Abstract:Abstract Isotretinoin has been a widely prescribed medication in the field of dermatology for almost 50 years. It has been heavily scrutinized because of its teratogenicity, association with mood disturbance, and now its potential for sexual dysfunction. Ophthalmic adverse sequelae, although frequent, are the subject of less scrutiny. A 22-year-old woman was referred urgently to dermatology with a 6-month history of severe nodulocystic facial acne and extensive ice pick scarring. She had not responded to a combination retinoid and benzyl peroxide cream and an oral tetracycline antibiotic. She had an intrauterine device for contraception, and her medical history was otherwise unremarkable. Her most recent eye exam was 4 years previously for a mild convergent squint. Low-dose isotretinoin was commenced in conjunction with oral erythromycin. Her dose of isotretinoin was uptitrated until control of her acne was achieved. Six weeks into treatment, a routine ocular exam revealed bilateral early cataracts. Congenital cataracts could be excluded, and no other associated risk factors could be determined. In view of the temporal relationship a potential causal association with isotretinoin was postulated. She remained on isotretinoin after a discussion regarding the risk and benefit was had. She completed a further 4 months of therapy, averaging 0.25 mg kg−1 daily, with full clearance of her acne. She had no progression of the cataracts at a 6-month ophthalmological follow-up. Despite many ocular side-effects being generally reversible and mild, there is potential for significant harm and ocular morbidity to be caused by isotretinoin. Adverse events range from surface evaporative disease and blepharoconjunctivitis more commonly, to reports of rarer events such as cataracts. Data on cataracts in the context of isotretinoin are scant and largely historical. With the added pharmacovigilance accompanying the new UK Medicines and Healthcare products Regulatory Agency’s guidance on isotretinoin, there may be an opportunity to record the impact of the ocular and mucocutaneous adverse events that have been less well studied [Commission in Human Medicines. Abstract:We consider the singular limit of a perturbed Allen–Cahn model on a bounded two-dimensional domain: $\left\{\begin{array}{@{}ll@{}} u_t = \varepsilon^2 \Delta u - 2 (u - \varepsilon a) (u^2 - 1), & x \in \Omega \subset \mathbb{R}^2 \ \partial_n u = 0, & x \in \partial \Omega \end{array} \right.$ where $\varepsilon$ is a small parameter and a is an $O(1)$ quantity. We study equilibrium solutions that have the form of a curved interface. Using singular perturbation techniques, we fully characterize the stability of such an equilibrium in terms of a certain geometric eigenvalue problem, and give a simple geometric interpretation of our stability results. Full numerical computations of the time-dependent PDE as well as of the associated two-dimensional eigenvalue problem are shown to be in excellent agreement with the analytical predictions.

Abstract:A 66-year-old patient was hospitalized after the appearance of a deep ulcer in the posterior face of the right calf. She had purpuric spots on the lower limbs only, as well as parotiditis, especially on the left side, and was affected with dryness of the mouth but not of the eyes. The patient also presented, in contact with the cold, Raynaud's phenomenon. Physical examination revealed as pathologic signs: purpura of the lower limbs, a deep ulceration on the right calf, parotiditis, and a reduction in the pallaesthesia of the lower limbs. Biologically an increased sedimentation rate, elevation of the total proteins (particularly the gammaglobulins), positive antinuclear factor, reduction of the total complement and of the C3 fraction, circulating immunocomplexes, positive latex test, positive Sia test, and the presence of mixed cryoglobulin IgG-IgM type kappa, stood out. The patient was diagnosed as having Sjögren's syndrome, mixed cryoglobulinemia and Waldenström's purpura. Probably the two latter conditions characterized by the presence or circulating immunocomplexes are part of the same process. According to recently studies described in the literature the relationships between these three entities are discussed, as well as the possibility of developing lymphoproliferative syndromes under certain immune stimuli.

Abstract:Acne vulgaris is a chronic inflammatory disease of pilosebaceous follicles commonly affecting adolescents and young adults. This disease has a profound psychological impact on affected individuals and treatment has been shown to significantly improve both self-esteem and quality of life. Isotretinoin is an effective medication used primarily in severe cystic acne patients. Over the past 30 years, this medication has revolutionized the treatment of acne. However, despite its popularity there are numerous side effects associated with its use. Most of its side effects are predictable and dose dependent, which has led to the development of variable dose regimens. Unfortunately, rare but significant side effects (e.g., depression, inflammatory bowel disease) do occur and necessitate careful monitoring to improve clinical outcomes and minimize potential adverse events.

Mean PHQ-9 and CADI scores improved significantly following isotretinoin completion compared with baseline (P < 0.05). Twenty patients (12.0%) met the criteria for moderate or severe depression at baseline. Of these, five patients were on antidepressive therapy and two patients experienced worsening of symptoms on isotretinoin. In total 11 patients (6.5%) met the criteria for moderate or severe depression at the final follow-up; nine of these had reported no or mild depressive symptoms at baseline. Logistic regression analysis did not identify potential risk factors for psychiatric adverse effects with isotretinoin, including no correlation with the average daily or total cumulative dose. We report no cases of suicidal ideation, suicide or erectile dysfunction. A single case of low libido was noted; however, symptoms were mild and did not warrant discontinuation of treatment. The mean follow-up duration after discontinuation of treatment was 209 days. Our analysis did not identify an association of isotretinoin with new-onset depressive symptoms or sexual dysfunction above the background prevalence. The findings support an overall favourable risk profile of isotretinoin at the population level, but the possibility of rare, idiosyncratic adverse events cannot be excluded.

Abstract:BACKGROUND: The link between isotretinoin, treatment of a severe form of acne, and psychiatric disorders remains controversial, as acne itself could explain the occurrence of psychiatric disorders. This study aims at assessing the disproportionality of psychiatric adverse events reported with isotretinoin in the French National PharmacoVigilance Database, compared with other systemic acne treatments and systemic retinoids.MATERIALS AND METHODS: Data were extracted from the French National PharmacoVigilance Database for systemic acne treatments, systemic retinoids and drugs used as comparators. Each report was subjected to double-blind analysis by two psychiatric experts. A disproportionality analysis was performed, calculating the number of psychiatric ADRs divided by the total number of notifications for each drug of interest.RESULTS: Concerning acne systemic treatments: all 71 reports of severe psychiatric disorders involved isotretinoin, the highest proportion of mild/moderate psychiatric adverse events was reported with isotretinoin (14.1%). Among systemic retinoids, the highest proportion of severe and mild/moderate psychiatric events occurred with isotretinoin and alitretinoin.CONCLUSION: Our study raises the hypothesis that psychiatric disorders associated with isotretinoin are related to a class effect of retinoids, as a signal emerges for alitretinoin. Complementary studies are necessary to estimate the risk and further determine at-risk populations.

Comprehensive review of reports of menstrual irregularities associated with isotretinoin

Abstract:<p>In 1982, oral isotretinoin was first licensed as a treatment option for severe recalcitrant cystic acne in the United States(McElwee, Schumacher et al. 1991). Since its introduction into the pharmaceutical market, several instances of amenorrhea in women of child-bearing age taking isotretinoin have been reported(<a class="workspace-trigger" href="#b0020">Cox 1988</a>, McElwee, Schumacher et al. 1991, Saljoughi 2017). In each documented instance, the amenorrhea spontaneously resolved once the medication was discontinued. The Patient Introductory Brochure for iPLEDGE, the risk management distribution program mandated by the U.S. Food and Drug Administration (FDA) for isotretinoin, does not currently include menstrual irregularities as a side effect of treatment; thus, patients who experience this side effect may also experience the unnecessary stress of a possible pregnancy, or, if a minor, explaining a lack of menses to their parent/guardian. This review synthesizes the limited literature available on this subject to advocate for the more widespread acknowledgment of menstrual irregularities as a side effect of isotretinoin therapy.</p><p>To perform the literature review, the search was conducted on June 13, 2020 on Pubmed using the following search terms: (((isotretinoin) AND (oligomenorrhea))) OR ((isotretinoin) AND (amenorrhea)) OR ((isotretinoin) AND (PCOS)) OR ((isotretinoin) AND (menstrual irregularities)) OR ((isotretinoin) AND (polycystic ovary syndrome)). All years available were included. Articles were excluded if they were not published in English or if the article did not address the topic of menstrual irregularities in the setting of isotretinoin, with or without the presence of PCOS. A total of 6 articles met our criteria (<a class="workspace-trigger" href="#f0005">Figure 1</a> and <a class="workspace-trigger" href="#t0005">Table 1</a>), which are detailed below.</p>
Occurrence of pregnancy and pregnancy outcomes during isotretinoin therapy

Abstract:BACKGROUND: Isotretinoin, a teratogen, is widely used to treat cystic acne. Although the risks of pregnancy during isotretinoin therapy are well recognized, there are doubts about the level of adherence with the pregnancy prevention program in Canada. Our objective was to evaluate the effectiveness of the Canadian pregnancy prevention program in 4 provinces: British Columbia, Saskatchewan, Manitoba and Ontario.METHODS: Using administrative data, we identified 4 historical cohorts of female users of isotretinoin (aged 12-48 yr) for the period 1996 to 2011. We defined pregnancy using International Statistical Classification of Diseases and billing codes. One definition included only cases with documented pregnancy outcomes (high-specificity definition); the other definition also included individuals recorded as receiving prenatal care (high-sensitivity definition). We studied new courses of isotretinoin and detected pregnancies in 2 time windows: during isotretinoin treatment only and up to 42 weeks after treatment. Live births were followed for 1 year to identify congenital malformations.RESULTS: A total of 59 271 female patients received 102 308 courses of isotretinoin. Between 24.3% and 32.9% of participants received prescriptions for oral contraceptives while they were taking isotretinoin, compared with 28.3% to 35.9% in the 12 months before isotretinoin was started. According to the high-specificity definition of pregnancy, there were 186 pregnancies during isotretinoin treatment (3.1/1000 isotretinoin users), compared with 367 (6.2/1000 users) according to the high-sensitivity definition. By 42 weeks after treatment, there were 1473 pregnancies (24.9/1000 users), according to the high-specificity definition. Of these, 1331 (90.4%) terminated spontaneously or were terminated by medical intervention. Among the 118 live births were 11 (9.3%) cases of congenital malformation. Pregnancy rates during isotretinoin treatment remained constant between 1996 and 2011.INTERPRETATION: Adherence to the isotretinoin pregnancy prevention program in Canada was poor during the 15-year period of this study.

Abstract:Abstract Isotretinoin is an effective treatment against acne vulgaris, but it also causes many side effects during and after the treatment. The relationship between the changes in the levels of plasma trace elements of patients with AV after 3 months of isotretinoin treatment and the side effects was investigated in the study. Plasma samples of 35 patients were collected before and after isotretinoin treatment. Samples were analyzed by Inductively Coupled Plasma–Mass Spectrometer. After treatment, the levels of phosphorus, magnesium, and zinc in plasma increased statistically, while the level of potassium decreased ( p < 0.05). The treatment had differing effects on zinc levels based on the gender of the individuals. With treatment, the levels of zinc in the plasma of men showed a greater increase compared to women ( p = 7.3e-04). Additionally, the correlation matrix analysis revealed a strong correlation ( R > 0.8) between magnesium and calcium. According to the study results, the change in phosphorus and potassium levels shows that isotretinoin affects kidney functions. The results suggest that phosphorus, potassium, magnesium, and zinc are associated with fatigue, dry skin and chapped lips, hair loss, and sebum secretion, respectively. Consequently, the study emphasizes the need for a comprehensive pre-treatment assessment, including monitoring of liver and kidney function as well as the levels of phosphorus and potassium in patients.

Ocular surface problems following topical medication

Abstract:Background: Adverse effects following topical medication account for a significant ocular morbidity. Toxic and allergic reactions are second in frequency among all external eye diseases (after keratokonjunctivitis sicca). Knowledge on the clinical background that predisposes to such reactions is scarce. This study aims to identify the factors that render commonly used topical medications to powerful ocular irritants. Patients and methods: 168 consecutive tertiary referrals with external eye disease problems were studied. The adverse effects on the ocular surface were analysed using a modified Wilson's classification (1983). Results: At least 39 out of 168 (24 %) patients had problems related to topical medication. Factors that predisposed to adverse reactions included a compromised ocular surface in 17 patients (43 %), long-term drug exposure in 8 (21 %), intensified treatment in 8 (21 %), concomitant acute disorders in 4 (10 %), and additive drug toxicity (> 3 medications) in 2 patients (5 %). Toxic papillary keratokonjunctivitis was the most common adverse reaction (n = 31; 80 %) and was associated in 13 patients with epithelial defects, in 2 with keratinisation of the ocular surface and in one with conjunctival scarring. Aminoglycoside antibiotics were the drugs that were most frequently involved in adverse ocular surface reactions. Conclusion: Toxic reactions are far more common than allergic ones. They are frequent in eyes with compromised surface, and after long-term or intensified therapy.
Evidence for a probable causal relationship between tretinoin, acitretin, and etretinate and intracranial hypertension

Abstract:With the recognition that vitamin A and isotretinoin may cause intracranial hypertension, the authors reviewed 331 case reports of ocular side effects associated with the three other marketed retinoids: tretinoin, acitretin, and etretinate. The reports were drawn from the National Registry of Drug-Induced Ocular Side Effects, the World Health Organization (WHO), the Food and Drug Administration, and medical journals between 1979 and 2003. There were 21 cases of intracranial hypertension associated with these three retinoids, leading to an inference that they are probably causally related to intracranial hypertension by WHO criteria. The lack of positive rechallenge data precludes the inference of a definite causal relationship to intracranial hypertension by WHO criteria. The inference of an independent causal role of these retinoids is further cautioned by the fact that six patients were concurrently using tetracycline or minocycline. Even so, the data suggest that all retinoids may, in rare instances, cause intracranial hypertension.

Abstract:Isotretinoin is a revolutionary medicine for the treatment of acne vulgaris, with new studies showing evidence of excellent clinical outcomes in treating rosacea. After 30 years of clinical experience, new insights are being gained into dosing strategies, recurrence prevention, and dose-related side effects. Previous controversial associations with inflammatory bowel diseases and mood disorders have hampered the use of this medication in some clinical situations, with new evidence disproving these claims. The teratogenicity of this medication mandates iPLEDGE compliance with government regulations. Clinicians should be knowledgeable about this medication including its side effects, teratogenicity, and its controversies in order to adequately counsel patients, dissuade fears, and obtain the best clinical outcome when treating acne and rosacea.

Abstract:Background Isotretinoin treatment for acne can reduce adverse psychiatric outcomes in adults, but there has been little investigation of the incidence of psychiatric outcomes in treated adolescents. Methods This retrospective cohort study using the Rochester Epidemiology Project identified 606 patients aged 12–18 prescribed isotretinoin over a 10‐year period between January 1, 2008 and December 31, 2017. Medical records were reviewed to identify psychiatric diagnoses before and during isotretinoin therapy, as well as psychiatric symptoms not captured by formal diagnoses and changes to isotretinoin dosing because of psychiatric diagnoses or symptoms. Results One hundred seventy‐seven (29.2%) had a psychiatric diagnosis prior to isotretinoin initiation, but 98 (16.2%) had a new psychiatric diagnosis or psychiatric symptom while taking isotretinoin. Patients with a psychiatric history were no more likely than those without to receive a new psychiatric diagnosis during treatment (4.5% vs. 3.7%; p = .650), but did experience more psychiatric symptoms, primarily low mood and mood swings (23.7% vs. 7.7%; p

Abstract:Purpose: To investigate the impact of oral isotretinoin therapy in choroidal thickness, central macular thickness (CMT), and retinal nerve fibre layer (RNFL) thickness using optical coherence tomography (OCT).Patients and methods: Choroidal thicknesses, CMT, and RNFL thickness of 64 eyes were evaluated at baseline and the end of the third month of isotretinoin therapy by spectral-domain OCT. For assessment of choroidal thickness, OCT measurements were obtained at the fovea with 6 additional measurements at adjacent locations (at 500–1000–1500 µm temporal to the fovea and 500–1000–1500 µm nasal to the fovea).Results: There was not a statistically significant difference between the baseline and third-month follow-up measurements of choroidal thicknesses at seven distinct locations (p > 0.05). Similarly, RNFL thickness and CMT did not change with a mean dose of 30 (±6) mg per day isotretinoin therapy during follow-up (101.82 vs 102.24, p = 0.079; 217.77 vs 217.25, p = 0.731, respectively).Conclusion: After the use of oral isotretinoin for 3 months, no significant side effects have been observed in choroidal thickness, CMT, and RNFL thickness by OCT.

Effect of isotretinoin on CYP2D6 and CYP3A activity in patients with severe acne

Abstract:Aims: Previously, retinoids have decreased CYP2D6 mRNA expression in vitro and induced CYP3A4 in vitro and in vivo. This study aimed to determine whether isotretinoin administration changes CYP2D6 and CYP3A activities in patients with severe acne. Methods: Thirty-three patients (22 females and 11 males, 23.5 ± 6.0 years old) expected to receive isotretinoin treatment completed the study. All participants were genotyped for CYP2D6 and CYP3A5. Participants received dextromethorphan (DM) 30 mg orally as a dual-probe substrate of CYP2D6 and CYP3A activity at two study timepoints: pre-isotretinoin treatment and with isotretinoin for at least 1 week. The concentrations of isotretinoin, DM and their metabolites were measured in 2-h postdose plasma samples and in cumulative 0-4-h urine collections using liquid chromatography-mass spectrometry. Results: In CYP2D6 extensive metabolizers, the urinary dextrorphan (DX)/DM metabolic ratio (MR) (CYP2D6 activity marker) was numerically, but not significantly, lower with isotretinoin administration compared to pre-isotretinoin (geometric mean ratio [GMR] [90% confidence interval (CI)] 0.78 [0.55, 1.11]). The urinary 3-hydroxymorphinan (3HM)/DX MR (CYP3A activity marker) was increased (GMR 1.18 [1.03, 1.35]) and the urinary DX-O-glucuronide/DX MR (proposed UGT2B marker) was increased (GMR 1.22 [1.06, 1.39]) with isotretinoin administration compared to pre-isotretinoin. Conclusions: Administration of isotretinoin did not significantly reduce CYP2D6 activity in extensive metabolizers, suggesting that the predicted downregulation of CYP2D6 based on in vitro data does not translate into humans. We observed a modest increase in CYP3A activity (predominantly CYP3A4) with isotretinoin treatment. The data also suggest that DX glucuronidation is increased following isotretinoin administration.

Abstract:Abstract Background/Aims Isotretinoin is a potent retinoid medication widely used to treat severe acne. Isotretinoin’s adverse effects range from well-documented dermatological side effects like dry skin to less common systemic manifestations. Sacroiliitis can arise from rheumatic and nonrheumatic causes. There has been limited attention in the literature to the rare occurrence of sacroiliitis as an adverse effect of isotretinoin. The precise mechanisms remain poorly understood, but the emergence of case reports suggests a potential link in immunomodulation through the alteration of cytokine balance and degradation of synovial membranes in joints. We present two patients with severe acne history who developed bilateral sacroiliitis within several weeks after starting isotretinoin Methods Case 1: A 19 year old male, started on 20 mg daily isotretinoin for acne vulgaris, experienced severe hip and lower back pain after one month, impacting his mobility. On examination, he displayed painful hip movements. Lumbar flexion was restricted and painful, with a 2 cm measurement on the Schober’s test. Medical investigation showed an elevation of erythrocyte sedimentation rate (ESR): 66mm/h, and C-reactive protein (CRP): 81 mg/l. HLA B27 was negative. X-ray radiography of the sacroiliac joint was normal, but the MRI examination showed the presence of bilateral sacroiliitis. Case 2: A 34 year old female received a six-months isotretinoin treatment for severe acne, with doses ranging from 20-40 mg daily. Then, in 2018, after a six-year gap, she received another isotretinoin course at 30 mg daily. Four months later, she began to experience gradual, intermittent lower back pain associated with 30 minutes of early morning stiffness. Her vital signs were stable, but she exhibited restricted and painful lumbar flexion. Her investigations revealed normal ESR and CRP. HLA B27 was negative. Her sacroiliac joint X-ray showed normal results, but her MRI indicated bilateral active sacroiliitis. Results Following the discontinuation of isotretinoin in both cases, NSAIDs were administered, which effectively relieved the pain. Our management of isotretinoin-induced sacroiliitis follows guidelines adapted from the ASAS-EULAR algorithm for axial spondyloarthritis. The algorithm entails an initial treatment with NSAIDs if no contraindications and to be titrated up to the maximum effective dose before considering biological/ targeted synthetic disease-modifying antirheumatic drugs. Conclusion The two cases highlight the rare but noteworthy association between isotretinoin therapy and sacroiliitis. In patients presenting with unexplained musculoskeletal symptoms during isotretinoin treatment at any point, clinicians should raise suspicions on the adverse effects of medication.
