Abstract:Objective To explore the comprehensive influence factors of nurses' job burnout and to provide evidence for the making of preventive measures.Methods A total of 1500 nurses in Hangzhou city were recruited and investigated with Maslach Burnout Inventory(MBI),Nurses' Job Stressor Scale,General Self-Efficacy Scale and Trait Coping Questionnaire.Results Hierarchical regression analysis and structural equation modeling showed that the variables of personal coping resources(self-efficacy,positive coping and perceived social support)and variables of organizational factors(workload and organi-zational atmosphere)had significant influence on the three factors of job burnout.Conclusions The integration of various measures such as strengthening nurses' self-efficacy,positive coping and perceived social support as well as reducing workload and improving organizational atmosphere is the effective strategy to prevent nurses' job burnout.

Abstract:American psychoanalyst Freudenberger first proposed the concept of "work burnout" in 1974, referring to the long-term fatigue and physical and mental exhaustion resulting from work pressure. [ 1 ] His phenomenon has been recognized as a chronic stress-related problem by the World Health Organization, particularly prevalent in the service industry and healthcare sector. [ 2 ] Work burnout among nurses has become a significant concern as it not only affects their well-being but also impacts the quality of patient care. [ 3–5 ] The factors that influence work burnout are numerous and complex and involve many external factors such as social and cultural backgrounds. [ 6 ] They are also subject to current medical models, hospital policies, and other difficult-to-control factors. [ 7 ] Perhaps shifting the focus to controllable internal factors of work burnout would be more helpful in addressing these issues. [ 8 ]

Relationship between nurse burnout, patient and organizational outcomes: Systematic review

Abstract:Background: Burnout, characterized by emotional exhaustion, depersonalization, and decreased personal accomplishments, poses a significant burden on individual nurses' health and mental wellbeing. As growing evidence highlights the adverse consequences of burnout for clinicians, patients, and organizations, it is imperative to examine nurse burnout in the healthcare system. Objective: The purpose of this review is to systematically and critically appraise the current literature to examine the associations between nurse burnout and patient and hospital organizational outcomes. Design and data sources: A systematic review following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses was conducted. PubMed, CINAHL, PsychInfo, Scopus, and Embase were the search engines used. The inclusion criteria were any primary studies examining burnout among nurses working in hospitals as an independent variable, in peer-reviewed journals, and written in English. The search was performed from October 2018 to January 2019 and updated in January and October 2020. Results: A total of 20 studies were included in the review. The organizational-related outcomes associated with nurse burnout were (1) patient safety, (2) quality of care, (3) nurses' organizational commitment, (4) nurse productivity, and (5) patient satisfaction. For these themes, nurse burnout was consistently inversely associated with outcome measures. Conclusions: Nurse burnout is an occupational hazard affecting nurses, patients, organizations, and society at large. Nurse burnout is associated with worsening safety and quality of care, decreased patient satisfaction, and nurses' organizational commitment and productivity. Traditionally, burnout is viewed as an individual issue. However, reframing burnout as an organizational and collective phenomenon affords the broader perspective necessary to address nurse burnout. Influence of Effort-Reward Imbalance on Turnover Intention in Nurses

Abstract:Objective To investigate the relationship between effort-reward imbalance and job burnout and turnover intention.Methods Six hundred and eighty nurses in three municipal comprehensive hospitals in Hangzhou were surveyed utilizing Questionnaires on general conditions,Effort-Reward Imbalance Inventory,Maslach Burnout Inventory and Turnover Intention Inventory,and 470 effective questionnaires were obtained(69.1%).Results Among the nurses surveyed,31.9%of the nurses were in the state of effort-rewards imbalance,and 82.3%of nurses were overloaded in job. Effort-reward imbalance was significantly related to job burnout and turnover intention(P0.01),and effort-reward imbalance and job burnout had directly impact on turnover intention(P0.05).Conclusion To decrease the turnover intention of nurses and improve the nursing development,the measures of salary increase and job promotion optimization should be adopted to inverse the state of effort-reward imbalance and prevent nurses from job burnout.In addition,more attention should be paid to young nurses and contract nurses to decrease the turnover intention.

Abstract:BACKGROUND: Burnout, depression, and suicide among health care professionals and learners have reached a rampant level. This systematic review aimed to synthesize literature findings of nursing student burnout and its impact on psychological well-being and academic performance.METHOD: This systematic review followed PRISMA guidelines and included data-based studies on nursing student burnout published in peer-reviewed journals between January 2015 and January 2020.RESULTS: This review included 17 articles. Four major themes were identified: 1) a negative relationship between burnout and student self-concept, 2) a negative association between burnout and student engagement, 3) risk factors contributing to student burnout, and 4) interventions to mitigate student burnout. Promoting positive self-concept, engagement, and resilience may alleviate student academic burnout.CONCLUSION: Student burnout has negative influences on health and academics, and interventions to mitigate burnout should be considered early in nursing programs. Abstract:<section class="article-section__content"><h3 class="article-section__sub-title section1"> Background</h3><p>Burnout affects the quality of work and health of newly graduated nurses, increases the turnover rate among newly graduated nurses and leads to a shortage of nurses. Reducing job burnout among newly graduated nurses is critical. However, the factors that impact burnout among newly graduated nurses are currently not well known.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Objective</h3><p>This exploratory study aims to identify the factors that influence burnout among newly graduated nurses in China.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Methods</h3><p>We conducted a cross‐sectional study of newly graduated nurses from 15 cities in 13 provinces in China. We used online questionnaires to conduct the survey and obtain the results in this study. A STROBE checklist was used to report the results.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Results</h3><p>Of the 2400 questionnaires distributed, 2071 (86.29%) were returned. Job satisfaction, professional rank, hospital level, professional values, and core competence were negatively correlated with burnout, while negative emotions were positively correlated with burnout. Women were more prone to burnout than men.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Conclusions</h3><p>Burnout among newly graduated nurses was divided into three dimensions based on the Maslach Burnout Inventory Scale. Our findings provided new evidence that newly graduated nurses had average levels of emotional exhaustion and depersonalization and low levels of personal accomplishment. Job satisfaction, professional rank, hospital level, gender, professional values, negative emotions, and core competence were predictors of job burnout among NGNs in China.</p></section><section class="article-section__content"><h3 class="article-section__sub-title section1"> Relevance to clinical practice</h3><p>Nursing managers should pay attention to burnout management, standardize training about professional values, core competence, and coping mechanisms, and promote mental health and job satisfaction among newly graduated nurses.</p></section>

Abstract:Abstract Background Workforce studies often identify burnout as a nursing ‘outcome’. Yet, burnout itself—what constitutes it, what factors contribute to its development, and what the wider consequences are for individuals, organisations, or their patients—is rarely made explicit. We aimed to provide a comprehensive summary of research that examines theorised relationships between burnout and other variables, in order to determine what is known (and not known) about the causes and consequences of burnout in nursing, and how this relates to theories of burnout. Methods We searched MEDLINE, CINAHL, and PsycINFO. We included quantitative primary empirical studies (published in English) which examined associations between burnout and work-related factors in the nursing workforce. Results Ninety-one papers were identified. The majority ( n = 87) were cross-sectional studies; 39 studies used all three subscales of the Maslach Burnout Inventory (MBI) Scale to measure burnout. As hypothesised by Maslach, we identified high workload, value incongruence, low control over the job, low decision latitude, poor social climate/social support, and low rewards as predictors of burnout. Maslach suggested that turnover, sickness absence, and general health were effects of burnout; however, we identified relationships only with general health and sickness absence. Other factors that were classified as predictors of burnout in the nursing literature were low/inadequate nurse staffing levels, ≥ 12-h shifts, low schedule flexibility, time pressure, high job and psychological demands, low task variety, role conflict, low autonomy, negative nurse-physician relationship, poor supervisor/leader support, poor leadership, negative team relationship, and job insecurity. Among the outcomes of burnout, we found reduced job performance, poor quality of care, poor patient safety, adverse events, patient negative experience, medication errors, infections, patient falls, and intention to leave. Conclusions The patterns identified by these studies consistently show that adverse job characteristics—high workload, low staffing levels, long shifts, and low control—are associated with burnout in nursing. The potential consequences for staff and patients are severe. The literature on burnout in nursing partly supports Maslach’s theory, but some areas are insufficiently tested, in particular, the association between burnout and turnover, and relationships were found for some MBI dimensions only.

Abstract:ObjectiveTo investigate the prevalence and influencing factors of job burnout among nurses in a tertiary general hospital， providing a reference for medical institutions to develop effective management for job burnout.Methods684 nurses were investigated with general demographic questionnaire and Maslach burnout inventory （MBI）.ResultsAmong the three dimensions of job burnout， the scores ［median （25 percentile， 75 percentile）］ of emotional exhaustion （EE）， depersonalization （DP） and personal achievement （PA） were 25 （17， 33）， 5 （2， 9）， and 30 （23， 37）， respectively. Low PA was the most common problem （62.6%）， followed by EE （43.4%）， and DP （24.6%）. Logistic regression analysis found that age， marital status， job type， weekly working time， annual income， self-rated health were the influencing factors of severe EE （P<0.05）； self-rated health was the influencing factors of severe DP （P<0.05）； and employment mode， self-rated health were the influencing factors of severe PA （P<0.05）.ConclusionThe detection rate of job burnout among nurses is high， especially reflected in the EE and DP. Self-rated health is the main associated factor of job burnout. It is suggested that nurses should be intervened in a multi-faceted and multi-angle manner to alleviate job burnout.
The Correlation Research Between Nurse' Burnout and Psychosocial Factors in the Work Setting

Abstract:Objective To investigate the relationship between nurses' burnout and relative organizational psychosocial factors.Methods A total of 425 nurses finished questionnaires, including job burnout, control experience( personal control and control demand), role stress(role conflict and role ambiguity ) and workload and leader support by self evaluation.Results A large percent of nurses reported work stress(48.7%), workload(55.1%). Each factor of job burnout is significantly correlatied to organizational factors(r=0.122～0.491). Regression statistics suggested workload, personal control and role conflict are important forecast factors.Conclusions There is significant correlation between nurses′ burnout and organizational psychosocial factors, the related resolution strategies were discussed.

Abstract:Objectives To systematically estimate the global prevalence of burnout among n students, and examine the associated factors of burnout in this population. Design A systematic review and meta-analysis. Review methods and data sources PubMed, Embase, Web of Science, and CINAHL were searched from inception to June 30th, 2022. Two researchers independently screened studies, extracted data and assessed the quality of included studies. The random-effects model was used to estimate the global prevalence of burnout among nursing students. Subgroup analysis, meta-regression analysis, publication bias, and sensitivity analysis were also conducted. Results A total of 21 studies were included, involving 10,861 nursing students. In the random-effects model, the pooled prevalence of burnout was 23.0 % (95 % CI = 15.6–30.5 %) in nursing students. The pooled prevalence of emotional exhaustion, depersonalization and reduced personal accomplishment was 47.1 %, 32.2 %, and 43.5 %, respectively. Main associated factors of burnout included demographic (e.g., age and grade), educational (e.g., workload, academic satisfaction, and incivility experience), physical (e.g., sleep quality and physical activity), and psychological (e.g., self-efficacy and personality traits) factors. Conclusions Burnout is common in nursing students, with demographic, educational, physical, and psychological factors affecting their burnout. Early screening of burnout and interventions to prevent and reduce burnout should be considered for nursing students.

Abstract:Objective To explore the level of job burnout among nurses working in second-or third-tier hospitals in China and to find out the contributing factors of job burnout.Methods Based on national nursing human resource research,the Maslach Burnout Inventory was used to measure the burnout level of 9 698 nurses from 179 second-or third-tier hospitals in China.Results China′s nurses experienced moderate level of emotion exhaustion and depersonalization,and diminished personal accomplishment.Nurses with different lengths of service,working in varied units,having a license or not,working overtime or not,working in different levels of hospitals,experienced significantly different levels of job burnout(P<0.05 for all).The locations of hospitals and the regions where the hospitals were situated affected nurses′ burnout levels(P<0.05 for both).Conclusion Nurses with longer service length,working overtime,and those who worked in ICU,in municipalities,in third-tier hospitals,or worked in east China had higher level of burnout.Nursing managers should pay more attention to the contributing factors of job burnout,take measures to lessen burnout levels among nurses,in an effort to stabilize the nursing team and improve the quality of nursing care.

Abstract:Background Occupational burnout is a mental health problem that among nurses may lead not only to physical and psychological complications, but also to a decrease in the quality of patient care. Considering the stressful nature of surgery, operating room nurses may be at a greater risk. Therefore, the present study aimed to identifying factors associated with the occupational burnout from the perspective of operating room nurses. Materials and methods This qualitative study was conducted in Iran in 2021 using conventional content analysis. Sampling was performed using purposeful sampling method with maximum variation. In order to collect data, individual, semi-structured interviews were performed with 18 operating room nurses. Interviews continued until data saturation. Data were recorded, transcribed and analyzed using steps proposed by Graneheim and Lundman. Data management was performed using MAXQDA-2020 software. Four criteria provided by Guba and Lincoln were used to improve the study’s trustworthiness and rigor. Results A main theme, 4 categories and 15 subcategories were extracted from the data. The main theme is “gradual burnout due to job tension” and categories and subcategories include organizational factors (manager incompetence, organizational indifference, ambiguity in organizational role, organizational inconsistency), interpersonal factors (surgeon aggression, surgeon authoritarianism, surgeon failure to manage stress, unprofessional behavior of nurses), Occupational nature factors (psychological factors and occupational hazards) as well as individual factors (occupational attitude, unprofessional behavior, emotional involvement, demographic factors, physical factors). Conclusion Numerous factors associated with burnout in operating room nurses, which may put the personnel under more pressure. According to these factors, nursing managers and operating room nurses can consider effective strategies to prevent or coping with burnout.

A bibliometric analysis of global publications in the field of nurse burnout may provide insights into the current state of the field to help guide future nursing management and promote high-quality research. 12 Therefore, to provide a comprehensive overview of research trends in burnout among nurses, highlight the contributions of leading countries, leading institutions, and authoritative scholars, and provide insight into potential future collaborations and research directions, we carried out the present study. The search was conducted in Web of Science on publications from January 1, 2003 to December 31, 2022, with a retrieval date of March 24, 2023. The retrieval strategy was as follows: TS = (nurse OR nurses OR nursing) AND TS = (burnout OR burn-out OR burn out). There were no limitations with respect to the language, and type of documents were limited to article. The included literature was checked manually by two authors independently. Abstract:PURPOSE: Everyday nursing practice under demanding conditions, high mental and physical strain may result in occupational burnout. There is still a need for studies on the comprehensive identification of burnout among nursing staff. This study aimed to evaluate factors affecting occupational burnout among nurses, including job satisfaction, life satisfaction, and dispositional optimism.METHODS: The study involved 625 survey participants with a mean age of 49.47 years and was conducted between January and December 2018. The inclusion criteria were job experience >1 year, current nurse work activity, and written informed consent to participate in the study. The study used standardized research tools such as Maslach Burnout Inventory (MBI), Life Orientation Test-Revised version (LOT-R), Satisfaction with Life Scale (SWLS), and Satisfaction with Job Scale (SWJS). Also, sociodemographic data were collected using a self-developed questionnaire. The STROBE guidelines were followed.RESULTS: Emotional exhaustion (EE) was significantly greater in respondents with medium life satisfaction (SWLS). Also, EE was significantly greater in pessimists and those with a neutral orientation than in optimists (LOT-R). Housing conditions and family-related problems were significant factors differentiating depersonalization (DEP). Reduced personal accomplishment (PA) was more significant in respondents with a bachelor's degree than in those with a master's degree. Also, decreased PA was significantly greater in respondents with low life satisfaction than those with medium life satisfaction. Life satisfaction and life orientation were significant factors differentiating job dissatisfaction among the health-related and psychological variables.CONCLUSION: Burnout was found to be related to individual, interpersonal and organizational feelings. There was an interrelationship between personality traits, where a higher level of occupational burnout was found among nurses with a pessimist attitude. Health programs should be implemented to identify and eliminate burnout through mental health support, improved communication skills, optimized teamwork, and evidence-based interventions.

Abstract:Objective To investigate the current status and influencing factors of compassion fatigue in clinical nurses,and to provide theoretical references for prevention of nurse burnout and promotion of nurse retention.Methods Totally 992 clinical nurses from ten tertiary hospitals were enrolled through random cluster sampling to complete general information questionnaire,the Professional Quality of Life Scale (Pro-QOL-V),the Resilience Scale for Adults(RSA) and the Jefferson Scale of Empathy·Health Professionals(JSE-HP).Results The prevalence of moderate and high risk of secondary stress and burnout were 71.1％,73.2％,respectively;clinical nurses with work experience of 6 to 10 years presented the highest level of burnout(P＜0.01);lower empathy and resilience,as well as adverse work environment were risk factors of compassion fatigue.Multiple regression analysis showed that the influencing factors of nurse burnout were compassion satisfaction,secondary stress,resilience,workload,recognition,empathy,and payment,which explained 70.5％ of the total variance.Conclusion Secondary stress,burnout and compassion satisfaction in clinical nurses are influenced by nurses' empathy,resilience and work environment,and nursing administrators should pay attention to receiving recognition as positive predictors of burnout,except for workload and payment.
Influencing Factors of Professional Identity, Health Behavior and their Correlation with Job Burnout in Nursing Staffs in Pension Institution

Abstract:Objectives: In this study, we investigated factors impacting nurses' professional identity in elderly care institutions and their connection to occupational burnout. Methods: Using convenience sampling, 394 nurses from local elderly care facilities were surveyed from May 2022 to May 2023. The Elderly Nurses' Professional Identity Scale, Occupational Burnout Scale, and Health Behavior Scale were employed. Single-factor and multiple-factor logistic regression analyses identified risk factors influencing professional identity. Results: Results revealed a negative correlation (r = -0.392, p < .05) between professional identity and occupational burnout. We found statistically significant differences with respect to age, education, income, working hours, number of elderly cared for, family support, and professional qualification (p 10 hours, caring for >10 elderly individuals, and lack of family support were independent risk factors (OR = 0.608, 2.494, 0.555, 0.630, 2.638, p < 0.05) influencing professional identity. Additionally, nurses' health behavior was linked to occupational burnout. Conclusion: Nurses in elderly care institutions face low professional identity, which negatively correlates with occupational burnout. Factors such as advanced age, low income, high workload, and insufficient family support contribute to this. Moreover, nurses' health behavior significantly affects their burnout levels.

Methods: From January 2022 to December 2022, 12,704 clinical nurses from 32 general hospitals in Hunan Province were selected as research participants by convenience sampling. Negative psychology, emotion management, self-efficacy, emotional intelligence and burnout in clinical nurses were measured, and structural equation models were constructed to explore their impact on burnout in clinical nurses. Results: Clinical nurses' negative psychology had a positive effect on burnout ( β =0.60, 95% CI: 0.63– 0.66), emotional intelligence ( β =˗0.08, 95% CI: ˗0.10, ˗0.06) and the self-efficacy of emotion management ( β =− 0.60, 95% CI: ˗0.05, ˗0.03) on burnout. Moreover, emotional intelligence and emotion management self-efficacy played a mediating role between negative psychology and burnout in nurses. Conclusion: Clinical nurses' negative psychology had a positive impact on burnout, and emotional intelligence and the self-efficacy of emotion management could alleviate the influence of negative psychology on burnout among nurses. Nurses' emotional intelligence and emotion management self-efficacy can be improved through practical training to help them cope with emotionally loaded situations and reduce stress responses. However, nursing often faces significant occupational pressures, eg time pressure, as well as reduced social support, high workload levels, treatment uncertainty and the risk of negative emotional reactions from exposure to distressed and dying patients. 1,2 Since the COVID-19 pandemic, a large number of nurses have experienced considerable levels of anxiety and depression, even though they have gained a sufficient level of self-efficacy. 3 The pandemic has also had a stressful impact on the psychological states of nursing students, causing severe distress and exhaustion. 4,5 These stressors can lead nurses to experience symptoms of burnout, which, in turn, affect the quality of the care they deliver and patient safety and prognosis. 6 Burnout is a state of physical, emotional and mental exhaustion and fatigue, typically caused by work-related demands in a person's life. 7 Notably, as of 1 January 2022, burnout has become the 11th Revision (ICD-11) statistical classification of medically diagnosed diseases and related health problems, 8 further highlighting the serious impact of burnout on the health of caregivers. It is particularly important to explore how burnout develops, given its devastating effects on nurses and patients, as well as the high risk of nurses experiencing burnout during their careers due to ineffective management causing chronic work stress that can lead to serious negative psychological problems among caregivers. In recent years, a growing body of research has focused on burnout and related factors among nurses. Abstract:BACKGROUND: There is literature scarcity relating to burnout and other work stresses in relation to Chinese nursing-workforce performance.OBJECTIVES: To assess the relationship between self-esteem versus burnout; and effort-reward ratio versus favorability to the work environment, among Chinese nurses.METHODS: We employed four validated questionnaires in the assessment burnout, self-esteem, effort-reward ratio, and favorability of nursing workplace; Maslach-Burnout Inventory (MBI), Rosenberg's self-esteem (RS), Effort-Reward imbalance (ERI) and Work-Environment Scale questionnaires (WES). Linear and ordinal regression models were utilized to assess the relationships between the variables. Analyses were conducted by using SPSS at a 95% level of significance.RESULTS: We assessed 487 (Mean age: 38.8±7.1 years) nurses from three hospitals. Higher self-esteem was associated with a lower level of emotional exhaustion (Unstandardized coefficient: -0.579, p-Value<0.001); and a lower level of depersonalization (Unstandardized coefficient: -0.212, p-Value = 0.001). The relationship between self-esteem and personal achievement did not reach statistical significance. A higher effort-reward ratio was associated with less likelihood that nurses would consider their work environment favorable (Logit estimate of -0.832, p-Value = 0.014).CONCLUSION: Lower self-esteem is associated with increased burnout. A higher effort-reward ratio is associated with an enhanced perception work environment as unfavorable. We recommend psychosocial intervention programs and amendments in nursing policies to improve effort-reward imbalance among Chinese nurses.

Abstract:Background and Objectives: Several factors can compromise patient safety, such as ineffective teamwork, failed organizational processes, and the physical and psychological overload of health professionals. Studies about associations between burn out and patient safety have shown different outcomes. Objective: To analyze the relationship between burnout and patient safety. Materials and Methods: A systematic review with a meta-analysis performed using PubMed and Web of Science databases during January 2018. Two searches were conducted with the following descriptors: (i) patient safety AND burnout professional safety AND organizational culture, and (ii) patient safety AND burnout professional safety AND safety management. Results: Twenty-one studies were analyzed, most of them demonstrating an association between the existence of burnout and the worsening of patient safety. High levels of burnout is more common among physicians and nurses, and it is associated with external factors such as: high workload, long journeys, and ineffective interpersonal relationships. Good patient safety practices are influenced by organized workflows that generate autonomy for health professionals. Through meta-analysis, we found a relationship between the development of burnout and patient safety actions with a probability of superiority of 66.4%. Conclusion: There is a relationship between high levels of burnout and worsening patient safety.

Abstract:Job burnout is an occupational hazard induced by prolonged exposure to excessive work-related stress -a state of physical or emotional exhaustion that also involves a sense of reduced accomplishment and loss of personal identity. Knowledge jobs involve many health-promoting features, but stress and burnout may be caused by the rapid increase in the knowledge intensity of work, new job demands, and emerging new professional subcultures emphasizing the excessive commitment to work. Although there are studies on job burnout and predictors among nurses worldwide, evidence specific to Chinese nurses is lacking. The purpose of this review is to synthesize current research on predictors related to burnout affecting Chinese nurses. We searched the Chinese databases including National Knowledge Infrastructure (CNKI), WanFang, China Science and Technology Journal Database, and Chinese Biological Medical Database (CBM); and international databases including Cochrane Library, PubMed, EMBASE, and Web of Science published up to 30th August 2022. The 125 studies identified assessed a range of predictors of job burnout. The results showed good working relationships and internally controlled individuals were found to be predictive of job burnout. Moreover, numerous predictors produced contradictory data. There are still challenges with how to systematically operationalize such a complicated phenomenon to effectively prevent or reduce job burnout.
