Abstract:We evaluated the pathogenic role of Corynebacterium urealyticum in the development of encrusted pyelitis (EP) and encrusted cystitis (EC), and their clinical consequences in renal transplant recipients. During a 4-year period, we studied seven renal transplant recipients with EP and two with EC. The records of 320 other renal transplant patients studied during the same period were used as a control group. C urealyticum (> or = 10(5) CFU/ml) was isolated from 4 patients with EP (urine 3, blood 1) and from 1 patient with EC (urine). Alkaline urines with struvite crystals, microscopic hematuria, and sterile conventional urine cultures were present in all our cases. All the patients with EP developed obstructive uropathy with deterioration of the renal function and pyelonephritis (4 patients) or renal abscesses (3 patients). Chronic urinary discomfort and macroscopic hematuria were present in the 2 patients with EC. Long-term vesical and ureteral catheterization were considered the most important risk factors for the development of EC and EP, respectively. Vancomycin was successfully used in 5 cases, but all the patients required a derivative procedure or a surgical resection of the incrustations to improve. We conclude that EP and EC should be investigated in renal transplant patients who develop pyelonephritis, obstructive uropathy, or chronic urinary symptoms. EP and EC could lead to the loss of their grafts. C urealyticum appears to have a pathogenic role in these entities.

Abstract:Simulation-based training is an effective tool for acquiring practical skills, specifically to train new surgeons in a controlled and hazard-free environment, it is however important to measure participants cognitive load to decide whether they are ready to go into a real surgery. In the present study we measured performance on a surgery simulator of medical students and interns, while their brain activity was monitored by a mobile EEG device. 38 medical studentswere underwent 3 experiments undergoing a task with Simbionix simulator, while their brain activity was measured using a single-channel EEG device (Aurora by Neurosteer). On each experiment, participants performed 3 repeats of a simulator task using laparoscopic hands. The retention between tasks was different on each experiment, to examine changes in performance and cognitive load biomarkers that occur during the task or as a results of night sleep consolidation. The participants behavioral performance improved with trial repetition in all 3 experiments. In Exps. 1 & 2, the theta band activity significantly decreased with better individual performance, as exhibited by some of the behavioral measurements of the simulator. The novel VC9 biomarker (previously shown to correlate with cognitive load), exhibited a significant decrease with better individual performance shown by all behavioral measurements. In correspondence with previous research, theta decreased with lower cognitive load and higher performance and the novel biomarker, VC9, showed higher sensitivity to load changes. Together, these measurements might be for neuroimaging assessment of cognitive load while performing simulator laparoscopic tasks. This could potentially be expanded to evaluate efficacy of different medical simulations to provide more efficient training to medical staff and to measure cognitive and mental load in real laparoscopic surgeries.

Abstract:Abstract Laparoscopic surgery can be exhausting and frustrating, and the cognitive load experienced by surgeons may have a major impact on patient safety as well as healthcare economics. As cognitive load decreases with increasing proficiency, its robust assessment through physiological data can help to develop more effective training and certification procedures in this area. We measured data from 31 novices during laparoscopic exercises to extract features based on cardiac and ocular variables. These were compared with traditional behavioural and subjective measures in a dual-task setting. We found significant correlations between the features and the traditional measures. The subjective task difficulty, reaction time, and completion time were well predicted by the physiology features. Reaction times to randomly timed auditory stimuli were correlated with the mean of the heart rate ( $$r = - 0.29$$ r = - 0.29 ) and heart rate variability ( $$r = 0.4$$ r = 0.4 ). Completion times were correlated with the physiologically predicted values with a correlation coefficient of 0.84. We found that the multi-modal set of physiology features was a better predictor than any individual feature and artificial neural networks performed better than linear regression. The physiological correlates studied in this paper, translated into technological products, could help develop standardised and more easily regulated frameworks for training and certification.

Machine learning-based analysis of operator pupillary response to assess cognitive workload in clinical ultrasound imaging

Abstract:Introduction: Pupillometry, the measurement of eye pupil diameter, is a well-established and objective modality correlated with cognitive workload. In this paper, we analyse the pupillary response of ultrasound imaging operators to assess their cognitive workload, captured while they undertake routine fetal ultrasound examinations. Our experiments and analysis are performed on real-world datasets obtained using remote eye-tracking under natural clinical environmental conditions. Methods: Our analysis pipeline involves careful temporal sequence (time-series) extraction by retrospectively matching the pupil diameter data with tasks captured in the corresponding ultrasound scan video in a multi-modal data acquisition setup. This is followed by the pupil diameter pre-processing and the calculation of pupillary response sequences. Exploratory statistical analysis of the operator pupillary responses and comparisons of the distributions between ultrasonographic tasks (fetal heart versus fetal brain) and operator expertise (newly-qualified versus experienced operators) are performed. Machine learning is explored to automatically classify the temporal sequences into the corresponding ultrasonographic tasks and operator experience using temporal, spectral, and time-frequency features with classical (shallow) models, and convolutional neural networks as deep learning models. Results: Preliminary statistical analysis of the extracted pupillary response shows a significant variation for different ultrasonographic tasks and operator expertise, suggesting different extents of cognitive workload in each case, as measured by pupillometry. The best-performing machine learning models achieve receiver operating characteristic (ROC) area under curve (AUC) values of 0.98 and 0.80, for ultrasonographic task classification and operator experience classification, respectively. Conclusion: We conclude that we can successfully assess cognitive workload from pupil diameter changes measured while ultrasound operators perform routine scans. The machine learning allows the discrimination of the undertaken ultrasonographic tasks and scanning expertise using the pupillary response sequences as an index of the operators' cognitive workload. A high cognitive workload can reduce operator efficiency and constrain their decision-making, hence, the ability to objectively assess cognitive workload is a first step towards understanding these effects on operator performance in biomedical applications such as medical imaging.
Evaluation of cognitive load and emotional states during multidisciplinary critical care simulation sessions

Abstract:Background The simulation in critical care setting involves a heterogeneous group of participants with varied background and experience. Measuring the impacts of simulation on emotional state and cognitive load in this setting is not often performed. The feasibility of such measurement in the critical care setting needs further exploration. Methods Medical and nursing staff with varying levels of experience from a tertiary intensive care unit participated in a standardised clinical simulation scenario. The emotional state of each participant was assessed before and after completion of the scenario using a validated eight-item scale containing bipolar oppositional descriptors of emotion. The cognitive load of each participant was assessed after the completion of the scenario using a validated subjective rating tool. Results A total of 103 medical and nursing staff participated in the study. The participants felt more relaxed (−0.28±1.15 vs 0.14±1, P<0.005; d=0.39), excited (0.25±0.89 vs 0.55±0.92, P<0.005, d=0.35) and alert (0.85±0.87 vs 1.28±0.73, P 0.05). Conclusion A well-designed complex high fidelity critical care simulation scenario can be evaluated to identify the relative cognitive load of the participants’ experience and their emotional state. The movement of learners emotionally from a more negative state to a positive state suggests that simulation can be an effective tool for improved knowledge transfer and offers more opportunity for dynamic thinking.

Abstract:Simulation &Gaming, Ahead of Print. BackgroundRecent advances in non-invasive physiologic monitoring leverage machine learning to provide unobtrusive, real-time assessments of a learner's cognitive load (CL) as they engage in specific tasks. However, the performance characteristics of these novel composite physiologic CL measures are incompletely understood.ObjectivesWe aimed to 1) explore the feasibility of measuring CL in real time using physiologically-derived inputs; 2) evaluate the performance characteristics of a novel composite CL measure during simulated virtual reality resuscitations; and 3) understand how this measure compares to traditional, self-reported measures of CL.MethodsNovice (PGY1-2 pediatric residents) and expert (pediatric emergency medicine fellows and attendings) participants completed four virtual reality simulations as team leader. The scenario content (status epilepticus versus anaphylaxis) and level of distraction (high versus low) were manipulated. Cognitive load was measured in all participants using electroencephalography and electrocardiography data ("real-time CL") as well as through self-report (NASA-TLX). Scenario performance also was measured.ResultsComplete data were available for 6 experts and 6 novices. Experts generally had lower CL than novices on both measures. Both measures localized the most significant differences between groups to the anaphylaxis scenarios (real-time CL [low-distraction] Cohen's d -1.33 [95% CI -.2.56, -0.03] and self-reported CL [high-distraction] Cohen's d -1.41 [95% CI -2.67, -0.10]). No consistent differences were seen with respect to level of distraction. Performance was similar between the two groups, though both exhibited fewer errors over time (F(3,48) = 5.75, p = .002).ConclusionIt is feasible to unobtrusively measure cognitive load in real time during virtual reality simulations. There was convergence between the two CL measures: in both, experts had lower CL than novices, with the most significant effect size differences in the more challenging anaphylaxis scenarios.

Abstract:Summary The identification of novel targets for controlling the sheep blowfly is an important process for discovering new generation compounds that are safer and more environmentally friendly. Experiments in our laboratory have indicated a role for serine proteases in influencing egg hatching in the sheep blowfly. Isolation of the proteases involved using benzamidine affinity chromatography identified a group of molecules with homology to both trypsin and chymotrypsin-like proteases. These proteases represent potential new targets for controlling egg hatching in the sheep blowfly.

Abstract:Background. Problem solving and critical thinking are top future skills. High-fidelity simulations improve critical thinking, but also increase students’ cognitive load, possibly limiting their learning. Educators should therefore consider learning outcomes, problems that require critical thinking, the relationship between working and long-term memory, and intrinsic and extraneous cognitive loads when developing simulation scenarios. Objective. To report on the application of cognitive load theory (CLT) and students’ responses in terms of problem solving and new insights during and after a simulation experience. Methods. A high-fidelity simulation, targeting multilevel communication, teamwork and prioritisation of learning outcomes, was designed according to CLT. Eighty students participated in presimulation knowledge, skills and attitudes acquisition and 10 participated in the simulation. A qualitative descriptive design was followed and data were collected through video/audio recordings of the simulation and reflection session, supported by educator and critical observer notes. Qualitative content analysis allowed comprehensive summarisation of students’ problem-solving abilities and emerging new insights. Results. Eighty second-year radiography students formed the target population, with 10 simulation participants comprising the sample. Communication with health professionals was good, but lacking towards patients. Intraprofessional team collaboration was suboptimal, but interprofessional team collaboration was good. Students were mostly unfamiliar with the prioritisation responsibility. Upon reflection, students came to new insights regarding teamwork and prioritisation. Conclusion. After application of CLT, critical thinking to facilitate problem solving during simulation was suggested and post-simulation reflection facilitated new insights. The exposure of large groups to the benefits of simulation validates further investigation.

Abstract:Abstract Background Cognitive load theory (CLT) uses an understanding of brain architecture for educational design, with implications for simulation. Since working memory is limited, minimising extraneous cognitive load improves learning of new tasks (intrinsic load) and use of existing knowledge (germane load). This study evaluates the effectiveness of low-fidelity simulation (LFS) utilising CLT principles in the assessment and management of the deteriorating patient (AMDP). Method CLT design principles informed the choice of LFS and simulation design. The self-rated ability of 13 undergraduate nurses across seven aspects of AMDP was measured in a pre-post design. Results Self-rated ability increased from 2.98 (SD = 0.19) to 4.47 (SD = 0.12) (p < 0.001), with improvements across all AMDP aspects. Conclusion This study indicates that CLT informed design has benefits for simulation. LFS may be preferred to high fidelity simulation for AMDP teaching and medical simulation for novice learners.
Identification of Cognitive Workload during Surgical Tasks with Multimodal Deep Learning

Abstract:The operating room (OR) is a dynamic and complex environment consisting of a multidisciplinary team working together in a high take environment to provide safe and efficient patient care. Additionally, surgeons are frequently exposed to multiple psycho-organisational stressors that may cause negative repercussions on their immediate technical performance and long-term health. Many factors can therefore contribute to increasing the Cognitive Workload (CWL) such as temporal pressures, unfamiliar anatomy or distractions in the OR. In this paper, a cascade of two machine learning approaches is suggested for the multimodal recognition of CWL in four different surgical task conditions. Firstly, a model based on the concept of transfer learning is used to identify if a surgeon is experiencing any CWL. Secondly, a Convolutional Neural Network (CNN) uses this information to identify different degrees of CWL associated to each surgical task. The suggested multimodal approach considers adjacent signals from electroencephalogram (EEG), functional near-infrared spectroscopy (fNIRS) and eye pupil diameter. The concatenation of signals allows complex correlations in terms of time (temporal) and channel location (spatial). Data collection was performed by a Multi-sensing AI Environment for Surgical Task & Role Optimisation platform (MAESTRO) developed at the Hamlyn Centre, Imperial College London. To compare the performance of the proposed methodology, a number of state-of-art machine learning techniques have been implemented. The tests show that the proposed model has a precision of 93%.

Abstract:Abstract Cognitive load is a key mediator of cognitive processing that may impact clinical reasoning performance. The purpose of this study was to gather biologic validity evidence for correlates of different types of self-reported cognitive load, and to explore the association of self-reported cognitive load and physiologic measures with clinical reasoning performance. We hypothesized that increased cognitive load would manifest evidence of elevated sympathetic tone and would be associated with lower clinical reasoning performance scores. Fifteen medical students wore Holter monitors and watched three videos depicting medical encounters before completing a post-encounter form and standard measures of cognitive load. Correlation analysis was used to investigate the relationship between cardiac measures (mean heart rate, heart rate variability and QT interval variability) and self-reported measures of cognitive load, and their association with clinical reasoning performance scores. Despite the low number of participants, strong positive correlations were found between measures of intrinsic cognitive load and heart rate variability. Performance was negatively correlated with mean heart rate, as well as single-item cognitive load measures. Our data signify a possible role for using physiologic monitoring for identifying individuals experiencing high cognitive load and those at risk for performing poorly during clinical reasoning tasks.

O160: Novel Monitoring of General Surgeons' Intraoperative Cognitive Load following Surgical Sabermetrics Principles

Abstract:Abstract Introduction Surgeons strive to continually enhance performance. Currently, assessment relies on subjective, retrospective methods-hindering real-time improvement and error prevention. Learning from sports, surgeons can utilise surgical sabermetrics for data-driven analysis. Cognitive load(CogL) impacts performance, with overload impairing skills and judgement. Sabermetrics can be leveraged to measure CogL objectively(e.g.Electrodermal activity(EDA)) and subjectively. This study aimed to combine these measures and explore their relationship with surgeon factors and patient outcomes. Methods Fourteen surgeons conducting 58 benign surgery cases participated. Surgeons’ intraoperative EDA was captured using a digital sensor. Subjective data collected included (i)Surgery Task-Load-Index(SURG-TLX)), (ii)mood, (iii)sleep-disturbance and (iv)risk-aversion. EDA underwent decomposition into (a)tonic (background autonomic arousal) and (b) phasic(response to events) components. Patient outcomes included (i)30-day morbidity and (ii)case difficulty. Results Overall, mean phasic-EDA was 0.038μS and mean tonic-EDA was 0.483μS(ranges 0-1:1 indicating highest CogL). Mean SURG-TLX was 47.128(/120, where 120 is the highest workload). Negative mood related to changes in tonic-EDA(p<0.05,F=11.84). EDA climbed throughout each case (Tonic:p<0.05,F=2.035;Phasic:p<0.05,F=1.626). SURG-TLX were weakly correlated with sleep(p<0.05,ρ=-0.273) and moderately correlated with case duration(p<0.05, ρ=0.598), but not EDA(p>0.50). Discussion This study is the first in the UK to report combined CogL measures gathered during live surgery. Awareness of factors that affect CogL crucially allows for the management of factors to prevent overload and to inform targeted interventions for performance improvement. Using digital sensors to measure CogL provides opportunity for automated, real-time performance assessment, advanced feedback, and behavioural modifications.

Abstract:The aptitude-oriented exercises from almost all domains impose cognitive load on their operators. Evaluating such load poses several challenges owing to many factors like measurement mode and complexity, nature of the load, overloading conditions, etc. Nevertheless, the physiological measurement of a specific genre of cognitive load and subjective measurement have not been reported along with each other. In this study, the electroencephalography (EEG)-driven machine learning (Support Vector Machine (SVM)) model is sought along with the support of NASA's Task Load Index (NASA-TLX) rating scale for a novel purpose in workload exploration of operators. The Cognitive Load Theory (CLT) was used as the foundation to design the intrinsic stimulus (Spot the Difference task), as most workloads operators are exposed to are notably intrinsic. The SVM-based three-level classification accuracy ranged from 85.4 to 97.4% ( p < 0.05), and the NASA-TLX-based three-level classification accuracy ranged from 88.33 to 97.33%. The t- test results show that the neurometric indices contributing to the classification significantly differed ( p < 0.05) for every level. The NASA-TLX scale was utilised for validation in its basic form after the validity (Pearson correlation coefficients 0.338 to 0.805 ( p < 0.05)) and reliability (Cronbach's α = 0.753) test. This modeling is beneficial to phase out particular-level cognitive exercises from the curriculum during under or overload workload (critical) conditions.

Abstract:Abstract Background Ultrasound is a safe and effective diagnostic tool used within several specialties. However, the quality of ultrasound scans relies on sufficiently skilled clinician operators. The aim of this study was to explore the validity of automated assessments of upper abdominal ultrasound skills using an ultrasound simulator. Methods Twenty five novices and five experts were recruited, all of whom completed an assessment program for the evaluation of upper abdominal ultrasound skills on a virtual reality simulator. The program included five modules that assessed different organ systems using automated simulator metrics. We used Messick’s framework to explore the validity evidence of these simulator metrics to determine the contents of a final simulator test. We used the contrasting groups method to establish a pass/fail level for the final simulator test. Results Thirty seven out of 60 metrics were able to discriminate between novices and experts ( p < 0.05). The median simulator score of the final simulator test including the metrics with validity evidence was 26.68% (range: 8.1–40.5%) for novices and 85.1% (range: 56.8–91.9%) for experts. The internal structure was assessed by Cronbach alpha (0.93) and intraclass correlation coefficient (0.89). The pass/fail level was determined to be 50.9%. This pass/fail criterion found no passing novices or failing experts. Conclusions This study collected validity evidence for simulation-based assessment of upper abdominal ultrasound examinations, which is the first step toward competency-based training. Future studies may examine how competency-based training in the simulated setting translates into improvements in clinical performances.

Abstract:The total amount of mental activity applied to working memory at a given point in time is called cognitive load, which is an important factor in various activities in daily life. We have proposed new feature quantities that reflect the time-series changes in the power of typical frequency bands in electroencephalogram (EEG) for use in examining the relationship between brain activity and behavior under cognitive load. We also measured heart rate variability (HRV) and spontaneous skin conductance responses (SCR) to examine functional associations among brain activity, autonomic activity, and behavior under cognitive load. Additionally, we applied our machine learning model previously developed using EEG to the estimation of arousal level to interpret the brain–autonomic–behavior functional association under cognitive load. Experimental data from 12 healthy undergraduate students showed that participants with higher levels of infra-slow fluctuations of alpha power have more cognitive resources and thus can process information under cognitive load more efficiently. In addition, HRV reflecting parasympathetic activity correlated with task accuracy. The arousal level estimated using our machine learning model showed its robust relationship with EEG. Despite the limitation of the sample size, the results of this pilot study suggest that the information processing efficiency of the brain under cognitive load is reflected by time-series fluctuations in EEG, which are associated with an individual's task performance. These findings can contribute to the evaluation of the internal state of humans associated with cognitive load and the prediction of human behaviors in various situations under cognitive load.

Classification of Cognitive Load and Expertise for Adaptive Simulation using Deep Multitask Learning

Abstract:Simulations are a pedagogical means of enabling a risk-free way for healthcare practitioners to learn, maintain, or enhance their knowledge and skills. Such simulations should provide an optimum amount of cognitive load to the learner and be tailored to their levels of expertise. However, most current simulations are a one-type-fits-all tool used to train different learners regardless of their existing skills, expertise, and ability to handle cognitive load. To address this problem, we propose an end-to-end framework for a trauma simulation that actively classifies a participant's level of cognitive load and expertise for the development of a dynamically adaptive simulation. To facilitate this solution, trauma simulations were developed for the collection of electrocardiogram (ECG) signals of both novice and expert practitioners. A multitask deep neural network was developed to utilize this data and classify high and low cognitive load, as well as expert and novice participants. A leave-one-subject-out (LOSO) validation was used to evaluate the effectiveness of our model, achieving an accuracy of 89.4% and 96.6% for classification of cognitive load and expertise, respectively.

Learning Challenges, Teaching Strategies, and Cognitive Load: Insights From the Experience of Seasoned Endoscopy Teachers

Abstract:Purpose: Learners of medical procedures must develop, refine, and apply schemas for both cognitive and psychomotor constructs, which may strain working memory capacity. Procedures with limitations in visual and tactile information may add risk of cognitive overload. The authors sought to elucidate how experienced procedural teachers perceived learners' challenges and their own teaching strategies in the exemplar setting of gastrointestinal endoscopy. Method: The authors interviewed 22 experienced endoscopy teachers in the United States, Canada, and the Netherlands between May 2016 and March 2019 and performed thematic analysis using template analysis method. Interviews addressed learner challenges and teaching strategies from the teacher participants' perspectives. Cognitive load theory informed data interpretation and analysis. Results: Participants described taking steps to "diagnose" trainee ability and identify struggling trainees. They described learning challenges related to trainees (performance over mastery goal orientation, low self-efficacy, lack of awareness), tasks (psychomotor challenges, mental model development, tactile understanding), teachers (teacher-trainee relationship, inadequate teaching, teaching variability), and settings (internal/external distractions, systems issues). Participants described employing strategies that could match intrinsic load to learners' levels (teaching along developmental continuum, motor instruction, technical assistance/takeover), minimize extraneous load (optimize environment, systems solutions, emotional support, define expectations), and optimize germane load (promote mastery, teach schemas, stop and focus). Conclusions: Participants provided insight into possible challenges while learning complex medical procedures with limitations in sensory channels, as well as teaching strategies that may address these challenges at individual and systems levels. Using cognitive load theory, the authors provide recommendations for procedural teachers.
An Integrated Electroencephalography and Eye-Tracking Analysis Using eXtreme Gradient Boosting for Mental Workload Evaluation in Surgery

Abstract:Objective: We aimed to develop advanced machine learning models using electroencephalogram (EEG) and eye-tracking data to predict the mental workload associated with engaging in various surgical tasks. Background: Traditional methods of evaluating mental workload often involve self-report scales, which are subject to individual biases. Due to the multidimensional nature of mental workload, there is a pressing need to identify factors that contribute to mental workload across different surgical tasks. Method: EEG and eye-tracking data from 26 participants performing Matchboard and Ring Walk tasks from the da Vinci simulator and the pattern cut and suturing tasks from the Fundamentals of Laparoscopic Surgery (FLS) program were used to develop an eXtreme Gradient Boosting (XGBoost) model for mental workload evaluation. Results: The developed XGBoost models demonstrated strong predictive performance with R2 values of 0.82, 0.81, 0.82, and 0.83 for the Matchboard, Ring Walk, pattern cut, and suturing tasks, respectively. Key features for predicting mental workload included task average pupil diameter, complexity level, average functional connectivity strength at the temporal lobe, and the total trajectory length of the nondominant eye's pupil. Integrating features from both EEG and eye-tracking data significantly enhanced the performance of mental workload evaluation models, as evidenced by repeated-measures t-tests yielding p-values less than 0.05. However, this enhancement was not observed in the Pattern Cut task (repeated-measures t-tests; p > 0.05). Conclusion: The findings underscore the potential for machine learning and multidimensional feature integration to predict mental workload and thereby improve task design and surgical training. Application: The advanced mental workload prediction models could serve as instrumental tools to enhance our understanding of surgeons' cognitive demands and significantly improve the effectiveness of surgical training programs.

Abstract:<h3>Background</h3><p>Virtual reality simulators combined with head-mounted displays enable highly immersive virtual reality (VR) for surgical skills training, potentially bridging the gap between the simulation environment and real-life operating room conditions. However, the increased complexity of the learning situation in immersive VR could potentially induce high cognitive load thereby inhibiting performance and learning. This study aims to compare cognitive load and performance in immersive VR and conventional VR simulation training.</p><h3>Methods</h3><p>A randomized controlled trial of residents (<em>n</em> = 31) performing laparoscopic salpingectomies with an ectopic pregnancy in either immersive VR or conventional VR simulation. Cognitive load was estimated by secondary-task reaction time at baseline, and during nonstressor and stressor phases of the procedure. Simulator metrics were used to evaluate performance.</p><h3>Results</h3><p>Cognitive load was increased by 66% and 58% during immersive VR and conventional VR simulation, respectively (<em>p</em> </p><h3>Conclusion</h3><p>Immersive VR simulation training induces a higher cognitive load and results in a poorer performance than conventional VR simulation training in laparoscopy. High extraneous load and element interactivity in the immersive VR are suggested as mechanisms explaining this finding. However, immersive VR offers some potential advantages over conventional VR such as more real-life conditions but we only recommend introducing immersive VR in surgical skills training after initial training in conventional VR.</p>
