Abstract:Transient electroretinograms to a reversing color-contrast checkerboard pattern (P-ERG) were recorded in a protanomalous, a deuteranomalous, and a normal observer. Alternate monochromatic checks were of constant wavelength (630 nm red-531 nm green), while the relative energies were varied systematically. When changing the radiance ratio 630 nm-531 nm of the stimulus, the normal subject exhibited a P-ERG to all stimuli with only a relative amplitude minimum at a distinct radiance ratio, whereas the color-deficient observers failed to show a P-ERG at some color contrast 630 nm-531 nm, the radiance ratio of which was different in the protan and deutan. From the radiance ratio of color contrast for the smallest potential in the normal observer, we conclude that the green- and red-sensitive cone mechanism provides a difference signal which generates the response. The data from the color-deficient observer support the view that color discrimination in protans and deutans is reduced because the input of one type of photoreceptor is missing.
Pattern-reversal electroretinogram in response to chromatic stimuli: I. Humans

Abstract:We have studied the steady-state PERG in human subjects in response to red-green plaid patterns modulated either in luminance or in chromaticity or both. By varying the relative luminance of the red and green components, a value could be obtained at which the PERG amplitude was either minimum or locally maximum. This always occurred at equiluminance, as measured by standard psychophysical techniques. PERG amplitude and phase were measured as a function of spatial and temporal frequency of sinusoidal contrast reversal. In both space and time, the response to chromatic patterns was low-pass, while that to luminance was band-pass, and extended to higher spatial and temporal frequencies. The phase of the PERG to chromatic stimuli was systematically lagged compared with that to luminance stimuli, by an amount corresponding to about 20 ms under our experimental conditions. The variation of phase with temporal frequency suggested an apparent latency of about 67 ms for color contrast compared with 47 ms for luminance. These estimates were confirmed with separate measurements of transient PERGs to abrupt contrast reversal. For both luminance and chromatic stimuli, the amplitude of PERGs increases with increasing stimulus contrast. By summing vectorially the responses to appropriate luminance and chromatic contrasts, we were able to predict with accuracy the response as a function of color ratio (ratio of red to total luminance). The above findings all agree with those reported in the accompanying paper for the monkey PERG (Morrone et al., 1994), and indicate that the differences in response latency and integration time of luminance and chromatic stimuli observed by psychophysical and VEP techniques may arise at least in part from the properties of retinal mechanisms.
[Blue-sensitive mechanisms in pattern ERG and VEP]

Abstract:Pattern onset-offset ERGs and VEPs in response to square-wave stripe patterns of different wavelengths and spatial frequencies under chromatic yellow adaptation (two-channel Maxwellian view) were studied in three subjects. One channel provided a steady homogeneous adapting field of 570 nm, while the other channel, containing the patterns, was superimposed on the adapting field (33 degrees). The spectral sensitivity curve of the ERG had a prominent peak in the blue (460 nm) and a shoulder in the red and green (above 500 nm). For wavelengths below 500 nm the shape of the psychophysically determined sensitivity curve to the onset and offset of the pattern was in close agreement with the electroretinal sensitivity curves determined for a low-amplitude criterion. With almost all wavelengths there were two components in the same VEP response at the same time: a slow negative wave with maximum sensitivity at 460 nm and an earlier fast positive component with maximum sensitivity in the red-green region. When the spatial frequency of a 460 nm test pattern was varied under yellow adaptation the onset response in the ERG showed only very slight spatial tuning. In the onset response of the VEP, however, the slow component showed prominent spatial tuning (around 2 c/deg) under the same conditions.
Spatial frequency of the human short-wavelength-sensitive (blue) cone mechanism. Psychophysical studies and pattern-reversal visual evoked potentials

Abstract:The interactions of spatial and chromatic processing of the short-wavelength-sensitive cone mechanism were studied in humans with patterned (checkerboard) stimuli of various spatial frequency (10, 22, 44, and 85 min of are respectively), under steady exposure to yellow light (575 nm, 390 cd/m2). Psychophysical studies and pattern-reversal visual evoked potentials were employed. Parameters of the transient pattern-reversal visual evoked potentials (pattern reversal rate of 2.4 s-1) especially observed were the latencies of P2 (P100) and N3 and the amplitude of P2-N3. It was only with the largest applicable check size (85 min of arc) that both the psychophysical studies and visual evoked potentials could succeed in satisfactorily isolating the short-wavelength-sensitive cone mechanism. Pattern-reversal visual evoked potential latencies are recommended in the evaluation of this cone mechanism because of their smaller variance and higher selectivity in isolating the short-wavelength-sensitive cone mechanism than the amplitude. The peak sensitivity of this cone mechanism was shown to be about 449 nm at the corneal level. The short-wavelength sensitive cone mechanism represented the characteristics of low spatial resolution and long latencies of the pattern-reversal visual evoked potentials.
A comparison of three techniques to estimate the human dark-adapted cone electroretinogram

Abstract:The dark-adapted cone electroretinogram (ERG) is difficult to isolate because of unwanted rod intrusion. We compare dark-adapted cone estimates derived using three techniques. The first uses the cone response on a moderate rod saturating background to estimate the dark-adapted cone response. The second uses red and blue flashes to tease apart cone and rod responses (red-minus-blue technique, [Investigative Ophthalmology and Visual Science 31 (1990) 2283]). The third uses a bright flash to temporarily saturate rods, followed by a test flash that generates a putative cone-only response (2-flash technique [Investigative Ophthalmology and Visual Science 36 (1995) 1603]). By subtracting the cone estimates from 'mixed' ERG responses in the dark, rod isolated responses can be derived. The rod phototransduction parameters, derived using a computational model, are similar using the light-adapted and 2-flash cone estimates, but differ using the red-minus-blue estimates. The 2-flash cone estimate gives a cone waveform similar to the dark-adapted response of a patient with Oguchi stationary night blindness (a patient with no rod ERG responses and normal cone ERG responses). The growth of the cone response during light adaptation to steady backgrounds causes significant differences between the light-adapted and 2-flash cone waveforms at times beyond the first few milliseconds.

Abstract:It was previously revealed that Wnt signaling is activated in mesothelioma cells. Although epidermal growth factor receptor (EGFR) is expressed in mesothelioma cells, EGFR-tyrosine kinase inhibitors (TKIs) are not effective for mesothelioma treatment. However, in non-small cell lung cancer, the blocking of Wnt signaling has been identified to enhance the anticancer effect of EGFR-TKIs. To confirm the anticancer effect of blocking Wnt signaling in combination with EGFR-TKI treatment in mesothelioma, the present study evaluated the effect of simultaneous suppression of human dishevelled-3 (Dvl-3) expression with Dvl-3 small interfering RNA (siRNA) and of EGFR inhibition with gefitinib on mesothelioma cell viability. Mesothelioma cell lines with and without β-catenin gene expression were transfected with Dvl-3 siRNA and were cultured with gefitinib, and cell viability, colony formation and cell cycle analyses were performed. Dvl-3 siRNA downregulated the expression of Dvl-3 in mesothelioma cells. The combination of Dvl-3 siRNA with gefitinib acted synergistically to induce concomitant suppression of cell viability and colony formation, suggesting that inhibition of Wnt signaling by downregulating Dvl-3 with siRNA and inhibiting EGFR with gefitinib leads to significant antitumor effects.

Abstract:Amblyopia is generally a unilateral disorder, defined by at least a difference of two lines of visual acuity between both eyes with the best-corrected visual acuity, a decrease in contrast sensitivity, and a decrease in stereopsis. Pattern electroretinogram (PERG) is a noninvasive technique that provides a retinal biopotential and is a highly sensitive indicator of changes in the macular area. Abstract:The International Society for Clinical Electrophysiology of Vision (ISCEV) Standard for full-field electroretinography (ERG) describes a minimum procedure, but encourages more extensive testing. This ISCEV extended protocol describes an extension to the ERG Standard, namely the photopic negative response (PhNR) of the light-adapted flash ERG, as a well-established technique that is broadly accepted by experts in the field. The PhNR is a slow negative-going wave after the b-wave that provides information about the function of retinal ganglion cells and their axons. The PhNR can be reduced in disorders that affect the innermost retina, including glaucoma and other forms of optic neuropathy. This document, based on existing literature, provides a protocol for recording and analyzing the PhNR in response to a brief flash. The protocol includes full-field stimulation, a frequency bandwidth of the recording in which the lower limit does not exceed 0.3 Hz, and a spectrally narrowband stimulus, specifically, a red flash on a rod saturating blue background. Suggested flash strengths cover a range up to and including the minimum required to elicit a maximum amplitude PhNR. This extended protocol for recording the PhNR provides a simple test of generalized retinal ganglion cell function that could be added to standard ERG testing.

Abstract:AbstractThe International Society for Clinical Electrophysiology of Vision (ISCEV) standard for full-field electroretinography (ERG) describes a minimum procedure, but encourages more extensive testing. This ISCEV extended protocol describes an extension to the ERG standard, namely the dark-adapted (DA) red flash ERG. The DA red flash ERG can be incorporated conveniently within the ISCEV standard ERG protocol after a minimum of 20-min DA and recorded after the DA 0.01 ERG to a flash strength of 0.3 phot cd s m−2, eliciting a waveform with two positive peaks in healthy individuals. The first positive component is the cone-mediated x-wave with a peak at 30–50 ms; the second is a rod-mediated b-wave with a peak time of approximately 100 ms. Shorter DA times may be desirable to shorten the recording time or to alter the prominence of the early cone-mediated x-wave relative to the rod-mediated b-wave. The DA red flash ERG is used to aid the diagnosis of achromatopsia (rod monochromacy), cone dystrophy and other forms of cone system dysfunction, including “Bradyopsia” (RGS9/R9AP-retinopathy), when the DA red flash ERG x-wave is preserved in the absence of ISCEV standard LA ERGs. The DA red flash ERG can also help determine the origin of residual DA ERGs in cases of severe rod dysfunction, for example in disorders such as vitamin A deficiency, fundus albipunctatus (RDH5-retinopathy), Oguchi disease (SAG- or GRK1-retinopathy) and some rod-cone dystrophies. To shorter DA periods, the x-wave may be elicited without the following rod b-wave, shown to be helpful in abbreviated protocols for children.

A Temporal White Noise Analysis for Extracting the Impulse Response Function of the Human Electroretinogram

Abstract:PURPOSE: We introduce a method for determining the impulse response function (IRF) of the ERG derived from responses to temporal white noise (TWN) stimuli.METHODS: This white noise ERG (wnERG) was recorded in participants with normal trichromatic vision to full-field (Ganzfeld) and 39.3° diameter focal stimuli at mesopic and photopic mean luminances and at different TWN contrasts. The IRF was obtained by cross-correlating the TWN stimulus with the wnERG.RESULTS: We show that wnERG recordings are highly repeatable, with good signal-to-noise ratio, and do not lead to blink artifacts. The wnERG resembles a flash ERG waveform with an initial negativity (N1) followed by a positivity (P1), with amplitudes that are linearly related to stimulus contrast. These N1 and N1-P1 components showed commonalties in implicit times with the a- and b-waves of flash ERGs. There was a clear transition from rod- to cone-driven wnERGs at ∼1 photopic cd.m<sup>-2</sup>. We infer that oscillatory potentials found with the flash ERG, but not the wnERG, may reflect retinal nonlinearities due to the compression of energy into a short time period during a stimulus flash.CONCLUSION: The wnERG provides a new approach to study the physiology of the retina using a stimulation method with adaptation and contrast conditions similar to natural scenes to allow for independent variation of stimulus strength and mean luminance, which is not possible with the conventional flash ERG.TRANSLATIONAL RELEVANCE: The white noise ERG methodology will be of benefit for clinical studies and animal models in the evaluation of hypotheses related to cellular redundancy to understand the effects of disease on specific visual pathways.

Abstract:The pattern electroretinogram (PERG) is a localized retinal response evoked by a contrast-reversing pattern, usually a black and white checkerboard, which provides information about macular and retinal ganglion cell function. This document, from the International Society for Clinical Electrophysiology of Vision (ISCEV; www.iscev.org) presents an updated and revised Standard for clinical PERG testing. This replaces the 2013 and all earlier versions. Minimum protocols for basic PERG stimuli, recording methods and reporting are specified, to promote consistency of methods for diagnosis and monitoring purposes, while responding to evolving clinical practices and technology. The main changes in the updated ISCEV Standard for clinical PERG include expanded guidance about large stimulus fields, stimulus parameters for simultaneous PERG and pattern visual evoked potential recording, baseline drift correction, and use of consistent ambient room lighting. These changes aim to provide a clinically relevant document about current practice which will facilitate good quality recordings and inter-laboratory comparisons.

Ultraviolet- and short-wavelength cone contributions alter the early components of the ERG of young zebrafish

Abstract:The electroretinogram (ERG) is a commonly used measure to examine retinal processing in both basic and clinical research. The purpose of this study was to determine the retinal mechanisms responsible for the developmental differences found in the zebrafish ERG waveform. The ERG of young zebrafish possesses a voltage-negative response to ultraviolet- and short-wavelength stimuli, but not to middle- and long-wavelength stimuli; the ERG of adult zebrafish does not possess this response component. ERGs were obtained from young zebrafish before and after the introduction of either aspartate, or a combination of APB (DL-2-amino-4-phosphonobutyric acid) and PDA (cis-2,3-piperidinedicarboxylic acid) in order to suppress the responses of various types of retinal neurons. Log irradiance versus response amplitude functions of the ERG response to 200-ms stimuli of various wavelengths at various times following stimulus onset (70 and 120 ms) was derived as well as spectral sensitivity. Aspartate eliminated all voltage-positive responses regardless of stimulus wavelength; irradiance-response functions following aspartate were similar to the early responses of young control fish to ultraviolet- and short-wavelength stimuli. APB + PDA produced similar but not identical results as aspartate, suggesting that the combination of these agents does not completely eliminate all post-receptoral contributions to the ERG. Spectral sensitivity functions derived from aspartate-exposed subjects at various time measurements were dominated by contributions from ultraviolet- and short-wavelength-sensitive cone types. These wavelength-dependent ERG responses are similar to those found in humans with enhanced S-cone syndrome. Finally, ERG waveform differences across stimulus wavelength suggest that the circuitry of ultraviolet- and short-wavelength cone types is different to that of middle- and long-wavelength cone types in young zebrafish.
Update on the range of visual electrophysiology tests and heir application with clinical examples

Abstract:The International Society for clinical electrophysiology of vision (ISCEV) publishes open‐access standards and guidelines for routine electrophysiological tests of the visual system in order to optimize diagnostic value, to promote consistency and to allow meaningful inter‐laboratory comparisons. The ISCEV also publishes extended protocols with the aim of promoting convergence of methods that are not yet standardized, for tests that are widely used but often performed as an addition to the minimum ISCEV standard procedures (see www.ISCEV.org for all publications). This talk will outline the main ISCEV standard methods including the full‐field electroretinogram (ERG), pattern ERG (PERG) and multifocal ERG (mfERG). Several extended full‐field ERG protocols will also be highlighted, including the dark‐adapted (DA) red flash ERG, photopic On–Off ERG and S‐cone ERG. The use of these methods in the clinic will be illustrated in cases of inherited and acquired retinal disorders, to show how the extended protocols complement standard techniques, to aid diagnosis and patient management and to probe and detail retinal function phenotypes. The Standard full‐field ERG is recorded under dark‐adapted (DA) and light‐adapted (LA) conditions and provides a measure of generalized rod and cone system function at the level of the outer and inner retina, with minimal contribution from the macula. The mfERG assesses cone system function across the posterior pole and the PERG assesses macular and macular retinal ganglion cell function. The DA red flash ERG is an extended protocol that provides a measure of generalized dark‐adapted retinal function, with the advantage of having both a cone system x‐wave and a rod system b‐wave within the same waveform. This enables the relative involvement of rod and cone systems to be assessed simultaneously and can help determine the origins of severely attenuated standard DA strong flash ERGs. The DA red flash ERG is also useful for assessing cone system function in photophobic patients, and in patients with variable ability to comply with longer ERG protocols. The photopic On–Off ERG is a response to an extended light stimulus (duration 150 or 200 ms) and is used to assess generalized cone system function of both the On and Off retinal pathways, useful when there is a locus of dysfunction that is post‐phototransduction or post‐receptoral, when On and Off retinal systems may be similarly, selectively or specifically affected e.g., in different forms of congenital stationary night blindness (CSNB) or in forms of toxic retinopathy. The S‐cone ERG (short‐wavelength or “blue” flash ERG) is used to probe the function of the S‐cone retinal pathway, and can aid diagnosis and characterization in a variety of retinal dystrophies and disorders e.g., distinguishing between achromatopsia and S‐cone monochromacy.

Age-dependencies of the electroretinogram in healthy subjects

Abstract:Objective: The purpose of this study was to evaluate the age-dependency of amplitudes and implicit times in the electroretinograms (ERGs) of healthy individuals and provide clinicians and researchers with a reference for a variety of stimulus paradigms. Design and methods: Full-field electroretinography was conducted on 73 healthy participants aged 14-73 using an extended ISCEV standard protocol that included an additional 9 Hz flicker stimulus for assessing rod function and special paradigms for isolated On-Off and S-cone responses. Correlation coefficients and best-fit regression models for each parameter's age-dependency were calculated. Results: Dark-adapted ERGs, in particular, displayed notable age-related alterations. The attenuation and delay of the b-wave with higher age were most significant in the dark-adapted, rod-driven 0.001 cd s/m2 flash ERG. The age-dependent reduction of the a-wave amplitude was strongest in the standard dark-adapted 3 cd s/m2 flash condition. Cone-driven, light-adapted responses to either flash or flicker stimuli displayed comparatively small alterations at higher age. S-cone function tended to diminish at an early age, but the effect was not significant in the whole population. Conclusion: The results suggest that rod and cone function decline at different rates with age, with rods being generally more affected by aging. Nonetheless, response amplitudes displayed a wide variability across the whole sample.
The intrinsically photosensitive retinal ganglion cell (ipRGC) mediated pupil response in young adult humans with refractive errors

Abstract:Purpose: The intrinsically photosensitive retinal ganglion cells (ipRGCs) signal environmental light, with axons projected to the midbrain that control pupil size and circadian rhythms. Post-illumination pupil response (PIPR), a sustained pupil constriction after short-wavelength light stimulation, is an indirect measure of ipRGC activity. Here, we measured the PIPR in young adults with various refractive errors using a custom-made optical system. Methods: PIPR was measured on myopic (-3.50 ± 1.82 D, n = 20) and non-myopic (+0.28 ± 0.23 D, n = 19) participants (mean age, 23.36 ± 3.06 years). The right eye was dilated and presented with long-wavelength (red, 625 nm, 3.68 × 1014 photons/cm2/s) and short-wavelength (blue, 470 nm, 3.24 × 1014 photons/cm2/s) 1 s and 5 s pulses of light, and the consensual response was measured in the left eye for 60 s following light offset. The 6 s and 30 s PIPR and early and late area under the curve (AUC) for 1 and 5 s stimuli were calculated. Results: For most subjects, the 6 s and 30 s PIPR were significantly lower (p < 0.001), and the early and late AUC were significantly larger for 1 s blue light compared to red light (p < 0.001), suggesting a strong ipRGC response. The 5 s blue stimulation induced a slightly stronger melanopsin response, compared to 1 s stimulation with the same wavelength. However, none of the PIPR metrics were different between myopes and non-myopes for either stimulus duration (p > 0.05). Conclusions: We confirm previous research that there is no effect of refractive error on the PIPR.
Effect of Single and Combined Monochromatic Light on the Human Pupillary Light Response

Abstract:The pupillary light reflex (PLR) is a neurological reflex driven by rods, cones, and melanopsin-containing retinal ganglion cells. Our aim was to achieve a more precise picture of the effects of 5-min duration monochromatic light stimuli, alone or in combination, on the human PLR, to determine its spectral sensitivity and to assess the importance of photon flux. Using pupillometry, the PLR was assessed in 13 participants (6 women) aged 27.2 ± 5.41 years (mean ± SD) during 5-min light stimuli of purple (437 nm), blue (479 nm), red (627 nm), and combinations of red+purple or red+blue light. In addition, nine 5-min, photon-matched light stimuli, ranging in 10 nm increments peaking between 420 and 500 nm were tested in 15 participants (8 women) aged 25.7 ± 8.90 years. Maximum pupil constriction, time to achieve this, constriction velocity, area under the curve (AUC) at short (0-60 s), and longer duration (240-300 s) light exposures, and 6-s post-illumination pupillary response (6-s PIPR) were assessed. Photoreceptor activation was estimated by mathematical modeling. The velocity of constriction was significantly faster with blue monochromatic light than with red or purple light. Within the blue light spectrum (between 420 and 500 nm), the velocity of constriction was significantly faster with the 480 nm light stimulus, while the slowest pupil constriction was observed with 430 nm light. Maximum pupil constriction was achieved with 470 nm light, and the greatest AUC0-60 and AUC240-300 was observed with 490 and 460 nm light, respectively. The 6-s PIPR was maximum after 490 nm light stimulus. Both the transient (AUC0-60) and sustained (AUC240-300) response was significantly correlated with melanopic activation. Higher photon fluxes for both purple and blue light produced greater amplitude sustained pupillary constriction. The findings confirm human PLR dependence on wavelength, monochromatic or bichromatic light and photon flux under 5-min duration light stimuli. Since the most rapid and high amplitude PLR occurred within the 460-490 nm light range (alone or combined), our results suggest that color discrimination should be studied under total or partial substitution of this blue light range (460-490 nm) by shorter wavelengths (~440 nm). Thus for nocturnal lighting, replacement of blue light with purple light might be a plausible solution to preserve color discrimination while minimizing melanopic activation.

Abstract:It is argued that non-genotoxic mechanisms may be more important than genotoxic mechanisms in the genesis of cancer and that the use of tests for genotoxicity to screen chemicals for carcinogenicity in the hope of being able to prevent human cancer is ill-based. The general applicability of the two-stage model for carcinogenesis is also questioned. It is argued that tumour promotion as a second step in a two-stage process is only one of many possible non-genotoxic mechanisms involved in carcinogenesis and not a very important one at that. Many cancers arise against a background of prolonged disturbance of hormonal status leading to hypertrophy and/or hyperplasia of the tissues in which cancers eventually arise. This sequence of events does not fit the two-stage paradigm according to which genetic-damage (initiation) is followed by the stimulation of cell proliferation (promotion). In hormonal carcinogenesis cellular proliferation precedes genetic damage. It is suggested that the genetic damage that occurs as a secondary event in hormonal carcinogenesis may be due, not to exposure to environmental (exogenous) mutagens, but to endogenous mutagens generated during normal metabolic processes in hyperplastic and hypermetabolic cells. Perhaps the rate of damage to DNA in such cells overwhelms the capacity for DNA repair. Cancer is an ageing-related disease. The incidence and severity of both non-neoplastic and neoplastic lesions are greatly enhanced by over-nutrition in laboratory rats.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract:To extract the nonlinear component of the electroretinogram (ERG) from the posterior pole (pp) of the human ocular fundus and to evaluate the possibility of its clinical application, three types of stimulus modes - double-flash, single-flash, and delayed single-flash stimuli - were produced using a conventional electrophysiological system. A large hexagonal element was presented on a CRT monitor, and ppERGs were recorded from 14 normal eyes and 16 eyes of eight highly myopic patients. The nonlinear component of the ppERG was obtained by subtracting the single-flash ERG and delayed single-flash ERG responses from the double flash ERG response. Three peaks were commonly observed in the nonlinear component of ppERG, all of which were significantly depressed in patients with high myopia. ppERGs that can be obtained using a simple algorithm might be useful in clinical applications.
