Abstract:Objective: To analyze the current treatment status and prognostic regression of the chronic NK cell lymphoproliferative disorder (CLPD-NK). Methods: We retrospectively analyzed the clinical features, treatment and prognosis of 18 patients with CLPD-NK who were treated at our Hospital between September 2016 and September 2022. Results: Eighteen patients were included: three patients were treated with chemotherapy, five patients underwent immune-related therapy, one patient was treated with glucocorticoids alone, five patients were administered granulocyte colony-stimulating factor, blood transfusion therapy, or anti-infection therapy, followed by observation and follow-up, and four patients were observed without treatment. Fifteen patients survived, including two patients who achieved complete remission (CR) and seven patients who achieved partial remission (PR), of whom one patient progressed to Aggressive NK-cell leukemia (ANKL) and sustained remission after multiple lines of treatment; three patients were not reviewed, of which one patient was still in active disease, three patients developed hemophagocytic syndrome during treatment and eventually died, one of them had positive Epstein-Barr virus (EBV) expression. The 5-years overall survival rate was 83%. Conclusion: Most patients with CLPD-NK have inert progression and a good prognosis, whereas some patients have a poor prognosis after progressing to ANKL and combined with hemophagocytic syndrome. Abnormal NK cells invading the center suggest a high possibility of ANKL development, and immunosuppressants and hormones are effective treatments for this disease.
Cutaneous/ Subcutaneous Involvement - an Immune Privileged Site Associated with Poor Outcomes in Systemic Peripheral T/ NK Cell Lymphoma.

Abstract:Introduction Patients (pts) with systemic peripheral T/NK-cell lymphomas can have cutaneous involvement and their outcomes data are limited in the modern era. We aimed to retrospectively analyse pts who had T/NK-cell lymphomas with cutaneous involvement - PTCL-NOS or extranodal NK/T-cell lymphoma (ENKTL). We excluded cases if they ever had diagnosis of any Primary cutaneous T-cell lymphomas. Methods We conducted a retrospective study of pts with a histopathological diagnosis of T/NK-cell lymphoma established between 2002 and 2022 and identified from the Lymphoma Outcome Database at our center. Pts with a specific diagnosis of PTCL-NOS and ENKTL according to the WHO classification were included if they had biopsy proven involvement of skin. These pts were retrospectively assessed for demographic, clinical, laboratory data, pathology variables, response to treatment and survival outcomes. Fisher's exact test or Chi-square test was used to evaluate the association between two categorical variables. Kaplan-Meier method was used to estimate the time-to-event endpoints including progression-free survival (PFS) and overall survival (OS). Patients without progression or death were censored at the last follow-up date. Appropriate approval was gained from our institution's IRB. RESULTS A total of 356 pts with cutaneous involvement by T/NK-cell lymphoma were analyzed and on further review 45 pts were found to meet the diagnosis of PTCL-NOS ( n=29) or ENKTL ( n=16) with histologically proven cutaneous/subcutaneous involvement according to WHO guidelines. The median follow-up time was 14.5 months (range: 1.84-128.6 months). Twenty-three (51%) pts were men and 22 (49%) women. Median age at diagnosis was 56 years (range, 46-66), with 60% pts less than 60 years old. Thirty (70%) pts were diagnosed as having stage IV disease per Ann Arbor classification. Seven (28%) pts had involvement of an extranodal site other than skin. ECOG PS was 0 in 75% of evaluable pts. Among PTCL-NOS pts, 14/23 (48%) neoplasms were CD4+, 5 (17%) were CD8+; 5 were TCR-gamma+ and 6 pts were TCR-beta+. Of the samples evaluated, CD30 was expressed in 14/28 (50%) cases of PTCL-NOS and 6/12 (50%) of ENKTCL. Bone Marrow involvement was present in 10% of pts. There was no standard frontline systemic therapy approach in this cohort. CHOP-based therapy was used in 15/29 (52%) pts with PTCL-NOS; 8/16 (50%) pts with ENKTL while received asparaginase base therapy. No pts received frontline radiation in the advanced stage setting. Among the 33 evaluable pts in the cohort, the rate of OR to the 1st line treatment was 57.6% (95% CI: 39.2 ~ 74.5%). The rate of CR to the 1st line treatment was 54.5% (95% CI: 36.4 ~ 71.9%). Among PTCL-NOS pts, 12/20 (60%) achieved CR, whereas in ENKTL pts 6/13 (46%) achieved CR to frontline treatment. Fifteen (30%) pts in the cohort had primary refractory disease. Seven pts with PTCL NOS had limited cutaneous/ subcutaneous involvement and received only skin directed therapy without systemic therapy. This pt subset had no evidence of progression, a behavior consistent with primary cutaneous PTCL-NOS. Advanced stage disease was the only variable that had an association with response ( p=0.004). In the whole cohort, the 2=year PFS was 28% with a median PFS of 9.07 (95% CI 4.5-21.98) months. For pts with stage I/II disease, the 2-year PFS was 71% ( 95%CI 0.45 - 1 ) with a median PFS of 177.4 months (95%CI -11.79 - NA). For pts with stage III/ IV disease, the 2-year PFS was 14% (95%CI 0.05 - 0.38 ) with a median PFS of 4.8 months (95%CI 3.19 - 11.99)(p=0.0018). In the whole cohort, the 2-year OS was 47% with median OS of 22.67 ( 95%CI 13.53 - NA) months. For pts with stage I/II disease, the median OS was 177.37 months and for pts with stage III/ IV disease, the median OS was 13.63 months (95%CI 11.99 - NA) (p=0.0018 ) CONCLUSION: Cutaneous / subcutaneous involvement is an uncommon occurrence in pts with T/ NK cell lymphoma and is currently not incorporated in any prognostic scoring system. Systemic T/NK- lymphomas with cutaneous involvement have poor prognosis in our series and underscore the importance of considering skin biopsy in pts presenting with suspicious lesions. We hypothesize that the cutaneous/ subcutaneous tumor microenvironment creates an immune privileged site for chemo-refractoriness. A better understanding incorporating a combined modality approach and novel therapies are needed to improve the outcomes for these pts. Abstract:Chronic lymphoproliferative disorder of natural killer cells (CLPD-NK) is a rare disease characterized by a persistent increase in NK cells in peripheral blood and is generally asymptomatic. If present, symptoms may include fatigue, B symptoms (fever, night sweats, and unintentional weight loss), autoimmune-associated diseases, splenomegaly, and infection due to neutropenia. Peripheral neuropathy, however, is uncommon with an incidence of 3%. Neurolymphomatosis is a neurological manifestation of non-Hodgkin lymphoma and leukemia in which neurotropic neoplastic cells infiltrate the nerves. Moreover, neurolymphomatosis caused by CLPD-NK is extremely rare, with even fewer cases of autonomic dysfunction. We report a case of neurolymphomatosis associated with CLPD-NK and developed autonomic dysfunction, including orthostatic hypotension and gastrointestinal symptoms.

Clinical and genetic profile of Chinese patients with indolent natural killer-cell lymphoproliferative disorder of the gastrointestinal tract

Abstract:Indolent natural killer cell lymphoproliferative disorder of the gastrointestinal tract (iNKLPD-GI) is an uncommon, recently recognized lymphoid proliferation of mature NK cells primarily manifesting in the GI tract. Unlike NK/T lymphoma, iNKLPD-GI exhibits a rather indolent clinical course, underscoring the need for cautious management to prevent unnecessary interventions. However, clinical and molecular features of this entity have not been thoroughly understood. This study aimed to add more information to the current knowledge of this disease. Seven patients with iNKLPD-GI were included in our study. Clinical data included initial symptoms, endoscopic manifestations, pathological features, and therapies. Besides, next-generation sequencing was arranged to explore the underlying genetic mechanism of this disease. In our study, iNKLPD-GI in the urinary bladder was first identified. Edema of extremities (3, 42.8 %) was the most prevalent onset symptom which was reported for the first time. Pathological and immunohistological features were found to display the phenotype of NK cells. Unlike extranodal NK/T cell lymphoma, Epstein-Barr virus-encoded small RNA (EBER) were negative in all patients. Moreover, we found that two patients harbored JAK3 mutation. Apart from JAK3 K563_C565del previously reported in the literature, we discovered new JAK3 mutation sites. Other mutations including BRAF, KRAS, and SH2B3 were also identified. In conclusion, iNKLPD-GI was an indolent atypical NK-cell proliferation with diverse clinical characteristics. "Watch and wait" therapy was preferable to intense chemotherapy. Recurrent JAK3 mutation may be the underlying mechanism responsible for the neoplastic nature of the disease and may serve as a potential target for patients with severe symptoms.

Abstract:We present a case of lymphocytosis assumed and managed initially as a chronic lymphocytic leukemia. Shortly after initial visit, the patient’s condition deteriorated rapidly with hepatosplenomegaly, pleural effusion, ascites, and skin lesions. Flow cytometry (FC) showed the presence of clonal T-cell population, reported as T-cell lymphoma. Due to rapid clinical deterioration, urgent therapy with cyclophosphamide, doxorubicin, vincristine, etoposide, prednisone was initiated, but with minimal response. This prompted further diagnostic testing and demonstrated tumor cells positivity for CD3, CD30, and TCL1 markers. The diagnosis was changed to T-cell prolymphocytic leukemia. The patient responded well to alemtuzumab (anti-CD52 monoclonal antibody) and reached complete remission. FC is an essential modality for assessing and screening circulating lymphocytes when a lymphoproliferative disorder (LPD) is suspected. There are several LPDs that present with different degrees of clonal lymphocytosis. Reactive lymphocytosis should be appropriately investigated. Indolent LPDs can be surveyed by the internist or family physician, while more aggressive LPDs typically require management by hematologists.
Clinicopathological features and individualized treatment of kidney involvement in B-cell lymphoproliferative disorder

Abstract:Background: Due to the various clinical and pathological manifestations of kidney involvement in lymphoproliferative disorder (LPD), the whole spectrum of kidney disease in LPD is still unclear, and data on kidney prognosis is scarce. Methods: We retrospectively reviewed the renal pathology profiles from January 2010 to December 2021, and 28 patients with B-cell LPD combined with intact renal biopsy data were included. Results: There were 20 men and eight women aging 41 to 79 years at the time of renal biopsy (median age 62 years). According to hematological diagnosis, patients were classified into four groups: chronic lymphocytic leukemia (CLL) (group1, n=7), Waldenström macroglobulinemia/lymphoplasmacytic lymphoma (WM/LPL) (group 2, n=8; WM, n=6; LPL, n=2), Other non-Hodgkin's lymphomas (NHL) (group3, n=7; diffuse large B-cell lymphoma (DLBCL), n=2; mucosa-associated lymphoid tissue (MALT) lymphoma, n=4; Low grade B-cell lymphoma, n=1), and monoclonal gammopathy of undetermined significance/monoclonal gammopathy of renal significance (MGUS/MGRS) (group 4, n=6). Median serum creatinine (Scr) level was 129 (range,59-956) umol/L. Eight patients (29%) were presented with acute kidney injury (AKI), and five patients (18%) required hemodialysis upon admission. Twenty-three patients (82%) presented with proteinuria (median protein excretion, 2.14 g/d), 11(39%) of whom had the nephrotic syndrome. Interstitial malignant infiltration was the most frequent renal lesion (n=6). Eight patients underwent immunohistochemistry of renal tissues, of which three patients (CLL, n=1; LPL, n=1; WM, n=1) had confirmed lymphoma infiltrates, and the infiltrating cells in the remaining five patients (CLL, n=1; MALT lymphoma, n=2; MGUS, n=2) were considered unrelated to lymphoma. The most common glomerular diseases were renal amyloidosis (n=4) and membranous nephropathy (n=4). Only 20 patients were treated, 13 of whom were treated with rituximab separately or in combination. The median follow-up time was 11 months. Of these, six had achieved hematological response, complete response in five cases. Eight had achieved renal response. At the end-of-study visit, four patients died and two progressed to end stage kidney disease (ESKD). Conclusion: In conclusion, the clinicopathological spectrum of renal involvement in BLPD is diverse. Renal biopsy and immunohistochemistry are required for early diagnosis and prognostic assessment.
Clinical features and lymphocyte immunophenotyping analysis in primary immunodeficiency patients with non-transplant lymphoproliferative disorders

Abstract:Lymphoproliferative disorders (LPD) comprise a heterogeneous group and are originally classified into the "Disease of immune dysregulation" category. Of 96 Taiwanese patients during 2003-2022, 31 (median 66, range 0.03-675 months) developed LPD, mainly including palpable lymphadenopathy (in 10 patients), intestinal lymphadenopathy associated with refractory inflammatory bowel disease (IBD in 8) and hepatosplenomegaly (in 7) during long-term follow-up (median 144, range 3-252 months). They distributed in the categories of antibody deficiency (2 CVID, 2 TTC37, PIK3CD, PIK3R1 and AICDA each), phagocyte (4 CYBB, 1 STAT1 and 1 IFNRG1), immune dysregulation (2 FOXP3, 2 XIAP and 2 HLH), combined immunodeficiencies (2 IL2RG; CD40L, ZAP70 and unknown each), syndromic features (2 STAT3-LOF, 1 WAS and 1 ATM) and three with anti-IFN-γ autoantibodies. An increased senescent (CD8 + CD57+) and CD21-low, disturbed transitional B (CD38 + IgM++), plasmablast B (CD38++IgM-), memory B (CD19 + CD27+) and TEMRA (CD27-IgD-) components were often observed in cross-sectional immunophenotyping and trended to develop LPD.
Primary Nodal Epstein-Barr Virus-Positive T-Cell/NK-Cell Lymphoma: Real-World Experience

Abstract:Introduction: Primary nodal Epstein-Barr virus-positive T-cell/NK-cell lymphoma (PTCL-EBV) is a disease entity newly recognized in the World Health Organization’s classification of hematolymphoid tumors, 5th edition (WHO-HAEMS5) and the International Consensus Classification of Mature Lymphoid Neoplasms (ICC). Previously, it was classified as a subtype within peripheral T-cell lymphoma, not otherwise specified, and was known to have a poor prognosis. However, the clinical features and treatment outcomes are not well known. Methods: This retrospective observational study was conducted on patients diagnosed with PTCL-EBV at Samsung Medical Center through a pathology review from 2000 to 2020. We analyzed the clinical data from 14 patients, immunohistochemistry, and survival outcomes including overall survival (OS) and progression-free survival (PFS) for each treatment regimen. PFS was defined as the time from the start of chemotherapy to the confirmation of disease progression on imaging, and hematopoietic stem-cell transplantation (HSCT) was considered a consolidation treatment. OS was defined as the time from diagnosis to the time of death. Results: 25% (1 out of 4) were beta-F1 positive, and 67% (4 out of 6) were T-cell receptor gamma (TCRγ) positive. T-cell intracellular antigen (TIA-1) and granzyme B exhibited positive results in all cases (3 out of 3), whereas the NK-cell marker CD56 was positive in only 11% of patients (1 out of 9). CD3 was observed in all of the patients (11 out of 11). The CD4 was 43% positive (3 out of 7). The CD8 was investigated in 8 patients, with 37.5% positive (3 out of 8). Hepatosplenomegaly was observed in 55% of patients (6 out of 11), and 70% (7 out of 10) of patients displayed B symptoms at the time of diagnosis. Patients who received CHOP (cyclophosphamide, doxorubicin, vincristine, prednisolone) or CVP (cyclophosphamide, vincristine, prednisolone) treatment had a median PFS of 2.2 months (95% CI: 1.9–2.5 months), and patients who received other treatments had a median PFS of 5.1 months (NA). The objective response rate (ORR) for ICE/dexa (ifosfamide, carboplatin, etoposide, dexamethasone) as the first- or second-line treatment was 100% (3 out of 3). But ORR of CHOP or CVP as the first-line treatment was 33.3% (3 out of 9). The median OS for the group that received HSCT (3 out of 11) after achieving a response was 34.6 months (95% CI: 0–74.6 months), and the median OS for the group that did not receive HSCT (8 out of 11) was 5.0 months (95% CI: 2.1–7.9 months) ( p = 0.04). Conclusions: In conclusion, in the context of PTCL-EBV, despite a limited sample size, the ICE/Dexa regimen shows potential benefits in terms of ORR and PFS. Furthermore, the application of HSCT following the attainment of a complete response may prove advantageous.

Frequent Activating Mutations of JAK-STAT Pathway Genes in Natural Killer Cell Lymphomas

Abstract:Abstract Background Natural killer cell lymphomas (NKCLs) are rare diseases with poor prognosis. There are few studies that reported oncogenic mutations in this disease. Identifying mutations critical to the neoplastic transformation of NK cells is crucial for the development of targeted therapies. Methods We performed RNA sequencing (RNA-Seq) on NKCL cases (n=17), malignant NK cell lines (n=3) and resting or activated normal NK cells (n=3) to analyze the genome-wide mutation profile in NKCLs. All SNVs detected by RNA-Seq were validated by Sanger sequencing using the corresponding genomic DNA (gDNA). We expanded our analysis to include specific sequencing of the SH2 domains of STAT3 and STAT5B using 37 additional NKCL cases and 6 NK cell lines. Retroviral or lentiviral transduction was performed to express SOCS6 or STAT3 shRNA in NK cell lines, respectively. After transduction, cells were tracked by quantifying the % of GFP+ cells. Apoptosis was assessed by quantifying AnnexinV-PE stained cells. Western blot was performed on 6 NK cell lines using p-STAT3 (Tyr705) antibody. q-MSP and q-RT-PCR were used to detect SOCS6 promoter methylation and mRNA expression in NK cell lines, respectively. Results RNA-Seq showed frequent oncogenic mutations in JAK/STAT pathway members STAT3 (18%), STAT5B (6%), BRAF (6%) and MAP2K1 (6%). Targeted Sanger sequencing of 37 additional NKCL cases showed one patient with activating STAT3 and two patients with activating STAT5B mutations leading to 7.4% (4 of 54) and 6% (3 of 50) total mutation frequency, respectively. STAT3 and STAT5B mutations were located in the SH2 domain and three of four STAT3 mutations and all STAT5B mutations were previously observed in NK- or T-LGL leukemia cases. Oncogenic activities of two other JAK-STAT pathway genes, BRAF (G469A), MAP2K1 (K57N) have been reported in solid tumors and leukemias. The JAK/STAT pathway mutations were present in a total of 53% of the NKCL cases with RNA-Seq data available (n=17). Intriguingly, targeted sequencing revealed oncogenic STAT3 mutations in 50% of malignant NK cell lines (n=6), which were associated with p-STAT3 expression detected by western blot.STAT3 knock-down resulted in reduced growth in a NK cell line with STAT3 mutation. In agreement with Kimura et. al. Leuk Lymphoma 2013, we were not able to detect JAK3 mutations as reported previously. In an accompanying DNA methylation analysis, we observed epigenetic silencing of SOCS6, a negative regulator of JAK-STAT3 signaling, in NKCL samples. Reintroduction of SOCS6 showed negative selection pressure associated with increased apoptosis in limiting concentrations of IL2 in two SOCS6-null NK cell lines with activating STAT3 mutations suggesting a possible cooperation of oncogenic JAK-STAT pathway mutations and the epigenetic silencing of a negative regulator of this pathway. Conclusions We have identified a high incidence of activating mutations of STAT3, STAT5B and other oncogenic JAK-STAT pathway genes in NKCLs. SOCS6 was frequently methylated in NKCLs with corresponding low gene expression. There was evidence suggestive of cooperation of genetic and epigenetic mechanisms in the activation of the JAK-STAT pathway in NKCLs. This study suggests that JAK-STAT pathway inhibition may be a therapeutic option in NKCLs. Disclosures: No relevant conflicts of interest to declare.

Abstract:Natural killer/T-cell lymphomas (NKTCL) represent rare and aggressive lymphoid malignancies. Patients (pts) with relapsed/refractory disease after Asparaginase (ASPA)-based chemotherapy have a dismal prognosis. To better define the role of allogeneic hematopoietic stem cell transplantation (allo-HSCT), we conducted a retrospective analysis of data shared with the European Society for Blood and Marrow Transplantation (EBMT) and cooperating Asian centers. We identified 135 pts who received allo-HSCT between 2010 and 2020. Median age was 43.4 years at allo-HSCT, 68.1% were male. Ninety-seven pts (71.9 %) were European, 38 pts (28.1%) Asian. High Prognostic Index for NKTCL (PINK) scores were reported for 44.4%; 76.3% had >1 treatment, 20.7% previous auto-HSCT, and 74.1% ASPA-containing regimens prior to allo-HSCT. Most (79.3%) pts were transplanted in CR/PR. With a median follow-up of 4.8 years, 3-year progression-free(PFS) and overall survival were 48.6% (95%-CI:39.5–57%) and 55.6% (95%-CI:46.5–63.8%). Non-relapse mortality at 1 year was 14.8% (95%-CI:9.3–21.5%) and 1-year relapse incidence 29.6% (95%-CI:21.9–37.6%). In multivariate analyses, shorter time interval (0–12 months) between diagnosis and allo-HSCT [HR = 2.12 (95%-CI:1.03–4.34); P = 0.04] and transplantation not in CR/PR [HR = 2.20 (95%-CI:0.98–4.95); P = 0.056] reduced PFS. Programmed cell death protein 1(PD-1/PD-L1) treatment before HSCT neither increased GVHD nor impacted survival. We demonstrate that allo-HSCT can achieve long-term survival in approximately half of pts allografted for NKTCL.

Abstract:Human natural killer (NK) cells are functionally regulated by killer cell immunoglobulin-like receptors (KIRs) and their interactions with HLA class I molecules. As KIR expression in a given NK cell is stochastically established, KIR repertoire perturbations reflect a dominance of discrete NK-cell subsets as the consequence of adaptation of the NK-cell compartment to exogenous agents, more often represented by virus infection. Although inhibitory interactions between KIR and their cognate HLA class I ligands abrogate effector responses of NK cells, they are also required for the functional education of NK cell. The biology and molecular specificities of the activating KIRs are less well defined, and most interactions with presumed HLA class I ligands are weak. Interestingly, epidemiologic studies link activating KIR genes to resistance against numerous virus infections. Chronic lymphoproliferative disorder of NK cells (CLPD-NK) is an indolent NK cell disease characterized by a persistent increase of circulating NK cells (usually exceeding 500 NK cells/mm(3)). The mechanism through which NK cells are induced to proliferate during CLPD-NK pathogenesis is still a matter of debate. Accumulating data suggest that exogenous agents, in particular viruses, might play a role. The etiology of CLPD-NK, however, is largely unknown. This is likely due to the fact that not a single, specific agent is responsible for the NK cells proliferation, which perhaps represents the expression of an abnormal processing of different foreign antigens, sharing a chronic inflammatory background. Interestingly, proliferating NK cells are typically characterized by expression of a restricted pattern of KIR, which have been demonstrated to be mostly represented by the activating form. This finding indicates that these receptors may be directly involved in the priming of NK cells proliferation.

Interim Analysis of a Prospective Multicenter Phase II Study for Advanced‐stage or Relapsed/refractory Natural Killer/t Cell Lymphoma

Abstract:Background: Advanced-stage or relapsed/refractory (r/r) natural killer/T cell lymphoma (NKTCL) has a poor prognosis. The anti-programmed death 1 (PD-1) antibody has demonstrated satisfactory efficacy and good tolerance in treating r/r NKTCL. In this report, we present the interim analysis results of a prospective multicenter phase II study. Methods: Patients with advanced-stage or r/r NKTCL were eligible for participation. The DAPT regimen chemotherapy was administered as the protocol treatment, which included dexamethasone 10 mg on days 1–3, azacytidine 100 mg on days 1–5, pegaspargase 3750 IU on day 1, and tislelizumab 200 mg on day 6. Cycles were repeated every 21 days. The primary endpoint was the overall response rate (ORR) after the protocol treatment. Results: A total of 29 eligible patients were enrolled, with a median age of 52 years (range, 35 to 72 years) and a male-to-female ratio of 2.2:1. Eighteen patients had newly diagnosed disease with stage IV, while 11 had r/r disease. Among 21 patients with available response, the ORR and complete response rates were 76% and 33% for the entire cohort, 85% and 39% for newly diagnosed stage IV disease, and 63% and 25% for r/r disease, respectively. Six patients underwent autologous hematopoietic stem cell transplantation. The median progression-free survival was 10.7 months (range, 1.3–20.9 months). The most common adverse events were neutropenia (17%), elevated alanine aminotransferase (13%), and decreased fibrinogen (10%). No treatment-related deaths were observed. Conclusion: The DAPT regimen chemotherapy is an effective and tolerable treatment for advanced-stage or r/r NKTCL. Abstract:Post-transplant lymphoproliferative disorders of T- or NK-cell origin (T/NK-PTLD) are rare entities and their genetic basis is unclear. We performed targeted sequencing of 465 cancer-related genes and high-resolution copy number analysis in 17 T-PTLD and 2 NK-PTLD cases. Overall, 377 variants were detected, with an average of 20 variants per case. Mutations of epigenetic modifier genes (TET2, KMT2C, KMT2D, DNMT3A, ARID1B, ARID2, KDM6B, n=11). and inactivation of TP53 by mutation and/or deletion(n=6) were the most frequent alterations, seen across disease subtypes, followed by mutations of JAK/STAT pathway genes (n=5). Novel variants, including mutations in TBX3 (n=3), MED12 (n=3) and MTOR (n=1), were observed as well. High-level microsatellite instability was seen in 1 of 14 (7%) cases, which had a heterozygous PMS2 mutation. Complex copy number changes were detected in 8 of 16 (50%) cases and disease subtype-specific aberrations were also identified. In contrast to B-cell PTLDs, the molecular and genomic alterations observed in T/NK-PTLD appear similar to those reported for peripheral T-cell lymphomas occurring in immunocompetent hosts, which may suggest common genetic mechanisms of lymphoma development.

Abstract:7563 Background: Roughly 80% of pts with peripheral T-cell lymphomas (PTCL) respond to CHOP-based therapy but 5-yr survival ranges between 20-40%. The high relapse rate after complete response (CR) suggests more sensitive response assessment, such as minimal residual disease (MRD) testing, could have prognostic or predictive value. Next generation sequencing (NGS) of the T-cell receptor (TCR) can detect a known TCR clonotype at 10 -5 . In a prospective study, we evaluated the utility of MRD testing via TCR NGS in PTCL (NCT03297697). Here we report results for pts pre- and post-autologous stem cell transplant (ASCT). Methods: This investigator-initiated, multi-institutional, prospective cohort study assessed MRD testing by TCR NGS in PTCL. Pts with previously untreated PTCL not otherwise specified (NOS), angioimmunoblastic T-cell lymphoma (AITL), anaplastic large cell lymphoma (ALCL), PTCL with T-follicular helper (TFH) phenotype, or monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL) treated with anthracycline-based curative-intent therapy were eligible. Baseline TCR (TRG or TRB) clonotypes were established from diagnostic tissue or peripheral blood if tissue was not available. TCR clonality was determined positive via the LymphoTrack TRG/TRB Assays – MiSeq (Invivoscribe, San Diego, CA) when top % reads were ≥2.5% and ≥2x above background. Samples were obtained at diagnosis (pre-treatment), following induction therapy (EOT/pre-ASCT), and serially post-ASCT. Results: Of 43 enrolled pts, 19 underwent consolidative ASCT, of which 10 had post-ASCT samples available for MRD testing (Table). Median age at ASCT was 62 years (range: 38-77). Baseline TCR clonotypes were identified in 10/10 pts. At pre-ASCT evaluation, 7/10 pts had detectable TCR MRD (‘pos’) and 3/10 pts had TCR MRD below the level of detection (‘neg‘). Of the 3 MRD neg pts pre-ASCT, 2 had CR and 1 had PR by PET-CT. All 7 TCR MRD pos pts pre-ASCT had CR by PET-CT. Of 7 MRD pos pts pre-ASCT, 6 remained MRD pos and 1 became MRD neg post-ASCT. Of the 4 MRD neg pts post-ASCT, none have relapsed at 52, 31, 34, and 31 mo post-ASCT. Of 7 MRD pos pts pre-ASCT, 3 have relapsed following ASCT (at 3, 9, and 29 mo post-ASCT). Conclusions: In this cohort of patients with PTCL receiving consolidative ASCT in CR1, all 4 pts with negative TCR MRD post-ASCT remain in remission at median follow up of 32.5 mo post-ASCT. Abstract:Background: Peripheral T-cell lymphomas (PTCLs) are uncommon and their frequency is regionally heterogeneous. Several studies have been conducted to evaluate the clinical features and treatment outcomes of this disease entity, but the majority of these were conducted in limited areas, making it difficult to comprehensively analyze their relative frequency and clinical features. Furthermore, no consensus treatment for PTCLs has been established. Therefore, we conducted an Asia-specific study to understand the relative frequency of PTCLs and assess treatments and their outcomes in Asian patients. Methods: We performed a multinational, multicenter, prospective registry of adult patients with PTCLs that was named as the International Cooperative non-Hodgkin T-cell lymphoma prospective registry study where thirty-two institutes from six Asian countries and territories (Korea, China, Taiwan, Singapore, Malaysia, and Indonesia) participated. Findings: A total of 486 patients were registered between April 2016 and February 2019, and more than a half of patients (57%) had stage III or IV. Extranodal natural killer (NK)/T- cell lymphoma was the most common subtype (n = 139,28.6%), followed by angioimmunoblastic T-cell lymphoma (AITL, n = 120,24.7%), PTCL-not otherwise specified (PTCL-NOS, n = 101,20.8%), ALK-positive anaplastic large cell lymphoma (ALCL, n = 34,6.9%), and ALK-negative ALCL (n = 30,6.2%). The median progression-free survival (PFS) and overall survival (OS) were 21.1 months (95% CI,10.6-31.6) and 83.6 months (95% CI, 56.7-110.5), respectively. Upfront use of combined treatment with chemotherapy and radiotherapy showed better PFS than chemotherapy alone in localized ENKTL whereas consolidation with upfront autologous stem cell transplantation (SCT) provided longer PFS in advance stage ENKTL. In patients with PTCLs other than ENKTL, anthracycline-containing chemotherapies were widely used, but the outcome of those regimens was not satisfactory, and upfront autologous SCT was not significantly associated with survival benefit, either. The treatment outcome of salvage chemotherapy was disappointing, and none of the salvage strategies showed superiority to one another. Interpretation: This multinational, multicenter study identified the relative frequency of each subtype of PTCLs across Asian countries, and the survival outcomes according to the therapeutic strategies currently used. Abstract:Abstract Introduction :Extranodal NK/T-cell lymphoma, nasal type (NKTCL) is an aggressive subtype of non-Hodgkin lymphoma that is characterized by extranodal presentation. Central nervous system (CNS) involvement in NKTCL is very rare and carries dismal outcome, but lacks clear consensus on optimal treatment. This study summarized the clinicopathological features, treatment patterns and survival outcomes of NKTCL patients with CNS involvement, in order to improve the understanding of CNS involvement in NKTCL. Methods: 1019 patients diagnosed with extranodal NKTCL, from August 2010 to January 2021 were reviewed, and patients with CNS involvement were included in the analysis. The CNS involvement was diagnosed either pathologically or clinically. Due to the difficulties and potential hazards of brain biopsy, pathologic confirmation of CNS involvement was conducted by identifying malignant cells in the cerebrospinal fluid (CSF) by cytology and flow cytometry analysis. Clinically diagnosed CNS involvement was based upon the characteristic radiographic findings by magnetic resonance imaging (MRI), computed tomography (CT) or positron emission tomography (PET/CT), Circulating EBV-DNA was detected by quantitative polymerase chain reaction (PCR). Overall survival (OS) was defined as time from confirmation of CNS disease to death from any cause or last follow up. Results: Among 1019 patients diagnosed with NKTCL, 14 cases (1.3%) with CNS involvement were included. The general characteristics of these patients are listed in Table 1. Of these 14 patients, the median age was 40.5 years (range, 14-57 years) at the time of diagnosis of CNS involvement. Approximately 71.4%(10/14)of patients were men. The median time between initial diagnosis of NKTCL and diagnosis of CNS involvement was 3.0 months (range, 0-141.0 months). 5 patients suffered CNS involvement at initial diagnosis of NKTCL. The other 9 CNS involvement were identified as disease progression. All patients underwent radiological examination, which disclosed the lesion location in CNS of 9 patient. The other 5 patients were diagnosed with CNS involvement by the examination of CSF. The elevated protein concentration and leukocytosis in CSF were identified in 8 and 2 patients, respectively. Among these patients with CNS involvement, the median number of treatment lines after the diagnosis of CNS involvement was 1.5 (range, 1-4). The best response was complete response (CR) in 11 patients, partial response (PR) in 1 patient, stable disease in 1 patient and progressive disease in 1 patient. CR was achieved when patients were treated with pegaspargase- containing regimens(n=4), anti-PD-1 antibodies-containing regimens(n=3), chidamide-containing regimens(n=2) and HD-MTX-containing regimens(n=2). After achieving CR, 2 patients received anti-PD-1 antibodies as maintenance treatment, with the duration of response of 26 and 72 months, respectively. With the median follow-up of 24.5 months (range, 4-72 months), 5 patients died of NKTCL, and the median OS was not reached. 9 patients presented with high circulating EBV-DNA level at diagnosis of CNS involvement, and the post-treatment EBV-DNA descended to the normal level in 7 patients, among whom 6 patients achieved CR. Conclusions: The incidence of CNS involvement in NKTCL is low. Compared with previous studies, more patients received anti-PD-1 antibodies- and chidamide-containing regimens, which may contribute to the improved response rate and survival outcomes. Measurement of dynamic changes in EBV-DNA levels during the treatment process can be applied to monitor the treatment response. Figure 1 Figure 1. Abstract:Introduction: Nodular lymphocyte-predominant Hodgkin lymphoma (NLPHL) is a rare B-cell malignancy with a paucity of malignant cells embedded in a diverse tumor microenvironment (TME). The histological composition of the TME is known to influence outcomes, with nodular B-cell rich TME (classical histology, Fan patterns A&B) associated with indolent clinical course, while T-cell infiltration or diffuse growth (variant histology, Fan patterns C-F) may resemble aggressive T/cell-histiocyte rich large B-cell lymphoma (THRLBCL) to which NLPHL can transform. Molecular features of NLPHL and THRLBCL remain to be discovered. To gain further insights into the biology of these lymphomas, we recruited NLPHL and THRLBCL cases as part of the Atlas of Blood Cancer Genomes (ABCG) initiative, a consortium consisting of 26 institutions. Design: We collected comprehensive clinicopathological, treatment and outcome data from 154 NLPHL and 21 THRLBCL patients, with centralized pathology review performed by a panel of dedicated hematopathologists. Fan patterns of the NLPHL samples were documented in all samples where possible. RNA sequencing was performed on formalin-fixed paraffin-embedded (FFPE) diagnostic tumor samples (n=129 NLPHL; n=17 THRLBCL). Tumor-infiltrating immune cell compositions were enumerated using FARDEEP with signature matrix LM22 from CIBERSORT. Results: Median age at diagnosis for the patients with THRLBCL and NLPHL was 54 years (interquartile ratio [IQR] 43-65) and 40 years (IQR 24-53), respectively. The majority of patients with NLPHL had limited stage (74%), whereas the patients with THRLBCL predominantly had advanced stage at diagnosis (95%). In both diseases, most patients were males (NLPHL, 69%; THRLBCL, 76%). Spleen involvement was present in 11 NLPHL patients (8%) and histological growth pattern was documented in 112 patients (73%). Out of those, 47 (42%) presented with variant histology and 65 with classical histology. Variant histology was enriched in NLPHL patients with splenic involvement. During a median follow-up of 6.4 years (IQR 3.8-10.1), 12 NLPHL patients and 9 THRLBCL patients died. Patients with THRLBCL had worse 10-year overall survival (OS, 31% vs 89%; p<0.001) and progression-free survival (PFS, 30% vs 66%; p=0.002) compared to patients with NLPHL. Moreover, NLPHLs with variant histology had inferior 10-year OS compared to NLPHLs with classical histology (81% vs 91%; P=0.03; Figure 1). According to in silico immunophenotyping, THRLBCL cases had the highest median proportions of M1-macrophages (11.0%) and CD8+ T-cells (17.0%), followed by NLPHLs with variant histology (M1-macrophages, 6.5%; CD8+ T-cells, 14.5%), with the lowest proportion observed in NLPHLs with classical histology (M1-macrophages, 5.2%; CD8+ T-cells, 10.9%). In contrast, the median proportions of naïve B cells followed an opposite trend across subtypes (NLPHL classical histology, 15.5%; NLPHL variant histology, 11.0%; THRLBCL, 2.0%), whereas the median proportion of CD4+ T-cells was similar across both lymphomas (NLPHL classical histology, 33.9%; NLPHL variant histology, 33.9%; THRLBCL, 32.4%). Despite differences in the overall transcriptomic landscape, gene expression patterns posited THRLBCL and NLPHL as comprising a biological continuum. THRLBCL shared more similarities with NLPHL with variant histology, while NLPHLs with classical histology exhibited upregulation of genes related to cell growth and proliferation through the MYC pathway (P<0.001). Finally, we identified a strong association between THRLBCL and upregulation of checkpoint genes (P<0.001) and gene expression related to inflammatory response (P<0.001) (Figure 2). Furthermore, NLPHLs with a high checkpoint score (above the 75th percentile) were associated with spleen involvement (P<0.001) and worse outcome (5-year PFS 61% vs 87%; P=0.02). Conclusion: Our study provides the largest comprehensive clinical and transcriptomic analyses of NLPHL and THRLBCL to-date. The findings suggest that the inferior outcomes observed in THRLBCL and in a subset of NLPHL are attributable to a higher degree of inflammation in the TME, increased expression of checkpoint genes, and a high proportion of macrophages.

Abstract:Objectives: Primary GI T-cell lymphoproliferative diseases (T-LPD) are heterogeneous entities, which raise difficult diagnosis and therapeutic challenges. We have recently provided evidences that lymphomas complicating coeliac disease (CD) arise from innate-like lymphocytes, which may carry NK receptors (NKRs). Design: NKRs expression was compared by flow cytometry in intraepithelial lymphocytes (IEL) from CD, type I or type II refractory CD (RCD). NKp46 was next assessed by immunohistochemistry in paraffin-embedded biopsies from 204 patients with CD, RCDI, RCDII or GI T-cell lymphomas and from a validation cohort of 61 patients. The cytotoxic properties of an anti-NKp46 monoclonal antibody conjugated to pyrrolobenzodiazepine (PBD) was tested ex vivo in human primary tumour cells isolated from fresh duodenal biopsies. Results: NKp46 (but not CD94, NKG2A, NKG2C, NKG2D) was significantly more expressed by malignant RCDII IEL than by normal IEL in CD and RCDI. In paraffin biopsies, detection of >25 NKp46+ IEL per 100 epithelial cells discriminated RCDII from CD and RCDI. NKp46 was also detected in enteropathy-associated T-cell lymphomas (EATL, 24/29) and in monomorphic epitheliotropic intestinal T-cell lymphomas (MEITL, 4/4) but not in indolent T-LPD (0/15). Treatment with anti-NKp46-PBD could efficiently and selectively kill human NKp46+ primary IEL ex vivo. Conclusion: NKp46 is a novel biomarker useful for diagnosis and therapeutic stratification of GI T-LPD. Strong preclinical rationale identifies anti-NKp46-PBD as a promising therapy for RCDII, EATL and MEITL.
Prevalence, clinical features, and survival outcome trends of 627 patients with primary cutaneous lymphoma over 29 years: a retrospective review from single tertiary center in Korea

Abstract:The relative frequency of primary cutaneous lymphoma (PCL) subtypes shows wide variation across different geographical regions. This retrospective study was conducted in a tertiary referral center located in Korea to describe the relative frequency, demographics, survival outcomes, and temporal trend in PCL. A total of 627 PCL cases diagnosed between January 1994 and December 2022 were included. The majority of PCL cases (87.2%) were of T-/NK-cell lineage (CTCL), while the remaining cases (12.8%) were B-cell lineage lymphomas (CBCL). The prevalence of mycosis fungoides (MF) in CTCL increased significantly over time, while other CTCL subtypes, including primary cutaneous extranodal NK/T-cell lymphoma and subcutaneous panniculitis-like T-cell lymphoma (SPTCL), decreased in frequency. Notably, the prevalence of CD4-positive small/medium T-cell lymphoproliferative disorder showed a substantial increase over time. Primary cutaneous marginal zone lymphoma was consistently the commonest CBCL subtype. Survival analysis demonstrated that CTCL had a more favorable 5-year overall survival (OS) than CBCL. OS rate of MF, SPTCL, and primary cutaneous peripheral T-cell lymphoma, NOS improved significantly over time. This study provides comprehensive insights into the dynamic change in the relative frequency and overall survival of PCL subtypes over time.
