Abstract:NO(3) can be stabilized in solid neon either by codeposition at 4.3 K of a Ne:O(2) mixture with a Ne:NO mixture that has been passed through a microwave discharge or, in higher yield, by codeposition of a Ne:NO mixture with a Ne:O(2) mixture, followed by annealing of the deposit at approximately 7 K and exposure of the solid to near ultraviolet radiation. All of the previously reported bands of NO(3) between 700 and 3000 cm(-1) were observed, most with neon-matrix shifts of less than 2.5 cm(-1). The infrared spectra of eight isotopic species of NO(3) were obtained. The observed isotopic shifts demonstrate the occurrence of extensive mixing of ground-state levels of e(') symmetry and their strong vibronic interaction with the B (2)E(') state. Photodissociation of NO(3) by irradiation of the deposit at wavelengths longer than 520 nm leads to new absorptions near the fundamentals of NO and O(2) and other new absorptions at relatively low frequencies. These absorptions were depleted and NO(3) regenerated by subsequent near ultraviolet irradiation of the deposit, suggesting the stabilization of a weakly bound NO(O(2)) complex in solid neon.

Abstract:Abstract Background Inflammatory bowel disease (IBD) patients may experience a suboptimal response to anti-tumour necrosis factor (TNF) therapy. In China, these data are limited. The EXPLORE study aimed to assess the incidence and indicators of suboptimal response to first-line anti-TNF agents in IBD patients in real-world practice in the newly industrialised countries. Methods The EXPLORE study was a multinational, retrospective chart review study. In the China subgroup, adult patients from 10 centres diagnosed with ulcerative colitis (UC) or Crohn’s disease (CD) and who initiated first anti-TNF treatment between March 2010 and March 2015, were included. The cumulative incidence (CI) of suboptimal response was assessed over 24 months since the anti-TNF initiation. The indicators of suboptimal response included: IBD-related hospitalisation, dose-escalation, discontinuation including switch to another anti-TNF, non-biologic therapy augmentation, or IBD-related surgery. Primary non-response (PNR) and secondary loss of response (SLOR) were defined as any suboptimal response indicator at <4 and ≥4 months after anti-TNF initiation, respectively. Results Overall, 287 first-line anti-TNF treated patients (35 UC; 252 CD) were included: male, UC 54.3% (n = 19), CD 74.6% (n = 188); mean (SD) age (years), UC 43.1 (14.2), CD 31.9 (11.3); median (min-max) disease duration (years), UC 1.0 (0–9), CD <1 year (0–21); median (min-max) follow-up (months), UC 27.6 (24–60), CD 40.0 (24–60). At 12 and 24 months, the CI of suboptimal response to first anti-TNF treatment was 51.4% and 75.7% in UC, 45.4% and 57.0% in CD, respectively (Figure 1). The median time to the first suboptimal response was 7.2 months in UC and 14.3 months in CD. The CI of PNR was 31.2% in UC and 33.7% in CD. The CI of SLOR was 29.4% and 64.7% in UC, 17.7% and 35.2% in CD at 12 and 24 months, respectively. The most frequent first suboptimal response indicator was ‘discontinuation of anti-TNF therapy’ in UC patients (56.3%) while for CD patients it was ‘IBD-related hospitalisation’ (56.1%) followed by ‘augmentation with non-biological therapy’ in both cohorts (UC 31.3%; CD 22.8%). Conclusion Approximately three-quarters of UC and over half of CD patients experienced a suboptimal response to their first anti-TNF agent at 24 months. Given the high unmet needs observed with anti-TNF therapies in China, IBD patients may benefit from greater optimisation of care and new biologic options.

Abstract:Abstract Background Despite advancements, understanding the tissue pharmacokinetics of Anti-tumour necrosis factor alpha monoclonal antibodies (aTNF) and addressing cases of primary non-response, secondary loss of response, and unexplained treatment failures in some patients pose ongoing challenges. The aim of this study was to investigate factors associated with intestinal tissue levels of aTNF, anti-drug antibodies and cytokines in paediatric patients with Crohn's Disease (CD). Methods A prospective study of paediatric CD patients treated with aTNF, who underwent ileocecal resection or colonoscopy between June 2020 and January 2023 was performed. Tissue drug levels of aTNF, anti-drug antibodies and pro-inflammatory cytokines ((TNF alpha (TNF-α), interleukin-17 (IL-17), interleukin-1 beta (IL-1β), interferon gamma (IFN-γ)) were analysed in tissue culture supernatant using ELISA. Logistic regression estimated odds ratios (OR) adjusted for potential confounding from disease localization, way of sampling, presence of inflammation, and type of anti TNF (aOR). Results Twenty-seven CD patients (18 females, 66.7%) with a median disease duration of 50 (Interquartile range [IQR] 23 – 69) months were analysed. Fourteen (52%) received adalimumab (ADA), thirteen (48%) received infliximab (IFX), with a median aTNF therapy duration of 17 (IQR 4.5 – 41.5) months. Tissue aTNF detection was 100% in ADA-treated and 81.8% in IFX-treated patients. A positive association between intestinal inflammation and the level of free aTNF in tissue was observed (aOR=3.03, 95% CI=0.82-5.29). The ratio of aTNF level in tissue to serum was higher in the inflamed tissue (aOR=0.21, 95% CI=0.039-0.38). Despite an adequate level of aTNF in the serum, no measurable level was detected in the tissue in 13 patients. When comparing anti-drug antibodies levels in serum and tissue, we found a notable discrepancy, with 19 patients having undetectable serum levels, while tissue levels were measurable. We found higher tissue IL-17 levels in patients who had been treated with aTNF therapy for a longer period (aOR=0.15, 95% CI=0.031-0.274), independent of disease duration and patient age. An association was found between the level of IFN-γ and the duration of follow-up, as well as with the duration of aTNF administration. However, it was not possible to determine which of these associations is responsible for this relationship. Conclusion Our study highlights significantly elevated free drug levels in inflammatory tissue compared to non-inflammatory tissue. Our findings also align with recent insights linking aTNF failure to upregulation of mucosal IL17, providing a possible molecular rationale for improved treatment response with treatment targeting the IL-23 receptor after aTNF treatment failure.

Abstract:Background: Obesity is common among patients with pediatric Crohn's disease (PCD). Some adult studies suggest obese patients respond less well to anti-TNF treatment. This study sought compares anti-TNF response and anti-TNF levels between pediatric patients with normal and high body mass index (BMI). Methods: The COMBINE trial compared anti-TNF monotherapy to combination therapy with methotrexate in patients with PCD. In this secondary analysis, a comparison of time-to-treatment failure among patients with normal BMI versus BMI Z score > 1, adjusting for prescribed anti-TNF (infliximab or adalimumab), trial treatment assignment (combination versus monotherapy), and relevant covariates. Median anti-TNF levels across BMI category was also examined. Results: Of 224 participants (162 infliximab initiators and 62 adalimumab initiators), 111 (81%) had a normal BMI and 43 (19%) had a high BMI. High BMI was associated with treatment failure among adalimumab initiators [7/10 (70%) vs 12/52 (23%), HR 0.29, p=.007] but not infliximab initiators. Additionally, adalimumab-treated patients with a high BMI had lower adalimumab levels compared to those with normal BMI (median 5.8 ug/mL vs 12.8 ug/mL, p=0.02). Infliximab trough levels did not differ between BMI groups. Conclusions: Overweight and obese patients with PCD are more likely to experience adalimumab treatment failure than those with normal BMI. Higher BMI was associated with lower drug trough levels. Standard adalimumab dosing may be insufficient for overweight children with PCD. Among infliximab initiators, there was no observed difference in clinical outcomes or drug levels, perhaps due to weight-based dosing and/or greater use of proactive drug monitoring.

Abstract:Abstract Background Anti-TNF therapy is established as a mainstay in IBD management. Recent data raise controversies regarding efficacy of these agents, sustainability of response, and identification of factors that may influence outcomes, including dose escalation and need for long-term concurrent immunosuppression (Lancet Gastro.Hep.2019;4:341). Methods We report a retrospective cohort study of 699 IBD patients on anti-TNF at our centre between 2005-2022. Clinical data were obtained from patient records and pharmacy database (TABLE 1). Primary outcomes were overall loss of response (LOR) at any timepoint; and secondary LOR (after an initial clinical response to therapy). Cox regression analysis was performed to identify variables independently affecting outcome. Kaplan-Meyer survival curves were constructed to demonstrate overall response rates (persistence on drug) and secondary LOR (Figure 1a-d). Dose adjustments were made proactively in a Virtual Clinic. Results 204 patients had a diagnosis of ulcerative colitis (UC), 481 Crohn’s Disease (CD) and 14 IBD-unclassified. 126 patients received both infliximab (IFX) and adalimumab (ADA). Median duration on IFX was 42 months (range 0-204); median duration on ADA was 48.5 months (range 0-180). 48% of patients on IFX underwent dose escalation, as did 40% on ADA. Overall, 58 patients (7.6%) remained on treatment at 120 months. Overall survival rates for IFX at 12 months were 83% (82%CD vs. 82%UC); and at 36 months were 71% (69%CD vs. 74%UC). Overall survival rates for ADA at 12 months were 84% (89%CD vs. 59%UC); and at 36 months were 72% (78% CD vs. 42% UC) (Figure 1a-b). Secondary LOR rates for IFX at 12 months were 6% (5%CD vs. 9%UC); and at 36 months were 16% (15%CD vs.19%UC). Secondary LOR rates for ADA at 12 months were 7% (5%CD vs. 19%UC); and at 36 months were 16% (12% CD vs. 42% UC) (Figure 1c-d). There was no difference in survival analysis for IFX between CD and UC. On Cox regression analysis, for IFX, immunomodulation was associated with reduction in overall LOR (HR=0.48, p=1.07e-05) but not secondary LOR (HR=0.78, p=0.31) (Figure 1e-f). For ADA, outcomes were better for CD vs. UC for both overall LOR (HR=0.35, p=3.29e-07) and secondary LOR (HR=0.35, p=9.99e-05). Conclusion We demonstrate encouraging response rates in the first three years of therapy, with a small proportion remaining on therapy beyond 120 months. Survival rates on IFX were similar for UC and CD, but for ADA were significantly lower in UC. Secondary LOR was not affected by immunomodulation. Abstract:Abstract Background Prevalence of inflammatory bowel disease (IBD) is increasing in China. The EXPLORE study evaluated the incidence and indicators of suboptimal responses to first-line anti-tumor necrosis factor (TNF) in patients with ulcerative colitis (UC) or Crohn’s disease (CD). We present results for the mainland China subgroup. Methods A retrospective chart review was performed in adults with IBD at 10 centers in mainland China who initiated anti-TNF therapy between 01 March 2010 and 01 March 2015. The cumulative incidence of suboptimal response to first-line anti-TNF therapy was assessed over 24 months using the Kaplan–Meier method. Indicators of suboptimal response were: dose escalation, discontinuation, augmentation with non-biologic therapy, or IBD-related surgery/hospitalization. At site initiation, a survey was conducted with participating physicians to identify barriers to anti-TNF use. Results Of 287 patients (72% male) examined, 16/35 (45.7%) with UC and 123/252 (48.8%) with CD experienced a suboptimal response to first-line anti-TNF therapy at any point during the observation period (median 27.6 and 40.0 months, respectively). At 1 and 2 years post anti-TNF initiation, the cumulative incidence of suboptimal response was 51.4% and 75.7% for UC and 45.4% and 57.0% for CD, respectively. Median time to first suboptimal response was 7.2 months for UC and 14.3 months for CD. The most frequent indicator of suboptimal response was discontinuation of anti-TNF therapy (9/16, 56.3%) for UC and IBD-related hospitalization for CD (69/123, 56.1%) followed by augmentation with non-biologic therapy for both cohorts (5/16, 31.3% for UC and 28/123, 22.8% for CD). Dose escalation was the least frequent indicator of suboptimal response to anti-TNF therapy (CD: 4/123, 3.3%; UC: not cited as an indicator). The cumulative incidence of suboptimal response within 4 months of first-line anti-TNF therapy (primary non-response) was over 30% in both cohorts. Financial reasons and reimbursement were identified by surveyed physicians as the most common barriers to prescribing an anti-TNF therapy. Conclusions Over one-half of patients with IBD are at risk of experiencing a suboptimal response to first-line anti-TNF therapy at 2 years post-initiation in China. Trends of Utilization of Tumor Necrosis Factor Antagonists in Children With Inflammatory Bowel Disease: A Canadian Population-Based Study

Abstract:Background: Population-based studies examining the prevalence of anti-tumor necrosis factor (anti-TNF) antagonist utilization in children and young adults with inflammatory bowel disease (IBD) are lacking. We aimed to describe the trend of anti-TNF utilization in pediatric IBD over time. Methods: Survival analyses were performed for all patients diagnosed with IBD before age 18 years in the province of Manitoba to determine the time from diagnosis to first anti-TNF prescription in different time eras (2005-2008, 2008-2012, 2012-2016). Results: There were 291 persons diagnosed with IBD (157 with Crohn's disease [CD] and 134 with ulcerative colitis [UC]) over the study period. The likelihood of being initiated on an anti-TNF by 1, 2, and 5 years postdiagnosis was 18.4%, 30.5%, and 42.6%, respectively. The proportion of persons aged <18 years utilizing anti-TNFs rose over time; in 2010, 13.0% of CD and 4.9% of UC; by 2016, 60.0% of CD and 25.5% of UC. For those diagnosed after 2012, 42.5% of CD and 28.4% of UC patients had been prescribed an anti-TNF antagonist within 12 months of IBD diagnosis. Initiating an anti-TNF without prior exposure to an immunosuppressive agent increased over time (before 2008: 0%; 2008-2012: 18.2%; 2012-2016: 42.8%; P < 0.001). There was a significant reduction in median cumulative dose of corticosteroids (CS) in the year before anti-TNF initiation (2005-2008: 4360 mg; 2008-2012: 2010 mg; 2012-2016: 1395 mg prednisone equivalents; P < 0.001). Conclusions: Over a period of 11 years, anti-TNFs are being used earlier in the course of pediatric IBD, with a parallel reduction in the cumulative CS dose.
PREDICTORS OF PRIMARY NON-RESPONSE AND DRUG DURABILITY FOR ADALIMUMAB AND INFLIXIMAB IN PEDIATRIC INFLAMMATORY BOWEL DISEASE

Abstract:Abstract BACKGROUND Anti-tumor necrosis factor (anti-TNF) therapies have become first-line therapy for children with inflammatory bowel disease (IBD). Studies describing drug durability and loss of response to anti-TNF therapies are limited in children with IBD. This study evaluates predictors of primary non-response and long-term durability in children with IBD. METHODS This was a single-center retrospective review of patients with IBD aged 4-17 years that initiated anti-TNF therapy (infliximab or adalimumab) from January 1, 2014 - December 31, 2019. Clinical and laboratory data were recorded at time of anti-TNF initiation, at 14 weeks, 12 months, and 36 months. Kaplan-Meier analysis and log-rank tests evaluated long-term durability of anti-TNF therapy. Predictors of primary non-response (discontinuation of anti-TNF within 14 weeks) and long-term durability were assessed using Cox regression analysis and time-dependent receiver operating characteristic curves identified cut points. RESULTS A total of 456 patients were initiated on anti-TNF therapy (183 adalimumab (86% CD and 14% UC/IC) and 273 infliximab (69% CD and 31% UC/IC)). Eighty one patients were lost to follow up or transferred care. Thirty seven (8%) patients were considered primary non-responders. Seven of these patients received adalimumab (3 CD, 4 UC/IC) and 30 received infliximab (7 CD, 23 UC/IC). In CD, baseline ESR >31 mm/h was predictive of primary non-response (p<0.05). In UC/IC, mono-therapy at diagnosis, baseline albumin <4 g/dL, and age at diagnosis <15.6 years were significant predictors of primary non-response. Eighty patients discontinued anti-TNF therapy between 14 weeks and 36 months. Reasons for drug discontinuation included loss of response (56%), drug reaction (24%), low therapeutic level/antibodies (19%), miscellaneous (1%). Amongst patients with CD that completed induction, week 14 lab values of albumin >3.9 g/dL, hemoglobin >11.8 g/dL, CRP <1.2 mg/dL and calprotectin <650 ug/g were associated with improved drug durability at 1 year. Patients with UC/IC had improved drug durability at 1 year if they received infliximab compared to adalimumab. The three year drug durability for both adalimumab and infliximab was slightly above 70%. Amongst patients with CD the three year drug durability was nearly 80% for both therapies. The three year drug durability with UC/IC was 37% for adalimumab and 56% for infliximab. CONCLUSIONS Less than 10% of patients are primary non-responders to anti-TNF therapies; higher baseline ESR is a predictor in patients with CD, and a lower baseline albumin and younger age are predictors for patients with UC/IC. At 1 year, patients with a lesser inflammatory burden after induction appeared to have improved drug durability. Three-year durability is >75% for patients with CD on either therapy, while the durability is less for patients with UC/IC.

Anti-TNF-α Agents in the Treatment of Crohn's Disease: A Systematic Review

Abstract:Background: Up to now, there has been no effective treatment to cure Crohn's disease (CD), the advent of anti-tumor necrosis factor-α (TNF-α) agents has brought the patients a new option. Aims: To systematically evaluate the efficacy and safety of anti-TNF-α agents in the treatment of CD. Methods: Electronic database including MEDLINE/PubMed, OVID, Elsevier ScienceDirect, ISI Web of Knowledge, Cochrane Central Register of Controlled Trials, CBM, CNKI and VIP were retrieved. Besides, the references of articles retrieved and abstracts of some important conferences were also searched manually. Only randomized controlled trials (RCT) of anti-TNF-α agent therapy for CD were included. The quality of selected articles was assessed by Jadad scale and meta-analysis was conducted using RevMan 4.2 software. Results: Fifteen RCTs articles with high quality (Jadad score ≥3) were selected for this study. Compared with placebo, anti-TNF-α agents achieved significantly higher rates of clinical response (44.9% vs. 30.0%, RR=1.39, 95% CI: 1.16-1.67, P=0.0004) and clinical remission (29.3% vs. 18.7%, RR=1.63, 95% CI: 1.26-2.09, P=0.0001). The difference in fistula closure rate between these two groups was also significant (40.6% vs. 25.2%, RR=1.74, 95% CI: 1.23-2.45, P=0.002). Subgroup analysis showed that, both in induction therapy and maintenance therapy, the clinical response rate and remission rate in treatment group were higher than those in placebo group. Rates of adverse events were similar in treatment group and placebo group (77.0% vs. 75.5%, RR=1.01, 95% CI: 0.96-1.07, P=0.57), so as the rates of serious adverse events (11.6% vs. 12.6%, RR=0.87, 95% CI: 0.73-1.04, P=0.13). Conclusions: Anti-TNF-α agents are superior to placebo in the treatment of CD and have similar risks of adverse events and serious adverse events. Long-term trials with large samples are needed for the assessment of its safety and tolerance.

Response to treatment (defined clinically and biochemically) was determined at week 142. Faecal samples were processed by GA-map®, with targeted 16S rRNA PCR sequencing. Serum samples were analysed with targeted liquid chromatography tandem mass spectrometry for bile acids and tryptophan metabolites. Multivariate and univariate analyses, as well as Spearman’s rank test, were performed. Results Two-hundred and twenty-four patients with Crohn’s disease commencing anti-TNF therapy were included (54.1% anti-TNF responders, 48.6% female, median age 37.2 (26.5-50.0) years, 56.9% receiving immunomodulator-based treatment). A significantly higher proportion of non-responders received corticosteroid treatment compared to responders at baseline (p=0.0009). Sixty-nine bacterial taxa were identified. Significantly lower abundances of Clostridium difficile and Mycoplasma were identified in responders relative to non-responders at baseline (q-value=0.046 and q-value=0.034, respectively). No significant differences in the abundances of Faecalibacterium prausnitzii, Shigella or Escherichia Coli were found (all q-values > 0.05). Eighteen serum bile acids and thirteen tryptophan metabolites were identified. Significantly higher serum abundances of cholic acid, tryptophan and 3OH-kynurenine were identified in responders at baseline (q-values=0.049, 0.033, and 0.033, respectively). Exploration of microbial–metabolite associations revealed negative correlations between secondary bile acids and several bile salt hydrolase producing bacteria, such as Bifidobacterium (r ≤ -0.50). Positive correlations were also identified between xanthurenic acid and Bifidobacterium and Roseburia intestinalis (r ≥ 0.50). Increased abundances of short-chain fatty acid producing bacteria, such as Bifidobacterium and Bacteroides vulgatus, were noted in responders at week 14. Conclusion We have identified baseline differences in the faecal microbiota and serum metabolome of anti-TNF responders and non-responders. These findings require validation in larger cohorts to determine their robustness for potential use in treatment personalisation. References 1. Abstract:Abstract Background In the last 2 decades, the introduction of biological therapies has been a significant milestone in the management of inflammatory bowel diseases. Despite the recent emergence of new biological therapies and small molecules, Anti-TNF agents still play a crucial role in treatment and still being used as first line biologic treatments rather than anti-integrins and anti-cytokines in quite a number of countries.Although many studies have been conducted previously to compare the efficacy of these drugs, there is still not a clear conclusion. In this study, we aimed to compare the persistence of infliximab (IFX) to adalimumab (ADA) in the treatment of Crohn's at a tertiary IBD center. Methods This was a retrospective cohort analysis of patients with Crohn's disease either received adalimumab or infliximab as a first line biologic treatment. A total of 411 patients were included to this analysis. Demographic information such as gender, age, montreal classification, smoking and prior medications were noted from patients electronic medical records. Weighted Kaplan-Meier and Cox models were used to assess the outcomes. Results Out of the 411 patients included in the study, 165 had used infliximab, and 246 had used adalimumab as a first-line treatment. Age and gender of the patients in both groups did not differ from each other statistically. 87% of the patients treated with adalimumab and 84% of the patients treated with infliximab were thiopurine experienced. Montreal classification differed between two groups (in IFX group, 60% of the patients had either sticturing or penetrating behaviour where as in ADA group, only 46% of the patients had either sticturing or penetrating behaviour, p<0.05). In ADA group, only 48% of the patients were receiving concomitant azathioprine treatment where as in IFX group, 60% of the patients were receiving concomitant azathioprine treatment. Primary non-response rates were not different between IFX and ADA group, 4.5% vs 4.6% respectively. In addition, clinical response rates after first year were 73.8% and 72.8% respectively. Drug persistency rates of adalimumab and infliximab in moderate to severe ulcerative colitis patients were not statistically different from each other (Figure 1). Conclusion In conclusion, it seems that there is no difference in drug persistence rates and in efficacy rates of the medications between infiximab and adalimumab treated moderate to severe Crohn's disease patients. Although disease character and concomitant treatment presence differ between two groups, these differences did not show any significant effect.

Abstract:Objectives Paediatric and adult inflammatory bowel disease (pIBD, aIBD) patients may lose response to anti‐TNF treatment within the first year. Adult‐extrapolated weight‐based dosing is incorrect in children, due to age‐related pharmacokinetic differences. We investigated biomarkers for initial and maintenance of response to infliximab (IFX) or adalimumab (ADA), comparing pIBD and aIBD patients. Methods In this prospective, observational study, pIBD (n = 24) and aIBD (n = 21) patients were included when initiating anti‐TNF. Escalation from standard dosing and continued anti‐TNF at 12 and 18 months were assessed. Biomarkers included clinical laboratory parameters, faecal calprotectin (FCP) and IFX trough levels (TLs). Plasma proteomics was performed in pIBD. Results During our study, treatment escalation (in clinical loss of response) occurred more common in pIBD versus aIBD (p = 0.02). We established that IFX therapy escalation in pIBD patients was not due to low infliximab levels. We identified 9 pro‐inflammatory proteins that were elevated in patients losing response. Conclusion Anti‐TNF exposure‐response relationship may be different in pIBD versus aIBD. No biomarkers for maintained response were identified, but 9 inflammatory proteins were of interest as potential predictors for loss of response in pIBD.

Abstract:Abstract Background Perianal Crohn's disease(PCD) significantly impacts quality of life with poor long-term prognosis. Anti-tumor necrosis factor(anti-TNF) therapy improves fistula closure rates. However, achieving permanent closure remains challenging. Our aim is to evaluate the relation between antiTNF(infliximab (IFX) and adalimumab(ADA) serum concentrations at induction(w2 and 6), and clinical and radiological outcomes at w24 and w52 Methods We conducted a single tertiary center, retrospective, cohort study including patients with an established diagnosis of PCD treated with antiTNF because of perianal activity. Variables related to their PCD(phenotype, location, fistulas type) were collected. Regarding treatment, we collected serum levels at week 2,6,24 and 52, concomitant treatment and setons presence. We defined clinical response as the absence of drainage on physical examination and clinical remission as the absence of external fistula openings. Radiological response was defined as the absence of T2 hypersignal, gadolinium enhancement, abscess and proctitis in pelvic MRI Results 65 patients were included, baseline characteristics are in Table1. None of the demographic characteristics collected were statistically significant related to clinical or radiological response although non smokers(p=0.01), ileal(p=0.02) and non-stricturing disease(p=0.01) had statistically significant higher drug levels. Taking into account the clinical response at w52, IFX mean levels at w2 were 25.8μg/mL(SD 4.1) in non responders and 30.9μg/mL(SD 14) in responders(p=0.39). At w6 they were 17.2μg/mL(SD 12.2) and 19.4μg/mL(SD 13.8) respectively(p=0.7). ADA mean levels at w2 were 13.3μg/mL(SD 7.7) in non responders and 14μg/mL(SD 6.3) in responders(p=0.87). At w6 they were 10.1μg/mL(SD 3.3) and 12μg/mL(SD 6.1) respectively(p= 0.59). For radiological response at w52 IFX mean levels at w2 were 27μg/mL(SD 15.3) in non responders and 32.7μg/mL(SD 14.5) in responders(p=0.45). At w6 the mean levels were 15.9μg/mL(SD 6.7) and 23.7μg/mL(SD 14.8) respectively(p=0.27). In ADA group the mean levels at w2 were 14.8μg/mL(SD 7.6) in responders and only one patient did not respond. At w6 ADA mean levels were 12.3μg/mL(SD 5.9) in non responders and 12.7μg/mL(SD 6.2) in responders(p=0.94). Early response at w24 was related with a long-term response at w52, 89.9% of the patients who responded at w52, had already responded at w24. Conclusion In our study we observed that almost 90% of the patients who had an early response also responded at w52, so trying to achieve an early response should be an aim in clinical practice. Despite the limited number of patients, our study shows a trend in the relationship between higher antiTNF levels and clinical and radiological response rates

Abstract:Abstract Background/Purpose Our objective was to investigate real‐world outcomes and treatment strategies in individuals affected by DADA2 using over 10‐year period real‐life experience. Methods This descriptive analysis encompassed all adult/pediatric patients with DADA2 from our Vasculitis Research Center prospective database. Patients on anti‐TNF therapy have been specifically examined, analyzing the treatment's duration, indications, and outcomes. Treatment responses were based on physicians' assessments and categorized as full response (symptom‐free with normal acute phase reactants) or partial/no response. Results Totally 32 patients (Adult/Childhood age: 19/13) were analyzed. Anti‐TNF agents were prescribed to 27 of the 32 patients. Over a median follow‐up of 58 months on anti‐TNF therapy, 10 patients (35.7%) exhibited a complete response, predominantly in cases with nervous system or skin involvement. Partial responses were observed in the other 10 patients (35.7%). Currently, 20/ 27 patients remain on anti‐TNF treatment. Among the seven who are not on anti‐TNF now: five died (four of them with a late diagnosis, one could not continue due to cardiomyopathy), one refused treatment and one had a cure after bone marrow transplantation. We have become aware that four patients increased their dose interval and one returned to the normal interval after an increase in CRP. The first patient was diagnosed in 2013 and over the last 10 years, 6/32 (18.8%) of the patients died. Conclusion Anti‐TNF treatment is beneficial for vasculitic and inflammatory lesions. The clinical course of patients is diverse, especially if the diagnosis is delayed, with a mortality rate of up to 20% over a 10‐year period.

Impact of antitumour necrosis factor therapy on surgery in inflammatory bowel disease: a population-based study

Abstract:Objective It is unclear whether widespread use of biologics is reducing inflammatory bowel disease (IBD) surgical resection rates. We designed a population-based study evaluating the impact of early antitumour necrosis factor (TNF) on surgical resection rates up to 5 years from diagnosis. Design We evaluated all patients with IBD diagnosed in Cardiff, Wales 2005–2016. The primary measure was the impact of early (within 1 year of diagnosis) sustained (at least 3 months) anti-TNF compared with no therapy on surgical resection rates. Baseline factors were used to balance groups by propensity scores, with inverse probability of treatment weighting (IPTW) methodology and removing immortal time bias. Crohn’s disease (CD) and ulcerative colitis (UC) with IBD unclassified (IBD-U) (excluding those with proctitis) were analysed. Results 1250 patients were studied. For CD, early sustained anti-TNF therapy was associated with a reduced likelihood of resection compared with no treatment (IPTW HR 0.29 (95% CI 0.13 to 0.65), p=0.003). In UC including IBD-U (excluding proctitis), there was an increase in the risk of colectomy for the early sustained anti-TNF group compared with no treatment (IPTW HR 4.6 (95% CI 1.9 to 10), p=0.001). Conclusions Early sustained use of anti-TNF therapy is associated with reduced surgical resection rates in CD, but not in UC where there was a paradoxical increased surgery rate. This was because baseline clinical factors were less predictive of colectomy than anti-TNF usage. These data support the use of early introduction of anti-TNF therapy in CD whereas benefit in UC cannot be assessed by this methodology.

Abstract:Introduction: Anti-tumour necrosis factor (TNF) therapy has greatly improved treatment outcomes in patients with inflammatory bowel disease (IBD), but long-term use is associated with cutaneous reactions, susceptibility to infections and frequent injections or hospital visits. Several non-controlled studies have demonstrated that dose reduction is feasible for a subset of patients, provided that early detection of a disease flare is possible. Here, we aim to compare the effectiveness of interval lengthening with standard dosing in maintaining remission in young patients with IBD. Methods and analysis: In this international, prospective, non-inferiority, partially randomised patient preference trial, we aim to recruit 148 patients aged 12-25 years with luminal Crohn's disease or ulcerative colitis in sustained remission (ie, three consecutive in-range faecal calprotectin (FC) results or recently confirmed endoscopic remission). In the interventional arm, the dosing interval will be lengthened from 8 to 12 weeks for infliximab users and from 2 to 3 weeks for adalimumab users. In the control group, standard dosing will be continued. Rapid tests will be performed for FC every 4 weeks and for anti-TNF trough levels every 12 weeks. The primary outcome is the cumulative incidence of out-of-range FC results at 48-week follow-up. Secondary endpoints include time to get out-of-range FC results, cumulative incidence of adverse effects, proportion of patients progressing to loss of response and identification of predictors of successful interval lengthening. Ethics and dissemination: The protocol has been approved by the Medical Ethics Review Committee of the University Medical Centre Groningen and is pending at the other participating centres. Abstract:Tumor necrosis factor-α (TNF-α) antagonists, the first biologics approved for treating patients with inflammatory bowel disease (IBD), are effective for the induction and maintenance of remission and significantly improving prognosis. However, up to one-third of treated patients show primary nonresponse (PNR) to anti-TNF-α therapies, and 23%-50% of IBD patients experience loss of response (LOR) to these biologics during subsequent treatment. There is still no recognized predictor for evaluating the efficacy of anti-TNF drugs. This review summarizes the existing predictors of PNR and LOR to anti-TNF in IBD patients. Most predictors remain controversial, and only previous surgical history, disease manifestations, drug concentrations, antidrug antibodies, serum albumin, some biologic markers, and some genetic markers may be potentially predictive. In addition, we also discuss the next steps of treatment for patients with PNR or LOR to TNF antagonists. Therapeutic drug monitoring plays an important role in treatment selection. Dose escalation, combination therapy, switching to a different anti-TNF drug, or switching to a biologic with a different mechanism of action can be selected based on the concentration of the drug and/or antidrug antibodies.

Abstract:Abstract Background Anti-TNF-α antibody treatment in Crohn's disease patients has been established, but loss of treatment response has become a problem in recent years. No studies have investigated the endoscopic findings related to loss of response particularly in the long-term treatment. In this study, we investigated the endoscopic findings associated with diminished efficacy of anti-TNF-α antibodies in Crohn's disease patients treated over a long period of time. Methods A total of 79 patients with Crohn's disease who were observed for at least 1 year after the anti-TNF-α antibody treatment was initiated at our hospital were included in this study. These patients were stratified into cases with and without loss of response to the anti-TNF-α antibody treatment, and the endoscopic findings were compared between the two groups. A case with loss of response was defined as one in which the dose was increased or the duration of treatment was shortened during the anti-TNF-α antibody treatment or the anti-TNF-α antibody preparation was changed to another due to lack of response to the initial preparation. The findings examined were aphtha, erosion, cobblestone-like mucosa, fistula, multiple ulcer, longitudinal ulcer, and anal lesion. Mann-Whitney's U test, χ2 test, and logistic regression analysis were used for statistical analysis, and differences with p < 0.05 were considered significant. Results The mean observation period for all patients was 85.5 months, and the longest period was 188 months. Of the 79 patients included, 29 met the criteria with loss of response, and 50 did not (loss of response rate 37%). Comparisons between the loss of response group and the no loss of response group by univariate analysis showed that anal lesions (66% vs 42%, p = 0.028), stenosis (75% vs 48%, p = 0.021), and multiple ulcers (82% vs 53%, p = 0.011) were significantly more frequent in the loss of response group. The multivariate analysis on these items identified anal lesions (OR: 3.06, 95% CI 1.04-9.04, p = 0.042) and multiple ulcers (OR: 5.33, 95% CI 1.59-17.9, p = 0.0068) as independent risk factors for loss of response. Conclusion In this study, Crohn's disease patients with endoscopic findings of anal lesions and multiple ulcers were prone to loss of response to the anti-TNF-alpha antibody treatment after long-term administration. This result is considered to be very useful for developing a treatment strategy for patients requiring long-term anti-TNF-α antibody treatment.
