastern and Western Europe. Methods We analyze a newly gathered spatially detailed data resource comprising 228 regions with well-established demographic methods to assess the contribution of specific causes of death to the evolution of sex mortality differentials (SMDs) since the mid-1990s. Results Our results show that declines in SMDs were mostly driven by a reduction of male excess mortality from cardiovascular diseases and neoplasms (about 50–60% and 20–30%, respectively). In Western Europe, trends in deaths from neoplasms contributed more to the reduction of SMDs, while among regions located in Eastern-Central Europe narrowing SMDs were mostly driven by changes in cardiovascular disease-related deaths. Moreover, men show up to three times higher mortality levels from external causes as compared to women in several analyzed regions. But in absolute terms, external deaths play only a minor role in explaining SMDs due to their small contribution to overall mortality. Conclusions We conclude that examining the regional development of SMDs is useful for introducing targeted social and health policies in order to reduce and prevent mortality inequalities between women and men.

Abstract:In most cases, mortality is analysed considering summary indicators (e.~g. $e_0$ or $e^{\dagger}_0$) that either focus on a specific mortality component or pool all component-specific information in one measure. This can be a limitation, when we are interested to analyse the global evolution of mortality patterns without loosing sight of specific components evolution. The paper analyses whether there are different patterns of mortality decline among developed countries, identifying the role played by all the mortality components. We implement a cluster analysis using a Functional Data Analysis (FDA) approach, which allows us to consider age-specific mortality rather than summary measures as it analyses curves rather than scalar data. Combined with a Functional Principal Component Analysis (PCA) method it can identify what part of the curves (mortality components) is responsible for assigning one country to a specific cluster. FDA clustering is applied to 32 countries of Human Mortality Database and years 1960--2010. The results show that the evolutions of developed countries follow the same pattern (with different timing): (1) a reduction of infant mortality, (2) an increase of premature mortality, (3) a shift and compression of deaths. Some countries are following this scheme and recovering the gap with precursors, others do not show signs of recovery. Eastern Europe countries are still at stage (2) and it is not clear if and when they will enter into phase (3). All the country differences relates the different timing with which countries undergo the stages identified by clusters. The cluster analysis based on FDA allows therefore a comprehensive understanding of the patterns of mortality decline for considered countries.
The contribution of avoidable mortality to life expectancy differences and lifespan disparities in the European Union: a population-based study

Abstract:Background: Twenty years after the 2004 European Union (EU) enlargement, life expectancy differences between established (EMS) and new member states (NMS) remain large. Contributing to this gap are deaths that can be avoided through preventive services or adequate medical treatment. We estimate the impact of reducing avoidable mortality on life expectancy and lifespan disparities in the enlarged EU. Methods: Using World Health Organization mortality database data, we analysed the potential of reducing avoidable mortality, as defined by Eurostat and the Organisation for Economic Cooperation and Development, to close the mortality gap between NMS and EMS. We decomposed the changes in life expectancy and lifespan disparity by age and cause using linear integral decomposition. Findings: Averting all avoidable deaths across the EU from 2005 to 2019 would decrease the average life expectancy gap from 5.8 to 2.4 years in men and 3.3-2 years in women and eliminate the lifespan disparity gap. Had NMS achieved the average EMS avoidable mortality rates during the same period, the average life expectancy gap would have been reduced to 1.8 years in men and 1.6 years in women, and the lifespan disparities gap would have been reversed. Avoidable circulatory and injury-related deaths in middle and older age drove the observed mortality changes. Interpretation: Our results suggest that the gap in life expectancy and lifespan disparity across the EU could be reduced by strengthening health systems and investing in averting circulatory and injury-related deaths in middle and older age in NMS. Abstract:The study of mortality patterns is a popular research topic in many areas. We are particularly interested in mortality patterns among main causes of death associated with age-gender combinations. We use symbolic data analysis (SDA) and include three dimensions: age, gender, and patterns across main causes of death. In this study, we present an alternative method to identify clusters of EU countries with similar mortality patterns in the young population, while considering comprehensive information on the distribution of deaths among the main causes of death by different age-gender groups. We explore possible relationships between mortality patterns in the identified clusters and some other sociodemographic indicators. We use EU data of crude mortality rates from 2016, as the most recent complete data available.
Alpha 1D- and alpha 1A-adrenoceptors mediate contraction in rat renal artery.

Abstract:To investigate the alpha 1-adrenoceptor subtype(s) mediating contraction in rat renal artery, we have compared the effect of the alpha 1-adrenoceptor antagonists, 5-methylurapidil, BMY 7378 (8-(2-(4-(2-methoxyphenyl)-1-piperazinyl) ethyl) 8-azaspiro (4.5) decane-7,9-dione 2HCl) and chloroethylclonidine on functional responses to noradrenaline. A clear blockade by chloroethylclonidine (10(-4) M) of noradrenaline-induced contraction was observed and, along with this effect. pKB values of 9.12 and 8.40 for BMY 7378 and 9.75 and 10.06 for 5-methylurapidil were obtained, indicating that the renal artery expresses the alpha 1D-adrenoceptor subtype as the one involved in contraction and not only the alpha 1A subtype as has been reported.

Abstract:Introduction: Despite the improvements in European health systems, a large number of premature deaths are attributable to treatable mortality. Men make up the majority of these deaths, with a significant gap existing between women and men's treatable mortality rate in the EU. Aim: This study aims to identify the healthcare-related factors, including health expenditures, human and physical resources, and hospital services use associated with treatable mortality in women and men across European countries during the period 2011–2019. Methods: We use Eurostat data for 28 EU countries in the period 2011–2019. We estimate a panel data linear regression with country fixed effects and quantile linear regression for men and women. Results: The results found (i) differences in drivers for male and female treatable mortality, but common drivers hold the same direction for both sexes; (ii) favorable drivers are GDP per capita , health expenditures, number of physicians per capita , and (only for men) the average length of a hospital stay, (iii) unfavorable drivers are nurses and beds per capita , although nurses are not significant for explaining female mortality. Conclusion: Policy recommendations may arise that involve an improvement in hospital bed management and the design of more specific policies aimed at healthcare professionals.

Abstract:An empirical question that has motivated demographers is whether there is convergence or divergence in mortality/longevity around the world. The epidemiological transition is the starting point for studying a global process of mortality convergence. This manuscript aims to provide an update on the concept of mortality convergence/divergence. We perform a comprehensive examination of nine different mortality indicators from a global perspective using clustering methods in the period 1990-2030. In addition, we include analyses of projections to provide insights into prospective trajectories of convergence clubs, a dimension unexplored in previous work. The results indicate that mortality convergence clubs of 194 countries by sex resemble the configuration of continents. These five clubs show a common steady upward trend in longevity indicators, accompanied by a progressive reduction in disparities between sexes and between groups of countries. Furthermore, this paper shows insights into the historical evolution of the convergence clubs in the period 1990-2020 and expands their scope to include projections of their expected future evolution in 2030.

Abstract:Age-specific mortality improvements are non-uniform, neither across ages nor across time. We propose a two-step procedure to estimate the rates of mortality improvement (RMI) in age-specific death rates (ASDR) at ages 85 and above for ten European countries from 1950 to 2019. In the first step, we smooth the raw death counts and estimate ASDR using four different methods: one parametric (gamma-Gompertz-Makeham), two non-parametric (P-splines and PCLM), and a novel Bayesian procedure to handle fluctuations resulting from ages with zero death counts. We compare the goodness of fit of the four smoothing methods and calculate the year-to-year ASDR differences according to the best-fitting one. We fit a piecewise linear function to these differences in the second step. The slope in each linear segment captures the average RMI in the respective year range. For each age, we calculate the goodness of fit in the last linear segment to assess how informative the estimated RMI of current mortality change is. The estimated rates of mortality improvement or deterioration (RMI) can be used to make short-term social, health, and social planning, as well as more precise mortality forecasts.

Abstract:Socioeconomic adversity is among the foremost fundamental causes of human suffering, and this is no less true in old age. Recent reports on socioeconomic inequalities in mortality rate in old age suggest that a low socioeconomic position continues to increase the risk of death even among the oldest old. We aimed to examine the evidence for socioeconomic mortality rate inequalities in old age, including information about associations with various indicators of socioeconomic position and for various geographic locations within the World Health Organization Region for Europe. The articles included in this review leave no doubt that inequalities in mortality rate by socioeconomic position persist into the oldest ages for both men and women in all countries for which information is available, although the relative risk measures observed were rarely higher than 2.00. Still, the available evidence base is heavily biased geographically, inasmuch as it is based largely on national studies from Nordic and Western European countries and local studies from urban areas in Southern Europe. This bias will hamper the design of European-wide policies to reduce inequalities in mortality rate. We call for a continuous update of the empiric evidence on socioeconomic inequalities in mortality rate.

Abstract:This article presents a detailed analysis of the global mortality burden of the COVID-19 pandemic across 569 regions in 25 European countries. We produce age and sex-specific excess mortality and present our results using Age-Standardised Years of Life Lost (ASYLL) in 2020 and 2021, as well as the cumulative impact over the two pandemic years. Employing a robust forecasting approach that considers regional diversity and provides confidence intervals, we find notable losses in 362 regions in 2020 (440 regions in 2021). Conversely, only seven regions experienced gains in 2020 (four regions in 2021). Most importantly, we estimate that eight regions suffered losses exceeding 20 years of life per 1,000 population in 2020, whereas this number increased to 75 regions in 2021. The contiguity of the regions investigated in our study also reveals the changing geographical patterns of the pandemic. While the highest excess mortality values were concentrated in the early COVID-19 outbreak areas during the initial pandemic year, a clear East-West gradient appeared in 2021, with regions of Slovakia, Hungary, and Latvia experiencing the highest losses. This research underscores the importance of regional analyses for a nuanced comprehension of the pandemic’s impact.

Abstract:Turbulent socio-economic development, recent political challenges, and remarkable regional diversity with deep historical roots make Ukraine an important case study for understanding mortality trends in Eastern Europe. In this paper, we provide the first comprehensive, spatially detailed analysis of cause-specific mortality trends and patterns in Ukraine, focusing on the period 2006-19. We rely on official mortality data and use various demographic and spatial analysis techniques. Our results suggest a notable attenuation of the long-standing West-East and West-South-East mortality gradients. Cardiovascular mortality at older ages largely explains the gap between the vanguard (lowest mortality) and laggard (highest mortality) areas, especially for females and in the most recent period. By contrast, the impact of mortality from external causes has greatly diminished over time. Hotspot analyses reveal strong and persistent clustering of mortality from suicide, HIV, and lung cancer. Further research should focus on in-depth assessment of the mechanisms causing the observed patterns.
The short-term association between environmental variables and mortality: evidence from Europe

Abstract:Using fine-grained, publicly available data, this paper studies the short-term association between environmental factors, i.e., weather and air pollution characteristics, and weekly mortality rates in small geographical regions in Europe. Hereto, we develop a mortality modeling framework where a baseline model describes a region-specific, seasonal trend observed within the historical weekly mortality rates. Using a machine learning algorithm, we then explain deviations from this baseline using features constructed from environmental data to identify anomalies and extreme events. We illustrate our proposed modeling framework through a case study on more than 550 NUTS 3 regions (Nomenclature of Territorial Units for Statistics, level 3) in 20 European countries. Using interpretation tools, we unravel insights into which environmental features are most important when estimating excess or deficit mortality relative to the baseline and explore how these features interact. Moreover, we investigate harvesting effects through our constructed weekly mortality modelling framework. Our findings show that temperature-related features are most influential in explaining mortality deviations from the baseline over short time periods. Furthermore, we find that environmental features prove particularly beneficial in southern regions for explaining elevated levels of mortality, and we observe evidence for a harvesting effect related to heat waves.

Abstract:Abstract Background Life Expectancy (LE) at birth, the average number of years a person is expected to live from birth, supposing constant age-specific mortality, is an essential indicator of a country or region's well-being, reflecting policy options at the regional level and highlighting progress and gaps in societal health, including at the sub-national level. In this study, our goal was to explore patterns on groups of European Union (EU) member states according to sub-national gaps in LE at birth, between 2000 and 2020. Methods A longitudinal retrospective analysis on differences in LE at birth among NUTS2 regions in 20 EU countries (due to data availability issues) was performed, taking the largest absolute difference between NUTS2 regions, in 2000-2020, as the country value. A hierarchical agglomerative cluster analysis was then performed. All data was retrieved from Eurostat and analysis performed in R version 4.0.0. Results 3 clusters were identified: (1) Belgium, France, Czechia, Hungary, Portugal and Spain (cluster mean: 3.78); (2) Austria, Slovenia, Croatia, Romania, Bulgaria, Poland, Finland, Germany, Italy and Greece (cluster mean: 2.44); (3) Denmark, Netherlands, Sweden and Lithuania (cluster mean: 1.17). Conclusions 3 clusters of EU member states based on sub-national gaps in LE at birth were identified. Further studies are necessary to assess possible associated factors, such as health expenditure and health system efficiency, in order to better inform public policy and close the gap on existing inequalities. Key messages • Important sub-national differences in LE at birth exist in the EU, with considerable variability across groups of countries. • These results may have an important impact on policy-making and inequalities.

Greece since the 1960s: the mortality transition revisited: a joinpoint regression analysis

Abstract:Mortality transition in Greece is a well-studied phenomenon in several of its aspects. It is characterised by an almost constant increase in life expectancy at birth and other ages and a parallel decrease in death probabilities. The scope of this paper is a comprehensive assessment of the mortality transition in Greece since 1961, in the light of holistic analysis. Within this paper, life tables by gender were calculated and the temporal trends of life expectancy at several ages were examined. Moreover, a cluster analysis was used in order to verify the temporal changes in the mortality patterns. The probabilities of death in large age classes are presented. Furthermore, the death distribution was analysed in relation to various parameters: the modal age at death, mode, left and right inflexion points and the length of the old age heap. Before that, a non-linear regression method, originating from the stochastic analysis, was applied. Additionally, the Gini coefficient, average inter-individual differences, and interquartile range of survival curves were examined. Finally, the standardised rates of the major causes of death are presented. All the analysis variables were scholastically examined for their temporal trends with the method of Joinpoint Regression analysis. Mortality transition in Greece after the year 1961 is asymmetrical with a gender and an age-specific component, leading to the elevation of life expectancy at birth over time. During this period, the older ages' mortality decreases, but at a slower pace than that of the younger ones. The modal age at death, mode, the left and right inflexion points and the width of the old age heap denote the compression of mortality in the country. The old age death heap shifts towards older ages, while at the same time, the variability of ages at death decreases, being verified by the Gini Coefficient and average inter-individual differences. As a result, the rectangularization of survival curves is evident. These changes have a different pace of transition over time, especially after the emergence of the economic crisis. Finally, the major causes of death were the diseases of the circulatory system, neoplasms, diseases of the respiratory system and others. The temporal trends of these diseases differ according to the diseases and gender. Greece's mortality transition is an asymmetrical stepwise process characterised by its gender and age-specific characteristics. This process, despite being a continuous one, is not linear. Instead, a combination of serious developments over time governs the country's modern mortality regime. The evaluation of Greece's mortality transition through the lens of more advanced analytical methods may provide new insights and methodological alternatives for assessing mortality transition in other countries of the world.
Effects of vaccination with Haemophilus influenzae on adrenoceptor function of tracheal and parenchymal strips.

Abstract:Haemophilus influenzae is a bacterium that can be isolated from the deeper airways of asthmatic patients. We investigated the effect of vaccination with H. influenzae on alpha and beta adrenoceptor function in guinea-pig tracheal spirals and lung parenchymal strips. The tracheal spirals from H. influenzae-vaccinated animals showed significantly less relaxation to isoproterenol as compared to controls, independent of whether the trachea was maximally contracted with carbachol or only exhibited an intrinsic tone. Furthermore, an increased contractile response to carbachol was observed in these spirals. To isoproterenol in the presence of a beta-2 adrenergic antagonist (H35/25), or to salbutamol alone, the tracheal preparations from H. influenzae-vaccinated animals also showed a decreased relaxation. These results suggest involvement of both beta-1 and beta-2 subtype adrenoceptors. On the other hand, lung parenchymal strips from vaccinated guinea-pigs relaxed significntly more to these drugs. This effect was not influenced by H35/25 but could be inhibited by phenoxybenzamine. Histamine-induced contraction did not differ between the groups. These results indicated that H. influenzae causes a partial blockade of the beta adrenoceptors in tracheal spirals and, therefore, may have important implications in asthmatic bronchitis. In contrast, parenchymal lung strips of the H. influenzae-pretreated group showed an increased relaxation.
All-cause and cause-specific mortality of different migrant populations in Europe

Abstract:This study aimed to examine differences in all-cause mortality and main causes of death across different migrant and local-born populations living in six European countries. We used data from population and mortality registers from Denmark, England & Wales, France, Netherlands, Scotland, and Spain. We calculated age-standardized mortality rates for men and women aged 0–69 years. Country-specific data were pooled to assess weighted mortality rate ratios (MRRs) using Poisson regression. Analyses were stratified by age group, country of destination, and main cause of death. In six countries combined, all-cause mortality was lower for men and women from East Asia (MRRs 0.66; 95 % confidence interval 0.62–0.71 and 0.76; 0.69–0.82, respectively), and Other Latin America (0.44; 0.42–0.46 and 0.56; 0.54–0.59, respectively) than local-born populations. Mortality rates were similar for those from Turkey. All-cause mortality was higher in men and women from North Africa (1.09; 1.08–1.11 and 1.19; 1.17–1.22, respectively) and Eastern Europe (1.30; 1.27–1.33 and 1.05; 1.01–1.08, respectively), and women from Sub-Saharan Africa (1.34; 1.30–1.38). The pattern differed by age group and country of destination. Most migrants had higher mortality due to infectious diseases and homicide while cancer mortality and suicide were lower. CVD mortality differed by migrant population. To conclude, mortality patterns varied across migrant populations in European countries. Future research should focus both on migrant populations with favourable and less favourable mortality pattern, in order to understand this heterogeneity and to drive policy at the European level.

Abstract:A class of Farrow-structure-based reconfigurable bandpass FIR filters for integer sampling rate conversion is introduced. The converters are realized in terms of a number of fixed linear-phase FIR subfilters and two sets of reconfigurable multipliers that determine the passband location and the conversion factor, respectively. Both Mth-band and general FIR filters can be realized, and the filters work equally well for any integer factor and passband location. Design examples are included, demonstrating their efficiency compared with modulated regular filters. In addition, in contrast to regular filters, the proposed ones have considerably fewer filter coefficients that need to be determined in the filter design process.

Abstract:During the past two decades, major investigative interest has been focused on the determinants of chronic renal disease and interventions to retard the inexorable progression to end-stage renal disease. Recent studies have provided a theoretic framework for anticipating that angiotensin-converting enzyme (ACE) inhibitors, and possibly calcium antagonists, may preferentially retard the progression of renal disease. Whereas the majority of available clinical trials have assessed the effects of ACE inhibitors in patients with insulin-dependent diabetes mellitus (IDDM), there are relatively few long-term studies that have evaluated the renal protective effects of ACE inhibitors and calcium antagonists in patients with nondiabetic renal disease. Although clinical trials have been initiated using both of these drug classes as monotherapy, theoretical considerations suggest that fixed-dose combinations of an ACE inhibitor and a calcium antagonist might be appealing as renal protective agents. Several lines of evidence suggest that the renal microcirculatory effects of coadministration of both agents should be complementary. Similarly, recent observations suggest that the two classes may act in a complementary manner to countervail pathogenetic mechanisms at the level of the mesangium. A recent study in type II diabetic patients demonstrated that combination therapy with an ACE inhibitor and a calcium antagonist induced the greatest reduction in proteinuria, and reduced the rate of decline in glomerular filtration rate (GFR) more than did either agent alone at the same level of blood pressure reduction. Based on such considerations, recent randomized prospective studies have been initiated to compare the renal protective effects of combination calcium antagonist-ACE inhibitor therapy versus monotherapy with agents of either of these two antihypertensive classes.
Socioeconomic Factors and All Cause and Cause-Specific Mortality among Older People in Latin America, India, and China: A Population-Based Cohort Study

Abstract:Background: Even in low and middle income countries most deaths occur in older adults. In Europe, the effects of better education and home ownership upon mortality seem to persist into old age, but these effects may not generalise to LMICs. Reliable data on causes and determinants of mortality are lacking.Methods and Findings: The vital status of 12,373 people aged 65 y and over was determined 3-5 y after baseline survey in sites in Latin America, India, and China. We report crude and standardised mortality rates, standardized mortality ratios comparing mortality experience with that in the United States, and estimated associations with socioeconomic factors using Cox's proportional hazards regression. Cause-specific mortality fractions were estimated using the InterVA algorithm. Crude mortality rates varied from 27.3 to 70.0 per 1,000 person-years, a 3-fold variation persisting after standardisation for demographic and economic factors. Compared with the US, mortality was much higher in urban India and rural China, much lower in Peru, Venezuela, and urban Mexico, and similar in other sites. Mortality rates were higher among men, and increased with age. Adjusting for these effects, it was found that education, occupational attainment, assets, and pension receipt were all inversely associated with mortality, and food insecurity positively associated. Mutually adjusted, only education remained protective (pooled hazard ratio 0.93, 95% CI 0.89-0.98). Most deaths occurred at home, but, except in India, most individuals received medical attention during their final illness. Chronic diseases were the main causes of death, together with tuberculosis and liver disease, with stroke the leading cause in nearly all sites.Conclusions: Education seems to have an important latent effect on mortality into late life. However, compositional differences in socioeconomic position do not explain differences in mortality between sites. Social protection for older people, and the effectiveness of health systems in preventing and treating chronic disease, may be as important as economic and human development.
