Abstract:Objective To explore the clinical effects of internal fixation with plate by the improved anterolateral approach to treatment of terminal femoral fractures.Methods Internal fixation with plate by improved anterolateral approach was applied to 78 patients with complex comminuted bone fractures of terminal femora from August 2008 to January 2012.Results During the follow-up of 10 months to 2 years,73 patients reached complete healing without screw relaxation,plate rupture and malformation(e.g.limb shortening and knee varus or eversion);the other 5 ones who suffered from nonunion reached complete healing by the second stage bone grafting.The excellent and good rate of knee joint function was up to 91 %.Conclusion With simple procedure,reliable fixation,integral protection of knee extensor device,mini-operative damage and propitiousness to functional recovery of knee joint,the internal fixation with plate by the improved anterolateral approach is a relatively good therapeutic approach to the complex comminuted bone fractures of terminal femora.
Biomechanical testing of a computationally optimized far cortical locking plate versus traditional implants for distal femur fracture repair

Abstract:Background This study experimentally validated a computationally optimized screw number and screw distribution far cortical locking distal femur fracture plate and compared the results to traditional implants. Methods 24 artificial femurs were osteotomized with a 10 mm fracture gap 60 mm proximal to the intercondylar notch. Three fixation constructs were used. (i) Standard locking plates secured with three far cortical locking screws inserted according to a previously optimized distribution in the femur shaft ( n = 8). (ii) Standard locking plates secured with four standard locking screws inserted in alternating plate holes in the femur shaft (n = 8). (iii) Retrograde intramedullary nail secured proximally with one anterior-posterior screw and distally with two oblique screws (n = 8). Axial hip forces (700 and 2800 N) were applied while measuring axial interfragmentary motion, shear interfragmentary motion, and overall stiffness. Findings Experimental far cortical locking plate results compared well to published computational findings. Far cortical locking femurs contained the highest axial motion within the potential ideal range of 0.2–1 mm and a sheer-to-axial motion ratio < 1.6 at toe-touch weight-bearing (700 N). At full weight-bearing (2800 N), Standard locking-plated femurs had the only axial motion within 0.2–1 mm but had an excess shear-to-axial motion ratio. Nail-implanted femurs underperformed at both forces. Interpretation For toe-touch weight-bearing, the far cortical locking construct provided optimal biomechanics to allow moderate motion, which has been suggested to encourage early callus formation. Conversely, at full weight-bearing, the standard locking construct offered the biomechanical advantage on fracture motion.

Abstract:Joint-preserving surgical treatment of complex unstable proximal humerus fractures remains challenging, with high failure rates even following state-of-the-art locked plating. Enhancement of implants could help improve outcomes. By overcoming limitations of conventional biomechanical testing, finite element (FE) analysis enables design optimization but requires stringent validation. This study aimed to computationally enhance the design of an existing locking plate to provide superior fixation stability and evaluate the benefit experimentally in a matched-pair fashion. Further aims were the evaluation of instrumentation accuracy and its potential influence on the specimen-specific predictive ability of FE. Screw trajectories of an existing commercial plate were adjusted to reduce the predicted cyclic cut-out failure risk and define the enhanced (EH) implant design based on results of a previous parametric FE study using 19 left proximal humerus models (Set A). Superiority of EH versus the original (OG) design was tested using nine pairs of human proximal humeri ( N = 18, Set B). Specimen-specific CT-based virtual preoperative planning defined osteotomies replicating a complex 3-part fracture and fixation with a locking plate using six screws. Bone specimens were prepared, osteotomized and instrumented according to the preoperative plan via a standardized procedure utilizing 3D-printed guides. Cut-out failure of OG and EH implant designs was compared in paired groups with both FE analysis and cyclic biomechanical testing. The computationally enhanced implant configuration achieved significantly more cycles to cut-out failure compared to the standard OG design ( p < 0.01), confirming the significantly lower peri-implant bone strain predicted by FE for the EH versus OG groups ( p < 0.001). The magnitude of instrumentation inaccuracies was small but had a significant effect on the predicted failure risk ( p < 0.01). The sample-specific FE predictions strongly correlated with the experimental results (R 2 = 0.70) when incorporating instrumentation inaccuracies. These findings demonstrate the power and validity of FE simulations in improving implant designs towards superior fixation stability of proximal humerus fractures. Computational optimization could be performed involving further implant features and help decrease failure rates. The results underline the importance of accurate surgical execution of implant fixations and the need for high consistency in validation studies.

Aspirin and prostacyclin treatment of diabetic dogs.

Abstract:Spontaneously diabetic dogs were treated initially (the first 1/2 yr) with i.v. prostacyclin and oral aspirin and then for 1 yr with varying doses of aspirin alone. At this time cataracts prevented visualization of retinae, and one eye of each animal was removed for examination of the vasculature of the trypsin digested retinae. Studies of the blood during the course of treatment showed an elevation of plasma cyclic adenylic acid and marked lowering of inordinately high platelet count and platelet aggregation. Treatment was stopped and after an additional 1/2 year the animals were euthanized and the fellow eye removed for retinal study. Although just three diabetic animals survived for binocular retinal histology, there were no discernible differences in the retinal vasculature in the fellow eyes of a given animal, and in only one of these three animals were there clear signs of diabetic retinopathy. We must conclude, therefore, that in spite of relative normalization of platelet function in the parameters measured during the course of treatment, we cannot yet claim a salutary effect of treatment on the diabetic retina. Cataracts prevented us from having better information on diabetic retinae prior to treatment and a redesigned study would have to include unilateral histological retinal examination before any treatment is started to determine if, indeed, improved platelet physiology slows or reverses changes in the diabetic retina.
Finite element analysis of a novel anatomical plate in posterolateral plateau fractures

Abstract:Objective: This study aims to analyze the biomechanical characteristics of posterolateral plateau fractures fixed by a novel anatomical plate using finite element analysis. Methods: A three-dimensional digital model of the full length of right tibiofibula was obtained by CT scanning. A posterolateral tibial plateau fracture model was then created. The acquired fracture model was assembled with 4 groups of internal fixations: Group A, novel anatomical plate; Group B, straight buttress plate; Group C, oblique T-shaped locking plate; Group D, two lag screws. Axial loads of 500, 1,000 and 1,500 N perpendicular to the horizontal plane were used to simulate the stress on the lateral plateau of a 65 kg person standing, walking and fast running. Results: Vertical displacements of the posterolateral fragments in each of the four groups gradually increased under loads from 500 N to 1,500 N. The maximum displacement of the fracture fragment in four groups were all located on the lateral side of the proximal part, and the displacement gradually decreased from the proximal part to the distal end. The maximum displacement values under the axial load of 1,500 N was in the following order: novel anatomical plate (1.2365 mm) < oblique T-shaped locking plate (1.314 mm) < two lag screws (1.3747 mm) < straight buttress plate (1.3932 mm). As the axial load increased, the stress value of the different internal fixation models gradually increased. The stress behavior of the same internal fixation model under different loads was similar. The maximum stress value under the axial load of 1,500 N was in the following order: novel anatomical plate (114.63 MPa) < oblique T-shaped locking plate (277.17 MPa) < two lag screws (236.75 MPa) < straight buttress plate (136.2 MPa). Conclusion: The patients with posterolateral plateau fractures fixed with a novel anatomical plate in standing, walking and fast running can achieve satisfactory biomechanical results, which lays the foundation for future applications. At the same time, clinical fracture types are often diverse and accompanied by damage to the soft tissue. Therefore, the ideal surgical approach and appropriate internal fixation must be selected based on the patient's injury condition.

Abstract:Proceedings of the Institution of Mechanical Engineers, Part H: Journal of Engineering in Medicine, Ahead of Print. A finite element model with realistic bone geometries is developed to design optimal internal fixation during the fibula healing process in this study. The effect of bone plate parameters on fibula fracture healing is studied. The relationship between differences in plate length, thickness and working length, and bone healing performance is focused. The optimal combination form of the bone plate parameters was selected by the orthogonal experimental design and fracture block strain to achieve bone healing maximize the performance. The model results show that the maximum equivalent force of the bone plate was below the material yield limit; the higher mean contact stresses in the bone fragments indicate that the bone plate is prone to higher contact stresses when they are long. The working length of the bone plate has a greater effect on callus healing than the thickness and length of the bone plate. The optimal internal fixation option for distal fibula fractures is achieved when it provides the stability required for internal fixation during bone healing. It ensures lower contact stresses in the fibula as well as maximum Young's modulus during callus healing process.

Abstract:PurposeDealing with high-energy fractures of the tibial plateau remains a challenge despite advances in implants, surgical approaches, and imaging methods. It is difficult to achieve sufficient fixation stability of a posterior T-shaped buttress (PTB) plate, the most commonly used implant. A tibial locking plate, called the proximal posterior medial tibial (PPMT) plate, that aims to improve fixation strength for tibial plateau split fractures was recently introduced. This study evaluates the biomechanical strength of this plate and compares it to that of a proximal medial tibial (PMT) plate and a PTB plate.MethodsTibial plateau fractures were simulated using a human tibia model. Either a PMT, PPMT, or PTB plate was virtually implanted into each fracture model. The posteromedial fragment vertical subsidence was measured under a 2000-N joint contact force. The maximum equivalent stress on the bone plate and bone screw and the construct stiffness were determined.ResultsThe PMT plate allowed the least posteromedial fragment subsidence and produced the highest construct stiffness. The PPMT plate showed higher stiffness than that of the PTB plate. The maximum equivalent stress was the smallest for the PMT plate.ConclusionThis study showed that PMT and PPMT plates are the most biomechanically stable fixation constructs for posteromedial split tibial plateau fractures.

Abstract:There is no consensus about the optimal internal fixation selection for treatment of posterolateral tibial plateau fracture. This study described a novel plate through an anterolateral approach for posterolateral tibial plateau fractures (PTPFs). We evaluated the biomechanical performance of a novel plate and two conventional internal implants and investigated the anatomic feasibility of the novel plate. The fracture models were randomly assigned into six groups: Groups A–C were the model groups of posterolateral split fracture, fixed with the posterior buttress plate, the lateral locking plate, and the novel plate, respectively. Groups D–E were the model groups of posterolateral depression fracture, fixed with the posterior buttress plate, the lateral locking plate, and the novel plate, respectively. We evaluated the biomechanical performance of six model groups by the biomechanical testing and finite element analysis. Progressively increasing axial compressive loads were applied to each synthetic fracture model by using a customized indentor under 250–750 N loads. Meanwhile, we dissected 12 fresh frozen knee specimens and fixed them with the novel plate through the anterolateral approach. We recorded the adjacency of the novel plate to important anatomic structures. Biomechanical testing showed that the novel plate had the least displacement, followed by the posterior buttress plate, and the lateral plate had the most displacement in posterolateral split fracture. There was no significant difference in the displacement between the novel plate and the lateral plate at different loads in posterolateral depression fractures. And the posterior buttress plate showed the most displacement. In the finite element analysis, the maximum stress values of Groups A, B, and C were 383.76, 414.63, and 305.07 MPa under the load of 750 N, respectively. The maximum stress values of Groups D, E, and F were 474.28, 436.31, and 413.4 MPa under the load of 750 N, respectively. In the anatomic study, the placement of the novel plate had a low risk of damage to the important anatomic structures of knee posterolateral corner. The novel plate could be a great choice for the treatment of PTPFs due to better biomechanical performance and easy manipulation.

Abstract:Background: Complex bone plateau fractures have been treated with bilateral plate fixation, but previous research has overemphasized evaluating the effects of internal fixation design, plate position, and screw orientation on fracture fixation stability, neglecting the internal fixation system's biomechanical properties in postoperative rehabilitation exercises. This study aimed to investigate the mechanical properties of tibial plateau fractures after internal fixation, explore the biomechanical mechanism of the interaction between internal fixation and bone, and make suggestions for early postoperative rehabilitation and postoperative weight-bearing rehabilitation. Methods: By establishing the postoperative tibia model, the standing, walking and running conditions were simulated under three axial loads of 500 N, 1000 N, and 1500 N. Accordingly, finite element analysis (FEA) was performed to analyze the model stiffness, displacement of fractured bone fragments, titanium alloy plate, screw stress distribution, and fatigue properties of the tibia and the internal fixation system under various conditions. Results: The stiffness of the model increased significantly after internal fixation. The anteromedial plate was the most stressed, followed by the posteromedial plate. The screws at the distal end of the lateral plate, the screws at the anteromedial plate platform and the screws at the distal end of the posteromedial plate are under greater stress, but at a safe stress level. The relative displacement of the two medial condylar fracture fragments varied from 0.002–0.072 mm. Fatigue damage does not occur in the internal fixation system. Fatigue injuries develop in the tibia when subjected to cyclic loading, especially when running. Conclusion: The results of this study indicate that the internal fixation system tolerates some of the body's typical actions and may sustain all or part of the weight early in the postoperative period. In other words, early rehabilitative exercise is recommended, but avoid strenuous exercise such as running.

Abstract:The selection of internal fixation as the primary fixation modality for the patient is one of the challenges for the surgeon treating the patient in question. A model of the lateral tibial plateau fracture was established. Three different configurations of internal fixators namely L bone plate, T bone plate, and screw-washer were analyzed. Three stages after surgery were simulated to assess the displacement of bone plates, screws, washers, and the stress shielding ratio in the fracture area.At three stages after surgery, the T bone plate showed better stability for patients during rehabilitation compared with the remaining two schemes, and the screw-washer scheme was the least stable due to the larger internal fixation displacement and stress shielding ratio in the fracture area. In contrast, the L bone plate scheme showed better stability in the early stages after surgery but was second only to the screw-washer scheme in the middle and late stages after surgery. The T bone plate showed better stability and became a new selection for surgeons to treat related patients. At three stages after surgery, the T bone plate has better biomechanical stability compared to the L bone plate and screw-washer schemes.

Abstract:This paper describes the use of an optical vortex trap for the transport and fusion of single femtoliter-volume aqueous droplets. Individual droplets were generated by emulsifying water in acetophenone with SPAN 80 surfactant. We demonstrate the ability of optical vortex traps to position trapped droplets precisely while excluding surrounding aqueous droplets from entering the trap, thereby preventing unwanted cross contamination by other nearby droplets. Additionally, the limitation of optical vortex traps for inducing droplet fusion is illustrated, and a remedy is provided through modulation of the spatial intensity profile of the optical vortex beam. Spatial modulation was achieved by translating the computer-generated hologram (CGH) with respect to the input Gaussian beam, thereby shifting the location of the embedded phase singularity (dark core) within the optical vortex beam. We present both simulated and experimentally measured intensity profiles of the vortex beam caused by translation of the CGH. We further describe the use of this technique to achieve controlled and facile fusion of two aqueous droplets.
Impact of augmentation strategy variations on the mechanical characteristics of patients with osteoporotic proximal humerus fractures with medial column instability

Abstract:Introduction: Low bone density and lack of medial support are the two most important factors affecting the stability of locking plate fixation for osteoporotic proximal humeral fractures (PHFs). This study aimed to compare the biomechanical characteristics of PHILOS locking plates combined with calcar screws, bone cement, fibular allografts, and medial locking plate support strategies for treating osteoporotic PHFs with medial column instability. Methods: A three-part osteoporotic PHF (AO 11-B3.2) model with metaphyseal loss was generated using 40 synthetic humeri and fixed via four distinct medial support strategies. All models were mechanically tested to quantify the mechanical characteristics. Subsequently, finite element models were created for each biomechanical test case. The stress distribution and displacement of the four different fixation structures were analyzed using finite element analysis. Results: The results demonstrated that the PHILOS locking plate combined with the medial locking plate, exhibited the greatest stability when subjected to axial, shear, and torsional loading. Furthermore, the PHILOS locking plate combined with bone cement showed structural stability similar to that of the PHILOS locking plate combined with fibular allograft but with lower stress levels on the fracture surface. Discussion: In conclusion, the PLP-MLP fixation structure showed superior biomechanical properties under axial, shear, and torsional loading compared to other medial support methods. Repairing the medial support when treating osteoporotic PHFs with medial column instability can enhance the mechanical stability of the fracture end in both the short and long term.
Finite element analysis and optimization studies on tibia implant of SS 316L steel and Ti6Al4V alloy

Abstract:Tibial fractures account for approximately 15% of all fractures, typically resulting from high-energy trauma. A critical surgical approach to treat these fractures involves the fixation of the tibia using a plate with minimally invasive osteosynthesis. The selection and fixation of the implant plate are vital for stabilizing the fracture. This selection is highly dependent on the plate's stability, which is influenced by factors like the stresses generated in the plate due to the load on the bone, as well as the plate's length, thickness, and number of screw holes. Minimizing these stresses is essential to reduce the risk of implant failure, ensuring optimal stress distribution and promoting faster, more effective bone healing. In the present work, the finite element and statistical approach was used to optimize the geometrical parameters of the implant plate made of SS 316L steel and Ti6Al4V alloy. A 3D finite element model was developed for analyzing the stresses and deformation, and implant plates were manufactured to validate the results with the help of an experiment conducted on the universal testing machine. A strong correlation was observed between the experimental and predicted results, with an average error of 8.6% and 8.55% for SS316L and Ti6Al4V alloy, respectively. Further, using the signal-to-noise ratio for the minimum stress condition was applied to identify the optimum parameters of the plate. Finally, regression models were developed to predict the stresses generated in SS316L and Ti6Al4V alloy plates with different input conditions. The statistical model helps us to develop the relation between different geometrical parameters of the Tibia implant plate. As determined by the present work, the parameter most influencing is implant plate length. This outcome will be used to select the implant for a specific patient, resulting in a reduction in implant failure post-surgery.
Experimental and probabilistic analysis of distal femoral periprosthetic fracture: a comparison of locking plate and intramedullary nail fixation. Part A: experimental investigation

Abstract:The following is a two-part study. Part A evaluates biomechanically intramedullary (IM) nails vs. locking plates for fixation of femoral fractures in osteoporotic bone. Part B of this study introduces a deterministic finite element model of each construct type and investigates the probability of periprosthetic fracture of the locking plate compared with the retrograde IM nail using Monte Carlo simulation. For Part A, an extra-articular, metaphyseal wedge fracture pattern was created in 11 osteoporotic fourth-generation composite femurs. Fixation was performed with a locking plate or a retrograde IM nail. Axial, torsion and bending cyclic loading to simulate post-operative damage accumulation were performed followed by ramped load to failure. Locking plates proved to be more stable (using stiffness as the determining factor) in osteoporotic bone as observed under low load cycle conditions. However, some of these advantages were offset by a greater incidence of sudden periprosthetic fracture observed under ramped loading conditions. Cadaveric, osteoporotic femurs included as a case study also exhibited periprosthetic fracture, but failure was accompanied by catastrophic comminution of the cortex. Periprosthetic failure at the implant end including bone comminution is difficult to salvage with revision fixation. The weakened trabecular matrix and thinned cortex of osteoporotic bone may increase the incidence of periprosthetic fracture. It is, therefore, essential for the surgeon to consider all possible loading scenarios when recommending an ideal implant for the osteoporotic patient.

Finite Element Analysis of Three Internal Fixations for the Posteromedial Split Fracture Fragment in Tibial Plateau Fractures

Abstract:Posteromedial tibial plateau fracture is one of the most challenging traumatic fractures. We aimed to compare and explain the biomechanical advantages and disadvantages of different internal fixation methods for the treatment of the posteromedial split fracture fragment in tibial plateau fractures. Finite element models of the tibial plateau fracture were constructed. Three different internal fixations were developed to treat the posteromedial split fracture fragment in tibial plateau fractures finite element models: (a) the novel anatomic locking plate fixation model, (b) the common anatomic locking plate fixation model, and (c) the reconstruction plate fixation model. We applied the same loading conditions to each model: 320 N axial compression. Under the same condition, the von Mises stress (VMS) and displacement distribution of the three internal fixations and tibia plateau were studied. The stress values of the novel anatomic locking plate are lower than the common anatomic locking plate and the reconstruction plate. Additionally, the novel anatomic locking plate fixation system exhibits smaller maximum displacement. In conclusion, our study indicated that the novel anatomic locking plate resulted in a lower stress distribution in the plate and screws, and better stability than the common anatomic locking plate and the reconstruction plate for the posteromedial split fracture fragment in tibial plateau fractures fixation under the same loading conditions. Thus, for the posteromedial split fracture fragment in tibial plateau fractures, the use of the novel anatomic locking plate internal fixation is recommended.

Abstract:Axial micromotion between bone fragments can stimulate callus formation and fracture healing. In this study, we propose a novel mechanically compliant locking plate which achieves up to 0.6 mm of interfragmentary motion as flexures machined into the plate elastically deflect under physiological load. We investigated the biomechanical performance of three compliant plate variations in comparison to rigid control plates with small and large working lengths in a comminuted bridge plating scenario using humeral diaphysis surrogates. Under static axial loading, average interfragmentary motion was 6 times larger at 100 N (0.38 vs. 0.05 mm) and nearly three times larger at 350 N (0.58 vs. 0.2 mm) for compliant plates than rigid plates, respectively. Compliant plates delivered between 2.5 and 3.4 times more symmetric interfragmentary motion than rigid plates (p < 0.01). The bi-phasic stiffness of compliant pates provided 74%-96% lower initial axial stiffness up to approximately 100 N (p < 0.01), after which compliant plate stiffness was similar to rigid plates with increased working length (p > 0.3). The strength to failure of compliant plates under dynamic loading was on average 48%-55% lower than rigid plate groups (p < 0.01); however, all plates survived cyclic fatigue loading of 100,000 cycles at 350 N. This work characterizes the improvement in interfragmentary motion and the reduction in strength to failure of compliant plates compared to control rigid plates. Compliant plates may offer potential in comminuted fracture healing due to their ability to deliver symmetric interfragmentary motion into the range known to stimulate callus formation while surviving moderate fatigue loading with no signs of failure.
Finite element method-based simulation on bone fracture fixation configuration factors for biodegradable embossed locking compression plate

Abstract:As an evolution, biodegradable implants need to maximize mechanical performance thereby may lead to confusion in selection of the biodegradable material and implant design to the fracture site. This requires selecting a unique fixation configuration to fit within the fractured bone, factors of which can be bone-plate clearance, interfragmentary gap, alteration in screw fixation position and variation in the number of screws whose configuration optimization can re-maximize the mechanical performance of the biodegradable implant. Therefore, these factors have been optimized based on the induced minimum stress using the finite element method-based simulation for which biodegradable embossed locking plates (BELCP) via screws made of Mg-alloy have been fitted over two fragments of femur body (as hollow cylindrical cortical bone). An average human weight of 62 kg is applied to one segment of the femur for all different configurations of each factor, where another segment is assumed to be fixed. By this simulation, the most optimal fixation configuration was found at a minimum induced stress value of 41.96 MPa which is approximately 85%, 18%, 6% and 48% respectively less than all maximum stress induced configurations in each of the factor. This optimized configuration was at the minimum clearance between bone and plate with a 3 mm interfragmentary gap using 8 screws where the locking screw begins to apply from the center of the BELCP. Overall, BELCP may be a better biodegradable implant plate for bone fracture fixation with these optimized fixation configurations as the improved mechanical performance after experimental validation.

Abstract:Optimal surgical positioning of cable-screw pairs in repairing periprosthetic femur fractures near the tip of a total hip implant still remains unclear. No studies in the literature to date have developed a fully three-dimensional finite element (FE) model that has been validated experimentally to assess these injury patterns. The aim of the present study was to evaluate the biomechanical performance of three different implant–bone constructs for the fixation of periprosthetic femoral shaft fractures following total hip arthroplasty. Experimentally, three bone–plate repair configurations were applied to the periprosthetic synthetic femur fractured with a 5 mm gap near the tip of a total hip implant. Constructs A, B, and C, respectively, had successively larger distances between the most proximal and the most distal cable-screw pairs used to affix the plate. Specimens were oriented in 15° adduction, subjected to 1000 N of axial force to simulate the single-legged stance phase of walking, and instrumented with strain gauges. Computationally, a linearly elastic and isotropic three-dimensional FE model was developed to mimic the experimental setup. Results showed excellent agreement between experimental versus FE analysis strains, yielding a Pearson linearity coefficient, R 2 , of 0.90 and a slope for the line of best data fit of 0.96. FE axial stiffnesses were 601 N/mm (Construct A), 849 N/mm (Construct B), and 1359 N/mm (Construct C). FE surface stress maps for cortical bone showed maximum von Mises values of 74 MPa (Construct A), 102 MPa (Construct B), and 57 MPa (Construct C). FE stress maps for the metallic components showed minimum von Mises values for Construct C, namely screw (716 MPa), cable (445 MPa), plate (548 MPa), and hip implant (154 MPa). In the case of good bone stock, as modelled by the present synthetic femur model, optimal fixation can be achieved with Construct C.
